过继性免疫细胞治疗是生物治疗技术的一种,对自体免疫细胞进行体外扩增或基因修饰,然后将其回输给肿瘤患者来治疗的方法,被认为是继手术、放、化疗之后的第四种治疗方式,在临床治疗中广泛应用。
嵌合抗原受体又称CAR,是模拟TCR功能的人工受体,将单链抗体scfv取代TCR的,并引入4-1BB或CD28等胞内信号域。单链抗体特异靶向肿瘤细胞表面抗原,然后通过铰链区和跨膜区将信号传递至细胞内,胞内信号域再将信号转化为活化信号,激活效应细胞;效应细胞通过分泌穿孔素或产生细胞因子杀伤肿瘤细胞,同时效应细胞本身也扩增,进一步扩大免疫杀伤作用。
随着2017年8月以来美国FDA先后批准靶向CD19的两个CAR-T产品上市(诺华制药的kyriamh和凯特制药的yescarta),CAR-T细胞在血液病肿瘤治疗的需求不断升高。但是目前过继性细胞治疗还是基于自体细胞的回输治疗方式,每个患者都有单独制备CAR-T细胞,操作复杂,生产制备成本非常昂贵,诺华制药kyriamh产品的上市价格为每例47.5万美元,凯特制药的yescarta价格也要37.5万美元。另外,大部分肿瘤患者在进行CAR-T细胞治疗前均经过反复多次的其他方式治疗,尤其是放化疗,这就导致从其外周血获得的T淋巴细胞非常少,且这些T淋巴细胞在体外非常难扩增。因此,CAR-T细胞临床治疗的应用推广受到很大的限制。
健康供体外周血T细胞数量多、活率高,用以制备CAR-T和细胞扩增相对比较容易;如果能用于替代肿瘤患者自体CAR-T细胞来治疗,即同种异体CAR-T细胞(通用型CAR-T细胞),将极大降低CAR-T细胞制备的周期和治疗的费用。同种异体细胞是指属于相同物种但在遗传上有所差异个体的细胞。同种异体CAR-T细胞用于肿瘤治疗的缺点在于,免疫排异问题。在免疫活力的宿主中,同种异体细胞迅速收到排异,这是一种称为宿主抗原移植物排异(HvG)的过程,而这限制了转移细胞的效力。在免疫无活力的宿主中,能够植入同种异体细胞,但它们的内源性T细胞受体特异性可能将宿主组织识别为外来物,导致移植物抗宿主病(GvHD),这可能会导致严重的组织损伤和死亡。
有鉴于此,特提出本发明。
发明内容
本发明的目的是为了一种通用型CAR-T细胞的制备方法,敲除效率高,制备成本低。
为了实现本发明的上述目的,特采用以下技术方案:
用于敲除CAR-T细胞的TCR基因的sgRNA,以TCR的ɑ链的恒定区设计得到。
优选地,sgRNA的序列为SEQ ID NO.1-8中的任一种。
用于敲除CAR-T细胞的B2M基因的sgRNA,以B2M基因的功能域设计得到。
优选地,sgRNA的序列为SEQ ID NO.9-12中的任一种。
用于敲除CAR-T细胞的PD1基因的sgRNA,以PD1基因的功能域设计得到。
优选地,sgRNA的序列为SEQ ID NO.13-17中的任一种。
一种通用型CAR-T的制备方法,敲除CAR-T细胞的TCR基因、B2M基因和PD1基因中的任一种或多种,采用上述的sgRNA的序列进行基因敲除。
T细胞(抗原)受体(T cell receptor,TCR)为所有T细胞表面的特征性标志,以非共价键与CD3结合,形成TCR/CD3复合物。TCR的作用是识别抗原。TCR是由两条不同肽链构成的异二聚体,由α、β两条肽链组成,每条肽链又可分为可变区(V区),恒定区(C区),跨膜区和胞质区等几部分;其特点是胞质区很短。TCR分子属于免疫球蛋白超家族,其抗原特异性存在于V区;V区(Vα、Vβ)又各有三个高变区CDR1、CDR2、CDR3,其中以CDR3变异最大,直接决定了TCR的抗原结合特异性。在TCR识别MHC-抗原肽复合体时,CDR1,CDR2识别和结合MHC分子抗原结合槽的侧壁,而CDR3直接与抗原肽相结合。
TCR分为两类:TCR1和TCR2;TCR1由γ和δ两条链组成,TCR2由α和β两条链组成。外周血中,90%-95%的T细胞表达TCR2;而且任一T细胞只表达TCR2和TCR1之一。
B2M(β2微球蛋白)则是HLA1类分子的重要组成成分,该基因编码的轻链与a链一起形成HLA1类分子,对识别和提呈内源性抗原肽,与辅助受体CD8结合,对CTL(细胞毒性T细胞)的识别起限制性作用。
PD-1(programmed death 1)程序性死亡受体-1:是表达在T细胞表面的一种重要的免疫抑制跨膜蛋白。PD-1主要限制慢性炎症、感染或癌症中的T细胞活性,PD-1有两个配体,PD-L1和PD-L2。在肿瘤的微环境中,肿瘤细胞能够表达PD-L1或者PD-L2。癌细胞逃避免疫系统摧毁的一种方法,是通过配体联接到T细胞的PD-1蛋白上。当配体与PD-1联接以后,T细胞就不能够发现肿瘤和向免疫系统发出攻击肿瘤的信号。
CRISPR/CAS9是现有常用的一种基因敲除手段,其工作原理是crRNA(CRISPR-derived RNA)通过碱基配对与tracrRNA(trans-activating RNA)结合形成tracrRNA/crRNA复合物,此复合物引导核酸酶Cas9蛋白在与crRNA配对的序列靶位点剪切双链DNA。而通过人工设计这两种RNA,可以改造形成具有引导作用的sgRNA(short guide RNA),足以引导Cas9对DNA的定点切割。
作为一种RNA导向的dsDNA结合蛋白,Cas9效应物核酸酶是已知的第一个统一因子(unifying factor),能够共定位RNA、DNA和蛋白,从而拥有巨大的改造潜力。将蛋白与无核酸酶的Cas9(Cas9 nuclease-null)融合,并表达适当的sgRNA,可靶定任何dsDNA序列,而sgRNA的末端可连接到目标DNA,不影响Cas9的结合。因此,Cas9能在任何dsDNA序列处带来任何融合蛋白及RNA,这为生物体的研究和改造带来巨大潜力。
本发明选用特定的sgRNA将CAR-T细胞的TCR、PD1和B2M基因中的任一种或多种敲除,敲除效率高。
本发明制得的通用型CAR-T细胞不会引起移植物无抗宿主反应(graft versushost reaction,GVHR)、也不会引起宿主抗移植物反应(host versus graft reaction,HVGR),同时因为T细胞表面PD1因而不能与肿瘤细胞表面的PD-L1或PD-L2结合使得T细胞能最大程度发挥对肿瘤的杀伤作用。
sgRNA可采用不同的方式对特定基因进行敲除,如sgRNA与CAS9蛋白一起转入待敲除的细胞中实现基因敲除,也可以sgRNA合成双链然后插入含有CAS9序列的载体中,该载体转入待敲除的细胞中实现基因敲除。
进一步地,所述sgRNA与CAS9蛋白实现基因敲除。
进一步地,所述sgRNA以含有该sgRNA和编码CAS9蛋白基因的载体的形式进行基因敲除。
进一步地,敲除CAR-T细胞的TCR基因和B2M基因。

本发明提供的sgRNA,经大量实验筛选得到以下序列的sgRNA具有更好的特异性,靶向TCR、B2M及PD1基因的sgRNA特异性强。CAR-T细胞内同时实现TCR、B2M和/或PD1基因敲除,敲除的效率高。
进一步地,所述基因敲除同时进行。
进一步地,相应基因的sgRNA分别连入携带有CAS9的载体中,然后一起加入CAR-T细胞的转染体系中进行转染。
进一步地,所述载体为pX330-spCAS9-HF1载体。
进一步地,所述转染为脂质体转染或电转。
进一步地,所述转染步骤如下:200μL的OPTI-MEM中加入1μg pX330-spCAS9-HF1-TRAC(TCRɑ链基因)的TRAC sgRNA表达质粒,1μg pX330-spCAS9-HF1-B2M的B2M sgRNA或1μgpX330-spCAS9-HF1-PD1的PD1sgRNA表达质粒,再加入4μL PEI或者6μL lipo2000试剂,混合均匀后室温静置15min,滴入铺有1×106个的CAR-T细胞的培养孔(六孔板)中。6-8h后将细胞换回新鲜培养基培养。
换液48h后通过流式细胞分选(FACS)分析TCR/B2M/PD1基因KO细胞比例,结果表明TCR/B2M/PD1敲除效率分别高达91.33%、81.14%、54.57%,说明所设计sgRNA体内活性良好。
进一步地,所述CAR-T细胞为针对不同疾病的CAR-T细胞。
进一步地,所述CAR-T细胞采用以下方法制备:
针对不同疾病制备的CAR结构,包装慢病毒,得到含有CAR结构的慢病毒;
所述慢病毒转入T细胞中,分选得到所述CAR-T细胞。
本发明还提供了上述的通用型CAR-T细胞的制备方法制得的通用型CAR-T细胞。
与现有技术相比,本发明的有益效果为:
(1)本发明公开了针对CAR-T细胞的TCR基因、B2M基因和PD1基因设计的sgRNA,这些sgRNA特异性好,敲除效率高。
(2)本发明利用基因编辑技术制备通用型CAR-T细胞的方法,即利用CRISPR/CAS9技术同时敲除CAR-T细胞的TCR/B2M/PD-1基因,制备成本低。

具体实施方式
下面将结合实施例对本发明的实施方案进行详细描述,但是本领域技术人员将会理解,下列实施例仅用于说明本发明,而不应视为限制本发明的范围。实施例中未注明具体条件者,按照常规条件或制造商建议的条件进行。所用试剂或仪器未注明生产厂商者,均为可以通过市售购买获得的常规产品。
CAR意为嵌合抗原受体(Chimeric Antigen Receptor),当CAR结构与肿瘤相关抗原抗体融合表达在T细胞的细胞膜表面时,能够赋予T细胞特异性识别对应肿瘤细胞的能力,此时的T细胞即为CAR-T细胞。一个完整的CAR结构通常包括上膜信号区、抗体区(通常来源于对应单克隆抗体的scFV段)、胞外铰链区、跨膜区、胞内信号区。通过基因修饰过的T细胞(CAR-T)能够对带有其针对的抗原的细胞产生特异性更强的细胞杀伤作用,从而能够达到对肿瘤等治疗的效果。
实施例1
CAR质粒构建
CAR结构设计包含顺序连接的信号肽、胞外结合区、铰链区、跨膜区和胞内信号区的嵌合抗原受体(CAR),并进行载体构建。胞外结合区的核苷酸序列是在抗CD19的嵌合抗原受体的抗原结合区(在此命名为anti-CD19scFv,其来源于小鼠)的核苷酸序列的基础上进行人源化改造而获得的(将其命名为FMC63A),将包含上述成分的CAR进行基因合成并利用基因工程的方法连接至慢病毒表达载体中,转化至大肠杆菌DH5α中,并利用无内毒素质粒提取试剂盒抽提得到最终质粒。
本实施例中抗体片段为CD19,此外还有CD20,BCMA等。
CAR质粒构建图谱见图1。
实施例2
CAR慢病毒包装
1.开始病毒包装前一天,对293T细胞以5*10^6个/皿进行传代。
2.倒置显微镜观察细胞生长密度在80%-90%,细胞形态良好一致。
3.拿出质粒、Opti-MEM和lipo2000,平衡到室温。
4.准备2个15ml离心管,分别配制质粒准备液和lipofectamine 2000准备液。质粒为第二代三质粒系统,表达质粒与剩余两种包装质粒量为2:1:1,lipofectamine 2000与总的质粒质量比为3:1。
5.将质粒准备液和lipofectamine 2000准备液分别混匀,静止5min。
6.将lipofectamine 2000准备液加入质粒准备液中形成转染工作液,混匀。孵育20min使其形成混合物。
7.将转染工作液逐滴加入准备好的293T细胞中,摇动培养皿,使转染工作液均匀分布在整个培养皿中。放置37℃,5%CO2中培养。次日用移液管吸掉细胞培养上清,加入新鲜完全培养基10ml,继续在37℃,5%CO2中培养。
8.病毒上清收集:转染48小时后收集病毒上清液,再加入新鲜完全培养基10ml;次日第二次收集病毒上清,然后通过离心(3000×g、10min、4℃)的方法。
9.离心上清用0.45μm的过滤膜过滤(常用低蛋白结合的PES膜)。
10.过滤后的细胞上清液如果暂时不进行进一步处理,则分装并于-80℃冰箱冻存。
本实施例采用二代三质粒系统包装慢病毒,此外还可用第三代四质粒系统来包装。
实施例3
从健康志愿者外周血中分离T细胞
1)健康供者外周血的采集:采集的外周血4℃冰箱暂存,24h内,经由配备有恒温设备的转运车运至GMP实验室进行分离和培养。
2)外周血单个核细胞(PBMC)的制备:用移液管吸取DPBS(Dulbecco's PhosphateBuffered Saline)或者生理盐水加入到步骤(1)采集的外周血中(1:1),稀释,将血细胞稀释液,缓慢加入装有淋巴细胞分离液(Ficoll或者Histopaque-1077)的离心管中,以800g离心20min后,吸取淋巴细胞分离液上方的白膜层细胞,转入一个新的离心管中,加入Lonzax-vivo 15培养基离心后弃上清,保留离心管底部的细胞沉淀,即得到外周血单个核细胞。
3)T细胞的分离和激活:将得到的外周血单个核细胞进行计数,按照1:1的比例加入偶联CD3/CD28抗体的beads,轻轻震荡20min,利用磁力架的吸附作用,得到CD3阳性的T细胞,此时的T细胞处于激活状态,加入完全培养基(Lonza x-vivo15+IL-2),对T细胞进行培养扩增。本实施例分选T为CD3/CD28正选,T细胞的分离还可以利用EasySepTM Human T CellEnrichment Kit从PBMC中进行负选得到T细胞。
实施例4
CAR-T细胞制备
利用偶联CD3/CD28抗体的beads将PBMC中的T细胞分选和激活后,用Lonza x-vivo15培养基将细胞密度调至4×106cell/mL,加入终浓度为10ug/ml的助转剂并按照MOI=5:1的比例加入包装有CAR的慢病毒,4h后补加完全培养基至浓度为1×106cell/m,次日换液,48h后观察细胞的状态并流式检测CAR表达。
实施例5
设计特异性靶向TARC(TCRα链的恒定区)、B2M编码基因和PD1基因的sgRNA、构建特异靶向TCRα链、B2M编码基因和PD1基因的CRISPR/CAS9质粒。
利用常用CRISPR设计软件(http://crispr.mit.edu),此外还有http://crispr.tefor.net/,http://zifit.partners.org/ZiFiT/,https://crispr.cos.uni-heidelberg.de/,http://www.rgenome.net/cas-designer/等对从NCBI上下载的人类TARC、B2M和PD1进行sgRNA设计,B2M和PD1针对其蛋白功能域,得到的具体序列如表1-3所示。

将上述序列以及与其互补的一条链分别进行基因合成,同时,在每条正向序列前面加上CACC,同时在反向序列前面加上AAAC,即添加酶切位点,合成后稀释至终浓度为100μM,将合成的序列片断各取1μL,与8μL10×Annealing Buffer混合,离心。所得产物用T4连接酶连接到经BbsI酶切处理后的pX330-spCAS9-HF1载体(Addgene Plasmid#108301见图2)中。连接产物使用感受态DH5a转化后,挑选克隆使用通用引物U6进行测序,将正确插入sgRNA序列的克隆摇菌提取质粒备用。
上述内容以targeting PD-1sgRNA1为例进行说明:
正向序列 5’-CACCgtctgggcggtgctacaact-3’;
反向互补 3’-cagacccgccacgatgttgaCAAA-5’。
通过退火形成双链将上述分别进行基因合成,合成后稀释引物至终浓度为100μM,将合成的序列片断各取1μL,与8μL 10×Annealing Buffer混合,离心于95℃加热3min后缓慢冷却到室温(正反向引物通过退火后形成带有粘性末端的双链)。所得产物用T4连接酶连接到经BbsI酶切处理后的pX330-spCAS9-HF1载体。连接产物使用感受态DH5a转化后,挑选克隆使用通用引物U6进行测序,将正确插入sgRNA序列的克隆摇菌提取质粒备用。
实施例6
TCR/B2M/PD1基因敲除及细胞纯化
在六孔板的孔中铺1×106T细胞,在200μL的OPTI-MEM中加入1μg pX330-spCAS9-HF1-TRAC的TRAC sgRNA表达质粒,1μg pX330-spCAS9-HF1-B2M的B2M sgRNA或1μg pX330-spCAS9-HF1-PD1的PD1 sgRNA表达质粒,再加入4μL PEI或者6μL lipo2000试剂,混合均匀后室温静置15min,滴入孔中。6-8h后将细胞换回新鲜培养基培养。换液48h后通过流式细胞分选(FACS)分析TCR/B2M/PD1基因KO细胞比例,得到敲除效率。具体结果如表4所示。

其中,“1号+9号+13号”的结果见图3-5,TCR/B2M/PD1敲除效率分别高达91.33%、81.14%、54.57%,说明所设计sgRNA体内活性良好。
另外通过实施TCR/B2M同时敲除,结果如表5所示。

其中,“1号+9号”流式检测结果如图6,双敲效率达到61.3%。
同时还通过Surveyor测定确定基因编辑效率(注:DKO指TCR与B2M同时敲除,数字代表所使用sgRNA),“1号+9号+13号”结果如图7-9所示。
此外,编辑工具还可以以RNA及RNP形式通过电转及病毒感染的方式进入细胞。
实施例7
三基因敲除CAR-T细胞的体外活性
体外杀伤实验,CAR-T/T-cell/DKO/TKO共四株效应细胞,靶细胞为K562和K562-CD19,共培养6h,采用7-AAD和Annexin-V检测凋亡效率,TKO细胞对K562-CD19杀伤效果最好,结果见图10和图11。图10中,A、B、C和D图依次测定的是Donor3 IFN-r、Donor3 IFN-r、Donor2 IL-2和Donor3 IL-2。
尽管已用具体实施例来说明和描述了本发明,然而应意识到,在不背离本发明的精神和范围的情况下可以作出许多其它的更改和修改。因此,这意味着在所附权利要求中包括属于本发明范围内的所有这些变化和修改。

Claims (14)
1.用于制备通用型CAR-T细胞的sgRNA,其特征在于,由以下sgRNA组成:SEQ ID NO.1所示的TCR基因的sgRNA、SEQ ID NO.9所示的B2M基因的sgRNA和SEQ ID NO.13所示的PD1基因的sgRNA。
2.一种通用型CAR-T细胞的制备方法,其特征在于,敲除CAR-T细胞的TCR基因、B2M基因和PD1基因,采用权利要求1提供的sgRNA的序列进行基因敲除。
3.根据权利要求2所述的通用型CAR-T细胞的制备方法,其特征在于,所述sgRNA与CAS9蛋白实现基因敲除。
4.根据权利要求2所述的制备方法,其特征在于,所述sgRNA以含有该sgRNA和CAS9蛋白的载体的形式进行基因敲除。
5.根据权利要求2所述的通用型CAR-T细胞的制备方法,其特征在于,所述TCR基因、B2M基因和PD1基因敲除同时进行。
6.根据权利要求5所述的制备方法,其特征在于,相应基因的sgRNA分别连入携带有CAS9的载体中,然后一起加入CAR-T细胞的转染体系中进行转染。
7.根据权利要求6所述的通用型CAR-T细胞的制备方法,其特征在于,所述载体为pX330-spCAS9-HF1载体。
8.根据权利要求6所述的制备方法,其特征在于,所述转染为脂质体转染或电转。
9.根据权利要求2-8任一项所述的通用型CAR-T细胞的制备方法,其特征在于,所述CAR-T细胞为针对不同疾病的CAR-T细胞。
10.根据权利要求9所述的制备方法,其特征在于,所述CAR-T细胞采用以下方法制备:
针对不同疾病制备的CAR结构,包装慢病毒,得到含有CAR结构的慢病毒;
所述慢病毒转入T细胞中,分选得到所述CAR-T细胞。
11.根据权利要求10所述的制备方法,其特征在于,所述CAR结构包含顺序连接的信号肽、胞外结合区、铰链区、跨膜区和胞内信号区的嵌合抗原受体。
12.根据权利要求11所述的制备方法,其特征在于,所述CAR结构为scFv-CD137-CD3ζ,其中,scFv针对不同疾病设置。
13.根据权利要求12所述的制备方法,其特征在于,所述scFv为抗CD19、抗CD20、抗BCMA中任一种。
14.权利要求2-13任一项所述的通用型CAR-T细胞的制备方法制得的通用型CAR-T细胞。
