Abstract

技术领域

背景技术
本发明涉及作为丝氨酸蛋白酶抑制剂的化合物的用途。蛋白酶或蛋白水解酶主要存在于从细菌和病毒到哺乳动物的生物体中。蛋白酶通过水解肽键消化和降解蛋白质。丝氨酸蛋白酶(EC.3.4.21)在活性位点主要是有活性的丝氨酸残基,具有共同的特征。存在两种主要类型的丝氨酸蛋白酶;胰凝乳蛋白酶/胰蛋白酶/弹性蛋白酶样和枯草杆菌蛋白酶样丝氨酸蛋白酶,它们具有相同空间排列的催化性His、Asp和Ser,但蛋白骨架完全不同。然而,超过20个家族(S1-S27)的丝氨酸蛋白酶已被鉴定,基于结构相似性和其他功能证据将它们分成6族类:SA、SB、SC、SE、SF和SG。胰凝乳蛋白酶/胰蛋白酶/弹性蛋白酶样家族的丝氨酸蛋白酶被细分成两类。“大型”类(约230个残基)包括大部分哺乳动物酶,比如胰蛋白酶、胰凝乳蛋白酶、弹性蛋白酶、激肽释放酶和凝血酶。″小型″类(约190个残基)包括细菌酶。

丝氨酸蛋白酶具有共同的催化机制,特征在于使用胰凝乳蛋白酶编号体系在位置195处有一个特别反应性的Ser残基。丝氨酸蛋白酶的实例包括胰蛋白酶、类胰蛋白酶、胰凝乳蛋白酶、弹性蛋白酶、凝血酶、纤溶酶、激肽释放酶、补体C1、顶体蛋白酶、溶酶体蛋白酶、蚕茧酶、α-水解蛋白酶、蛋白酶A、蛋白酶B、丝氨酸羧肽酶π、枯草杆菌蛋白酶、尿激酶(uPA)、因子Vila、因子IXa和因子Xa。由于丝氨酸蛋白酶在调节多种生理过程中的作用,它们多年以来已被广泛研究,并且是药物靶向研究的主要焦点。
涉及丝氨酸蛋白酶的过程包括血液凝固、纤维蛋白溶解、受精、发育、恶性肿瘤、神经肌肉形成模式和发炎。公知的是,这些化合物可抑制多种循环蛋白酶,以及在组织中活化或释放的蛋白酶。还已知的是,丝氨酸蛋白酶抑制剂可抑制关键的细胞过程,比如粘附、迁移、自由基产生和凋亡。另外,动物实验表明,静脉内给药的丝氨酸蛋白酶抑制剂、变体或表达丝氨酸蛋白酶抑制剂的细胞可提供抵御组织损伤的保护。
丝氨酸蛋白酶抑制剂还已被预测在多种临床领域疾病的治疗中具有潜在的有益用途,所述临床领域比如肿瘤学、血液学、神经病学、肺医学、免疫学、炎症和传染性疾病。丝氨酸蛋白酶抑制剂还在如下疾病的治疗中有利:血栓性疾病、哮喘、肺气肿、肝硬化、关节炎、癌、黑素瘤、再狭窄、动脉粥样硬化、创伤、休克和再灌注损伤。有用的综述参见Expert Opin.Ther.Patents(2002),12(8)。丝氨酸蛋白酶抑制剂还公开在美 国公布的专利申请US 2003/0100089和2004/0180371,和美国专利6,784,182、6,656,911、6,656,910、6,608,175、6,534,495和6,472,393中。
白细胞减少症指总白细胞计数减少至低于约4.0×109细胞/L。通常,该减少是多形核中性白细胞(PMN)数目减少的结果(嗜中性粒细胞减少症),其数量通常小于2.0×109细胞/L,通常低于1.0×109细胞/L。嗜中性粒细胞减少症可以由病毒感染(例如流行性感冒、麻疹、肝炎病毒、水痘、登革热和黄热病、HIV)或不可抗拒的细菌感染包括粟粒性结核和败血病引起。此外,嗜中性粒细胞减少症由于在恶性疾病或血管炎和自身免疫疾病的化疗中使用的照射或药物治疗而引起发展。诱导的嗜中性粒细胞减少症的药物实例为磺酰胺、抗甲状腺药、抗组胺药、抗微生物剂、吩噻嗪和各种镇痛药、镇静剂和抗炎剂或各种有毒化学品。传染剂、药物和有毒化学品诱导细胞死亡可影响嗜中性粒细胞和/或骨髓中的其前体细胞。髓细胞样谱系细胞的抗体在免疫介导的疾病比如系统性红斑狼疮或幼年型类风湿性关节炎中可看到。最后但并非最不重要的各种形式的先天性嗜中性粒细胞减少症已被描述。嗜中性粒细胞减少症的后果不仅来自于循环中PMN的损害,而且来自于传染剂、药物、照射和有毒化学品对骨髓中干细胞和有丝分裂细胞的损害,或者由于细胞分裂减慢、DNA链复制、RNA形成阻断或有丝分裂纺锤体微管的破坏引起。
例如由于对恶性肿瘤、实体瘤或癌症的化疗引起的嗜中性粒细胞减少症导致宿主反应受损,由于感染引起的发病率和死亡率显著。例如,用环磷酰胺、甲氨蝶呤和氟尿嘧啶对早期乳腺癌的化疗导致患有败血症的30%患者出现中性白细胞减少症事件,其要求延迟进一步抗癌治疗或减少剂量。20-30%的剂量减少与患有淋巴瘤的患者中的完全反应速率降低和存活率减少有关,或者与无-继发性复发存活有关。尽管抗菌疗法改善了中性白细胞减少症的败血症,但是每年接受骨髓中毒性化疗的约5%患者由于感染相关并发症死亡。
例如对于基因疗法或嗜中性粒细胞输注制剂,体外处理嗜中性粒细胞及其前体细胞与细胞死亡的增加有关,这是由于骨髓细胞诱导细胞凋亡引起的。
用于治疗嗜中性粒细胞减少症的现有试剂包括G-CSF、GM-CSF,G-CSF共轭聚乙二醇成为加入聚乙二醇的G-CSF。但是上述批准的试剂在减少嗜中性粒细胞减少症及其并发症的风险方面的有效性和相当大的功效在肿瘤学中仍有明显的问题。在为了采集周围血液干细胞而接受G-CSF的健康供体中,观察到很少的脾破裂,但更通常脾体积增加、肺中气体交换障碍和单纯的急性损伤性中风和心肌梗死病例。G-CSF引起骨髓发育不良综合征和急性髓细胞性白血病的证据不是很清楚,需要在进一步的未来长期研究中进行分析。
尽管这些方法很有希望,仍然需要治疗嗜中性粒细胞减少症的改善的治疗性、预防性或诊断性方法。本发明提供一种改善的且可靠的治疗、诊断或预防嗜中性粒细胞减少症的方法,其包括向需要其的受试者给药治疗有效量的丝氨酸蛋白酶抑制剂。
本发明已经实现了如根据前述显而易见的这些目的及其它目的。
发明内容
本发明涉及一种治疗或预防患有嗜中性粒细胞减少症的患者的方法,其包括向所述有需要的受试者给药治疗有效量的丝氨酸蛋白酶抑制剂。优选地,所述丝氨酸蛋白酶抑制剂是激肽释放酶抑制剂,优选地,所述激肽释放酶抑制剂选自hK2、hK3、hK4、hK5、hK6、hK7、hK8、hK9、hK10、hK11、hK12、hK13、hK14、hK15抑制剂或其混合物。最优选地,所述激肽释放酶抑制剂选自hK2、hK4、hK11、hK5、hK14抑制剂或其混合物。更加优选地,所述激肽释放酶抑制剂为hK2抑制剂。优选地,所述丝氨酸蛋白酶抑制剂选自:SEQ ID N°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ ID N°16、SEQ ID N°18或其混合物。
本发明还公开了在治疗或预防患者中嗜中性粒细胞减少症的方法中使用的丝氨酸蛋白酶抑制剂,所述嗜中性粒细胞减少症是由于感染、败血病、化疗、辐射、有毒化学品或作为任一种药物的副作用引起的。优选地,嗜中性粒细胞的数量和/或活化状态减少。所述丝氨酸蛋白酶抑制剂也用于治疗或预防其中嗜中性粒细胞经历细胞死亡的糖尿病患者中的皮肤溃疡,或患有与皮肤缺氧状态相关的外周性动脉疾病和嗜中性粒细胞功能障碍和细胞凋亡的患者中发展的皮肤溃疡。
所述丝氨酸蛋白酶抑制剂还用于治疗或预防辐射诱导的骨髓细胞损害的方法,所述骨髓细胞损害如在治疗恶性肿瘤、核电站事故或使用核武器的过程中存在的。优选地,所述丝氨 酸蛋白酶抑制剂是激肽释放酶抑制剂,优选地,所述激肽释放酶抑制剂选自hK2、hK3、hK4、hK5、hK6、hK7、hK8、hK9、hK10、hK11、hK12、hK13、hK14、hK15抑制剂或其混合物。优选地,所述丝氨酸蛋白酶抑制剂选自:SEQ ID N°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ ID N°16、SEQID N°18或其混合物。
进一步公开了在用于体外制备嗜中性粒细胞及其骨髓前体的丝氨酸蛋白酶抑制剂,
-在向嗜中性粒细胞减少症或骨髓系统遗传性紊乱的患者输注骨髓细胞之前,进行基因疗法的分子操作,
-或者,对于向患有嗜中性粒细胞减少症或嗜中性粒细胞功能障碍的患者输注中性粒细胞及其骨髓前体。
优选地,所述丝氨酸蛋白酶抑制剂选自:SEQ ID N°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ ID N°16、SEQID N°18或其混合物。
本发明进一步提供一种预防患者的骨髓细胞细胞凋亡的方法,其包括向所述需要其的患者给药治疗有效量的丝氨酸蛋白酶抑制剂:
(1)对于基因疗法,在转染骨髓细胞期间或之后进行,
(2)对于血细胞生成重建,在血液干细胞动员期间进行,和/或
(3)对于基因疗法的血细胞生成重建或通过输注中性粒细胞治疗嗜中性粒细胞减少症,在髓细胞样谱系的细胞输注期间进行。
优选地,所述丝氨酸蛋白酶抑制剂选自:SEQ ID N°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ ID N°16、SEQID N°18或其混合物。
本发明还提供用于诊断、预测、预防或治疗哺乳动物中中性粒细胞减少症的试剂盒,特征在于所述试剂盒包含丝氨酸蛋白酶抑制剂、任选的试剂和/或使用说明书。优选地,所述丝氨酸蛋白酶抑制剂包括可检测标签或者可以结合可检测标签形成可检测的复合物。而且,优选地,所述丝氨酸蛋白酶抑制剂是激肽释放酶抑制剂,优选地,所述激肽释放酶抑制剂选自hK2、hK3、hK4、hK5、hK6、hK7、hK8、hK9、hK10、hK11、hK12、hK13、hK14、hK15抑制剂或其混合物。优选地,所述丝氨酸蛋白酶抑制剂选自:SEQ ID N°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ IDN°16、SEQ ID N°18或其混合物。

具体实施方式
胰凝乳蛋白酶超家族的一些丝氨酸蛋白酶,包括t-PA、纤溶酶、u-PA和血液凝固级联的蛋白酶,是除了包含丝氨酸蛋白酶催化结构域之外,还包含部分负责调节其活性的其他结构域的大分子(Barrett,1986;Gerard等人,1986;Blasi等人,1986)。其中,重要的丝氨酸蛋白酶是胰蛋白酶样酶,例如胰蛋白酶、类胰蛋白酶、凝血酶、激肽释放酶和因子Xa。丝氨酸蛋白酶的靶点与比如以下过程相关:血液凝固;补体介导的水解、免疫应答、炎症、疼痛感觉、肾小球肾炎、胰腺炎、癌症、调节受精、细菌感染和病毒成熟。通过抑制对具体靶点有高特异性的丝氨酸蛋白酶,可以在体内抑制许多生物学过程,这可能对宿主有明显的作用。
丝氨酸蛋白酶抑制剂(serpins)包括多组蛋白质,这些蛋白质形成已包含超过100个来自多种生物体(例如病毒、植物和人)的成员的超家族。Serpins已演化超过5亿年,在系统发生学中分成有抑制功能和非抑制功能的蛋白质(Hunt and Dayhoff,1980)。非抑制性丝氨酸蛋白酶抑制剂比如卵清蛋白缺乏蛋白酶抑制活性(Remold-O′Donnell,1993)。serpin家族成员的主要功能似乎是中和过表达的丝氨酸蛋白质酶活性(Potempa等人,1994)。serpin在细胞外基质重塑、炎症反应的调节和细胞迁移中起作用(Potempa等人,1994)。丝氨酸蛋白酶抑制剂分成如下家族:牛胰腺胰蛋白酶抑制剂(Kunitz)家族,也称为基础蛋白酶抑制剂(Ketcham等人,1978);Kazal家族;链霉菌属枯草杆菌蛋白酶抑制剂家族;serpin家族;大豆胰蛋白酶抑制剂(Kunitz)家族;马铃薯抑制剂家族;和包曼-毕尔克(Bowman-Birk)家族(Laskowski等人,1980;Read等人,1986;Laskowski等人,1987)。属于丝氨酸蛋白酶抑制剂家族的丝氨酸蛋白酶抑制剂包括纤溶酶原活化因子PAI-1、PAI-2和PAI-3,C1酯酶抑制剂,α-2-抗纤维蛋白溶素、contrapsin、α-1-抗胰蛋白酶、抗凝血酶III、蛋白酶连接蛋白I、α-1-抗胰凝乳蛋白酶、蛋白C抑制剂、肝素辅助因子II和生长激素调节蛋白(Carrelletal.,1987;Sommeretal.,1987;Suzuki等人,1987;Stump等人,1986)。
许多丝氨酸蛋白酶抑制剂具有宽的特异性,能够抑制胰凝乳蛋白酶超家族的蛋白酶(包括血液凝固丝氨酸蛋白酶)和链霉菌属枯草杆菌蛋白酶超家族的丝氨酸蛋白酶(Laskowski等人,1980)。serpins对丝氨酸蛋白酶的抑制已被综述在Travis等人 (1983)、Carrelletal.(1985)和Sprengers等人(1987)中。可获得大量完整抑制剂和切割形式的serpinα-1-抗胰蛋白酶的晶体学数据(Read等人,1986),所述完整抑制剂包括BPTI、Kazal、SSI、大豆胰蛋白酶和马铃薯抑制剂家族的成员。虽然这些丝氨酸蛋白酶抑制剂是具有不同大小和序列的蛋白质,但是迄今为止研究的完整抑制剂都共同具有从所述分子表面伸出的特征环,称为反应位点环,该环包含所述同源丝氨酸蛋白酶的活性位点的识别序列(Levin等人,1983)。不同丝氨酸蛋白酶抑制剂中环的结构相似性是显著的(Papamokos等人,1982)。每种抑制剂的特异性被认为主要是由紧邻丝氨酸蛋白酶对抑制剂的可能的切割位点氨基末端的氨基酸的特性确定。该氨基酸被称为Pi位点残基,被认为可与丝氨酸蛋白酶活性位点的丝氨酸形成酰键(Laskowski等人,1980)。serpin是否拥有抑制功能强烈依赖于位于编码区羧基末端附近的反应位点环的铰链区中的共有序列。在所述反应位点环外部,不同家族的丝氨酸蛋白酶通常是结构上不相关的,但Kazal家族和链霉菌属枯草杆菌蛋白酶家族的呈现出一些结构和序列相似性。
如本文使用的,提供下述定义是为了促进对本发明的理解。
“一个”或“一种”是指“至少一个(种)”或“一种或多种”。
术语“包含”通常用于表示包括的意思,即允许存在一个或多个特征或组分。

“片段”是指与底物活性位点的相应序列在长度上共享至少40%氨基酸的序列。可以使用这些序列的条件是它们显示出与它们的天然来源序列相同的性质。这些序列与底物活性位点序列在长度上共享超过70%,优选超过80%,特别是超过90%,更特别是超过95%氨基酸。

术语“激肽释放酶”涉及腺体或组织激肽释放酶。腺体或组织激肽释放酶是丝氨酸蛋白酶亚家族,具有高度的底物特异性,并且在多种组织和生物流体中的表达不同。术语“激肽释放酶”在20世纪30年代第一次出现于文献中,当时在胰(胰在希腊语是“Kallikreas”)分离物中发现了大量蛋白酶(Kraut等人1930,Werle 1934)。现在,激肽释放酶被分成两组,血浆激肽释放酶和组织激肽释放酶,这两组激肽释放酶的分子量、底物特异性、免疫学特征、基因结构和所释放激肽的类型显著不同。激肽释放酶包括15种同源单链,~25-30kDa的分泌型丝氨酸内肽酶的家族,有直系同源物存在于来自至少6个哺乳动物目的物种中。这些激肽释放酶是hK2、hK3、hK4、hK5、hK6、hK7、hK8、hK9、hK10、hK11、hK12、hK13、hK14和hK15。优选地,激肽释放酶是hK2、hK4、hK11和hK14。
如本文使用的“抗体”指一类在抗原刺激之后由免疫系统的B细胞产生的血浆蛋白。哺乳动物(即人类)抗体是IgG、M、A、E或D类的免疫球蛋白。当用于本发明目的时,术语“抗体”包括,但不限于,多克隆抗体、单克隆抗体、嵌合抗体、人源化的、人抗体、内在化抗体、中和抗体、抗独特型抗体、其免疫活性片段或衍生物、具有免疫活性的重组蛋白质和结合激肽释放酶或膜锚定的丝氨酸蛋白酶的免疫共轭物。

如本文使用的“疾病”指由于各种原因(比如感染、遗传缺陷或环境应激)引起的生物体的部分、器官或系统的病理学病症,其以可识别的体征或症状为特征。用于治疗目的的“哺乳动物”指分类为哺乳动物的任何动物,包括人、家养和农场动物以及动物园动物、体育动物或宠物,例如狗、马、猫、奶牛、猴子等。优选地,哺乳动物是人。

“抑制剂”指多肽或化合物,其优选地通过结合激肽释放酶或丝氨酸蛋白酶来特异性抑制所述激肽释放酶或丝氨酸蛋白酶的功能。
“反应性Serpin环”或“反应位点环”或RSL是、指出现于serpin中并参与与可能的靶蛋白酶相互作用的暴露的柔性反应位点环。从易切断键的氨基侧的残基开始,离开所述键往外,残基通常称为P1、P2、P3等。所述易切断键后的残基称为P1’、P2’、P3’等。通常,RSL由6-12个氨基酸残基组成。

有利地,本发明的丝氨酸蛋白酶抑制剂可以是丝氨酸蛋白酶胰蛋白酶样酶,优选地为激肽释放酶的抑制剂。本发明的激肽释放酶抑制剂选自hK2、hK3、hK4、hK5、hK6、hK7、hK8、hK9、hK10、hK11、hK12、hK13、hK14或hK15抑制剂。优选地,激肽释放酶抑制 剂选自hK2、hK4、hK11、hK5和hK14抑制剂。更优选地,激肽释放酶抑制剂是hK2抑制剂。

这些嵌合体抑制剂蛋白已经通过以下方式获得:修饰α1-抗胰凝乳蛋白酶(rACT)的RSL以改变该serpin的特异性,已知rACT可抑制一大组人类的酶,比如胰凝乳蛋白酶、肥大细胞糜蛋白酶、组织蛋白酶G、前列腺激肽释放酶hK2和PSA(hK3)。通过噬菌体展示技术选择作为酶hK2底物的肽序列(如在国际专利申请PCT/IB2004/001040中所述的)已被用于替换RSL的易切断键和邻近氨基酸残基。重组体抑制剂通常是由细菌生产的,并通过亲和色谱法纯化。
另外,本申请人还已发现,以编码蛋白C抑制剂(PCI)的RSL的底物五肽替换位于rACTWT的RSL结构中的残基P3-P3’,会导致能够抑制激肽释放酶hK2和hK3的嵌合体抑制剂(MDCI)的产生。

根据本发明的抗体可以选择性地结合激肽释放酶或丝氨酸蛋白酶,而不会结合(或弱结合)非靶向多肽。它们也可以结合天然存在的激肽释放酶或丝氨酸蛋白酶或其重组体多肽。本发明的抗体可以结合细胞表达的,即由包括细胞表面、结合膜、细胞质或分泌物形式的细胞表达的激肽释放酶或丝氨酸蛋白酶。它们也可以结合激肽释放酶或丝氨酸蛋白酶上的一个或多个结构域,包括细胞质结构域、跨膜结构域和/或胞外结构域。或者,它们可以结合天然和/或变性形式的任一种激肽释放酶或丝氨酸蛋白酶。
本领域技术人员应当理解,抗体针对的激肽释放酶或丝氨酸蛋白酶的区域或抗原决定部位可以随着预期的应用而变化。
根据本发明的抗体可以识别和结合激肽释放酶或丝氨酸蛋白酶的任何部分,包括细胞质结构域、跨膜结构域和/或胞外结构域、或其任何部分,比如其片段或衍生物。
根据本发明的抗体可以是多克隆制品,其包括针对免疫原上不同抗原决定部位的不同抗体群,比如激肽释放酶或丝氨酸蛋白酶用作免疫原。
多克隆抗体可以通过本领域熟知的方法制备。通常,可以使用分离的激肽释放酶或丝氨酸蛋白酶或其片段作为免疫原产生任一种抗体(例如,单克隆的、多克隆的等)。另外,免疫原可以是包括融合V5、His、麦芽糖结合蛋白、GST或人Ig的全部或部分靶多肽的融合蛋白。例如,之前已经使用具有例如融合麦芽糖结合蛋白的人hepsin的细胞外结构域的融合蛋白产生了多克隆抗体(Y Kazama,等人,1995J Biol Chem 270:66-72)。

本发明还预期其中激肽释放酶抑制剂和/或丝氨酸蛋白醇抑制剂为多聚肽体形式的情况。
如在WO 98/18943(Kajava等人)和WO2004087766(Universitéde Lausanne)中公开的“多聚肽体”是一种高亲和力分子,其利用了诱导异常细胞信号的多聚原理,将所述文献的全部内容并入本文作为参考。所述多聚化结构域由人软骨寡聚基质蛋白(COMP)部分组成,所述蛋白融合绞链区或间隔基(优选地包含19个来自人IgA的氨基酸)和能够结合受体(配体)的结构域(结合域)。多聚肽体分子的原理允许强结合在表达高水平激肽释放酶标记物和丝氨酸蛋白酶的细胞或组织上。“十聚肽体(Decabodies)”是以相同原理构建的,不同在于其具有十个臂,因此具有十个结合域。
通常,根据本发明的疾病是其中多形核白细胞、嗜中性粒细胞的数量存在由于感染、败血病、辐射、化疗、药物副作用或有毒化学品作用而引起减少的问题的疾病。
本发明还包括在糖尿病性皮肤溃疡中局部施用激肽释放酶抑制剂,以预防嗜中性粒细胞的细胞死亡,从而恢复嗜中性粒细胞的细胞结构和功能。

应理解的是,本发明组合物的合适剂量将取决于接受者的年龄、性别、健康和体重,(如果有的话)并行疗法的类型和所需效果的性质。
合适的剂型将取决于疾病、抑制剂和给药方式;可能的剂型包括片剂、胶囊剂、锭剂、牙科糊剂、栓剂、吸入剂、溶液、软膏剂和肠胃外贮库剂。由于本发明还涵盖对(例如所述抑制剂的)氨基酸的氨基酸修饰,这可用于将所述抑制剂交联于不溶于水的基质或其他大分子载体,以增加溶解性、吸附性和通过血脑屏障的渗透性。这样的修饰是本领域熟知的,并且可选地会消除或减弱所述肽的任何可能的不利副作用等。

“可检测的标记物”是用于诊断目的的可检测的分子或检测部分,例如具有具体结合性质的酶或肽,例如抗生蛋白链菌素或辣根过氧化物酶。检测部分进一步包括化学部分,比如可以通过结合于特定同族可检测部分例如标记的抗生物素蛋白进行检测的生物素。
优选地,可检测的标记物包括荧光标记物和在本领域中通常用于MRI-CT成像的标记物。许多荧光材料是已知的,并且可用作标记物。这些包括例如荧光素、罗丹明、金胺、得克萨斯红、AMCA蓝和萤光黄。
本发明的激肽释放酶抑制剂或丝氨酸蛋白酶抑制剂可以载有作为检测部分的放射性标记物,比如同位素3H、14C、32P、35S、36Cl、51Cr、57Co、58Co、59Fe、90Y、121I、124I、125I、131I、111In、211At、198Au、67Cu、225Ac、213bu、99Tc和186Re。当使用放射性标记物时,那么可以利用已知的现有计数方法来鉴定并定量具体的结合数目。在其中标记物为酶的情况下,检测可以通过现有技术中已知的目前使用的比色技术、分光光度技术、荧光分光光度技术,安培测定技术或气体定量技术中的任一种来完成。
在体内成像的实例中,本发明的标记物可以共轭成像剂而非放射同位素,所述成像剂包括但不限于磁共振成像增强剂。螯合基团的实例包括EDTA、卟啉、多胺冠醚和多肟。顺磁离子的实例包括钆、铁、锰、铼、铕、镧、钬和铒。
本发明的另一个目的是提供一种在哺乳动物中诊断、预后、预防或治疗嗜中性粒细胞减少症的试剂盒,所述试剂盒包含本发明的组合物,任选的试剂和/或使用说明书。
本发明的试剂盒可以进一步包括单独的药物剂型,所述药物剂型包含例如抗癌试剂,选自化疗剂、抗表皮生长因子受体抗体、放射免疫治疗剂、及其组合。
通常,所述试剂盒包括容器以及贴在所述容器上的标签或包装说明书。合适的容器包括,例如瓶子、小瓶、注射器等。所述容器可由多种材料比如玻璃或塑料制成。所述容器装有有效治疗病症的组合物,并且可具有无菌入口(例如所述容器可以是静脉内溶液袋或具有可以通过皮下注射针头穿透的塞子的小瓶)。所述标签或包装说明书标明所述组合物用于治疗的选择病症,比如嗜中性粒细胞减少症。
可选地或另外地,所述试剂盒可以进一步包括第二个(或第三个)容器,其含有可药用缓冲液,比如抑菌性注射用水(BWFI)、磷酸盐缓冲盐水、林格溶液和葡萄糖溶液。其可以进一步包含根据商业和使用者的观点来看需要的其它材料,包括其它缓冲液、稀释剂、过滤器、针头和注射器。
本发明还公开了本发明的组合物作为药理学工具在开发和标准化用于在哺乳动物中诊断、预后、预防或治疗嗜中性粒细胞减少症的体外和体内测试系统中的用途。本发明还涵盖了用于在组织样品中诊断、预后、预防或治疗嗜中性粒细胞减少症的检测测定,其包括用本发明的组合物接触组织样品,确定并测量检测标记物的量,并且将该量与所述组织样品中存在或不存在嗜中性粒细胞减少症相关联。

为了评价嗜中性粒细胞在体外的生存力,将来自健康供体的周围血液进行红细胞溶解,分离嗜中性粒细胞或外周血单核细胞(PBMCs)。除非另有说明,在96孔微量滴定板(5×105个细胞/孔)中进行在RPMI10%FCS中的培养。基于结合荧光膜联蛋白V-蛋白结合或通过FACS(荧光活化细胞分类器)分析测量CD11b或CD16表面表达来评价嗜中性粒细胞或PBMCs的细胞凋亡百分数。
实施例1:MDPK67b以剂量依赖性方式降低体外嗜中性粒细胞的编程性细胞死亡,但是对于T-细胞存活没有显著的影响

图1a:当用MDPK67b培养时,嗜中性粒细胞和T细胞的膜联蛋白-V染色。用作为指示的、浓度范围6μM至60μM的MDPK67b培养细胞24或48小时,或用作为对照的PBS培养。通过膜联蛋白V染色和FACS分析评价细胞凋亡。显示的白细胞群是根据其在正向散射/侧向散射FACS点图(嗜中性粒细胞)中的形态门控的,或者由阳性染色CD3(T细胞)门控的。

如所指示的、浓度范围60μM(稀释1)至60pM(稀释7)的MDPK67b或MDOKG9培养嗜中性粒细胞18小时。如上所述评价细胞凋亡。
结论:60μM下至0.6μM剂量的MDPK67b抑制嗜中性粒细胞的细胞凋亡。MDOKG9在保护嗜中性粒细胞进入细胞凋亡方面具有类似的作用。该作用对于嗜中性粒细胞有特异性,MDPK67B没有抑制单核细胞或淋巴细胞的细胞凋亡。
实施例2:MDPK67b介导的保护嗜中性粒细胞抗细胞凋亡与培养条件无关。
图2:通过膜联蛋白-V染色MDPK67b处理的嗜中性粒细胞对各种细胞培养条件的比较。
用所示浓度的MDPK67b培养嗜中性粒细胞。不含MDPK67b的PBS作为对照。将嗜中性粒细胞以5×106/ml(高密度)或3×105/ml(低密度)铺板(100μl/孔),通过膜联蛋白V染色和FACS分析评价嗜中性粒细胞的细胞凋亡。同时评价在在代替RPMI10%FCS的无血清培养基(X-Vivo 15)中5×106/ml嗜中性粒细胞的培养。
结论:MDPK67b抑制体外嗜中性粒细胞的细胞凋亡与细胞密度和生长培养基中存在或不存在血清无关。
实施例3:Src酪氨酸激酶抑制剂PP2逆转MDPK67b介导的嗜中性粒细胞细胞凋亡的降低

用所示浓度的MDPK67b培养嗜中性粒细胞,通过FACS评价表达高水平CD16或CD11b的嗜中性粒细胞的百分数。所显示代表性的FACS图。

在存在或不存在Src酪氨酸激酶抑制剂PP2(最终浓度10μM)下,用所示浓度的MDPK67b培养嗜中性粒细胞。通过FACS分析测量高表达嗜中性粒细胞的CD11b和CD16的细胞凋亡和相对频率。
结论:MDPK67b以剂量依赖性提高以高水平表达CD16和CD11b的嗜中性粒细胞频率,其与细胞凋亡减少有关。在MDPK67b的存在下高表达嗜中性粒细胞的CD11b的频率增加和细胞凋亡减少可以在Src酪氨酸激酶抑制剂PP2的存在下逆转。用阻断细胞内信号通路的其他激酶抑制剂包括PI3K抑制剂Ly294002和ERK抑制剂PD98059观察到类似作用。
实施例4:在保护嗜中性粒细胞免于细胞凋亡方面,MDPK67b比G-CSF的作用更好

结论:MDPK67b对嗜中性粒细胞细胞凋亡的作用没有受到G-CSF的影响,单独的G-CSF仅仅对于嗜中性粒细胞细胞凋亡有轻微保护作用。
实施例5:MDPK67b减少细胞抑制性药物诱导的嗜中性粒细胞的细胞凋亡

图5a:用MDPK67b和依托泊苷处理的嗜中性粒细胞的膜联蛋白-V染色。在用MDPK67b(6μM)加依托泊苷(125μg/ml)、单独的依托泊苷或PBS培养细胞18小时。通过膜联蛋白V染色和FACS分析评价细胞凋亡。相应的白细胞群是根据其在正向散射/侧向散射FACS点图中的形态门控的,。

如所指示的、单独的MDPK67b(0.06μM)或MDPK67b(0.06μM)加递增浓度的依托泊苷(μg/ml)或PBS培养细胞18小时。如上所述,通过膜联蛋白V染色评价细胞凋亡,并进行FACS分析。
图5c:用MDPK67b和依托泊苷处理的嗜中性粒细胞的CD16染色。用作为指示的、单独的MDPK67b(0.06μM)或MDPK67b(0.06μM)加递增浓度的依托泊苷(μg/ml)或PBS培养细胞18小时。通过FACS分析评价高表达嗜中性粒细胞的CD16的百分数。
结论:甚至高剂量(至多125μg/ml)的细胞抑制性药物依托泊苷只是部分地阻断MDPK67b的细胞凋亡减少作用。
实施例6:在白血病细胞系和供体衍生的单核细胞和嗜中性粒细胞中KLK表达的RT-PCR分析
材料和方法:
在含有10%灭活胎牛血清的合适标准介质中培养DUDU-145、PC-3、T47D、OVCAR-3、HL-60、THP1和U937细胞系,并37℃下,用5%的CO2培养。分离单核细胞和嗜中性粒细胞。按照制造商的说明,使用Trizol试剂(Life Technologies,Inc.)和PureLink Micro-to-Midi试剂盒(Invitrogen)从细胞中提取出全部的RNA,在20-μl反应物中,使用上标III(Invitrogen)将2μg的全部RNA逆转录到第一链cDNA中。
使用对于每种激肽释放酶有特异性的引物进行PCR反应,并使用肌动蛋白引物作为对照。所有的引物都已经描述在文献(Harvey TJ等人,J Biol Chem,2000Dec 1;275(48):37397-406.Yousef GM等人,J Biol Chem.2001 Jan 5;276(1):53-61.Yousef GM等人,Cancer Res.2001Apr 15;61(8):3425-31)中。根据PCR反应,使用从包括DU-145、PC-3、T47D、OVCAR-3的不同细胞系中分离的RNA作为KLK表达的阳性对照(Harvey TJ等人,J Biol Chem,2000 Dec1;275(48):37397-406)。
循环条件取决于靶基因,主要如在Harvey TJ等人,(J Biol Chem,2000 Dec1;275(48):37397-406)中描述的。使PCR混合物在2%琼脂糖凝胶上进行电泳移动,并通过菲啶溴红染色显影。当显示时,在电泳之后从再一个2%琼脂糖凝胶上切下预测尺寸的DNA带,对回收DNA测序。

结果:
表2:在白血病细胞系和供体衍生的单核细胞和嗜中性粒细胞中,通过RT-PCR分析获得的15KLK基因的表达方案。使用的下述符号代表:++中等表达/高表达;+,低表达;(1)对预测尺寸的PCR产品测序,证实其为正确序列。
结论:
在白血病细胞系和分离的人血细胞中KLK表达水平的RT-PCR分析显示表达了多种KLK,并且对于KLK蛋白酶家族,不同细胞具有非常多种的表达方案。在KLK表达水平方面的这些差异可能涉及激肽释放酶抑制剂对于离体培养这些细胞具有不同作用,如描述的防止嗜中性粒细胞的细胞凋亡的保护作用。

Claims (22)
1.一种用于治疗或预防患有嗜中性粒细胞减少症的患者的方法,其包括向所述有需求的患者给药治疗有效量的丝氨酸蛋白酶抑制剂。
2.权利要求1所述的方法,特征在于所述丝氨酸蛋白酶抑制剂是激肽释放酶抑制剂。
3.权利要求2所述的方法,特征在于所述激肽释放酶抑制剂选自hK2、hK3、hK4、hK5、hK6、hK7、hK8、hK9、hK10、hK11、hK12、hK13、hK14、hK15抑制剂或其混合物。
4.权利要求3所述的方法,特征在于所述激肽释放酶抑制剂选自hK2、hK4、hK11、hK5、hK14抑制剂或其混合物。
5.权利要求4所述的方法,特征在于所述激肽释放酶抑制剂是hk2抑制剂。
6.权利要求1所述的方法,其中所述丝氨酸蛋白酶抑制剂选自SEQ ID N°2、SEQID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ ID N°16、SEQ ID N°18或其混合物。
7.在治疗或预防患者的嗜中性粒细胞减少症的方法中使用的丝氨酸蛋白酶抑制剂,所述嗜中性粒细胞减少症是由于感染、败血病、化疗、辐射、有毒化学品或作为任何药物的副作用引起的。
8.权利要求7所述的在治疗或预防嗜中性粒细胞减少症的方法中使用的丝氨酸蛋白酶抑制剂,其中嗜中性粒细胞的数量和/或活化状态受损害。
9.权利要求8所述的丝氨酸蛋白酶抑制剂,用于治疗或预防皮肤溃疡的方法中,所述皮肤溃疡为在其中嗜中性粒细胞遭受细胞死亡的糖尿病患者中,或者为在患有与皮肤缺氧状态相关的外周性动脉疾病和嗜中性粒细胞功能障碍及细胞凋亡的患者中发展的。
10.权利要求7所述的丝氨酸蛋白酶抑制剂,用于治疗或预防辐射诱导的骨髓细胞损伤的方法中,所述骨髓细胞损伤如在恶性肿瘤的治疗、核电站事故或使用核武器中出现的。
11.权利要求7-10所述的丝氨酸蛋白酶抑制剂,其中所述丝氨酸蛋白酶抑制剂是激肽释放酶抑制剂。
12.权利要求11所述的丝氨酸蛋白酶抑制剂,特征在于所述激肽释放酶抑制剂选自hK11、hK3、hK4、hK5、hK6、hK7、hK8、hK9、hK10、hK11、hK111、hK13、hK14、hK15抑制剂或其混合物。
13.权利要求7-12所述的丝氨酸蛋白酶抑制剂,其中所述丝氨酸蛋白酶抑制剂选自SEQ ID N°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ ID N°16、SEQ ID N°18或其混合物。
14.在体外制备嗜中性粒细胞及其骨髓前体中使用的丝氨酸蛋白酶抑制剂
-在向嗜中性粒细胞减少症或骨髓系统的遗传性紊乱的患者输注骨髓细胞之前,进行基因疗法的分子操作,
-或者,对于向患有嗜中性粒细胞减少症或嗜中性粒细胞功能障碍的患者输注使用嗜中性粒细胞及其骨髓前体。
15.权利要求14所述的丝氨酸蛋白酶抑制剂,其中所述丝氨酸蛋白酶抑制剂选自SEQ ID N°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ ID N°16、SEQ ID N°18或其混合物。
16.一种预防患者的骨髓细胞细胞凋亡的方法,其包括向所述需要其的患者给药治疗有效量的丝氨酸蛋白酶抑制剂:
(1)对于基因疗法,在转染骨髓细胞期间或之后进行,
(2)对于血细胞生成重建,在血液干细胞转移期间进行,和/或
(3)对于基因疗法的血细胞生成重建或通过输注嗜中性粒细胞治疗嗜中性粒细胞减少症,在髓细胞样谱系的细胞输注期间进行。
17.权利要求16所述的方法,其中所述丝氨酸蛋白酶抑制剂选自SEQ IDN°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ ID N°14、SEQ ID N°16、SEQ ID N°18或其混合物。
18.一种用于诊断、预测、预防或治疗哺乳动物中嗜中性粒细胞减少症的试剂盒,特征在于所述试剂盒包含丝氨酸蛋白酶抑制剂,任选的试剂和/或使用说明书。
19.权利要求18所述的试剂盒,其中所述丝氨酸蛋白酶抑制剂包括可检测的标记物,或者可结合于可检测的标记物以形成可检测的复合物。
20.权利要求18-19所述的试剂盒,其中所述丝氨酸蛋白酶抑制剂是激肽释放酶抑制剂。
21.权利要求20所述的试剂盒,其中所述激肽释放酶抑制剂选自hK20、hK3、hK4、hK5、hK6、hK7、hK8、hK9、hK10、hK11、hK120、hK13、hK14、hK15抑制剂或其混合物。
22.权利要求18-21所述的试剂盒,其中所述丝氨酸蛋白酶抑制剂选自SEQ ID N°2、SEQ ID N°4、SEQ ID N°6、SEQ ID N°8、SEQ ID N°10、SEQ ID N°12、SEQ IDN°14、SEQ ID N°16、SEQ ID N°18或其混合物。
