技术领域
本发明涉及一种使用生物聚合物的伤口敷料的制备方法及利用其方法制备的使用生物聚合物的伤口敷料。根据本发明制备的伤口敷料可具有抑制伤口感染和防止粘连的优异效果。
背景技术
术后伤口感染是手术患者最常见的院内感染,占所有被感染病人的38%,根据美国疾病预防控制中心和保存中心(CDC)全国医院感染监控(NNIS)系统报告,手术部位感染(SSI)是第三个最常见的院内感染,据住院患者统计,所述感染占全部医院感染的14~16%。另一份报告显示,美国每年执行2700万例手术,其中发生SSI有67.5万例,并且欧洲每年执行3000万例手术,其中发生SSI有90万例左右。SSI对发病率和恢复期是一个不利因素,对于患者、施术者和整体医疗保健行业会造成严重的问题,导致住院期间延长,由此带来医疗保险预算的增加。由于外科手术技法和无菌技术多年来发展,许多国家的SSI发生率已显著降低,但是,这种进展引起到其他问题,例如耐药菌株增加和高风险手术患者增加。尤其,糖尿病或肥胖患者的手术显示增加了SSI的风险,并且在这一领域发病率的增加也是有关从事人员主要关注的焦点。世界卫生组织预计,糖尿病患者的数量将会从2010年的1.71亿增加到3.66亿,从而要对SSI引起格外注意。
在为防止SSI所做的努力中,除了严格的无菌环境和此基础上的手术技术应用外,在预防方面上,可以考虑使用抗生素。然而,抗生素长期的直接血管内给药可导致抗生素耐药性及所得到的毒性风险,因此使用抗生素需要谨慎的考虑。并且,在专利号为1,321,818和专利公开号为2014/0038915A1的美国专利中公开了以局部使用作为目的的产品(和)如可吸收的庆大霉素胶原植入物(GCI),它的出现可以帮助解决这种问题。
与直接血管内注射相比,庆大霉素的局部使用可以在伤口部位形成显著更高浓度,反而可致庆大霉素在血液中的浓度相对降低,因此与血管内注射相比,可以减少副作用或毒性的风险。此外,通过这种方法,可以期待避免抗生素长期低浓度给药引起的耐性问题、以及高浓度庆大霉素起到与广谱抗生素同样的作用。实际上,已经确认局部高浓度治疗导致庆大霉素耐药菌株死亡,据报告显示,在诸多领域的外科手术例(GI,cardiovascularand orthopeadic surgery)中,用于预防或治疗目的的这种产品的使用,会减少SSI风险。
另外,术后粘连是正常彼此分开的周围器官或组织之间的连接,其是由在包括由炎症、切口、擦伤或外科手术等引起的创伤的伤口愈合过程中纤维组织或外渗物的过度生成以及血液的凝固而产生的。在任一种外科手术后可能会引起粘连,并且由于在手术后愈合过程中在手术部位周围的器官或组织之间的粘连,可能会引起严重的临床后遗症。
据报道,通常手术后器官粘连的发生率的范围是55%~93%(Ann.RoyalColl.Surg.Engl.,75,147-153,1993)。腹部手术后发生粘连较多。尽管这些粘连中的一些可能发生自发分解,但是在大多数的情况下甚至是在伤口愈合以后仍然存在有粘连,这可能导致多种后遗症。后遗症的种类是多种多样的。根据美国的统计数据,已知在随后的手术中所述术后粘连(作为主要的症状)伴随有49%至74%的肠闭塞、15%至20%的不育症、20%至50%的慢性骨盆痛、和19%的肠穿孔(Eur.J.Surg.,Suppl577,32-39,1997)。
腹膜内粘连的形成机制已在Granger发表的论文中有详细的描述(Infert.Reprod.Med.Clin.North Am.,5:3,391-404,1994)。Granger认为在手术后产生的渗出物中由凝血过程中产生的纤维蛋白引起粘连。炎性渗出物富含纤维蛋白,所述纤维蛋白在伤口表面形成血块。当纤维蛋白降解时,间皮再生,这通常导致伤口的愈合。纤维蛋白的降解或纤维蛋白溶解依赖于纤维蛋白溶酶原转变为纤维蛋白溶酶(其为纤维蛋白溶解酶),并且该反应由存在于间皮和基础基质中的组织型纤溶酶原激活物(tPA)所促进。然而,如果不发生纤维蛋白溶解,则炎性细胞和成纤维细胞渗透到所述纤维蛋白基质中,导致组织粘连。在这种方式中,通过一系列的纤维蛋白生成机制和纤维蛋白溶解机制而发生粘连,并且这两个机制之间的关系是复杂的且与伤口的愈合过程是密切相关的。
作为预防所述粘连的多种方法之一,已针对预防相邻组织之间粘连形成的抗粘连剂进行深入研究,所述抗粘连剂是通过在组织的伤口愈合过程中采用屏障形成物理屏障来进行预防所述粘连,这与表面活性剂的作用相似。这些屏障所采用的抗粘连屏障可根据它们的类型大致地分为两类:一类是包括薄膜型、非交织型和海绵型的膜型屏障,另一类是包括凝胶型的溶液型屏障。
所述膜型抗粘连材料的实施例包括氧化-再生纤维素、膨体聚四氟乙烯(以下称为“ePTFE”)、由改性透明质酸、羧甲基纤维素钠和化学改性剂等组成的膜剂。所述溶液型抗粘连材料的实施例包括乳酸盐林格溶液、右旋糖酐-70溶液、肝素溶液、羧甲基纤维素钠溶液、透明质酸溶液、硫酸软骨素溶液、聚乙二醇溶液、泊洛沙姆溶液等。在这些溶液型抗粘连材料中,乳酸盐林格溶液、右旋糖酐-70溶液、肝素溶液等具有主要的机制,即在腹膜的愈合过程中诱导纤维蛋白覆盖的表面相互分离。尽管它们是已被用于通过使组织相互分离而抑制粘连的制剂,但是由于它们能被腹膜快速吸收,因此不能得到令人满意的抗粘连效果(Am.Surg.,63,775-777,1983)。同时,由于聚乙二醇等在体内不能降解,仅可采用当吸收时可通过代谢途径排出的低分子量材料。然而,使用所述低分子量材料会导致过快吸收,使得其不能在较长的时间内作为有效的屏障防止粘连。
同时,在专利号为4,141,973的美国专利中公开的透明质酸是由交替连接的β-D-N-乙酰葡糖胺和β-D-葡萄糖醛酸组成的线性大分子多糖,并且已知即使当其被植入或注入体内时,其仍具有优异的生物相容性。然而,还由于在相对较短的时间内在体内的分解和吸收,其在作为抗粘连剂的性能方面仍存在限制。为了尝试改善这些缺点,专利号为6,387,413Β1的美国专利公开了通过添加例如为羧甲基纤维素的聚合物化合物而制备透明质酸凝胶组合物,以实现对透明质酸凝胶在物理性质方面进行补充的目的。迄今为止,由此开发的材料显示出了预防粘连的潜力,但是由于主要采用化学交联方法,仍然存在有与不便于除去交联剂或添加剂和方法复杂相关的问题以及毒性和安全性的问题。
另外,胶原蛋白是世界上最丰富的蛋白之一,几乎可从所有的生命体中提取得到。在组织工程中主要采用的胶原蛋白是从牛的皮或肌肉或者猪皮中提取的。然而,据报道胶原蛋白本身是一种参与免疫应答的蛋白质,通过除去表现主要免疫功能的端肽进行免疫应答最小化的胶原蛋白还具有螺旋结构,但胶原蛋白表面的氨基酸序列等可能会引起免疫应答。尤其,针对牛中提取的胶原蛋白来说,没有充分的科学依据证明百分百可以确保疯牛病(BSE)因子的安全性。
发明内容
技术问题
作为为开发一种可以替代高价胶原材料(其在庆大霉素胶原植入物中起到载体作用)的物质、以及为开发一种多功能性材料(其实现手术后伤口愈合和防止粘连的目的)而进行深入研究的结果,本发明的发明人发现,通过采用以透明质酸为基础的生物聚合物,可以制备具有抑制伤口感染和防止粘连的效果,并具有柔和质感和均匀表面的伤口敷料。
技术方案
根据第一实施例,公开一种伤口敷料的制备方法,其包括以下步骤:使用抗生素和透明质酸或其盐来制备溶液;将所制备的溶液在旋转下进行冷冻;以及将所述冷冻溶液进行干燥。
根据第二实施例,公开一种伤口敷料的制备方法,其包括以下步骤:i)使用抗生素和透明质酸或其盐来制备溶液;ii)将所制备的溶液进行冷冻;iii)将所述冷冻溶液进行解冻;iv)将所述解冻溶液重新进行冷冻;以及v)将所述重新冷冻的溶液进行干燥。在所述实施例中,步骤ii)至iii)可重复进行2~6次。
根据第三实施例,公开一种伤口敷料的制备方法,其包括以下步骤:i)使用抗生素和透明质酸或其盐来制备溶液;ii)将所制备的溶液低温放置8小时以上;iii)将所述低温放置溶液进行冷冻;以及iv)将所述冷冻溶液进行干燥。根据所述实施例,低温可以为0~10℃。
技术效果
根据本发明的方法制备的透明质酸伤口敷料具有柔和质感和均匀表面形态,因此可以替代胶原蛋白。并且,根据本发明的方法制备的透明质酸伤口敷料具有抑制伤口感染和防止粘连的优异效果。

本发明的目的是提供一种使用透明质酸作为生物聚合物来制备伤口敷料的方法及由此制备的伤口敷料。
本说明书中所使用的术语“透明质酸(Hyaluronic acid)”是指以N-乙酰葡糖胺(N-acetyl glucosamin)和葡萄糖醛栓(glucuronic acid)为基本单位组成的线性聚合物,发现它在分子量达数百万的几乎所有的生物有机体中结构相同,主要作为细胞外基质(Extracellular Matrix)的成分。由于透明质酸不管物种之间差异都具有相同的结构,因此它被称为是一个没有免疫应答的聚合物,与提取源并不相关。并且迄今为止,它被使用在诸如退行性关节炎、白内障、皱纹改善、药物递送(drug delivery)、干细胞领域的支架载体、化妆品的保湿维持成分等许多领域,是一种安全的天然多糖类。
最初,透明质酸是从家畜中提取的,如牛或鸡,但如今通过微生物的发酵而生产。因此,可确保不受对人体有害的如疯牛病或禽流感等因子的感染,并且可替代相对高价的胶原蛋白。
然而,当使用透明质酸制造产品时,例如一般的垫、贴片和片材,在执行冷冻干燥工序后疏松易碎,并且在冷冻过程中生成带纹理或具有粗糙表面的干燥物,由此存在问题,如不可能制备与现有胶原蛋白产品质感相同的产品。
根据一个实施例,提供一种伤口敷料的制备方法,其包括以下步骤:使用抗生素和透明质酸或其盐来制备溶液;将所制备的溶液在旋转下进行冷冻;以及将所述冷冻溶液进行干燥。
在所述实施例中,所述抗生素可以是至少一种选自头孢菌素、β-内酰胺、氨基糖苷类、大环内酯类、喹诺酮类和四环素类的抗生素,但并不限定于此。
在所述实施例中,所述透明质酸盐可以是至少一种选自透明质酸钠、透明质酸钾、透明质酸钙、透明质酸镁、透明质酸锌、透明质酸钴和透明质酸四丁基铵的透明质酸盐,但并不限定于此。
在所述实施例中,所述透明质酸的分子量可以为10,000~3,000,000道尔顿。
在所述实施例中,所述透明质酸的浓度可以为0.1~5%(w/v)。
在所述实施例中,所述抗生素与透明质酸的浓度比为2:1至1:10。
在所述实施例中,所述溶液的pH值可以为4.0~8.0。
根据另一实施例,公开一种伤口敷料的制备方法,其包括以下步骤:i)使用抗生素和透明质酸或其盐来制备溶液;ii)将所制备的溶液进行冷冻;iii)将所述冷冻溶液进行解冻;iv)将所述解冻溶液重新进行冷冻;以及v)将所述重新冷冻的溶液进行干燥。
在所述实施例中,步骤ii)至iii)可重复进行2~6次。
在所述实施例中,所述抗生素可以是至少一种选自头孢菌素、β-内酰胺、氨基糖苷类、大环内酯类、喹诺酮类和四环素类的抗生素,但并不限定于此。
在所述实施例中,所述透明质酸盐可以是至少一种选自透明质酸钠、透明质酸钾、透明质酸钙、透明质酸镁、透明质酸锌、透明质酸钴和透明质酸四丁基铵的透明质酸盐,但并不限定于此。
在所述实施例中,所述透明质酸的分子量可以为10,000~3,000,000道尔顿。
在所述实施例中,所述透明质酸的浓度可以为0.1~5%(w/v)。
在所述实施例中,所述抗生素与透明质酸的浓度比为2:1至1:10。
在所述实施例中,所述溶液的pH值可以为4.0~8.0。
根据另一实施例,提供一种伤口敷料的制备方法,其包括以下步骤:i)使用抗生素和透明质酸或其盐来制备溶液;ii)将所制备的溶液低温放置8小时以上;iii)将所述低温放置溶液进行冷冻;以及iv)将所述冷冻溶液进行干燥。
在所述实施例中,所述低温可以为0~10℃。
在所述实施例中,所述抗生素可以是至少一种选自头孢菌素、β-内酰胺、氨基糖苷类、大环内酯类、喹诺酮类和四环素类的抗生素,但并不限定于此。
在所述实施例中,所述透明质酸盐可以是至少一种选自透明质酸钠、透明质酸钾、透明质酸钙、透明质酸镁、透明质酸锌、透明质酸钴和透明质酸四丁基铵的透明质酸盐,但并不限定于此。
在所述实施例中,所述透明质酸的分子量可以为10,000~3,000,000道尔顿。
在所述实施例中,所述透明质酸的浓度可以为0.1~5%(w/v)。
在所述实施例中,所述抗生素与透明质酸的浓度比为2:1至1:10。
在所述实施例中,所述溶液的pH值可以为4.0~8.0。
根据本发明的方法制备的透明质酸伤口敷料具有柔和质感和均匀型表面,因而有望替代胶原蛋白。
具体实施方式
下面,根据实施例更详细地说明本发明。这些实施例仅仅是用于具体说明本发明,本发明的范围不局限于这些实施例,这对于本发明所属的的普通技术人员来说是显而易见的。
<实施例>
实施例1:制备含有庆大霉素的透明质酸伤口敷料(旋转式冷冻)
将0.6%的庆大霉素(Gentamicin)和0.9%的NaCl溶解在0.3%(w/v)透明质酸钠溶液中,用稀NaOH溶液调节pH至6.5。然后,将该溶液倒入圆筒形烧杯浸渍于-20℃水池中,在100rpm下旋转并进行冷冻。冷冻结束后,使用冷冻干燥机干燥72小时(图1)。其结果,从图1可知,获得了柔软及表面光滑的伤口敷料。
实施例2:制备含有庆大霉素的透明质酸伤口敷料(旋转式冷冻)
通过与上述实施例1相同的方法制备含有庆大霉素的透明质酸伤口敷料(图2),不同的是,所加入的透明质酸钠的浓度为0.5%(w/v)。其结果,从图2可知,获得了柔软及表面光滑的伤口敷料。
实施例3:制备含有庆大霉素的透明质酸伤口敷料(旋转式冷冻)
通过与上述实施例1相同的方法制备含有庆大霉素的透明质酸伤口敷料(图3),不同的是,所加入的透明质酸钠的浓度为0.8%(w/v)。其结果,从图3可知,获得了柔软及表面光滑的伤口敷料。
实施例4:制备含有庆大霉素的透明质酸伤口敷料(冷冻/解冻)
将0.6%的庆大霉素和0.9%的NaCl溶解在0.8%(w/v)透明质酸钠溶液中,用稀NaOH溶液调节pH至6.5。然后,取50ml该溶液放入正方形培养皿,-20℃下进行第一次冷冻5小时,然后在室温下进行解冻。解冻结束后,在与第一次冷冻温度相同的条件下进行第二次冷冻5小时,然后在冷冻干燥机中干燥72小时(图4)。其结果,从图4可知,获得了柔软及表面光滑的伤口敷料。
实施例5:制备含有庆大霉素的透明质酸伤口敷料(低温放置)
将0.6%的庆大霉素和0.9%的NaCl溶解在1.0%(w/v)透明质酸钠溶液中,用稀NaOH溶液调节pH至6.5。然后,取50ml该溶液放入培养皿,置于10℃环境下放置8小时,在-20℃下进行冷冻后,在冷冻干燥机中干燥72小时(图5)。其结果,从图5可知,获得了柔软及表面光滑的伤口敷料。
比较例1:制备含有庆大霉素的透明质酸伤口敷料(通常的方法)
将0.6%的庆大霉素和0.9%的NaCl溶解在0.8%(w/v)透明质酸钠溶液中,用稀NaOH溶液调节pH至6.5。取50ml该溶液放入正方形培养皿,在-20℃下冷冻5小时后(图6),在冷冻干燥机中干燥72小时(图7)。
从图6和图7可知,冷冻溶液表面生成纹理,或者表面产生不均匀,由此确认干燥物在使用过程中易碎或对伤口部位使用困难。
比较例2:制备含有庆大霉素的透明质酸伤口敷料Ⅱ
将0.6%的庆大霉素和0.9%的NaCl溶解在0.8%(w/v)透明质酸钠溶液中,没有调节pH,并取50ml该溶液放入正方形培养皿,在-20℃下进行第一次冷冻5小时后,将所述培养皿置于室温下进行解冻。然后,所述解冻结束后,在与第一次冷冻温度相同的条件下进行第二次冷冻5小时,其后在冷冻干燥机中干燥72小时(图8)。其结果,从图8可知,确认了伤口敷料表面不均匀,且易碎。
<实验实施例>
实验实施例1:在大鼠盲肠/腹腔壁磨损模型中进行抗粘连试验
采用大鼠盲肠/腹腔壁磨损模型评价上述实施例中制备的伤口敷料的抗粘连性能。作为试验动物,采用7周Sparague-Dawley雄性大鼠(SLC,日本)并对其进行分组(n=5/组)。为了诱导粘连,通过腹腔注射Ketamin·KCL(0.lml/100g)对试验动物进行麻醉,然后腹部剃毛并用70%乙醇消毒,其后在腹部的中线做4~5cm切口。从切开的部位取出盲肠,用灭菌的纱布摩擦所述盲肠的浆膜(1.2cm×1.2cm)直到发生出血,采用试剂勺使相对的腹膜损伤(1.2cm×1.2cm)。用5-0尼龙缝线固定与所述摩擦损伤区域1cm远的两个点,使得两个损伤的表面接触,从而促使粘连的形成。
将生理盐水注入空白对照组。对于实验组,将通过实施例1~5以及比较例1和2中制备的敷料(1cm×1cm)敷贴于磨损部位,然后,缝合腹膜和皮肤。在手术结束后,在一周内随意给动物喂食饲料和水,然后处死,并评价粘连。采用粘连评价系统,得到的分数计算总和以及求平均值(Am.J.0bstet.Gynecol.,146,88-92,1983)。结果示于表1。
根据以下分为O至5等级的标准评价粘连的程度(O:无粘连,1:一个薄膜状粘连,2:两个或两个以上薄膜状粘连,3:病灶局部厚粘连,4:片状局部粘连,5:伴随有血管形成或一个或一个以上片状密集粘连的非常厚的粘连)。
根据以下分为1至4等级的标准评价粘连的强度(1:容易被非常弱的力释放的薄膜状的粘连,2:需要适当的力来释放粘连,3:采用相当大的压力释放的粘连,4:难于释放的或者需要非常高的压力来释放的非常强的粘连)。

如表1所示,与对照组相比较,根据本发明的制备方法制备的实施例1至实施例5的敷料使用组显示出显著的组织粘连减少。这也可从图9和图10中确认得到,图9示出使用对照组敷料的粘连结果照片,图10示出使用实施例5的敷料的粘连结果照片。
另外,与对照组相比较,根据通常的制备方法制备的比较例1和2的敷料使用组显示出轻微的组织粘连减少效果,但由于敷料的组织不均匀,因而确认到与实施例1至实施例5的敷料使用组相比较,粘连减少率明显较低。
实验实施例2:在小鼠肠道感染模型中进行抑制感染试验
采用Balb/C雄性小鼠肠道感染模型评价上述实施例5中制备的伤口敷料的抑制感染性能。抑制感染试验组分为Control试验组、CLP试验组、以及实施例5的伤口敷料使用组,每组使用10只Balb/C雄性小鼠(其中,Control试验组使用6只),并将所述伤口敷料(1×1cm2)插入在小鼠体内。具体地说,试验前一天将小鼠腹部毛剃除,试验当天通过腹腔注射麻醉剂阿佛丁(300mg/kg)对小鼠进行麻醉大约20分钟。轮流使用聚维酮和70%乙醇,将被麻醉的小鼠腹部消毒3次。然后,在腹部离中线靠左1cm的部位做2~3cm切口。从切开的部位取出盲肠,用黑丝(Black silk)扎好距底部3/4处后,使用21G注射针穿出一个孔,从盲肠中挤出少量的内容物。对于CLP试验组,将盲肠重新放回腹部后进行缝合。对于实施例5中制备的伤口敷料使用组,将盲肠放回腹部后,在腹中放入伤口敷料(1×1cm2)后进行缝合。关于感染菌数量,通过以下方法进行测定:从手术结束6小时和12小时后的小鼠中采取腹中产生的腹水,将其放入LB Plate中培养大约18小时,然后测定Colony数量。结果示于图11。
如图11所示,对于根据本发明的制备方法制备的实施例5的伤口敷料使用组来说,当培养手术结束6小时后采取的腹水时,没有生成菌落(colony),相反地,在CLP试验组中测出1.5×103cfu/ml。对于手术结束12小时后的腹水,确认到:在Control试验组和CLP试验组中,分别测出1.5×103cfu/ml和1.5×107cfu/ml,相反地,在根据本发明的制备方法制备的实施例5的伤口敷料使用组中没有生成菌落。

Claims (5)
1.一种用于抑制伤口感染和防止粘连的具有均匀表面的敷料的制备方法,其包括以下步骤:
i)使用抗生素和透明质酸或其盐来制备pH为4.0~8.0的溶液;
ii)将所制备的溶液在0~10℃放置8小时以上;
iii)将所述低温放置的溶液进行冷冻;以及
iv)将所述冷冻溶液进行干燥。
2.根据权利要求1所述的用于抑制伤口感染和防止粘连的具有均匀表面的敷料的制备方法,其中,所述透明质酸盐是至少一种选自透明质酸钠、透明质酸钾、透明质酸钙、透明质酸镁、透明质酸锌、透明质酸钴和透明质酸四丁基铵的透明质酸盐。
3.根据权利要求1所述的用于抑制伤口感染和防止粘连的具有均匀表面的敷料的制备方法,其中,所述透明质酸的分子量为10,000~3,000,000道尔顿。
4.根据权利要求1所述的用于抑制伤口感染和防止粘连的具有均匀表面的敷料的制备方法,其中,所述透明质酸的浓度为0.1~5%(w/v)。
5.根据权利要求1所述的用于抑制伤口感染和防止粘连的具有均匀表面的敷料的制备方法,其中,所述抗生素是至少一种选自头孢菌素系、β-内酰胺系、氨基糖苷系、大环内酯系、喹诺酮系和四环素系的抗生素。
