技术领域
本公开涉及一种手术紧固件,并且更具体地,涉及一种包括接合构件的手术部件和有关的施加方法。
背景技术
在如直接或间接的腹股沟疝的疝中,一部分肠突出穿过支撑腹壁中的缺损从而形成疝囊。该缺损可使用制造相对大切口的开放式手术操作来修补,并且疝通过缝合被封闭在腹壁外面。网状物通过缝合线而连接在开口上从而提供加固。
相反,如内窥镜或腹腔镜的微创手术操作目前可用来修补疝。在腹腔镜操作中,在腹部通过小切口来执行手术,而在内窥镜操作中,通过狭窄的内窥镜管或插管插入穿过身体中的小切口来执行手术。
目前,用于疝修补的微创手术技术利用例如手术钉、吻合钉和夹子的手术紧固件来将网状物固定至组织,从而对用于促进组织向内生长的修补和结构来提供加固。手术紧固件可以借助手术紧固件施加器穿过网状物且进入下面的组织来布设。
例如在涉及不规则或不平坦的表面几何形状的情况下,或在当内部身体结构易受移动和移位时的情形中,用手术紧固件使网状物附于疝缺损上就可能存在挑战性。然而,期望的是使穿过网状物来布设的手术紧固件的数量和尺寸最小化以使对下面组织的创伤最小化。因此,将期望的是提供一种构造有最佳的网状物保持性能同时使插入组织的手术紧固件的部分或尺寸最小化的手术紧固件。
发明内容
依照本公开的一个方案,一种手术紧固件被公开,其包括头部和组织捕捉部。所述头部包括枢转地布置在所述头部的外表面上的接合构件。所述组织捕捉部限定有纵轴线且延伸离开所述头部。至少一个螺旋形的螺纹沿所述组织捕捉部的外表面而被限定。所述组织捕捉部被构造为绕所述纵轴线沿第一径向方向旋转,且所述接合构件被构造为从所述头部沿所述第一径向方向向外枢转。

具体实施方式
现在参考附图,将描述本公开的手术紧固件,其中在若干视图中同样的附图标记代表同样的元件。如在此使用的,术语“操作者”可指的是本公开的手术紧固件的任何使用者,如护士、医生或临床医生。进一步地,术语“远侧”指的是手术紧固件的离操作者较远的部分或其组件,而术语“近侧”指的是手术紧固件的更靠近操作者的部分或其组件。
先参考图1、图2A、图2B、图3A以及图3B,示出了依照本公开的实施例的手术紧固件10。如所示,手术紧固件10可具有基本上圆锥的构造,且限定了纵轴线“A”。手术紧固件10包括头部20和组织捕捉部40。
头部20具有基本上平的、类似圆盘的外形,该外形具有近侧表面22a和远侧表面22b。头部20限定了各自正交于纵轴线“A”且彼此正交的第一横轴线“B1”和第二横轴线“B2”。头部20包括从头部20径向向外突出所布置的两个相对的螺纹部24a、24b。相对的螺纹部24a、24b可具有渐细的构造且分别包括螺纹26a、26b,其被构造用于与运送装置(例如手术敲钉器)的部分相接合,这将在下文更进一步地描述。一对径向缺口28a、28b限定在头部20的螺纹部24a、24b之间,且可被定位用于与运送装置的其它部分相接合,这将在下文更进一步地描述。

接合构件30a、30b可与头部20的外表面整体形成。在一些实施例中,接合构件30a、30b可以是经由例如铜焊或焊接、活动铰接或粘附来连接至头部20的其余部分的可分离组件。
如此,接合构件30a、30b被构造为相对于头部20的其余部分径向向外地枢转和折曲或摆动。转到图4,接合构件30a、30b的运动允许手术紧固件10在第一、未展开状态和第二、展开的构造之间转换,在第一、未展开状态,接合构件30a、30b朝头部20的其余部分径向向内地接近,在第二、展开的构造,接合构件30a、30b相对于头部20的其余部分径向向外地布置。当接合构件30a、30b布置在相应的径向缺口28a、28b内时,接合构件30a、30b可限定一曲率半径,该曲率半径接近由在径向缺口28a、28b内的头部20的外表面所限定的曲率半径,以有助于折成手术紧固件10的第二、展开的构造。
头部20的远侧表面形成在组织捕捉部40的近侧端42上。如图所示,组织捕捉部40可具有从其近侧端42向远侧尖端44逐渐变细的基本上圆锥的外形。尖端44限定了尖角表面,该尖角表面被构造用于穿透例如网状物和/或组织,这将在下文更进一步地描述。组织捕捉部40进一步包括围绕其形成的螺纹46。如图所示,组织捕捉部40可限定最大直径“Ds”(图3B),该最大直径“Ds”小于由头部20所限定的外直径“DH1”。如此,头部20可形成凸缘表面,该凸缘表面径向地突出离开组织捕捉部40的近侧端42。
螺纹46沿着组织捕捉部40遵循大体螺旋形的远侧向下的轨迹。螺纹46可在组织捕捉部40的近侧端42处具有第一终点,并且可在组织捕捉部40的远侧尖端44处具有第二终点。在一些实施例中,螺纹46可具有布置在组织捕捉部40的近侧端42和远侧端44之间的第一终点和第二终点。螺纹46可具有适于手术紧固件的任何期望的构造,例如单螺纹或双螺纹。螺纹46可通过任何合适的方式(如蚀刻或模制)而形成在组织捕捉部40上,以使得螺纹46与组织捕捉部40整体形成,或螺纹46可通过粘附、铜焊或焊接而连接至组织捕捉部40。
将理解的是,手术紧固件10可具有适于其预期目的的任何构造。在Corradi等人的公开号2011/0282401的美国专利申请中公开了一种示例性手术紧固件,该专利的全部内容通过引用合并于此。
现在转到图5,示出了总体被指定为100的运送装置的远侧部。运送装置100可具有任何合适的构造(如手术紧固件施加器或敲钉器)以使手术紧固件10前移穿过网状物“M”并且进入下面的组织“T”。在授予Shipp的专利号为8,114,099的美国专利中详细地描述了一种合适的运送装置,该专利的全部内容通过引用合并于此。
运送装置100可包括外管部102和布置在其中的线圈构件104。线圈构件104为基本上弹性的构件,该弹性构件可具有挠性的外形,以使得运送装置100的外管部102在弯曲载荷下偏置以返回到静止位置。线圈构件104固定地布置在外管部102内,且能以任何合适的方式(例如粘附、铜焊或焊接)连接至外管部102的内表面。线圈构件104包括具有直径“D”的主体105,且主体105以限定间距“P”的成螺旋形缠绕的构造来布置。
手术紧固件10可以以任何合适的方式(例如一叠或一列)装载到运送装置100内。运送装置100的外管部102被定尺寸为使得接合构件30a、30b被外管部102的内表面约束在第一、未展开状态。线圈构件104的连续缠绕被布置为使得在线圈构件104的相邻缠绕圈之间限定有纵向间隔“S”。如此,线圈构件104具有的构造为使得手术紧固件10的相对螺纹部24a、24b可布置在线圈构件104的主体105的相邻缠绕圈之间的间隔“S”内。
运送装置100可还包括一对径向对置的齿形件(tines)106a、106b,该齿形件106a、106b在外管部102的内部纵向地延伸以接合手术紧固件10。具体地,径向对置的齿形件106a、106b被定尺寸为使得每个径向对置的齿形件106a、106b可布置于在手术紧固件10的相对螺纹部24a、24b之间限定的相应缺口28a、28b内(图1)。因此,布置在手术紧固件10的相应径向缺口28a、28b内的手术紧固件10的接合构件30a、30b可被构造且被排列为使得接合构件30a、30b能径向地偏离手术紧固件10的相对螺纹部24a、24b,从而限定了一间隔用于相应的齿形件106a、106b穿过其延伸。
在使用时,运送装置100被致动,使得齿形件106a、106b在外管部102内旋转。由于手术紧固件10的相对的螺纹部24a、24b与由线圈构件104所限定的纵向间隔“S”螺纹地接合,齿形件106a、106b的旋转迫使相对的螺纹部24a、24b在线圈构件104内绕纵轴线“A”(图1)转动,这引起手术紧固件10沿由线圈构件104的主体105所限定的螺旋轨迹来向远侧前移穿过运送装置100的外管部102。随着手术紧固件10靠近运送装置100的远侧端,手术紧固件10的组织捕捉部40和远侧尖端44从外管部102突出出来。齿形件106a、106b的进一步的接合和旋转引起手术紧固件10穿透且前移穿过例如网状物“M”,并且进入组织“T”。
网状物“M”可为被构造为覆盖期望的组织部分(例如疝缺损)的任何合适的网状材料。组织“T”可为组织(例如腹壁)的任何的外部部分或内部部分。
现在转到图7,将描述手术紧固件10的展开。一旦手术紧固件10的头部20前移经过运送装置100的外管部102(图5)的远侧端,布置在相对的螺纹头部24a、24b之间的接合构件30a、30b就能够自由地径向向外折曲。因此,接合构件30a、30b绕其与手术紧固件10的头部20的连接点来枢转,且旋转离开头部20一定度数。每个接合构件30a、30b都能在例如自每个相应的接合构件30a、30b与手术紧固件10的头部20的连接点起测量的大约0度和大约180度之间枢转。因此,从手术紧固件10的俯视平面图中,每个相应的接合构件30a、30b可布置在沿一对横轴线“B1”、“B2”测量的相应的第一象限Q1和第三象限Q3中,所述一对横轴线“B1”、“B2”沿手术紧固件10的头部20的近侧表面而限定。因此,每个相应的相对螺纹部24a、24b可布置在相应的第二象限Q2和第四象限Q4中。将理解的是,手术紧固件10可具有任何构造,以使得相对的螺纹部24a、24b或接合构件30a、30b中的任意一个可布置在象限Q1、Q2、Q3、Q4的任意一个中。

由于接合构件30a、30b径向向外地布置处于第二、展开的构造中,因此更大面积的网状物“M”被手术紧固件10所接合且抵靠组织“T”而被压紧。因此,接合构件30a、30b有助于网状物“M”抵靠组织“T”的固定接合,且提供增强的网状物保持性能,例如用于长期的使用或以尽量减少由于组织“T”的移位或由于施加至手术紧固件10、网状物“M”或组织“T”的其它外部载荷而使手术紧固件10与网状物“M”脱离。如此,手术紧固件10的构造提供了最佳的网状物保持性能,使得同无接合构件30a、30b的手术紧固件相比,可以采用更少量的手术紧固件10。此外,通过提供具有接合构件30a、30b的手术紧固件10的头部20,提供了最佳的网状物保持性能而没有增加组织捕捉部40插入组织“T”的部分。

Claims (16)

1.一种手术紧固件,包括:
头部,其包括枢转地布置在所述头部的外表面上的接合构件;以及
组织捕捉部,其限定有纵轴线且延伸离开所述头部,至少一个螺旋形的螺纹沿所述组织捕捉部的外表面而被限定;
其中,所述组织捕捉部被构造为绕所述纵轴线沿第一径向方向旋转,且所述接合构件被构造为从所述头部沿所述第一径向方向向外枢转。
2.根据权利要求1所述的手术紧固件,其中所述头部包括螺纹部。
3.根据权利要求2所述的手术紧固件,其中所述接合构件与所述头部的螺纹部径向地相邻布置。
