技术领域
本发明涉及用于鉴别和/或分离Tr1细胞、休眠Tr1细胞和/或活化Tr1细胞的方法,经过富集的Tr1细胞、休眠Tr1细胞和/或活化Tr1细胞的群体,去除Tr1细胞、休眠Tr1细胞和/或活化Tr1细胞的群体,以及用于诊断或治疗慢性炎症性疾病、自身免疫疾病、过敏性疾病、癌症和器官移植病症的方法和试剂盒。
背景技术
免疫系统的体内平衡依赖于针对入侵病原体的免疫应答与针对自身抗原的免疫耐受之间的平衡。中枢和外周耐受都是用于诱导和维持T细胞耐受的重要机制。在过去的十年里,已经对在维持外周免疫耐受方面扮演重要角色的调节性T细胞(Treg)给予了众多关注。现在已经公认Treg可以分成两种不同的亚型:天然Treg(nTreg)和诱导型Treg群体,例如TGF-β诱导的Treg细胞、Tr1细胞或Th3细胞。因为这些Treg群体在外周免疫耐受中扮演重要角色,所以能够鉴别、分离或富集这些不同的Treg亚组以便促进对于免疫病症(例如慢性炎症性疾病、自身免疫疾病或过敏性疾病)的诊断、评估或治疗已经变成一个关键议题。
举例来说,自nTreg被鉴别以来,其一直是通过CD25的表达来进行鉴别。然而,CD25也是常规T细胞(convT)的一种活化标志物,因此不是Treg细胞所专有的。通过鉴别例如Foxp3、CD127(WO2007140472)、CD45RA(WO2007/117602)、CD134(WO2009/036521)和CD49d(WO2009/047003)等细胞表面或细胞内标志物已经在人类nTreg的辨别方面取得了最新进展。关于Tr1细胞,其特异性鉴别仍然存在困难且主要基于IL-10分泌辨别。因此需要提供用于更好地且更精确地辨别Tr1细胞的方式。
此外,Tr1细胞已被证实可用于细胞疗法中,Tr1细胞注射入患有克罗恩病(Crohn’sdisease)的患者体内以改善其状况。鉴于Tr1细胞在细胞疗法中的用途,因此需要更精确地鉴别和分离Tr1细胞。
发明人已经发现可使用CD127和CD62L细胞表面标志物特异性地鉴别Tr1细胞。实际上,除CD4标志物和任选的CD25标志物以外,另外使用CD127和CD62L这两种标志物允许在常规的休眠和活化T细胞以及休眠和活化nTreg中辨别Tr1细胞。
发明内容
本发明的一个目的是一种鉴别Tr1细胞群体的方法,其包含检测细胞群体上的CD4、CD127和CD62L标志物的细胞表面表达,其中表达:

具体实施方式
Tr1细胞是一种独特的细胞群体,其可通过下文所述的特异性细胞因子分泌特征进行表征。Tr1细胞是从已经遭遇抗原的原初CD4+T细胞分化而来。Tr1细胞因此可以两种状态在活体内发现:活化和休眠。如上所述,使用表面标志物辨别Tr1细胞、尤其是休眠和活化Tr1细胞当前不可行,因为针对这些群体的特异性表面表型无法获得。
发明人意外地发现,休眠Tr1细胞呈现以下表型CD4+CD25-CD127lo/-CD62Llo/-,且活化Tr1细胞呈现以下表型CD4+CD25+CD127lo/-CD62Llo/-。这尤其令人吃惊,因为所属领域中众所周知休眠T细胞典型地表达高水平的CD62L。
本发明提供用于鉴别、分离、富集和/或去除人类Tr1细胞、尤其是人类休眠Tr1细胞和人类活化Tr1细胞的方法。
本发明还提供获得的Tr1细胞、休眠Tr1细胞和活化Tr1细胞群体或组合物和使用方法。
本发明的一个目的是一种鉴别Tr1细胞群体的方法,其包含检测细胞群体上的CD4、CD127和CD62L标志物的细胞表面表达,其中表达CD4、低水平的CD127和低水平的CD62L的细胞是Tr1细胞。

术语“休眠”在所属领域中是众所周知的,并且是指未增殖、未产生细胞因子且未在表面上表达常规免疫细胞活化分子例如CD25的免疫细胞或细胞群体。休眠T细胞可能是原初T细胞或记忆T细胞。
术语“活化”在所属领域中是众所周知的,并且是指发生增殖和/或产生细胞因子并在其表面上表达常规免疫细胞活化分子例如CD25的免疫细胞或细胞群体。
特别是对于T淋巴细胞来说,从休眠到活化状态的转换是通过T细胞与其特异性抗原遭遇或通过用活化性细胞因子或有丝分裂原进行活化来调节的。
关于CD127lo/-、CD62Llo/-或CD25Llo/-使用的术语“低”或“lo”或“lo/-”在所属领域中是众所周知的,且是指目的细胞标志物的表达水平,也就是说细胞标志物的表达水平与所述细胞标志物在整体分析的细胞群体中的表达水平相比是较低的。更具体地说,术语“lo”是指一种独特细胞群体的细胞标志物的表达水平低于一种或一种以上其它独特细胞群体。
术语“高”或“hi”或“明亮”在所属领域中是众所周知的,且是指目的细胞标志物的表达水平,也就是说细胞标志物的表达水平与所述细胞标志物在作为整体被分析的细胞群体中的表达水平相比是高的。
术语“+”和“”在所属领域中是众所周知的,且是指目的细胞标志物的表达水平,也就是说对应于“+”的细胞标志物的表达水平是较高或中等的,也称为“+/-”,而对应于“-”的细胞标志物的表达水平则是较低或没有的。一般来说,在染色强度的前2%、3%、4%或5%范围内的细胞被指定为“hi”,而落入群体的前50%范围内的细胞被分类为“+”。低于荧光强度的50%以下的那些细胞被指定为“lo”细胞,且小于5%的被指定为“-”细胞。

根据本发明,Tr1细胞在活化后能够分泌高水平的IL-10和中等水平的TGF-β。Tr1细胞部分地由其独特的细胞因子特征进行表征:其产生高水平的IL-10,中等水平的TGF-β和中等水平的IFN-γ,但产生很少的或不产生IL-4或IL-2。典型地在细胞培养物中,在用T淋巴细胞的多克隆活化物(例如抗CD3+抗CD28抗体或白细胞介素-2、PMA+离子霉素(ionomycin))活化之后对细胞因子的产生进行评估。或者,在细胞培养物中,在用抗原呈递细胞所呈递的特异性T细胞抗原活化之后对细胞因子的产生进行评估。高水平的IL-10对应于至少约500pg/ml,典型地大于约1000、2000、4000、6000、8000、10000、12000、14000、16000、18000或20000pg/ml或以上。中等水平的TGF-β对应于至少约100pg/ml,典型地大于约200、300、400、600、800或1000pg/ml或以上。中等水平的IFN-γ对应于介于0.1pg/ml与至少400pg/ml之间的浓度,典型地大于约600、800、1000、1200、1400、1600、1800或2000pg/ml或以上。很少的或无IL-4或IL-2对应于小于约500pg/ml,优选小于约250、100、75或50pg/ml或以下。
Tr1细胞可以在操作上用细胞表面标志物进行表征。这些细胞表面标志物可以用与细胞表面标志物特异性地结合的试剂进行识别。举例来说,Tr1细胞表面上的蛋白质、碳水化合物或脂质可以用对特定蛋白质或碳水化合物具有特异性的抗体在免疫学上进行识别(关于针对标志物的抗体的使用,请参见Harlow,UsingAntibodies:ALaboratoryManual(ColdSpringHarborPress,ColdSpringHarbor,N.Y.,1999);也可参见实施例)。在Tr1细胞表面上存在的和在这些细胞的表面上不存在的标志物群集都是Tr1细胞的特征。因此,可以通过使用细胞表面标志物进行阳性和阴性选择来选择Tr1细胞。与Tr1细胞所表达的细胞表面标志物(“阳性标志物”)结合的试剂可用于对Tr1细胞进行阳性选择(即保留对细胞表面标志物进行表达的细胞)。相反地,阴性选择依赖于某些细胞表面标志物不被Tr1细胞表达这一事实。因此,“阴性标志物”(即不存在于Tr1细胞的细胞表面上的标志物)可用于消除群体中的不是Tr1细胞的那些细胞,所述消除是通过除去与对阴性标志物具有特异性的试剂结合的细胞来进行的。
在一个实施方案中,基于检测到的细胞表面标志物的表达对细胞进行辨别是通过比较细胞表面标志物的表达与对照细胞群体的平均表达实现。举例来说,可将标志物在Tr1细胞上的表达与来源于和Tr1细胞相同的样品的其它细胞上相同标志物的表达进行比较。通过标志物表达来辨别细胞的其它方法包括使用试剂组合通过流式细胞术选择细胞(参见GivanA,FlowCytometry:FirstPrinciples,(Wiley-Liss,NewYork,1992);OwensMA&LokenMR.,FlowCytometry:PrinciplesforClinicalLaboratoryPractice,(Wiley-Liss,NewYork,1995))。
“试剂组合”意思是至少两种试剂,其结合于Tr1细胞表面上存在(阳性标志物)或不存在(阴性标志物)的细胞表面标志物,或结合于阳性和阴性标志物的组合。举例来说,使用对Tr1细胞表面标志物具有特异性的抗体的组合可以从多种样品/组织中分离和/或富集Tr1细胞。
对在从样品获得的细胞针对表型特征进行选择,使用识别种特异性多种标志物的抗体来富集和选择Tr1细胞。“经过富集的”(如用在经过富集的细胞群体中)的定义可基于,在经过分级的细胞组中具有特定标志物的细胞的数目比在未经过分级的细胞组中具有所述标志物的细胞的数目增加。在特定实施方案中,使用结合对Tr1细胞具有特异性的细胞表面标志物的试剂并使用细胞分选分析法(例如荧光活化细胞分选(FACS)、固相磁珠等)分离这些细胞,从而从细胞群体富集Tr1细胞。在一些实施方案中,可使用分选细胞的方法的组合,例如先使用磁性选择,后使用FACS。为了增强富集,使用细胞表面标志物将阳性选择与阴性选择进行组合用于分离Tr1细胞。预期使用细胞表面标志物对Tr1细胞的分离/富集可以任何次序进行。因此,阳性选择步骤之后可紧跟阴性选择步骤,或反之亦然。也预期可通过组合阳性选择与阴性选择步骤来进行分离/富集。因此,分离/富集是通过首先进行所述方法的阳性选择步骤,随后进行所述方法的阴性选择步骤来进行,或反之亦然。
根据本发明的“抗X抗体”或“X抗体”是可特异性地结合到X上的抗体。举例来说,抗CD127抗体或CD127抗体能够结合CD127。根据本发明使用的抗体包括(但不限于)重组抗体、多克隆抗体、单克隆抗体、嵌合抗体、人类单克隆抗体、人源化或灵长类动物源化单克隆抗体和抗体片段。市场上可以购得众多的淋巴细胞生物标志物特异性抗体。这些抗体包括抗CD127、抗CD4、抗CD62L和抗CD25抗体。“抗体”是指包含来自免疫球蛋白基因的框架区的多肽或其片段,其特异性地结合抗原并识别抗原。公认的免疫球蛋白基因包括κ、λ、α、γ、δ、ε和μ恒定区基因,以及多种免疫球蛋白可变区基因。轻链被分类为κ或λ。重链被分类为γ、μ、α、δ或ε,这又分别定义免疫球蛋白类别IgG、IgM、IgA、IgD和IgE。典型地,抗体的抗原结合区对于结合特异性和亲和力是最重要的。示例性免疫球蛋白(抗体)结构单元包含四聚体。每个四聚体由两对相同的多肽链构成,每对具有一个“轻链”(约25kD)和一个“重链”(约50-70kD)。每个链的N末端界定约100到110或更多个氨基酸的可变区,其主要负责抗原识别。术语可变轻链(VL)和可变重链(VH)分别是指这些轻链和重链。抗体是例如作为完整免疫球蛋白存在,或作为通过用各种肽酶进行消化而产生的多个公认片段存在。因此,举例来说,胃蛋白酶通过铰链区中的二硫键消化抗体以便产生F(ab)′2,这是一种Fab二聚体,Fab自身为通过二硫键与VH-CH1结合的轻链。F(ab)′2可以在温和条件下还原,以便破坏铰链区中的二硫键,从而将F(ab)′2二聚体转化为Fab′单体。Fab′单体实质上就是具有部分铰链区的Fab(参见FundamentalImmunology(Paul编辑,第3版1993))。虽然各种抗体片段是依据完整抗体的消化来定义,但所属领域的技术人员将了解到,所述片段可以化学方法或通过使用重组DNA方法从头合成。因此,如本文所用的术语抗体还包括通过修饰完全抗体而产生的抗体片段,或使用重组DNA方法从头合成的抗体片段(例如单链Fv),或使用噬菌体展示文库鉴别的抗体片段(例如参见McCafferty等人,Nature348:552-554(1990))。在一些实施方案中,靶标的高亲和力配体可代替抗体使用。当涉及蛋白质或肽时,短语“与抗体特异性地(或选择性地)结合”或“与......特异性地(或选择性地)免疫反应”是指可确定蛋白质的存在的结合反应,通常用在异源蛋白质群体和其它生物制品中。在所述条件下与抗体的特异性结合需要基于针对特定蛋白质的特异性而被选择的抗体。举例来说,可以选择多克隆抗体以便只获得与被选择的抗原特异性地免疫反应但不与其它蛋白质特异性地免疫反应的那些多克隆抗体。这种选择可通过去掉与其它分子交叉反应的抗体来实现。
优选地,“标记”或“可检测部分”与抗体共价地或非共价地连接。标记可以通过光谱学、光化学、生物化学、免疫化学、化学的手段或其它物理手段检测。荧光染料是尤其有用的标记。将标记与抗体连接的方法对于所属领域的一般技术人员是众所周知的。尤其优选的标记是通过连接子与抗体连接的标记,所述连接子容易通过在生理条件下与预定酶接触而被裂解或分拣或发生水解。抗体也可以与磁性粒子(例如顺磁性微珠(MiltenyiBiotec,Germany))结合。被经过磁性标记的抗体结合的活化T细胞可使用包括(但不限于)磁性细胞分选的技术来分离。针对CD127、CD62L、CD4和CD25以及许多其它分化群(clusterofdifferentiation)的被适当标记的抗体可以在市场上购得,并且对于所属领域的一般技术人员是已知的。抗体可以在接触样品之前或之后或者在接触CD之前或之后进行标记。CD抗体可以通过接触与CD抗体结合的被标记抗体来进行标记。如本文所用,术语“CD”或“分化群”或“共同决定子”是指抗体所识别的细胞表面分子。
根据本发明,Tr1细胞群体可以通过检测细胞群体上的CD4、CD127和CD62L标志物的细胞表面表达来鉴别:表达CD4(即CD4+)且表达低水平的CD127和CD62L(即CD127lo/-和CD62Llo/-)的细胞对应于Tr1细胞。
根据本发明,休眠Tr1细胞群体可以通过检测细胞群体上的CD4、CD25、以及至少CD127和CD62L二者之一的标志物的细胞表面表达来鉴别:表达CD4(即CD4+)且表达低水平的CD127和CD62L二者中的至少一种(即CD127lo/-和/或CD62Llo/-)、优选低水平的CD127和CD62L,但不表达CD25(即CD25-)的细胞对应于休眠Tr1细胞。
在本发明的一个实施方案中,还评估了标志物Foxp3,并且表达CD4(即CD4+)且表达低水平的CD127和CD62L二者中的至少一种(即CD127lo/-和/或CD62Llo/-)、优选低水平的CD127和CD62L,但不表达CD25(即CD25-)且另外不表达Foxp3(即Foxp3-)的细胞对应于休眠Tr1细胞。
根据本发明,活化Tr1细胞群体可以通过检测细胞群体上的CD4、CD25、CD127和CD62L标志物的细胞表面表达来鉴别:表达CD4和CD25(即CD4+CD25+)且表达低水平的CD127和CD62L(即CD127lo/-和CD62Llo/-)的细胞为活化Tr1细胞。
在一个实施方案中,还评估了标志物Foxp3,并且表达CD4和CD25(即CD4+CD25+)且表达低水平的CD127和CD62L(即CD127lo/-和/或CD62Llo/-)且表达Foxp3(即Foxp3+)的细胞为活化Tr1细胞。
用于鉴别/选择所述Tr1细胞的与所述标志物(CD4、CD25、CD127、CD62L和Foxp3)结合的优选试剂是抗体。在另一个实施方案中,抗体与荧光染料或磁性粒子结合。在另一个实施方案中,细胞选择是通过流式细胞术、荧光活化细胞分选、磁性选择、亲和色谱或淘选或其组合来进行。

用于分拣、分离或选择细胞的方法包括(但不限于)使用被抗体包被的磁珠的磁性分拣(Schwartz等人,美国专利第5,759,793号),和使用附着到固体基质(例如平板)上的抗体的亲和色谱或“淘选”。提供精确分拣的其它技术包括荧光活化细胞分选仪(FACS),其可具有不同的复杂程度,例如具有多个彩色通道、低角度和迟钝的光散射检测通道或阻抗通道。死亡细胞可通过使用与死亡细胞相关的染料(例如碘化丙啶,LDS)进行选择来消除。红血球可通过例如淘析、溶血或Ficoll-Paque梯度来去除。对被选择的细胞的活力不会过度有害的任何技术都可使用。
适宜地,抗体可与标记进行结合用于多种不同目的:例如磁珠,其用于使Tr1细胞的分拣更容易;生物素,其以高亲和力结合到抗生物素蛋白或抗生蛋白链菌素上;荧光染料,其可与荧光活化细胞分选仪一起使用;半抗原等。多色分析可与FACS一起使用,或与免疫磁性分拣和流式细胞术结合使用。多色分析是基于多种表面抗原用于分拣细胞而受到关注:例如非CD4+免疫细胞标志物(CD8、CD14、CD16、CD19、CD36、CD56、CD123、TCRγ/δ和血型糖蛋白(glycophorin)A)、CD4、CD25、CD127和CD62L。可用于多色分析中的荧光染料包括(但不限于)藻胆蛋白,例如藻红蛋白和别藻蓝蛋白;荧光素;和德克萨斯红(Texasred)。
磁性分拣是一种利用磁场梯度将磁性物质选择性地保留在容器(例如离心管或柱)内的方法。可通过经由特异性相互作用(包括免疫亲和相互作用)将磁性粒子结合到细胞表面上而对Tr1细胞进行磁性标记。合适容器内的含有Tr1细胞的悬浮液然后暴露于足够强度的磁场梯度下,以便分开Tr1细胞与悬浮液中的其它细胞。容器然后用合适流体洗涤以去除未标记的细胞,从而获得经过纯化的Tr1细胞悬浮液。
大部分磁性标记系统使用表面共价结合有单克隆抗体或抗生蛋白链菌素的超顺磁粒子。在细胞分拣应用中,这些粒子可用于阳性选择,其中对目的细胞进行磁性标记并将其保留,或用于阴性选择,其中对大部分不想要的细胞进行磁性标记并将其保留。所用粒子的直径相差很大,从MACS粒子(MiltenyiBiotec)和StemSep(TM)胶体(StemCellTechnologies)的约50-100nm到EasySep(R)(StemCellTechnologies)和Imag粒子(BDBiosciences)的150-450nm到Dynabeads(DynalBiotech)的4.2μm不等。所用粒子的类型受用于分拣被标记细胞的磁体技术影响。
存在两类重要的磁性分拣技术,它们出于便利和实用原因皆使用永磁体而不是电磁体。第一类是基于柱的高梯度磁场分拣技术,其使用弱磁性小粒子标记目的靶标,并在填充有可磁化基质的柱中分拣这些靶标。当对柱施加磁场时,在紧靠基质元件表面的地方产生极高的梯度。高梯度是分拣用这些相对弱磁性粒子标记的靶标所必需的。第二类是基于试管的技术,其使用磁性更强的粒子标记目的靶标。这些目的靶标然后在离心型试管内通过用试管外部的磁体产生的磁场梯度进行分拣。这种方法的优点是它不依赖于可磁化基质来产生梯度,因此不需要昂贵的一次性柱,或涉及不便的清洁和去污程序的可再生柱。
一旦放在磁体内,被靶向细胞就会朝着磁场强度最高的一个或一个以上区域迁移,并被保留在磁场内,而未标记的细胞则被排出。然后可在从磁场移出被靶向细胞之后收集被靶向细胞,并进行使用。在需要阴性选择的情况下,未标记的细胞被保留并可用于多种应用。
FACS允许基于细胞亚群通过激光束时的光散射性质对其进行分拣。前向光散射(FALS)与细胞大小有关,且直角光散射(也称为侧向散射特征(SSC))与细胞密度、细胞内容物和核-胞质比率(即细胞复杂度)有关。因为细胞可以用荧光结合抗体进行标记,所以所述细胞可进一步通过抗体(荧光)强度进行表征。
在特定实施方案中,使用免疫磁性色谱分离本发明的Tr1细胞。举例来说,将抗CD4抗体附着到磁珠上。这些经过抗体标记的磁珠是用作亲和纯化的基础。将T细胞的经过抗体标记的部分施加到磁性亲和柱上。不粘附的细胞被丢弃,粘附细胞通过移除磁场而从磁性柱上洗提。在另一个实施方案中,细胞首先用抗体(例如抗CD4)进行标记,然后用携带磁珠或磁球的二级抗体进行标记。
在另一个实施方案中,可使用对Tr1细胞具有免疫反应性的二级抗体对Tr1细胞群体进行富集。二级抗体的使用在所属领域中是通常已知的。典型地,二级抗体为对第一抗体的恒定区具有免疫反应性的抗体。优选的二级抗体包括抗兔、抗小鼠、抗大鼠、抗山羊和抗马免疫球蛋白,且可以在市场上购得。在市场上可购得的试剂盒提供与标记试剂(例如但不限于磁性粒子和荧光染料)结合的二级抗体。
在本发明的一个实施方案中,用于富集Tr1细胞、休眠Tr1细胞或活化Tr1细胞群体的方法可包括:
-使细胞群体与结合于非CD4+细胞的标志物的至少一种试剂接触,从而去除非CD4+细胞,

因此,对受试对象施用的经过扩增的Tr1细胞通过其抑制活性抑制不希望的免疫应答,从而对与不希望的免疫应答相关的病症进行治疗。

在另一个实施方案中,受试对象是受包括(但不限于)以下各项的自身免疫病症或疾病/病状影响的人:红斑狼疮、寻常性天疱疮、甲状腺炎、血小板减少性紫癜、格雷夫斯病、糖尿病、幼年糖尿病、自发性自身免疫糖尿病、重症肌无力、爱迪生氏病(Addison′sdisease)、关节炎性病症(例如类风湿性关节炎、强直性脊柱炎、幼年特发性关节炎、牛皮癣性关节炎)、多发性硬化、牛皮癣、葡萄膜炎、自身免疫性溶血性贫血、硬皮病、炎症性肠病(例如自身免疫炎症性肠病、克罗恩病、结肠炎、与食物过敏有关的肠炎)、斯耶格伦氏综合征(Sjorgen′sdisease)、桥本氏病(Hashimoto′sdisease)、重症肌无力、自身免疫性多内分泌病综合征、I型糖尿病(TIDM)、自身免疫性胃炎、自身免疫性葡萄膜视网膜炎、多肌炎和甲状腺炎,以及人类狼疮作为代表的全身性自身免疫疾病。如本文所用的“自体抗原”或“自身抗原”是指哺乳动物天生具有且在所述哺乳动物疾病中具有免疫原性的抗原或表位。
本发明的细胞还可用于预防或治疗移植反应,例如移植物抗宿主疾病(GVHD)、造血干细胞移植和移植物排斥。
在另一个实施方案中,受试对象受过敏或哮喘病症影响。过敏或哮喘病症的实例包括(但不限于)哮喘、特应性皮炎、过敏性鼻炎、结膜炎、湿疹、接触过敏、吸入性过敏、摄入性过敏和过敏性反应。
Tr1细胞是使用所属领域中众所周知的方法(例如过继细胞转移)引入到患者体内。简言之,从目标供体提取混合细胞群体。取决于应用,细胞可以在好转期内提取,或在疾病活动期间提取。典型地,这是通过抽取全血并利用白细胞去除术收获粒细胞来进行的。举例来说,大体积白细胞去除术(LVL)已被证实可最大化血液白细胞产量。收获的淋巴细胞可使用基于Tr1特异性细胞标志物(例如本文所述者)的细胞分拣技术进行分拣,然后输注到患者、典型地细胞供体(除了供体与受体不同的GVHD)中以用于过继免疫抑制。将大约103到1011、优选地104到1010、优选地105到109、优选地106到109、更优选地106到108个Tr1细胞注入患者中。
如本文所用的术语“有效量”意思是治疗物质或组合物的量,所述量足以缩减(到任何程度)或改善病症或其一种或一种以上症状的严重性和/或持续时间、预防病症进展、引起病症消退、预防与病症有关的一种或一种以上症状的复发、发展、发生或进展、检测病症或者促进或提高另一种疗法(例如预防或治疗药剂)的预防或治疗效果。有效量将随着受试对象的年龄、性别、种族、物种、一般情况等、所治疗病症的严重性、施予的特定药剂、治疗持续时间、任何并行治疗的性质、所用的药学上可接受的载体和在所属领域的技术人员的知识和专门技能范围内的类似因素而改变。适当时,在任何个别情形中的“有效量”可由所属领域的一般技术人员通过参考有关教科书和文献和/或通过使用常规实验来确定(例如参见Gennaro等人编辑,Remington′sTheScienceandPracticeofPharmacy,第20版,(2000),LippincottWilliamsandWilkins,BaltimoreMD;Braunwald等人编辑,Harrison′sPrinciplesofInternalMedicine,第15版,(2001),McGrawHill,NY;Berkow等人编辑,TheMerckManualofDiagnosisandTherapy,(1992),MerckResearchLaboratories,RahwayNJ)。

因此,在被移植的细胞群体中不存在Tr1细胞可以增加期望的免疫应答,因为没有Tr1细胞发挥其抑制活性。
在一个实施方案中,所述方法是用于治疗正接受癌症治疗的受试对象。
在一个实施方案中,被移植的细胞群体是抗原特异性效应T细胞群体。优选地,所述T细胞群体对肿瘤抗原(例如黑素瘤的MART-1(MelanA),或黑素瘤、甲状腺髓样癌、小细胞肺癌、结肠和/或支气管鳞状细胞癌的MAGE1、2、3)具有特异性。
在所述实施方案中,打算移植的细胞群体中的休眠和/或活化Tr1细胞在施予欲治疗的患者之前在活体外被去除。
调节性T细胞的去除已被证实可增强癌症患者中的疫苗介导型免疫性(Dannull等人,2005,115(12):3623-3633)。因此,在本发明的另一个实施方案中,提出了Tr1细胞、休眠Tr1细胞和/或活化Tr1细胞的去除可用在细胞疗法中,以用于治疗所述Tr1细胞可能对于治疗有害或不利的癌症或传染病。举例来说,在癌症治疗中,在疫苗接种/免疫治疗方案之前对Tr1细胞进行短暂去除可以值得关注,所述短暂去除是通过根据本发明的去除方法用一种装置在离体情况下对患者血液去除Tr1细胞并将血液立刻再注入患者体内来实现的。因此,在所述实施方案中,休眠和/或活化Tr1细胞从患者血液中的去除是通过一种装置在离体情况下进行。
本发明的另一个目的是一种用于活体外监测受试对象中的治疗效果的方法,其包含在治疗之前和之后鉴别生物样品中的休眠和/或活化Tr1细胞群体,其中治疗之后休眠和/或活化Tr1细胞群体的增加对应于对治疗的响答。
在所属领域中众所周知,在罹患自身免疫或炎症性疾病(例如多发性硬化、哮喘、寻常性天疱疮和类风湿性关节炎)的受试对象中,Tr1细胞数量减少。因此,在治疗后可观察到休眠和/或活化Tr1细胞群体增加的受试对象为对治疗有应答的受试对象。
根据本发明,如上文所述对休眠和/或活化Tr1细胞群体进行鉴别。
在一个实施方案中,生物样品在治疗之前从受试对象获得,并且在治疗之后在不同时间(例如治疗期间每周或每月)从受试对象获得。

实施例
实验程序
Tr1细胞分离
如Brun等人(InternationalImmunopharmacology,2009)所述对Tr1细胞进行分离。简言之,通过Ficoll密度离心来分拣外周血细胞,并在存在食物抗原的情况下以每毫升2×106个细胞进行培养,以便允许食物抗原特异性Tr1细胞进行增殖。在培养13天后,通过极限稀释法在3周内对细胞进行克隆。然后使用CD3/CD28刺激剂和细胞因子(IL-2和IL-4)使生长的克隆进行扩增。通过细胞的细胞因子产生特征来评估克隆的Tr1细胞身份,所述细胞因子产生特征显示高IL-10产量、中等IFN-γ产量和低IL-4产量(参见Brun等人,InternationalImmunopharmacology,2009)。为了产生小鼠Tr1细胞,在7天期间在存在IL-10、抗IL-12和抗IL-4的情况下用抗CD3和抗CD28单克隆抗体活化脾细胞。获得的细胞群体在经过照射的同源脾细胞和抗CD3抗体上在3周内进行克隆。评估生长的克隆的细胞因子分泌特征。显示高IL-10产量、中等IFN-γ产量和低IL-4产量的克隆被鉴别为Tr1细胞。
CD4+T淋巴细胞和Treg细胞分离
通过基于Ficoll-PlaquePLUS梯度(GEhealthcare)的密度沉降,从来源于健康志愿者的全血的血沉棕黄层制剂中分离PBMC。从梯度界面回收的细胞在RPMI1640培养基中洗涤两次,计数,并立即用于MACS和染色。简言之,使用CD4分离试剂盒(Dynal)从总PBMC阴性地选择CD4+T细胞,得到纯度为92-98%的CD4+细胞群体。如Battaglia等人(ThejournalofImmunology,2006)所述,对纯化的CD4+Treg细胞进行活体外扩增。简言之,用被抗CD3/抗CD28Ab包被的磁珠(DynabeadsCD3/CD28T细胞扩增物,DynalInvitrogen)对经过分离的CD4+T细胞进行活化。细胞在存在X-vivo15培养基的情况下培养,所述培养基补充有5%混合的AB型人血清(Sigma)、1%青霉素/链霉素以及100nM雷帕霉素。进行3轮刺激,每轮7天,且从第2轮刺激开始添加IL-2(100U/ml)。
流式细胞术研究
为了对人细胞进行流式细胞术研究,使用以下抗体:被PE结合的抗CD127(来自BeckmanCoulter的克隆R34.34)、被FITC标记的抗CD4Ab(来自BectonDickinson,克隆#RP4-T4)、被PeCy5标记的抗CD62L(来自BectonDickinson的克隆Dreg56)和被别藻蓝蛋白结合的抗CD25(来自BectonDickinson的克隆M-A251)。根据制造商说明书,使用被PE结合的抗Foxp3Ab(来自BD的克隆259D/C7)和胞浆内染色试剂盒(BD)对人Foxp3进行胞浆内染色。对于小鼠细胞的流式细胞术研究来说,使用的抗体为被PECy7结合的抗CD4(克隆RM4-15)、被APC结合的抗CD25(克隆PC61)、被PE结合的FoxP3(克隆MF23)和被PE结合的抗CD62L(克隆Mel14)。所有的抗小鼠抗体都购自BectonDickinson。
基因表达研究
如Dayem等人(CompFunctGenom,2003)所述使用DNA微阵列分析来进行基因表达研究。为此,使细胞样品裂解,并用Trizol试剂(Invitrogen)对RNA进行稳定。用购自Qiagen的RNeasy微型试剂盒纯化RNA,并使用购自Ambion的DNAse(无DNA)去除残余量的剩余DNA。用NanoDrop(Thermo)和AgilentBioanalyzer2100确定所分离的RNA的浓度和质量。使RNA杂交到人Affymetrix微阵列(Affymetrix-HuGene-1_0-st-v1)上。
结果
首先通过CD25分子的组成型表达来描述天然CD4+调节细胞(nTreg细胞)。此外,CD25也是所有类型的T细胞的活化标志物,因此无法区别nTreg细胞与活化的常规T细胞(ConvT细胞)。同样的规则也适用于主调节基因Foxp3的表达,Foxp3的表达在nTreg细胞上是组成型的并且在活化convT细胞上被上调。
在发现Foxp3和CD25作为Treg细胞的组成型标志物之后的几年里,分子CD127已经显示在Treg细胞表面上组成型地低,但在所有常规T细胞的表面上组成型地高度表达,这提供了一种在这两种群体被活化时仍对其进行区分的方法。

○CD3+CD4+Foxp3+CD25+CD127hi=活化convT细胞
○CD3+CD4+Foxp3-CD25-CD127hi=休眠convT细胞
除nTreg细胞外,免疫耐受中还涉及另一种充分研究的调节细胞群体。这种称为Tr1淋巴细胞的群体的表型也通过上述分子的表达来表征。结果如下:
○CD3+CD4+Foxp3+CD25+CD127lo=活化Tr1细胞
○CD3+CD4+Foxp3-CD25-CD127lo=休眠Tr1细胞
这些结果证实休眠Tr1细胞可通过CD25和CD127的表达或通过Foxp3、CD25和CD127的表达与nTreg细胞加以区分。然而,活化Tr1细胞显示与nTreg细胞相同的表型(图1)。
为了测试另一种表面分子是否可以区分这两种群体,我们测试了人nTreg和Tr1细胞上的表达CD62L的细胞的数目。大量的休眠和活化Treg细胞都表达CD62L,而只有少量的休眠或活化Tr1细胞表达CD62L(图2)。重要的是,CD62L、CD127和CD25染色的平均荧光强度(MFI)显示,如先前所报道,nTreg组成型地表达CD62L。相比之下,Tr1细胞中组成型地缺乏CD62L(图3)。因此,当纳入以上标志物小组中时,CD62L表型可区别活化Tr1细胞与nTreg细胞。对于所有四个细胞群体,CD127染色的MFI都较低。CD25染色的MFI在nTreg细胞上组成型地高,在活化Tr1细胞上较高,且在休眠Tr1细胞上被下调为低MFI。这一观察也通过RT-PCR实验得到证实(图4)。

图4显示表面CD62L在区别活化Tr1细胞与nTreg细胞方面的作用也适用于来源于小鼠的细胞群体。
用经过荧光标记的CD4、CD25、CD127和CD62L特异性抗体对来自两个健康人类个体的外周血白细胞进行染色。基于通过CD4+CD25-CD127lo表型表征的细胞亚组来确定表达CD62L的细胞的百分比。
图5A和5B显示,在CD4+CD25-CD127lo群体中,标志物CD62L允许区分CD62Llo群体与CD62Lhi群体。因此,标志物CD4、CD25和CD127不足以辨别细胞群体中的休眠Tr1细胞。
总之,表1显示如何依据所用的标志物来辨别每种群体。

Claims (12)
1.一种鉴别休眠Tr1细胞群体的方法,其包括检测细胞群体上的CD4、CD25、CD127和CD62L标志物的细胞表面表达,其中表达:
-CD4和
-低水平的CD127、
-低水平的CD62L、
且不表达
-CD25
的细胞是休眠Tr1细胞。
2.一种鉴别活化Tr1细胞群体的方法,其包括检测细胞群体上的CD4、CD25、CD127和CD62L标志物的细胞表面表达,其中表达:
-CD4、
-CD25、
-低水平的CD127、和
-低水平的CD62L
的细胞是活化Tr1细胞。
3.一种用于分离休眠Tr1细胞群体和/或活化Tr1细胞群体的方法,其包括
-根据权利要求1的方法鉴别休眠Tr1细胞群体和/或根据权利要求2的方法鉴别活化Tr1细胞群体,和
-分离所述群体。
4.一种分离的休眠Tr1细胞群体和/或活化Tr1细胞群体,其是通过根据权利要求3所述的方法获得的。
5.一种用于富集细胞群体中的休眠Tr1细胞和/或活化Tr1细胞的方法,其包括:
-根据权利要求1的方法鉴别休眠Tr1细胞和/或根据权利要求2的方法鉴别活化Tr1细胞,
-选择所述细胞,
从而获得休眠Tr1细胞和/或活化Tr1细胞被富集的细胞群体,其中休眠Tr1细胞和/或活化Tr1细胞的百分比是富集之前休眠Tr1细胞和/或活化Tr1细胞的百分比的至少两倍。
6.一种富集的休眠和/或活化Tr1细胞群体,其是根据权利要求5的方法获得的。
7.一种用于去除细胞群体中的休眠Tr1细胞和/或活化Tr1细胞的方法,其包括:
-根据权利要求1的方法鉴别休眠Tr1细胞和/或根据权利要求2的方法鉴别活化Tr1细胞,
-去除所述细胞,
从而获得休眠Tr1细胞和/或活化Tr1细胞被去除的细胞群体,其中休眠Tr1细胞和/或活化Tr1细胞的百分比是去除之前休眠Tr1细胞和/或活化Tr1细胞的百分比的至多0.75倍。
8.一种医药组合物,其包含根据权利要求4所述的分离的休眠Tr1细胞群体和/或活化Tr1细胞群体,或根据权利要求6所述的富集的休眠Tr1细胞群体和/或活化Tr1细胞群体。
9.根据权利要求4所述的分离的休眠Tr1细胞群体和/或活化Tr1细胞群体或者根据权利要求6所述的富集的休眠和/或活化Tr1细胞群体在制备用于预防或治疗与感染或移植或过敏性疾病有关的免疫应答、或用于预防或治疗炎症性病症或自身免疫病症的药物中的用途。
10.一种医药组合物,其包含休眠Tr1细胞与活化Tr1细胞的混合物,其中所述混合物是通过体外扩增根据权利要求3的方法获得的分离的休眠Tr1细胞群体而获得的,其中在存在抗原的情况下或者在存在有丝分裂原的情况下,并且在存在至少一种抗CD3单克隆抗体的情况下或者在存在自体PBL、白细胞、PBMC、抗原呈递细胞或人工抗原呈递细胞的情况下培养所述休眠Tr1细胞,由此扩增所述休眠Tr1细胞。
11.休眠Tr1细胞和活化Tr1细胞的混合物在制备用于治疗与感染或移植或过敏性疾病有关的免疫应答、或用于预防或治疗炎症性病症或自身免疫病症的药物中的用途,其中所述混合物是通过体外扩增根据权利要求3的方法获得的分离的休眠Tr1细胞群体而获得的,其中在存在抗原的情况下或者在存在有丝分裂原的情况下,并且在存在至少一种抗CD3单克隆抗体的情况下或者在存在自体PBL、白细胞、PBMC、抗原呈递细胞或人工抗原呈递细胞的情况下培养所述休眠Tr1细胞,由此扩增所述休眠Tr1细胞。
12.用于在治疗之前和之后从受试对象获得的生物样品中根据权利要求1的方法鉴别休眠Tr1细胞群体和/或根据权利要求2的方法鉴别活化Tr1细胞群体的试剂在制备用于体外监测所述受试对象中的治疗效果的诊断剂中的用途,其中治疗之后休眠Tr1细胞群体和/或活化Tr1细胞群体的增加对应于对治疗的响答。
