Abstract

技术领域
本发明涉及治疗糖尿病的方法。更特别地,本发明涉及通过施用有效量的载脂蛋白A-IV来治疗二型糖尿病的方法。
技术背景
糖尿病的发生是分布广泛的,美国大约有8%的人群患有糖尿病。糖尿病是一种特征在于由于身体不能有效产生和/或利用胰岛素而引起的高血糖的慢性疾病。糖尿病可以导致多种身体并发症,包括但不限于,肾衰竭、失明、神经损伤、心脏病、睡眠窒息和乳糜泻病。例如,在美国,糖尿病是肾衰竭、失明、截肢、中风和心脏病发作的主要诱因。此外,在美国,糖尿病是死亡的第六大原因,并且已经显示出缩短中年成人的预期寿命约五至十年。
糖尿病的最常见形式是二型糖尿病(下文中称为“T2DM”)。T2DM的特征在于高血糖症、胰岛素抵抗、β-细胞功能异常和失调的肝糖原生成。患有T2DM的人发生了葡萄糖刺激的胰岛素分泌的丧失,这与在胰岛素分泌的第一阶段中储存的胰岛素颗粒从β-细胞的释放受损有关。在胰岛素分泌的第二阶段中,患有T2DM的人发生了逐渐丧失响应于葡萄糖刺激而活跃地合成胰岛素的能力。

发明概述

本发明的实施方案涉及治疗需要的受试者的T2DM的方法和用于治疗T2DM的药物组合物。在一个实施方案中,公开了治疗糖尿病的方法。在一个特定的实施方案中,公开了治疗需要的受试者的T2DM的方法,其中该方法包括将有效量的载脂蛋白A-IV(下文中称为“apo A-IV”)或其生物活性类似物施用于受试者。
在一个实施方案中,治疗T2DM的方法有效地降低受试者的血糖水平。在一个特定的实施方案中,该方法有效地将受试者的血糖水平降低约20至50%。在进一步的实施方案中,该方法有效地将受试者的血糖水平降低约40%。在再进一步的实施方案中,该方法有效地将血清水平基本上恢复至正常水平。

在一个实施方案中,全身性施用apoA-IV或其生物活性类似物。apoA-IV或其类似物的全身性施用选自口服、皮下、静脉内、肌内和腹膜内施用。
在另一个实施方案中,公开了一种药物组合物。在一个特定的实施方案中,药物组合物包含apoA-IV或其生物活性类似物。在另一个实施方案中,apoA-IV或其类似物配制用于施用于受试者以治疗T2DM。在该特定的实施方案中,还提供了用于治疗需要的受试者的T2DM的方法,其中该方法包含将有效量的药物组合物施用于受试者。

载脂蛋白A-IV或其生物活性类似物可以是糖基化人或未糖基化的。实施例11中描述了重组未糖基化人和小鼠载脂蛋白A-IV的制备。全长野生型载脂蛋白(SEQ ID NO.1)的多核苷酸显示为图18中的SEQ IDNO.4。实施例1-10中使用的载脂蛋白是未糖基化的。可以根据分子生物学领域中的已知方法来制备apoA-IV。例如,可以通过传统分子克隆技术来制备apoA-IV。

在一个特定的实施方案中,药物组合物可以进一步包含药物学上可接受的载体。药物学上可接受的载体包括该领域中常用的广泛多样的已知稀释剂(即,溶剂)、填充剂、增量剂、粘合剂、悬浮剂、崩解剂、表面活性剂、润滑剂、赋形剂、润湿剂等。药物组合物优选是含水的,即,是液体制剂,并且优选包含无热原水。根据药物制剂的形式,这些载体可以单独或结合使用。如果需要,所得到的制剂可以掺入一种或多种药物制剂领域中广泛使用的稳定剂、缓冲剂、防腐剂、着色剂、香料、调味剂等。
载脂蛋白A-IV或其生物活性类似物可以配制成选自片剂、胶囊、颗粒剂、丸剂、注射液、溶液、乳液、悬浮液和糖浆的剂型。药物组合物的形式和施用途径不受限制,并且可以适当地选择。例如,片剂、胶囊、颗粒剂、丸剂、糖浆、溶液、乳液和悬浮液可以口服施用。此外,注射液(例如,皮下、静脉内、肌内和腹膜内)可以单独静脉内施用或结合含有葡萄糖、氨基酸和/或其他物质的常规补充物一起施用,或可以单独地肌内、皮内、皮下和/或腹膜内施用。
可以根据这种类型的药物领域中的已知方法,使用药物学上可接受的载体来制备本发明的用于治疗T2DM的药物组合物。例如,根据已知的方法,使用赋形剂如蔗糖、乳糖、葡萄糖、淀粉、甘露糖醇等;粘合剂如糖浆、阿拉伯树胶、山梨糖醇、黄芪胶、甲基纤维素、聚乙烯吡咯烷酮等;崩解剂如淀粉、羧甲基纤维素或其钙盐、微晶纤维素、聚乙二醇等;润滑剂如滑石、硬脂酸镁、硬脂酸钙、硅石等及润湿剂如月硅酸钠、甘油等,来制备如片剂、胶囊、颗粒剂、丸剂等的口服形式。
可以根据已知的方法,合适地使用用于溶解活性成分的溶剂如乙醇、异丙醇、丙二醇、1,3-丁二醇、聚乙二醇、芝麻油等;表面活性剂如山梨聚糖脂肪酸酯、聚氧乙烯山梨聚糖脂肪酸酯、聚氧乙烯脂肪酸酯、氢化蓖麻油的聚氧乙烯酯、卵磷脂等;悬浮剂如纤维素衍生物(包括羧甲基纤维素钠、甲基纤维素等)、天然树胶(包括黄芪胶、阿拉伯树胶等)及防腐剂如对羟基苯甲酸酯、苯扎氯铵、山梨酸盐等,来制备注射液、溶液、乳液、悬浮液、糖浆等。
本发明的用于治疗糖尿病的药物组合物中所含的活性成分的比例可以从大的范围中适当地选择。
在一个特定的实施方案中,需要T2DM治疗的受试者是哺乳动物。哺乳动物可以选自人、非人灵长类动物、犬、猫、鼠、牛、马、猪和兔类。在一个特定的实施方案中,哺乳动物是人。在另一个实施方案中,apoA-IV或其生物活性类似物可以施用于受试者以用于治疗T2DM,其中该受试者是肥胖的。或者,apoA-IV可以施用于受试者以用于治疗T2DM,其中该受试者不是肥胖的。
在再另一个实施方案中,参照图1,公开了装置1。在一个实施方案中,装置1包含之前讨论的药物组合物的储器10。在进一步的实施方案中,存储器10包含小瓶12。小瓶12可以由不抑制药物组合物的功能的任何材料形成。例如,小瓶12可以包含玻璃和/或塑料。此外,小瓶12可以包含可刺穿的隔膜,通过其可以取出药物组合物。使用中,由注射器20的针22刺穿小瓶的隔膜14,通过注射器20从小瓶12中取出药物组合物,并且通过注射到需要的受试者中来施用药物组合物。
实施例
以下非限制性实施例说明了本发明的方法。
实施例1:ApoA-IV敲除小鼠的葡萄糖耐受不良
实验方案。获得了雄性apoA-IV敲除(“下文中称为“KO”)小鼠。野生型(下文中称为“WT”)小鼠作为对照。ApoA-IV KO和WT小鼠获自辛辛那提大学(辛辛那提,OH)保持的群体。给apoA-IV KO和WT小鼠喂食正常饮食(chow diet)。在进行葡萄糖耐受测试之前,apoA-IV KO小鼠和WT小鼠禁食五小时。在葡萄糖耐受测试中,给apoA-IV KO小鼠和WT小鼠腹膜内注射约2mg/g体重剂量的葡萄糖,并且在注射葡萄糖后约0、15、30、60和120分钟测量血糖。葡萄糖耐受测试进行两次,一次在三个月大时,再一次在十六个月大时。
实验结果。如图2中所示的,apoA-IV KO小鼠相对于WT小鼠是葡萄糖耐受不良的。具体地,图2显示了腹膜内注射葡萄糖后两小时,WT小鼠中的血糖水平低于apoA-IV KO小鼠中的血糖水平。尽管不受理论束缚,这些研究的含义在于apoA-IV是正常葡萄糖动态平衡(至少在雄性中)必需的。此外,如图3中所示,在十六个月大测试时,apoA-IV KO小鼠显示出提高的葡萄糖耐受不良。具体地,图3显示了在十六个月大时测试的apoA-IV KO小鼠中的血糖水平高于三个月大时测试的apoA-IV KO中的血糖水平。尽管不受理论束缚,这些研究的含义在于apoA-IV KO小鼠的葡萄糖耐受随着年龄变差。

实施例2:ApoA-IV敲除小鼠中的葡萄糖耐受的恢复
实验方案。在证明apoA-IV KO小鼠是葡萄糖耐受不良后,进行了一系列深入的研究来确定将apoA-IV施用于apoA-IV KO小鼠是否使葡萄糖耐受恢复至正常水平。具体地,进行了一系列研究以不仅确定施用的apoA-IV的量,而且确定在进行葡萄糖耐受测试之前施用apoA-IV的最佳时间。
给ApoA-IV雄性KO小鼠腹膜内注射约0.25、0.5、1和2μg/g体重剂量的apoA-IV。还给ApoA-IV KO小鼠腹膜内注射盐水溶液来作为对照。给小鼠注射apoA-IV或盐水溶液后,按照之前讨论的,在三个月大时进行葡萄糖耐受测试。具体地,在注射apoA-IV或盐水溶液后约两小时进行葡萄糖耐受测试。实验结果表明恢复apoA-IV KO小鼠的葡萄糖耐受的最佳时间为进行葡萄糖耐受测试之前约两小时施用apoA-IV。
实验结果。如图4中所示,apoA-IV施用于apoA-IV KO小鼠将葡萄糖耐受恢复至正常水平。具体地,图4中显示了注射apoA-IV的apoA-IVKO小鼠中的血糖水平低于注射盐水溶液的apoA-IV KO小鼠中的血糖水平。此外,如图4中所示,注射apoA-IV的apoA-IV KO小鼠中的血糖水平在注射最高剂量的apoA-IV的apoA-IV KO小鼠中是最低的;相似地,注射apoA-IV的apoA-IV KO小鼠中的血糖水平在注射最低剂量的apoA-IV的apoA-IV KO小鼠中是最高的。因此,发现了葡萄糖耐受改善的程度取决于施用的apoA-IV的剂量,较高的剂量导致改善的葡萄糖耐受。
实施例3:ApoA-IV在恢复apoA-IV敲除小鼠的葡萄糖耐受中的特异性
实验方案。为了确定apoA-IV的特异性,我们将载脂蛋白AI(下文中称为“apoA-I”)施用于apoA-IV KO小鼠。ApoA-I是由小肠上皮细胞产生的蛋白质,小肠上皮细胞也产生apoA-IV。ApoA-I具有许多与apoA-IV共同的功能。给apoA-IV KO小鼠腹膜内注射1μg/g重量剂量的apoA-I。还给ApoA-IV KO小鼠腹膜内注射盐水溶液来作为对照。给小鼠注射apoA-I或盐水溶液后,按照之前讨论的,在三个月大时进行葡萄糖耐受测试。具体地,在注射apoA-I或盐水溶液后约两小时进行葡萄糖耐受测试。
实验结果。如图5中所示,apoA-I施用于apoA-IV KO小鼠未能恢复或改善葡萄糖耐受。
实施例4:apoA-IV敲除小鼠中葡萄糖耐受恢复的机制
实验方案。为了确定ApoA-IV改善apoA-IV KO小鼠中葡萄糖耐受的机制,我们测量了apoA-IV KO小鼠中葡萄糖诱导的胰岛素分泌。更具体地,按照之前讨论的在三个月大时的葡萄糖耐受测试过程中,我们测量了葡萄糖诱导的胰岛素分泌。在该研究中,在进行葡萄糖耐受测试前两小时,给apoA-IV KO小鼠腹膜内注射约1μg/g体重剂量的小鼠apoA-IV。在进行葡萄糖耐受测试前约两小时,给apoA-IV KO小鼠注射盐水溶液作为对照。
实验结果。如图6中所示的,在注射盐水的apoA-IV KO小鼠中不存在I期胰岛素分泌。然而,如图6中所示的,在进行葡萄糖耐受测试前两小时腹膜内注射apoA-IV时,apoA-IV KO小鼠中的I期胰岛素分泌恢复。
实施例5:ApoA-IV在高脂肪饮食的apoA-IV敲除小鼠和野生型小鼠中的功效。
实验方案。给apoA-IV KO和WT小鼠长期喂食购自Dyets(Bethlehem,PA)的高脂肪半纯化的、营养完全的实验饮食(AIN-93M),持续10周。高脂肪饮食含有约20g脂肪(即,约19g乳脂和1g大豆油以提供必需脂肪酸)/100g饮食。将apoA-IV KO和WT小鼠单独圈养在温度控制(约22±1℃)和光照控制(约12h光/12暗)的生态动物饲养场中的盆状笼子(tub cage)中,玉米芯垫层。按照之前讨论的,在三个月大时进行葡萄糖耐受测试。在进行葡萄糖耐受测试之前,apoA-IV KO小鼠和WT小鼠禁食五小时。在葡萄糖耐受测试中,给apoA-IV KO小鼠和WT小鼠腹膜内注射约2mg/g体重剂量的葡萄糖。
实验结果。如图7中所示,apoA-IV KO小鼠相对于WT小鼠呈现出更高的葡萄糖耐受不良。具体地,图7显示了腹膜内注射葡萄糖后两小时,WT小鼠中的血糖水平低于apoA-IV KO小鼠中的血糖水平。
实施例6:高脂肪饮食的ApoA-IV敲除和野生型小鼠中葡萄糖耐受的恢复
实验方案。进行了一系列涉及三个月大时向高脂肪饮食(20%重量脂肪,19%乳脂和1%红花油)饲养14周的apoA-IV KO和WT小鼠施用apoA-IV的研究。具体地,给apoA-IV KO和WT小鼠腹膜内注射约1μg/g体重剂量的小鼠apoA-IV。还给apoA-IV KO和WT小鼠腹膜内注射盐水溶液以用作对照。注射apoA-IV或盐水溶液后,进行了葡萄糖耐受测试。具体地,注射apoA-IV或盐水溶液后两小时进行葡萄糖耐受测试。
实验结果。如图8中所示,apoA-IV KO小鼠中apoA-IV的施用显著改善了葡萄糖耐受。具体地,图8显示了注射apoA-IV的apoA-IV KO小鼠中的血糖水平低于注射盐水溶液的apoA-IV KO小鼠中的血糖水平。前一句是多余的,因为下一句描述了相同的情况。尽管本文中没有包括数据,但还发现了长期喂食高脂肪饮食的WT小鼠中apoA-IV的施用也显著改善葡萄糖耐受。
实施例7:具有T2DM的小鼠中葡萄糖耐受的恢复
实验方案。为了证实apoA-IV能有效促进具有T2DM的动物的葡萄糖耐受,从Jackson Laboratories(Bar Harbor,Maine)获得杂合性KKCg-A/J小鼠(下文称为“Cg-A/J”)。Cg-A/J小鼠在八周大时发生高血糖症、高胰岛素血症、肥胖和葡萄糖耐受不良。这些小鼠中糖尿病的主要诱因是由与编码刺鼠相关蛋白和黑素皮质素-IV受体的拮抗剂的Ay突变的多基因相互作用而产生的胰岛素抵抗。给Cg-A/J喂食正常饮食。此外,从喂食正常饮食的十至十四周,血糖水平显著提高。
在十四周大时,通过腹膜内注射向Cg-A/J小鼠施用小鼠apoA-IV(约1μg/g体重)或盐水溶液(作为对照)。然后在约0、0.5、1、2、3、4、5、7、11和24小时测定血糖。
实验结果。如图9中所示的,相对于盐水对照,apoA-IV在降低Cg-A/J小鼠的血糖水平方面具有显著作用。尽管注射盐水溶液的Cg-A/J小鼠在24小时的研究期间中保持稳定的血糖水平,注射apoA-IV的Cg-A/J小鼠经历了超过10个小时的血糖水平下降,并且在该时间段的大部分时间中,血糖水平与我们研究的C57BL/6J动物相当。从这个研究中,我们得出结论,apoA-IV的施用在降低Cg-A/J小鼠的血糖中是有效的。
实施例8:ApoA-IV KO、ApoA-I KO和WT小鼠中的血清淀粉样P成分的水平
实验方案。进行了一系列涉及测定喂食致动脉粥样硬化的饮食的ApoA-IV KO、ApoA-I KO和WT小鼠中的血清淀粉样A蛋白成分(下文中称为“SAP”)的水平的研究。ApoA-IV KO、ApoA-I KO和WT小鼠获自辛辛那提大学。SAP是淀粉样P成分(下文中称为“AP”)的血清形式,是首先被确认为称为淀粉状蛋白的体内病理性沉积的五边形组分的25kDa五聚体蛋白。SAP在人体中的行为类似于C-反应性蛋白。具体地,在喂食致动脉粥样硬化饮食12周后,在apoA-IV KO、apoA-I KO和WT小鼠中测定apoA-IV KO、apoA-I KO和WT小鼠中的血浆SAP水平。通过Western印迹分析测定了血浆SAP的水平。
实验结果。如图10中所示的,apoA-IV KO小鼠(对应于小鼠编号1、8和10)中的SAP水平相对于apoA-I KO小鼠(对应于小鼠编号28、29和30)和WT小鼠(对应于小鼠编号19、20和25)中的SAP水平提高。

实施例8:人ApoA-IV降低进行腹膜内葡萄糖耐受测试的野生型小鼠的血糖水平
实验方案。进行了研究来确定将人apoA-IV施用于野生型小鼠是否将影响进行葡萄糖耐受测试的小鼠的血糖水平。
给三个月大的野生型小鼠腹膜内注射约1μg/g体重剂量的人apoA-IV。作为对照,给另一组野生型小鼠腹膜内注射盐水溶液。注射人apoA-IV或盐水溶液后,进行了葡萄糖耐受测试。具体地,注射apoA-IV或盐水溶液后约两小时和禁食五小时后进行葡萄糖耐受测试。收集尾血,并通过血糖仪进行测量。
实验结果。如图12中所示,人apoA-IV在降低进行葡萄糖耐受测试的野生型小鼠的血糖中是有效的。
实施例9:小鼠ApoA-IV在进行腹膜内葡萄糖耐受测试的野生型雌性小鼠中的作用
实验方案。进行了研究来确定将小鼠apoA-IV施用于雌性野生型小鼠是否影响进行葡萄糖耐受测试的小鼠的血糖水平。
给三个月大的雌性野生型小鼠腹膜内注射约1μg/g体重剂量的小鼠apoA-IV。作为对照,给另一组雌性野生型小鼠腹膜内注射盐水溶液。注射人apoA-IV或盐水溶液后,进行了葡萄糖耐受测试。具体地,注射apoA-IV或盐水溶液后约两小时和禁食五小时后进行葡萄糖耐受测试。收集尾血,并通过血糖仪进行测量。
实验结果。如图13中所示,小鼠apoA-IV在降低进行葡萄糖耐受测试的野生型雌性小鼠的血糖中是有效的。
实施例10:人ApoA-IV刺激人胰岛的胰岛素释放
高纯度人胰岛由弗吉尼亚大学,Axon Cells,提供。将胰岛在含有10%FBS和11mM葡萄糖的RPMI1640中,在37℃下,95%空气和5%CO2的潮湿气氛中培养48小时。然后将四组50IEQ胰岛在含有3.0mM葡萄糖的常规KRB(129mM NaCl、4.8mM KCl、2.5mM CaCl2、1.2mMMgSO4、1.2mM KH2PO4、5mM NaHCO3、10mM HEPES和0.2%BSA)中,在37℃下预孵育1h。然后在10μg/ml人A-IV存在或不存在下,将前两组的胰岛在含有3.0mM葡萄糖的常规KRB中孵育一小时,并进一步在10μg/ml人A-IV存在或不存在下,与20mM葡萄糖再孵育一小时。将后两组的胰岛在10μg/ml人A-IV存在或不存在下,在含有3.0mM葡萄糖的30mM KCl KRB(103.8mM NaCl、30mM KCl、2.5mM CaCl2、1.2mM MgSO4、1.2mM KH2PO4、5mM NaHCO3、10mM HEPES和0.2%BSA)加250μmol/1二氮嗪中孵育一小时,并进一步在10μg/ml人A-IV存在或不存在下,与20mM葡萄糖再孵育一小时。在各次的一小时孵育结束时收集培养基。通过ELISA试剂盒(Millipore)测量胰岛素水平。
如可从图14看到的,当通过30mM KCl加250μM二氮嗪将人胰岛最大去极化时,10μg/ml hA-IV对胰岛素分泌显示出明显的刺激作用。
实施例11未糖基化ApoA-IV的制备
将人和小鼠apoA-IV cDNA包含在pSP65维持载体中,并且Afl III限制性位点进行工程化以恰好在用于成熟apoA-IV蛋白的编码序列的5’端。从维持载体切除基因,并接合至pET30表达载体中。将构建体转染至大肠杆菌BL-21(DE3)细胞中,并且在补充了卡那霉素(30μg/ml)的Luria-Bertani培养基中在37℃下生长。在诱导细胞中的apoA-IV蛋白合成后,收获细胞并超声波处理。通过His-结合亲和柱色谱和渗析来纯化来自裂解产物的apoA-IV蛋白。将所得到的apoA-IV蛋白在盐水中稀释至1.0mg/ml的终浓度。
Claims (24)
1.一种用于治疗需要的受试者中的II型糖尿病的方法,该方法包括将有效量的载脂蛋白A-IV或与载脂蛋白A-IV具有至少90%同一性的其生物活性类似物施用于受试者。
2.一种降低需要的受试者中的血糖水平的方法,该方法包括将有效量的载脂蛋白A-IV或与载脂蛋白A-IV具有至少90%同一性的其生物活性类似物施用于受试者。
3.一种用于在需要的受试者中将葡萄糖耐受基本上恢复至正常水平的方法,该方法包括将有效量的载脂蛋白A-IV或与载脂蛋白A-IV具有至少90%同一性的其生物活性类似物施用于受试者。
4.权利要求1-3任一项的方法,其中所述载脂蛋白A-IV生物活性类似物与载脂蛋白A-IV具有至少99%的同一性。
5.权利要求1-4任一项的方法,其中所述受试者是人。
6.权利要求1-5任一项的方法,其中将有效量的载脂蛋白A-IV施用于受试者,其中该载脂蛋白的氨基酸序列为:
