Description
本发明涉及测定样品中γ-羟基丁酸浓度的方法以及适用于实施所述方法的组合物和药盒。本发明还涉及将所述方法应用于微量滴定板或自动分析仪(用于随机存取分析的设备,通常在临床实验室中使用)的用途。
γ-羟基丁酸(4-羟基丁酸,C4H8O3)通常简写为GHB,是一种内源性物质,也是一种治疗性药物,这种药物在许多国家是不合法的,也是一种天然存在的物质,在中枢神经系统、藤本植物、牛肉、小柑橘类水果以及几乎所有其他生物中发现少量的这种物质。γ-羟基丁酸目前在美国受到管制,并且由爵士制药公司(Jazz Pharmaceuticals)以Xyrem的名称销售。
在医疗方面,GHB用作全身麻醉剂,用于治疗诸如失眠、临床忧郁症、发作性睡眠病和酗酒等病症,以及用于提高运动成绩。
在意大利,商品名为Alcover(ATCN 07BB)的GHB被用于酗酒的治疗,用于急性戒酒和长期戒除酒瘾的媒介。在美国,食品和药物管理局(FDA)允许使用商品名为Xyrem的GHB来降低发作性睡眠病患者猝倒发作的次数。
当以其钠盐或钾盐形式使用GHB时,会消耗大量过多的钠或钾,患有心功能不全、高血压或肾功能降低的人群应当考虑这些。当与食物一起摄入时,GHB钠盐的生物利用度被显著降低,因此建议在饭后等待至少2小时再摄入药剂。由于其强烈的镇静作用,患者在服用GHB钠盐之后的六个小时不应当驾驶或操作重型机械。
临床试验中Xyrem带来的副作用包括:头痛、恶心、鼻咽炎、头晕、嗜睡、呕吐、尿失禁、意识错乱、呼吸困难、触感减退、感觉异常、不安、眩晕以及视力模糊。
γ-羟基丁酸(GHB)还是一种违禁化合物,其在美国和其他国家已经成为与药物有关的昏迷的主要原因。事实上,在美国GHB过量使用的数量现在已经取代了MDMA(摇头丸)过量使用。在二十世纪六十年代美国医学界抵制GHB,但是由于其自由穿过血脑屏障和抑制意识的能力(导致兴奋)以及毒性作用的原因,GHB已经在许多人群中变得流行。GHB还在互联网上作为睡眠辅助药品和抗抑郁剂以及减肥产品兜售,尽管这些用途没有得到经证实的科学研究的证实并且可能带有潜在致命的曲解。从二十世纪八十年代后期当类固醇受到管制时GHB首先作为类固醇的替代品开始,GHB已经发展成耗尽急诊室服务、破坏那些已经失去对GHB成瘾的亲人的人的生活以及威胁那些对GHB成瘾的人的家庭/朋友的多方面医疗噩梦。到目前为止,对大部分执法人员、医务人员/法医人员以及父母而言,GHB仍然是神秘的事物。
在非医学方面,还用作中枢神经系统(CNS)抑制剂的GHB还被作为滥用的药物使用。其具有许多毒品别名,包括液体摇头丸和液体X。在低剂量的情况下,GHB能够引起兴奋状态、对运动和音的乐增加的愉悦感、增加的性欲以及增加的社交能力。在高剂量的情况下,GHB可以引发恶心、头晕、嗜睡、焦虑、视力模糊、呼吸抑制、记忆缺失、意识丧失以及死亡。在摄取大剂量的情况下或者在其与酒精混合的情况下,GHB的效果能够持续1.5至3小时或甚至更久。
通常,娱乐性使用的剂量在500mg至3000mg之间,在浓度为1g/ml的情况下相当于约0.5至3ml的液体(这并不总是这样)。当用作娱乐性药物时,GHB可以以纯液体形式或通常以1g/ml的标准化浓度溶于水中的GHB盐的形式获得,因此这一浓度是用于医疗用途而合法销售的药物Xyrem浓度的两倍。
众所周知,溶于水和/或酒精饮料中的GHB盐是危险的,因为可能不知道GHB的浓度,所以难以精确判断被摄入的GHB的实际剂量。由于用于娱乐性用途而销售的GHB没有经过标准化,因此不可能区分在黑市上购买的GHB溶液的实际浓度。在1ml水中能够溶解超过1g的GHB钠盐,所以GHB钠盐溶液可能实际上比醇GHB液体更浓。还报道了其他盐形式,例如GHB钾盐、GHB钙盐和GHB镁盐,但是到目前为止钠盐是最常用的。
在胃中以及在血液中循环时某些化合物被转化成GHB。GBL或γ-丁内酯是这种前药中的一种。其他前药包括1,4-丁二醇(1,4-B)。可能还存在关于这些前驱体的其他毒性。1,4-B和GBL通常以纯液体的形式获得,尽管用于工业用途如除漆剂或清漆稀释剂时可以将它们与其他更有害的溶剂混合。
GHB可以在秘密实验室中制备,并且据说在美国使用的大部分GHB是在其境内非法制造的。虽然在某些其他国家可以睡眠紊乱的处方药形式获得,但是由于与其用途有关的危险的原因,在1990年FDA禁止了GHB(在美国)。但是,在2002年7月17日,GHB得到批准用于治疗通常与发作性睡眠病有关的猝倒。
四十多年前在法国首次合成了GHB,用作可能的麻醉剂,但是由于其不良副作用的原因,GHB受到美国医学界的抵制。由于许多国家开始认识到这些问题,所以GHB在任何地方的合法使用正在减少。GHB在1987年作为罕用药重新出现,其被研究用于治疗称为发作性睡眠病/猝倒的睡眠紊乱的组合。大约在同一时期,类固醇使用者被告知类固醇可能提高人体的生长激素的产生(在深度睡眠状态)。但是,由于过量用药数量的增加,在1990年11月,其被责令下架。不幸的是,其已经获得了作为娱乐性药物和迷奸药的地位,并且已经危险地变得普遍。作为提高对GHB本身的限制的结果,其“类似物”或能够在人体内转化为GHB的相关化合物已经变得越来越流行。
必须完整地说明GHB的作用。GHB明显具有至少两方面的作用,其刺激最近被表征的且被合适地命名的“GHB受体”以及GABAB受体。如果GHB确实是神经递质,则通常GHB会仅达到在其具有对GABAB受体相对较弱的亲合力的同时足以对GHB受体起作用的浓度。但是,在娱乐性使用的过程中,GHB在大脑中能够达到相对于基态水平非常高的浓度,并且也能够对GABAB受体起作用。GHB对GABAB受体的作用可能是镇静效果的原因。GHB介导的GABAB受体刺激抑制多巴胺释放还引起天然镇静神经甾体(像其他GABAB激动剂,如巴氯芬)的释放。在动物中,GHB的镇静效果可以通过GABAB拮抗剂(阻滞剂)来终止。
GHB受体在GHB诱导的行为影响中的关联性更具争议。似乎难以置信,当GHB受体被密集表达在大脑的包括皮层的许多区域中以及其存在于GHB作用的高亲和力位置时,GHB受体是不重要的。仅存在对GHB受体的有限的研究。但是,存在GHB受体引起谷氨酸盐释放的证据,谷氨酸盐是一种刺激性神经递质。选择性刺激GHB受体而非GABAB受体的药物如反式-4-羟基丁烯酸和4-(对-氯苄基)-GHB在动物中引起抽搐,并且不产生适合GHB的响应。
GHB受体的激活并不单独解释GHB的成瘾性质;使用选择性GABAB拮抗剂和GHB类似物的研究表明GHB受体和GABAB受体对于多巴胺释放和滥用倾向性而言都是重要的,所述GHB类似物为GHB受体的选择性拮抗剂但是不激活GABAB受体。仅激活所述受体中的一种而不是两种的化合物似乎并不诱导急性多巴胺释放或产生对GHB自身而言典型的滥用可能性。
通常高剂量的GHB通过其对GABAB受体作用起镇静作用,而较低剂量通过GHB受体的激活起刺激作用。这可以解释对于GHB中毒而言典型的镇静与刺激性质的矛盾的混合,以及使用GBH作为睡眠药剂的个体所经历的所谓的“回弹”效应,他们在若干小时的GHB诱导的深度睡眠之后突然觉醒。这是由于以下事实的原因:由于新陈代谢在刺激GABAB受体功能的阈值以下的原因而使体内GHB的浓度降低,然后刺激导致觉醒的GHB受体。
在低剂量下GHB对大脑的镇静作用产生兴奋或愉悦的感觉,因为抑制被减弱。当升高剂量时,发生深度昏迷。心率也可能被减弱或减慢。对神经系统的作用可以导致称为肌阵挛的肌肉收缩痉挛,产生癫痫发作状的动作。当与GHB的主要镇静作用结合时,其他作用如意识错乱、记忆缺失、呕吐和不规则呼吸是危险的。与GHB组合的其他药物,特别是乙醇,可以使抑制作用恶化并增加致命结果的可能性。低剂量GHB的期望的作用可能听起来诱人,但是高剂量(错误地)的后果可能是致命的。GHB的剂量反应变化非常急剧,这意味着剂量的微小增加可能导致症状和风险的急剧增加。可变的作用是指一匙的量在某一次可能是最好的,但是在下一次可能变成过量的。知晓GHB与其他化合物混合时发生的结果也是重要的。例如,将GHB与酒精或其他镇静剂混合甚至更可能导致死亡。所述作用持续约四小时并且可能非常突然地消除。
此外,这种药物近来在媒体中被称作约会迷奸药,这与酒精和药物氟硝安定的方式基本相同。GHB本身具有肥皂味或咸味,但是在溶液中稀释后,其几乎是无味的,因此,当与某些饮料混合时,其难以检测。此外,服用之后的GHB的识别是复杂的,因为其在体液中存留的持续时间短。到目前为止,在本领域内已经开发了若干检测方法:体液或毛发以及饮料中γ-羟基丁酸(GHB)的甄别方法。
例如,Kintz等描述了一种通过气象色谱(GC)/质谱(MS)检测毛发中GHB的方法。所述方法要求用二氯甲烷净化毛发样品然后在GHB-d6的存在下在0.01N的NaOH中培养过夜,然后中和并在酸性条件下在乙酸乙酯中萃取(J.Forensic Sci.,2003,Jan;48(1):195-200)。Shen等开发了类似的方法(Fa Yi Xue Za Zhi;2006年2月;22(1):48-51),他们提供了毛发中GHB的GC/MS检测法。这两种方法的缺点不仅在于GC/MS检测方法耗时和复杂,而且还在于为了采集正常生长的对应时期的样品最快在可疑事件发生一个月之后才能采集毛发样品。因此,原位和现场检测是不可能的。
到目前为止,检测血液和尿液中GHB的方法已经几乎局限于色谱方法,如GC-MS、LC/LC-MS、HPLC、PHLC-MS或毛细管电泳(CE)。
例如,等开发了测定血液和尿液样品中GHB的检测方法(Forensic SciInt.2007年8月6日;170(2-3):133-8)。这一方法在若干提取、酸化和离心步骤之后也使用GC/MS分析。Liu等(Fa Yi Xue Za Zhi;2007年4月;23(2):120-2,129)、Paul等(J Anal Toxicol.2007年7-8月;30(6):375-9)、Ferrara等(J Farm Biomed Anal.1993年6月;11(6):483-7)、Villain等(J Chromatogr B Analyt Technol Biomed Life Sci.2003年7月15日;792(1):83-7)、McCusker等(J Anal Toxicol.1999年9月;23(5):301-5)、Elian(Forensic Sci Int.2000年4月10日;109(3):183-7)和Blanchet等(J ChromatogrB Analyt Technol Biomed Life Sci.2002年4月5日;769(2):221-6)提供了类似的方法(与若干调节和/或准备步骤联合的GC/MS)。
气相色谱(GC)法通常也用于检测饮料和/或酒精饮料中的GHB(例如,Liu等,FaYi Xue Za Zhi;2007年4月;23(2):120-2,129)。
GHB含量的其他测试方法包括用于快速甄别酒精饮料和尿液中GHB的颜色测试,其特征在于,GHB在酸性溶液中被转化为GBL,GBL在氢氧化钠的存在下与盐酸羟胺反应形成氧肟酸盐。当氧肟酸盐在酸性条件下与氯化铁反应时形成紫色的络合物(Zhang等;Fa Yi Xue Za Zhi,2006年12月;22(6):424-7)。
另外,通过带有间接UV检测和电子喷雾电离离子捕获质谱确认的毛细管电泳(CE)测定人尿液中GHB的方法在本领域中是已知的(Baldacci等,J Chromatogr A,2003年3月21日;990(1)-2:99-110)。该分析基于液体提取和具有间接UV吸收检测的毛细管区带电泳(CZE)。背景电解液由4mM烟酸(用于间接检测的化合物)、3mM精胺(电渗逆转)和组氨酸(添加至达到6.2的pH值)组成。在具有40cm有效长度的内径50微米的毛细管、作为内标的1-辛基磺酸、214nm处的溶质检测和电导率2.4mS/cm的稀释尿液的情况下,能够检测≥2μg/ml的GHB浓度。
另外,Bravo等开发了基于酶的GHB检测方法,其中以使用来自真氧产碱杆菌(Ralstonia eutropha)的GHB脱氢酶的两步检测方法测定样品中的GHB含量。这一方法由至少两个连续步骤构成,其中第一步在于使样品与GHB-氧化还原酶和被氧化的辅因子接触,导致被氧化的辅因子的还原,其中第二步在于使样品和被还原的辅因子与第二氧化还原酶以及色原或染料接触,导致形成可检测的化合物(Bravo等,J Forensic Sci,2004年3月,Vol.49,No.2:379-87)。这一方法还获得了US 6,703,216B2的专利。
本领域中所有已知的方法都要求大量的设备和/或高技能人员和/或花费长时间实施。需要一种新型、简单并且快速的方法,这种方法也可以由较低水平的实验室人员实施,不需要大量设备来实施,并且还非常快速,这使得所述方法也适用于紧急检测。
因此本发明的目的在于提供一种检测GHB的方法,对于应用到微量滴定板测读器或自动分析仪而言,所述方法能够容易地实施并且不需要大量和昂贵的设备。本发明的另一目的在于提供一种适用于实施本发明方法的药盒和组合物。
所述方法应当还具有易于应用于临床化学分析器的优点,并且显示与标准色谱方法良好的相关性。此外,对于样品中的高水平或低水平而言,GHB测定应当都是可能的。
出乎意料地发现,可以通过用能够将GHB转化为琥珀半醛(SSA)的酶和被氧化的辅因子培养样品使得所述被氧化的辅因子被还原并测定被还原的辅因子的量来检测样品中的GHB含量。然后,被还原的辅因子量的测定能够与样品中的GHB浓度直接关联。
因此,本发明提供一种检测样品中γ-羟基丁酸(GHB)浓度的方法,而所述方法包括以下步骤:
a)培养样品和能够使GHB转化为琥珀半醛(SSA)的酶并还原被氧化的辅因子,
b)测定被还原的辅因子的量,以及
c)使被还原的辅因子的测定量与样品中的GHB浓度相关联。
本发明还提供一种用于针对GHB分析样品的组合物,所述组合物包含以下成分:
a)能够使GHB转化为琥珀半醛(SSA)的酶,以及
b)被氧化的辅因子。
此外,本发明提供一种适用于实施本发明方法的药盒。
本发明还涉及将本发明的方法应用到微量滴定板或自动分析仪(通常在检测临床化学参数如电解质、酶、滥用药物、治疗药物、肿瘤标记物、激素、心脏病标记物等的临床实验室中使用的随机存取分析)的用途。
在本发明的方法中,可以从任何种类的体液中采集样品,也可以从任何种类的饮料,包括含有酒精的饮料中,或者从任何种类的食物提取物中采集样品。在从体液中采集样品的情况下,所述样品优选选自血液、血清、血浆、淋巴液、细胞或组织派生提取物、骨髓液、唾液、眼球液、精液、脑提取物、脊髓液、关节液、胸腺溶液、腹水或纯化的物质和羊水,但不限于此,其中最优选血液、血清或血浆以及随机尿样。可能的血浆样品包含EDTA、柠檬酸盐和肝素血浆。在从饮料或饮料类产品中采集样品的情况下,所述样品优选选自酒精饮料或非酒精饮料,所述酒精饮料有含有酒精的饮料,如啤酒、葡萄酒、烈酒以及含有酒精的混合饮料,如鸡尾酒和“波普甜酒”,所述非酒精饮料例如碳酸化饮料或汽水、果汁,如柠檬水、碳酸化矿泉水、Coca Cola、Ginger Ale、BitterLemon、Tonic Water或Sprite,但不限于此。根据本发明,还可以从饮料状食品产品如酪乳、牛奶、优酪乳、酸奶和乳酪中采集样品。其他可能的样品是茶和含有茶的产品、可可和含有可可的产品以及咖啡、咖啡替代品和其他含有咖啡的产品。
根据本发明,GHB可以源自GHB或其类似物。另外,术语“GHB”包括术语“γ-羟基丁酸”、“γ-羟基丁酸盐”、“4-羟基丁酸”、“羟丁酸盐”、“γ-羟基钠丁酸盐”、“γ-羟基丁酸钠”、“γ-羟基丁酸分解产物”、“γ-羟基丁酸单钠盐”、“4-羟基丁酸(4-hydroxy butanoicacid)”、“4-羟基丁酸盐”、“4-羟基丁酸钠”、“4-羟基丁酸单钠盐”、“4-羟基丁酸钠盐”、“钠γ羟基丁酸盐”等,但不限于此,这些术语可以互换使用。还包括用以下术语和化合物名称表示的物质:“4HB”、“4-03-00-00774(Beilstein Handbook Reference)”、“4-OHB”、“502-85-2”、“52352-27-9”、“591-81-1、“AIDS-156012”、“AIDS 156012”、“BRN 1720582”、“C00989”、“CHEBI:30830”、“DEA No.2100”、“EB 27”、“Gam-OH”、“γ-OH”、“LMFA01050006”、“NSC84223”、“Somsanite”、“WY 3478”、“WY-3478”、“羟基丁酸钠”等,但不限于此。还包括并且能够通过本发明的方法检测的是GHB类似物、盐及其异构体,这些物质在结构上与GHB相关,产生相同或高度相似的药理学作用并且可以用作本发明权利要求1所述的酶的酶作用物(substrate)。所述GBH盐可以是前述的GHB或其类似物以及能够转化为GBH的前驱形式(proform)或前驱体(例如醚或酰胺),GHB的前驱体包括但不限于“γ丁内酯”、“1,4-丁二醇(1,4-butaneol)”以及结构上与GHB相关、产生相同或相似药理学作用(直接地或在代谢之后)并且能够通过本发明提供的方法直接地或在准备步骤之后检测的任何其他化合物。术语“GBL”、“γ丁内酯”、“4-羟基丁酸内酯”、“1,4-丁内酯”、“4-丁酸内酯”等被互换地使用并且表示“二氢-2(3H)-呋喃酮”。术语“前驱体”涉及允许化学地或在体内代谢之后产生GHB的化合物。
根据本发明,可以检测5μM(~0.5mg/L)至100000μM(~10g/L)之间,优选25μM至50000μM之间并最优选50μM至20000μM之间的水平。当检测尿样时,能够检测的GHB浓度优选在0μM至2000μM之间,更优选在20μM至500μM之间,进一步优选在30μM至150μM之间并且当检测治疗浓度时最优选为100μM,并且优选使用100μM的界限区分内源性和/或治疗性水平与具有潜在毒性作用的阳性水平。当检测血浆或血清样品时,能够检测的GHB浓度优选在0μM至1000μM之间,更优选在10μM至500μM之间,进一步优选在20μM至100μM之间,并且优选使用50μM的界限区分内源性和/或治疗性水平与具有潜在毒性作用的阳性水平。具有对于直接分析而言过高浓度的样品可以通过本领域技术人员已知的手段稀释。
在本发明方法中使用的酶可以是能够使GHB转化为琥珀半醛(SSA)并还原被氧化辅因子的任何酶。这种酶的实例是氧化还原酶。根据本发明的术语“氧化还原酶”包括能够催化电子从一个分子(还原剂,也称为氢受体或电子给体)向另一分子(氧化剂,也称为氢供体或电子受体)转移的任何酶。特别优选的氧化还原酶包括脱氢酶和还原酶,其中最优选GHB脱氢酶(GHB-DH)、SSA-还原酶、葡萄糖醛酸还原酶和醛还原酶。在本发明的方法中也可以使用一种或多种之前定义的酶的混合物。
适用于本发明方法的酶可以是源自天然的或重组的,但是优选源自合成制造的基因构造(质粒)。根据本发明,可以使用本领域已知的任何方法制备重组酶。这些方法包括但不限于基于在自然界找到的起始物的化合物的合成以及完全合成体系的制备。
根据本发明,优选两种不同的制备酶的方法。下面,从GHB-DH的分离、克隆和定序开始(制备基因构造),针对酶GHB-DH的制备示例性地描述根据本发明优选的方法:
由真氧产碱杆菌(Ralstonia eutropha)、恶臭假单胞菌(Pseudomonas putida)和/或副百日咳博多氏杆菌(Bordetella parapertussis)进行GHB-DH的分离、克隆和定序。GHB-DH的上游引物或下游引物可以用于扩展基因。然后以重组组氨酸标记融合蛋白的形式制备GHB-DH,并使用组氨酸结合磁珠或超流柱纯化。基因的编码序列可以克隆到pQE-30UA介质中,并且如所指出的那样使重组GHB-DH表达和纯化。
1.在大肠杆菌(E.coli)中表达
使用来自真氧产碱杆菌(Ralstonia eutropha)、恶臭假单胞菌(Pseudomonas putida)和/或副百日咳博多氏杆菌(Bordetella parapertussis)的构建作为起始物质。根据本方法,对于GHB-DH表达的优化而言,存在三种可能的变量:(1)大肠杆菌(E.coli)株(M15、BL-21和XL1-blue MRF’)、(2)诱导体浓度(IPTG)和(3)诱导的培养温度(18℃-37℃)。包括来自真氧产碱杆菌(Ralstonia eutropha)、恶臭假单胞菌(Pseudomonas putida)和/或副百日咳博多氏杆菌(Bordetella parapertussis)的序列的构建pQE-30UA介质例如在具有或不具有乳糖(lac)抑制体辅助质粒pREP4的XL1-blue中被转化。通过SDS-PAGE和使用抗组氨酸标签抗体的蛋白质印迹法核实正确重组蛋白的表达。平行地,进行来自XL1-blue表达的溶解的GHB-DH聚集体的小规模IMAC。然后,质粒将在表达株的选定组中被再转化。
2.在毕赤酵母(Pichia pastoris)中的表达
向生长培养基中提供介质pPICZαA,介质pPICZαA含有标靶重组蛋白质的α-因子分泌信号。可以将感兴趣的基因的多个复制品并入到单个细胞中。可以通过增加Zeocine浓度来选择大量的复制品。将来自原始构建(参见上文)的GHB-DH序列再克隆到没有组氨酸组氨酸标签标记的介质pPICZαA中。在毕赤酵母(P.pastoris)中的转化通过化学方法进行并通过对Zeocine的抗性来选择。
根据本发明的辅因子应当理解为非蛋白质化合物,其与酶紧密结合并需要其催化活性。它们可以分别被称为“辅助”分子和离子,它们协助生化转化。根据本发明的被氧化的辅因子为在本发明的方法中能够被还原的任何辅因子。
可以在本发明的方法中使用的辅因子包括烟酰胺辅因子、黄素辅因子、醌辅因子和酮酸,但不限于此,而烟酰胺辅因子包括烟酰胺腺嘌呤二核苷酸(NAD)、烟酰胺腺嘌呤二核苷酸磷酸(NADP)、烟酰胺1,N6-亚乙烯基腺嘌呤二核苷酸以及烟酰胺1,N6-亚乙烯基腺嘌呤二核苷酸磷酸,但不限于此。黄素辅因子包括含有黄素基团或其活性部分的那些辅因子。根据本发明的黄素辅因子的实例是核黄素、异咯嗪、黄素单核苷酸(FMN)和黄素腺嘌呤二核苷酸(FAD),但不限于此。根据本发明,醌辅因子应当理解为含有醌基团的辅因子。醌辅因子的实例是吡咯并喹啉醌(PQQ),但是不限于此。酮酸的实例包括α酮戊二酸,但不限于此。可以在本发明方法中使用的辅因子的类似物也在本发明的范围内。
特别优选的辅因子是烟酰胺腺嘌呤二核苷酸(NAD+)和烟酰胺腺嘌呤二核苷酸磷酸(NADP)。
根据本发明,NAD+由两个核糖环组成,一个环具有连接在其1’碳原子上的腺嘌呤,而另一个为这一位置上的烟酰胺。这两个糖杂环部分通过5’碳原子由两个磷酸基团的桥连接在一起。在NAD(P)+中,连接在腺嘌呤上的糖环在2’位上有另外的磷酸基团。辅因子烟酰胺腺嘌呤二核苷酸(NAD+)和烟酰胺腺嘌呤二核苷酸磷酸(NADP)的类似物是例如3-乙酰基吡啶-NADH、3-乙酰基吡啶-NADPH、3-吡啶甲醛-NADH、3-吡啶甲醛-NAPDH、硫代烟酰胺-NADH以及硫代烟酰胺-NADPH,但不限于此,并且也可以在本发明的方法中使用。此外,在本发明的方法中可以使用一种或多种如前文分别定义的辅因子及其类似物的混合物。
在本发明方法中,以0.001mM至1000mM,优选0.01mM至100mM,更优选0.1mM至10mM并最优选2mM的浓度使用NAD+。
在本发明的方法中,辅因子NAD(P)+用于接受电子,因此NAD(P)+被还原为NAD(P)H。本发明的方法通常可以以若干不同的方式实施,例如在微量滴定板或自动分析仪中。在本文中,微量滴定(或微量培养板)是具有用作小试管的小管状“孔”的平板。微量培养板的每个孔通常保持几微升至几百微升的液体。微量培养板可以通过机器人操作,也可以通过人工操作。所述机器人可以是将液体样品抽吸出或分配到这些培养板中的液体处理器,或者是将培养板在设备之间传送的“培养板移动器”。在板孔中发生的事件随后可以通过专用微量滴定板测读器检测。优选地,高强度灯发射穿过微量滴定孔的光,并且由微量培养板孔中的反应产物吸收或发出的光通过检测器定量。根据本发明的合适的检测模式为吸收、荧光强度、冷光发光、时间分辨荧光以及荧光偏振,其中特别优选吸收。
根据本发明的自动分析仪为使用专门液体处理和流动技术的自动化分析仪。根据本发明的可能的自动化分析仪的实例是分段流动分析仪、流动注射分析仪或具有透析模块的自动化分析仪。根据本发明的典型的自动分析仪可以是Abbott公司的Aeroset、Alcyon300、C8000/16000和Ci 8200/16200,Beckman公司的Synchron Cx5、Synchron Cx4、Synchron Cx7、Synchron Lx20和UniCell DxC800,Thermo公司的Kone T20、T20 XT、T30和T60,Olympus公司的Reply Analyzer、AU 400、AU 600、AU 800、AU 640和AU 2700/5400,Ortho公司的Vitros Fusion 5.1和Vitros 340,Roche公司的Cobas Mira、Hitachi 704、Hitachi 717、Hitachi 911、Hitachi 902、Hitachi 912、Hitachi 917、CobasIntegra 400/700/800、Modular P 800、Cobas C501、Cobas C111、Modular D2400、CobasBIO、Cobas FARA和Cobas 6000,Siemens(Bayer)公司的Advia 1200、Advia 1650、Advia 1800和Advia 2400,Siemens(Dade)公司的Dimension RxL Dimension Xpand plusDimension Vista 3000T和Dimension Vista 1500,ABX公司的Pentra 60和Pentra 120,ELITech公司的Selectra E/XL,Randox公司的RX Daytona和RX Imola,以及Technicon公司的RA 500、RA 1000和RA XT,但不限于此。根据本发明的自动分析仪也被称为“临床化学分析仪”。
对于样品中GHB的浓度测定,必须测定被还原的辅因子的量。用于测量被还原的辅因子量的可能的检测模式有吸收、荧光强度、冷光发光、时间分辨荧光以及荧光偏振,其中特别优选吸收。优选通过使用分光光度计测量样品的吸收。根据本发明,分光光度计为能够测量强度的光度计(检测光强度的装置),所述强度为颜色的函数,或更具体而言为光波长的函数。根据本发明存在多种可能的分光光度计。用于将其分类的最重要的特性是它们工作的波长、它们使用的检测技术、它们获得光谱的方式以及它们被设计用于测量的强度变化的光源。分光光度计的其他重要的特征包括光谱带宽和线性范围。一般来说,在本发明的方法中可以使用两种不同类型的分光光度计:单束和双束分光光度计。双束分光光度计测量两个不同光路的光强度的比值,而单束分光光度计测量绝对光强度。虽然比值检测更简单并且通常更稳定,但是单束装置也有优点,例如它们能够具有更大的动态范围。
根据本发明方法处理的样品的光密度可以在1nm至1000nm的波长处测量,优选在100nm至650nm的波长处,更优选在280nm至450nm的波长处,并最优选在340nm的波长处。
当实施本发明的方法时,可以独立地选择一个或多个参数,所述参数包括pH、缓冲剂、离子强度、一种或多种盐的存在和浓度、变体和辅因子的存在和浓度、任选试剂、温度、持续时间(培养时间)以及反应体积,但不限于此。可以以任何组合选择所述参数以产生期望的结果。
可以在pH2至pH13,优选在pH8至pH12选择反应混合物的pH,并且最优选为pH10。为了保证pH适合本发明的方法,优选在分析中加入缓冲剂。可能的有用缓冲剂包括乙酸盐、N-二羟基甘氨酸、邻苯二甲酸盐、硼酸盐、三氯乙酸盐、磺基水杨酸盐、磷酸盐、酒石酸盐、柠檬酸盐、琥珀酸盐、马来酸、2,2-二(羟甲基)-2,2’,2”-三乙醇胺、3,3-二甲基戊二酸、3-N-吗啉丙磺酸(MOPS)、丙二酸、1,3-双三(羟甲基)甲基氨基丙烷(Bis-TRIS)、三(羟甲基)氨基甲烷(TRIS)、三(羟甲基)氨基甲烷-马来酸(TRIS-马来酸盐)、三(羟甲基)氨基甲烷-丙二酸(TRIS-丙二酸盐)、3-N-(三羟甲基)甲基氨基-2-羟基丙烷羟基丙烷丙磺酸(TAPSO)、2-(三(羟甲基)甲基氨基)乙磺酸(TES)、1,4-哌嗪二(乙磺酸)(PIPES)、4-吗啉乙磺酸(MES)、N-2-羟乙基哌嗪-N’-2-乙磺酸(HEPES)、硫酸盐、氨基酸(例如甘氨酸)、2-氨基-2-甲基-1,3-丙二醇(AMPD)、咪唑、三乙醇胺、N,N-二(2-羟乙基)-2-氨基乙磺酸(BES)、N-环己基-2-氨基乙磺酸(CHES)、TRIS-HCl以及本领域技术人员已知的其他缓冲剂。最优选的是AMPD缓冲剂。
根据本发明的方法,样品的培养在0℃至100℃的温度下进行,优选18℃至70℃,更优选28℃至50℃,进一步优选35℃至40℃,并且最优选在37℃。
根据本发明的方法,样品的培养时间在1秒至若干天之间,优选在10秒至24小时之间,更优选在30秒至5小时之间,进一步优选在1分钟至60分钟之间,还优选在3分钟至12分钟之间,并且最优选5分钟至8分钟的培养。
在本发明方法中使用的酶浓度可以选自0.1至10000μg/mL,优选1至1000μg/mL,更优选10至100,进一步优选30至80,并且最优选为约60μg/mL。
根据本发明的方法,反应体积可以选自1μl至100μl,优选50μl至10μl,更优选100μl至1μl,并且最优选250μl。
在本发明的范围内,在所述方法中可以包括另外的反应物。这些反应物可以包括能够使GHB前驱体转化从而形成通过本发明方法能够检测的化合物的反应物。为了使酯转化为GHB,可以加入酯酶来使GHB的酯、内酯如GBL等转化为GHB。可以加入酰胺酶从而类似地使GHB的酰胺化形式转化为GHB。
在本发明方法中任选包含的物质可以选自聚合物试剂,如聚乙烯基吡咯烷酮、聚乙烯醇、阿拉伯树胶、明胶、藻胶、角叉藻聚糖、酪蛋白、白蛋白、甲基纤维素、未封端的聚乙二醇、封端的聚乙二醇、多糖(例如蔗糖)以及其他天然和合成的聚合物材料及其组合,以及非聚合物试剂,如单糖(例如葡萄糖)和丙三醇,但不限于此。其它试剂可以含有稳定剂和杀菌剂。
另外,在本发明的方法中可以包括其他任选组分,如蛋白质,例如牛血清白蛋白;糖类,如麦芽糖、葡萄糖、蔗糖、海藻糖、丙三醇等;高分子量化合物,如聚乙二醇和本领域已知的其他化合物;以及金属离子,例如钠、镁、钾、钙和本领域已知的其他金属离子。在使用金属离子作为其他任选组分的情况下,这些金属离子可以用作酶活化剂和/或稳定剂。其他任选组分是螯合化合物,如乙二胺四乙酸(EDTA)。优选地,糖类以0.1至50%(w/v)的浓度使用,优选从0.15至20%(w/v),更优选从1至5%(w/v)。蛋白质优选以具有0.001至50%(w/v)的浓度的溶液使用,优选从0.0015至20%(w/v),更优选从0.01至2%(w/v)。金属离子优选以具有0.001至1000mM金属离子浓度的溶液使用,优选从0.1至100mM,更优选从0.15至80mM。EDTA优选以具有0.001至50mM浓度的溶液使用,优选从0.01至2mM,进一步优选从0.1至1.5mM,并且最优选0.8mM。
另外,可以进行上述步骤以外的步骤。其他步骤例如可以选自以下步骤,但是不限于此:
样品的特殊处理,如样品的脱蛋白质或者根据单个测试的性能进行加热;通过以下方式来使酶纯化的再现性最佳化:通过转化的细菌培养物的更强的溶解,通过用Ni2+-带电柱在本性和变形条件下洗脱被表达的蛋白质和/或通过向洗脱的蛋白质中加入蛋白酶抑制剂和/或防腐剂(例如NaN3)——可以通过SDS-PAGE分析或者通过评价解决内容体形成并使其再折叠的方法来评价被表达的融合蛋白的完整性和蛋白质污染物;通过在没有GHB-DH的存在下用NAD+预先培养样品、通过稀释样品、通过加入包括乳酸脱氢酶的糖解酶的抑制剂(例如草酸)和EDTA或者通过将样品加热钝化来使样品中的NADH的不依赖于GBH-DH的酶形成或非酶形成最小化;加入血红蛋白抑制剂,如亚硝酸盐和亚硝酸钠盐;对于碱性pH,加入其他合适的缓冲剂,如TRIS-HCl、CAPS、CAPSO和/或AMP。根据本发明,上述所有额外的步骤可以在本发明的方法中单独进行或以任何组合的形式进行。
本发明还涉及适用于实施上述方法的药盒。适用于实施本发明方法的药盒至少包括能够使GHB转化为琥珀半醛(SSA)的酶和被氧化的辅因子。所述药盒还可以含有任选的组分,如缓冲剂物质、能够使GHB前驱体转化为可检测形式的其他试剂、提高分析方法性能的物质、蛋白质、蔗糖、高分子量化合物、金属离子以及螯合化合物,但不限于此,这些化合物全部都在上文中进行了详细地描述。
本发明还涉及适用于分析GHB样品的组合物,所述组合物至少包含能够使GHB转化为琥珀半醛(SSA)的酶和被氧化的辅因子。所述组合物中含有的酶优选为氧化还原酶,更优选为脱氢酶,并且最优选为GHB脱氢酶(GHB-DH)。在所述组合物中可以另外包含以上详细描述的其他化合物和试剂。在所述组合物中包含的被氧化的辅因子优选为NAD+。
本发明还涉及上述方法应用于微量滴定板或自动分析仪的用途。

图1示出了样品(例如血清、尿、饮料)中GHB浓度的检测原理。在分析方法中,使用NAD+作为辅因子,氧化还原酶(GHB脱氢酶,GHB-DH;EC 1.1.1.61)将GHB氧化成琥珀半醛(SSA)。在药物(GHB)浓度与酶活性之间存在直接关系。通过加入酶使反应开始,并且通过检测其将NAD+转化成NADH的能力来用分光光度计在340nm处测定GHB-DH活性。
图2示出了关于GHB-DH动力学的微量培养板分析法。在5分钟的时间里以微量培养板分析法检测GHB-DH活性与加入到反应体积中的GHB浓度的函数关系。在分析法中使用的GHB浓度(以mM表示)在右侧的位置标出。
图3示出了适用于自动分析仪(临床化学分析仪)的两种分析方式。两种方式都描述了在Kone T30分析仪上进行的单试剂分析。在将试剂放置在自动分析仪上之前,必须通过混合如图所示的Ra、Rb和Rc或Ra’和Rb’来制备含有缓冲剂、抑制剂、辅因子和酶的试剂混合物。然后,使所述试剂与样品或校准器接触,在自动分析仪中培养所示时间并在培养所示时间之后检测。缩写:OxA(草酸);Cal(校准器)。
图4示出了在Kone T30自动分析仪上进行的双试剂分析方式。在将试剂混合物R1放置在自动分析仪上之前,必须通过混合所示的R1a和Rb来制备含有缓冲剂和抑制剂的试剂R1a和含有辅因子的试剂R1b。然后,在加入含有GHB-DH的试剂R2之前,使试剂R1a:R1b与样品或校准器接触2分钟。在自动分析仪中将最终反应混合物培养并测量另外的6分钟。缩写:OxA(草酸);Cal(校准器);Con L(含有~150μM GHB的尿对照样);Con H(含有~800μM GHB的尿对照样).

图6示出了在临床化学分析仪(Kone T30,Thermo;双试剂分析方式;总培养时间为8分钟)上运行的一系列分析(掺加回收试验(spiking recovery experiments))的结果用于证实所述方法对尿样的准确性。在这一实例中,用0、100、500和1000μM的GHB掺加来自正常供体(n=5)的尿。掺加回收百分比(%)计算为在掺加的样品中观察到的(O)GHB浓度对预期的(E)GHB浓度的%值(O/E)。
图7示出了在临床化学分析仪(Kone T30,Thermo;双试剂分析方式;总培养时间为8分钟)上运行的一系列分析(掺加回收试验)的结果用于证实所述方法对血清样的准确性。在这一实例中,用0、150和800μM的GHB掺加来自正常供体(n=5)的血清。掺加回收百分比(%)计算为在掺加的样品中观察到的(O)GHB浓度对预期的(E)GHB浓度的%值(O/E)。

图9示出了在临床化学分析仪(Kone T30,Thermo;双试剂分析方式;总培养时间为8分钟)上运行的一系列分析的结果。通过本发明的酶方法测定来自中毒患者的GHB阳性尿样(n=9)的浓度,并将其与图8中列举的IC参照方法关联。用0.9%NaCl溶液将具有2000μM(~200mg/L)的初始浓度的尿样稀释5至100倍,并重新分析。检测的GHB浓度以mg/L给出。

图11示出了来自正常供体(n=10)的尿样中GHB内源性水平与来自中毒患者(n=9)的尿样中GHB浓度的对比。所有样品都用本发明的酶方法检测(Kone T30,Thermo;双试剂分析方式;总培养时间为8分钟)。检测的GHB浓度以μM给出。用0.9%NaCl溶液将具有2000μM的初始浓度的患者样品稀释5至100倍,并重新分析。
Claims (20)
1.一种测定样品中γ-羟基丁酸(GHB)浓度的方法,其中所述方法包括以下步骤:
a)培养所述样品和酶,所述酶能够通过还原被氧化的辅因子使GHB转化为琥珀半醛(SSA),其中通过加入含有草酸和EDTA的缓冲剂来稀释所述样品
b)检测被还原的辅因子的量
以及
c)使被还原的辅因子的检测量与样品中GHB浓度关联。
2.如权利要求1所述的方法,其中步骤a)的酶为氧化还原酶。
3.如权利要求2所述的方法,其中所述酶为GHB脱氢酶、SSA还原酶、葡萄糖醛酸还原酶和/或醛还原酶。
4.如权利要求1至3中任一项所述的方法,其中所述酶源自合成制造的基因构建。
5.如权利要求1至4中任一项所述的方法,其中所述被氧化的辅因子为NAD+。
6.如权利要求1至5中任一项所述的方法,其中在100nm和650nm之间的波长下进行所述被还原的辅因子的检测。
7.如权利要求6所述的方法,其中在340nm的波长下进行所述被还原的辅因子的检测。
8.如权利要求1至7中任一项所述的方法,其中在28至50℃的温度下进行所述样品的培养。
9.如权利要求8所述的方法,其中在37℃的温度下进行所述样品的培养。
10.如权利要求1至9中任一项所述的方法,其中培养时间在1至60分钟之间。
11.如权利要求10所述的方法,其中所述培养时间为8分钟。
12.如权利要求1至11中任一项所述的方法,其中反应体积的pH值在8至12之间。
13.如权利要求12所述的方法,其中所述pH值为10。
14.一种用于分析GHB样品的组合物,包含以下组分:
a)能够将GHB转化为琥珀半醛(SSA)的酶,
b)被氧化的辅因子
以及
c)含有草酸和EDTA的缓冲剂,所述草酸具有在1至100mM之间的浓度范围,其中所述浓度优选为10mM,所述EDTA具有在0.1至1.5mM之间的浓度范围,其中所述浓度优选为0.8mM。
15.如权利要求14所述的组合物,其中所述酶为脱氢酶。
16.如权利要求14或15中任一项所述的组合物,其中所述被氧化的辅因子为NAD+,其浓度范围在0.1至10mM之间,其中所述浓度优选为2mM。
17.一种适用于实施权利要求1至13中任一项所述方法的药盒。
18.如权利要求17所述的药盒,所述药盒含有权利要求14至16中任一项所述的组合物。
19.如权利要求1至13中任一项所述的方法在微量滴定板中应用的用途。
20.如权利要求1至13中任一项所述的方法在自动分析仪中应用的用途。
