中国专利101695576[仅摘要]公开了医学超声无菌耦合剂,其通过以下重量份的组分制备:氢氧化钠5-20,乙二醇150-450,羟苯乙酯2-9,卡波姆10-50,盐酸氯己定(CHX-HCL)0.2-0.8,乙二胺四乙酸(EDTA)0.3-1.2,和蒸馏水725-2,000。耦合剂具有优良的声匹配和耦合功能,避免了超声探头的腐蚀和膨胀,具有消毒以及杀菌功能,避免了刺激和敏化皮肤粘膜并且能够实现超声探头和皮肤黏膜的同步、快速和持续的消毒和杀菌,且有效地较低了在医学超声检查和治疗过程中由于超声探头(治疗头)与患者身体的接触引起的潜在的医院内交叉感染风险。该耦合剂可适用于经皮肤粘膜、经阴道和经直肠超声检测和治疗。
美国专利4,542,745公开了超声乳液,其依靠其组分提供了具有期望的衰减特性的乳液。乳液的外相是水和速率增强剂混合物。增强剂是合适的醇,如乙二醇、丙二醇、或丙三醇(甘油)。油相,如硅酮液体组成了乳液的悬浮相。
美国专利2005/074407公开了体内生物相容和生物蚀解润滑剂和超声耦合液或凝胶,包含聚乙烯吡咯烷酮(PVP)和/或聚乙烯醇(PVA)。耦合液或凝胶包含聚乙烯吡咯烷酮和/或聚乙烯醇水溶液,该聚乙烯醇水溶液中加入湿润剂,如亚烷基二醇和/或聚亚烷基二醇以获得期望的接触(触觉,tactile)和干燥特性。另外,这类液体和凝胶可以通过加入有机和无机交联剂制备。

发明内容
本发明涉及水超声接触液,如专利权利要求限定的,包含药物等级甘油三酯和药学上可接受的乳化剂和可选地药学上可接受的湿润剂。
具体实施方式
如本发明使用的术语超声接触液表示在超声探头和组织之间用来去除空气的介质,因此主要目的是在超声传感器/探头和待研究的组织之间提供声耦合。该术语与以下术语同义:“接触液”、“接触剂”、“耦合液”、“声液”、“声耦合液”和“声耦合剂”。
本发明的目的是解决在超声成像中遇到的问题,其中,因为超声在具有不同衰减的介质中至少部分传播,观察到信号强度的差别。这导致超声图像,其中,在信号强度上存在大量的区域差异。该问题使得难于解释图像以及难于调整获得参数以便平衡信号强度中的区域差异。
本发明的另一个目的是解决当插入液体至体腔时遇到的问题,以及取决于超声波是否至少部分在器官或腔(其中已给予液体)中传播,可观察到强度上存在的差异。
本发明的又一目的是解决当插入液体至体腔或心脏系统时遇到的问题以使得超声成像成为可能,以及超声成像在液体填充腔中具有高度不同的强度并且对于邻近腔的组织的信号强度是无可比拟的。
本发明涉及水超声接触液,其在超声成像期间可以用于体腔或切除腔,特别地当在手术期间进行所述超声成像时。根据本发明的液体组合物提供了与待测组织相等的声波的衰减。
在本发明实施方式中,提供了水超声接触液,包含药物等级的甘油三酯、药学上可接受的乳化剂和可选地,药学上可接受的湿润剂,其中,甘油三酯含量在8-240g/1000ml接触液的范围,以及乳化剂的含量是0.4-18g/1000ml接触液,以及可选地,湿润剂是1.2-30g/1000ml接触液的量。
在本发明的另一实施方式中,提供了超声接触液,其中,甘油三酯含量是在10-200g/1000ml接触液的范围,以及乳化剂的含量是0.5-15g/1000ml接触液,以及可选地,湿润剂是1.2-25g/1000ml接触液的含量。
在本发明的又一实施方式中,提供了超声接触液,其中,甘油三酯的含量在50-110g/1000ml接触液,乳化剂的含量是2-10g/1000ml接触液,以及可选地,湿润剂是5-15g/1000ml接触液的含量。
使用的药物等级的甘油三酯可以选自植物油或动物脂肪。植物油可优选选自由以下组成的组:大豆油、橄榄油、棕榈油和椰子油(copra oil)。可以使用的动物脂肪的实例是乳脂肪、鱼油和鱼肝油。
使用的药学上可接受的乳化剂可以选自由以下组成的组:卵磷脂(如蛋黄卵磷脂、大豆卵磷脂)、乳化蜡、鲸蜡硬脂醇(cetearyl alcohol)、聚山梨醇酯20、鲸蜡硬脂醇醚(鲸蜡硬脂醇聚醚,ceteareth)20。
药学上可接受的湿润剂可以选自由以下组成的组:甘油、乳酸、多元醇、丙二醇、山梨糖醇。
对于在体内和切除腔使用的超声接触液,生理范围的pH是必要的,以及通过使用本领域通常已知的方法,液体中的pH可以调整至合适用于预期临床应用的pH水平,例如,在6至8范围,优选约7。进一步,根据本发明的液体是用于声学检查/腔成像的生物相容的无菌液体。
调整液体的重量摩尔渗透压浓度(osmolality)和盐度至生理可接受的值在本领域内通常是已知的以及这种调整方法对于本发明是适用的。
在本发明具体的实施方式中,根据本发明的超声接触液包括90g大豆油、5.5g卵磷脂和10g丙三醇以及生理盐水以达到1000ml。
本发明涉及水超声接触液,在超声检查前或检查期间,其可以给药至患者(人或动物),其是生物相容的、无菌的且具有的衰减系数α至少30倍大于水的衰减系数,(即,α>0.066)。在本发明实施方式中,衰减系数α是0.20至1.10,优选0.80。在本发明实施方式中,接触液具有标定(target)为0.80dB/(MHz·cm)的衰减系数α,其与成年人脑的α在同一量级。
本发明的作用(效果)如图2A(现有技术)所示,其中,用NaCl(0.9%)填充具有腔的仿真模型,并且在液体填充的腔的底部处和底部下看见高强度信号。该高强度信号部分掩盖了腔底部下方的结构,并且这在临床情况下也经常遇见。在图2B(本发明)中,用根据本发明的液体(其提供根据本发明标准的声波衰减)填充腔。观察到腔的浸渍结构(dippingstructure)至少具有如图2A所见相同的强度,而底部腔处及其范围之外的强度在振幅上降低并且与邻近介质的强度更相似。
实施例
实施例1
在仿真模型(图2)和新鲜小猪尸体(图3)上评估根据本发明的接触液的成像特性。
仿真模型(Phantom)
在仿真模型中,将针插入在适当处(in oasis)形成的腔中,并且通过平面线形阵列(Vingmed Ultrasound System FiVe)成像。在图2左(A)显示用常规盐水制作的图像和在图2右(B)显示用具有高衰减的根据本发明的接触液制作的图像。使用相同的获得参数(获取参数,acquisition parameter),但是每种情况下的整体增益(global gain)被调整以提供“最佳图像”,如通过有经验的超声操作者限定的。在(A)中腔的底部下的亮区在(B)中几乎完全消失。因此用根据本发明的具有衰减的造影液制作的图像产生了更准确的图片而不具有(A)中的伪迹。
小猪脑
在比较使用盐水或使用根据本发明的接触液的成像质量的实验中,使用近期解剖的小猪。制作于新鲜的小猪脑的切除腔(用图3的箭头标记)使用生理盐水填充切除腔成像(a)以及使用根据本发明的接触液成像(b)。获得参数在两种情况下是一致的。主导信号增强在图像的更深部分是通过来自头骨基部的强的反射引起的。两张图(a和b)中切除腔下的强度的差别是显著的。即使片的方向与图中的并不完全一致,在b中的切除腔底部正下方显然不存在人为的信号增强。在a和b中的更深的骨反射中,也可以看见信号强度的差别。
实施例2
已经进行了评估根据本发明的超声接触液安全性的动物研究。
该研究中包括8只大鼠和6只猪。包括的大鼠用于实质内注射至脑,包括的小猪用造影液注射至蛛网膜下腔空间。评估动物行为、EEG记录、组织病理学和免疫组织化学。
在该安全研究中,在大鼠和猪中未检测到用于改善在超声导向操作中的图像质量的根据本发明的接触液的负面的临床事件。因此,根据本发明的超声接触液在检测环境下表现是安全的。
Claims (15)
1.组合物在制备手术中超声成像中的水超声接触液中的应用,所述组合物包含药物等级的植物油和药学上可接受的卵磷脂并且可选地包含药学上可接受的湿润剂,其中,所述植物油含量在50-110g/1000ml组合物的范围以及卵磷脂的量是2-10g/1000ml组合物,以及可选地湿润剂的量是5-15g/1000ml。
2.根据权利要求1所述的应用,其中,所述植物油选自由以下组成的组:大豆油、橄榄油、棕榈油和椰子油。
3.根据以上权利要求中任一项所述的应用,其中,所述卵磷脂选自由以下组成的组:蛋黄卵磷脂和大豆卵磷脂。
4.根据权利要求1或权利要求2所述的应用,其中,所述湿润剂选自由以下组成的组:乳酸和多元醇。
5.根据权利要求3所述的应用,其中,所述湿润剂选自由以下组成的组:乳酸和多元醇。
6.根据权利要求4所述的应用,其中,所述多元醇选自由以下组成的组:甘油、丙二醇和山梨糖醇。
7.根据权利要求5所述的应用,其中,所述多元醇选自由以下组成的组:甘油、丙二醇和山梨糖醇。
8.根据权利要求1所述的应用,其中,所述组合物包含pH调节添加剂。
9.根据权利要求8所述的应用,其中,以将pH设置为6至8的范围的量加入所述pH调节添加剂。
10.根据权利要求9所述的应用,其中,以将pH设置为约7的量加入所述pH调节添加剂。
11.根据权利要求1或权利要求8所述的应用,其中,所述组合物包含90g大豆油、5.5g卵磷脂和10g甘油以及生理盐水以达到1000ml。
12.根据权利要求1或权利要求8所述的应用,其中,所述组合物具有衰减系数α>0.066。
13.根据权利要求11所述的应用,其中,所述组合物具有衰减系数α>0.066。
14.根据权利要求12所述的应用,其中,所述组合物具有在0.20至1.10的范围的衰减系数α。
15.根据权利要求14所述的应用,其中,所述组合物具有约0.80的衰减系数α。
