Description
Technical Field
The invention relates to the technical field of artificial hemi-pelvic prostheses, in particular to a sacrum pubis bilateral osseointegration hemi-pelvic prosthesis.
Background
The pelvis is a position where bone tumors easily occur, and reconstruction of the pelvis function after resection of the pelvic tumor is a great challenge for the bone oncologist due to the complex anatomical structure of the pelvis. At present, the typing of tumor resection and reconstruction of pelvic tumors is mainly Enneking typing, and is divided into: type I: cutting off the ilium; type II: cutting the periphery of the acetabulum; type III: cutting ischia and pubis; and IV, type: the scope of resection involves the sacrum. For the Enneking I + II + III area pelvic malignancies. The task of reconstructing the semi-pelvic prosthesis is particularly difficult due to a series of problems such as an excessively large resection range, a difficult fixing mode and difficult acetabulum reconstruction position.
At present, the methods mainly adopted for pelvic reconstruction include artificial prosthesis reconstruction, joint fusion, reconstruction of allogeneic bone, inactivation and replantation of autologous tumor bone and the like. Among them, techniques such as joint fusion, allograft bone reconstruction, autologous tumor bone inactivation and replantation are often eliminated due to factors such as high infection rate, limitation of bone source, high tumor recurrence risk, etc. At present, the reconstruction of the artificial semi-pelvic prosthesis gradually occupies the dominant position of the treatment of the malignant pelvic tumor due to the factors of good initial stability, easy acceptance of the appearance, relatively quick functional recovery and the like. To date, the clinically common prostheses are mainly: saddle-shaped prostheses, ice cream cone prostheses, built-up prostheses, and customized prostheses. At present, the prosthesis on the market is mainly a pure acetabular prosthesis, and a pubic symphysis part is not added. However, the pubic symphysis can resist shear forces and the pubic symphysis, as part of the anterior pelvic ring (consisting of the sacrum and the two lateral hip bones, each consisting of the ilium, ischium and pubis), plays an important role in maintaining the stability of the pelvic ring. Therefore, the prosthesis without pubic symphysis may have insufficient shear resistance, and the screws are often loosened and even pulled out by the load from different directions during daily activities, which finally results in the failure of the prosthesis. Moreover, prostheses in the market are often fixed only by screws, and the fixing structure only has a function of mechanical fixation, cannot realize bone ingrowth and further cannot realize long-term fixation of the metal prostheses and human bones.
