Abstract
本发明公开了一种新生儿肺炎的辅助诊断试剂盒及应用,涉及免疫细胞检测领域。该试剂盒包括检测如下免疫功能指标的试剂:γδT细胞比例、Vδ1+细胞比例、Vδ2+细胞比例、Vδ1+NKP30+细胞比例以及Vδ1细胞/Vδ2细胞比值,通过检测免疫功能指标的试剂实现对T细胞以及NK细胞的数目检测,从而为诊断新生儿肺炎的发生提供参考依据。
Description
技术领域
本发明涉及免疫细胞检测领域,具体而言,涉及一种新生儿肺炎的辅助诊断试剂盒及应用。
背景技术
新生儿肺炎是新生儿留院治疗的最常见病因之一,也是造成新生儿死亡的重要原因。新生儿肺炎特别是在早产儿中具有较高的发病率,是造成新生儿死亡的重要病因之一。统计数据显示,2015年中国新生儿肺炎导致的死亡数估计为5400例,占活产的0.3‰,占5岁以下儿童总死亡的3%。
按发病时间,新生儿肺炎可分为先天性、早发性(<7d)和晚发性(7d以上)三种。其中早发性肺炎(Early-onset pneumonia,EOP)可能与围产期感染等因素相关,而绒毛膜羊膜炎及FIRS(fetal inflammatory response syndrome)是其主要危险因素。这两者又可引起胎膜早破及早产,进一步加重新生儿患早发性肺炎的可能。
由于新生儿呼吸道并发症与非传染性呼吸道并发症的共同存在,很难准确诊断新生儿EOP,此外,使用细菌或病毒培养法检测时间长,检测准确度低,很可能导致诊断延迟。目前,临床上对EOP的治疗一般都是采用抗生素或抗病毒药物,但是超过7天使用这些药物将导致新生儿免疫功能发育和肠道微生物群建立受损。
鉴于此,特提出本发明。
发明内容
本发明的第一目的在于提供一种新生儿肺炎的辅助诊断试剂盒。该检测试剂盒可以及早的辅助诊断出新生儿肺炎的发生。
本发明的第二目的在于提供一种检测免疫功能指标的试剂在制备新生儿肺炎辅助诊断试剂盒中的应用。
本发明是这样实现的:
新生儿肺炎的辅助诊断试剂盒,其包括检测如下免疫功能指标的试剂:γδT细胞比例、Vδ1+细胞比例、Vδ2+细胞比例、Vδ1+NKP30+细胞比例以及Vδ1细胞/Vδ2细胞比值。
免疫功能指标具体为:γδT细胞占淋巴细胞的比例、Vδ1+占γδT细胞的比例、Vδ2+占γδT细胞的比例、Vδ1+NKP30+细胞占Vδ1细胞的比例和Vδ1/Vδ2细胞比值。
通过检测γδT细胞占淋巴细胞的比例可以实现对血液中γδT细胞水平的检测,肺炎患者γδT细胞占淋巴细胞比例下降,则预测可能有早发性肺炎的发生。若同时满足如下条件:Vδ1+占γδT细胞的比例下降、Vδ2+占γδT细胞的比例上升、Vδ1+NKP30+细胞占Vδ1细胞的比例升高和Vδ1/Vδ2细胞比值降低,则诊断有新生儿早发性肺炎发生。
在本发明应用较佳的实施例中,上述试剂包括如下抗体:
抗CD3抗体、抗γδ抗体、抗Vδ1抗体、抗Vδ2抗体和抗NKP30抗体。
CD3分子通过盐桥与T细胞抗原受体(TCR)相连,参与T细胞的信号转导,主要用于标记胸腺细胞、T淋巴细胞及T细胞淋巴瘤。采用抗CD3抗体作为检测试剂盒中的其中一种单克隆抗体,可以对T细胞的水平进行监测,从而为新生儿肺炎的发生提供参考依据。
γδT细胞是执行固有免疫功能的T细胞,其TCR由γ和δ链组成。此类细胞主要用于杀伤癌细胞与协助DC细胞识别发现癌细胞抗原,然后将这些抗原进行杀伤或是传递给其他细胞。采用抗γδ抗体作为检测试剂盒中的其中一种单克隆抗体,可以对抗原的水平进行监测,从而为新生儿肺炎的发生提供参考依据。
NKP30是NK细胞表面的一种重要的活化性受体,可接收相应配体如BAG-6、B7-H6等分子信号进而激活NK细胞,发挥抗肿瘤和抗感染免疫作用。同时NKP30也是肿瘤细胞和病原体发生免疫逃逸的重要靶点,可通过多种机制逃逸NK细胞杀伤。此外,NKP30还参与了NK细胞和树突状细胞(DCs)间的双向免疫调节。采用抗NKP30抗体作为检测试剂盒中的其中一种单克隆抗体,可以对NK细胞的水平进行监测,从而为新生儿肺炎的发生提供参考依据。
在本发明应用较佳的实施例中,上述抗体带有荧光标记,优选地,荧光标记选自APC-Cy7、APC-R700、AF647、BV510、PE、BV421、BB515、BV605、PE-Cy7、BB700、Alexa Fluor350-790、Cy3、Cy5、Dylight 488-680、PE-Alexa Fluor、PE-Cy5和PE-Cy7中的任意一种。
通过荧光标记单克隆抗体,可以实现不同免疫功能指标的流式细胞仪检测分析。
在本发明应用较佳的实施例中,上述新生儿肺炎是指新生儿早发性肺炎。
在本发明应用较佳的实施例中,上述检测免疫功能指标的试剂在制备新生儿肺炎辅助诊断试剂盒中的应用。免疫功能指标包括:γδT细胞比例、Vδ1+细胞比例、Vδ2+细胞比例、Vδ1+NKP30+细胞比例以及Vδ1细胞/Vδ2细胞比值。
免疫功能指标具体为:γδT细胞占淋巴细胞的比例、Vδ1+占γδT细胞的比例、Vδ2+占γδT细胞的比例、Vδ1+NKP30+细胞占Vδ1细胞的比例和Vδ1/Vδ2细胞比值。
通过检测γδT细胞占淋巴细胞的比例可以实现对血液中γδT细胞水平的检测,肺炎患者γδT细胞占淋巴细胞比例下降,则预测可能有早发性肺炎的发生。若同时满足如下条件:Vδ1+占γδT细胞的比例下降、Vδ2+占γδT细胞的比例上升、Vδ1+NKP30+细胞占Vδ1细胞的比例升高和Vδ1/Vδ2细胞比值降低,则诊断有新生儿早发性肺炎发生。
检测免疫功能指标的试剂包括如下抗体:

在本发明应用较佳的实施例中,上述抗体带有荧光标记;优选地,荧光标记选自APC-Cy7、APC-R700、AF647、BV510、PE、BV421、BB515、BV605、PE-Cy7、BB700、Alexa Fluor350-790、Cy3、Cy5、Dylight 488-680、PE-Alexa Fluor、PE-Cy5和PE-Cy7中的任意一种。
检测方法包括用流式细胞仪对待测样本的如下免疫功能指标进行检测分析:γδT细胞占淋巴细胞的比例、Vδ1+占γδT细胞的比例、Vδ2+占γδT细胞的比例、Vδ1+NKP30+细胞占Vδ1细胞的比例以及Vδ1/Vδ2细胞比值。通过流式细胞仪可以对γδT细胞中的各细胞亚群进行分析。
在本发明应用较佳的实施例中,上述方法包括将待测样本与试剂盒中的多个单克隆抗体混合后,在4-6℃下避光孵育10-30min,再进行流式细胞仪的检测分析。通过孵育,保证血液中的抗原与检测试剂盒中的单克隆抗体结合,从而有利于后续的流式细胞仪检测。
在本发明应用较佳的实施例中,上述制备方法还包括将抗体储存在-80℃-4℃。
抗体若为粉末剂则可-20℃保存,且使用前按使用者需求加入适量磷酸盐缓冲液(PBS)溶解,获得需要的浓度即可。若为液体制剂,则不需溶解,可以直接使用。
本发明具有以下有益效果:
本发明通过新生儿肺炎的辅助诊断试剂盒中的试剂,可以实现对T细胞以及NK细胞的数目检测,从而为诊断新生儿肺炎的发生提供参考依据。本发明还提供了检测免疫功能指标的试剂在制备新生儿肺炎辅助诊断试剂盒中的应用,从而及早的辅助诊断出新生儿肺炎的发生。

具体实施方式
为使本发明实施例的目的、技术方案和优点更加清楚,下面将对本发明实施例中的技术方案进行清楚、完整地描述。实施例中未注明具体条件者,按照常规条件或制造商建议的条件进行。所用试剂或仪器未注明生产厂商者,均为可以通过市售购买获得的常规产品。
以下结合实施例对本发明的特征和性能作进一步的详细描述。
实施例1
从市场上购买获得带荧光标记的抗人CD3抗体,抗人γδ抗体,抗人Vδ1抗体、抗人Vδ2抗体和抗人NKP30抗体。
将带荧光标记的单克隆抗体抗Vδ1抗体、抗Vδ2抗体和抗NKP30抗体分装于不透光的棕色塑料小管或标准抗体试剂管中,每管装一种单克隆抗体。根据使用次数分装对应剂量的抗体,抗体试剂管的规格为0.5-2mL。分装后,置于4℃保存。
实施例2
本实施例提供了人外周血单核细胞的制备方法,具体包括如下步骤:抽取出生后第7天的新生儿外周血1mL,将1mL全血在1500rpm离心10min后,取离心管底部的细胞成分加入5倍体积FACS裂解液(BD,USA),溶血处理10min后,将上述细胞裂解液置于1500rpm离心10min,采用PBS洗涤细胞2次备用。
实施例3
本实施例提供了试剂盒的使用方法。首先对分离的人外周血单核细胞(PBMC)进行染色标记,从每个抗体管中吸取1μl抗体,与PBMC混合,置于4℃下避光孵育30min后,在流式细胞仪上检测分析T细胞,γδT细胞及NK细胞的占比。
实验例1
本实验例中,取16例早发性肺炎患儿作为实验例,16例早发性肺炎患儿是2018年6月至10月从暨南大学附属宝安区妇幼保健院新生儿科收集的住院患者,结合起病时间(<出生后7d)、临床症状及胸片进行诊断。30例对照来自同时期在本新生儿科住院的未感染肺炎的健康人。所有研究对象均为中国籍。在研究对象监护人签署知情同意书后,从电子病历采集研究对象性别、出生孕周、出生体重、APGAR评分、肺炎发病时间、抗生素使用情况、辅助呼吸情况,住院时长等信息。
所有研究对象于出生后第7d采集外周血1ml进行免疫检测。本项目得到暨南大学附属宝安区妇幼保健院医学伦理委员会的批准。对研究对象的γδT细胞中各细胞亚群进行分析。
实验结果参照图1所示,由图1可知,相比于对照组,图1中的A图显示,肺炎患者的γδT细胞占T淋巴细胞的比例下降(P=0.008),在γδT细胞中,图1中的B图,C图和D图显示,患者的Vδ2+细胞比例显著上升,而Vδ1+细胞比例显著下降,Vδ1/Vδ2比值显著降低,且均呈极显著水平(P<0.001)。此外,图1中的E图显示,NKP30阳性的Vδ1细胞在Vδ1细胞中的比例显著上升(P=0.006)。
实验例2
本实验例对实验例1中16例早发性肺炎患者以下5个免疫指标与疾病严重程度指标的关系进行了关联性分析:γδT细胞占淋巴细胞的比例、Vδ1+占γδT细胞的比例、Vδ2+占γδT细胞的比例、Vδ1+NKP30+细胞占Vδ1细胞的比例以及Vδ1/Vδ2细胞比值。其中疾病严重程度指标包括抗生素使用时间(antibiotics,days)、辅助呼吸时间(assistedventilation,days)以及住院时间(hospital stay,days)。
分析结果参照图2所示,由图2可知,随着Vδ2+占γδT细胞的比例的上升对应需要更长时间的抗生素治疗,更长的辅助呼吸时间以及住院时间,呈正相关;随着Vδ1+占γδT细胞的比例下降也需要更长时间的抗生素治疗,更长的辅助呼吸时间以及住院时间,呈正相关;Vδ1/Vδ2细胞比值下降也需要更长时间的抗生素治疗,更长的辅助呼吸时间以及住院时间,呈正相关;只有Vδ1+NKP30+细胞占Vδ1细胞的比例在患者体内上升的条件下,仅需要较短的抗生素治疗,较短的辅助呼吸时间以及住院时间,呈负相关;
上述结果显示,本发明提供的5个细胞参数指标的变化可以用于新生儿肺炎的预后跟踪等临床应用。
实验例3
根据实验例2中的5个指标参数的变化,进一步通过小数据量的临床检测,验证了该5个指标用于临床判断新生儿肺炎免疫功能状态的可行性。新入住了3个健康新生儿以及6个临床诊断为肺炎的新生儿,通过对9例新生儿进行5个指标分析,结果显示这5个指标在肺炎患儿中均显著异常。数据参照图3-图7所示,6例诊断为肺炎的新生儿Vδ1+占γδT细胞的比例(图4)和Vδ1/Vδ2细胞比值(图7)均显著低于3例健康新生儿;而6例诊断为肺炎的新生儿Vδ2+占γδT细胞的比例(图5)和Vδ1+NKP30+细胞占Vδ1细胞的比例(图6)均显著高于3例健康新生儿,6例诊断为肺炎的新生儿γδT细胞占淋巴细胞的比例(图3)与3例健康新生儿无显著差异,这与实验例1和实验例2的结果相对应。本实验例的验证数据表明5个免疫细胞指标可以作为临床辅助判断新生儿免疫功能的免疫学指标。

1.检测免疫功能指标的试剂在制备新生儿早发性肺炎辅助诊断试剂盒中的应用,其特征在于,所述免疫功能指标包括:γδT细胞占淋巴细胞的比例、Vδ1+占γδT细胞的比例、Vδ2+占γδT细胞的比例、Vδ1+NKP30+细胞占Vδ1细胞的比例和Vδ1/Vδ2细胞比值,所述试剂盒的检测样本为外周血。
2.根据权利要求1所述的应用,其特征在于,所述试剂包括如下抗体:
抗CD3抗体、抗γδ抗体、抗Vδ1抗体、抗Vδ2抗体和抗NKP30抗体。
3.根据权利要求2所述的应用,其特征在于,所述抗体带有荧光标记。
