Abstract
The invention relates to an ilium ingrowth compound fixation half-pelvis prosthesis, which is characterized in that the half-pelvis prosthesis comprises: the ilium fixing assembly comprises ilium fixing flanks, ilium fixing platforms and cone connecting columns, and the ilium fixing flanks and the ilium fixing platforms are integrally connected to form an L-shaped structure; the ilium fixing flank is provided with a plurality of locking cortical bone screw holes and sacroiliac joint screw holes, the cortical bone screw is locked on the inner and outer ilium cortex through the locking cortical bone screw holes, and the sacroiliac joint screw is locked on the sacroiliac joint through the sacroiliac joint screw holes; the cone joint column is cylindrical, the upper end of the cone joint column is integrally connected to the lower part of the ilium fixing platform, and the lower end of the cone joint column is provided with a cone joint groove; the manual mortar cup is connected with the lower part of the cone connection column in a cone manner. The invention can be widely applied to the bone defect reconstruction of the continuous middle section of pelvis after the pelvis tumor is resected, and can be obviously improved no matter the function and the life quality of a patient recover from a recent structure to a long-term one.
Description
Technical Field
The invention relates to an artificial prosthesis, in particular to an ilium ingrowth compound type fixed half-pelvis prosthesis.
Background
Pelvic bone tumors are a common disease in pelvic injuries. Pelvic tumors can be classified into primary tumors and metastatic tumors, the former mainly being chondrosarcoma, osteosarcoma, chordoma, neurogenic tumors, etc., and the latter being bone metastases, which are common in breast cancer, renal cancer, prostate cancer, lung cancer, etc. The pelvic tumor is hidden in onset, difficult to diagnose in early stage, often has a large invasion range, complicated anatomy around the pelvis and adjacent to a plurality of important viscera, large operation difficulty, more complications, poor curative effect and high mortality rate. Various pelvic tumor resection reconstructive procedures have emerged from the past when pelvic tumors were mostly resected or modified by half pelvic resection amputation. Any excision of pelvic tumors creates a bone defect, most of which needs to be reconstructed to maintain continuity and stability of the trunk and pelvis, and pelvis and lower extremities. Such reconstruction can enable the patient to sit, stand and walk to maintain necessary activities of daily living, so that the development of pelvic tumor resection reconstruction operations at home and abroad is increasing. However, pelvic tumor resection and reconstruction still present significant challenges to orthopedics practitioners, and problems that may occur before, after and during surgery, such as excessive bleeding, difficult tumor resection, difficult reconstruction stability, complex selection of reconstruction modes, and the like. The main purpose of pelvic tumor resection and reconstruction is: complete tumor excision; complete resection with minimal cost including blood loss, complications, pain, wound healing, etc.; providing pelvic stability; a reliable and well functioning pelvis is obtained. The general principle of reconstruction mode selection is: selecting a proper and simple reconstruction party as far as possible; the age and activity of the patient and the nature of the tumor are fully considered to select an appropriate reconstruction mode; different reconstruction modes are adopted according to the loads and the geometric forms of the pelvic tumors related to different areas.
The main reconstruction method adopted in the prior art is artificial prosthesis reconstruction, joint fusion, autologous tumor bone inactivation and allogenic bone reconstruction. Wherein the joint fusion patient has obviously limited postoperative activity and poorer functions. The major problems with autologous and allogeneic bone remodeling are non-healing and infection. So with the continuous progress of artificial materials, metal prostheses are increasingly used for pelvic reconstruction, and in the past, customized prostheses are adopted, namely, artificial half pelvis with fixed size is manufactured before operation according to the defect condition of each patient. Complications and problems of early application are more, such as incapability of adjusting the prosthesis according to the actual excision size in operation; in addition, soft tissue coverage is difficult due to the larger size of the prosthesis, and thus there are many complications of the wound after surgery. The main problem of the existing semi-pelvic prosthesis is that the existing semi-pelvic prosthesis is fixed by a screw, so that bone ingrowth can not be realized, the real integration of a metal prosthesis and human bones can not be realized, and further long-term fixation of the prosthesis can not be realized.
Disclosure of Invention
In view of the above, it is an object of the present invention to provide an ilium ingrowth compound fixation semi-pelvic prosthesis.
In order to achieve the above purpose, the present invention adopts the following technical scheme: an ilium ingrowth composite fixation semi-pelvic prosthesis, comprising: the ilium fixing assembly comprises ilium fixing side wings, ilium fixing platforms and cone connecting columns, wherein the ilium fixing side wings and the ilium fixing platforms are integrally connected to form an L-shaped structure; the ilium fixing side wings are provided with a plurality of locking cortical bone screw holes and sacroiliac joint screw holes, the cortical bone screws are locked on the inner and outer ilium cortex through the locking cortical bone screw holes, and the sacroiliac joint screws are locked on the sacroiliac joints through the sacroiliac joint screw holes; the cone joint column is cylindrical, the upper end of the cone joint column is integrally connected to the lower part of the ilium fixing platform, and the lower end of the cone joint column is provided with a cone joint groove; the manual mortar cup is connected with the lower part of the cone connection column in a cone manner.
In a preferred embodiment, the contact surfaces of the ilium fixation wings and ilium fixation platforms with the ilium osteotomy face and ilium outer plate, respectively, are pore structures with pore diameters of 600-1000 microns and porosities of 55-85%.
In a preferred embodiment, the interface of the ilium fixation side wings with the ilium outer plate cortex is anatomically designed with the ilium outer plate surface features.
In a preferred embodiment, the artificial cup is a super-radius polyethylene spherical shell, and a plurality of holes are formed in the artificial cup.
Due to the adoption of the technical scheme, the invention has the following advantages: 1. the invention optimizes the anatomic attachment and nail way design of the ilium outer plate, can realize the stable mechanical fixation of the ilium fixing component and ilium, and provides a foundation for initial stabilization and long-term bone ingrowth. 2. The contact surfaces of the ilium fixing flank and the ilium fixing platform of the invention with the ilium osteotomy surface and the ilium outer plate are pore structures with pore diameters of 600-1000 micrometers and porosities of 55-85%, and the pore structures can realize long-term bone ingrowth. The invention can be widely applied to the bone defect reconstruction of the continuous middle section of pelvis after the pelvis tumor is resected, and can be obviously improved no matter the function and the life quality of a patient recover from a recent structure to a long-term one.

Detailed Description
The present invention will be described in detail with reference to the accompanying drawings and examples. It is to be understood, however, that the drawings are designed solely for the purposes of providing a better understanding of the invention and are not to be construed as limiting the invention.
As shown in fig. 1, the present invention provides a semi-pelvic prosthesis comprising an ilium fixation assembly 10 and an artificial cup 20. Wherein, the ilium fixation assembly 10 comprises an ilium fixation side wing 11, an ilium fixation platform 12 and a taper post 13, and the ilium fixation side wing 11 and the ilium fixation platform 12 are integrally connected to form an L-shaped structure. Two locking cortical screw holes 14 and 3 sacroiliac joint screw holes 15 are formed in the ilium fixing flank 11, the cortical screw passes through the inner and outer ilium cortex through the locking cortical screw holes 13, and the sacroiliac joint screw passes through the sacroiliac joint screw holes 14 to realize mechanical fixation of the ilium fixing component 10 and ilium. The tapered post 13 is cylindrical and integrally connected at its upper end to the lower portion of the ilium fixation platform 12 and has a tapered groove (not shown) at its lower end. An artificial cup 20 is connected to the lower part of the cone connection column 13 in a cone manner, and the artificial cup 20 is a base for supporting and fixing an acetabular liner.
In a preferred embodiment, the contact surfaces of the ilium fixation wings 11 and the ilium fixation platforms 12 with the ilium osteotomy face and the ilium outer plate, respectively, are pore structures with pore diameters of 600-1000 microns and porosities of 55-85%, which allow for long term bone ingrowth.
In a preferred embodiment, the interface of the ilium fixation wings 11 with the ilium outer plate cortex is anatomically designed with the ilium outer plate surface features to better affix the ilium outer plate cortex.
In a preferred embodiment, the artificial cup 20 is a super-radius polyethylene spherical shell, and the artificial cup 20 is provided with a plurality of holes to facilitate bone cement fixation and tendon and other soft tissue attachment during surgery.
The foregoing embodiments are only for illustrating the present invention, wherein the structures, connection modes, manufacturing processes, etc. of the components may be changed, and all equivalent changes and modifications performed on the basis of the technical solutions of the present invention should not be excluded from the protection scope of the present invention.
Claims (2)
1. An ilium ingrowth composite fixation semi-pelvic prosthesis, comprising:
the ilium fixing assembly comprises ilium fixing flanks, ilium fixing platforms and cone connecting columns, and the ilium fixing flanks and the ilium fixing platforms are integrally connected to form an L-shaped structure; the ilium fixing side wings are provided with a plurality of locking cortical bone screw holes and sacroiliac joint screw holes, the cortical bone screws are locked on the inner and outer ilium cortex through the locking cortical bone screw holes, and the sacroiliac joint screws are locked on the sacroiliac joints through the sacroiliac joint screw holes; the cone joint column is cylindrical, the upper end of the cone joint column is integrally connected to the lower part of the ilium fixing platform, and the lower end of the cone joint column is provided with a cone joint groove;
the artificial mortar cup is connected with the lower part of the cone connection column in a cone manner;
the contact surfaces of the ilium fixing flank and the ilium fixing platform with the ilium osteotomy surface and the ilium outer plate are pore structures with pore diameters of 600-1000 micrometers and pore ratios of 55-85 percent;
the contact surface of the ilium fixing flank and the ilium outer plate bone cortex adopts the anatomical design of the ilium outer plate surface characteristics.
2. The ilium ingrowth composite fixation semi-pelvic prosthesis of claim 1, wherein the artificial cup is a super-radius polyethylene spherical shell, and the artificial cup is provided with a plurality of holes.
