Abstract

Description
本发明的技术领域

背景
肿瘤在拥有高度复杂的免疫系统的宿主中形成,所述免疫系统包括能够识别和摧毁转化细胞的各种淋巴细胞亚类。现在广泛接受的是,尽管淋巴细胞可以持续侦查肿瘤形成,但是癌细胞形成了逃避免疫监视的分子策略,所述分子策略在宿主免疫系统的压力下被竞争性地选择。这种称作“癌症免疫编辑”的动态过程被认为构成癌症免疫疗法的主要障碍。
在多种免疫逃避机制当中,最近显示白血病和淋巴瘤原代样品经常下调非经典的MHC(主要组织相容性复合物)蛋白ULBP1,所述ULBP1对表达反受体NKG2D的γδT-细胞识别血液肿瘤至关重要(Lanca,T.等人,“TheMHCclassIbproteinULBP1isanonredundantdeterminantofleukemia/lymphomasusceptibilitytogammadeltaT-cellcytotoxicity”,Blood115:2407-2411;2010)。
γδT细胞是占据1-10%健康个体外周血淋巴细胞(PBL)并且能够靶向实验室中所测试的大部分血液肿瘤细胞系的天然样淋巴细胞。然而,已证实许多淋巴样白血病原代样品对充分活化的Vγ9Vδ2T-细胞(γδPBL的优势亚类)有抗性(Lanca,T.等人,“TheMHCclassIbproteinULBP1isanonredundantdeterminantofleukemia/lymphomasusceptibilitytogammadeltaT-cellcytotoxicity”,Blood115:2407-2411,2010;Gomes,A.Q.等人,“IdentificationofapaneloftencellsurfaceproteinantigensassociatedwithimmunotargetingofleukemiasandlymphomasbyperipheralbloodgammadeltaTcells”,Haematologica95:1397-1404,2010)。另外,涉及体内施用Vγ9Vδ2T细胞的激活物的临床试验已经显示有限的成功,客观响应仅限于10-33%的血液瘤或实体瘤患者。由于未曾报道过客观响应,因此甚至更适宜的是涉及过继转移活化/扩充的Vδ2+细胞的试验的结果。实际上,当重复注射入患者时,单纯离体扩充肿瘤在体内试图逃避其监视的自体Vδ2+T细胞可以被判定为治疗作用甚微。
癌症免疫疗法依赖于细胞毒性淋巴细胞识别肿瘤细胞。γδT细胞是在各种动物肿瘤模型中发挥至关重要作用的MHC非限制性杀伤淋巴细胞群体。尽管如此,已显示了大比例的人血液肿瘤对用特定激动剂活化的γδ外周血淋巴细胞(PBL)有抗性,其中所述的特定激动剂针对高度优势的Vγ9Vδ2T细胞受体(TCR)。这可能构成对当前γδ-T细胞介导的免疫疗法的重要限制。
因此,至关重要的是研究这样的策略,所述策略赋予γδT细胞以额外的识别手段以探测抵抗体内存在的天然组分的肿瘤。
天然细胞毒性受体由A.Moretta和合作者在十多年前鉴定,并且显示在天然杀伤(NK)细胞的抗肿瘤功能中发挥关键性协同作用。实际上,NKp30和NKp46被公认为是两种最特异的NK细胞标记。
vonLilienfeld-Toal,M.,J.Nattermann等人撰写的文献(2006)."ActivatedgammadeltaTcellsexpressthenaturalcytotoxicityreceptornaturalkillerp44andshowcytotoxicactivityagainstmyelomacells."ClinExpImmunol144(3):528-533公开了NK受体在外周血γδT淋巴细胞上的表达。然而,公开的γδT细胞不同于本发明中所述的那些。与本发明相反,该文献也揭示NKp30不在这类γδT细胞中表达。其中一些差异是:
·处理(IFN-γ,TNF-α+抗CD3单克隆抗体+IL-1β+IL-2+IL-15);
·γδT细胞的表型(NKp30-和NKp46-;另外,62%的γδT细胞属于Vδ2+亚型;
·少于20%的γδT细胞是CD56+;
·少于20%的γδT细胞是CD8+;
·NKp44的表达水平不受γδTCR/CD3复合物的刺激作用调节(因此,涉及在那些细胞上诱导NKp44表达的不同分子机制)
该文献描述了表达NKp44的外周血γδT细胞。NKp44是有功能的并且参与肿瘤细胞(骨髓瘤细胞)的识别和消除。然而,仅8±7%的γδT细胞表达NKp44,并且大部分的(62%)γδT细胞属Vδ2+亚型(它们是Vδ1-)。NKp30和NKp46不由γδT细胞表达,并且少于20%的这些细胞是CD8+或CD56+。
因此,所述的γδT细胞系完全不同于我们鉴定的γδT细胞系:通常在我们的细胞系中多于95%的γδT细胞属于Vδ1+亚型,并且高百分比的这些Vδ1+γδT细胞(一般多于30%)表达有功能的NKp44。重要地,NKp44显示与NKp30发挥协同作用,以大大增强识别和杀伤肿瘤细胞的能力(图5B)。NCR之间的这种合作对获得所需的效果(消除癌疾病)至关重要,但其并不存在于先前鉴定的NKp44+γδT细胞中。
由Rey,J.,C.Veuillen等人撰写的文献(2009)."NaturalkillerandgammadeltaTcellsinhaematologicalmalignancies:enhancingtheimmuneeffectors."TrendsMolMed15(6):275-284公开NKG2D受体在NK细胞上和γδT细胞上的共同表达。该文献也宣称γδT细胞的扩充和活化可以产生抗肿瘤反应。然而,与本发明相反,这些γδ细胞是NKp30-和NKp46-。
文献WO00/44893(PalmettoRichlandMemorialHos)公开一种基于施用基本上纯化的γδT淋巴细胞来治疗白血病的方法。该方法包括产生淋巴细胞活化及其扩充。主要差异是:
·处理(平板结合的固定化抗TCR抗体+照射的“饲养”肿瘤B细胞);
·γδT细胞(NKp30-、NKp44-、NKp46-、CD8-)。
文献WO2011/053750(埃默里大学)公开了一种减少患者中癌症的方法,所述方法包括步骤:分离细胞毒细胞群体如γδT细胞;施用治疗药和所述细胞毒细胞。其与本发明的主要差异在于γδT细胞经基因修饰而抵抗化疗药。
概述
本发明涉及外周血淋巴细胞系,所述外周血淋巴细胞系包含表达功能性天然细胞毒性受体(NCR)的Vδ1+γδT细胞或由其组成。在一个更优选的实施方案中,所述天然细胞毒性受体包括NKp30或由其组成。
在一个更优选的实施方案中,所公开的细胞系还包含Vδ2+γδT细胞。

在一个更优选的实施方案中,本发明中公开的组合物可以用于治疗病毒性感染。即,当所治疗的病毒来自疱疹病毒科(Herpesviridae)或逆转录病毒科(Retroviridae)之时,等等。
所公开的主题还包括产生所述细胞系的方法,所述方法括分离γδPBL。在一个优选实施方案中,起始样品可以是包括外周血样品的血液或其级分,所述级分包括暗黄层细胞、单个核细胞和低密度单个核细胞。所述细胞可以使用本领域已知的技术如密度梯度离心,从起始血液样品获得。总γδPBL可以采用磁标记的抗TCRγδ+抗体借助阳性选择或通过耗尽/消除TCRγδ(-)细胞进行分离。γδPBL可以维持在任何适合的哺乳动物细胞培养基如RPMI1640或IMDM中。

在所公开的方法的一个更优选的实施方案中,可以实施所述γδTCR激动剂和γc-细胞因子的添加直到至少40%、更优选地至少50%、60%、70%、75%、80%、85%、95%、100%的细胞表达NKp30。

发明概述

发明详述
公开的主题涉及了包含γδT细胞的外周血淋巴细胞细胞系、其组合物及生产方法以及在医学中(即在癌症治疗中)的用途。这些γδT细胞表达Vδ1+T细胞受体(TCR)和功能性天然细胞毒性受体(NCR)。NCR在Vδ1+T细胞中被由γc-细胞因子和Vδ1+TCR协同提供的AKT依赖性信号所上调。
本发明公开了新的Vδ1+PBL亚类,所述亚类能够靶向对充分活化的Vγ9Vδ2PBL有高度抗性的血液学肿瘤。这种Vδ1+群体所拥有的特化杀伤细胞功能归因于天然细胞毒性受体的诱导表达,所述天然细胞毒性受体已经大多被视为NK特异性标记。已显示,虽然Vδ1+或Vδ2+细胞均不以组成型方式表达NCR,但这些NCR可以在Vδ1+细胞中被γc-家族细胞因子和Vδ1+TCR协同提供的AKT依赖性信号所选择性上调。还显示NKp30和NKp44均在NCR+Vδ1+PBL中有功能,并且对它们增强的淋巴细胞性白血病细胞靶向作用作出关键贡献。

在诱导性NCR当中,基于以下事实而言,NKp30对Vδ1+T细胞的抗肿瘤活性是最重要的:表达NKp30的细胞比例(图3D);当NKp30触发时Vδ1+T细胞的细胞毒性有更多增强(图5A);和当阻断NKp30时,白血病细胞杀死显著减少(图5B)。尽管如此,NKp44(而不是NKp46)也在NCR+Vδ1+细胞中有功能(图5A),并且似乎与NKp30协同用于增强的肿瘤靶向作用(图5B)。
NKp30和NKp44均已经涉及人NK细胞对病毒感染细胞的识别。对肿瘤而言,抗体介导的阻断实验展示在骨髓瘤和黑色素瘤细胞靶向方面的重要作用。另外,缺少NCR表达已经在临床上与AML患者中的不良存活关联起来了。
Vδ1+T细胞是胎儿/早期生活期间的优势γδT细胞亚类,此时它们已经能够响应于病毒性感染。在成人中,Vδ1+T细胞扩充已经与CMV感染、HIV-1感染和上皮源来源或造血来源的肿瘤相关。过继转移活化的Vδ1+T细胞的一个诱人方面在于,它们可以显示特别良好的组织归巢能力,因为与它们的循环型Vδ2+对应物相反,Vδ1+细胞是偏好组织结合的淋巴细胞(Groh,Spies,Science1998)。有趣地,粘膜表面处Vδ1+T细胞的丰度已经被归因于IL-15,后者诱导控制TCR基因重排的染色质修饰。

本发明描述了Vδ1+细胞上NCR表达的有效诱导取决于TCR刺激;在TCR不存在的情况下,γc细胞因子可以仅实现非常温和的NCR表达上调(图4A-B)。因此,TCR信号位于的NCR+Vδ1+淋巴细胞的NCR介导肿瘤细胞识别上游。
从临床角度,本发明描述了离体诱导NCR的方法,并且描述了一种非常可行的将大量细胞扩充并注射至患者中的方式。可以通过施用低剂量IL-2(这似乎足以支持NCR表达),潜在地在体内维持这些细胞的活化状态。
PHA/IL-2活化的NKp30+γδT细胞偏好地表达CD11c和CD8。CD11c是通常在其他淋巴细胞中表达的黏附分子,并且已经显示参与肿瘤细胞的识别。
材料和方法
人外周血γδT细胞的分离。外周血采自匿名健康志愿者,用PBS(优选地,InvitrogenGibco)稀释1:1(v/v),并且优选地在Ficol-Paque(优选地,Histopaque-1077,Sigma)以体积比1:3(1份ficol对3份稀释的血液)在1500rpm和25℃时离心30分钟。将含有单个核细胞的界面收集并(在PBS中)洗涤,并且借助正选择(优选地,采用FITC-标记的抗抗体)或借助负选择(采用生物素标记抗体的混合物;优选地,MiltenyiBiotec),通过磁性细胞分选法分离γδT细胞(至高于95%的纯度)。

结果
用泛T细胞促分裂原活化的γδPBL培养物的增强的细胞毒性。

PHA刺激的γδPBL培养物的优越细胞毒功能是一个令人惊讶的发现,因为已显示HMB-PP是高度优势的Vγ9Vδ2PBL亚类的十分强力的激活物(Morita,C.T.,Jin,C.,Sarikonda,G.和Wang,H.2007.Nonpeptideantigens,presentationmechanisms,andimmunologicalmemoryofhumanVgamma2Vdelta2Tcells:discriminatingfriendfromfoethroughtherecognitionofprenylpyrophosphateantigens.ImmunolRev215:59-76)。与HMB-PP活化的γδPBL相比,PHA刺激的培养物显示改进的针对各种抗性白血病细胞系的细胞毒性,如Bv-173、REH或HPB-ALL(图1B-C),所述抗性白血病细胞系已经显示缺少关键性NKG2D配体ULBP1的表达。这些数据表明,泛T细胞促分裂原PHA能够增加中期(1-3周)γδPBL培养物的溶细胞潜力,这可能对过继细胞免疫疗法具有巨大价值。
在用泛T细胞促分裂原活化的γδPBL上诱导NCR表达
研究了PHA活化的γδPBL培养物的增强细胞毒性潜在的机制。以上数据可以由受体如NKG2D或DNAM-1或2B4的差异性表达解释。先前显示这些受体均参与杀伤淋巴细胞对肿瘤细胞的识别。然而,这些候选者均不在PHA-和HMB-PP活化的γδPBL培养物之间差异性表达(图2A)。相反并且出乎意料地,天然细胞毒性受体NKp30(NK细胞细胞毒性的一种重要触发物)在PHA刺激的γδPBL上特异性存在(图2B;图7)。另外,其他NCR家族成员,NKp44和NKp46,也在这些样品中选择性地表达(图2C-D;见下文)。
NKp30+细胞的比例随培养时间稳定增加(图2E),表明NKp30诱导与细胞增殖之间的关联。虽然不太可能归因于新鲜样品中十分低的背景(图2E),但可能的是,组成型方式表达NKp30的某个微小亚类可能在PHA刺激的γδPBL培养物中优先被扩充。为解决这个问题,采用经FACS高度纯化(>99%)的NKp30(-)细胞进行实验,所述试验证实了在PHA+IL-2刺激时,NKp30(-)细胞能够与未分选细胞同样有效地获得NKp30表达(图2F)。另外,在这类条件下,NKp30(-)和NKp30+细胞以相似的程度增殖,这进一步反驳NKp30+细胞在这类条件下的优先扩充。这些结果表明当通过PHA/IL-2处理活化γδPBL时,从头诱导NKp30表达,这与细胞增殖耦合。
NCR由增殖的Vδ1+T细胞选择性表达
考虑到HMB-PP已经证实是Vγ9Vδ2细胞的最佳激动剂,不过与HMB-PP相反,PHA处理偏好地扩充γδPBL当中的Vδ2(-)细胞(图3A)。这不归因于在两种实验条件下Vδ2+细胞凋亡的差异(图8)。如此显著扩充的最可能Vδ2(-)群体(图3A)是Vδ1+细胞,因为其他亚类在健康成人的外周血中极为罕见。当评估Vδ1相比较Vδ2TCR的使用时,在经PHA活化的培养物中发现Vδ1+细胞大幅度富集(19日后,占全部γδT细胞的>80%)(图3B;图9)。相反,并且如所述,经HMB-PP活化的培养物逐渐由Vδ2+细胞占优势(图3A)。
在用PHA+IL-2刺激的分离的Vδ1+或Vδ2+细胞的平行培养物中检查NKp30表达的诱导。尽管新鲜分离的Vδ1+和Vδ2+细胞均不表达NKp30(图10),但是在Vδ1+细胞中而不在Vδ2+细胞中强烈诱导了这种NCR(当PHA+IL-2处理时)(图3C)。另外,通过进行CFSE稀释,证实了NKp30+细胞的惊人积累,伴随Vδ1+细胞的逐步分裂(图3C)。这些数据表明PHA/IL-2培养物中Vδ1+细胞的活化诱导了NKp30表达,同时伴有细胞增殖。
在培养物中2-3周之后通常检测到高百分比(>50%)的NKp30+细胞情况下,而NKp44(约30%)和NKp46(<20%)在较低比例的Vδ1+细胞中表达(图3D)。另外,大部分的NKp44+或NKp46+Vδ1+细胞也表达NKp30(图3D)。因此将NKp30视为诱导性NCR+Vδ1+亚类的提供信息最多的标记。
NCR诱导需要AKT依赖性γc细胞因子和TCR信号
剖析了NCR+Vδ1+T细胞分化所需要的特定信号。首先,拆开活化方案的两个组分IL-2和PHA。IL-2或其相关的γc细胞因子IL-15单独即足以诱导一些NKp30表达,但是与PHA/IL-2(或PHA/IL-15)组合相比较而言这种作用是轻微的(图4A)。在另一方面,PHA单独使用不能够使培养物保持活力,这与γc细胞因子在γδT细胞存活中、尤其当活化/增殖时的关键作用一致。
虽然PHA已经是广泛使用的T细胞促分裂原,但是它也是一种能够交联一系列表面受体(包括TCR)的非生理性化合物。因此,PHA刺激作用的分子介体可能是Vδ1+TCR复合物。比较了PHA和特异性交联TCR复合物的CD3ε链的OKT3mAb(与IL-2或IL-15组合时)在Vδ1+T细胞中诱导NKp30表达的能力。OKT3在这些测定法中完全能够模拟PHA(图4A-B),因此在增殖的Vδ1+T细胞中诱导NKp30(图11)。另外,PHA/IL-2培养物中的TCRγδ阻断作用防止NKp30诱导(图4C)。这些数据表明,PHA处理提供了TCR信号以诱导Vδ1+PBL上的NCR表达。另外,单独或与OKT3(或PHA)组合的细胞因子处理之间的差异凸显出这个过程中γc细胞因子和TCR信号之间的明显协同作用(图4A-B)。
为了进一步探索NCR诱导的分子机制,使用γc细胞因子受体和/或TCR信号传导下游的关键信号转导途径的化学抑制剂。尽管阻断JAK信号传导在任何NCR诱导之前触发了广泛的细胞死亡,但是与PI-3K/AKT抑制剂LY294002的共孵育特异性阻止了增殖的Vδ1+T细胞中的NKp30诱导(图4D)。AKT参与转导γc细胞因子信号和TCR信号,包括TCRγδ信号。相反,MAPK/Erk抑制剂UO126对增殖的Vδ1+T细胞中的NKp30诱导没有可检测的影响(图4D)。重要地,LY294002的选择性影响将NCR诱导与细胞增殖作用拆分开了,因此证实了Vδ1+T细胞增殖是诱导NKp30表达必需的(图3C;图11),但不是足够的(图4D)。综上,这些数据表明AKT依赖性γc细胞因子和TCR信号协同作用以诱导Vδ1+T细胞中的NKp30表达。
有功能的NKp30和NKp44通过Vδ1+PBL触发肿瘤细胞杀伤。
采用功能获得实验和功能丧失实验来评价NCR调节作用对Vδ1+富集(>80%;图9A)的PBL培养物的影响,所述PBL培养物以与图3D中相似的水平表达NCR。首先,通过使用反向Ab依赖性细胞毒性测定法,显示出NKp30或NKp44的交联(而不是NKp46的交联)产生高度明显的P815肿瘤细胞靶溶解增加(图5A)。这些数据证实了诱导的NKp30和NKp44是有功能的并且介导肿瘤细胞杀伤。为了评估它们是否在靶向白血病细胞中发挥非冗余作用,使用NCR特异性mAb进行受体阻断实验。当阻断NKp30和NKp44时观察到肿瘤细胞溶解的显著增加(图5B)。如从图5A中的结果所预期,NKp46阻断不影响肿瘤细胞杀伤。有趣地,还清楚地观察到NKp30和NKp44之间的协同效应。应当指出,在杀伤测定法期间,TCRγδ阻断在任何情形下(单独或与抗NCRmAb组合)均是中性事件(图5B)。这些数据暗示,NCR+Vδ1+PBL对白血病细胞的靶向是一个大多通过NKp30和NKp44的协同功能所介导的TCR非依赖性事件。
NCR+Vδ1+PBL是靶向耐药性原发性淋巴细胞性白血病的特化杀伤细胞。
为了充分表征NCR+Vδ1+PBL的抗肿瘤潜力,将NKp30+细胞经FACS分选成高纯度(>99%)(图6A)并且进行一系列功能测定。如预期(图3D),分选的NKp30+细胞也表达NKp44和NKp46(图6B),而且,当仅用IL-2培养2周时,这三种NCR在纯化细胞的表面上大体上稳定(图6C)。这些数据证实了稳定NCR+Vδ1+T细胞亚类的便利扩充。
当评估NKp30+细胞的细胞毒功能时,观察到对耐药性白血病细胞系Bv173(连同其他)的靶向作用增加(与NKp30(-)对应物相比)(图6D)。这与粒酶B的更高表达相关(图6E)。另外,NKp30表达也与CD56表达的较高程度相关(图12),所述CD56表达先前已经与人淋巴细胞(包括Vδ2+T细胞)的细胞毒性关联。
最后,采用从慢性淋巴细胞性白血病患者获得的原代样品,进行功能性杀伤测定。重要地,HMB-PP/IL-2活化的γδPBL不表达NCR(图2B)。将它们的抗肿瘤溶细胞活性与分离自经PHA/IL-2活化的γδPBL培养物的NCR+细胞比较。观察到从6位不同供体获得的NCR+γδPBL在消除原代B-CLL细胞方面一致地更高效(图6F-G)。
本发明当然不以任何方式限于所述的实施方案,并本领域技术人员能预见到其修改的许多可能性,而不背离如所附权利要求书中所定义的本发明基本构思。
以下权利要求阐述本发明的具体实施方案。
Claims (23)
1.一种淋巴细胞细胞系,其中包含表达功能性天然细胞毒性受体的Vδ1+γδT细胞,其中所述天然细胞毒性受体包括NKp30。
2.根据权利要求1所述的细胞系,其由表达功能性天然细胞毒性受体的Vδ1+γδT细胞组成。
3.根据权利要求1所述的细胞系,其中还包含Vδ2+γδT细胞。
4.根据权利要求1所述的细胞系,其中所述淋巴细胞来自外周血样品。
5.根据权利要求1所述的细胞系,其中所述天然细胞毒性受体包括NKp44。
6.根据权利要求1所述的细胞系,其中所述天然细胞毒性受体包括NKp46。
7.根据权利要求1所述的细胞系,其中包含表达粒酶B的细胞。
8.根据权利要求1-7中任一项所述的细胞系,其中所述细胞系包含80%-95%的表达功能性天然细胞毒性受体的功能性Vδ1+γδT细胞。
9.组合物,其中包含根据权利要求1-8中任一项所述的细胞系的细胞。
10.根据权利要求9所述的组合物,其中所述组合物是可注射用的组合物。
11.根据权利要求1-8中任一项所述的细胞系的细胞或者根据权利要求9或10所述的组合物在制备用于自体或异源过继细胞疗法、肿瘤或癌症治疗、肿瘤或癌症免疫疗法和/或白血病治疗的药物中的用途。
12.根据权利要求11所述的用途,其中所述药物用于治疗急性淋巴母细胞性白血病、急性髓性白血病、慢性髓性白血病、慢性淋巴细胞白血病、多发性骨髓瘤、Burkitt氏淋巴瘤、滤泡淋巴瘤、乳腺癌、肺癌、前列腺癌、结肠癌、膀胱癌、肾细胞癌或皮肤黑色素瘤。
13.根据权利要求11所述的用途,其中所述药物用于治疗病毒性感染。
14.根据权利要求13所述的用途,其中所述病毒来自疱疹病毒科(Herpesviridae)或逆转录病毒科(Retroviridae)。
15.生产权利要求1-8中任一项所述的细胞系的方法,所述方法包括分离γδPBL和在适当的培养基中培育这些细胞,同时有规律添加γδTCR激动剂和至少一种γc-家族细胞因子,直至至少40%的细胞表达NKp30。
16.权利要求15所述的方法,其中γδTCR激动剂是植物凝集素PHA、抗CD3单克隆抗体、抗γδTCR单克隆抗体或其混合物。
17.根据权利要求15所述的方法,其中γδTCR激动剂浓度的范围是0.01-100μg/ml。
18.权利要求15-17中任一项所述的方法,其中所述γc-细胞因子是IL-2、IL-4、IL-7、IL-9、IL-15、IL-21或其混合物。
19.根据权利要求18所述的方法,其中γc-细胞因子浓度的范围是1-10000U/mL。
20.根据权利要求19所述的方法,其中γc-细胞因子浓度的范围是100-1000U/mL。
21.根据权利要求15-17中任一项所述的方法,其中添加所述γδTCR激动剂和γc-细胞因子的时间范围是2-60日。
22.根据权利要求21所述的方法,其中添加所述γδTCR激动剂和γc-细胞因子的时间范围是9-25日。
23.根据权利要求22所述的方法,其中添加所述γδTCR激动剂和γc-细胞因子的时间范围是10-15日。
