Abstract

本发明提供了一种玻璃体或视网膜手术切口用粘合剂,包括医用胶和复合在所述医用胶上的温敏性水凝胶,所述温敏性水凝胶的相变温度为34℃~35℃。本发明还提供了一种玻璃体或视网膜手术切口用粘合系统,包括:注射器,所述注射器的针头内填充有医用胶,所述注射器的针筒内填充有温敏性水凝胶溶液,所述温敏性水凝胶的相变温度为34℃~35℃。本发明首先将医用胶注入手术切口,密合手术切口,防止细菌侵入且保持切口处平整,然后使温敏性水凝胶原位凝固在医用胶上,维持玻璃体内部稳定的压力,减少对眼底的影响。而且,所述医用胶和温敏性水凝胶均为可生物降解物质,在人体内逐渐生物降解,无需二次手术取出,不会造成二次损伤。

Description

玻璃体或视网膜手术切口用粘合剂及粘合系统
技术领域
本发明涉及眼睛相关疾病治疗技术领域,尤其涉及一种玻璃体或视网膜手术切口用粘合剂及粘合系统。
背景技术
视网膜、玻璃体疾病是眼科常见疾病,发病位于眼球后节,往往对视力造成较大的不可逆性损伤,其主要包:视网膜,脉络膜,视神经及玻璃体的炎症,肿瘤,各类血管的病变,以及各种变性疾病与多系统疾病引起的眼部病变。目前常见的眼底疾病有糖尿病视网膜病变,老年性黄斑病变,视网膜静脉阻塞等等。目前针对于大对数视网膜、玻璃体疾病的手术需要巩膜手术切口,去除视网膜附近或病变的玻璃体组织,从而为视网膜的治疗提供一定的操作空间。
目前处理巩膜眼底手术切口的方法主要有以下几种方法:一、传统手术缝合法;二、巩膜隧道密闭法;三、微创手术法。缝合方法能够较好的维持眼内压,但是由于缝合线对眼球的刺激、拆线二次损伤等原因,目前临床上缝合法应用很少。
巩膜隧道密闭法是在手术时确定巩膜切口部位,沿着眼球纵向轴的方向制备长1-2mm,直径23G的巩膜隧道切口,眼科显微手术器械可以通过巩膜隧道伸入到眼底进行操作,手术完毕后球结膜复位,利用眼内的压力推动巩膜隧道切口的内瓣向外运动,使得内瓣与外瓣紧密结合从而起到切口闭合目的。微创手术方法是利用巩膜钉横向刺破球结膜将顶端送达需要进行巩膜切口的部位,垂直穿透眼球壁将巩膜钉固定于巩膜上,眼科显微手术器械可以通过巩膜钉伸入到眼底进行操作,手术操作完毕后直接去掉巩膜钉,利用巩膜与球结膜的张力对微创切口进行闭合。上述两种方法虽然不存在二次损伤,但是,由于巩膜隧道切口与微创切口对于垂直于巩膜的力具有较好的抵抗能力,但是对于来自眼眶与眼睑等部位的力抵抗能力较弱,往往造成玻璃体、硅油、气体等眼内容物外漏引起较严重的结膜炎、结膜溃疡、脉络膜炎等,同时由于眼内物外漏引起一过性低眼压,可造成眼底血管突然出血,视网膜、脉络膜、睫状体脱离等,与此同时由于巩膜切口闭合不紧密容易引起眼内炎等严重的并发症。因此目前急需一种安全、有效、刺激小的视网膜、玻璃体手术切口封闭方法。
发明内容
本发明的目的在于提供一种玻璃体或视网膜手术切口用粘合剂及粘合系统,本发明提供的粘合剂用于密合玻璃体或视网膜手术切口时能够防止细菌侵入,维持玻璃体内部稳定的压力,减少对眼底的影响,且不会造成二次损伤。
本发明提供了一种玻璃体或视网膜手术切口用粘合剂,包括医用胶和复合在所述医用胶上的温敏性水凝胶,所述温敏性水凝胶的相变温度为34℃~35℃。
本发明提供的粘合剂包括医用胶,医用胶的目的在于粘合玻璃体或视网膜手术切口使切口保持自然形态,防止细菌侵入。在本发明中,所述医用胶选自e氰基丙烯酸、6B系胶或纤维蛋白生物型胶。
本发明提供的粘合剂包括复合在所述医用胶上的温敏性水凝胶,所述水凝胶原位复合在所述医用胶上,能够对抗眼球压力,维持玻璃体内部稳定的压力,减少对眼底的影响。本发明采用相变温度为34℃~35℃的温敏性水凝胶,在医用胶上原位凝固成凝胶后,在手术切口部位可以凝胶形式存在,直至降解。在一个实施例中,所述温敏性水凝胶选自PLGA-PEG-PLGA温敏性水凝胶或壳聚糖-甘油磷酸钠温敏性水凝胶。
本发明还提供了一种玻璃体或视网膜手术切口用粘合系统,包括:注射器,所述注射器的针头内填充有医用胶,所述注射器的针筒内填充有温敏性水凝胶溶液,所述温敏性水凝胶的相变温度为34℃~35℃。
本发明提供的粘合系统采用注射器分别装载医用胶和温敏性水凝胶,在密合玻璃体或视网膜手术切口时使用,使医用胶先密合手术切口,然后使温敏性水凝胶原位凝固在医用胶上,从而达到密合效果,防止细菌侵入,且能够维持玻璃体内部稳定的压力,减少对眼底的影响。
在本发明中,所述注射器为1mL注射器,针头内填充有医用胶,针筒内填充有温敏性水凝胶溶液,所述医用胶和温敏性水凝胶溶液的体积比为(10~15)μL:(0.5~1)mL。
所述粘合系统中,所述医用胶选自e氰基丙烯酸、6B系胶或纤维蛋白生物型胶。所述温敏性水凝胶选自PLGA-PEG-PLGA温敏性水凝胶或壳聚糖-甘油磷酸钠温敏性水凝胶。所述温敏性水凝胶溶液的质量浓度为10%~20%,优选为15%。
本发明提供的粘合系统使用方法如下:
按照常规微创方法进行视网膜或玻璃体手术,巩膜钉距离眼底手术切口1cm处横向刺破球结膜(避免伤到巩膜),在将巩膜钉的尖端对准巩膜预先设定手术切口部位进行垂直切口,手术过程中一定要保证巩膜切口的正上方球结膜的完整性,便于粘合剂的注射;
手术完毕,去掉巩膜钉时,将注射器内的粘合剂注射到巩膜切口的正上方球结膜下,针头内的医用胶首先被推注到巩膜伤口处,医用胶接触伤口后粘合伤口,使得伤口处于平整状态便于伤口的修复,但是医用胶强度较弱,不能够对抗眼内压力,后续温敏性水凝胶注射到巩膜切口的正上方,然后用恒温热风枪(37℃不会伤害球结膜,前端直径1cm)局部加热水凝胶注射部位,使得温敏性水凝胶快速凝固,在人体内以固体状态存在,利用球结膜的张力包裹固态的水凝胶,抵消玻璃体对巩膜的压力。
本发明提供的粘合系统中,医用胶粘合固定巩膜切口使之处于平整状态,再利用眼内压与温敏性水凝胶球结膜向内的压力维持巩膜伤口处于稳定状态,既能很好的维持眼内压力,同时保持巩膜伤口的平整性便于伤口的自身修复。本发明提供的粘合系统首先将医用胶注入手术切口,密合手术切口,防止细菌侵入且保持切口处平整,然后使温敏性水凝胶原位凝固在医用胶上,维持玻璃体内部稳定的压力,减少对眼底的影响。而且,本发明采用的医用胶和温敏性水凝胶均为可生物降解物质,在人体内逐渐生物降解,无需二次手术取出,不会造成二次损伤。

具体实施方式
为了进一步说明本发明,下面结合实施例对本发明提供的玻璃体或视网膜手术切口用粘合剂及粘合系统进行详细地描述,但不能将它们理解为对本发明保护范围的限定。
实施例1
准确称取PLGA-PEG-PLGA温敏水凝胶1.5g,其相变温度为34℃,无菌条件先加入无菌纯净水定溶到10mL,4℃条件下缓慢溶解7天,制备无菌温敏水凝胶溶液,分装于一次性无菌1mL注射器针筒内部;将科峰医用胶e氰基丙烯酸分装于无菌1mL注射器的注射针头内部,配合1ml注射器4℃保存备用。
实施例2
以新西兰白兔作为实验对象,以实施例1制备的粘合系统进行研究,以生理盐水作为对照组。
对新西兰白兔肌肉注射戊巴比妥钠(30mg/kg)、速眠新(0.1ml/kg)进行麻醉,待兔肌反射消失后进行眼睛表面麻醉、消毒。距离眼球12点钟方向1cm处,距角膜缘4mm,利用无菌一次性注射器针头(25G)刺破球结膜,在将无菌针头对准眼球12点钟预先设定好的切口位置垂直入针,制备眼底手术切口手术模型。
利用实施例1制备的粘合系统通过球结膜将生物材料注射到球结膜切口处,注射器的头部接触巩膜切口,缓慢推注医用胶粘合巩膜手术切口,抬高注射器针头注射温敏水凝胶于巩膜切口与球结膜之间,待巩膜隆起5mm左右的滤泡时停止注射(温敏水凝胶的压力与眼内容物的眼里相当),吸水棉签去除眼表、结膜囊内的液体,恒温热风枪快速凝固球结膜下的温敏水凝胶,如图1和图2所示,图1为本发明实施例对照组提供的眼底手术后巩膜切口状况,图2为本发明实施例实验组提供的眼底手术后巩膜切口状况。
术后一周裂隙灯观察球结膜损伤切口的变化,如图3和图4所示,图3为本发明实施例对照组提供的眼底手术一周后巩膜切口状况,图4为本发明实施例实验组提供的眼底手术一周后巩膜切口状况,图3和图4为裂隙灯放大25倍(25×)图像。由图3和图4可明显观察到对照组巩膜表面具有手术留下的切口,实验组可明显的观察到温敏水凝胶凝固于巩膜切口的正上方,保护手术切口免于严内容物外泄。
术后一个月后病理检测巩膜伤口恢复情况,如图5和图6所示,图5为本发明实施例对照组提供的眼底手术一个月后巩膜切口状况,图6为本发明实施例实验组提供的眼底手术一个月后巩膜切口状况,提5和图6为放大100倍(100×)图像。由图5和图6可知,一个月后对照组巩膜伤口恢复,纤维组织增生较少,巩膜切口局部菲薄;实验组组巩膜基本恢复,巩膜手术伤口处组织增生较厚,维持巩膜整体的完整性。
以上所述仅是本发明的优选实施方式,应当指出,对于本技术领域的普通技术人员来说,在不脱离本发明原理的前提下,还可以做出若干改进和润饰,这些改进和润饰也应视为本发明的保护范围。

Claims (6)

1.一种玻璃体或视网膜手术切口用粘合剂,包括医用胶和复合在所述医用胶上的温敏性水凝胶溶液,所述温敏性水凝胶的相变温度为34℃~35℃,所述温敏性水凝胶溶液的质量浓度为10%~20%;所述温敏性水凝胶选自PLGA-PEG-PLGA温敏性水凝胶或壳聚糖-甘油磷酸钠温敏性水凝胶。
2.根据权利要求1所述的粘合剂,其特征在于,所述医用胶选自e氰基丙烯酸、6B系胶或纤维蛋白生物型胶。
3.一种玻璃体或视网膜手术切口用粘合系统,其特征在于,包括:注射器,所述注射器的针头内填充有医用胶,所述注射器的针筒内填充有温敏性水凝胶溶液,所述温敏性水凝胶的相变温度为34℃~35℃,所述温敏性水凝胶溶液的质量浓度为10%~20%;
所述温敏性水凝胶选自PLGA-PEG-PLGA温敏性水凝胶或壳聚糖-甘油磷酸钠温敏性水凝胶。
4.根据权利要求3所述的粘合系统,其特征在于,所述注射器为1mL注射器。
5.根据权利要求4所述的粘合系统,其特征在于,所述医用胶和温敏性水凝胶溶液的体积比为(10~15)μL:(0.5~1)mL。
6.根据权利要求3所述的粘合系统,其特征在于,所述医用胶选自e氰基丙烯酸、6B系胶或纤维蛋白生物型胶。
