Abstract
一种治疗后交叉韧带撕脱骨折的可吸收固定袢,属于骨科医疗器械技术领域,用于关节镜下治疗后交叉韧带撕脱骨折。其技术方案是:固定袢的两端为平行相对的固定平板,固定袢的中间为向上的拱起,拱起由压板和弧形板组成,压板为平板,两个弧形板的上端分别连接在压板的两端,在固定袢两端的固定平板上分别有穿线孔,固定袢的材质为可吸收材料。本发明可以使骨折面贴合更加紧密,有利于愈合;本发明可在体内完全降解吸收,避免了术后关节腔内异物留存的风险,解决了二次手术取出内固定的难题;本发明可提供最佳化硬度,降低了术后内固定断裂的风险。本发明结构简单、使用方便、效果极好,能够使后交叉韧带撕脱骨折的治疗获得最佳化的预后疗效。
Description
技术领域
本发明涉及一种用于关节镜下治疗后交叉韧带撕脱骨折的可吸收固定袢,属于骨科医疗器械技术领域。
背景技术
后交叉韧带撕脱骨折是一种运动医学领域暴力损伤,常见于车祸伤或高水平运动员竞技运动损伤。传统的治疗方法是切口膝关节后部对骨折块进行固定,但是由于膝关节后部血管和神经分布丰富,很容易造成损伤,临床治疗中难度很大。
随着精准医疗和微创理念的推广普及,对于后交叉韧带胫骨后髁撕脱骨折,临床上所选用的治疗方法是使用不可吸收线或钢丝行套扎韧带止点连接部或撕脱骨块,即关节镜下撕脱骨折套扎固定术。然而此种手术治疗方法存在的严重不足之处是:为了恢复后交叉韧带的固有张力,牵拉线需要拉紧,牵引线收紧后压迫韧带,对韧带产生隔断或切割作用,造成韧带止点连接部的局部血液循环不良和骨折愈合不良,或造成撕脱骨折块的切割而破坏复位后的骨质连续性。
目前临床上的另一种治疗方法是使用袢钢板固定后交叉韧带撕脱骨折块,将局部拉应力转变为压应力,虽然此法可消除牵拉线的切割作用,其仍存在一些明显缺点:第一,袢钢板为钛合金钢性水平设计材料,其设计用途为关节镜下进行前后交叉韧带重建时用于胫骨隧道外口或股骨隧道外口的移植物固定,并不可应用于关节腔内骨折复位固定。对于后交叉韧带撕脱骨折患者,尤其是陈旧性损伤,必须进行骨折创面新鲜化处理,其后果是获得良好骨折愈合的同时,也必然造成骨折创面的不匹配性,那么由于压迫固定所造成的骨折块与胫骨侧创面发生类似于“跷跷板”现象,骨折创面难以完美贴合,加之术后随着膝关节的屈伸活动,会进一步造成骨折块的微动现象,不利于骨折愈合,甚至造成骨折的不愈合。第二,由于袢钢板为不可吸收材料,因此其在术后属于关节腔内的异物,与此同时,牵拉线可能在术后膝关节屈伸过程中发生疲劳断裂,故而关节腔内的袢钢板存在较大成为关节内游离体的风险,一旦术后袢钢板发生游离,将会严重破坏关节软骨等关节腔内结构。第三,术后1年半至2年间需取出关节内固定物,后交叉韧带胫骨侧固定的袢钢板的关节镜下取出难度很大,单纯使用常规的关节镜手术前入路难以观察到此位置的内固定物,必须创建关节镜后内侧入路才可完成内固定物取出术,然而首次骨折复位手术时可能造成袢钢板周围的骨质破坏后增生,包埋袢钢板而使其更加难以被取出,这都会给患者带来额外的二次创伤,二次手术亦会增加患者总的治疗费用。
综上所述,目前临床治疗后交叉韧带撕脱骨折的措施难以达到令人满足的效果,后交叉韧带撕脱骨折成为临床治疗中的难题,特别是目前采用的精准医疗和微创手术中存在的问题得到有效解决,直接影响了微创手术在后交叉韧带撕脱骨折治疗中的应用和推广,有必要尽快解决。
发明内容
本发明所要解决的技术问题是提供一种治疗后交叉韧带撕脱骨折的可吸收固定袢,这种可吸收固定袢可以使骨折块与胫骨创面贴合紧密,加快骨折的愈合,并可避免固定袢两端磨损关节软骨,同时这种可吸收固定袢可以避免术后关节腔内异物留存的风险,解决了二次手术取出内固定物的难题。
解决上述技术问题的技术方案是:
一种治疗后交叉韧带撕脱骨折的可吸收固定袢,固定袢为中间拱起的长条形结构,固定袢的两端为平行相对的固定平板,固定袢的中间为向上的拱起,拱起由压板和弧形板组成,压板为平板,两个弧形板的上端分别连接在压板的两端,两个弧形板的下端分别固定平板圆滑过渡连接,压板的长度大于或等于拱起长度的1/2,弧形板的圆弧直径为4-10mm,在固定袢的拱起两端的固定平板上分别有穿线孔,固定袢的材质为可吸收材料。
上述治疗后交叉韧带撕脱骨折的可吸收固定袢,所述固定袢放置在后交叉韧带撕脱骨折块上方,固定袢中间的拱起覆盖在骨折块的表面,固定袢的板面向下方倾斜40-60度。
上述治疗后交叉韧带撕脱骨折的可吸收固定袢,固定袢的材质为磷酸三钙和聚乳酸合成材料,采用微粒弥散技术均匀混合磷酸三钙和聚乳酸,其中磷酸三钙重量组分占总重量百分比的20-40%,聚乳酸重量组分占总重量百分比的80-60%。
上述治疗后交叉韧带撕脱骨折的可吸收固定袢,固定袢的磷酸三钙重量组分占总重量百分比的30%,聚乳酸重量组分占总重量百分比的70%。
本发明的有益效果是:
本发明的固定袢的中部采用拱起设计,拱起覆盖在骨折块的表面,不仅具有可将拉应力转换成压应力的优势,同时可以消除造成骨折块微动的“跷跷板”现象,使得骨折块与胫骨创面不论在伸直固定位或屈伸活动时,均获得优于现有的袢钢板的固定,使骨折面贴合更加紧密,拱起亦可避免固定袢两端磨损关节软骨的风险;本发明的固定袢材质为磷酸三钙和聚乳酸合成材料,可提供了最佳化的吸收性和强度,并且可在体内完全降解吸收,避免了术后关节腔内异物留存的风险,解决了二次手术取出内固定的难题;本发明的磷酸三钙和聚乳酸合成材料可提供最佳化硬度,降低了术后内固定断裂的风险,同时其完全降解吸收时限为24个月,远远超过骨折固定愈合时间,能够满足作为骨折内固定的固定需求。
本发明结构简单、使用方便、效果极好,能够使后交叉韧带撕脱骨折患者的治疗获得最佳化的预后疗效。
本发明是钢板固定袢的首创,以最简单的设计解决了长期没有得到有效解决的难题,满足了精准医疗和微创手术的要求,对微创手术在后交叉韧带撕脱骨折治疗中的应用和推广提供了有力的科技支持,可以在临床中得到有效应用。

具体实施方式
图1、2显示,本发明的固定袢1为中间拱起的长条形结构,固定袢1的两端为平行相对的固定平板2,固定袢1的中间拱起3由压板5和弧形板6组成,压板5为平板,两个弧形板6的上端分别连接在压板5的两端,两个弧形板6的下端分别固定平板2圆滑过渡连接。采用这样的结构,拱起3覆盖在骨折块6的表面,不仅具有可将拉应力转换成压应力的优势,同时可以消除造成骨折块8微动的“跷跷板”现象,使得骨折块8与胫骨7创面不论在伸直固定位或屈伸活动时,均获得优于现有的袢钢板的固定,使骨折面贴合更加紧密,拱起3亦可避免固定袢1两端磨损关节软骨的风险。
图1、2显示,拱起3的压板5的长度大于或等于拱起3长度的1/2,以有效地覆盖骨折块8。
图1、2显示,拱起3的圆弧直径为4-10mm。在实际制作中,可以制成不同圆弧直径的弧形板6的拱起3,以适应不同大小的骨折块8的压紧要求。本发明的实施例有4种规格,拱起3的长度为4mm、6mm、8mm、10mm,其中压板5的长度分别是2mm、3mm、4mm、5mm,两端的弧形板6的圆弧直径分别是4mm、6mm、8mm、10mm,固定袢1的整体宽度为4mm。
图1、2显示,在固定袢1的拱起3两端的固定平板2上分别有穿线孔4,用于穿过牵拉线10。
图3、4、5显示,固定袢放置在后交叉韧带撕脱骨折块上方,固定袢中间的拱起覆盖在骨折块的表面。
图3、4、5显示,在对固定袢1进行牵拉时,需要将牵拉线10穿过胫骨骨隧道9,胫骨骨隧道9与胫骨平台有50度左右的夹角,因此固定袢1的方向也需要向下方倾斜40-60度,本发明的一个实施例为50度。
本发明的固定袢1的材质为可吸收材料磷酸三钙和聚乳酸合成材料,可提供了最佳化的吸收性和强度,并且可在体内完全降解吸收,避免了术后关节腔内异物留存的风险,解决了二次手术取出内固定的难题。同时磷酸三钙和聚乳酸合成材料可提供最佳化硬度,降低了术后内固定断裂的风险,其完全降解吸收时限为24个月,远远超过骨折固定愈合时间,能够满足作为骨折内固定的固定需求。
本发明的固定袢1的磷酸三钙和聚乳酸合成材料,它由磷酸三钙、聚乳酸组成,其中磷酸三钙重量组分占总重量百分比的20-40%,聚乳酸重量组分占总重量百分比的80-60%。
优选的,本发明的固定袢的磷酸三钙重量组分占总重量百分比的30%,聚乳酸重量组分占总重量百分比的70%。
本发明的使用方法如下:
本发明可联合使用Smith & Nephew 关节镜下测量尺,以确定骨折块8与骨折创面的大小,选择合适型号的可吸收固定袢1。
常规关节镜下探查后交叉韧带撕脱骨折处情况后,胫骨导向器定位自胫骨结节内侧至骨折块8远端钻取一直径5mm骨隧道9,导入连接不可吸收牵拉线10(Fiber Wire高强线)后的可吸收固定袢1,关节镜下扣压固定骨折块8,将牵拉线10从骨隧道9拉出并于骨隧道9远端使用纽扣钢板固定。
Claims (4)
1.一种治疗后交叉韧带撕脱骨折的可吸收固定袢,其特征在于:固定袢(1)为中间拱起的长条形结构,固定袢(1)的两端为平行相对的固定平板(2),固定袢(1)的中间为向上的拱起(3),拱起(3)由压板(5)和弧形板(6)组成,压板(5)为平板,两个弧形板(6)的上端分别连接在压板(5)的两端,两个弧形板(6)的下端分别固定平板(2)圆滑过渡连接,压板(5)的长度大于或等于拱起(3)长度的1/2,弧形板(6)的圆弧直径为4-10mm,在固定袢(1)的拱起(3)两端的固定平板(2)上分别有穿线孔(4),固定袢(1)的材质为可吸收材料。
2.根据权利要求1所述的治疗后交叉韧带撕脱骨折的可吸收固定袢,其特征在于:所述固定袢(1)放置在后交叉韧带撕脱骨折块(8)上方,固定袢(1)中间的拱起(3)覆盖在骨折块(8)的表面,固定袢(1)的板面向下方倾斜40-60度。
3.根据权利要求2所述的治疗后交叉韧带撕脱骨折的可吸收固定袢,其特征在于:固定袢(1)的材质为磷酸三钙和聚乳酸合成材料,采用微粒弥散技术均匀混合磷酸三钙和聚乳酸,其中磷酸三钙重量组分占总重量百分比的20-40%,聚乳酸重量组分占总重量百分比的80-60%。
4.根据权利要求3所述的治疗后交叉韧带撕脱骨折的可吸收固定袢,其特征在于:固定袢的磷酸三钙重量组分占总重量百分比的30%,聚乳酸重量组分占总重量百分比的70%。
