Abstract

背景技术
在支架植入处采用各种结构方案的分枝支架的脉管内植入的现有技术系统通常会带来伤害,尤其是在动脉分枝处。在分枝支架的植入中最频繁的问题包括,由于植入的支架的撑条覆盖内腔以及发生血栓集中,而导致动脉侧向分支的部分或完全闭合、动脉侧向分支的收缩、脉管分枝处的切开、动脉侧向分支的横截面减小。
为了避免这样的状况,当前时每个分支植入一个支架,同时单独的系统用于每个植入支架的脉管内植入,或者植入一个安装在复杂系统上的叉形支架。
利用现有技术的方法进行分枝支架的脉管内植入操作的实施是极为复杂的,并且其负担在于具有支架的动脉存在多种后操作并发症的极高可能性。它们必须在利用非常现代的设备进行的外科手术中实施。此外,进行植入的操作者需要具有高技术水平和非常丰富的经验。
发明内容
因此,需要用于分枝支架的新型的、更加高效的结构方案,以及用于这种支架的脉管内植入的系统,以便即使是经验不足的操作者也能方便在分枝部位处的植入,并且提供较高的安全性和操作功效。
本发明的目的在于增加功效和方便植入程序,从而增加操作的安全性、促进更加广泛地使用支架、并提出能够在囊状物上扩展的更加通用的支架构造,囊状物允许其植入在具有不同形状和尺寸的脉管中以及相对于主脉管具有不同相互结构的分支的脉管中。
从国际申请WO0174273A1中已知一种由例如镍和钛合金的材料制成的自延伸支架,其包括两个区段——近侧区段和远侧区段,其中一个区段具有较大的直径,这些区段通过至少一个纵向连接器连接。该连接器(或定位成彼此靠近的若干连接器)是直线形的,并且仅仅定位在支架的一侧处,从而在支架的两个区段之间的另一侧处形成自由空间。在植入脉管内的情况下,支架的连接器附着到脉管的内壁,并且其弯曲成使得支架的各区段改变其相互位置(它们之间存在角度),它们之间的自由空间自由地增加。

异形囊状物2包括三个不可分离的部分:较小直径的远侧部分2.1;较大直径的近侧部分2.2;以及具有限定长度的中间部分2.3,其位于远侧部分2.1和近侧部分2.2之间。囊状物的远侧部分可以比囊状物的近侧部分长或短。囊状物的两个还可以大致相等。囊状物的中间部分被调节到支架的远侧部分和近侧部分之间的距离,并且其长度可以为从0.5到8mm,例如从0.5到3mm或从2到4mm,或从3到8mm。
异形囊状物2的远侧部分2.1的长边缘与中间部分2.3的边缘之间具有角度θ。分枝支架1以可分离的方式夹持在异形囊状物2上,使得具有分枝支架1的扩大表面区域1.4的支架单元与异形囊状物2的中间部分2.3相一致,支架1.1的远侧部分安装在囊状物2.1的远侧部分上,支架1.2的近侧部分安装在囊状物2.2的近侧部分上。
在用于分枝支架的脉管内植入的系统的内部管5上紧固有至少一个环7,优选三个环6、7、8,在X射线下是可见的,使得环6与异形囊状物2的近侧部分2.2的长边缘的起始部分相一致,环7与异形囊状物2的远侧部分2.1的长边缘的起始部分相一致,环8与异形囊状物2的远侧部分2.1的长边缘的末端部分相一致。支架以可拆卸的方式张紧在系统的囊状物上,使得环6与支架1的近侧边缘相一致,环8与支架1的远侧边缘相一致,环7与支架的远侧部分1.1的近侧边缘相一致,换言之,环7与支架的远侧部分的边缘相一致,位于具有扩大表面区域1.4的支架单元的一侧处。
用于本发明的支架的脉管内植入的系统被引入到身体内,使得引导线材P2被递送到定位在动脉的主分支内的分枝部位。
然后,用于本发明的支架的脉管内植入的系统在引导线材P2上被引导。
用于支架的脉管内植入的系统的内部管5被置于引导线材P2上。
然后,当在引导线材P2上移动用于支架的脉管内植入的整个系统,而同时通过X射线控制三个环6、7、8的位置时,分枝支架1与异形囊状物2一起被非常精确地定位在分枝部位处:该系统在引导线材P2上被引入到动脉的主分支中,使得环7将处于与脉管的交叉拱部(龙骨瓣)所处的相同的高度。借助于控制标记7的位置,能够将支架1的远侧部分的近侧边缘精确地定位成与脉管的交叉拱部(龙骨瓣)对齐。
这样引入用于分枝支架的脉管内植入的系统使得分枝支架的远侧部分1.1在主分支中位于动脉分叉之后,分枝支架的近侧部分1.2位于动脉分叉之前,支架的中间部分与具有特定扩大表面区域1.4的支架单元一起位于侧向分支的内腔中。
然后,流体在6到20atm的压力下被引入到异形囊状物2,这使得异形囊状物2膨胀,由此打开分枝支架1。打开的分枝支架1使脉管的内腔变宽,从而消除收缩。
在分枝支架1完全打开的情况下,其中间部分与具有特定扩大表面区域1.4的支架单元一起精确地定位在侧向分支的内腔中,这允许血液自由流过动脉的侧向分支。
然后,从异形囊状物2移除流体,从血液循环系统收回具有空异形囊状物2的整个系统。最后,收回引导线材P2。打开的分枝支架1保持在动脉存在收缩的部位处,从而使得该部位永久变宽。
Claims (7)
1.一种分枝支架,其包括两个圆柱形部分:较小直径的远侧部分(1.1)和较大直径的近侧部分(1.2),该远侧部分和近侧部分通过相对布置的两个纵向连接器(1.3)连接,这两个连接器具有的长度(L1)为0.5到8mm,并且形成具有扩大表面区域(1.4)的支架单元,连接器具有曲线形状,尤其是正弦曲线形状。
2.根据权利要求1所述的支架,其中支架的远侧部分(1.1)的直径与近侧部分的(1.2)的直径的比在1.0:1.1到1.0:2.0的范围内。
3.一种用于分枝支架的脉管内植入的系统,包括近侧部分和远侧部分,该系统包括外部管、内部管和异形囊状物,其中在系统的远侧部分中,在标记有在X射线下可见的三个环(6、7、8)的内部管(5)上具有该异形囊状物(2),该异形囊状物的近侧部分(2.2)紧固到外部管(4)的外表面,该异形囊状物的远侧部分(2.1)紧固到内部管(5)的外表面,根据权利要求1所述的分枝支架(1)以可拆卸的方式夹持在异形囊状物(2)上,异形囊状物(2)包括三个不可分离的部分:较小直径的远侧部分(2.1);较大直径的近侧部分(2.2);以及长度为从0.5到8mm的中间部分(2.3),该中间部分位于远侧部分(2.1)和近侧部分(2.2)之间,环(7)的位置与夹持在囊状物(2)上的支架(1)的远侧部分(1.1)的近侧边缘相一致。
4.根据权利要求3所述的系统,其中异形囊状物(2)的远侧部分(2.1)的直径与近侧部分(2.2)的直径的比在1.0:1.1到1.0:2.0的范围内。
5.根据权利要求3或4所述的系统,其中异形囊状物(2)的远侧部分(2.1)的长边缘与异形囊状物(2)的中间部分(2.3)的边缘之间具有角度θ,该角度处于15°到65°的范围内。
6.根据权利要求3、4或5所述的系统,其中在分枝支架(1)以可分离的方式夹持在异形囊状物(2)上的情况下,分枝支架(1)的具有扩大表面区域(1.4)的单元与异形囊状物(2)的中间部分(2.3)相一致。
7.一种借助于包括分枝支架的脉管内系统引入和定位分枝支架的方法,其中在分枝部位处将引导线材(P2)引入到脉管的主分支,在这种情况下,在分枝部位处,引入包括如权利要求1所述的支架(1)的如权利要求3所述的系统,在X射线下可见的环(7)表明夹持在囊状物(2)上的支架(1)的远侧部分(1.1)的近侧边缘的位置被精确地定位在脉管的分叉拱部所处的部位处,从而在主分支中,支架(1)的远侧部分(1.1)位于动脉分叉之后,支架(1)的近侧部分(1.2)位于动脉分叉之前,支架的中间部分与具有特定扩大表面区域(1.4)的支架单元一起定位在脉管的侧向分支的内腔中,然后在6到20atm的压力下将流体引入到囊状物(2),植入支架(1),从囊状物(2)移除流体,收回该系统和引导线材(P2)。
