技术领域
本发明涉及外科手术医疗器械领域,尤其是涉及一种适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置。
背景技术
腹股沟斜疝是儿童常见的外科疾病之一,诊治原则与成人有很大不同,手术方法与操作技术也有独到之处。目前国内外小儿外科专家认为,儿童腹股沟斜疝是由于先天性鞘状突未闭合引起,这也是手术方式的理论基础,即通过行鞘状突高位结扎即可达到治疗目的,而不再进行腹股沟管的加强修补。在此理论基础下目前常用的手术方式有经腹股沟区小横纹疝囊高位结扎和腹腔镜下疝囊(鞘状突)高位结扎术。国内外目前未能开展腹腔镜技术的医疗机构基本选用经腹股沟区小横纹疝囊高位结扎,该手术方式切口1.5cm,解剖以及分离结扎过程中可能出现结扎疝囊时周围组织暴露不充分,将其一并结扎使睾丸向腹股沟管移位造成医源性隐睾的潜在风险;另外小切口开放手术破坏了腹股沟的解剖结构,在剥离疝囊时可能损伤精索血管、神经和提睾肌。国内腹腔镜下疝囊(鞘状突)高位结扎的方式多采用经脐和脐旁双空钳子辅助下内环口缝合结扎,个别医疗机构自制“疝气针”行单孔操作,但总体因自制“疝气针”技术、经济因素给术者和患儿造成一定的困难。这些缺点至今仍然在影响着儿童腹股沟斜疝手术的疗效。近年来,出现了适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置,普遍采用一个直线型针和一个弧形针配合,或者仅采用弧形针,上述装置对手术医生提出了更高的要求,降低了手术的安全性,现有技术中出现的安全性能较高的高位结扎装置,采用磁性构件实现手术线的勾线打结,但是可操作性差,此外,磁性构件与主体部件固定连接,制作成本大大提高,而且两个不同的部件的连接强度会随着使用时间变长而降低,带来了安全隐患。
发明内容
针对上述技术问题,本发明提供了一种可操作性强、安全系数高、制作成本低,精度更高的适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置。
为了实现上述目的,本发明采取以下技术方案:
一种适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置,包括手柄、挡板和中空的针体以及可以插入针体中的钩针,所述的针体包括上部针体和下部针体,所述的钩针包括螺旋拉环和拉钩,所述的针体内壁上具有供拉钩沿着针体的纵向滑动的横向刻槽Ⅰ,所述的横向刻槽Ⅰ的端部连接有半圆刻槽,所述的半圆刻槽位于针体的内壁,所述的半圆刻槽可以供拉钩在针体内转动,所述的针体的侧壁上开设有容纳孔,所述的下部针体的材料为形状记忆合金,所述的形状记忆合金以镍52%质量百分比,余量为钛的镍钛形状记忆合金为基体,表面覆有仿生纳米材料的二氧化钛膜,二氧化钛膜的厚度为60-70纳米,所述的形状记忆合金的转变温度为39摄氏度,所述的半圆刻槽距离挡板的距离大于容纳孔距离挡板的距离,所述的容纳孔和半圆刻槽之间设置有供拉钩在针体内纵向滑动的横向刻槽Ⅱ,所述的横向刻槽Ⅱ与半圆刻槽和容纳孔相连,所述的形状记忆合金在室温下为直线形,当达到转变温度后下部针体的端部能够变形进入所述的容纳孔中,所述的下部针体的内壁上覆有加热丝,所述的下部针体设置有温度传感器,所述的温度传感器与显示屏相连,所述的显示屏位于手柄上,所述的形状记忆合金进入容纳孔的深度与半圆刻槽的槽底齐平,所述的手柄上设置有加热丝的开关,所述的半圆刻槽、横向刻槽Ⅰ和横向刻槽Ⅱ刻入针体内壁的深度相同,所述的横向刻槽Ⅱ为一斜坡,该斜坡由半圆刻槽向手柄的方向倾斜。
所述的二氧化钛膜的厚度为60纳米。
所述的形状记忆合金的制备方法为:选择高温稳定的坩埚,加入质量百分比为52%的镍和余量的钛,得到具有稳定转变温度的镍钛形状记忆合金,然后采取分段控制、真空熔炼和惰性气体保护的熔炼工艺,形成浇筑,将二氧化钛在镍钛形状记忆合金的表面进行覆膜,控制覆膜层的厚度为60-70纳米,得到均匀排列、表面光洁、无裂纹的形状记忆合金。所述的二氧化钛膜通过离子注入的方式进行覆膜。
所述的下部针体端部的外外径尺寸为2.2mm,内径尺寸为1.8mm。
所述的拉钩为鱼钩状。
本发明采用以上技术方案具有以下有益效果:
(1)通过二氧化钛覆膜的设置,大大降低了镍对身体的有害性,减少了随身体的伤害,提高了医疗器械与人体的生物相容性,大大提高了装置的安全性能;
(2)半圆刻槽、横向刻槽Ⅰ和横向刻槽Ⅱ以及与容纳孔的配合设置,大大提高了对手术线进行钩线的准确性和操作性,大大降低了对人体的伤害,使得钩手术线更加易于操作,提高了手术医生的手术效率,节省手术时间;
(3)形状记忆合金配合温度传感器和显示屏的设置,减少了手术针的使用数量,使得形状记忆合金发生变形时可控可见,便于医生根据病人疝囊的大小和形状,调整针体的走向,提高手术的成功率;
(4)横向刻槽Ⅱ的斜坡设置,使得钩针在钩上手术线后,可以沿着横向刻槽Ⅱ的坡度滑动一定的距离,使得手术线在钩针上钩的更加牢固。

形状记忆合金的制备方法为:选择高温稳定的坩埚,加入质量百分比为52%的镍和余量的钛,得到具有稳定转变温度的镍钛形状记忆合金,然后采取分段控制、真空熔炼和惰性气体保护的熔炼工艺,形成浇筑,将二氧化钛在镍钛形状记忆合金的表面进行覆膜,控制覆膜层的厚度为60-70纳米,得到均匀排列、表面光洁、无裂纹的形状记忆合金。二氧化钛膜通过离子注入的方式进行覆膜。
下部针体4端部的外径尺寸为2.2mm,内径尺寸为1.8mm。
拉钩6为鱼钩状。
本发明的装置的使用过程为:
经脐孔植入腹腔镜进入腹腔进行腹腔检查,了解内环口的大小,并确定内环口体表的投影处的位置,在腹壁体表投影位置做切口,深达深筋膜,将手术线穿入中空的针体中,并经过下部针体4的端部引出,将针体的端部刺入置入腹腔约5厘米,将加热丝11的电源接通,然后按住加热丝的开关12,启动加热丝,并观察显示屏13上的温度显示,当温度到达转变温度后,控制下部针体4自动弯曲绕过疝内环扣,使得下部针体4的端部弯曲进入容纳孔9中,将钩针放入针体中,拉钩6卡入横向刻槽Ⅰ7,沿着横向刻槽Ⅰ7钩针进入针体内,到达横向刻槽Ⅰ7的末端后,转动钩针使得拉钩6沿着半圆刻槽8在针体内进行转动,转动到横向刻槽Ⅱ10和半圆刻槽8的交接处时,将拉钩6卡着横向刻槽Ⅱ10的坡度滑动一定距离后,钩针由针体的空腔向外拉出,当拉钩6到达容纳孔9的位置处,将容纳孔9中的手术线拉出,关闭加热丝的开关14,当显示屏13显示的温度低于形状记忆合金的转变温度后,下部针体4变成直线,将针体取出进行结扎,完成手术。
上面结合附图对本发明进行了实例性的描述,显然本发明具体实现并不受上述方式的限制,只要采用了本发明的方法构思和技术方案进行的各种改进,或未经改进直接应用于其他场合的,均在本发明的保护范围内。
Claims (6)
1.一种适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置,包括手柄(1)、挡板(2)和中空的针体以及可以插入针体中的钩针,所述的针体包括上部针体(3)和下部针体(4),所述的钩针包括螺旋拉环(5)和拉钩(6),其特征在于:其中所述的针体内壁上具有供拉钩沿着针体的纵向滑动的横向刻槽Ⅰ(7),所述的横向刻槽Ⅰ(7)的端部连接有半圆刻槽(8),所述的半圆刻槽(8)位于针体的内壁,所述的半圆刻槽(8)用于供拉钩(6)在针体内转动,所述的针体的侧壁上开设有容纳孔(9),所述的下部针体(4)的材料为形状记忆合金,所述的形状记忆合金以镍52%质量百分比、余量为钛的镍钛形状记忆合金为基体,表面覆有仿生纳米材料的二氧化钛膜,二氧化钛膜的厚度为60-70纳米,所述的形状记忆合金的转变温度为39摄氏度,所述的半圆刻槽(8)距离挡板(2)的距离大于容纳孔(9)距离挡板的距离,所述的容纳孔(9)和半圆刻槽(8)之间设置有供拉钩(6)在针体内纵向滑动的横向刻槽Ⅱ(10),所述的横向刻槽Ⅱ(10)与半圆刻槽(8)和容纳孔(9)相连,所述的形状记忆合金在室温下为直线形,当达到转变温度后下部针体(4)的端部能够变形进入所述的容纳孔(9)中,所述的下部针体(4)的内壁上覆有加热丝(11),所述的下部针体(4)设置有温度传感器(12),所述的温度传感器(12)与显示屏(13)相连,所述的显示屏(13)位于手柄(1)上,所述的形状记忆合金进入容纳孔(9)的深度与半圆刻槽(8)的槽底齐平,所述的手柄(1)上设置有加热丝的开关(14),所述的半圆刻槽(8)、横向刻槽Ⅰ(7)和横向刻槽Ⅱ(10)刻入针体内壁的深度相同,所述的横向刻槽Ⅱ(10)为一斜坡,该斜坡由半圆刻槽(8)向手柄(1)的方向倾斜。
2.根据权利要求1所述的适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置,其特征在于:其中所述的二氧化钛膜的厚度为60纳米。
3.根据权利要求1所述的适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置,其特征在于:其中所述的形状记忆合金的制备方法为:选择高温稳定的坩埚,加入质量百分比为52%的镍和余量的钛,得到具有稳定转变温度的镍钛形状记忆合金,然后采取分段控制、真空熔炼和惰性气体保护的熔炼工艺,形成浇筑,将二氧化钛在镍钛形状记忆合金的表面进行覆膜,控制覆膜层的厚度为60-70纳米,得到均匀排列、表面光洁、无裂纹的形状记忆合金。
4.根据权利要求3所述的适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置,其特征在于:其中所述的二氧化钛膜通过离子注入的方式进行覆膜。
5.根据权利要求1所述的适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置,其特征在于:其中所述的下部针体(4)端部的外径尺寸为2.2mm,内径尺寸为1.8mm。
6.根据权利要求1所述的适用于腹腔镜下腹股沟斜疝疝囊高位结扎的装置,其特征在于:其中所述的拉钩(6)为鱼钩状。
