Abstract

本申请为申请日为2005年11月15日、申请号为200580039281.0、发明名称为“用于分子成像的超声造影剂”的发明专利申请的分案申请。
本发明涉及用于分子成像的新型超声造影剂(UCA)以及其成像方法。
在过去的15年中,已经开发了大量安全和实用的超声造影剂(UCA),诸如增强多普勒信号的充气微泡,以及封装了壳的液滴(Hall C.S.等人(2000)J.Acoust.Soc.Am.108(6),3049-3057)。
理想的超声造影剂应当具有尽可能多的下列特征:
-在血液中稳定和充分的寿命,例如,允许在30分钟或更久的时间内检测靶器官;
-微粒的大小小于8微米,以便使它们能够通过毛细血管;
-无毒,或者毒性可接受;
-充分的反射增强;
-易于生产和临床应用;
-允许高度特异性定向(targeting)。
而且,超声造影剂应当优选可与诸如飞利浦超声成像系统的现有超声成像系统一起使用。
已经开发了包括金属或具有磁性的金属氧化物的不同微粒,用作用于磁共振成像(MRI)的造影剂。US2002/0136693描述了用于诊断目的的制剂,其包含磁性微粒,所述磁性微粒包括磁性双金属氧化物(double metaloxide)/氢氧化物或磁性金属或者任选的络合剂。US2003/0082237描述了毫微粒(nanoparticle),其由块状共聚缩氨酸(block copolypeptides)或同聚物聚电解质(homoplymer polyelectrolyte)构建成具有泡囊的内层和外层的球体。毫微粒的外层或内层可以包括金属或金属氧化物,其任选用于部位选择性医学成像。
金属毫微粒可以用作超声造影剂。尤其对靶向金属毫微粒感兴趣,由于它们的组织特异性导致了较高的局部超声造影剂浓度,因而增加的反射增强,以及因而具有用这些稳定剂获取分子信息的特性。非靶向金属毫微粒超声造影剂将不在所需组织内积聚,且它们的浓度将不会高到以较低的频率可以检测到。通过使用较高的频率,可以获得反射增强的显著提高,如图2、3和4中所示。然而,到目前为止,高频不能用于例如器官研究,这是由于如表1中所示的穿透深度限制。

增加金属毫微粒的浓度,将增强超声的反射率,尤其在超声辐射具有足够的穿透深度的相对低频处。专利WO 02/11771和Bekeredjian等人(2002),Ultrasound Med.&Biol.28(5),691-695)描述了结合了金的微管(gold-bound microtubule)作为超声造影剂的可能应用。与常规造影剂(微泡)相比,这种结合了金的微管显示了造影活性更长的持久性。然而,绝对强度通常较低。因此,需要包括金属毫微粒的备选化合物,其导致金属毫微粒的局部高浓度。
本发明的目的是提供用于分子成像的备选超声造影剂(UCA)及其成像方法。本发明的优点是,提供了导致金属毫微粒局部高浓度的化合物。

由于人体组织的强声阻抗差别,与基质微粒相关的大量金属毫微粒是声反射器,并且相对于当前商业UCA具有优点,例如,微泡稳定并且可以以与当前靶向造影剂相同的方式被修改。
使用本发明的群集的金属毫微粒,可以在例如1至20MHz频率获得足够的反射增强。其通常用于器官或组织的超声成像。
而且,通过用治疗剂涂覆基质或者通过将治疗剂封装在基质中,可以将本发明的基质微粒用于药物的输送。

而且,根据本发明的特定方面,包括大量金属毫微粒的基质微粒也可以使用其他成像方法检测,允许使用本发明的微粒用于组合的成像技术。
本发明的另一方面是成像或诊断方法,包括将根据本发明的造影剂施予动物或人类患者,并且执行该动物或人类的超声成像检查。作为选择,根据本发明的另一方面,造影剂被施予动物或人类组织,用于体外(ex vivo)诊断。
本发明涉及包括大量金属毫微粒的基质微粒在超声造影剂中的使用,以及超声造影剂的制备和设计。
用作血流中造影剂的基质微粒优选具有小于10微米的直径,例如8微米、特别地约3微米或者低至约1微米。应当具有穿透血管壁的能力的造影剂的直径优选在纳米范围,例如,本发明提供了具有直径在250nm和1nm之间、而且更优选的在100和25nm之间的基质微粒。直径低于25nm的基质微粒在体内将具有短的滞留时间,适用于短滞留时间很重要的应用。在本发明的一方面,该微粒优选具有足够的体内滞留时间,允许在它们降解和/或被人体排出之前,超声造影剂进行定向和/或执行对患者的超声检查。根据本发明的基质微粒中的金属毫微粒直径在1至100nm之间,优选小于50nm,更优选30nm或更小。不认为金属微粒的形状是关键性的或者是对本发明的限制。任何规则(例如,球形,多边形等)或不规则的形状都是可使用的。基质微粒的一些形状允许当定向组织时的较高的组装密度(packing density),例如,细长微粒将允许基本上比圆球形更低的组装密度。类似地,不认为在基质中金属微粒的微粒大小分布是关键性的或者是对本发明的限制,尽管在一些应用中,某些大小的范围将具有优势。已经描述了用于生产毫微粒的不同方法,包括在溶液中的成核作用(即,化学合成)和蒸汽冷凝或火焰(flame)或雾化(spray)技术(Gutsch等人(2002)KONA20,24-34;Axelbaum(2001)Powder Metall.43(3),323-325),然而,更最近描述的激光消融、流动液体上的真空蒸发(VERL)以及化学气相沉积(CVD)的,也是合适的。另外或作为选择地,通过过滤或离心可以获得合适大小的毫微粒分布。本发明中有用的任何用于将固体研磨为微粒大小的常规方法都可使用。
包括大量本发明的金属毫微粒的基质微粒的重要特征是它们的声阻抗,其致使它们适用作超声造影剂。声阻抗(Z)定义为介质中密度(ρ)和声速(c)的积(Kinsler等人,1982,Fundamentals of acoustics.3rd edition,Johon Wiley and sons,New York)。本发明的金属毫微粒的声阻抗应当显著高于人体组织的声阻抗,大多数人体组织的声阻抗在13-1.7×105g/cm2s的范围中,典型平均值为1.58×105g/cm2s。
本发明提供了,本发明的金属毫微粒具有至少35×105g/cm2s、更特别地至少50×105g/cm2s的声阻抗。最大声阻抗不是本发明的限制性因子,而设想大约为100-120×105g/cm2s。
具有在上述范围内的声阻抗的金属的范例是例如金、银、铂、钯、钨或钽、铼,或者它们的混合物,或者金属的合金,诸如铂和铱的合金(参见用于所选数目的金属的表2),所述金属适用于结合在本发明的基质中。

用于金属毫微粒的金属优选是化学稳定和无毒性或者已经由于合适的涂层呈现化学稳定的金属。在这点上,尤其感兴趣的是将合适的声阻抗特征与稳定性和无毒性或有限的毒性相结合的金属。在其他应用中,其中金属微粒被嵌入或合成在基质中,因此具有高声阻抗的毒性金属可以用于活体内应用。根据一个实施例,该金属是贵金属。根据本发明的特定方面,该金属是非磁性的。
包括大量本发明的金属毫微粒的基质微粒,可以用作各个靶向金属毫微粒层的备选。其还提出了WO 02/11771的蛋白质-金属集合体的备选,在此,通过使组装的蛋白质与金属微粒或金属盐接触而使微粒相关。正在获取的集合微粒具有不规则形状和大小,并且导致金属微粒的不均匀分布。结果,获得了金属微粒的低密度。本发明允许产生具有定义的大小、形状和基质微粒的受控分布和浓度的基质微粒。使用本发明的基质微粒,可以达到更高的微粒密度,并且因此增强了多级反射(superior reflection)。本发明的基质微粒提供了造影剂的改进的和可调整的化学和生物稳定性。另外,本发明的基质微粒易于生产,并且允许提高的工艺窗口(window)。而且,用某些化学或生物组,例如激素类似物、用于受体的缩氨酸模拟配体(peptidemimicking ligand),其允许器官、组织或细胞的特异性定向,可以有效地修改本发明的基质微粒。
为了用单层半径25nm的金属毫微粒覆盖半径0.5微米的圆形表面(约3.14平方微米),需要半径25nm的约360个毫微粒以覆盖所述圆形表面。[圆形面积/毫微粒面积=π5002/π252=785398/1963=400,圆形面积/正方形毫微粒面积=π5002/502=785398/2500=314,六边形组装:(圆形面积·六边形组装密度)/毫微粒面积=(π5002·0.9069)/π252=(785398·0.9069)/1963=362]。当使用半径15nm的金属毫微粒时,将需要约1000个微粒以覆盖所述圆形表面。根据本发明,当金属毫微粒群集在半径0.5微米的基质微粒中时,上述范例的约360个金属毫微粒(r=25nm)占据了大约5%的体积。上述范例的约1000个微粒(r=15nm)占据了约3%的体积。
通过增加基质微粒中金属毫微粒的比例,与上述单层相比,可以获得每单位表面金属毫微粒的更高密度。因而,本发明的一个实施例涉及包含至少2%、至少5%、至少10%、至少20%或者甚至至少50%(体积/体积)的金属毫微粒的基质微粒。
本发明上下文中的“基质”,指的是大量金属毫微粒可以位于其中并且被限制运动的材料。基质可以是固体材料(刚性的或挠性的),但也可以是液体。
液体的范例是诸如全氟化碳乳剂的乳剂,如在US 20040115192中所述。基质微粒可以是均匀的,也可以是不均匀的。基质可以具有有序结构,但这不是必需的。基质可以是渗透的,或者中空的。根据所需流变学特性,可以使用具有某一密度的基质材料。同样地,可以设想包括气泡的基质,以便于调整包括金属毫微粒的基质微粒的密度,尤其当在基质微粒中存在高浓度的具有高密度的诸如金的金属时。
包括大量金属毫微粒的基质微粒指的是不同的布置,其中金属毫微粒分布在基质中,或者附着于基质。其范例,而不局限于此,是:
-金属毫微粒,其位于基质中。这些可以共价地或非共价地结合到基质,可以由基质的成分包围,或者位于基质的小孔中(图7a)。

-金属毫微粒,群集在微泡造影剂的壳内,或者附着于微泡造影剂的壳的外侧或内侧(图7c)。
-金属毫微粒,分散成液滴,其由聚合物、脂类、表面活化剂或甚至蛋白质的壳来稳定住(图7d)。这些稳定的液滴可以自行使用,例如当作为超声造影剂定向时。通过在液滴的壳内、上或之下添加金属毫微粒,可以获得反射增强的增加。在该实施例中,根据本发明的基质是液滴的分散。本发明的液滴具有纳米或最终的微米尺寸,具有与人体组织相比的低声不匹配。同样,与现有技术微泡相比,本发明的液滴具有增加的寿命。
根据本发明,大量金属微粒与生物相容和/或生物可降解基质相关联,以便于使金属毫微粒群集。在本领域中已经描述了适用于此目的的基质,并且其包括天然和合成碳水化合物、脂类或生理上可容忍的合成聚合物(包括适体),表面活化剂、含水的或有机的液体或其混合物或衍生物。
碳水化合物包括天然和合成结构多聚糖,诸如果胶和果胶片段,诸如聚半乳糖醛酸、糖胺多糖(glycosaminoglycan)和类肝素,例如肝磷脂、乙酰肝素(heparan)、角质素、皮肤素、软骨素和透明质酸,葡聚糖、纤维素和海洋生物多聚糖,诸如藻酸盐、角叉胶和壳聚糖,以及它们的衍生物。
可以使用作为基质的合成聚合物包括,但不局限于聚丙烯酸酯、聚乙烯吡咯烷酮、聚酰胺、聚酯、聚乙二醇和聚苯乙烯。而且,还设想多嵌段共聚物的基质,诸如多嵌段聚乳酸(PLA)、聚乙醇酸(PGA)、聚酸酐、聚磷腈或聚已内酯(PCL)。根据特定实施例,该金属毫微粒首先在一个上述材料上设置涂层,并且随后与相同材料或另一材料的基质聚集。
包括金属毫微粒的基质也可以具有脂类特性。脂类指的是合成或天然产生的化合物,其基本上是两性分子并且生物相容的。该脂类通常包括亲水成分和疏水成分。典型的脂类包括,例如,脂肪酸、中性脂肪、磷脂、糖脂、表面活性剂(表面活化剂)、脂肪醇、蜡、萜和类固醇。
脂类可以布置成微团(micelle),成为由脂类配制(formulate)的胶质实体。在某些实施例中,微团包括单层或六边形H2相位结构。在其他实施例中,微团可以包括双层结构。脂类也可以布置成泡囊。这些是基本上特征在于存在形成一个或多个内部空间的一层或多层壁或膜的球形实体。泡囊的一个范例是,包括脂类配制的壁或膜的那些。在这些泡囊中,脂类可以是单层或双层的形式,并且单或双层脂类可以用于形成一层或多层单或双层。在多于一个单层或双层的情况下,单或双层可以是同心的。脂类可以用于形成单室(unilamellar)泡囊(包括一层单层或双层),少数室(oligolamellar)泡囊(包括约二或三个单层或双层),多室(oligolamellar)泡囊(包括多于三个单层或双层)。脂类也可以布置成脂质体。这些基本上是两性分子化合物、脂类化合物的球形群集或集合体,通常以一层或多层同心层、例如双层的形式。在此,它们也可以称作脂类泡囊。脂质体可以从例如离子脂类和/或非离子脂类配制。从非离子脂类形成的脂质体也称作“类脂质体”(niosomes)。

因而,根据本发明的术语,根据金属毫微粒的定位,泡囊的内腔和/或壁是基质微粒。

任选地,基质微粒还涂覆由提供给它们亲水涂层的材料,以最小化血液成分的摄取量和/或对微粒-细胞相互作用的空间屏障,以便于最小化肝的摄取量。这种材料的一个范例是已知为tetronic908(US 4904497)的嵌段共聚物。
如US 6165440中所述,超声波可以用于获取对肿瘤血管的穿孔,间质组织中的微对流,和/或对癌细胞膜的穿孔。根据该原则,包括大量本发明的金属毫微粒的基质微粒可以用于获取将大分子治疗剂增强地输送到癌细胞中,而具有对正常组织的最小的热和机械损伤。
使用本发明的基质微粒,可以在较低的频率获得足够的反射增强,所述较低的频率通常用于人体中较深的器官或组织的超声成像。使用本发明的基质微粒,用约22MHz的频率执行超声成像,其允许7dB的反射增强。用30nm相当于50nm的银层的的群集银毫微粒层发现7dB的反射增强,如范例3中所描述的。群集毫微粒的较厚层,因而同样较大的基质微粒,以及具有较高声阻抗的金属,将更加增强反射率。
可以设想本发明的基质微粒的不同组合,诸如大量基质微粒,其包括相同的金属但是金属毫微粒的大小不同或者金属毫微粒的浓度不同;包括不同金属的毫微粒的混合物的基质;不同成分和/或形状的基质微粒的混合物,以及它们的其他组合。

用于肠内施予的配方可以在大范围内变化,正如本领域所已知的。一般而言,这种配方包括在水溶液或悬浮液中的诊断上有效的金属毫微粒量。可以使用糖浆、药酒等,其中使用了甜媒介物。作为选择,该配方可以是药片、糖衣丸、栓剂或具有云母和/或碳水化合物载体或粘合剂的胶囊等,该载体优选是乳糖和/或玉米淀粉或马铃薯淀粉。
对于肠外应用,尤其适合的是可注射消毒溶液,优选为油类或水溶液,以及悬浮液、乳剂或者植入物,包括栓剂。安瓿是方便的单位剂量。包括包含大量金属毫微粒的基质微粒的造影剂优选用于肠外应用,例如,作为可注射溶液。
本发明的诊断成分以常规方式用于超声过程。该诊断成分施予足够提供合适的显像的量,给热血动物或者系统化或局部地施予要成像的器官或组织,然后该动物接受医疗诊断过程。这种剂量可以广泛变化,这取决于使用的诊断技术以及要成像的器官。本发明的造影剂通常包括每升1微摩尔至1摩尔、优选0.1到100毫摩尔金属,并且通常剂量为每千克体重0.001至100微摩尔、优选0.1至10微摩尔金属。它们可以肠内和肠外地施予哺乳动物,包括人类。通常,诊断测量在施予之后约5-30分钟开始。
根据本发明的特定实施例,本发明的诊断成分用于成像,即显像组织样本或体外器官中的组织结构或靶分子,其在已经完全或部分地从动物或人体分离的组织样本或器官上。
设想本发明的造影剂的使用在宽范围的应用中,包括已经在本领域中描述用于造影成像的所有应用,例如,但不局限于显像和诊断组织、其部分或其中的结构,例如,作为示踪剂。例如,造影成像用于显像心血管系统,例如,壁运动分析、心肌灌注,识别梗塞区域或心肌中缺血,识别血凝块,或者肝脏,例如肝功能,检测肝肿瘤。设想的造影成像的其他应用包括,但不局限于,显像胃肠道、显像肿瘤、识别睾丸和卵巢扭转、评估肾脏和其他移植器官,生理压力、以及造影剂指导和控制的局部药物输送。
根据一方面,本发明的诊断成分用于不同成像方法中的组合使用。因而,根据它们的特征,包括大量本发明的金属毫微粒的基质微粒可能适用于X射线分析。因而,本发明的特定实施例涉及用于组合成像方法中的诊断成分。

范例1-作为频率的函数,在具有与普通人类组织相同声特性的材料之上的50nm金层和250nm PFO层的反射增强预测。
层的反射增强可以使用数学模型进行计算:

“r(k)”是不可压缩材料的振幅反射系数,
“t”是介质1(例如水)、介质2(超声造影层/剂)和介质3(例如,底物)之间的合成透射系数,
“r”是介质1(例如水)、介质2(超声造影层/剂)和基质3(例如,基底)之间的复反射系数(complex reflection coefficient)(参见图1),
“12”指示介质1(例如水)和2(例如金层)之间的界面和声音从1到2的方向。
“k”是造影层中超声波的波数。
“d”是造影层的厚度。
并且增强是20.log.(|r(k)|/|r0|) [等式2]
其中“r(k)”是不可压缩材料的振幅反射系数,r0是无造影剂的底物表面的振幅反射系数。
金属微粒不是生物可降解的,并且因此这些微粒应当足够小以通过肾排出。认为70nm是上限。PFO(全氟化碳)造影剂液滴通过不同的通路,并且微粒将溶解并通过肺时消失。在这些范例中,在实际尺寸为250nm的PFO微粒和较小的50nm金微粒之间进行了比较。
如对于250nm全氟化碳乳剂液滴层计算的增强符合对于具有脾组织的声特性(例如1.6×105g/cm2s)的材料之上的这种微粒层所观察到的超声反射增强,所述材料的声阻抗非常接近人类组织的普通声阻抗,例如1.58×105g/cm2s。
图2示出了血凝块、或者具有与普通人类组织相同的声特性的另一材料之上的,与250nm液体-全氟化碳、脂类-封装的毫微粒乳剂层相比,50nm的Au层的理论计算的反射增强,其作为频率的函数。该图标指示了50nm金层的反射增强仍然高于液体-全氟化碳的250nm层的反射增强。
范例2-具有与普通人类组织相同声特性的材料之上的,50nm铂层、50nm钨层、50nm金层和50nm钽层的理论预测的反射增强,其作为频率的函数。
使用上面在范例1中所述的公式,如图3中所示,计算作为频率的函数的血凝块或者具有与普通人类组织相同声特性的另一材料之上的,50nm铂层、50nm钨层、50nm金层和50nm钽层的反射增强。铂、钨和钽,正如金,由于都具有高密度和高纵向速度,因而具有与人体组织不同的高声阻抗(表3),获得了高反射增强,所以是良好的声反射器。

图4中示出了层厚和频率与血凝块或者具有与普通人类组织相同声特性的另一材料之上的金造影层的反射增强的相关性。这些结果指示,增加频率(减小波长)导致了反射增强增加。增加金属层厚导致反射增强增加。由于超声的穿透深度取决于换能器的频率,高频(获得较高反射增强)不能用于人体内较深的器官和其他组织的医学超声成像。
增加反射增强的另一途径可以通过显著增加金属微粒的直径而获得。在图5中,相对于金和银的液体-全氟化碳层的反射增强示为其层厚的函数。基于壳-封装的全氟化碳液滴的PFO,现有技术造影剂,例如用于吸收层途径,用作与金属毫微粒造影剂的对照。这些计算指示了,诸如银和金的具有高声阻抗的金属的反射增强,相同层厚情况下,具有比PFO造影剂更高的反射增强。
然而,不可接受的是使用显著较大的金属微粒,例如250nm金属微粒,由于这种较大的微粒将不能通过肾过滤而排出,并且将积聚在体内。
范例3-在聚合物底物上测量群集银毫微粒的反射增强。
本范例示出了小金属毫微粒的群集对直径在70nm之上的金属微粒提供了可接受的备选方案。
群集的银毫微粒沉积在2微米的聚合物底物上。银的量用光点直径为5mm的X-射线光电子能谱仪(XPS)测量,并且显示在表4中。

装配有22MHz换能器的Taberna Pro Medicum的数字超声成像系统用于测量具有和不具有30nm的群集银毫微粒的聚合物底物的反射。具有群集的银毫微粒的2μm聚合物底物的和2μm聚合物底物的累积反射强度(峰区),作为增益的函数,显示在图6中。30nm的群集银毫微粒使聚合物底物的反射率增强了7dB。
Claims (13)
1.一种用于医学诊断和成像的造影剂,包含大量金属毫微粒,其中,所述大量金属毫微粒被封装在非蛋白质的生物相容或生物可降解的基质微粒中和/或附着于非蛋白质的生物相容或生物可降解的基质微粒上,所述基质微粒的基质从由以下物质组成的组中选择:碳水化合物、脂类、合成聚合物、水成液、表面活化剂和有机液体,或者它们的混合物,其中所述金属毫微粒直径在1和100nm之间,特征在于所述金属毫微粒在所述基质中具有至少2%体积/体积的浓度。
2.根据权利要求1所述的包含大量金属毫微粒的造影剂,其中,所述基质是泡囊的壳。
3.根据权利要求1或2所述的造影剂,其中,所述金属毫微粒具有至少35.105g/cm2s的声阻抗。
4.根据权利要求1至3中任意一项所述的造影剂,其中,所述金属毫微粒具有50.105g/cm2s以上的声阻抗。
5.根据权利要求1至4中任意一项所述的造影剂,其中,所述金属毫微粒直径在1和50nm之间。
6.根据权利要求1至5中任意一项所述的造影剂,其中,所述金属是非磁性的。
7.根据权利要求6所述的造影剂,其中,所述非磁性金属从由以下物质组成的组中选择:金、银、铂、钯、钨或钽、铼,或者它们的混合物。
8.根据权利要求1至7中任意一项所述的造影剂,其中,所述金属为贵金属。
9.根据权利要求1至8中任意一项所述的造影剂,其中,所述基质微粒直径在1和8微米之间。
10.根据权利要求1至8中任意一项所述的造影剂,其中,所述基质微粒直径在25和250纳米之间。
11.包含大量金属毫微粒的非蛋白质基质微粒在制造超声造影剂中的用途,其中所述金属毫微粒直径在1和100nm之间并且所述金属毫微粒在所述基质中具有至少2%体积/体积的浓度,其中所述基质从由下列物质组成的组中选择:碳水化合物、脂类、合成聚合物、水成液和有机液体,或者它们的混合物。
12.权利要求1至10中任意一项所述的造影剂在制备用于通过执行超声成像检查而获取关于动物或人类患者的信息或诊断的组合物中的用途。
13.权利要求1至10中任意一项所述的造影剂在制备用于通过执行超声成像检查而对器官的离体组织样本进行成像的组合物中的用途。
