Abstract

Description

发明背景
用于慢性丙型肝炎的护理标准是聚乙二醇干扰素(peginteferon)加利巴韦林的组合1。用护理标准治疗之后,尽管很难预测个体患者是否将实现持续病毒学应答(SVR),但是整体的SVR比率为约50%2-4。
实现SVR的可能性随着一组患者和病毒因素而不同。例如,较年轻的患者,白种人和亚洲患者以及不具有晚期肝纤维化的个体更有可能在治疗之后清除HCV的感染5-8。相似地,感染了HCV基因型2或者3,而不是基因型1的患者和具有血清中的低基线HCVRNA水平的那些患者具有最好的治愈机会2-4,6,8。
如今只有在治疗起始之后,SVR的更精确预测才是可能的。不考虑HCV的基因型,在治疗4或者12周之后清除HCVRNA的个体比具有持续的病毒血症的那些个体具有实现SVR的更好的机会9。快速病毒学应答(RVR,在第4周时检测不到HCVRNA)是SVR的强预测值;相反地,不能实现早的病毒学应答(EVR,在第12周时HCVRNA超过两个对数值的下降)是独立于预处理特征的无应答的强预测值10。
能在对护理标准的潜在的应答者和非应答者之间预期地区分将对具有慢性丙型肝炎的患者的护理具有很大的影响。治疗决策将基于患者对护理标准应答概率而个性化。例如,具有用护理的当前标准实现SVR的最低的可能性的患者可以延迟治疗,直到直接作用的抗病毒剂是可获得的。
相反地,具有实现SVR的高可能性的患者可能更愿意用治疗方案立即起始治疗,所述治疗方案是已知的实体。
除了宿主和病毒因素,宿主的遗传多样性也影响对用护理标准治疗的应答11。最近来自全基因组相关研究的证据提示在IL-28b基因的启动子区中的单核苷酸多态性(SNPs)对用聚乙二醇干扰素加利巴韦林治疗的患者中的SVR的可能性产生了强烈的影响12-14。本分析的目的是在包括首次接受治疗的个体和对以前的聚乙二醇干扰素α-2b(12KD)加利巴韦林的疗程的无应答者的患者的多样组群中探查与早期病毒学应答(EVR)和SVR都相关的SNPs,所述患者接受了聚乙二醇干扰素α-2a(40KD)的单一疗法或者聚乙二醇干扰素α-2a(40KD)或常规的干扰素加利巴韦林的组合治疗。
发明概述

当前推荐的用于具有慢性丙型肝炎的患者的第一线治疗在携带基因型1或4病毒的患者中是聚乙二醇化干扰素α与利巴韦林组合持续48周,并且在携带基因型2或3病毒的患者中持续24周。在干扰素α治疗的一个或者多个疗程之后复发的患者以及以前未治疗过的患者中,发现与利巴韦林组合的治疗比干扰素α单一疗法更有效。然而,利巴韦林显示出明显的副作用,其包括致畸性和致癌性。此外,在约10%至20%的患者中,利巴韦林引起溶血性贫血,需要减少剂量或者中止利巴韦林治疗,所述溶血性贫血可能与利巴韦林三磷酸盐在红细胞中的积聚有关。因此,为降低治疗成本和不利事件的发生率,将治疗调整至更短的持续时间而不降低功效是所希望的。对于用聚乙二醇化干扰素α和利巴韦林治疗的基因型1的患者,缩短的“治疗持续时间”可以是,例如24周。对于用聚乙二醇化干扰素α和利巴韦林组合直接作用抗病毒剂治疗的基因型1的患者,缩短的治疗持续时间可以短至8周、12周或者16周。

单核苷酸多态性,"rs12979860"指的是通过它在SNPs(dbSNP,www.ncbi.nlm.nih.gov/SNP/)数据库的检索号鉴定的SNP,并且其定位于人19号染色体上的IL28b基因的启动子区内。
来自两个大的多国家研究(在实施例部分描述)的数据的这种分析结果证实和扩展了Ge等人12首先报导的用聚乙二醇干扰素加利巴韦林治疗之后在IL-28区的SNPs和SVR之间显著的关联。在本分析中,最显著的SNP(rs12979860)与EVR(p=5.0x10-26)和SVR(p=1.4x10-21)都相关。在治疗后实现了SVR的患者以及非应答者的大的亚群的良好的纯种性质是本研究的独特优势,所述患者不仅包括用聚乙二醇干扰素α-2a(40KD)加利巴韦林治疗的患者,而且也包括用聚乙二醇干扰素α-2a(40KD)单一疗法和常规的干扰素加利巴韦林的组合治疗的患者。
本研究允许产生一些独特的建议。首先,我们提示rs12979860的效应最有可能作用在EVR而非SVR上。在以前的对治疗的非应答者中和用聚乙二醇干扰素α-2a(40KD)单一疗法或者常规的干扰素加利巴韦林治疗的患者中可见与EVR而不是与SVR相关。我们将这解释为意指通过这种SNP作用的机制与血清中的HCVRNA水平快速第一相下降相关而不是与较慢的第二相下降或者肝细胞更新相关。HCVRNA中的第一相下降是被很好地描述的现象,其在基于干扰素的治疗的起始之后不久发生并且已经提示是特定患者对干扰素的敏感性的指标19。
然后,本研究中使用的特定的芯片允许我们排列除了rs12979860之外的SNPs系列的显著性,所述SNPs全部在以前的研究中被鉴定12-14,20,21。尽管在IL28b区鉴定了几个高度显著的标记,但是对rs1297980调整后它们均不与SVR显著地相关。在对rs12979860调整之后,只有一种SNP(rs8099917)是与EVR统计学上显著地相关;然而,在对多重比较调整之后,将不认为该相关是显著的。
此外,通过对rs12979860调整,我们能开始探查可能影响实现病毒学应答概率的潜在的基因-基因相互作用。如上面提到的,在原始分析中鉴定的IL28b区中的其他高度显著的SNPs在该处理后均没有保留;然而,揭示了定位于4号染色体上的以前未报导过的一系列SNPs。
在一些回顾性研究12-14,20-23包括本研究中取得了一致的结果,允许我们自信地指出在IL28b的启动子区的SNPs与用基于干扰素治疗的治疗结果高度相关。已经提示在本区域中的多个启动子中的一个的变化改变了干扰素λ的肝表达。干扰素λ是III型干扰素,其引发与I型干扰素(包括干扰素-α)的Jak/Stat途径重叠的信号途径24-26。我们假设在具有“允许的”SNP的患者中,HCV感染诱导包括III型干扰素(如IL28)的干扰素的局部产生。这导致了在肝中干扰素诱导的基因表达。这种假设与以前的报导一致,所述报导是对基于干扰素治疗的无应答者在治疗起始之前在他们的肝中具有刺激干扰素的基因(ISGs)的增加的肝表达,并且无应答者是处在明显的“预活化”状态中27。没有清楚地解释在内源的ISG诱导水平和对基于干扰素治疗的随后的应答性之间的这种明显的矛盾关系。一些研究者已经推测在轴上的类似于蛋白激酶PKR的ISG诱导的下游效应中的一些可能通过真核起始因子2α(eIF2a)的磷酸化抑制内源的蛋白质产生并且导致肝细胞抗拒外源的干扰素的进一步刺激,不能进一步减少蛋白质的产生28。通过内部的核糖体进入位点翻译HCVRNA并且其部分地是不依赖eIF2a的。因此,尽管是处在“预活化”状态,HCVRNA的复制和进一步病毒体的装配可以不减弱地持续28。这种假设解释了在我们的分析中为什么与更容易治疗的基因型相比,应答差的GWAS基因型与较低的病毒载量相关,尽管以前的分析提示增加基线病毒载量是差的治疗结果的指标6。虽然在这些患者中干扰素的内源产生部分地削弱了病毒复制,但是同时削弱了宿主细胞蛋白质的合成。可能有一个阈值,在所述阈值之上宿主细胞不能进一步降低蛋白质的产生或者增加ISGs的刺激。因此,外源的干扰素不能增加宿主应答。然而,矛盾地是,外源的聚乙二醇化干扰素的施用可以“拯救”对内源干扰素较不敏感的患者。基因多态性可能在ISG的表达中起作用。这引导我们推测最终有可能有选择地靶向干扰素途径以产生导致HCV根除的更有利的ISG谱。
解释4号染色体中SNP与SVR相关,然而不是与EVR相关的可能的机制是更具有挑战性的。最简单的解释可能需要该区域的基因,其涉及仅仅与SVR相关的现象如肝细胞更新。额外的宿主-宿主基因或者宿主-病毒基因的相互作用可能涉及在该机制中,通过该机制两个基因座均发挥它们的效应,并且所述相互作用需要更谨慎的评估。
发现在IL28b区中干扰素治疗敏感度基因座涉及用当前护理标准(SOC),即聚乙二醇干扰素加利巴韦林的治疗中。在不久的将来,可以设想将在起始治疗之前确定一些患者中IL28b基因型,并且将不仅通过HCV基因型使患者分层-如当前推荐的1-而且通过人基因型使患者分层。与当前治疗的规范比较,起始的治疗方案将基于病毒和人的基因型。在这种背景下将需要在预期的试验中处理利巴韦林的推定作用。
这种发现也涉及用于HCV治疗的直接作用抗病毒剂的正进行的发展计划。直接作用抗病毒剂独立于免疫功能发挥特定的抗病毒效应,但是认为发展中的至少一些药物类型可以受益于累加的(如果不是协同的)先天性免疫调节效应。实际上,有可能基于干扰素的治疗将仍然是治疗的骨干,因为需要防止抗性HCV的出现29。HCVRNA编码特定的蛋白质,其可以抑制I型干扰素的诱导。例如HCV的NS3-4A蛋白酶通过干扰干扰素调节因子-3(IRF-3)的磷酸化阻断dsRNA-诱导的干扰素的产生30。因此,NS3-4A蛋白酶是二元的治疗靶标,所述NS3-4A蛋白酶的抑制可以阻断病毒的复制和恢复HCVRNA复制的IRF-3控制。当蛋白酶抑制剂如Telaprevir与第二种小分子或者护理标准组合施用时具有强劲的抗病毒效果,所述蛋白酶抑制剂也抑制蛋白酶功能,HCV通过所述蛋白酶功能削弱宿主干扰素的应答。当在具有干扰素-顽固的和干扰素-敏感的IL28b表型的患者中组合使用直接作用抗病毒剂与聚乙二醇干扰素加利巴韦林时,观察治疗结果是否相似将是重要的。可能干扰素-顽固的患者将对三重治疗(直接作用抗病毒剂加聚乙二醇干扰素加利巴韦林)应答,如同他们对仅用直接作用抗病毒剂的单一疗法的应答。如果是这种情况,那么具有干扰素-顽固的的IL28b表型的患者可能对用药物如telaprevir治疗过程中的抗性突变的选择更敏感31。这种可能性提示不同的情形,其中具有干扰素-敏感的SNP(rs12979860)的患者可能受益于用聚乙二醇干扰素和利巴韦林进行的缩短的治疗,而具有干扰素-顽固基因型的那些患者可能是用于延长的治疗持续时间和/或更强化的治疗方案的候选者。备选地,直接作用抗病毒剂的不含干扰素的组合方案可能更适合具有干扰素-顽固基因型的患者。
我们独特的数据集,包括用于患者的聚乙二醇干扰素α-2a加利巴韦林治疗的登记试验,强调rs12979860SNP与EVR而不是SVR相关,所述患者是以前对用聚乙二醇化干扰素的护理标准治疗的非应答者。换句话说,这种SNP限定患者对干扰素的应答性而不是对治疗的最终反应,因此由于患者实现EVR的可能性,帮助鉴定可能或者不可能经历SVR的患者。独立于EVR预测SVR是不可能的。这对使用直接作用抗病毒剂和干扰素具有很大的关联。
用于干扰素应答性的基线遗传预测值的应用允许调整直接作用抗病毒剂以及使用干扰素的护理标准。定义为弱的干扰素应答的患者,即具有rs12979860中的两个T等位基因的患者,可以是小分子治疗法的差的候选者,特别地,如果除了SOC之外,以单一试剂提供小分子治疗剂,那么所述小分子治疗法依赖于内源或者外源干扰素介导途径的累加或者协同效应。将考虑由于有效的单一疗法,这些患者将具有发展药物抗性突变的增加的风险。
相反地,具有干扰素应答表型的患者,即具有rs12979860中的两个C等位基因的患者,将成为用于仅用SOC治疗或者SOC与小分子组合治疗的更短的疗程的优良的候选者。对干扰素应答性的遗传素质的额外的考虑是组合直接作用抗病毒剂的“调谐”。预测具有可接受的内源干扰素应答性的患者可以是用于靶向病毒功能的药物-如蛋白酶抑制剂(也具有内源干扰素应答的抑制作用)的极好的候选者,并且是用于降低病毒PAMP(病原体相关的分子模式,例如聚合酶抑制剂)量的药物的较差的候选者,因为这些降低病毒PAMP量的药物可以削弱患者通过他们内源干扰素的应答性帮助他们自愈的能力。相似地,与仅用直接作用抗病毒剂或者三重治疗(SOC与一种DAA)相比,经预测具有弱的干扰素应答性的患者可以是用于作为第一线治疗的“四重”治疗(2种直接作用抗病毒剂,加聚乙二醇干扰素和利巴韦林)的候选者。
实施例
方法
患者样品的收集和病毒学终点
分析了来自具有慢性丙型肝炎的一小组患者的样品,所述患者被登记到两个大的随机化的多国家的III期试验中3,15。在一个研究中,首次用干扰素治疗的患者被随机化到仅用聚乙二醇干扰素α-2a(40KD),或者用聚乙二醇干扰素α-2a(40KD)与利巴韦林的组合或者用常规的干扰素α-2a加利巴韦林的48周治疗中3。只有以前对聚乙二醇干扰素α-2a(12KD)加利巴韦林的12-周疗程无应答者适合第二次试验,其将患者随机化到使用聚乙二醇干扰素α-2a(40KD)的标准方案或者诱导剂量方案(所有患者接受标准剂量的利巴韦林)的48周或者72周治疗中15。在别处公开了研究的设计、选择标准和排除标准和这些试验的初步结果3,15。
将从同意参加遗传分析的患者收集的血液样品贮存在罗氏临床样本库(RocheClinicalSamplerepository)中。在罗氏临床样本库中提取DNA并且将其归一化至50ng/μL。最初使用Y-染色体特异的TaqMan测定法(应用生物系统,福斯特市,加利福尼亚州)进行样品的质量检查以评估DNA质量和性别与临床数据的一致性。
病毒学终点包括早期病毒学应答(EVR),其定义为从基线至12周的血清中检测不到HCVRNA(通过CobasAmplicorHCV测试,v2.0,检测极限50IU/mL)或者血清HCVRNA中≥2个对数的下降(通过CobasAmplicorHCV监测器测试,v2.0,定量极限600IU/mL),和持续病毒学应答(SVR),其定义为在24周未治疗的随访期结束时检测不到HCVRNA(<50IU/mL)。
在用于SVR的GWAS分析中,应答者组由来自首次进行干扰素的研究群体的具有SVR的全部基因型-1的患者组成。非应答者由以下组成:1)来自聚乙二醇化干扰素治疗再次攻击的研究群体的全部基因型-1非SVR患者,2)来自用聚乙二醇化干扰素和利巴韦林治疗的首次进行干扰素研究群体的基因型-1非SVR患者。
因此从我们的分析中排除了来自聚乙二醇化干扰素再次攻击的研究群体的全部应答者。
在用于EVR的GWAS分析中,EVR组由来自首次进行干扰素研究的群体的具有EVR的全部基因型-1的患者组成。非EVR组由下列组成:
1)来自聚乙二醇干扰素再次攻击的患者群体的全部患者和2)来自首次进行治疗的研究群体的用聚乙二醇化干扰素+利巴韦林治疗的基因型-1非EVR患者。
在非-基因型1患者中的自我报告的白种人中的IL28B区SNP进行了额外的探查分析,并且每次试验分别进行分析(参见结果部分中的详述)。
基因型数据分析
使用HumanOmnilQuad(v1.0)芯片和iScan扫描仪在IlluminaHDAssaySuper上进行对样品1,016,423种标记的基因分型。对于平行测定样品之间不一致的调用、过量的杂合性、低的调用率(<0.95)、聚类的分离、GenTrain得分、强度和聚类的宽度,执行起始的质量控制以清零SNPs。总共25个样品不能通过起始的质量检查或者随后的基因分型尝试。在质量控制之后,产生了具有基因型调用的1,002,139个SNPs。对于自我报告的白种人群体,计算了每个染色体的SNPs数目,等位基因频率的分布和哈迪-温伯格(HardyWeinberg)平衡。
统计分析
在单变量的逻辑回归模型中评估了在病毒学应答(EVR或SVR)和基线变量(作为连续变量输入的年龄、体重指数[BMI]、HCVRNA水平和ALT系数、以及作为分类变量输入的性别、HCV基因型、组织学诊断[存在或者缺失肝硬化]和种族)之间的关联。
在调整基线BMI、性别、年龄、病毒载量,ALT系数和主成分分析(PCA)组分之后,使用逻辑回归(PROCLOGISTIC,SASv9.2)检验个体SNPs和应答/非应答之间的关联。
基于Purcell等人16提出的PCA,使用整个群体("pgt")产生的家系数据集分析家系。用来自11个不同人种的受试者集合的HapMapIII期创立者补充整个群体并且将其在SASJMPGenomics(SASInstituteInc.,Cary,NC,美国)中分析。将PCA组分与或者不与异常值比较。限定异常值为与变异的前十个推断轴中的一个上的平均值的至少6个标准差的家系的个体。
在X和Y染色体上的或者在线粒体DNA中的SNPs与在HapMap三期创立者(发布号27)中未被基因分型的SNPs或者在已知的高度连锁不平衡的区域(Chr5,44-51.5Mb;Chr6,24-36Mb;Chr8,8-12Mb;Chr11,42-58Mb;Chr17,40-43Mb)中的SNPs一起被除去。使用1000的窗口大小,r2<0.25和100的窗口偏移,用PLINK16使保留的SNPs变少。
通过逻辑回归分析检验病毒学应答(EVR和SVR)和个体SNPs之间的关联。对基线特征(BMI、性别、年龄、基线HCVRNA水平,ALT系数和PCA组分)进行调整。通过似然比检验(LRT)评估显著性。
如下限定空值模型:
病毒学应答=BMI+性别+年龄+HCVRNA水平+ALT系数+PCA组分。在空值模型中假定SNPs的遗传效应是零。
如下限定备选模型:
病毒学应答=BMI+性别+年龄+HCVRNA水平+ALT系数+PCA组分+SNP。
使用LRT将每种备选模型(即具有SNPs)的最大似然估计值与相应的空值模型(即没有SNPs)的最大似然估计值比较,所述最大似然估计值具有卡方分布和等于不同参数的数目的自由度。将SNPs作为连续变量(0,1,2)输入该模型中。PCA组分代表分析的群体中的前5个组分。在自我报告的白种人群体和非-基因型1群体中运行相似的模型。
在白种人的"pgt"群体的IL-28区中的显著SNPs之间(p<10-5)计算了连锁不平衡(LD)。在Haploviewv4.117中用Gabriel等人18的方法推断的LD区组产生了r2的LD图。
结果
可获得总共406个首次进行治疗的患者的样品和以前对用聚乙二醇干扰素α-2b(12KD)治疗的疗程不应答的426个患者的样品。EVR的分析基于来自800个患者的数据,包括实现了EVR的363个个体(45%)和没有实现EVR的437个个体(55%)的数据。SVR的分析基于来自663个患者的数据,包括实现了SVR的245个患者(37%)和没有实现SVR的418个患者(63%)的数据。在表1中显示了EVR和SVR的分析中包括的患者的基线特征。
基因因素的单变量逻辑回归模型显示HCV基因型(p=1.50x10-25)、年龄(5.80x10-18)、ALT系数(p=7.50x10-10)、种族(p=9.20x10-7)、BMI(p=4.70x10-5)和组织学诊断(p=1.30x10-5)与EVR显著地相关,并且HCV基因型(p=4.30x10-27)、年龄(5.50x10-18)、ALT系数(p=2.10x10-8)、种族(p=2.10x10-8)、组织学诊断(p=5.40x10-7)、BMI(p=7.20x10-6)和HCVRNA水平(p=0.0022)与SVR显著地相关。值得注意的,性别与EVR和SVR都不显著地相关并且HCVRRA水平不与SVR显著地相关。
在整个基因型1群体中的GWAS结果
由于高的亲缘系数(指示高度的相关关系),排除了用于基因型分析的总共4个样品。分析来自基因型1的患者的数据,所述患者包括具有已知的EVR状态的627个患者(215个应答者[34.3%]和412个非应答者[65.7%])和具有已知的SVR状态的516个患者(128应答者[24.8%]和388个非应答者[75.2%])。
通过图1中的染色体呈现SVR和EVR的全基因组关联结果。在19号染色体上的IL-28区中鉴定了一系列的高度显著的p值。分位点-分位点图显示除了与19号染色体相关的p值的少数大的偏差,预期的p值和观察的p值非常一致(图2)。对SVR和EVR的逻辑回归分析分别揭示了12个和19个SNPs,p<10-5(表2)。
与SVR和EVR分别相关的前6个SNPs,是相同的并且落入19号染色体的IL-28区内。对于SVR和EVR,前两种SNPs是rs12979860(分别地,p=1.4xl0-21和p=5.0x10-26)和rs12980275(分别地,p=5.8x10-18和p=4.9x10-23)。剩余的6种SNPs与SVR相关(表2),不与EVR相关,均不位于19号染色体上,并且由于在罕见的纯合子类型(BB<10个个体)中观察到了很小的数目,四种(rs943897,rs17671102,rs4961441和rs1892723)可能是假相关。剩余的13种SNPs与EVR相关(表2),均不与EVR相关,只有一种(rs4803223)位于19号染色体上,并且两种(rs1189800和rs4975629)可能是假相关。
在表3中显示了用于全部群体和自我报告的白种人的亚群的对于SVR和EVR的任一模型具有p<10-5的全部SNPs的逻辑回归分析。在对rs12979860的效应调整之后,当重复该分析时,没有标记与SVR显著地相关,并且只有一种标记与EVR显著地相关(rs8099917,p=0.0130)。
随后在表3中呈现的IL-28区内的SNPs之间的连锁不平衡的研究显示了具有高度的连锁不平衡的一个聚类(在rs12979860和rs12980275之间,r2=0.98)和由rs12980275,rs12979860,rs8109886和rs8099917组成的下游的SNPs的第二个聚类(图3)。
在图1c中显示了对rs12979860调整后SVR的全基因结果。对rs12979860调整后SVR的逻辑回归分析结果和额外的非应答者的包括揭示了具有p<10-5的额外的10种SNPs(表4)。有趣地,5种最显著的额外SNPs的中的三种(rs10009948,rs10023606和rs7673763)位于4号染色体上。奇怪的是,这些SNPs只与SVR相关,而不与EVR相关。
在白种人的自我报告群体中rs12979860的探查分析
为了更好地表征和理解IL28B之间的相关性,我们首先探查了自我报告的白种人群体中的非基因型1中的rs12979860之间的关联。使用与GWAS中相同的逻辑回归模型,以5%标称的第一类误差,可见SVR和rs12979860C等位基因的数目之间临界显著的关联(OR=1.88,95%CI=[0.97;3.67],p=0.06)。相似地,可见EVR和rs12979860C等位基因的数目之间的显著的关联(OR=2.27,95%CI[1.12;4.70],p=0.02)。
此外,我们分别描述了两次试验中的rs12979860之间的相关。在首次进行治疗的群体中,可见EVR和rs12979860C等位基因之间的相关,OR接近5(OR=4.9895%CI[2.35;10.53],p=2.6X10-5)。在全部三种治疗层中总是可见相关性。
在用聚乙二醇化干扰素治疗失败的患者群体中,发现与持续病毒应答不相关。然而,当比较EVR(N=185)和非-EVR(N=154)(OR=1.91[1.22;2.96],p=0.003)时,持续存在与rs12979860的关联(OR=1.91[1.22;2.96],p=0.003)。该关联独立于基线处的病毒载量、性别、年龄、治疗和ALT系数。
