背景技术
淋巴瘤是由淋巴细胞(一种白细胞)病变引起的一类造血系统恶性肿瘤,临床一般包括淋巴结肿大但无痛感,发热,盗汗,消瘦,瘙痒以及乏力等全身症状。淋巴瘤根据不同分类方法有很多亚型,主要的分类方法是根据瘤细胞分为非霍奇金淋巴瘤(NHL)和霍奇金淋巴瘤(HL)两类。病理学特征在HL中表现为瘤组织内含有淋巴细胞、嗜酸性粒细胞、浆细胞和特异性的里-斯(Reed-Steinberg)细胞。NHL发病率远高于HL,约占90%,是具有很强异质性的一组独立疾病的总和。病理上主要是分化程度不同的淋巴细胞、组织细胞或网状细胞,根据NHL的自然病程,可以归为三大临床类型,即高度侵袭性、侵袭性和惰性淋巴瘤。淋巴瘤根据不同的淋巴细胞起源,还可以分为B细胞、T细胞和NK细胞淋巴瘤。
淋巴瘤的发病诱因不清,霍奇金淋巴瘤的风险因素包括巴尔病毒感染和疾病的家族历史,常见的非霍奇金淋巴瘤一般认为与基因突变,病毒及其他病原体感染、放射线、化学药物,合并自身免疫病等有关。
以弥漫大B细胞淋巴瘤(DLBCL)为例,它是发病率最高的NHL,在西方国家占成人NHL的25%~35%,在亚洲国家占成人NHL大于40%。其病理形态表现为体积大于正常B细胞的异常B细胞弥漫性生长,使正常的淋巴结结构完全消失。现阶段DLBCL的诊断主要依靠活检和B细胞免疫表型的证据而得出。大多数DLBCL患者表现为一个或多个淋巴结迅速肿大,结外部位通常为出现迅速增大的肿块。大约50%的患者处于临床分期Ⅰ-Ⅱ期。大部分患者没有明显的临床症状,约1/3的患者有B症状(发热、盗汗、体重减轻),且症状的出现多与肿瘤累及的部位有关,这给DLBCL的快速确诊增加了一定难度。同时,少数的一些DLBCL会因为病灶在胃部或胸部等原因,进一步增加确诊的难度,耽误治疗。因此,能够早期快速准确的发现DLBCL对患者至关重要。
由于成熟的红细胞是无核的,这就意味着成人外周血中出现有核红细胞均属病理现象,如果成人外周血内出现大量的晚幼红细胞或中幼红细胞,意味着骨髓中红细胞系增生明显活跃,则有可能是增生性贫血,而如果成人外周血中出现大量的原红细胞、早幼红细胞的话,意味着骨髓中幼稚红细胞异常增生并释放入血,则有可能是红血病或者红白血病,如果成人外周血中各个发育阶段的红细胞都可以见到,并且可以见到幼稚粒细胞及巨核细胞,则有可能是髓外造血。特别地,针对造血系统恶性疾病来说,骨髓充满大量白血病细胞使幼红细胞提前释放,有核红细胞以中幼晚幼为主。
目前,针对DLBCL的治疗主要以CHOP化疗方案为主,根据不同病情会调整不同的化疗方案,如R-CHOP化疗方案的使用(在CHOP方案中加入抗CD20的单克隆抗体-美罗华),可以使生存率显著性提高。由于DLBCL发病因素,发病年龄,病情分级的多样性,化疗方案需要实时跟进,但现阶段对于化疗效果的评估仍需要通过医生的主观经验和穿刺后得到的病理结果,这种对患者反复进行侵入性的穿刺手段对患者而言也是一种痛苦。因此,能简单有效评估患者预后的技术也是亟需的。
发明内容
针对上述的不足,本发明提供了检测红细胞DNA损伤信号的方法及其在淋巴瘤预后中的应用,本发明目的是为了解决现有技术中缺乏仅在体外通过对红细胞的检测分析实现对淋巴瘤预后评估的技术问题,具有检测红细胞DNA损伤信号特异性强以及灵敏度高的技术效果。
本发明提供了检测红细胞DNA损伤信号的方法,具体技术方案如下:
检测红细胞DNA损伤信号的方法,包括以下步骤:
S1:固定血样本:获取新鲜抽取的或低温保存的抗凝全血样本,按照1-2%体积比将所述抗凝血样本在室温环境下固定于1%-4%的多聚甲醛溶液当中,固定1小时后,形成含有血样本的固定液;
S2:清洗处理对照组和实验组:分别取用100-200μL体积的固定液设置为第一对照组和第一实验组,所述第一对照组和所述第一实验组均在室温环境下离心,弃上清液,保留底部细胞沉淀,随后用500μL 1×PBS重悬细胞沉淀,室温环境下离心,完成一次清洗,清洗步骤执行2-3次,得到去除多聚甲醛的第二对照组和第二实验组;
S3:染色处理第二实验组:取所述第二实验组加入Human FcR Binding,在4摄氏度下孵育15-20分钟,随后使用染色缓冲剂在室温避光环境下染色所述第二实验组,得到第三实验组,所述染色时间为15-30分钟,所述染色缓冲剂包括RNase的染色试剂和核酸染色剂,所述含RNase的染色试剂包括第一标记物标记的CD235a、第二标记物标记的CD47和第三标记物标记的CD71,所述核酸染色剂为Hoechst33342和Thiazole orange溶于1×PBS;
S4:重悬处理第二对照组:使用与所述第二实验组等体积的1×PBS重悬所述第二对照组,得到第三对照组;
S5:流式方法检测所述第三实验组以及所述第三对照组:将所述第三实验组和所述第三对照组放置于流式细胞仪中检测,得到检测结果;
S6:分析所述检测结果,即获得细胞DNA损伤信号。
在某些实施方式中,步骤S1中,所述抗凝全血样本来源于骨髓或者外周血,所述抗凝全血样本的体积为1-3μL,所述低温环境的温度为4℃。
在某些实施方式中,步骤S3中,所述离心的时间为4-6分钟。
在某些实施方式中,步骤S3中,对应1μL的抗凝全血样本,所述第一标记物标记的CD235a为0.1-0.15μg,所述第二标记物标记的CD47为0.03-0.1μg,所述第三标记物标记的CD71为0.1-0.15μg,所述Hoechst33342为0.014-0.08nmol,所述Thiazole orange为0.04-0.2μg;所述第一标记物为APC通道,所述第二标记物为PE通道,第三标记物为PerCP cy5.5通道。
在某些实施方式中,骤S5中,所述流式细胞仪的所述流式细胞仪的记录时间小于等于120s,记录个数小于等于100000个,样本流速30μL/min在某些实施方式中,所述步骤S6还包括如下步骤:
S61:根据所述第三对照组确定CD235a的阴性信号范围,从而在第三实验组中选择含有所述CD235a的阳性细胞群体;
S62:在步骤S61的基础上选择CD47表达量处于TOP 10%细胞群体;
S63:在步骤S62的基础上选择CD71表达量处于TOP 5%细胞群体;
S64:在步骤S63的基础上选择Hoechst33342和Thiazole orange双阳性的细胞群体,记录CD71表达量为TOP 5%的细胞群体中Thiazole orange和Hoechst33342呈现双阳的细胞群体数目及百分比,即为红细胞损伤信号。
本发明还提供了检测红细胞DNA损伤信号在淋巴瘤预后中的应用,基于上述检测红细胞DNA损伤信号的方法,对比患者自身DNA损伤信号比例的变化,同时建立健康人群和淋巴瘤疾病人群的红细胞DNA损伤信号数据库,将淋巴瘤疾病患者红细胞DNA损伤信号与数据库进行对比,随后评估淋巴瘤疾病患者预后。
本发明具有以下有益效果:本发明提供了检测红细胞DNA损伤信号的方法,通过筛选特定红细胞并进行特定红细胞损伤信号检测并放大的方式,预测淋巴瘤的转移和发展,具有步骤方法简单,检测周期短的优势。此外,检测血样本来源于的外周血或骨髓,检测过程对被检测人员的伤害小,且可实现微量检测,以在提高检测效率的同时减少病患的痛苦。本发明还将该方法用于淋巴瘤预后中,该方法特别适用于B细胞淋巴瘤的转移和发展预测,在临床上有着广泛的应用。

具体实施方式
为使本发明的目的、技术方案和优点更加清楚明白,以下结合具体实施例,并参照附图1-10,对本发明进一步详细说明。
在介绍本发明的具体实施内容之前,先对该实施内容中涉及到的名词进行简要的说明介绍:
红细胞DNA损伤信号:成熟的红细胞是没有细胞核的,如果在红细胞发育成熟的前期,染色单体或染色体的无着丝点断片,或因纺锤体受损而丢失整个染色体,在细胞分裂后期,部分DNA核酸片段仍然遗留在细胞质中,也就是说,这些残留的DNA信号会在成熟的红细胞胞质内单独形成一个或几个规则的次核,这些次核即为残留的DNA损伤信号。
CD235a:血型糖蛋白A,是一种单跨膜糖蛋白,表达于成熟红细胞和红系前体细胞,为红细胞表面特殊的标记蛋白。
CD47:也叫做整合素相关蛋白(integrin associated protein,IAP),是免疫球蛋白超家族成员,CD47广泛地表达于细胞的表面,可与信号调节蛋白α(Signal regulatoryproteinα,SIRPα)、血小板反应蛋白(thrombospondin-1,TSP1)以及整合素(integrins)相互作用,介导凋亡、增殖、免疫等一系列的反应,研究表明,CD47与红细胞的生存周期存在明显相关性,无论是在体内老化还是体外老化过程中,红细胞膜上CD47的表达量均出现显著下降趋势。
CD71:CD71即转铁蛋白受体(Transferrin receptor),是介导转铁蛋白-铁复合物摄取的膜蛋白,在红系细胞表面高度表达。研究表明,CD71表达阳性的细胞一般较幼稚的早期网织红细胞或较早期幼红细胞。
PBS:磷酸缓冲盐溶液(phosphate buffer saline)的缩写,一般作为溶剂,起溶解保护试剂的作用。
APC:别藻蓝蛋白(Allophycocyanin),是一种蓝绿色水藻中分离出来的藻胆(色素)蛋白。与其它藻胆蛋白一样,APC具有荧光性,伴随着非常高的光吸收和高量子效率。
PE:聚乙烯(polyethylene,简称PE)是乙烯经聚合制得的一种热塑性树脂。
Thiazole orange:噻唑橙是一种化学物质,化学式是C26H24N2O3S2。
Human FcR Binding:或者称作Human Fc Block,可以有效阻止细胞表面Fc受体的非特异结合,降低背景干扰获得更准确的结果。
本发明提供的利用红细胞DNA损伤信号预测淋巴瘤转移和发展的方法,基于CD235a、CD47和CD71标记蛋白组合,通过流式方法筛选出特定红细胞,使用核酸染料分析所述特定红细胞内的核酸信号,即为红细胞的DNA损伤信号,通过快速有效的对比患者自身DNA损伤信号比例的变化,建立高效、快速、高敏感的RBCs损伤检测体系,进而预测淋巴瘤患者的转移和发展情况。
具体而言,所述利用红细胞DNA损伤信号预测血液转移和发展的方法,以下简称为转移和发展预测方法,包括以下步骤:
S1:固定血样本:获取所述流式细胞仪的记录时间小于等于120s,记录个数小于等于100000个,样本流速30μL/min抗凝全血样本,按照1-2%体积比将所述抗凝血样本在室温环境下固定于1%-4%的多聚甲醛溶液当中,固定1小时后,形成含有血样本的固定液;
S2:清洗处理对照组和实验组:分别取用100-200μL体积的固定液设置为第一对照组和第一实验组,所述第一对照组和所述第一实验组均在室温环境下离心,弃上清液,保留底部细胞沉淀。随后用500μL 1×PBS重悬细胞沉淀,室温环境下离心,完成一次清洗。清洗步骤执行2-3次,得到去除多聚甲醛的第二对照组和第二实验组;
S3:染色处理第二实验组:取所述第二实验组加入Human FcR Binding,在4摄氏度下孵育15-20分钟,随后使用染色缓冲剂在室温避光环境下染色所述第二实验组,得到第三实验组。其中所述染色缓冲剂为含RNase的染色试剂以及核酸染色剂的组合,染色15-30分钟。其中所述含RNase的染色试剂为含第一标记物标记的CD235a,第二标记物标记的CD47以及第三标记物标记的CD71;所述核酸染色剂为含Hoechst33342和Thiazole orange的1×PBS;
S4:重悬处理第二对照组:使用与所述第二实验组等体积的1×PBS重悬所述第二对照组,得到第三对照组;
S5:流式方法检测所述第三实验组以及所述第三对照组:将所述第三实验组和所述第三对照组放置于流式细胞仪中检测,得到检测结果;
S6:分析所述分析结果:
S61:根据对照组确定CD235a的阴性信号范围,从而在检测组选择含有所述CD235a的阳性细胞群体;
S62:在步骤S61的基础上选择CD47表达量处于TOP 10%细胞群体;以及
S63:在步骤S62的基础上选择CD71表达量处于TOP 5%细胞群体;以及
S64:在步骤S63的基础上选择Hoechst33342和Thiazole orange双阳性的细胞群体,记录CD71表达量为TOP 5%的细胞群体中Thiazole orange和Hoechst33342呈现双阳的细胞群体百分比,得到损伤信号。
值得一提的是,在本发明的实施例中,选择CD235是因为其是红细胞表面特有的标记物,从而筛选出血样本中的红细胞;然后在红细胞里选择处于CD47表达量为TOP 10%的细胞群体,即选择了血液中较为新鲜的红细胞;接着在CD47表达量为TOP 10%的细胞群体中选择CD71表达量为TOP 5%的红细胞进一步选择出最新形成的红细胞,一方面能将目标细胞群体定位于较为新鲜的血液,另一方面具有信号放大的作用。
具体而言,在所述步骤S1当中,其中所述抗凝血样本为骨髓或者外周血,并且所述低温环境优选为4℃,此温度保存效果最佳,另外,本发明只需要获取微量的血样本即可,极大程度地提高测试效率以及减轻病患的痛苦,一般而言,只需要1-3μL的微量血样本。
在所述步骤S2当中,准备所述对照组以及所述实验组的可变因素应控制为一致,也就是说,所述对照组以及所述实验组应该在同样的环境下进行实验准备,以此方式确保不必要的干扰因素的干扰。
在所述步骤S3当中,按照2μL-15μL/test的标准加入Human FcR Binding,所述染色缓冲剂中所述第一标记物不同于所述第二标记物,以在检测时区别所述CD235a以及所述CD47,值得一提的是,对应不同标记物在检测时以不同的波长范围为准,在本发明的实施例中,选用APC标记的CD235a、PE标记的CD47以及PerCP cy5.5标记的CD71,其标记物的类型不受影响。
在所述步骤S5当中,选用流式细胞仪的型号为Beckman Coulter的CytoFLEX LX流式细胞仪,当然可以不只是用这特定类型的流式分析仪。在本发明的实施例中,所述流式分析仪的相关参数如下:
采集参数:记录个数100000个、记录时间120s、样本流速30μL/min;
通道选择:CD235a-APC为Red 660/10、CD47-PE为Yellow 585/42、CD71-PerCPcy5.5为B690/50、Hoechst33342为UV 450/45、Thiazole orange为Blue 525/40。
在所述步骤S5当中,所述流式分析仪分析得到的结果如图6所示,在分析图中可以清楚地看到被所述第一标记物标记的CD235a,并得到CD235a阳性细胞群体内CD47表达量处于TOP10%细胞群体,在该CD47表达量处于TOP10%细胞群体又得到CD71表达量处于TOP5%细胞群体,在该CD71表达量处于TOP 5%细胞群体又得到Hoechst33342和Thiazoleorange双阳性的细胞群体,最终计算得到DNA的损伤信号。
在所述步骤S6之后,在获取到损伤信号后,医用人员可快速有效的对比患者自身DNA损伤信号比例的变化,进而预判患者血液疾病的转移和发展情况。
另外,以下将以详细的实验数据作为该红细胞DNA损伤信号的检测方法的实验支持,但熟悉该技术的人应该明白,所述实验数据只是作为一个具体实施例存在,所述实验数据可以比例的方式进行类推。
在该实验例中,选用10μL的患者抗凝血样本,按照1%的体积比固定于1%-4%的多聚甲醛溶液中,即所述多聚甲醛溶液的体积为1000μL,室温固定1小时后可于4℃长期保存。随后从中取出含1μL血样本的固定液,对应1μL血样本采用200μL的1×PBS重悬对照组样本,200μL的染色缓冲剂重悬所述实验组样本,并且所述实验组样本中包括0.1-0.15μg APC标记的CD235a、0.03-0.1μg PE标记的CD47、0.1-0.15μg PerCP cy5.5标记的CD71、0.014-0.08nmolHoechst33342和0.04-0.2μg Thiazole orange的1××PBS。
也就是说,抗凝血样本以体积比1:100的比例添加至多聚甲醛溶液中,得到含有血样本的固定液。取出含有一定体积的血样本的固定液,进行后续染色标记操作,其中所述血样本和所述染色缓冲剂的体积比控制为1:200,对应1μL血样本的染色缓冲剂中含有0.1-0.15μgAPC标记的CD235a、0.03-0.1μg PE标记的CD47、0.1-0.15μg PerCP cy5.5标记的CD71、0.014-0.08nmol Hoechst33342和0.04-0.2μg Thiazole orange的1×PBS。
特别值得一提的是,本发明提供的所述转移和发展预测方法在每次测量时只要使用1μL的微量血样本,并且抗体孵育的时间为20-30分钟,单个样本检测时间在1分钟左右。然而血常规需要血样本5ml,并且其检测速度大概在1个小时,相较于传统的血常规检测方法,所述转移和发展预测方法微量检测,检测效果高,特异性佳,灵敏度高,在临床中有着非常重要的实际意义。
另外,本发明提供的所述利用红细胞DNA损伤信号预测血液转移和发展的方法特别适用于预测大B细胞淋巴瘤的转移和发展,已有具体的临床数据作为支持,说明如表1所示:

以上表格为5例NHL患者检测得到的数据图。如图所示,患者1分别在2019年2月19日和2019年3月26日进行住院治疗,该患者在2月份为DLBCL初发,对其进行抗感染治疗后DNA损伤信号很低,为0.2%。而在3月份时,虽然进行了R-CHOP化疗+美罗华化疗治疗,癌灶仍转移至他处,DNA损伤信号也升高至8.51%。通过这个数据可以看出该技术手段可以明显地指示出患者疾病预后不良好。
患者2和3在确诊NFL后,分别多次进行了相应的化疗方案,但其DNA损伤信号较高,患者2为12.65%,患者3更是达到了35.99%。提示患者可能对现进行的的化疗方案有耐受,需要进一步的方案调整。
患者4在多发骨髓瘤的同时合并肺部感染和急性重症胰腺炎,患者状态非常差,已放弃治疗。该技术手段检测获得的DNA损伤信号也较高为22.64%,与患者状态相呼应。
患者5在确诊DLBCL后进行了1次化疗,但化疗后患者出现发热,白细胞低,咳嗽咳痰等症状,需要进一步入住治疗,调整下一次化疗方案。该技术手段检测获得的DNA损伤信号较高为34%,提示了本次化疗对患者的效果并不佳。
通过数据分析,医用人员可以根据患者的红细胞DNA损伤信号预判淋巴瘤的转移和发展情况,特别是NHL的转移和发展。
上述仅本发明较佳可行实施例,并非是对本发明的限制,本发明也并不限于上述举例,本技术领域的技术人员,在本发明的实质范围内,所作出的变化、改型、添加或替换,也应属于本发明的保护范围。
Claims (7)
1.检测红细胞DNA损伤信号的方法,其特征在于,包括以下步骤:
S1:固定血样本:获取新鲜抽取的或低温保存的抗凝全血样本,按照1-2%体积比将所述抗凝血样本在室温环境下固定于1%-4%的多聚甲醛溶液当中,固定1小时后,形成含有血样本的固定液;
S2:清洗处理对照组和实验组:分别取用100-200μL体积的固定液设置为第一对照组和第一实验组,所述第一对照组和所述第一实验组均在室温环境下离心,弃上清液,保留底部细胞沉淀,随后用500μL1×PBS重悬细胞沉淀,室温环境下离心,完成一次清洗,清洗步骤执行2-3次,得到去除多聚甲醛的第二对照组和第二实验组;
S3:染色处理第二实验组:取所述第二实验组加入Human FcR Binding,在4摄氏度下孵育15-20分钟,随后使用染色缓冲剂在室温避光环境下染色所述第二实验组,得到第三实验组,所述染色时间为15-30分钟,所述染色缓冲剂包括RNase的染色试剂和核酸染色剂,所述含RNase的染色试剂包括第一标记物标记的CD235a、第二标记物标记的CD47和第三标记物标记的CD71,所述核酸染色剂为Hoechst33342和Thiazole orange溶于1×PBS;
S4:重悬处理第二对照组:使用与所述第二实验组等体积的1×PBS重悬所述第二对照组,得到第三对照组;
S5:流式方法检测所述第三实验组以及所述第三对照组:将所述第三实验组和所述第三对照组放置于流式细胞仪中检测,得到检测结果;
S6:分析所述检测结果,即获得细胞DNA损伤信号。
2.根据权利要求1或2任意一项所述的检测红细胞DNA损伤信号的方法,其特征在于,步骤S1中,所述抗凝全血样本来源于骨髓或者外周血,所述抗凝全血样本的体积为1-3μL,所述低温环境的温度为4℃。
3.根据权利要求1或2任意一项所述的检测红细胞DNA损伤信号的方法,其特征在于,步骤S3中,所述离心的时间为4-6分钟。
4.根据权利要求1或2任意一项所述的检测红细胞DNA损伤信号的方法,其特征在于,步骤S3中,对应1μL的抗凝全血样本,所述第一标记物标记的CD235a为0.1-0.15μg,所述第二标记物标记的CD47为0.03-0.1μg,所述第三标记物标记的CD71为0.1-0.15μg,所述Hoechst33342为0.014-0.08nmol,所述Thiazole orange为0.04-0.2μg;所述第一标记物为APC通道,所述第二标记物为PE通道,第三标记物为PerCP cy5.5通道。
5.根据权利要求1或2任意一项所述的的方法,其特征在于,步骤S5中,所述流式细胞仪的记录时间小于等于120s,记录个数小于等于100000个,样本流速30μL/min。
6.根据权利要求1所述的检测红细胞DNA损伤信号的方法,其特征在于,所述步骤S6还包括如下步骤:
S61:根据所述第三对照组确定CD235a的阴性信号范围,从而在第三实验组中选择含有所述CD235a的阳性细胞群体;
S62:在步骤S61的基础上选择CD47表达量处于TOP10%细胞群体;
S63:在步骤S62的基础上选择CD71表达量处于TOP5%细胞群体;
S64:在步骤S63的基础上选择Hoechst33342和Thiazole orange双阳性的细胞群体,记录CD71表达量为TOP5%的细胞群体中Thiazole orange和Hoechst33342呈现双阳的细胞群体数目及百分比,即为红细胞损伤信号。
7.检测红细胞DNA损伤信号在淋巴瘤预后中的应用,基于权利要求1-6任一项所述的检测红细胞DNA损伤信号的方法,其特征在于,对比患者自身DNA损伤信号比例的变化,同时通过建立健康人群和淋巴瘤疾病人群的红细胞DNA损伤信号数据库,将淋巴瘤疾病患者红细胞DNA损伤信号与数据库进行对比,随后评估淋巴瘤疾病患者预后。
