背景
感染对于公共健康是日益严重的问题,从加强护理病房(ICU)到创伤和伤口感染,到免疫受损(immune compromised)中的感染。感染与不可接受的发病率、死亡率和花费相关。根据最近的EPIC II研究的结果,感染的患者具有高的多的医院死亡率,与非感染患者的14.8%相比高达33.1%。尽管最近媒体的关注聚焦于耐甲氧西林金黄色葡萄球菌(Staphylococcus aureus)(MRSA),在过去十年里MDR病原体的逐渐逼近的威胁已经逐渐变为革兰氏阴性病原体绿脓假单胞菌(Pseudomonas aeurginosa,PA)或鲍曼不动杆菌(Acinetobacter baumannii,AB)的增长。

同样地,多药耐药性鲍曼不动杆菌(MDRAB)是对威胁生命的感染负责的日益紧迫且未解决的病原体,特别是在ICU环境中,包括呼吸机相关肺炎、菌血症、手术部位感染和脓毒症。由于对多种抗生素,包括碳青霉烯类的耐受性,治疗此类由MDRAB引起的感染是困难的,并且可归因的死亡率是高的。
由MDRPA和MDRAB引起的感染对很多种类的抗生素几乎总是耐受的,并且通常耐受医药处方集中的所有抗生素。因此,存在日益增多的由这些生物体引起的无法用任何可用的抗生素或组合治疗的真正广泛耐受的感染的报道。
与高度抗生素耐受性质相关联,由MDRPA或MDRAB引起的感染与高死亡率有关。例如,MDRPA与增加的由于脓毒症的死亡率风险相关,并且是ICU死亡率的重要预测指标。最后,受关注的是,缺乏具有抵抗这些病原体(agent)的新机制的新抗生素或正在开发的那些抗生素。例如,即便是最近被批准的抗生素替加环素对MDRPA是无效的。更受关注的是,与现有制剂相比,目前在II期或更后期临床试验的对MDRPA或MDRAB具有改进的功效的抗生素即使有也是极少数。这些令人担忧的趋势强调了对于治疗抗生素耐受且致命的革兰氏阴性感染的新策略的迫切需要。
另一种广泛传播的感染性疾病是疟疾。非重症疟疾可以用口服药物治疗。目前对恶性疟原虫(P.falciparum)感染的治疗是使用青蒿素类与其他抗疟药组合(被称为青蒿素组合疗法,或ACT),其可以降低对任何单一药物组分的耐受。这些另外的抗疟药包括:阿莫地喹、苯芴醇、甲氟喹或磺胺多辛/乙胺嘧啶。另一种推荐的组合是二氢青蒿素和哌喹。当被用于治疗非重症疟疾时,ACT是约90%有效的。治疗感染间日疟原虫(P.vivax)、卵形疟原虫(P.ovale)或三日疟原虫(P.malariae)通常不需要住院。间日疟原虫的治疗需要血液阶段的治疗(用氯喹或ACT)和用伯氨喹清除肝脏形式。严重疟疾的推荐治疗是抗疟疾药物的静脉内使用。对于严重疟疾,在儿童和成人两者中,青蒿琥酯均优于奎宁。
尽管组合疗法的潜在益处,对多种疟疾治疗的药物耐受在21世纪成为日益严重的问题。现在耐受通常抵抗除了青蒿素类之外的所有类别的抗疟疾药物。耐受性菌株的治疗正变的越来越依赖于该类别的药物。青蒿素类的花费限制了其在发展中世界的使用。在柬埔寨-泰国边界发现的疟疾菌株对包括青蒿素类的组合疗法耐受,并且因此可能是不可治疗的。寄生虫群体以亚治疗剂量暴露至青蒿素单一疗法超过30年以及低于标准的青蒿素类的可得性可能推动耐受性表型的选择。
发明内容
概述

2.抗微生物肽

Kinocidin被天然地释放到血流中以在血液、血浆和血清的背景中发挥其抗微生物作用。Kinocidin的C末端α螺旋当被分离时,具有与天然Kinocidin相似或甚至比天然Kinocidin更强的抗感染活性。RP-1(ALYKKFKKKLLKSLKRLG;SEQ ID NO:3)是获取自多种kinocidin肽的α螺旋基序的共有序列。如实验中示出的,RP-1具有抵抗广泛范围的人类病原体的抗微生物活性。

根据本公开的一种实施方案,提供了融合肽,其包括RP-1(SEQ ID NO:3)或其变体以及CXCL4的γ-核心(CPTAQLIATLKNGRKICLDLQ,SEQ ID NO:2)或其变体。

在一方面,一个、两个或三个氨基酸的添加可以在肽的N末端或C末端。例如,γ-核心的变体可以包括CPTAQLIATLKNGRKICLDLQP(SEQ ID NO:4)、CPTAQLIATLKNGRKICLDLQAP(SEQ ID NO:5)和CPTAQLIATLKNGRKICLDLQA(SEQ ID NO:7)。
然而,添加、删除或置换并不需要在肽的末端。例如,即使SEQ ID NO:2的Cys1和Cys17形成二硫桥,用脯氨酸置换Cys17(即CPTAQLIATLKNGRKIPLDLQ;SEQ ID NO:8)不影响融合肽的活性。同样地,CPTAQLIATLKNGPKICLDLQ(SEQ ID NO:6)也是γ-核心的合适变体。

在一些实施方案中,肽可以缀合至治疗剂、前体药物、肽、蛋白、酶、病毒、脂质、生物响应调节剂、药剂或PEG。

3.抗微生物肽的合成
本文描述的肽可以从商业来源订购,或部分地或完全地利用本领域熟知的方法(例如,化合方法和/或生物技术方法)合成。在某些实施方案中,根据本领域熟知的固相肽合成方案来合成肽。在另一种实施方案中,肽根据熟知的Fmoc方案在固体支持物上合成,根据本领域技术人员已知的方法用三氟乙酸从支持物上裂解并通过层析法纯化。在其他实施方案中,利用本领域技术人员熟知的生物技术方法合成肽。在一种实施方案中,通过本领域技术人员已知的重组DNA技术,将编码期望的肽的氨基酸序列信息的DNA序列连接到表达质粒(例如,并入了用于肽的亲和纯化的亲和标签的质粒)中,将质粒转染到用于表达的宿主生物体中,然后例如通过亲和纯化从宿主生物体或生长培养基分离肽。
还可以通过利用重组表达系统制备肽。通常,这包括将核酸分子插入到与分子异源(即正常不存在)的表达系统中。可以将编码本公开的肽的一个或多个期望的核酸分子插入到载体中。当插入多个核酸分子时,该多个核酸分子可以编码相同或不同的肽。异源核酸分子以相对于启动子和任何其他5’调节分子适当的有义(5’-3’)方向以及正确的阅读框插入到表达系统或载体中。
可以通过若干种方法获得纯化的肽。优选地通过常规技术以纯化的形式(优选地至少约80%或85%纯的、更优选地至少约90%或95%纯的)生产肽。取决于重组宿主细胞是否被制备为将肽分泌到生长培养基中(参见美国专利第6,596,509号到Bauer等,通过引用以其整体特此并入),可以通过离心(以将细胞成分和包含分泌的肽的上清液分开)然后继续对上清液硫酸铵沉淀来分离并纯化肽。使包含肽的级分在合适尺寸的葡聚糖或聚丙烯酰胺柱中经历凝胶过滤,以将肽和其他蛋白分开。如果需要,可以通过HPLC进一步纯化肽级分。
4.抗微生物组合物和制剂
还提供了包括如本文公开的任一种或多种肽的组合物和制剂。在一种实施方案中,组合物包括任一种或多种肽以及药学上可接受的载体。
“药学上可接受的载体”指可以用于本公开的组合物的任何稀释剂、赋形剂或载体。药学上可接受的载体包括离子交换剂、氧化铝、硬脂酸铝、卵磷脂、血清蛋白(诸如人血清白蛋白)、缓冲物质(诸如磷酸盐)、甘氨酸、山梨酸、山梨酸钾、饱和植物脂肪酸的部分甘油酯混合物、水、盐或电解质(诸如鱼精蛋白硫酸盐、磷酸氢二钠、磷酸氢钾、氯化钠、锌盐)、硅溶胶、三硅酸镁、聚乙烯吡咯烷酮、基于纤维素的物质、聚乙二醇、羧甲基纤维素钠、聚丙烯酸酯、蜡、聚乙烯-聚氧丙烯嵌段共聚物、聚乙二醇和羊毛脂。在本领域的标准参考文本Remington's Pharmaceutical Sciences,Mack Publishing Company中描述了合适的药物载体。它们优选地根据意图的施用形式,即口服片剂、胶囊、酏剂、糖浆剂等来选择,并与常规的药物实践一致。
本公开的药物组合物可以通过本领域熟知的方法来制备,诸如除其他以外,常规的颗粒化、混合、溶解、胶囊化、冻干或乳化工艺。组合物可以以多种形式生产,包括颗粒、沉淀物或微粒、粉末(包括冻干的、旋转干燥的或喷雾干燥的粉末、无定形粉末)、注射剂、乳剂、酏剂、悬浮剂或溶液。制剂可以任选地包含稳定剂、pH调节剂、表面活性剂、生物利用度调节剂和这些的组合。

本公开的组合物被配制用于药物施用至哺乳动物、优选人类。本公开的此类药物组合物可以以多种方式施用,优选胃肠外施用。
本公开的无菌可注射形式的组合物可以是水性或油质悬浮液。这些悬浮液可以根据本领域已知的技术使用合适的分散或湿润剂和悬浮剂来配制。无菌可注射制品还可以是在无毒、胃肠外可接受的稀释剂或溶剂中的无菌可注射溶液或悬浮液,例如作为在1,3-丁二醇中的溶液。可以采用的可接受的媒介物和溶剂是水、林格氏溶液和等渗氯化钠溶液。另外,无菌的固定油通常用作溶剂或悬浮介质。为了该目的,任何温和的(bland)固定油可以被采用,包括合成的甘油单酯和甘油二酯。同天然的药学上可接受的油诸如橄榄油或蓖麻油一样,脂肪酸诸如油酸及其甘油酯衍生物在注射剂的制备中是有用的,特别是其聚氧乙烯的形式。这些油溶液或悬浮液还可以包含长链醇稀释剂或分散剂,诸如羧甲基纤维素或通常用于药学上可接受的剂型包括乳剂和悬浮液的配制的相似分散剂。其他常用的表面活性剂诸如吐温类、司盘类(Spans)以及常用于药学上可接受的固体、液体或其他剂型的制备的其他乳化剂或生物利用度增强剂也可以用于配制的目的。化合物可以被配制用于通过注射诸如通过弹丸式(bolus)注射或连续注射来胃肠外施用。用于注射的单位剂型可以在安瓿中或在多剂量容器中。

5.方法
还描述了使用本公开的肽、组合物和制剂的方法。在一种实施方案中,所述方法用于预防或治疗微生物的感染。微生物可以是细菌(诸如革兰氏阴性细菌)或寄生虫(例如引起疟疾的微生物)。在一些实施方案中,感染是多药耐药性的,诸如由多药耐药性绿脓假单胞菌(MDRPA)或多药耐药性鲍曼不动杆菌(MDRAB)引起的感染。
所述肽、组合物和制剂对于治疗与感染有关的疾病或症状(诸如伤口脓肿、导管生物膜、肺炎和菌血症)也是有用的。
在一些实施方案中,治疗方法还包括同时地或依次地施用第二种第二抗微生物剂。此类制剂的非限制性实例包括亚胺培南、头孢他啶、粘菌素、氯喹、青蒿素、万古霉素和达托霉素。

实施例
实施例1.体外机理研究
本实施例在体外测试了两种肽RP-1和γ-RP-1的抗细菌活性,并评价了这些肽的多参数作用机制(MOA)。
RP-1具有N-ALYKKFKKKLLKSLKRLG-C(SEQ ID NO:3)的命名序列,并且是获取自多种kinocidin蛋白的α螺旋基序的共有序列。γ-RP-1(SEQ ID NO:1)是在RP-1的N末端进一步包括CXCL4蛋白的γ-核心的一部分的融合物。尚未显示γ-核心具有抗感染活性。
使用针对MDRPA和MDRAB优化的多参数流式细胞术平台评价RP-1和γ-RP-1机制。在这些研究中,包括杀微生物与抑微生物动力学、膜损伤、能量解偶联和古老程序性细胞死亡途径(半胱天冬酶/metacaspase诱导和磷酯酰丝氨酸样脂质可接近性)的机制的评价。单独地或与常规抗生素(例如亚胺培南、头孢他啶或粘菌素)组合来确定作用机制。
图1A和1B示出了通过流式细胞术在体外单独地以及与抵抗MDRPA(47085或PA-01)和MDRAB(19606)的原始型菌株的常规抗生素组合评价的RP-1和γ-RP-1的作用机制(MOA)。评价的参数:细胞尺寸(前向散射[FSC];渗透稳态(osmostasis));细胞质折射率(侧向散射[SSC];DNA/RNA凝缩);细胞/膜能量学(Δψ;[ENR]);细胞膜渗透性(PRM);磷酯酰丝氨酸可接近性([PSA];程序性细胞死亡途径);半胱天冬酶或metacaspase诱导([CSP];程序性细胞死亡途径)。
测量的参数的增加由红色(环绕区域中的灰色)光谱移动指示,而参数的降低朝向紫色光谱(上部中间到左侧区域中的黑色)移动;绿色(背景灰色)指示从基线无显著改变。这些数据强调了元肽(metapeptide)的独特、具体的机理特征。每张图整合了使用多色流式细胞术和分析方法生成的超过120万个数据点。
本实施例比较了单独的元肽(RP-1和γ-RP-1)、单独的相关标准抗生素(亚胺培南、头孢他啶或粘菌素)、相较于元肽加抗生素的组合。本研究评价了元肽对多个方面的靶细胞功能的影响:渗透稳态、细胞质/核酸凝缩、能量学、膜渗透性、磷酯酰丝氨酸样脂质翻转和metacaspase或半胱天冬酶样激活(程序性细胞死亡的替代性生物标志物)。结果显示1)RP-1和γ-RP-1具有具体的机制并且并非非区别性膜清洁剂;2)作用机制相当大地区别于常规抗生素的那些作用机制;以及3)γ-RP-1和RP-1抵抗MDRPA与MDRAB的机制不同。
实施例2.在多药耐药性鲍曼不动杆菌肺炎的中性粒细胞减少的鼠模型中静脉内Kinocidinsin的效力
本实施例在多药耐药性鲍曼不动杆菌肺炎的中性粒细胞减少的鼠模型中测试了RP-1和γ-RP-1的效力。
方法和材料

抗微生物剂:RP-1(pI/Mw:10.82/2162.78Da)是源自PF-4家族kinocidin的18氨基合成肽。γ-RP-1(pI/Mw:10.64/4444.5Da)是包含CXCL4kinocidinγ-核心基序–RP-1构建体的39氨基合成肽。将溶解于无菌ddH2O的储备肽在合适的缓冲液中稀释。购买为纯化的粉末的抗生素,并按照制造商的指南重悬于PBS中。
半径测定:通过在pH5.5或7.5径向扩散评价RP-1和γ-RP-1的效力。在缓冲的琼脂糖中孵育对数期生物体,并铺板。将研究的肽(10μg/孔)引入到孔中接种的基质中,并孵育(3小时,37℃)。在覆盖营养培养基(TSA)后,孵育测定(24小时),并且将抑菌圈定义为完全清除的半径(mm)减去孔半径。独立最小量,n=2。
CLSI测定:在MHB中,跨100-0.19μg/ml的范围,依照CLSI标准评价RP-1和γ-RP-1的最小抑制浓度(MIC)。独立最小量,n=2。
生物基质测定:评价了RP-1和γ-RP-1在人血液、血浆或血清中的效力。将肽添加到同时具有MDRAB(最终接种物105CFU/ml;肽范围1-50μg/ml;体积100μl)的生物基质。使用Mueller Hinton肉汤(MHB;CLSI)进行对照。孵育(持续搅拌;2h,37℃)后,通过在血琼脂上培养(n=3)定量存活,并定义为CFU/ml。独立最小量,n=2。
作用机制.通过流式细胞术研究肽或抗生素对于MDRAB的机制:1)前向散射(FSC;渗透稳态);2)侧向散射(SSC;细胞质凝缩);3)能量学(ENR;细胞质膜[CM]电势);4)CM透化作用(PRM);5)磷酯酰丝氨酸可接近性(PSA;与自溶有关的脂质翻转);和6)半胱天冬酶-和/或metacaspase-样激活(CSP;程序性细胞死亡)。
中性粒细胞减少性肺炎.在第0天感染中性粒细胞减少的小鼠(在研究的第-2、+3和+8天醋酸可的松[250mg/kg;皮下注射]+环磷酰胺[200mg/kg;腹腔注射])(雾化;MDRAB临床分离物HUMC-1;5x108CFU;肺Cx阳性)。感染后4小时或24小时开始处理,并持续3天。分组:对照(PBS);亚胺培南(15mg/kg;腹腔注射;每日2次);γ-RP-1(12.5mg/kg;静脉注射;每日1次);γ-RP-1(12.5mg/kg;静脉注射;每日1次)。然后监测存活至第21天。在终点或第21天,培养肺和脾。通过非参数分析比较结果。所有研究遵守研究所和AAALAC动物护理政策。
结果
对照和亚胺培南处理的小鼠分别具有0%或16%的存活(图6)。单独的RP-1或γ-RP-1处理分别具有28%(相对于对照p<0.01)或72%(相对于对照或亚胺培南p<0.01)的存活(图6)。RP-1或γ-RP-1与亚胺培南处理的组合分别实现了60%或75%的存活率(相对于对照和亚胺培南p<0.01,图6)。与对照或单独的亚胺培南处理相比,在γ-RP-1和组合处理组中,肺和脾中的总MDRAB荷载(CFU)显著减少(>2logCFU)。
图2-4和6-7显示,RP-1和γ-RP-1对离体人类血液中的以及中性粒细胞减少性肺炎的鼠模型中的MDRAB具有显著的效力。RP-1和γ-RP-1对MDRAB的作用机制包括多个靶标,包括细胞膜扰动、能量功能失调和程序性细胞死亡(图5)。
在另外的致死性中性粒细胞减少性鼠模型中,单独的γ-RP-1或与亚胺培南组合的kinocidin的全身性(静脉注射)施用对MDRAB肺炎实现了稳健的效力。这些结果确认RP-1和γ-RP-1为创新且有效的生物候选物,用于进一步评价以解决MDRAB感染的日益紧迫的威胁。
目前的数据还证实,γ-RP-1肽克服了抗感染肽开发的很多历史性障碍,包括在显著免疫受损的宿主中建立的感染的高度严格模型中的系统(静脉注射)持久性、安全性和有效性。
实施例3.在MDRPA皮肤感染的小鼠模型中RP-1和γ-RP-1的效力
除了实施例2中的检查系统效力的研究之外,本实施例进行了进一步研究,其中评价了RP-1和γ-RP-1在MDRPA皮肤/皮肤结构感染(SSSI)中的局部效力。
本实施例首先评价了若干种局部媒介物,并且将20%P188泊洛沙姆鉴定为适合用于临床前研究,已经在人类中测试、低花费、出色的肽稳定性、快速吸收并且不抑制肽的抗微生物活性。在该鼠SSSI模型中,剃光侧腹并在肽处理开始前4小时或24小时皮下引入MDRPA(6X105CFU;对数期)。为了解决肽可能引起的任何潜在的免疫刺激作用,一部分(astrength of)模型仅用媒介物处理一侧的侧腹,并用媒介物+肽(100μg)处理同一动物的对侧的侧腹。初始数据表明了在降低损伤严重度和CFU/脓肿方面的强的效力,而无明显的由任一种肽引起的皮肤毒性。
还进行了初步研究以评价在由耐甲氧西林金黄色葡萄球菌(MRSA)引起的SSSI中肽的效力。首先,媒介物(20%P188泊洛沙姆)不抑制元肽对导致显著的人SSSI的原始型MRSA菌株即LAC-USA300的活性。与PBS相比,媒介物允许出色的稳定性并且不抑制RP-1或g-RP-1的抗MRSA活性。接下来,本实施例在初步研究中使用与以上相同的模型系统(MRSA接种物,3x107CFU)测试了肽的局部制剂的效力。结果表明,在7天内,肽在抑制MRSA损害严重度方面发挥了显著的效力。
实施例4.RP-1和γ-RP-1对广泛的革兰氏阴性病原体的效力
测试了RP-1和γ-RP-1连同很多肽变体和合适的对照对抗广泛的革兰氏阴性人类病原体的效力。在下表中列出了肽。

实施例5.γ-RP-1的体外和体内抗疟疾活性
本实施例测试了γ-RP-1的抗疟疾活性。
图12显示了γ-RP-1对3D7恶性疟原虫的体外抗疟疾作用。剂量响应曲线表明,在体外在12.5mg/ml孵育96小时后的出色IC50曲线(一式三份的两个实验)。图13显示了γ-RP-1对人脑内皮细胞(HBEC)的体外毒性测定的结果。该图显示,在体外在12.5mg/ml孵育96小时后(一式三份的两个实验),对HBEC低的毒性。
在体内实验中,图14展示了图像和柱状图,以显示12.5mg/kg的γ-RP-1与相同浓度的氯喹相比,中等的体内抗疟疾活性(每组5只小鼠,每日静脉注射处理持续5日)。图15和16显示了γ-RP-1对表达萤光素酶的伯氏疟原虫ANKA的体内抗疟疾活性(图15:单剂量;图16:两倍剂量)。CQ:氯喹。
这些数据表明了γ-RP-1在治疗氯喹或青蒿素耐受性疟疾中的效力。
实施例6.γ-RP-1及其变体对广泛的革兰氏阴性病原体的效力
测试了RP-1和γ-RP-1连同很多肽变体和合适的对照对抗广泛的人类病原体的效力。在下表中列出了肽。
与γ-RP-1(g-RP-1)相比,g-A-RP-1在γ-核心的C末端包括另外的丙氨酸,g-P-RP-1在γ-核心的C末端包括另外的脯氨酸,并且17P-g-RP-1用脯氨酸替代γ-核心的Cys17。
