Abstract

分子标志物BUB1B基因及其应用,是一种用于多发性骨髓瘤诊断的分子标志物,具体涉及骨髓瘤细胞中的分子标志物BUB1B基因在制备诊断多发性骨髓瘤的产品中的应用。本发明首次发现了BUB1B基因表达与多发性骨髓瘤的相关性。即可以通过检测受试者中BUB1B的表达情况,来判断受试者是否患有多发性骨髓瘤或者判断受试者是否存在患有多发性骨髓瘤的风险,为临床医师提供诊断的依据和个性化预防方案或治疗方案。本发明发现了一种新的生物标记物‑BUB1B基因,相比传统的检测手段,基因诊断具有特异性好、灵敏度高、针对性强、诊断快速等优点,能够实现多发性骨髓瘤的早期诊断,降低多发性骨髓瘤的死亡率。

分子标志物BUB1B基因及其应用
技术领域
本发明属于分子诊断领域,涉及一种用于多发性骨髓瘤诊断的分子标志物,具体涉及骨髓瘤细胞中的分子标志物BUB1B基因在制备诊断多发性骨髓瘤的产品中的应用。
背景技术
多发性骨髓瘤(Multiple Myeloma,MM)是一种浆细胞异常增生的血液系统恶性肿瘤,约占所有血液系统恶性肿瘤的10%,随着我国人口的老龄化,MM的发病率都在上升。多发性骨髓瘤常伴有多发性溶骨性损害、高钙血症、贫血、肾脏损害等。由于正常免疫球蛋白的生成受抑制,因此容易出现各种细菌性感染。
目前多发性骨髓瘤的诊断主要依靠骨髓克隆性浆细胞比例和(或)活组织病理学检查作为MM诊断基础。2014年,国际骨髓瘤工作组(IMWG)对MM诊断标注进行了修订,定义骨髓克隆性浆细胞比例≥10%和/或活组织病理学检查证实为浆细胞瘤;同时伴有以下一种或多种情况:高钙血症、肾功能不全、贫血和溶骨性骨损害;将骨髓克隆性浆细胞比例≥60%、受累血清游离轻链与未受累血清游离轻链比值≥100、MRI检查发现局灶性病变部位>1处(每处检查的病灶直径≥5mm)作为极高危无症状MM生物学标志物纳入MM诊断标准。此外,MRI、低剂量全身CT、FDG/PET及PET/CT等现代影像学技术被纳入检查辅助方法。
到目前为止,很多生物标志物都不能很好的用来作为临床诊断标志。目前临床常用的诊断标志物是单克隆免疫球蛋白(M蛋白),在MM患者血清中M蛋白所属免疫球蛋白显著增高而非M蛋白所属免疫球蛋白含量显著下降。但修订后的MM诊断标准强调单克隆蛋白水平并不是诊断活动性MM所必需。
综上,目前多发性骨髓瘤诊断的方法复杂、检查项目多,缺少一种可靠、方便的临床诊断方法。新的研究表明,在多发性骨髓瘤患者中,检出BUB1B基因的表达显著增高,BUB1B基因的表达水平与骨髓瘤细胞的生长或抑制有直接关系。表明BUB1B是一种高危的骨髓瘤基因,可为多发性骨髓瘤的临床诊断提供可靠的判断。
BUB1B是有丝分裂检验点蛋白之一,本课题组证实在多发性骨髓瘤患者中的表达异常增高。其在MM中的异常表达通过影响染色体的不稳定性以及有丝分裂的过程等促进肿瘤的发展、转移及预后。因此,BUB1B的表达水平为肿瘤诊断、评估、预后等提供一个重要指标。
发明内容
解决的技术问题:本发明提供了一种分子标志物BUB1B基因的应用,可用于多发性骨髓瘤的诊断。相比于目前的诊断方法,大大减少了检查项目,降低成本;配合辅助诊断方法,准确率高;具有及时性,特异性,灵敏性。
技术方案:骨髓瘤细胞中的分子标志物BUB1B基因,序列如SEQ ID NO.1所示。

有益效果:本发明首次发现了BUB1B基因表达与多发性骨髓瘤相关。通过受试者中BUB1B的表达,来判断受试者是否患有多发性骨髓瘤或者判断受试者是否存在患有多发性骨髓瘤的风险,为临床医师提供诊断的依据和个性化预防方案或治疗方案。本发明发现了一种新的生物标记物-BUB1B基因,相比传统的检测手段,基因诊断具有特异性好、灵敏度高、针对性强、诊断快速等优点,能够实现多发性骨髓瘤的早期诊断,降低多发性骨髓瘤的死亡率。

图1显示利用基因芯片检测BUB1B基因在多发性骨髓瘤患者和正常人中的表达差异图。分析22例正常骨髓,44例意义未明的单克隆丙种球蛋白疾病(monoclonalgammopathy of undetermined significance,MGUS,MM前体疾病),351例MM初诊样本,发现BUB1B基因的表达水平在MM患者中要明显高于正常以及MGUS样本。
图2显示利用QPCR检测BUB1B基因在多发性骨髓瘤患者和正常人中的表达差异图。
图3显示利用免疫组化染色分析BUB1B在多发性骨髓瘤患者中的表达水平图,与正常浆细胞相比,BUB1B在MM样本中显著增高。
具体实施方式
下面结合附图和实施例对本发明作进一步详细的说明。以下实施例仅用于说明本发明而不用于限制本发明的范围。实施例中未注明具体条件的试验方法,通常按照常规条件,例如Sambrook等人,分子克隆:实验室手册(New York:Cold Spring HarborLaboratory Press,1989)中所述的条件,或按照制造厂商所建议的条件。
本发明提供了检测BUB1B基因表达的产品在制备诊断多发性骨髓瘤的工具中的应用。
进一步,上面所提到的检测BUB1B基因表达的产品包括:通过RT-PCR,实时定量PCR、免疫检测、原位杂交、芯片或高通量测序平台等检测BUB1B基因表达水平以诊断多发性骨髓瘤的产品。

优选地,所述诊断工具包括芯片、试剂盒、试纸或高通量测序平台。其中,高通量测序平台是一种特殊的诊断工具,检测BUB1B基因表达的产品可以应用于该平台实现对BUB1B基因的表达情况的检测。随着高通量测序技术的发展,对一个人的基因表达谱的构建将成为十分便捷的工作。通过对比疾病患者和正常人群的基因表达谱,容易分析出哪个基因的异常与疾病相关。因此,在高通量测序中获知BUB1B基因的异常与多发性骨髓瘤相关也属于BUB1B基因的用途,同样在本发明的保护范围之内。
本发明还提供了一种多发性骨髓瘤的工具,所述产品包括芯片、试剂盒、试纸、或高通量测序平台。
其中,所述芯片包括基因芯片、蛋白质芯片;所述基因芯片包括固相载体以及固定在固相载体的寡核苷酸探针,所述寡核苷酸探针包括用于检测BUB1B基因转录水平的针对BUB1B基因的寡核苷酸探针;所述蛋白质芯片包括固相载体以及固定在固相载体的BUB1B蛋白的特异性抗体;所述基因芯片可用于检测包括BUB1B基因在内的多个基因(例如,与多发性骨髓瘤相关的多个基因)的表达水平。所述蛋白质芯片可用于检测包括BUB1B蛋白在内的多个蛋白质(例如与多发性骨髓瘤相关的多个蛋白质)的表达水平。通过将多个与多发性骨髓瘤相关的标志物同时检测,可大大提高多发性骨髓瘤诊断的准确率。

所述高通量测序平台包括检测BUB1B基因表达水平的试剂。
所述试纸包括试纸载体和固定在试纸载体上的寡核苷酸,所述寡核苷酸能够检测BUB1B基因的转录水平。
与BUB1B基因的核酸序列杂交的探针可以是DNA、RNA、DNA-RNA嵌合体、PNA或其他衍生物。所述探针的长度没有限制,只要完成特异性杂交、与目的核苷酸序列特异性结合,任何长度都可以。所述探针的长度可短至25、20、15、13或10个碱基长度。同样,所述探针的长度可长至60、80、100、150、300个碱基对或更长,甚至整个基因。由于不同的探针长度对杂交效率、信号特异性有不同的影响,所述探针的长度通常至少是14个碱基对,最长一般不超过30个碱基对,与目的核苷酸序列互补的长度以15-25个碱基对最佳。所述探针自身互补序列最好少于4碱基对,以免影响杂交效率。
进一步,所述BUB1B蛋白的特异性抗体包括单克隆抗体、多克隆抗体。所述BUB1B蛋白的特异性抗体包括完整的抗体分子、抗体的任何片段或修饰(例如:嵌合抗体,scFv、Fab、F(ab’)2、Fv等)。只要所述片段能够保留与BUB1B蛋白的结合能力即可。用于蛋白质水平的抗体的制备时本领域技术人员公知的,并且本发明可以使用任何方法来制备所述抗体。
本发明的具体实施方案中,所述针对BUB1B基因的引物序列如下:正向引物序列如SEQ ID NO.3所示,反向引物序列如SEQ ID NO.4所示。
用于诊断多发性骨髓瘤的BUB1B基因及其表达产物的来源包括但不限于骨髓、血液、组织液、尿液、唾液等可以获得基因组DNA的体液。在本发明的具体实施方案中,用于诊断多发性骨髓瘤的BUB1B基因及其表达产物的来源是骨髓。
本发明的上下文中,“BUB1B基因”包括BUB1B基因以及BUB1B基因的任何功能等同物的多核苷酸。BUB1B基因包括与目前国际公共核酸序列GeneBank中BUB1B基因(NC_000015.10)DNA序列具有70%以上同源性,且编码相同功能蛋白质的DNA序列;
优选地,BUB1B基因的编码序列包括以下任意一种:
(1)序列表中SEQ ID NO.1所示的DNA序列;
(2)在严格条件下与1)限定的DNA序列杂交且编码相同蛋白质的DNA序列;
(3)与(1)或(2)限定的DNA序列具有70%、优选地,90%以上同源性,且编码相同功能蛋白质的DNA分子。
在本发明的具体实施方案中,所述BUB1B基因的编码序列是SEQ ID NO.1所示的DNA序列。
在本发明的上下文中,BUB1B基因表达产物包括BUB1B蛋白以及BUB1B蛋白的部分肽。所述BUB1B蛋白的部分肽含有与多发性骨髓瘤相关的功能域。
“BUB1B蛋白”包括BUB1B蛋白以及BUB1B蛋白的任何功能等同物。所述功能等同物包括BUB1B蛋白保守性变异蛋白质、或其活性片段,或其活性衍生物,等位变异体、天然突变体、诱导突变体、在高或低的严谨条件下能与BUB1B的DNA杂交的DNA所编码的蛋白质。
优选地,BUB1B蛋白是具有下列氨基酸序列的蛋白质:
(1)由序列表中SEQ ID NO.2所示的氨基酸序列组成的蛋白质;
(2)将SEQ ID NO.2所示的氨基酸序列经过一个或几个氨基酸残基的取代和/或缺失和/或添加且与SEQ ID NO.2所示的氨基酸序列具有相同功能的由SEQ ID NO.2所示的氨基酸序列衍生物的蛋白质。取代、缺失或者添加的氨基酸的个数通常为1-50个,较佳地1-20个,最佳地1-10个。
(3)与SEQ ID NO.2所示的氨基酸序列至少具有80%同源性(又称为序列统一性),更优选地,与SEQ ID NO.2所示的氨基酸序列至少约90%至95%的同源性,常为96%、97%、98%、99%同源性的氨基酸序列构成的多肽。
在本发明的具体实施方案中,所述BUB1B蛋白是具有SEQ ID NO.2所示的氨基酸序列的蛋白质。
通常,已知的是,一个蛋白质中一个或多个氨基酸的修饰不会影响蛋白质的功能。本领域技术人员会认可改变单个氨基酸或小百分比的氨基酸或对氨基酸序列的个别添加、缺失、插入、替换是保守修饰,其中蛋白质的改变产生具有相似功能的蛋白质。提供功能相似的氨基酸的保守替换表是本领域公知的。
通过添加一个氨基酸或多个氨基酸残基修饰的蛋白质的例子是BUB1B蛋白的融合蛋白。对于与BUB1B蛋白融合的肽或者蛋白质没有限制,只要所得的融合蛋白保留BUB1B蛋白的生物活性即可。
本发明的BUB1B蛋白也包括对SEQ ID NO.2所示的氨基酸序列的非保守修饰,只要经过修饰的蛋白质仍然能够保留BUB1B蛋白的生物学活性即可。在此类修饰蛋白质中突变的氨基酸数目通常是10个或者更少,例如6个或者更少,例如3个或者更少。
在本发明的上下文中,“多发性骨髓瘤”既包括判断受试者是否已经患有多发性骨髓瘤、也包括受试者是否存在患有多发性骨髓瘤的风险。
实施例1筛选多发性骨髓瘤患者和正常人中差异表达的基因
1、研究对象:
收集多发性骨髓瘤患者8例,正常人8例
多发性骨髓瘤患者的纳入标准:所有多发性骨髓瘤的诊断根据WHO诊断多发性骨髓瘤标准,所有初治多发性骨髓瘤患者骨髓单个核细胞标本获得病人的知情同意。
多发性骨髓瘤患者的排除标准:转移癌,反应性浆细胞增多症和未定性的单克隆免疫球蛋白病。
2、骨髓中总RNA的提取
(1)骨髓单个核细胞的分离:向收集到的骨髓标本中加入3倍体积的红细胞裂解液,用滴管反复吹打混匀后于冰上放置10~15min,使骨髓标本中的红细胞充分裂解;1200rpm/min离心5min,弃上清,PBS洗涤细胞1次。弃上清,加入1mL PBS吹打混匀后转移至1.5mL EP管中,1200rpm/min离心5min,弃上清。每个样本加1mL Trizol,吹打混匀。
(2)每1mL Trizol加入氯仿200μL,充分混匀剧烈震荡15秒,室温静置10min等待分层。
(3)于4℃12000rpm/min离心15min。离心后可见管中混合物液面分为3层,最上层为无色水相,含有RNA;中间层为白色絮状,含有蛋白层与DNA;最下层为红色的酚-氯仿有机相。
取上层水相约400μL至另一EP管中,注意避免吸入中间的蛋白层,每管加入等体积异丙醇,轻轻上下颠倒混匀,室温静置10min,于4℃12000rpm/min离心10min,可见管底白色沉淀。
弃上清,加入-20℃预冷的75%DEPC酒精1mL,悬起沉淀,4℃7500rpm/min离心5min。
弃上清,室温放置10-20min风干沉淀,依据沉淀量的多少酌情加入DEPC水溶解沉淀。-80℃冰箱保存。
3、RNA样品的质量分析
紫外分光光度计检测RNA样品,OD260/OD280为1.8-2.0。
4、基因芯片杂交及扫描
采用Affymetrix U133Plus 2.0microarrays检测RNA水平。多发性骨髓瘤患者组的数据可在NIH基因表达综合(GEO)下GSE2658检索号查得,正常组和未定性的单克隆免疫球蛋白病的基因芯片数据在GSE5900下。
5、芯片数据处理与分析
基因芯片的数据分析基于GCO1.1软件(Affymetrix),包含对与疾病相关相关的生存的单变量的对数等级检测。对于两组比值的自然对数绝对值大于2.0或小于0.5的基因作为差异表达基因。
6、统计学处理
采用SPSS 13.0统计软件进行数据分析,组间差异比较采用单因素方差分析法,P<0.05有显著意义。
7、结果
如图1所示,分析22例正常骨髓,44例意义未明的单克隆丙种球蛋白疾病(monoclonal gammopathy of undetermined significance,MGUS,MM前体疾病),351例MM初诊病人浆细胞基因芯片数据库,发现BUB1B基因的表达水平在MM患者中要明显高于正常以及MGUS样本),与正常人相比,多发性骨髓瘤患者BUB1B基因的MRNA水平显著升高,差异具有统计学意义(P<0.05)。
实施例2QPCR实验验证多发性骨髓瘤患者和正常人中差异表达的基因
1、研究对象
筛选标准同实施例1,多发性骨髓瘤患者46例,正常人66例。
2、骨髓中总RNA的提取
步骤同实施例1。
3、逆转录
根据TAKARA逆转录试剂盒进行RNA逆转录。采用10μL反应体系,分别加入5×PrimeScript RT Master Mix 2μL,总RNA 0.5μg,加一定量RNase Free Water至10μL。37℃反应15min,85℃反应5s。产物cDNA置-20℃冰箱保存。
4、QPCR
采用10μL反应体系,每个样本设置3个复孔,加样过程避光。配制以下反应体系:2×SYBGREEN Master Mix 5μL,正向引物0.5μL,反向引物(5pm)0.5μL,反向引物(5pm)0.5μL,模板cDNA 1μL,ddH2O 3μL。扩增FOXM1基因的正向引物序列为5’-GCACCGACAATTCCAAGCTC-3’(SEQ ID NO.3),反向引物序列为5’-TGTGCTTCGTTGTGGTACAGA-3’(SEQ ID NO.4);扩增GAPDH基因的正向引物序列为5’-GCACCGTCAAGGCTGAGAAC-3’(SEQID NO.5),反向引物序列为5’-TGGTGAAGACGCCAGTGGA-3’(SEQ ID NO.6),各项操作均于冰上进行。扩增程序为:95℃1min,95℃15s,60℃15s,72℃30s;共40个循环。以SYBR Green作为荧光标记物,在Light Cycler荧光实时定量PCR仪上进行反应,通过融解曲线分析和电泳确定目的条带,△△CT法进行相对定量。
5、统计学方法
结果数据都是以平均值±标准差的方式来表示,采用SPSS13.0统计软件来进行统计分析,两者之间差异采用t检验,认为当P<0.05时具有统计学意义。
6、结果
结果如图2所示,与正常人相比,多发性骨髓瘤患者的BUB1B基因的mRNA水平显著增高,差异具有统计学意义(P<0.05),结果同基因芯片实验。

1.骨髓瘤细胞中的分子标志物BUB1B基因,其特征在于序列如SEQ ID NO.1所示。
2.骨髓瘤细胞中的分子标志物BUB1B蛋白,其特征在于序列如SEQ ID NO.2所示。
3.骨髓瘤细胞中的分子标志物BUB1B基因在制备诊断多发性骨髓瘤的产品中的应用。
4.根据权利要求3所述的应用,其特征在于所述产品包括:通过RT-PCR、实时定量PCR、免疫检测、原位杂交、芯片或高通量测序平台检测BUB1B基因表达水平以诊断多发性骨髓瘤的产品。
5.根据权利要求4所述的应用,其特征在于所述用RT-PCR诊断多发性骨髓瘤的产品至少包括一对特异扩增BUB1B基因的引物;所述用实时定量PCR诊断多发性骨髓瘤的产品至少包括一对特异扩增BUB1B基因的引物;所述用免疫检测诊断多发性骨髓瘤的产品包括:与BUB1B蛋白特异性结合的抗体;所述用原位杂交诊断多发性骨髓瘤的产品包括:与BUB1B基因的核酸序列杂交的探针;所述用芯片诊断多发性骨髓瘤的产品包括:蛋白芯片和基因芯片,其中蛋白芯片包括与BUB1B蛋白特异性结合的抗体,基因芯片包括与BUB1B基因的核酸序列杂交的探针。
6.根据权利要求5所述的应用,其特征在于所述用实时定量PCR诊断多发性骨髓瘤的产品至少包括一对特异扩增BUB1B基因的引物,序列如:SEQ ID NO.3和SEQ ID NO.4所示。
7.一种用于诊断多发性骨髓瘤的工具,其特征在于所述工具能够检测样本中BUB1B基因的表达。
8.根据权利要求7所述的工具,其特征在于所述工具包括芯片、试剂盒、试纸或高通量测序平台。
9.根据权利要求8所述的工具,其特征在于所述芯片包括基因芯片或蛋白质芯片,所述基因芯片包括固相载体以及固定在固相载体的寡核苷酸探针,所述寡核苷酸探针包括用于检测BUB1B基因转录水平的针对BUB1B基因的寡核苷酸探针;所述蛋白质芯片包括固相载体以及固定在固相载体的BUB1B蛋白的特异性抗体;所述试剂盒包括基因检测试剂盒和蛋白免疫检测试剂盒;所述基因检测试剂盒包括用于检测BUB1B基因转录水平的试剂;所述蛋白免疫检测试剂盒包括BUB1B蛋白的特异性抗体;所述试纸包括用于检测BUB1B基因转录水平的试纸;所述高通量测序平台包括用于检测BUB1B基因转录水平的试剂。
