Abstract
腹膜透析液,其是由酸性的第1液、和第2液形成且经灭菌的腹膜透析液,所述第1液含有艾考糊精,所述第2液含有300~3,000mg/L的L‑组氨酸及pH调节剂,所述腹膜透析液中,所述第1液不含pH调节剂,灭菌后的所述第1液的pH为4.0~5.3,灭菌后的所述第2液的pH为6.5~8.0,灭菌后将第1液与第2液混合后的pH为6.5~7.5。
Description
技术领域
本发明涉及含有艾考糊精(icodextrin)的腹膜透析液。
背景技术
就作为肾功能衰竭的对症疗法之一的腹膜透析疗法而言,与利用人工肾脏进行的透析疗法相比,装置、器械不会成为大规模,时间上的约束也少,因此,作为家用医疗之一而受到关注。目前所用的多数腹膜透析液使用葡萄糖作为渗透剂。葡萄糖虽然具有比较安全且便宜的优点,但由于分子量小,所以会被迅速地从腹膜吸收,无法获得持续的脱水效果。根据以上情况,对能够在长时间留置时维持超滤的代替葡萄糖的渗透剂进行了探索,发现了作为葡萄糖聚合物的艾考糊精适合于腹膜透析液。
艾考糊精的分子量大,因此也不会经由腹膜被迅速地吸收,主要作为胶体渗透剂发挥作用,能够在维持与血浆的渗透压的同时获得脱水效果。目前,为了防止艾考糊精的分解·着色,使用艾考糊精的腹膜透析液以药液的pH成为5.0~5.5的范围的方式进行配制。
根据最近的研究,有报道称,如上所述的pH的腹膜透析液会实质性地降低腹腔巨噬细胞的免疫防御机制,针对细菌的进入而言,使腹膜炎的危险性增大。另外,也有报道称,在连续腹膜透析疗法导入时发热或腹痛会延长,或者在注射透析液时会出现腹痛。此外,还有报道称,pH为5.0~5.5的腹膜透析液对培养腹膜间皮细胞的抑制性非常高,为了减轻抑制性,使腹膜透析液的pH为6.5以上是有效的。
但是,腹膜透析液的pH会对艾考糊精的稳定性产生大的影响,如果直接增大pH,则艾考糊精在制造时或保存时分解为葡萄糖,因该葡萄糖的劣化而使得腹膜透析液发生着色,制品价值显著降低。即,作为5-羟甲基糠醛(其为葡萄糖的主要分解产物)的指标的284nm处的吸光度经时地增加。此外,当提高pH时,作为3-脱氧葡萄糖醛酮(其为葡萄糖的主要分解产物)的指标的228nm处的吸光度也pH依赖性地增加。此外,存在使用碳酸氢钠这样的碳酸盐来提高pH的方法,但在碱性条件下钙、镁会与碳酸发生反应而生成沉淀。
因此,作为降低pH来抑制艾考糊精的分解·着色,然后在施予时提高腹膜透析液的pH的方法,开发了到使用时为止分开收纳艾考糊精和pH高的药液成分,在临使用前进行无菌混合的制剂(专利文献1)。
但是,近年来,对腹膜透析液的稳定性、腹膜透析液的安全性的要求程度越来越高,希望开发出为不会对人体产生不利影响的生理中性区域的pH、并且能适当地抑制艾考糊精向葡萄糖的分解和着色的、稳定的腹膜透析液。此外,期望混合后的pH接近生理中性区域(具体而言为pH7.0)、且其稳定性高的腹膜透析溶液。

发明内容
发明所要解决的课题
本发明在于提供下述腹膜透析液,所述腹膜透析液能够提高艾考糊精在加热灭菌及其后的保存中的稳定性,而且在临施予前混合时,不会在混合后析出其他成分,并且可使pH接近生理中性区域,且其稳定性高。
用于解决课题的手段
上述课题通过以下的本发明的腹膜透析液实现。
腹膜透析液,其是由酸性的第1液、和第2液形成且经灭菌的腹膜透析液,所述第1液含有艾考糊精,所述第2液含有300~3000mg/L的L-组氨酸及pH调节剂,所述腹膜透析液中,灭菌后将第1液与第2液混合后的pH为6.7~7.5。
具体实施方式
以下,对本发明的腹膜透析液进行详细说明。本发明的腹膜透析液为下述双液型腹膜透析液,其由主要含有艾考糊精的第1液、和不含艾考糊精而含有碱性腹膜透析液的成分的第2液形成,在临使用前将第1液与第2液混合,所述腹膜透析液在混合后的pH为6.7~7.5。即,是非常接近中性(pH7)的腹膜透析液。
本发明的腹膜透析液中,第1液中的艾考糊精的含量为60.0~94.0g/L,优选为65.0~85.0g/L。艾考糊精的含量小于60.0g/L的情况下,在将第1液与第2液混合后的腹膜透析中,无法期待适当的透析,此外,第1液中含有的艾考糊精的含量大于94.0g/L的情况下,由艾考糊精产生的葡萄糖分解产物的含量变多,不优选。即,由此,能够提供适当地抑制在加热灭菌及其后的保存中由艾考糊精产生的葡萄糖分解产物,且能够进行适当的腹膜透析的优异的腹膜透析液。
在本发明的腹膜透析液中,灭菌后的第1液的pH为酸性区域,具体而言,优选为pH4.0~5.3的范围,更优选为pH4.5~5.3的范围。pH小于4.0的情况下,由艾考糊精产生的葡萄糖分解产物即5-羟甲基糠醛的量变多,且混合后的pH不会成为中性域,即不会成为优选的6.7~7.5,因此不优选。如果pH大于5.3,则由艾考糊精产生的葡萄糖分解产物即3-脱氧葡萄糖醛酮的量变多,且混合后的pH不会成为中性域,即不会成为优选的6.7~7.5,因此不优选。
第1液中的氯化钠的含量为1.42~2.34g/L,优选为1.85~2.15g/L。氯化钠是出于调节渗透压的目的而配合的,如果第1液中的氯化钠的含量大于2.34g/L,则由艾考糊精产生的葡萄糖分解产物的量变多,不优选。
在本发明的腹膜透析液中,从抑制由艾考糊精产生的葡萄糖分解产物的观点考虑,第1液不含碱性的pH调节剂。
在本发明的腹膜透析液中,灭菌后的第2液为中性至碱性区域,具体而言,优选为pH6.7~8.0的范围,更优选为pH6.7~7.5的范围。如果pH小于6.7或大于8.0,则混合后的pH不会成为6.7~7.5,因此不优选。
上述的pH可以通过pH调节剂来调整。作为pH调节剂,可以使用通常用的乳酸钠、柠檬酸三钠、磷酸三钠等,但不使用氢氧化钠及碳酸氢钠。
在本发明中,第2液包含氯化钠、乳酸钠、氯化钙及氯化镁中的至少一种。这些成分的量没有特别限定,只要含有与通常的腹膜透析液同样的量即可,优选氯化钠为16.6~347.8g/L,乳酸钠为21.3~448.0g/L,氯化钙为1.22~25.7g/L,以及氯化镁为0.24~5.10g/L。就上述成分而言,从抑制由艾考糊精产生的葡萄糖分解产物的量的观点考虑,优选将氯化钠以外的成分配合到不含艾考糊精的第2液中。
在本发明中,包含第2液的L-组氨酸(以下,称为组氨酸),其含量为300~3000mg/L,优选为600~2000mg/L。组氨酸是以抑制钙和镁的析出为目的而配合的,当第2液中的L-组氨酸的含量小于300mg/L时,混合液的pH经时下降,无法保持中性域,当大于3000mg/L时,则可能会出现因组氨酸的亚急性毒性而引起的症状,从而不优选。
本发明的腹膜透析液是将第1液和第2液在临施予之前混合而成的,pH优选为接近pH7,具体而言,优选为pH6.7~7.5。混合后的pH小于6.7时,巨噬细胞的免疫防御机制降低、对腹膜间皮细胞的抑制性高,混合后的pH大于7.5时,担心会对机体产生不良影响。
就本发明的腹膜透析液而言,将第1液和第2液分别填充·包装在聚丙烯制、聚氯乙烯制等的容器中,实施灭菌,在临使用前将第1液和第2液进行无菌混合。
特别地,对于本发明的腹膜透析液而言,优选将第1液和第2液分开收纳在下述医疗用袋体内,所述医疗用袋体具有通过利用能够打开的分隔机构将其内部隔开从而形成的第1室和第2室,所述第1室具有将医疗用袋体的内外连通的排出端口。
作为能够打开的分隔机构,可以举出在挤压第1室或第2室中的一方时可通过其收纳的第1液或第2液的液压而发生断裂的热封(heat seal)。由此,能够容易地进行第1液与第2液的混合。作为医疗用袋体,具体而言,可以举出Midpeliq(注册商标)(TERUMO株式会社制)或NICOPELIQ(注册商标)(TERUMO株式会社制)的容器。
本发明中,作为灭菌方法,可以举出高压灭菌器(autoclave)灭菌(高压蒸气灭菌),作为其条件,为110~130℃、25~45分钟,优选为115~125℃、30~40分钟。
本发明的腹膜透析液通常利用由聚丙烯-聚酰胺-聚丙烯形成的三层膜等透氧性膜材进行外包装,为防止容器内的透析液变质,也可以进一步用非透氧性膜材进行外包装。
作为上述的非透氧性膜材,可以举出例如包含乙烯-乙烯醇共聚物膜、聚乙烯醇膜、聚偏二氯乙烯膜等作为中间层的三层层压膜(例如,外层由聚酯膜、拉伸尼龙膜、拉伸聚丙烯膜等形成,内层由未拉伸聚丙烯膜形成的层压膜等)、包含铝层的层压膜(例如,由聚酯膜-铝层-未拉伸聚丙烯膜形成的层压膜等)、包含无机质蒸镀膜的层压膜(例如,由聚酯膜-硅蒸镀膜-未拉伸聚丙烯膜、拉伸尼龙膜-硅蒸镀膜-未拉伸聚丙烯膜、聚酯膜-铝蒸镀膜-未拉伸聚丙烯膜、氧化铝蒸镀聚酯膜-聚偏二氯乙烯膜-未拉伸聚丙烯膜形成的层压膜等)等。
以上,对本发明的腹膜透析液进行了详细说明,但本发明并不限定于此,可以在不脱离本发明的主旨的范围内进行各种改良及变更。
实施例
以下,举出本发明的具体的实施例,更详细地说明本发明。需要说明的是,本发明并不限定于以下实施例。
(实施例1)
将75.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。此外,将3.48g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙、0.051g的六水合氯化镁及112mg的组氨酸溶解在160mL的注射用水中,制备第2液。将840mL第1液、160mL第2液分别填充在聚丙烯制多腔室容器(Midpeliq(注册商标)(TERUMO株式会社制)的容器)中后,装入聚丙烯/尼龙/聚丙烯制的三边封袋中,进行脱气包装。然后,用高压灭菌器进行加热灭菌(121℃、30分钟)后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为4.9,第2液的pH为7.5,混合液的pH为7.1。
(实施例2)
将79.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。此外,将3.65g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙、0.051g的六水合氯化镁及112mg的组氨酸溶解在160mL的注射用水中,用氢氧化钠调节pH,制备第2液。将840mL第1液、160mL第2液在与实施例1同样的条件下进行填充、加热灭菌后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为4.8,第2液的pH为7.5,混合液的pH为7.1。
(实施例3)
将71.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。另外,将3.30g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙、0.051g的六水合氯化镁及224mg的组氨酸溶解在160mL的注射用水中,用氢氧化钠调节pH,制备第2液。将840mL第1液、160mL第2液在与实施例1同样的条件下进行填充、加热灭菌后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为4.8,第2液的pH为7.5,混合液的pH为7.2。
(实施例4)
将75.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。另外,将3.65g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙、0.051g的六水合氯化镁及56mg的组氨酸溶解在160mL的注射用水中,用氢氧化钠调节pH,制备第2液。将840mL第1液、160mL第2液在与实施例1同样的条件下进行填充、加热灭菌后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为5.0,第2液的pH为7.3,混合液的pH为7.1。
(实施例5)
将75.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。另外,将3.65g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙、0.051g的六水合氯化镁及448mg的组氨酸溶解在160mL的注射用水中,用氢氧化钠调节pH,制备第2液。将840mL第1液、160mL第2液在与实施例1同样的条件下进行填充、加热灭菌后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为5.0,第2液的pH为7.7,混合液的pH为7.4。
(比较例1)
将75.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。另外,将3.48g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙及0.051g的六水合氯化镁溶解在160mL的注射用水中,用氢氧化钠调节pH,制备第2液。将840mL第1液、160mL第2液在与实施例1同样的条件下进行填充、加热灭菌后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为5.0,第2液的pH为7.2,混合液的pH为6.5。
(比较例2)
将75.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。另外,将3.48g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙、0.051g的六水合氯化镁及28mg的组氨酸溶解在160mL的注射用水中,用氢氧化钠调节pH,制备第2液。将840mL第1液、160mL第2液在与实施例1同样的条件下进行填充、加热灭菌后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为5.0,第2液的pH为7.1,混合液的pH为6.6。
(比较例3)
将75.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。另外,将3.48g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙、0.051g的六水合氯化镁及1mg的氢氧化钠溶解在注射用水中,用氢氧化钠调节pH,制备第2液。将840mL第1液、160mL第2液在与实施例1同样的条件下进行填充、加热灭菌后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为5.0,第2液的pH为8.5,混合液的pH为7.2。
(比较例4)
将75.0g的艾考糊精和1.87g的氯化钠溶解在840mL的注射用水中,制备第1液。另外,将3.48g的氯化钠、4.48g的乳酸钠、0.257g的氯化钙、0.051g的六水合氯化镁及1g的碳酸氢钠溶解在160mL的注射用水中,制备第2液。将840mL第1液、160mL第2液在与实施例1同样的条件下进行填充、加热灭菌后,对第1液、第2液及混合液(腹膜透析液)的pH进行测定。第1液的pH为4.9,第2液的pH为7.9,混合液的pH为7.2。另外,在第2液中,从0个月开始产生了钙及镁的沉淀。
(经时变化试验)
对于实施例1~5以及比较例1~4,测定了混合液的pH、钙、镁的经时性变化。将结果示于表1和2中。需要说明的是,腹膜透析液的保存在40℃的恒温槽中进行。

如表1所示,比较例1和2中,混合液的pH从0个月开始就低于6.8。另外,就比较例3而言,如表2和3所示,混合后(0个月)即产生了钙及镁的沉淀(虽然为少量),因此钙及镁的浓度降低。之后(1~3个月),如表1所示,pH降低。就比较例4而言,如表2和3所示,从0个月即在刚制造后就产生了钙及镁的沉淀,因此钙和镁的浓度降低。
即,就本发明的腹膜透析液而言,混合时接近pH,而且也不会出现钙及镁的沉淀,并且在混合后也能够长时间保持稳定。具体而言,能够在从混合后到三个月为止的期间内,在40℃的恒温槽中将pH维持为6.7~7.5。
产业上的可利用性
如以上详细地说明过的,本发明的腹膜透析液可作为下述腹膜透析液在产业上进行利用,所述腹膜透析液能够最大限度地抑制在加热灭菌及其后的保存中由艾考糊精产生的葡萄糖分解产物,而且能够使混合液的pH非常接近生理中性区域(具体为pH7.0),并且在该状态下的稳定性优异。
本发明的腹膜透析液为下述腹膜透析液。
(1)本发明为一种腹膜透析液,其是由酸性的第1液、和第2液形成且经灭菌的腹膜透析液,所述第1液含有艾考糊精,所述第2液含有300~3000mg/L的L-组氨酸及pH调节剂,所述腹膜透析液中,灭菌后将第1液与第2液混合后的pH为6.7~7.5。
本发明的腹膜透析液为双液型腹膜透析液,其由主要含有艾考糊精的第1液、和不含艾考糊精而含有中性至碱性的腹膜透析液成分的第2液形成,在临使用前将第1液及第2液混合,所述腹膜透析液能够提高艾考糊精在加热灭菌及其后的保存中的稳定性,而且在临施予前混合时,不会在混合后析出其他成分,可使pH接近生理中性区域,并且其稳定性高。
另外,本发明的腹膜透析液也可为下述腹膜透析液。
(2)本发明为上述(1)所述的腹膜透析液,其是由酸性的上述第1液、和上述第2液形成且经灭菌的腹膜透析液,所述第1液包含60.0~94.0g/L的所述艾考糊精以及1.42~2.34g/L的氯化钠,所述腹膜透析液中,上述第1液不含pH调节剂,灭菌后的上述第1液的pH为4.0~5.3,灭菌后的上述第2液的pH为6.7~8.0,灭菌后将第1液与第2液混合后的pH为6.7~7.5。
(3)本发明为上述(1)或(2)所述的腹膜透析液,其中,上述第2液包含16.6~347.8g/L的氯化钠、21.3~448.0g/L的乳酸钠、1.22~25.7g/L的氯化钙、以及0.24~5.10g/L的氯化镁中的至少一种。
(4)本发明为上述(1)~(3)中任一项所述的腹膜透析液,其中,上述灭菌条件为110~130℃、25~45分钟。
(5)本发明为上述(1)~(4)中任一项所述的腹膜透析液,所述腹膜透析液能够在从混合后到三个月为止的期间内,在40℃的恒温槽中将pH维持为6.7~7.5。
Claims (5)
1.腹膜透析液,其是由酸性的第1液、和第2液形成且经灭菌的腹膜透析液,所述第1液含有艾考糊精,所述第2液含有300~3000mg/L的L-组氨酸及pH调节剂,所述腹膜透析液的特征在于,灭菌后将所述第1液与所述第2液混合后的pH为6.7~7.5。
2.如权利要求1所述的腹膜透析液,其是由酸性的所述第1液、和所述第2液形成且经灭菌的腹膜透析液,所述第1液包含60.0~94.0g/L的所述艾考糊精以及1.42~2.34g/L的氯化钠,所述腹膜透析液中,所述第1液不含pH调节剂,灭菌后的所述第1液的pH为4.0~5.3,灭菌后的所述第2液的pH为6.7~8.0,灭菌后将所述第1液与所述第2液混合后的pH为6.7~7.5。
3.如权利要求1或2所述的腹膜透析液,其中,所述第2液包含16.6~347.8g/L的氯化钠、21.3~448.0g/L的乳酸钠、1.22~25.7g/L的氯化钙、以及0.24~5.10g/L的氯化镁中的至少一种。
4.如权利要求1~3中任一项所述的腹膜透析液,其中,所述灭菌条件为110~130℃、25~45分钟。
5.如权利要求1~4中任一项所述的腹膜透析液,所述腹膜透析液能够在从混合后到三个月为止的期间内,在40℃的恒温槽中将pH维持为6.7~7.5。
