Abstract
本发明公开了一种药物，特别公开了一种治疗寻常型银屑病的中药组合物。该治疗寻常型银屑病的中药组合物，其特殊之处在于：各原料的重量配比（单位g）为：地黄10‑30、牡丹皮10‑20、赤芍10‑20、金银花10‑30、拳参6‑15、全蝎3‑6、苦参10‑20、地榆10‑20、地骨皮10‑20、威灵仙10‑20、白鲜皮10‑20、黄芩10‑20、丹参10‑30、防风10‑15、蝉蜕6‑15、地肤子10‑20、蛇床子10‑20、乌梢蛇3‑6、甘草3‑6。本发明通过清热解毒、祛风止痒、凉血消斑、活血化瘀来扶正祛邪、调节阴阳平衡，达到治疗寻常型银屑病的效果。其疗效显著，大大减轻了患者痛苦，复发率明显下降，提高了患者的生活质量；而且其毒副作用相对较小，病人可安心服用。本发明的中药，疗效好，见效快，无副作用，总有效率达92.0％。
Description
技术领域
本发明主要涉及中药领域，尤其涉及一种治疗寻常型银屑病的中药。
背景技术
银屑病俗称牛皮癣，是一种慢性炎症性皮肤病，病程较长，有易复发倾向，有的病例几乎终生不愈。该病发病以青壮年为主，对患者的身体健康和精神状况影响较大。临床表现以红斑，鳞屑为主，全身均可发病，以头皮，四肢伸侧较为常见，多在冬季加重。
本病的病因病机多种多样，如皮肤失养，风邪外袭；风湿毒邪侵袭，致气血瘀滞；血热壅塞，气机不利，肌肤失养；肝肾亏虚，肌肤失于濡润，外邪侵袭等。
目前治疗银屑病的方法，西医尚无特效疗法，基本以对症治疗、控制症状为主。主要有联合疗法、交替疗法、序贯和间歇疗法等。外用药有维生素D3类似物、糖皮质激素、蒽林、维A酸凝胶和霜剂、焦油类、免疫抑制剂等；内服药有甲氨蝶呤(MTX)、维A酸类、糖皮质激素；环孢素A、他克莫司、霉芬酸酯等免疫抑制剂、抗生素等；物理疗法有紫外线、光化学疗法（PUMA）、宽谱中波紫外线（BB-UVB）疗法、窄谱中波紫外线（NB-UVB）疗法、水疗等。
由于本病是一种慢性复发性疾病，不少患者需要长期医治，而各种疗法都有一定的不良反应，且易产生耐受性，治标不治本，容易反复。传统中医由于具有强调整体观念、协调阴阳、标本兼治、毒副作用小等优点，越来越被人们认可。
发明内容
本发明目的就是为了弥补已有技术的缺陷，提供一种专门治疗寻常型银屑病的中药组合物，它对于治疗银屑病疗效明显，安全方便，副作用小。
本发明是通过以下技术方案实现的：
一种治疗寻常型银屑病的中药组合物，其特征在于：各原料的重量配比（单位g）为:
地黄10-30、牡丹皮10-20、赤芍10-20、金银花10-30、拳参6-15、全蝎3-6、苦参10-20、地榆10-20、地骨皮10-20、威灵仙10-20、白鲜皮10-20、黄芩10-20、丹参10-30、防风10-15、蝉蜕6-15、地肤子10-20、蛇床子10-20、乌梢蛇3-6、甘草3-6。
一种治疗寻常型银屑病的中药组合物，其优选为：各原料的重量配比（单位g）为:
地黄30、牡丹皮15、赤芍15、金银花20、拳参10、全蝎6、苦参15、地榆15、地骨皮15、威灵仙15、白鲜皮15、黄芩15、丹参30、防风12、蝉蜕12、地肤子15、蛇床子15、乌梢蛇5、甘草6。
本发明的配制方法如下：将上述中药用冷水浸泡1小时后煎煮，煎煮水份超过中药2~3cm
，在水沸腾后用文火煎煮50分钟，过滤倒出药液即可。分早晚，一日两次。
具体实施方式：
一种治疗寻常型银屑病的中药组合物，其优选为：其各原料的重量配比（单位g）为: 地黄30、牡丹皮15、赤芍15、金银花20、拳参10、全蝎6、苦参15、地榆15、地骨皮15、威灵仙15、白鲜皮15、黄芩15、丹参30、防风12、蝉蜕12、地肤子15、蛇床子15、乌梢蛇5、甘草6。称取以上对应组的原料组分，按照汤药的常规煎法，加水煮成汤剂服用。分早晚，一日两次。
临床观察：
1 资料与方法
1.1 一般资料
纳入标准：①参照《中药新药临床研究指导原则》、欧阳恒主编的《新编中医皮肤病学》符合寻常型银屑病的中西医诊断标准；②年龄18～65岁；③近１个月内未系统接受抗银屑病治疗。
排除标准：①妊娠、准备妊娠或哺乳期妇女；②关节型、脓疱型、红皮病型银屑病患者；③对中药成分过敏者；④近２周内未服过类固醇药物；１周内未服用维甲酸类药物或外用类固醇制剂；⑤怀疑或确有酒精、药物滥用史者；⑥合并有心血管、脑血管、肝、肾及造血系统等严重原发性疾病、精神病患者；⑦自行退出或未完成整个病例观察而影响判断者。所有病例均为2015年1月～2016年１2月笔者医院皮肤科门诊患者共100例，随机分为治疗组和对照组，其中治疗组50例，男32例，女18例，年龄18~65岁，平均37.6岁，病程３个月~16年，平均6.6年；对照组50例，男26例，女24例，年龄19~65岁，平均35.4岁，病程１个月~12年，平均6.2年。２组患者在年龄、性别、病程上差异无统计学意义（P＞0.05），具有可比性。
1.2 治疗方法
所有患者在治疗前均签署知情同意书。对照组采用口服阿维Ａ胶囊（10mgx30粒/盒，重庆华邦制药股份有限公司）10mg，tid，连用60ｄ，改为维持量10mg，bid，持续用药待皮损进一步消退，逐渐减量停药，总共用药时间３个月。治疗组予以本发明中药组合物，连服３个月，服药期间忌食辛辣刺激之品，禁饮酒。
安全性评价
患者在治疗前及治疗期间每４周和治疗后检查血、尿常规，肝肾功能以及血脂，并
记录其他不良反应。
观察指标与观察方法
采用银屑病皮损面积和严重程度指数（PASI）评分法对２组患者治疗前、后症状改善情况进行评分。包括记录患者皮损面积、红斑程度、浸润肥厚、脱屑严重程度的变化，并记录患者瘙痒程度的变化。
疗效判定标准
通过PASI评分标准对患者的疗效进行评定。疗效指数=（治疗前PASI总分-治疗后PASI总分）／治疗前PASI总分×100%。
临床治愈：疗效指数大于95%；
显效：疗效指数60%~95%；
好转：疗效指数30%~59%；
无效：疗效指数小于30%；
总有效率=（进入统计的临床痊愈例数＋进入统计的显效例数＋进入统计的好转例数）/进入统计的例数×100%。
统计学处理
应用SPSS17.0统计软件，符合方差齐性、正态分布计量资料以x±s表示，比较采用
t检验，等级资料采用２组Ridit分析。P＜0.05差异有统计学意义。
结果
2组临床疗效比较 ２组治疗前PASI差别无统计学意义，２组组内治疗前后及２组治疗后PASI差别均有统计学意义。见表１。

治疗组R均值为0.4145，总体R治疗组均数95%的CI为0.3241，0.4822；对照组Ｒ均值为0.5842，总体Ｒ对照组均数95%的CI为0.5013，0.6716。治疗组与对照组Ｒ均值的95%的CI无重叠，不包括0.5，有统计学意义，且治疗组Ｒ均值小于对照组Ｒ均值，故治疗组疗效优于对照组，即治疗寻常型银屑病采用本发明中药组合物的临床疗效优于口服阿维Ａ胶囊的临床疗效。见表２。

本发明通过清热解毒、祛风止痒、凉血消斑、活血化瘀来扶正祛邪、调节阴阳平衡，达到治疗寻常型银屑病的效果。其疗效显著，大大减轻了患者痛苦，复发率明显下降，提高了患者的生活质量；而且其毒副作用相对较小，病人可安心服用。本发明的中药，疗效好，见效快，无副作用，总有效率达92.0％。
其中地黄、牡丹皮、黄芩凉血清热、消疹散瘀，配白鲜皮、防风、威灵仙共奏凉血祛风之效；赤芍、丹参清热凉血，活血祛瘀；金银花、拳参清热解毒、消痈散结；苦参、地肤子、蛇床子清热利湿，祛风止痒；全蝎息风镇痉、通络止痛、攻毒散结；地榆、地骨皮止血凉血、清热解毒 、凉血除蒸；乌梢蛇、蝉蜕祛风通络；甘草调和诸药之药性。
为证明本发明具有很好的疗效，列举典型病例如下：
病例1.
患者张某，女，34岁，务农，2015年3月18日诊。患病已2年余，累治不效，反复发作。轻时四肢及躯干部有散在红斑，大小不等，重时布满全身，上附白色鳞屑，瘙痒难忍，抓去后基部有出血点，西医确诊为银屑病，伴膝部关节疼痛，口干渴，脉浮滑，舌质稍红，苔薄白。以本发明汤剂调治，连服15剂，每日两次。 4月2日二诊， 红斑有所消退，鳞片明显减少，瘙痒有减。按前方再服15剂，皮疹消退大半，其色变淡，呈散在性分布，瘙痒基本消失，关节疼痛缓解。再服10剂加以巩固，皮疹全部消失，一切均如常人，嘱其注意饮食，不要过于疲劳，不要饮酒及食辛辣之物，至此再未来诊。后随访，已痊愈。

1.一种治疗寻常型银屑病的中药组合物，其特征在于：各原料的重量配比（单位g）为：地黄10-30、牡丹皮10-20、赤芍10-20、金银花10-30、拳参6-15、全蝎3-6、苦参10-20、地榆10-20、地骨皮10-20、威灵仙10-20、白鲜皮10-20、黄芩10-20、丹参10-30、防风10-15、蝉蜕6-15、地肤子10-20、蛇床子10-20、乌梢蛇3-6、甘草3-6。
2.如权利要求1所述的中药组合物，其特征在于：各原料的重量配比（单位g）为：地黄30、牡丹皮15、赤芍15、金银花20、拳参10、全蝎6、苦参15、地榆15、地骨皮15、威灵仙15、白鲜皮15、黄芩15、丹参30、防风12、蝉蜕12、地肤子15、蛇床子15、乌梢蛇5、甘草6。
