目前,业内常用的现有技术是这样的:
输卵管阻塞性不孕约占女性不孕的1/3,输卵管积水在输卵管病变中又最为 常见。对于轻度输卵管积水性不孕首选宫腹腔镜微创手术,但术后再粘连问题 会严重影响女性的生育功能。输卵管积水性不孕宫腹腔镜术后如何防治再粘连、 积水的复发、改善输卵管功能以及提高自然妊娠率等问题都十分棘手。
本病相当于中医学“不孕”、“带下病”等范畴。大多数医家认为瘀血为 输卵管积水的主要病理基础,但笔者认为瘀血阻络,瘀积日久,津液布散失司 而致痰湿内生,瘀阻水停,湿瘀互结,胞络阻塞,无以摄精成孕,故本病多为 湿瘀胶结所致。正如《灵枢·百病始生篇》云:“凝血蕴裹而不散,津液涩渗, 着而不去,而积皆成矣。”研制通管促孕合剂,重在活血化瘀,祛湿通络。
综上所述,现有技术存在的问题是:
(1)传统汤剂需专业中医师开具药方,煎药耗时较长,不利于推广,病人 依从性差。
(2)目前市面上尚无相关针对输卵管积水的中成药制剂,部分患者在手术 之后不能及时得到中药辅助治疗而存在再次复发的风险。
解决上述技术问题的难度和意义:
解决上述问题,可以从以下几点分析难点:1、如何采用简便的剂型,易于 配制,易于携带服用,提高患者依从性;2、如何巧妙组方,适合大部分输卵管 积水患者,易于推广。
如该问题得到解决,普遍生殖机构在术后均可采用该药治疗,将有效降低 输卵管积水性不孕患者术后的复发率,维持输卵管通畅时间,给患者带来充分 的备孕时间,将提高该病患者的总体妊娠率。
发明内容
针对现有技术存在的问题,本发明提供了一种通管促孕合剂及其制备方法。
本发明是这样实现的,一种通管促孕合剂,所述通管促孕合剂由赤芍80g、 白芍80g、牡丹皮50g、桃仁50g、香附50g、路路通50g、海螵蛸80g、王不 留行50g、冬瓜皮50g、牛膝50g、灸甘草30g、桂枝30g、水蛭10g、茯苓50g、 泽泻50g、水牛角80g、莱菔子50g、茜草80g和甜菊苷4g组成。方中赤芍能够 清热凉血,散寒瘀止痛;白芍养血调经,敛阴止汗,柔肝止痛,平抑肝阳;牡 丹皮清热凉血,活血化瘀;桃仁活血祛瘀,润肠通便,止咳平喘;香附疏肝解郁,理气宽中,调经止痛;路路通能够祛风活络,利水,通经;海螵蛸收敛止 血,涩精止带,制酸止痛,收湿敛疮;王不留行活血通经,下乳消肿,利尿通 淋;冬瓜皮利尿消肿;牛膝逐瘀通经,补肝肾,强筋骨,利尿通淋,引血下行; 炙甘草补脾和胃,益气复脉;桂枝发汗解肌,温通经脉,助阳化气,平冲降气; 水蛭破血通经,逐瘀消癥;茯苓利水渗湿,健脾,宁心;泽泻利水渗湿,泄热, 化浊降脂;水牛角清热凉血,解毒,定惊;莱菔子能够消食除胀,降气化痰; 茜草凉血,祛瘀,止血,通经。各药配伍调经止痛、活血通络、理气散瘀,共 奏理气和血,利水通络之效。
本发明的另一目的在于提供一种所述通管促孕合剂的制备方法,所述通管 促孕合剂的制备方法包括以下步骤:
步骤一,将赤芍80g、白芍80g、牡丹皮50g、桃仁50g、香附50g、路路 通50g、海螵蛸80g、王不留行50g、冬瓜皮50g、牛膝50g、灸甘草30g、桂枝 30g、水蛭10g、茯苓50g、泽泻50g、水牛角80g、莱菔子50g、茜草80g,加 水浸渍30分钟,煎煮两次;
步骤二,合并煎液,滤过,滤液静置,取上清液浓缩,加入甜菊苷4g和对 羟基甲苯甲酸乙酯0.2g,搅匀,加热溶解,冷却,滤过,继续加水至1000ml, 充分搅拌均匀,灌装,即得。
进一步,所述步骤一煎煮两次,第一次1.5小时,第二次1小时。

(1)取本品50ml,置分液漏斗中,加乙醚振摇提取3次,每次30ml,弃 去乙醚液,再加正丁醇提取3次,每次30ml,合并正丁醇液,蒸干,残渣加甲 醇1ml使溶解,作为供试品溶液;另取甘草次酸对照品,加甲醇制成每1ml含 1mg的对照品溶液;照薄层色谱法试验,吸取供试品溶液5μl,对照品溶液1μl, 分别点于同一硅胶GF254薄层板上,以石油醚30-60℃-甲苯-乙酸乙酯-冰醋酸 10:20:7:0.5为展开剂,展开,取出,晾干,置紫外光灯254nm下检视;供 试品色谱中,在与对照品色谱相应的位置上,显相同颜色的斑点;

为了使本发明的目的、技术方案及优点更加清楚明白,以下结合实施例, 对本发明进行进一步详细说明。应当理解,此处所描述的具体实施例仅仅用以 解释本发明,并不用于限定本发明。
下面结合附图对本发明的应用原理作详细的描述。
本发明实施例提供的通管促孕合剂由赤芍80g、白芍80g、牡丹皮50g、桃 仁50g、香附50g、路路通50g、海螵蛸80g、王不留行50g、冬瓜皮50g、牛 膝50g、灸甘草30g、桂枝30g、水蛭10g、茯苓50g、泽泻50g、水牛角80g、 莱菔子50g、茜草80g和甜菊苷4g组成。
如图1所示,本发明实施例提供的通管促孕合剂的制备方法包括以下步骤:
S101:将赤芍80g、白芍80g、牡丹皮50g、桃仁50g、香附50g、路路通 50g、海螵蛸80g、王不留行50g、冬瓜皮50g、牛膝50g、灸甘草30g、桂枝30g、 水蛭10g、茯苓50g、泽泻50g、水牛角80g、莱菔子50g、茜草80g和甜菊苷 4g,加水浸渍30分钟,煎煮两次,第一次1.5小时,第二次1小时;
S102:合并煎液,滤过,滤液静置24小时,取上清液浓缩至约900ml,加入 甜菊苷和对羟基甲苯甲酸乙酯,搅匀,加热溶解,冷却,滤过,继续加水至1000 ml,充分搅拌均匀,灌装,即得。
本发明实施例提供的通管促孕合剂为深棕色至棕黑色的澄清液体(贮存略 有沉淀);味微苦。久置有少量轻摇易散的沉淀。
下面结合鉴定方法对本发明的应用效果作详细的描述。
(1)取本品50ml,置分液漏斗中,加乙醚振摇提取3次,每次30ml,弃 去乙醚液,再加正丁醇提取3次,每次30ml,合并正丁醇液,蒸干,残渣加甲 醇1ml使溶解,作为供试品溶液。另取甘草次酸对照品,加甲醇制成每1ml 含1mg的对照品溶液。照薄层色谱法(中华人民共和国药典2010年版一部附录 ⅥB)试验,吸取供试品溶液5μl,对照品溶液1μl,分别点于同一硅胶GF254 薄层板上,以石油醚(30-60℃)-甲苯-乙酸乙酯-冰醋酸(10:20:7:0.5)为展开剂,展开,取出,晾干,置紫外光灯(254nm)下检视。供试品色谱中,在 与对照品色谱相应的位置上,显相同颜色的斑点。
(2)取本品100ml,加乙醚振摇提取3次,每次30ml,合并乙醚液,低温 挥干,残渣加甲醇1使溶解,作为供试品溶液。另取丹皮酚对照品,加甲醇制 成每1ml含1mg的对照品溶液。照薄层色谱法(中华人民共和国药典2010年 版一部附录ⅥB)试验,吸取供试品溶液10μl,对照品溶液2μl,分别点于同一 以羧甲基纤维素钠为黏合剂的硅胶G薄层板上,以环己烷-乙酸乙酯-冰醋酸(4: 1:0.1)为展开剂,展开,取出,晾干,喷以2%香草醛硫酸乙醇(1→10)溶液, 在105℃加热至斑点显色清晰。供试品色谱中,在与对照品色谱相应的位置上,显相同颜色的斑点。
相对密度应不低于1.02(中华人民共和国药典2010年版一部附录ⅦA)。
pH值应为3.5-5.5(中华人民共和国药典2010年版一部附录ⅦG)
其他应符合合剂项下有关的各项规定(中华人民共和国药典2010年版一 部附录ⅠJ)。
含量测定照高效液相色谱法(中华人民共和国药典2010年版一部附录VI D)测定。
色谱条件与系统适应性试验以十八烷基硅烷键合硅胶为填充剂,以乙腈 -0.1%磷酸溶液(12:88)为流动相,检测波长为230nm。理论塔板数按芍药苷 峰计算应不低于3000。
对照品溶液的制备精密称取芍药苷对照品,加流动相制成每1ml约含芍药 苷70μg的对照品溶液,即得。
供试品溶液的制备精密量取本品10ml,置50ml量瓶,加流动相溶解并定 容,摇匀,即得。
测定法分别精密吸取供试品溶液和对照品溶液各10μl,注入液相色谱仪, 测定,即得。
本品每1ml含芍药苷(C23H28O11)的不得少于0.20mg。
本发明实施例提供的通管促孕合剂主治理气和血,利水通络。用于输卵管 积水,盆腔炎,不孕症。
用法与用量摇匀后口服。一次20ml,一日3次;或遵守医嘱。每瓶装250m l。密闭,置阴凉干燥处。
下面结合临床实施例对本发明的应用效果作详细的描述。
输卵管阻塞在中医辩证当属“症瘕”“不孕”的范畴。究其病因,主要为 湿热、寒湿之邪上客胞宫、壅阻胞脉,致使气血瘀结,输卵管粘膜充血、肿胀、 渗出、粘连、闭塞、精卵难以结合所致。
实验随机选取输卵管造影确诊为输卵管阻塞性不孕症的128例患者,随机 分为两组。其中治疗组64例,年龄最小23岁,最大40.5岁,平均29.5岁。婚 龄最短2年,最长15年,平均7.5年;对照组35例,年龄最小23.5岁,最大 40岁,平均29岁,婚龄最短2年,最长14.5年,平均7.4年。两组病人,年 龄,病程均无统计学差异(P>0.05),有可比性。
对照组病人,于月经干净后4~7天,以生理盐水20ml、庆大霉素8万u、 地塞米松5mg、糜蛋白酶5mg行输卵管通液术,并同时给予环丙沙星0.4g,每 日两次,阿莫西林1.0g,每日三次,连服3~5天。治疗组病人给予通管促孕合 剂进行治疗,口服一次20ml,3次/日。两组病人均于治疗1个疗程后做输卵管 造影复查,并追踪随访15个月。
疗效标准拟定为:治愈:治疗1个疗程后输卵管造影复查示双侧输卵管通 畅,停药15个月内受孕者;有效:治疗1个疗程后输卵管造影复查示,输卵管 一侧通畅或双侧通而欠畅者;无效:未达到上述标准者。
经治疗后,治疗组总有效率为92.0%,对照组总有效率为84.0%,两组总 有效率比较,差异有统计学意义(P<0.05),故治疗组较对照组卵泡发育改良, 治疗组有效率高于对照组。见表2。

Claims (5)

1.一种通管促孕合剂,其特征在于,所述通管促孕合剂由赤芍80g、白芍80g、牡丹皮50g、桃仁50g、香附50g、路路通50g、海螵蛸80g、王不留行50g、冬瓜皮50g、牛膝50g、灸甘草30g、桂枝30g、水蛭10g、茯苓50g、泽泻50g、水牛角80g、莱菔子50g、茜草80g和甜菊苷4g组成。
2.一种如权利要求1所述通管促孕合剂的制备方法,其特征在于,所述通管促孕合剂的制备方法包括以下步骤:
步骤一,将赤芍80g、白芍80g、牡丹皮50g、桃仁50g、香附50g、路路通50g、海螵蛸80g、王不留行50g、冬瓜皮50g、牛膝50g、灸甘草30g、桂枝30g、水蛭10g、茯苓50g、泽泻50g、水牛角80g、莱菔子50g、茜草80g,加水浸渍30分钟,煎煮两次;
步骤二,合并煎液,滤过,滤液静置,取上清液浓缩,加入甜菊苷和对羟基甲苯甲酸乙酯,搅匀,加热溶解,冷却,滤过,继续加水至1000ml,充分搅拌均匀,灌装,即得。
3.如权利要求2所述的通管促孕合剂的制备方法,其特征在于,所述步骤一煎煮两次,第一次1.5小时,第二次1小时。
4.如权利要求2所述的通管促孕合剂的制备方法,其特征在于,所述步骤二合并煎液,滤过,滤液静置24小时,取上清液浓缩至900ml。
5.一种如权利要求1所述通管促孕合剂的鉴定方法,其特征在于,所述通管促孕合剂的鉴定方法包括:
(1)取本品50ml,置分液漏斗中,加乙醚振摇提取3次,每次30ml,弃去乙醚液,再加正丁醇提取3次,每次30ml,合并正丁醇液,蒸干,残渣加甲醇1ml使溶解,作为供试品溶液;另取甘草次酸对照品,加甲醇制成每1ml含1mg的对照品溶液;照薄层色谱法试验,吸取供试品溶液5μl,对照品溶液1μl,分别点于同一硅胶GF254薄层板上,以石油醚30-60℃-甲苯-乙酸乙酯-冰醋酸10:20:7:0.5为展开剂,展开,取出,晾干,置紫外光灯254nm下检视;供试品色谱中,在与对照品色谱相应的位置上,显相同颜色的斑点;
(2)取本品100ml,加乙醚振摇提取3次,每次30ml,合并乙醚液,低温挥干,残渣加甲醇1使溶解,作为供试品溶液;另取丹皮酚对照品,加甲醇制成每1ml含1mg的对照品溶液;照薄层色谱法试验,吸取供试品溶液10μl,对照品溶液2μl,分别点于同一以羧甲基纤维素钠为黏合剂的硅胶G薄层板上,以环己烷-乙酸乙酯-冰醋酸4:1:0.1为展开剂,展开,取出,晾干,喷以2%香草醛硫酸乙醇1→10溶液,在105℃加热至斑点显色清晰;供试品色谱中,在与对照品色谱相应的位置上,显相同颜色的斑点。
