Abstract
本发明涉及治疗乳腺小叶增生的药物组合物及其制备方法。该药物组合物是由如下重量份的中药原料制备而成的:白背风、三白草、风尾草、蛇莓、墨旱莲、益母草、仙鹤草、小肠风、当归、川芎、白芍、香附、巴戟天、半边莲、半枝莲、白英、鸡血藤、黑肉风。本发明药物组合物能有效治疗肝气郁结所致乳腺增生。

本发明涉及治疗的中药领域,具体涉及治疗乳腺小叶增生的药物组合物及其制备方法。
背景技术
乳腺增生病,中医学称之为“乳癖”,是乳腺组织的良性增生性疾病,其发病率占育龄妇女的40%,占全部乳腺疾病的75%,是最常见的乳腺疾病。随着社会环境、自然环境的变化,生活节奏的加快,人们竞争意识的增强以及饮食结构的改变,乳腺增生病的发病率有逐年上升的趋势。其发病年龄多在25岁~45岁,社会地位高、受教育程度高、初潮早、大龄初孕和绝经迟的妇女为本病高发人群。本病主要表现为乳房胀痛和乳房结块,并多随月经周期或情志改变而变化。
乳腺小叶增生是乳腺增生性疾病中最为常见的一种非肿瘤、非炎症性的增生性病变,均占乳房疾病中的70%左右,可发生于青春期以后的任何年龄的妇女。
引起乳腺小叶增生的原因很多,但主要与内分泌失调或精神情志有密切关系。育龄期妇女的乳腺受卵巢内分泌所控制,一旦卵巢功能受到某种因素的影响,例如情绪不稳定、心情不舒畅、过度劳累、性生活不和谐、生活环境变迁,或者过食含有激素的滋补品和长期使用含有激素成分的化妆品等等,均可影响人体内雌孕激素分泌的比例失调或分泌节律紊乱而引起乳腺组织增生。
乳腺小叶增生在临床上主要表现为:月经来潮前5—7天,乳房胀满疼痛,月经来潮乳房胀痛缓解,乃至消失,待下次月经来潮前又出现周期性的变化。
中医认为,此病发病原因多与脏腑机能失调、气血失和有关,病变脏腑责之肝脾,尤其是脾土虚弱之人 或过食辛辣肥甘厚味,损伤脾土,而致脾土运化功能失常,聚湿为痰或天生性格内向,情绪压抑,好生闷气或性情急躁、动则易怒 或因七情所伤,忧思过度,而致肝失疏泄,郁而成痰等,均可导致痰湿结聚,气血凝滞而形成肿块。
乳腺疾病虽然看似不严重,但是却拖不起。因为乳腺增生严重危害到女性朋友的身体健康,患了乳腺增生病以后有相当多的患者重视程度不够,迟迟不就诊或只求缓解乳痛症状,而意识不到乳腺增生病的潜在危险——属于慢性病,若不及时治疗,乳腺易产生癌变,部分乳腺增生长期迁延不愈,会发生乳腺良性肿瘤或发生恶性病变。
中药治疗
1、减压法:中医以为乳腺小叶增生发生与情至不畅有关。长时期情志不畅,不仅可导致乳腺小叶增生,还可诱发癌变的可能。有资料报告,乳腺小叶增生患者乳腺癌的发病率高出正常人群5倍。减压法的具体做法为,在内心不畅时不要反烦恼闷在心里,要用述说、呐喊、唱歌等方式抒发出来。此法虽不能立刻消除增生的乳腺,却能有力地切断其恶变的途径。

白背风20-40份、三白草20-40份、风尾草20-40份、蛇莓20-40份、墨旱莲20-40份、益母草20-40份、仙鹤草20-40份、小肠风15-25份、当归15-25份、川芎15-25份、白芍15-25份、香附15-25份、巴戟天15-25份、半边莲15-25份、半枝莲15-25份、白英15-25份、鸡血藤15-25份、黑肉风15-25份;
优选地,是由如下重量份的原料制备而成的:
白背风30份、三白草30份、风尾草30份、蛇莓30份、墨旱莲30份、益母草30份、仙鹤草30份、小肠风20份、当归20份、川芎20份、白芍20份、香附20份、巴戟天20份、半边莲20份、半枝莲20份、白英20份、鸡血藤20份、黑肉风20份。

白背风:狭叶醉鱼草Buddleia asiatica Lour.的根、叶、花。【性味】味辛、苦,性温。【功能与主治】有小毒。祛风活络,散瘀消肿,行气止痛,化痰止咳,驳骨,止血,除湿止痒,祛腐生新。
三白草:为三白草科植物三白草Saururus chinensis (Lour.)Baill.的干燥根茎或全草。根茎秋季采挖;全草全年均可采挖,洗净,晒干。【性味】 甘,辛,寒。【归经】 归肺、膀胱经。【功能主治】 清热解毒,利尿消肿。用于小便不利,淋沥涩痛,白带,尿路感染,肾炎水肿;外治疮疡肿毒,湿疹。
风尾草:又名鸡脚草、金鸡尾、井口边草、井边凤尾、井栏草、凤尾蕨、五指草。本品为凤尾蕨科植物凤尾草Pteris multifida Poir.的全草。四季可采,洗净,鲜用或晒干用。【性味】 淡、微苦,凉。【功能主治】 清热利湿,解毒止痢,凉血止血。用于痢疾、胃肠炎、肝炎、泌尿系感染、感冒发烧、咽喉肿痛、白带、崩漏、农药中毒;外用治外伤出血,烧烫伤。
蛇莓:【别名】 蛇泡草、蛇盘草、蛇果草、龙吐珠、宝珠草、三匹风、三叶莓、地杨梅、三爪风、三爪龙、三脚虎、红顶果。为蔷薇科蛇莓属植物蛇莓Duchesnea indica (Andr.)Focke,以全草入药。夏秋采收,鲜用或洗净晒干。【性味】 甘、酸,寒。有小毒。【功能主治】 清热解毒,散瘀消肿。用于感冒发热,咳嗽,小儿高热惊风,咽喉肿痛,白喉,黄疸型肝炎,细菌性痢疾,阿米巴痢疾,月经过多;外用治腮腺炎,毒蛇咬伤,眼结膜炎,疔疮肿毒,带状疱疹,湿疹。亦可试治癌症。并可用于杀灭孑孓、蝇蛆。
墨旱莲:为菊科植物鳢肠Eclipta prostrata L.的干燥地上部分。花开时采割,晒干。【性味与归经】甘、酸,寒。归肾、肝经。【功能与主治】滋补肝肾,凉血止血。用于肝肾阴虚,牙齿松动,须发早白,眩晕耳鸣,腰膝酸软,阴虚血热、吐血、衄血、尿血,血痢,崩漏下血,外伤出血。
益母草:为唇形科植物益母草Leonurus japonicus Houtt.的新鲜或干燥地上部分。鲜品春季幼苗期至初夏花前期采割;干品夏季茎叶茂盛、花未开或初开时采割,晒干,或切段晒干。【性味与归经】苦、辛,微寒。归肝、心包、膀胱经。【功能与主治】活血调经,利尿消肿,清热解毒。用于月经不调,痛经经闭,恶露不尽,水肿尿少,疮疡肿毒。【贮藏】干益母草置干燥处;鲜益母草置阴凉潮湿处。
仙鹤草:为蔷薇科植物龙牙草Agrimonia pilosa Ledeb. 的干燥地上部分。苦、涩,平。归心、肝经。收敛止血,截疟,止痢,解毒。用于咳血,吐血,崩漏下血,疟疾,血痢,脱力劳伤,痈肿疮毒,阴痒带下。
小肠风:为胡椒科植物山蒟Piper hancei Maxim.的全草。别名 辣椒姜、石蒟、格劳端、丢柄美、爬岩香、二十四症、上树风、山蒌。性味 味辛,性平。功效 祛风除湿,强腰膝,消肿止痛。传统应用 治感冒咳嗽、气喘、胃腹寒痛、风湿骨痛、手足麻木、腰膝无力、肌肉萎缩、跌打损伤。
当归:为伞形科植物当归Angelica sinensis (Oliv.) Diels的干燥根。秋末采挖,除去须根和泥沙,待水分稍蒸发后,捆成小把,上棚,用烟火慢慢熏干。【性味与归经】甘、辛,温。归肝、心、脾经。【功能与主治】补血活血,调经止痛,润肠通便。用于血虚萎黄,眩晕心悸,月经不调,经闭痛经,虚寒腹痛,风湿痹痛,跌扑损伤,痈疽疮疡,肠燥便秘。酒当归活血通经。用于经闭痛经,风湿痹痛,跌扑损伤。
川芎:为伞形科植物川芎Ligusticum chuanxiong Hort.的干燥根茎。夏季当茎上的节盘显著突出,并略带紫色时采挖。除去泥沙,晒后烘干,再去须根。【性味与归经】辛,温。归肝、胆、心包经。【功髓与主治】活血行气,祛风止痛。用于胸痹心痛,胸胁刺痛,跌扑肿痛,月经不调,经闭痛经,癓瘕腹痛,头痛,风湿痹痛。
白芍:为毛茛科植物芍药Paeonia lactiflora Pall.的干燥根。夏、秋二季采挖,洗净,除去头尾和细根,置沸水中煮后除去外皮或去皮后再煮,晒干。【性味与归经】苦、酸,微寒。归肝、脾经。【功能与主治】养血调经,敛阴止汗,柔肝止痛,平抑肝阳。用于血虚萎黄,月经不调,自汗,盗汗,胁痛,腹痛,四肢挛痛,头痛眩晕。【注意】不宜与藜芦同用。
香附:为莎草科植物莎草Cyperus rotundus L.的干燥根茎。秋季采挖,燎去毛须,置沸水中略煮或蒸透后晒干,或燎后直接晒干。【性味与归经】辛、微苦、微甘,平。归肝、脾、三焦经。【功能与主治】疏肝解郁,理气宽中,调经止痛。用于肝郁气滞,胸胁胀痛,疝气疼痛,乳房胀痛,脾胃气滞,脘腹痞闷,胀满疼痛,月经不调,经闭痛经。
巴戟天:为茜草科植物巴戟天Morinda officinalis How的干燥根。全年均可采挖,洗净,除去须根,晒至六七成干,轻轻捶扁,晒干。【性味与归经】甘、辛,微温。归肾、肝经。【功能与主治】补肾阳,强筋骨,祛风湿。用于阳痿遗精,宫冷不孕,月经不调,少腹冷痛,风湿痹痛,筋骨痿软。
半枝莲:为唇形科植物半枝莲Scutellaria barbata Don [S. rivularis Wall]的干燥全草。辛、苦,寒。归肺、肝、肾经。清热解毒,化瘀利尿。用于疔疮肿毒,咽喉肿痛,毒蛇咬伤,跌扑伤痛,水肿,黄疸。
半边莲:为桔梗科植物半边莲Lobelia chinensis Lour. 的干燥全草。辛,平。归心、小肠、肺经。利尿消肿,清热解毒。用于大腹水肿,面足浮肿,痈肿疔疮,蛇虫咬伤;晚期血吸虫病腹水。
白英:为茄科茄属植物白英Solanum lyratum Thunb. [S. dulcamara L. var. lyratum (Thunb.) Sieb. et Zucc., S. Dulcamara L. var. pubescens Blume, S.Dulcamara L. var. chinense Dunal],以全草入药。苦,微寒。有小毒。入肝、胃经。清热解毒,利湿消肿,抗癌。用于感冒发热,乳痈、恶疮,湿热黄疸、腹水,白带,肾炎水肿;外用治痈疖肿毒。
鸡血藤:为豆科植物密花豆Spatholobus suberectus Dunn的干燥藤茎。秋、冬二季采收,除去枝叶,切片,晒干。【性味与归经】苦、甘,温。归肝、肾经。【功能与主治】活血补血,调经止痛,舒筋活络。用于月经不调,痛经,经闭,风湿痹痛,麻木瘫痪,血虚萎黄。
黑肉风:又名褐毛黎豆、肉麻绳、黑血藤、牛豆藤。为豆科黑血藤Mucuna castanea Merr.,以老茎入药。【性味】涩,凉。【功能主治】清肺热,止咳,舒筋活血。主治肺燥咳,咳血,腰膝酸痛,月经不调,贫血,萎黄病。
本发明的另一方面是提供了本发明药物组合物的活性成分的制备方法,该方法是采用水提或40—80%体积百分浓度的乙醇提取。具体的制备方法如下:
方案一:取全部药材,加水提取2-3次,每次加水量相当于药材总重量的6-12倍,每次提取时间为1-3小时,合并提取液,滤过,滤液浓缩至70-80℃时相对密度为1.10-1.25的清膏,即得活性成分。
方案二:取全部药材,加水提取2-3次,每次加水量相当于药材总重量的6-12倍,每次提取时间为1-3小时,合并提取液,滤过,滤液浓缩至70-80℃时相对密度为1.05-1.20的清膏,加入乙醇,使含醇量为40-70%,静置12-24小时,滤过,滤液浓缩至70-80℃时相对密度为1.10-1.25的浸膏,即得活性成分。
方案三:取全部药材,用40-80%乙醇回流提取2-3次,每次乙醇用量为药材总量的4-10倍,提取时间为1-3小时,合并提取液,过滤,滤液浓缩至60-80℃时相对密度为1.10-1.25的浸膏,即得活性成分。
本发明所述的治疗乳腺小叶增生的药物组合物,可以与医药学上可接受的载体配合,制成各种常用制剂,如片剂、颗粒剂、胶囊剂、合剂等。
本发明的药学上可接受的载体包括但不限于以下:
稀释剂:淀粉、乳糖、糊精、微晶纤维素、无机盐等。
润湿剂与粘合剂:纯化水、乙醇、明胶、聚乙二醇、纤维素衍生物等。
崩解剂:淀粉、羧甲基淀粉钠、纤维素衍生物、交联聚维酮等。
润滑剂:硬脂酸镁、微粉硅胶、滑石粉、聚乙二醇等。
助溶剂:水、乙醇、甘油、丙二醇、液状石蜡、植物油等。
矫味剂:蔗糖、单糖、芳香剂等。
防腐剂:苯甲酸、山梨酸、甲酯、乙酯、丙酯等。
乳腺增生病原发病因有情志内伤、劳倦内伤、饮食内伤、痰浊、气滞、烦劳过等。乳癖的发生,可由单一脏腑偏胜致病,亦可由多个脏腑偏胜、偏衰致病。在临证中单独由心情抑郁引起的肝气郁结,脾失健运,痰湿内生的多见于青年妇女;复合因素致病的,多因肝肾不足、冲任失调引起,多见于中年及中年后妇女。
本发明白背风祛风利湿,行气活血;三白草清热解毒;风尾草解毒止痢,凉血止血;蛇莓散瘀消肿;墨旱莲滋补肝肾;益母草活血调经,利尿消肿,清热解毒;仙鹤草收敛止血;小肠风消肿止痛;当归补血活血,调经止痛;川芎活血行气,祛风止痛;白芍养血调经,敛阴止汗,柔肝止痛;香附疏肝解郁,理气宽中;巴戟天补肾阳,强筋骨;半枝莲、半边莲清热解毒,化瘀利尿;白英利湿消肿;鸡血藤活血补血,调经止痛;黑肉风清肺热,舒筋活血。全方相互协同,共凑健脾解郁、养血调经、调畅气机、软坚散结祛湿利尿之功,各药相伍,使脾胃营运顺畅、郁结得解,气血得以生养。本发明药物组合物用于治疗妇女乳腺增生效果显著。
本发明提供的一种治疗乳腺小叶增生的药物组合物具有以下优点:
1、本发明提供的药物制剂安全有效,具有健脾解郁、养血调经、调畅气机、软坚散结的功效,特别适合肝气郁结所致乳腺增生患者长期服用。本发明药物组合物具有健脾解郁、养血调经、调畅气机、软坚散结的功效,能促进乳房血液循环、调节免疫和内分泌平衡的作用。临床使用显示,治疗组乳痛症状减轻或消失及乳腺结节缩小或消失的情况均好于对照组,治疗乳腺小叶增生总有效率为97%,疗效明显优于对照组,且在临床使用过程中未发现有任何毒副作用,因此本发明是治疗乳腺小叶增生的有效药物,值得临床推广使用。
2、本发明中药组合物对肝脏没有损害,无毒副作用,可长期服用。
3、本发明提供的中药组合物制剂,成品采用纯中药配方结合现代先进生产工艺生产,携带和服用都很方便。

取白背风30g、三白草30g、风尾草30g、蛇莓30g、墨旱莲30g、益母草30g、仙鹤草30g、小肠风20g、当归20g、川芎20g、白芍20g、香附20g、巴戟天20g、半边莲20g、半枝莲20g、白英20g、鸡血藤20g、黑肉风20g,加水提取2次,两次加水量分别为药材总重量的10倍、8倍,两次提取时间分别为2小时、1小时,合并提取液,滤过,滤液浓缩至80℃时相对密度为1.10的浸膏,加入纯化水至总重量为药材总重量的3倍,灌装,即得本发明合剂。
实施例2:
取白背风20g、三白草20g、风尾草20g、蛇莓20g、墨旱莲20g、益母草20g、仙鹤草20g、小肠风15g、当归15g、川芎15g、白芍15g、香附15g、巴戟天15g、半边莲15g、半枝莲15g、白英15g、鸡血藤15g、黑肉风15g,用40%乙醇回流提取3次,每次乙醇用量分别为药材总量的10倍、8倍、4倍,提取时间分别3小时、2小时、1小时,合并提取液,过滤,滤液浓缩至60℃时相对密度为1.25的浸膏,加入辅料淀粉、乙醇、滑石粉,制得片剂。
实施例3:
取白背风40g、三白草40g、风尾草40g、蛇莓40g、墨旱莲40g、益母草40g、仙鹤草40g、小肠风25g、当归25g、川芎25g、白芍25g、香附25g、巴戟天25g、半边莲25g、半枝莲25g、白英25g、鸡血藤25g、黑肉风25g,加水提取3次,三次加水量分别相当于药材总重量的12倍、8倍、6倍,每次提取时间分别3小时、2小时、1小时,合并提取液,滤过,滤液浓缩至70℃时相对密度为1.20的清膏,加入乙醇,使含醇量为40%,静置24小时,滤过,滤液浓缩至70℃时相对密度为1.10的浸膏,加入纯化水至总重量为药材总重量的3倍,灌装,即得本发明合剂。
实施例4:
取白背风30g、三白草30g、风尾草30g、蛇莓30g、墨旱莲30g、益母草30g、仙鹤草30g、小肠风20g、当归20g、川芎20g、白芍20g、香附20g、巴戟天20g、半边莲20g、半枝莲20g、白英20g、鸡血藤20g、黑肉风20g,用乙醇回流提取2次,每次乙醇浓度分别为80%、60%,每次乙醇用量分别为药材总量的10倍、8倍,提取时间分别为4小时、3小时,合并提取液,过滤,滤液浓缩至80℃时相对密度为1.10的浸膏,加入辅料淀粉、乙醇、滑石粉,制得片剂。
实施例5:
取白背风20g、三白草30g、风尾草40g、蛇莓20g、墨旱莲30g、益母草40g、仙鹤草30g、小肠风15g、当归20g、川芎25g、白芍15g、香附20g、巴戟天25g、半边莲15g、半枝莲20g、白英25g、鸡血藤15g、黑肉风20g,加水提取3次,三次加水量分别为药材总重量的12倍、8倍、6倍,三次提取时间分别为3小时、2小时、1小时,合并提取液,滤过,滤液浓缩至70℃时相对密度为1.25的清膏,加入辅料淀粉、乙醇、糊精、硬脂酸镁,装入胶囊壳、制得胶囊剂。
实施例6:
取白背风40g、三白草20g、风尾草30g、蛇莓40g、墨旱莲20g、益母草30g、仙鹤草40g、小肠风20g、当归25g、川芎20g、白芍15g、香附15g、巴戟天25g、半边莲15g、半枝莲20g、白英25g、鸡血藤25g、黑肉风20g,加水提取2次,每次加水量分别相当于药材总重量的12倍、10倍,每次提取时间分别为3小时、2小时,合并提取液,滤过,滤液浓缩至80℃下相对密度为1.05的清膏,加入乙醇,使含醇量为70%,静置12小时,滤过,滤液浓缩至70℃时相对密度为1.25的浸膏,加入辅料淀粉、糊精、乙醇,制得颗粒剂。
实施例7:
称取白背风30g、三白草30g、风尾草30g、蛇莓30g、墨旱莲30g、益母草30g、仙鹤草30g、小肠风20g、当归20g、川芎20g、白芍20g、香附20g、巴戟天20g、半边莲20g、半枝莲20g、白英20g、鸡血藤20g、黑肉风20g,净选,干燥、粉碎,即得本发明粉剂。
临床治疗总结
1 资料与方法
1.1 一般资料 选择门诊的乳腺小叶增生患者200例,均经手法触诊结合乳腺红外线扫描或B超检查确诊。所有患者均排除了乳腺癌、乳腺纤维瘤、乳腺炎等。随机分为两组,各100例,两组在年龄、病程长短、怀孕次数、情绪变化、自觉症状等方面差异无统计学意义,具有可比性。
1.2 治疗方法
治疗组:给予本发明实施例1药物,每日3次,口服。
对照组:逍遥丸,每次8丸(相当于原药材3g),每日3次。
两组药物均3个月经周期为1个疗程,月经期间暂停服用。用药期间减少刺激性饮食,保持乐观的心态和有规律的生活。
2 结果
2.1 疗效标准 据中华全国中医学会外科分会乳腺专业委员会制定的标准:
显效:肿块消失,乳痛消失,停药3个月不复发;
有效:肿块最大直径缩小1/2以上,乳痛消失;
好转:肿块最大直径缩小<1/2,乳痛减轻,或肿块缩小1/2以上,乳痛未减轻者;
无效:肿块不缩小或反而增大变硬,或单纯乳痛缓解而肿块不缩小者。
2.2 统计学处理 采用X2检验

3 讨论
乳腺小叶增生一般认为与女性内分泌失调有关,孕激素水平低下而雌激素分泌过量,长期作用于乳腺组织的结果。目前西医常用雌激素受体抑制剂三苯氧胺治疗,但易引起月经紊乱的副作用,并有诱发子宫内膜癌的危险。
本发明药物组合物具有健脾解郁、养血调经、调畅气机、软坚散结的功效,能促进乳房血液循环、调节免疫和内分泌平衡的作用。临床使用显示,治疗组乳痛症状减轻或消失及乳腺结节缩小或消失的情况均好于对照组,治疗乳腺小叶增生疗效明显优于对照组,且在临床使用过程中未发现有任何毒副作用,因此本发明是治疗乳腺小叶增生的有效药物,值得临床推广使用。

6.如权利要求3所述的药物组合物,其特征在于它的活性成分是通过以下方法制备而成的:取全部药材,用40-80%乙醇回流提取2-3次,每次乙醇用量为药材总量的4-10倍,提取时间为1-3小时,合并提取液,过滤,滤液浓缩至60-80℃时相对密度为1.10-1.25的浸膏,即得所述活性成分。
