发明背景
疼痛是促使病人寻求医疗护理的最常见原因之一,疼痛也因此导致每年有大量的工作日丧失。大体上有三类疼痛:伤害性疼痛、神经性疼痛以及心因性疼痛。图1说明了不同类型疼痛和受影响的病症之间的关系,诸如痛觉异常(allodynia)和多发性硬化。

神经性疼痛是由神经、脊髓或大脑的异常引起的,并且是一种慢性类型的非恶性疼痛,估计人群的发病率超过1%。在帮助病人恢复控制他或她的生活方面,优化这些病人的止痛是至关重要的。
神经性疼痛的最常见的原因是神经的损伤或功能障碍。周围神经或起源于脊髓的神经的损伤或功能障碍导致脊髓处神经脉冲抑制解除,从而导致疼痛。在诸如脊髓损伤和多发性硬化的病症中,神经性疼痛也可由中枢介导,而不是周围神经介导的。
因此神经性疼痛可进一步细分成两类:周围神经性疼痛和中枢神经性疼痛,这取决于是周围还是中枢神经系统受到影响。
患有周围神经性疼痛的病人常常体会到感觉像烧伤或触电般疼痛的疼痛,而其他人描述他们的疼痛为感觉像极端寒冷或四肢发麻。该疼痛可因活动或在将衣服穿在受影响区域而恶化。该疼痛也可能遵循每日模式(daily pattern),其可意味着它在一天的某些时间恶化。
痛觉异常是一种周围神经性疼痛的类型。这是对一种典型的非疼痛刺激的疼痛反应,例如用指尖掠过该受影响的区域。随着反复刺激该疼痛会有增强趋势并从该受影响的区域扩散。痛觉异常可因对皮肤、关节、骨骼、肌肉或内脏或者静态或者动态地施加的化学、热力(冷或热),或机械的低强度或高强度的刺激的响应而诱发。据认为痛觉异常的存在是一种比导致神经性疼痛的具体疾病更适于给患有周围神经性疼痛的病人进行分组的方法。
很明显,患有神经性疼痛的病人的生活质量可受到它的极大影响。疼痛会干扰工作和社会活动,以及病人体会到的睡眠数量和质量。对减轻神经性疼痛的成功治疗会改善病人体会的疼痛的量以及改善病人的生活质量二者。
使用药学药物是对神经性疼痛进行的最常见治疗。止痛剂、抗抑郁药和抗惊厥药是使用中的常见药物类型。药物卡马西平(carbamezepine),其为一种抗惊厥药,是当前唯一的具有神经性疼痛适应症的FDA认证药物。在过去的市场研究中已经提出病人服用卡马西平时血液病的危险性有五到八倍的增加。在7%的病人中,已经显示他们的白细胞计数下降了25%。
将大麻属用作药物早已是众所周知的了,并且在19世纪期间,大麻属的制剂被推荐为用于治疗癔症、谵妄、癫痫、神经性失眠、偏头痛、疼痛、痛经的催眠镇静药。
直到近代,只能通过煎煮大麻属的制法(其然后可吞食,或由病人通过抽吸干燥植物材料的方式吸入大麻属的汽化物,可实现对病人的大麻属给药。近来的方法已经力图去寻找新的方式来将大麻素递送给病人,包括绕开胃部,及与肝脏相关的首过效应的那些方式(其会除去多达90%的有效摄入剂量),并避免病人不得不将不健康的焦油和相关的致癌物质吸入到他们的肺中。
包含特殊的、限定比例的大麻素类化合物的制剂可由与药学媒介和赋形剂组合的纯的、合成的或分离的大麻素类化合物。或者由源自大麻属植物的提取物配制。
大麻素类化合物是一组已知激活细胞中大麻素受体的化学制品。植物大麻素是源自大麻属植物的大麻素类化合物。内源性大麻素类化合物(Endocannabinoids)是在人体或其他动物体内发现的内生大麻素类化合物。所述植物大麻素可从植物中分离或经合成生产。当从植物分离植物大麻素时,可将其纯化至除去所有的天然存在的化合物,诸如其他次要的大麻素类化合物和植物分子诸如萜烯的程度。该纯化结果导致高于该目标大麻素类化合物的99% (w/w)的纯度。
先前已经表明,作为纯净的化合物给予的大麻素大麻二酚(CBD)可以部分地缓解神经性疼痛(Costa et al., 2004)。为了说明这点,使用大鼠坐骨神经的慢性压迫性损伤的神经性疼痛模型并测试具有热力和机械痛觉过敏和机械性痛觉异常的试验样品的效果。使用这些动物模型来预测试验化合物对神经性疼痛的效果。
最近申请人在他们的被授予的英国专利GB2439393中已经表明,包含限定比例的CBD:THC的植物提取物在治疗周围神经性疼痛上比单用纯化的成分更加有效。CBD与THC的有效比例在20:1-28:1之间。
专利申请PCT/GB2006/004063描述了大麻属的提取物的用途,其中THC和CBD的比例为大约1:1。发现该提取物在治疗以疱疹后神经痛为特征的周围神经性疼痛中是有利的。
神经性疼痛通常与多种复杂的疼痛刺激组合有关,因而难以有效地治疗,因为对治疗的响应是无法预测的。
申请人意外地发现,服用大麻素类化合物大麻萜酚(CBG)、大麻环萜酚(CBC)、次大麻二酚(CBDV)和四氢次大麻二酚(THCV)可有效地治疗神经性疼痛的动物模型。由于对常规的止痛治疗(诸如阿片剂和非甾体抗炎药)而言,神经性疼痛似乎是难以治疗的,在此描述的动物研究代表了对于临床情况(clinical settings)的重要发现。
公开概述
根据本发明的第一方面,提供一种或多种植物大麻素:大麻环萜酚(CBC)、大麻萜酚(CBG)、次大麻二酚(CBDV)和/或四氢次大麻二酚(THCV),其用于治疗神经性疼痛。
根据本发明的第二方面,提供一种或多种植物大麻素:大麻环萜酚(CBC)、大麻萜酚(CBG)、次大麻二酚(CBDV)和/或四氢次大麻二酚(THCV),在制备用于治疗神经性疼痛的药物中的用途。
根据本发明的第三方面,提供了一种治疗患有神经性疼痛的病人的方法,包括服用有效量的一种或多种植物大麻素,所述植物大麻素选自:大麻环萜酚(CBC)、大麻萜酚(CBG)、次大麻二酚(CBDV)和/或四氢次大麻二酚(THCV)。
这些大麻素类化合物可以同THC和/或CBD联合使用。
所述神经性疼痛优选为周围神经性疼痛,更优选周围神经性疼痛为痛觉异常。
大麻素类化合物优选以有效减轻神经性疼痛的剂量存在。
将大麻素类化合物的小剂量定义为低于10mg的大麻素类化合物的有效人体每日剂量,和将大麻素类化合物的大剂量定义为50mg或50mg以上的大麻素类化合物的有效人体每日剂量。将中等剂量定义为在10mg与50mg之间。
大麻素类化合物的有效人体每日剂量优选为在5mg与100mg之间。更优选大麻素类化合物的有效人体每日剂量在10mg与50mg之间。还更优选大麻素类化合物的有效人体每日剂量在12mg与24mg之间。
优选将所述大麻素类化合物进行包装,以供在延长的治疗期中使用。延长的治疗期将是更优选的,更优选多于3天,还更优选延长的治疗期为至少7天。
在本发明的进一步的实施方案中,一种或多种植物大麻素与一种或多种其他目前已批准用于治疗神经性疼痛的药用物质联合使用。这些药用物质包括例如:镇痛药、抗抑郁药和抗惊厥药。

详细描述
周围神经性疼痛是由在不同位置和不同疾病状态下引发许多不同机制的多种病因产生的。坐骨神经分支损伤模型(Spared nerve injury,SNI)由坐骨神经的局部结扎和交织(transaction)组成,其在典型的临床表现由热痛觉过敏和触觉痛觉异常所代表的神经性疼痛中演化而来。
慢性疼痛症状是通过使用足底试验装置(Ugo Basile, Varese, Italy)对热觉感受(thermoceptive)反应(其代表热痛觉过敏)以及用动态足底触觉计(Ugo Basile, Varese, Italy)在机械缩足反应阈值(其为机械性痛觉异常的指示)的变化来测试的。在对不同处理的各组小鼠(媒介或不同组合的药物)手术之前及之后测试伤害感受性反应。
下面的实施例详细描述使用四种不同的先前并未证实为具有减轻神经性疼痛的能力的大麻素类化合物所获得的结果,即CBG、CBC、CBDV 和THCV。将这些大麻素类化合物与先前已证实可用于治疗神经性疼痛的大麻素CBD进行比较。
实施例1:在两个神经性疼痛的动物模型中大麻素类化合物的效果
材料和方法
试验的大麻素类化合物有CGB、CBC、CBDV和THCV。此外还使用了大麻素CBD,因为先前已经证实了这种大麻素类化合物在用于该实施例的神经性疼痛的动物模型中的积极成果。所述大麻素类化合物由已经净化的大麻属植物的全植物提取物制备。将所述大麻素类化合物溶于林格氏溶液/0.5%二甲亚砜(DMSO)中,用于腹腔内(i.p.)给药。
在试验开始之前,在控制照明(12:12小时的亮:暗循环;6:00开灯)和环境条件(室温20-22℃,湿度为55-60%)下,将雄性CD-1小鼠(35-40g)置于室内至少1周,每笼3只。小鼠可以随意获取食物和自来水。试验程序经那不勒斯第二大学伦理委员会的批准。基于试验性研究中对动物的使用和保护,遵照IASP和欧共体(E.C. L358/1 18/12/86)的指导来进行动物护理。尽一切努力将动物的痛苦减至最小并减少所使用的动物数量。
在手术前进行行为测试以建立一个基线用于同手术后的数值相比较。根据Bourquin and Decosterd (2006)的方法诱发单神经病。
用戊巴比妥钠(60 mg/kg i.p.)麻醉小鼠。将右后肢固定在侧卧位并稍稍抬高。使用股骨作为标志在大腿中部进行切割。使坐骨神经在大腿中部三根分叉部的远侧暴露,并去除结缔组织;不拉伸神经结构而将坐骨神经的三个周围分支(腓肠神经、腓总神经和胫神经)暴露出来。
将胫神经和腓总神经结扎并交织在一起。轻巧地将带有弯形尖头的微型手术镊置于胫神经和腓总神经的下面以滑动神经周围的纤维(5.0丝, Ethicon, Johnson, and Johnson Intl, Brussels, Belgium)。对两个神经进行紧密的结扎。通过避免任何神经伸展或神经与手术工具接触来小心地保护腓肠神经。用丝5.0缝合线使两个不同的层面中的肌肉和皮肤相靠近。
强烈、可再生和持久的热痛觉过敏和机械痛觉异常样行为在未损伤的腓肠神经皮肤范围内都是可以测量的。所述SNI模型提供了一种独特的解剖学分布的优点,不需要将损伤处远侧的受损神经和未受损神经纤维相混合(co-mingling),诸如受损和未受损的神经和范围可以很容易的分辨并操作以做进一步的分析(即行为评估)。
所述假手术包括相同的手术,但不将神经进行结扎和交织。
小鼠的分组可如下划分,测试的每种大麻素类化合物都有其自身的小鼠组群:
i) 幼稚对照小鼠(n = 8);
ii) 用媒介处理的假手术小鼠(n = 8);
iii) 用大麻素类化合物治疗的假手术小鼠(n = 8);
iv) 用媒介处理的SNI小鼠(n = 8);
v) 用大麻素类化合物治疗的SNI小鼠(n = 8)。
每天给予大麻素类化合物,持续14天。所测试的所有大麻素类化合物的剂量是:2.5和5.0 mg/Kg。媒介溶液是林格氏溶液中0.5%的DMSO。
感受伤害性行为
通过使用动态爪触觉测试仪(Anesthesiometer) (Ugo Basile, Varese, Italy)测量机械痛觉过敏。允许小鼠在置于金属网表面的围栏的两隔室中一个隔室内自由活动。在进行任何测量之前,使小鼠适应试验环境。之后,通过自动化测试设备从试验箱的地板下面向小鼠后爪的趾面传递机械刺激。用电子提升力(10秒内0-30g)推动一根钢杆(2mm)。当该动物缩回其后爪时,该机械刺激自动撤销,所记录的力最接近0.1g。
通过使用足底试验装置(Ugo Basile, Varese, Italy)评估热痛觉过敏。在试验当天将每只动物放在带有玻璃地板的塑料笼(22cm x 17cm x 14cm; 长 x 宽 x 高)中。经过60分钟的适应期后,将该后爪的趾面暴露于透过该玻璃地板的热辐射束中。热辐射源由红外灯泡(Osram卤钨灯泡;8v,50w)组成。一个光电池检测从爪子处发射的光,并在爪子移动中断该反射光时将灯关掉。缩爪潜伏期自动地显示为最接近0.1秒;截止时间为20秒以防止组织损伤。
对热敏感性和机械敏感性疼痛伤害反应表示为热刺激缩足反应潜伏期(PWL) (以秒计)和机械刺激缩足反应阈值(PWT) (以克计)。
每只小鼠用作其自我的对照,在外科手术之前及之后都要进行反应测试。PWL和PWT由对治疗不知情的观察者来量化。
统计分析
行为和分子数据显示为均数± S.E.M. ANOVA,而后进行Student–Neuman–Keuls事后试验,以确定各组间的统计显著性。P<0.01被认为在统计上是有显著意义的。
结果
机械缩足反射阈值
图2、4、6、8和10展示了对测试的大麻素类化合物(CBC、CBG、CBDV、THCV及CBD (比测仪)各自)所获取的机械缩足反射阈值数据。这些直方图展示了使该动物缩回其后爪所需要的重量(以克计)。正如所能看到的,所有测试的大麻素类化合物都能允许患有SNI的动物在收回它的爪子之前增加所施加的重量数,且同样地能够在坐骨神经分支损伤之后3天、7天及14天时在不同程度上防止机械痛觉过敏。镇痛效果具有剂量依赖性,因为在接受5.0 mg/kg剂量大麻素类化合物的动物中观察到更好的效果。
热缩足反射潜伏期
图3、5、7、9和11展示了对试验的大麻素类化合物(CBC、CBG、CBDV、THCV及CBD (比测仪)各自)所获取的热缩足反射潜伏期数据。这些直方图展示了在动物将其爪子从热源缩回之前的时间(以秒计)。正如所能看到的,所有测试的大麻素类化合物都能够允许患有SNI的动物在缩回其爪子之前增加时间量,且同样地能够在保留神经损伤之后3天、7天及14天在不同程度上防止热痛觉过敏。
对于大麻素类化合物CBC、CBG、THCV和CBD而言,效果不表现为剂量依赖性的,因为用2.5和5 mg/kg治疗的动物具有相似的热缩足反射潜伏期,或者在更低的剂量上达到最大效果。
然而对于大麻素CBDV可观察到剂量依赖性效果。在神经损伤后的第3天用2.5mg/kg剂量治疗的动物具有与对照组相似的缩足反应潜伏期。然而,用5.0mg/kg剂量治疗的动物在将爪子从热源缩回之前能够增加时间量至在神经损伤后14天时达到的程度,即这组与幼稚和假手术对照动物具有相似的缩足反应潜伏期。
数据组合
为了确定所测试的大麻素类化合物的不同程度的效果,下表在表格形式中描述了这个实施例中生成的数据。
下面的表1描述用大麻素类化合物治疗对小鼠机械缩足反射阈值的效果。

如在上表1中所能看到的,大多数剂量为2.5mg/kg的大麻素类化合物在动物缩回其爪子之前表现出稍微的重量增加,从第3天到第7天再到第14天,这种效果似乎随着时间的推移而增加。然而使用2.5mg/kg剂量的大麻素CBDV甚至会在第3天的时间点极大地增加机械缩回潜伏期,意味着这种大麻素类化合物能够迅速地产生效果,而其他大麻素类化合物花费一周或更长时间才能变得有效果。
在5.0mg/kg剂量下除了THCV之外的所有大麻素类化合物都能够在动物缩回其爪子前增加所施加的重量。大麻素CBC和CBD在第7天表现出大的增长;然而这种潜伏期在第14天的时间点再次下降。
这些数据惊奇地证明在这两种试验剂量下的所有大麻素类化合物中,大麻素CBDV显示出具有最高的机械缩回潜伏期。大麻素CBG和THCV也显示出好的效果,因为它们具有相当高的机械缩回潜伏期。然而大麻素CBC和CBD在这个剂量下对治疗神经性疼痛是相对无效的。这个发现证明大麻素CBDV、THCV和CBG在其治疗有这项实验中使用的动物模型导致的神经性疼痛的能力上胜过CBD。
下面的表2描述了在小鼠的热缩足反射潜伏期上用大麻素的治疗效果。

正如从表2中所能看到的,大多数的大麻素类化合物在2.5 mg/kg的剂量下显示在动物缩回其爪子之前的时间量上稍有增加,这种效果随着时间从第3天到第7天再到第14天出现增加。在第3天的时间点仅有2.5mg/kg剂量的CBG在热缩足反射潜伏期的量上似乎有所增长。对于大麻素THCV,在第7天的时间点上似乎有大的增加,随后在之后的7天后下降。
在5.0mg/kg的剂量下,大麻素CBC、CBG和CBD能够在第三天的时间点增加在动物缩回其爪子之前的时间量。然而用大麻素治疗一周之后,所有的试验组都显示出热缩足反射潜伏期增加。经过14天的治疗之后THCV和CBDV二者都显示出大的增加,意味着大麻素类化合物的效果随着时间的推移而增强。
这些数据惊人地证明了在2.5mg/kg的剂量下,在所有大麻素类化合物中,大麻素CBG显示具有最好的热缩足反射潜伏期。大麻素CBDV和THCV也表现出好的效果,因为它们有着相当高的机械缩足反射潜伏期。然而显示大麻素CBC和CBD在这个剂量下治疗神经性疼痛是相对无效的。在5.0mg/kg的剂量下,显示出大麻素CBDV和THCV二者都是最有效的。这个发现证明了大麻素CBDV、THCV和CBG在治疗由这次实验中使用的动物模型导致的神经性疼痛的能力上胜过CBD。
可以使用下面的公式来估算人体剂量当量(HED):
对于一只小鼠的Km是3,和对于一只大鼠的Km是6,及对于一个人的Km是37。
因此,对于一个大约60kg的人,小鼠2.5mg/kg的剂量将等效于人的大约12mg的每日剂量。
Claims (10)
1.用于治疗神经性疼痛的一种或多种大麻素类化合物:大麻环萜酚(CBC)、大麻萜酚(CBG)、次大麻二酚(CBDV)和/或四氢次大麻二酚(THCV)。
2.如权利要求1要求的一种或多种大麻素类化合物,其中所述大麻素类化合物是大麻萜酚(CBG)、次大麻二酚(CBDV)和/或四氢次大麻二酚(THCV)。
3.如先前权利要求中任一项要求的一种或多种大麻素类化合物,其中所述神经性疼痛是周围神经性疼痛。
4.如权利要求3要求的一种或多种大麻素类化合物,其中所述周围神经性疼痛是痛觉异常。
5.如先前权利要求中任一项要求的一种或多种大麻素类化合物,其中所述大麻素类化合物以减轻神经性疼痛的有效人体日剂量存在。
6.如权利要求5要求的一种或多种大麻素类化合物,其中所述大麻素类化合物的有效人体日剂量在5mg与100mg之间。
7.如权利要求5或6要求的一种或多种大麻素类化合物,其中所述大麻素类化合物的有效人体日剂量在10mg与50mg之间。
8.如先前权利要求中任一项要求的一种或多种大麻素类化合物,其中所述大麻素类化合物经包装供在延长的治疗期间使用。
9.如权利要求8要求的一种或多种大麻素类化合物,其中所述延长的治疗期为至少7天。
