Abstract
本发明提供一种用于评价肠黏膜通透性的诊断药,其主要成分为壳多糖和/或壳聚糖。优选将所用的壳多糖和/或壳聚糖的重均分子量配制为1000~11600的范围。
Description
技术领域
本发明涉及一种评价肠黏膜通透性的技术,尤其涉及一种评价肠漏症(Leaky GutSyndrome:漏肠综合症)的程度的技术,以及找出或评价其诱发因子或抑制因子的技术。
背景技术
基于肠黏膜通透性的疾病种类繁多,例如,肠漏症(以下为方便起见,简称为“LGS”)即为一种。LGS是指食物分子或异物等从肠黏膜进入至血液中的症状,认领其伴有腹泻或过敏症状。
已知一种双糖试验可以用来判定是否属于LGS。
这是一种通过让受试者同时服用10g乳果糖(分子量≈340)和5g甘露醇(分子量≈180),并测定尿液中乳果糖与甘露醇的浓度比(L/M比),来评价肠道的损伤即渗漏程度的方法。此试验利用的原理是:从分子量的角度出发,在受试者身体健康的情况下,甘露醇会透过肠黏膜,而乳果糖基本不会透过肠黏膜。予以说明,测定中使用的是液相色谱-质谱联用仪等。
此外,动物实验已知还有FITC-dextran试验。
这是一种经口给予平均分子量约为4000的FITC-dextran(荧光素标记葡聚糖),并测定其在血液中浓度的试验。予以说明,测定中使用的是荧光定量装置等。
但是在现有技术中存在以下问题。
首先,由于未充分确定LGS的定义,因此未确立其检测方法和诊断方法。反之也可认为,由于未确立LGS的检测方法和诊断方法,因此未确定其定义。
实际上,由于双糖试验也使用尿液,若有肾功能障碍时,则无法准确测定。此外,由于乳果糖或甘露醇之类的糖会通过特殊的转运体被从肠黏膜吸收,也可能存在转运体自身造成的输送障碍。
此外,由于乳果糖的分子量较小,因此也存在不能评价是否处于分子量更大的物质被肠吸收(转移至血液)的状态的问题。
另一方面,FITC-dextran虽然在一定程度上分子量较大,但是由于FITC具有毒性,因此存在无法用于人体的问题。
而且,很多报告表明,肠炎患者或高脂肪食物过度摄取者的血液中有平均分子量为5000~8000的脂多糖,原本就不存在对大到这种程度分子量的渗漏性进行评价的技术。
即,现有技术存在以下问题:对以LGS为代表的肠黏膜通透性的评价是间接的,可信度不高,评价范围窄小。

发明内容
发明要解决的课题
本发明即为解决上述问题而完成的,其目的在于,提供一种能够直接且高可靠性地评价肠黏膜通透性程度的技术。作为一例,其目的在于提供一种诊断LGS等肠黏膜通透性增高的技术。
此外,本发明的目的在于,提供一种判定对肠黏膜的通透性产生影响的饮食品的技术。作为一例,其目的在于提供一种判定诱发或抑制LGS的饮食品的技术。
此外,本发明的目的在于,提供一种对于使肠黏膜通透性正常化的药物或其候选物质进行客观评价的技术。作为一例,其目的在于提供一种促进LGS治疗药、LGS缓解药、肠黏膜通透性调节药的开发的技术。
解决问题的技术方案
方案1为,一种用于评价肠黏膜通透性的诊断药,其主要成分为壳多糖和/或壳聚糖。
予以说明,该诊断药既可以用于经口给药,也可以用于灌肠给药。诊断对象也可以是人以外的动物。
方案2为,一种用于评价肠黏膜通透性的诊断药,其主要成分为壳多糖和/或壳聚糖,其特征在于,用于对受试者实施经口给药或灌肠给药,并在经过预定时间后测定血液中浓度。
所谓的预定时间可以适当设定,若经口给药,可以设定为30分钟,若灌肠给药,可以设定为5分钟。
方案3为,根据方案1或方案2所述的诊断药,其特征在于,将壳多糖和/或壳聚糖的重均分子量配制为1000~11600的范围。
所谓将重均分子量配制为1000~11600,是指满足下述任一情形:(1)平均分子量是在1000~11600范围中的任一个;(2)平均分子量的峰值为1000~11600之间的多个;(3)分子量为1000~11600的物质事实上同样存在。若属于(1)的情形时,可确认该分子量的物质是否透过肠黏膜。若属于(2)的情形,利用各峰值的半值宽度窄的药剂,通过一次试验,可以高精度地确认大致何种范围分子量的物质透过肠黏膜。若属于(3)的情形,通过一次试验,也能确认大致何种范围分子量的物质透过肠黏膜。
方案4为,根据方案2所述的诊断药,其特征在于,将给药量设为每公斤体重8.33mg~20.83mg的范围。
摄取量小于双糖试验,能够减轻受试者的负担。
方案5为,一种诊断方法,其特征在于,对动物(人以外的动物)经口给予或灌肠给予壳多糖和/或壳聚糖,并在经过预定时间后测定血液中该给予物的浓度,由此评价该动物肠黏膜的通透性。
所谓给予物的浓度,若仅给予壳多糖,是指壳多糖浓度;若仅给予壳聚糖,是指壳聚糖浓度;若给予壳多糖和壳聚糖的混合物,是指壳多糖和壳聚糖的混合物的浓度。
方案6为,一种诊断方法,其特征在于,对受试者经口给予或灌肠给予壳多糖和/或壳聚糖,并在经过预定时间后测定血液中的该给予物的浓度,由此评价该受试者肠黏膜的通透性。
方案7为,根据方案5或方案6所述的诊断方法,其特征在于,将壳多糖和/或壳聚糖的重均分子量配制为1000~11600的范围。
方案8为,根据方案6所述的诊断方法,其特征在于,将给药量设为每公斤体重8.33mg~20.83mg的范围。
方案9为,一种壳多糖和/或壳聚糖的用途,其特征在于,经口给药及经过预定时间后测定血液中浓度,或者灌肠给药及经过预定时间后测定血液中浓度,由此作为用于评价肠黏膜的通透性。
优选将壳多糖和/或壳聚糖的重均分子量配制为1000~11600的范围。
若给药对象为人时,给药量优选为每公斤体重8.33mg~20.83mg的范围。
方案10为,一种饮食品评价方法,其特征在于,让受试者食用单个或多个指定的饮食品,且在该食用期间或者在该食用之前或之后经口摄取壳多糖和/或壳聚糖,并在从该经口摄取起经过预定时间后测定血液中该摄取物的浓度,由此判定该饮食品是否可能成为影响该受试者肠黏膜通透性的因素。
优选将壳多糖和/或壳聚糖的重均分子量配制为1000~11600的范围。
摄取量优选为每公斤体重8.33mg~20.83mg的范围。
所谓影响,是指既包括增强肠黏膜的通透性或渗漏性,也包括减轻肠黏膜的通透性或渗漏性。关于LGS,增强渗漏性是指诱发LGS或使LGS恶化;所谓减轻渗漏性,是指抑制、缓解、改善、治愈LGS。
从抑制LGS的角度出发,指定的单个饮食品例如可举出:酸奶或乳清饮料。随之,也能够对宣传可调节肠道菌群的食品提供客观的性能指标。
从诱发LGS的角度出发,多个指定的饮食品例如可举出:牡蛎和葡萄酒、猪排和啤酒。
根据本发明,可以对每个人进行诱发因子、抑制因子的筛查。
所谓饮食之前或之后,是指例如可举出:饮食开始前20分钟、饮食结束后15分钟。
方案11为,一种饮食品评价药,其主要成分为壳多糖和/或壳聚糖,其特征在于,让受试者食用单个或多个指定的饮食品,且在该食用期间或者在该食用之前或之后经口摄取壳多糖和/或壳聚糖,并在从该经口摄取起经过预定时间后测定血液中该摄取物的浓度,由此判定该饮食品是否可能成为影响该受试者肠黏膜通透性的因素。
优选将壳多糖和/或壳聚糖的重均分子量配制为1000~11600的范围。
优选摄取量为每公斤体重8.33mg~20.83mg的范围。

方案13为,一种评价剂,其主要成分为壳多糖和/或壳聚糖,其用于以下评价:给予某种物质,再另外经口给予或灌肠给予该评价剂,并在给予所述某种物质之前和之后测定所述评价剂的血液中浓度,由此判定所述某种物质对肠黏膜通透性的正常化有无作用,当对正常化有作用时则评价该作用为何种程度。
某物质的给药对象既可以是人,也可以是人以外的动物。

方案14为,一种诊断装置,其特征在于,具备:浓度测定装置,其测定从受试者采集的血液中的壳多糖和/或壳聚糖的浓度;以及评价装置,其根据由浓度测定装置测定的浓度来评价该受试者肠黏膜的通透性。也可以称之为肠黏膜通透性评价装置。
发明的效果
根据本发明,能够直接且高可靠性地评价肠黏膜的通透性的程度。
此外,根据本发明,能够判定对肠黏膜的通透性产生影响的饮食品。
此外,根据本发明,能够客观地对促使肠黏膜的通透性正常化的医药或其候选物质进行评价。

具体实施方式
<LGS诱发模型的构建>
本实施方式中,作为评价肠道的渗漏性的例子设定为LGS,首先进行LGS诱发试验。
其中一个是使用阿司匹林和奥美拉唑的模型(以下简称“AO模型”)。根据文献(Innate Immun.2015Jul;21(5):537-45)对条件进行了改变。
试验概要:对小鼠一日2次、连续6天按每公斤体重100mg阿司匹林(100mg/kg)进行经口给药,且对小鼠一日2次、连续6天按每公斤体重10mg奥美拉唑(10mg/kg)进行腹腔内给药,并在第7天测定LGS的程度。
另一个是肠道缺血再灌注模型(以下简称“IR模型”)。根据文献(Gastroenterology.2001Feb;120(2):460-9.)等对条件进行了改变。
试验概要:持续夹住肠道30分钟以形成缺血状态,然后松开夹子再灌注,在30分钟后测定LSG的程度。予以说明,作为明示缺血时间为30分钟的标记,适当标记为IR(30)模型。
两个模型的概要如图1所示。予以说明,图1b中,示出了后述的IR(20)模型和IR(10)模型。
图2中示出了肠道的外观照片、HE染色图、电子显微镜照片。与正常组织相比,AO模型中,可见肠绒毛断裂,可认为发生了轻度LGS。此外,IR(30)模型中,肠绒毛受损较大,可认为发生了重度LGS。
<LGS的程度评价>
利用上述模型,可确认因分子量不同所造成的渗漏性。
首先,对于AO模型进行双糖试验。按500mg/kg一起经口给予乳果糖和甘露醇,蓄尿4小时,测定L/M比。结果示于图3。与均未给予阿司匹林和奥美拉唑而未诱发LGS的小鼠、即与对照相比,AO模型中L/M比提高至约2倍。由此,可确认处于如甘露醇这种分子量超过300的物质的渗漏性增高的状态。
接着,对于AO模型进行FITC-dextran试验。按600mg/kg经口给予FITC-葡聚糖,60分钟后取血,测定其在血浆中的浓度。结果示于图4。与对照相比,可确认血浆中的葡聚糖有一定程度的提高(图4b)。
另外,对于IR(30)模型进行FITC-dextran试验。按600mg/kg经口给予FITC-葡聚糖,30分钟后开始缺血,再灌注30分钟后取血,测定其在血浆中的浓度。结果示于图5。与对照相比,血浆中的葡聚糖浓度提高至约15倍。
将图4a和图5对比可知,IR(30)模型与AO模型相比,可认为是能够验证处于使分子量较大的物质更容易转移至血液的状态的模型。换言之,IR(30)模型可认为是能够评价重度LGS的模型。
反之,图3和图4b对比可知,可认为AO模型是能够验证处于尚未到使分子量较大的物质容易转移至血液的状态的模型。换言之,AO模型可认为是能够评价轻度LGS的模型。
<使用壳多糖-壳聚糖的LGS的评价>
接着,进行使用壳多糖-壳聚糖的LGS评价。
首先,对蟹类的甲壳实施脱蛋白处理、脱钙处理、脱乙酰处理,得到壳多糖和壳聚糖的混合物。然后,对壳多糖和壳聚糖的混合物进行低分子化处理,得到重均分子量为7900的壳多糖-壳聚糖试样。对低分子化的方法不做特别限定,可以是利用浓盐酸对壳多糖和壳聚糖的混合物进行加水分解的方法(日本专利特许5714963号),也可以是利用盐酸、醋酸、柠檬酸、乳酸等有机酸进行溶解后,利用壳聚糖酶进行低分子化处理的方法(日本专利特开2013-79217)。
接着,对于AO模型,在第7天经口给予上述试样2.50mg,60分钟后取血,测定了血浆中的壳多糖-壳聚糖的浓度(参照图1a)。
此外,对于IR(30)模型,在缺血开始30分钟前经口给予上述试样2.50mg,再灌注30分钟后取血,测定血浆中的壳多糖-壳聚糖的浓度(参照图1b)。
测定时,首先,按照常规从血液中提取血浆,用亚硝酸分解法,将血浆中的壳多糖-壳聚糖分解为构成单糖即壳糖(2,5-脱水-D-甘露糖)。然后,使该醛基使MBTH(3-甲基-2-苯并噻唑啉酮腙盐酸盐水合物)和氯化铁(Ⅲ)发生反应,显色为蓝色。最后,利用吸光度测定装置测定蓝色的程度,基于其色素量计算浓度。
结果参见图6。如图所示,可确认与AO模型相比,IR(30)模型更明显地使壳多糖-壳聚糖转移至血液中。这与所使用的试样的分子量较大、图4b和图5之间的关系相符。
因此,本申请发明人等研究了将其用作壳多糖-壳聚糖的LGS程度的诊断药,进而作为诊断肠黏膜通透性的诊断药(评价肠黏膜通透性的评价药)的可应用性。予以说明,虽然上述方法中利用吸光度进行浓度评价,但并不限定于此,只要能够检测血液中浓度,则无需特别区分壳多糖与壳聚糖。因此,在本申请中标记为壳多糖-壳聚糖,其含义是壳多糖和/或壳聚糖。
首先,配制盐酸或有机酸的溶解液,利用UF膜(截留分子量为3000、6000、10000等)将重均分子量为7900的壳多糖-壳聚糖试样分级为重均分子量为1000、3000、7900、11600的壳多糖-壳聚糖。图7表示所配制的试样的分布情况。此外,也附记了重均分子量的测定方法。
接着,对于IR模型,分别经口给予上述试样2.5mg,测定壳多糖-壳聚糖在血液中的浓度。予以说明,考虑到缺血时间为30分钟时对肠道的损伤过大,此处将缺血时间缩短为20分钟进行试验。将此模型标记为IR(20)模型。
对于IR(20)模型,各分子量的壳多糖-壳聚糖在血液中浓度的测定结果如图8所示。予以说明,图中还记载有对照即无缺血再灌注的结果。由该图可知,虽然重均分子量为11600的浓度略有降低,但是基本所有分子量的试样均有转移至血液中。
因此,将缺血时间调整为10分钟,进一步降低LGS的程度,诱发中等程度的LGS,研究试样的筛选性。将该试验标记为IR(10)模型。
对于IR(10)模型,各分子量的壳多糖-壳聚糖在血液中浓度的测定结果如图9所示。如图所示,可确认到重均分子量越小的试样越容易渗漏,而重均分子量越大的试样越不易渗漏。
由上述实验可知,如果利用壳多糖-壳聚糖,则能够评价何种程度分子量的物质会从肠道渗漏,此外能够评价处于何种程度分子量的物质会以何种程度从肠道渗漏的状态。
即,在经口给药和经过预定时间后测定血液中浓度,由此评价肠黏膜通透性时,可使用壳多糖和/或壳聚糖。
这也可认为得到了一种主要成分为壳多糖和/或壳聚糖的用于评价肠黏膜通透性的诊断药。
另外,也可认为得到了一种主要成分为壳多糖和/或壳聚糖的用于评价肠黏膜通透性的诊断药,其特征在于:用于让受试者经口摄取并经过预定时间后测定血液中浓度。
也可认为得到了一种评价肠黏膜通透性增高的技术。
予以说明,如图8及图9所示,IR模型能够通过调节缺血时间来诱发从轻度到重度的任意程度的LGS,可认为IR模型是一种能够构建具有通用性的客观评价体系的模型。
予以说明,对于IR(20)模型,按1.25mg/只小鼠、0.625mg/只小鼠分别经口给予重均分子量为1000的诊断药,测定减少给药量时的血液中浓度。结果如图10所示。由该图可知,即使按0.625mg/只小鼠经口给予诊断药,也能测定到显著性浓度。如果将其换算成体重60kg的人,则诊断药的给药量为1.25g。此外,根据同时参考对照的测定极限,理论上即使对60kg的人经口摄取0.500g,也能测定到显著性浓度。即,给药量或摄取量只要在每公斤体重8.33mg~20.83mg的范围即可。如果考虑到从体重5kg的儿童到体重200kg的成人,则可认为经口摄取量可设为0.04g~4.20g的范围。总之,无需如双糖试验那样的经口摄取15g,可认为是一种能够减轻受试者负担的诊断药。
在灌肠给药时,也是同样。
另外,虽然仍依赖检测系统,优选将诊断药的重均分子量配制为200~20000、优选1000~11600的范围。图11是诊断药的分子量的分布形态和浓度测定结果的示意图。图11a表示单峰的诊断药。能够判定在该分子量以下至少是否处于渗漏状态。图11b表示多峰的诊断药。能够判定何种程度的分子量以下容易渗漏。图11c表示分子量分布宽度相同的诊断药。这也能判定何种程度的分子量以下容易渗漏。
以上的诊断药或诊断方法具有以下优势:
·无毒性(可用于人,也可用于动物实验)。
·可经口给药。
·包括肠在内的生物体内不产生(能够进行直接的浓度测定)。
·在生物体内难以分解(能够进行直接的浓度测定)。
·正常状态下几乎不被肠道吸收。
·可以配制分子量。
<使用高度纯化壳多糖-壳聚糖的试验>
接着,使用分子量的分布更加尖锐的壳多糖-壳聚糖进行试验。图12是表示以后试验中所用的壳多糖-壳聚糖试样的分子量分布情况的图。与图7相比,分布尖锐,由另行分析的结果可知,分子量为823~1984的物质(盐酸盐的分子量约为1000~1200)占整体的98wt%,为高度纯化的试样。予以说明,在此将该试样简称为纯化试样。
对于IR(20)模型,使用纯化试样测定血清中壳多糖-壳聚糖的浓度。测定结果与肠道的外观和HE染色图如图13所示。虽然从HE染色图观察到黏膜障碍略有发展,但与图8相同,可确认处于分子量约为1000的壳多糖-壳聚糖渗漏到血液中的状态。
<安全性确认>
在对小鼠经口给予纯化试样的IR(10)模型中,测定循环血液中的壳多糖-壳聚糖量的经时变化。再灌注30分钟以后的经时变化如图14所示。经口给药量为2.5mg/只小鼠。
小鼠的循环血液量推算为其体重的1/13,当小鼠的体重为25g时,则其循环血液量应为1.93ml。另一方面,图14所示的AUC(Area Under the Curve,ROC曲线下面积)在0~8h时为162.16μg/ml。因此,8小时期间内渗漏至循环血液中的壳多糖-壳聚糖量为162.16*1.93=312.97μg。由以上可知,可认为经口给药量12.5%的纯化试样渗透至循环血液中(312.97/2.5=0.125)。予以说明,8小时后,小鼠仍然存活。
另外,向未经处置即未进行缺血再灌注处置的小鼠静脉给予纯化试样2.5mg,测定壳多糖-壳聚糖在血液中浓度的经时变化。测定结果如图15所示。确认到即使静脉给药,血液中浓度仍约为70μg/ml(通过外推法推算刚给药后的浓度,也约为150μg/ml),壳多糖-壳聚糖在到约2小时时从血液中迅速消失(如果重新绘制为对数标度,则呈直线型的浓度衰减)。予以说明,这应是肾排泄所引起的。予以说明,未发现静脉给药所导致的休克症状,2小时后小鼠仍然存活。
根据上述两个试验,可认为如下:
1)试验后小鼠仍然继续存活,再次确认到壳多糖-壳聚糖对生物体而言是安全的(至少证明立即产生严重影响的可能性极低)。
2)从血液中迅速消失的现象,也可证明壳多糖-壳聚糖对生物体而言是安全的。
3)在上述条件下,由于8小时后仍从血液中检测出壳多糖-壳聚糖,说明给药后经过一定时间后,仍可进行浓度测定,便于作为诊断药和评价药使用。此外,也能够掌握经时变化。
4)由于渗漏至血液中后会迅速消失,因此能够通过经口给药或灌肠给药来掌握肠的渗漏性和通透性的实时状态。也可认为能够更纯粹地掌握肠的状态。
<除IR模型以外的其他模型的通透性评价>
接着,对于如缺血再灌注这种直接对肠施加负荷的上述评价试验以外的其他模型,进行通透性或渗漏性的评价。
<食物过敏模型的通透性评价>
首先,对于对鸡蛋过敏的小鼠即OVA-IgE小鼠进行小肠的黏膜通透性评价。OVA过敏诱导步骤如图16所示,首先,向小鼠腹腔内给予OVA(卵清蛋白)使其致敏,然后持续经口给药,在致敏起28天后经口给予OVA和纯化试样而进行评价。予以说明,确认到OVA-IgE小鼠具有食物过敏的典型症状的重度腹泻。
壳多糖-壳聚糖在血液中浓度测定结果如图17所示。予以说明,为了对比,也示出了进行双糖试验的结果。如图所示可知,在发生食物过敏导致的腹泻时,处于肠的通透性增高的状态。予以说明,可知通过双糖试验能够明确判断通透性的增高。
<高脂肪食物模型和NASH诱发食物模型的通透性评价>
接着,对于分别持续给予高脂肪食物和非酒精性脂肪性肝炎(NASH)诱发食物的小鼠,进行通透性评价。
高脂肪食物和NASH诱发食物的成分如下所示。

予以说明,上述类脂质中的胆固醇量和碳水化合物中的果糖量分别如下所示:
高脂肪食物模型:先使6周龄雄性C57BL/6小鼠自由摄取上述高脂肪食物5个星期,然后经过断水断食21小时后,经口给予纯化试样2.5mg,再过1小时后从下腔静脉取血,测定壳多糖-壳聚糖量。
NASH诱发食物模型:先使6周龄雄性C57BL/6小鼠自由摄取上述NASH诱发食物4个星期,然后经过断水断食21小时后,经口给予纯化试样2.5mg,再过1小时后从下腔静脉取血,测定壳多糖-壳聚糖量。
结果如图18所示。对于高脂肪食物,虽然通透性没有提高,但是NASH诱发食物的通透性有明显提高。由此首先可以确认,在非酒精性脂肪性肝炎中处于肠的通透性增高的状态。其次,由于在该期间摄取该组成的高脂肪食物并未使肠的渗漏性增高,反之可知,如果利用壳多糖-壳聚糖,则可筛查何种食物会如何对肠产生影响。
<DSS诱发溃疡性结肠炎模型的通透性评价>
接着,对由DSS(葡聚糖硫酸钠)诱发溃疡性结肠炎的小鼠进行通透性评价。
溃疡性结肠炎模型:首先使小鼠自由饮用溶解有2.5wt%DSS的水。从开始自由饮水72小时后,灌肠给予或经口给予纯化试样。对于灌肠给药的小鼠,在给药后1小时后测定壳多糖-壳聚糖在血液中的浓度。对于经口给药的小鼠,在给药后4小时后测定壳多糖-壳聚糖在血液中的浓度。
图19是使小鼠在72小时(3天)饮用DSS时以及没有饮用DSS时的肠整体图像和HE染色图。如图所示,从开始自由饮水的72小时期间,肠黏膜未发现炎症,肠整体也未观察到明显的变化。
但是,如图20所示,不论是经口给药还是灌肠给药,都观察到壳多糖-壳聚糖渗漏到血液中。而且,继续观察后,自由饮用DSS的小鼠在从开始饮水96小时(4天)后,另行确认到发生了炎症。
即,令人吃惊的是,确认到通过使用壳多糖-壳聚糖,能够检测溃疡性结肠炎在发病前的肠黏膜的异常通透性增高。
<作为药理作用评价剂的壳多糖-壳聚糖>
如图21所示,不仅限于溃疡性结肠炎,一般认为肠黏膜的通透性增高在炎症或障碍发生之前产生。因此,通过使用壳多糖-壳聚糖来掌握通透性或渗漏性,能够实现发病前诊断、预防发病、预测发病以及治疗药等药理作用的评价。
首先,因食物摄取等而引起炎症时,在以往,如果不在发病前持续摄取,则无法判断哪个是病因食物或病因食物组,而根据本发明,能够在发病之前停止摄取,从而减轻负担并进行有效的筛查。
其次,本发明还可用于LGS治疗药、LGS缓解药、肠黏膜通透性调节药等药物开发时的筛选。即,给予某种物质(候选物质),再另外经口给予或灌肠给予壳多糖和/或壳聚糖,并在给予候选物质之前和之后测定壳多糖和/或壳聚糖在血液中浓度,由此评价候选物质对肠黏膜通透性的正常化有无作用,当对肠黏膜通透性的正常化有作用时则评价该作用为何种程度。也可提供作为采用上述使用方法的评价剂,其主要成分为壳多糖和/或壳聚糖。
另外,还可积累对溃疡性结肠炎或克罗恩病等炎症性肠病(IBD:InflammatoryBowel Disease),以及嗜酸性粒细胞性胃肠炎的见解。也可积累对肠易激综合症的见解。即,根据本发明能够判定病情处于活跃期还是正在缓解,判定治疗效果或药效评价如何,以及预测病情等。
<诊断药的应用>
另外,考虑到图18所示的结果,壳多糖-壳聚糖也可用作饮食品的评价药。
即,让受试者食用单个或多个指定的饮食品,且在该食用期间或者在该食用之前或之后经口摄取主要成分为壳多糖-壳聚糖的评价药,并在从该经口摄取起经过预定时间后测定血液中该壳多糖-壳聚糖的浓度,由此能够判定该饮食品对该受试者肠黏膜的通透性有无影响。
如果是LGS,则可判定是否诱发LGS或者是否抑制LGS。予以说明,根据饮食品的种类不同,评价药的经口摄取可以适当在食用前、食用期间或食用后进行。
如果使用该评价药,则不仅能够筛查一般性的诱发因子或抑制因子,还能对于每个人进行诱发因子或抑制因子的筛查。即,能够确定每个人的风险因素。
例如,首先让被检测者饮用评价药,然后让受试者边喝啤酒边吃肉。与此同时,每10分钟取血一次,由此能够观察LGS的经时性推移。结果发现,对于此人而言,其饮食组合在约20分钟导致中等程度LGS发病,因此可以忠告此人避免上述饮食组合。
而且,如果使用该饮食品评价药,则能够对例如宣传可调节肠道菌群或肠内环境功能的食品即益生元食品提供客观的性能指标。而且,还能评价益生元食品。
予以说明,还能构建应用上述技术的诊断装置。
即,可得到具备如下装置的诊断装置:浓度测定装置,其测定从受试者采集的血液中的壳多糖和/或壳聚糖的浓度;评价装置,其根据由浓度测定装置测定的浓度来评价该受试者肠黏膜的通透性。
浓度测定装置的基本技术例如可使用色谱法。从导入试样到计算浓度可以使用通用技术并适当进行自动化处理。
评价装置例如可以对所得到的色谱峰的位置或高度进行分析或是对峰面积进行计算等,由此判定渗漏性的程度。不仅能够判断是否属于重度LGS或轻度LGS,还能根据峰的分布或者不均并参考以前的数据,做出以下诊断:虽属于不易发生中等程度LGS或重度LGS的体质但属于轻度LGS经常发病的体质,或者属于一旦运动则容易中等程度LGS发病的体质,或者并非LGS而是另外的肠病等。
此外,也可制作特异性检测抗体,采用使用了该特异性检测抗体的ELISA试剂盒。据此,能够一次性评价大量的试样。
而且,用于测定或诊断的血液不回输到人体,而予以废弃。
【产业上的可利用性】
根据本发明,能够构建LGS的客观指标和诊断标准。此外,还能够研究探求LGS与各种疾病间的关系。例如,能够研究与慢性肾障碍、支气管哮喘、I型糖尿液病、食物过敏、酒精性肝炎、非酒精性脂肪性肝炎等的关联。还有助于肠病治疗药的开发。
Claims (4)
1.壳多糖和/或壳聚糖在制备用于评价肠黏膜的通透性的试剂中的用途,其特征在于,
经口给药及经过预定时间后测定血液中浓度,或者灌肠给药及经过预定时间后测定血液中浓度,由此作为用于评价肠黏膜的通透性。
2.壳多糖和/或壳聚糖在制备用于判定饮食品是否可能成为影响受试者肠黏膜通透性的因素的试剂中的用途,其特征在于,
让受试者食用单个或多个指定的饮食品,且在该食用期间或者在该食用之前或之后经口摄取壳多糖和/或壳聚糖,并在从该经口摄取起经过预定时间后测定血液中该摄取物的浓度,由此判定该饮食品是否可能成为影响该受试者肠黏膜通透性的因素。
3.壳多糖和/或壳聚糖在制备用于评价候选物质对肠黏膜通透性的正常化有无作用,当对正常化有作用时则评价该作用为何种程度的试剂中的用途,其特征在于,
给予所述候选物质,再另外经口给予或灌肠给予壳多糖和/或壳聚糖,并在给予所述候选物质之前和之后测定所述经口给予物或所述灌肠给予物的血液中浓度,由此评价所述候选物质对肠黏膜通透性的正常化有无作用,当对正常化有作用时则评价该作用为何种程度。
4.一种诊断装置,其特征在于,
具备:
浓度测定装置,其测定从受试者采集的血液中的壳多糖和/或壳聚糖的浓度;以及
评价装置,其根据由浓度测定装置测定的浓度来评价该受试者肠黏膜的通透性。
