Abstract

Description
本发明涉及与细胞因子与干扰素组合用于制备设计用于病毒疾病治疗的组合物的用途。
现有技术
干扰素(IFN)系统是哺乳动物中针对病毒疾病的防御的第一线。I型干扰素(其包括一些IFN-α和IFN-β亚型)是具有抗病毒活性的分子,其响应于病毒感染在哺乳动物细胞中诱导。IFN-α的作用由与表面细胞受体多亚基的相互作用介导,所述多亚基由两种受体亚基,INF-α受体1(IFNAR1)和IFNAR2组成。仅鉴定了一种IFNAR1链形式;识别了三种IFNAR2亚基的变体:一种是全长的,IFRNAR2c,以及两种截短的同种型,IFNAR2b和IFNAR2a。IFNAR2c变体涉及与配体的结合和信号的转导,而两种截短的形式,不具有细胞内结构域的IFNAR2b和IFNAR2a通过与IFNAR2c竞争结合IFN-α来抑制INF-α信号。
当IFN-α与受体结合时,IFN-α信号级联被启动。INF-α与受体的结合引起与IFNAR相关的酪氨酸激酶的活化(Janus激酶1(Jak1)和酪氨酸激酶2(Tyk2)),其使两种亚基IFNAR1和IFNAR2磷酸化。当被Tyk2或Jak1磷酸化时,磷酸化的IFNAR1提供了信号转导的结合点以及转录2的激活物(STAT2),所述转录2的激活物含有与Src的同源-2结构域。其他的STAT,包括STAT1、STAT3和STAT5,被连续地征募到受体来磷酸化和活化。活化的单体STAT1和STAT2然后被再次释放到细胞质中,在此它们形成杂二聚体并与干扰素调节因子9/蛋白P48结合,来形成称为干扰素-刺激的基因因子3(ISGF3)的活性转录因子复合物。该复合物被转位到核中并结合IFN刺激应答元件(ISRE)以启动目标基因的转录,包括某些抗病毒和免疫调节蛋白质。IFN-α还诱导其他STAT复合物的形成,包括STAT1/STAT1、STAT1/STAT3和STAT3/STAT3,其与敏感基因的启动子区域中活化的γ序列结合。在原代人类肝细胞中,IFN-α激活STAT1、STAT2、STAT3和STAT5,随后诱导广泛的抗病毒和促凋亡基因,这些基因可能促进IFN-α在人类肝脏中的抗肿瘤和抗病毒活性。
与IFN-α相反,在激活STAT1、STAT2和STAT3的II型IFN(IFN-γ)的情况下,与受体的结合导致STAT1被Jak1和Jak2专门地磷酸化,这跟随着STAT1的同二聚化和同二聚体的核转位。
病毒感染在全世界是很大的健康难题。在引起慢性感染的病毒中,导致B型肝炎(HBV)和C型肝炎(HCV)的病毒作为慢性病毒肝炎和肝硬化的致病因素是重要的;这些失调影响了全世界超过5亿人(约3亿被HBV感染,2亿被HCV感染)。HBV主要在垂直传播和免疫抑制个体中引起慢性感染。另一方面,HCV感染由于它在大多数情况下发展为慢性的倾向是值得注意的,这表明这种病毒具有发展的特别有效的机制来逃避干扰素系统。患有慢性HCV感染的患者,以及患有慢性B型肝炎的患者,不能响应干扰素治疗。在慢性B型肝炎中,在低于40%的病例中发生持续的抗病毒响应[1]。在慢性C型肝炎中,虽然被基因型HCV 2或3感染的大多数患者在使用聚乙二醇化的INF-α和病毒唑的组合治疗24或28周之后展现了持续的病毒学响应(SVR)[2],被基因型1感染的那些患者的仅50%使用这种治疗方式实现了SVR[2]。由于在西方世界和亚洲被HCV感染的患者的超过80%相应于基因型1,急迫地需要更有效的手段来提高IFN-α的抗病毒效果。在HCV和其他慢性病毒疾病中观察到的IFN-α抗性的根本机制仍然了解很少,非常需要找到在这些疾病中克服了对IFN-α治疗的抗性的治疗策略。
受病毒感染的细胞对干扰素-α的反应取决于几个决定因素,包括与病毒相关的那些以及与宿主相关的那特定几个。以及显示了各种HCV基因产物调节宿主对IFN治疗的反应并影响病毒疾病的严重度,特别是在肝炎疾病的情况中。注意到,HCV的非结构蛋白(NS5A)和结构蛋白(E2)与PKR相互作用,PKR是涉及响应于IFN的抗病毒状况发展的关键分子[3,4]。这可能阻断PKR,引起在HCV感染的细胞中IFN活性的抑制。另一方面,一些研究显示,IFN-α诱导的STAT1信号在具有HCV的转基因小鼠中和在患有慢性HCV的患者的肝活检中都受到影响[5,6]。
在HCV感染的肝脏样品中以及在携带基因组HCV复制子(全长)的肝细胞中观察到,在IFNAR2和STAT3mRNA数量上的显著降低。相关的发现是,在含有全长HCV复制子的肝细胞中IFN-α对STAT1、STAT2和STAT3的激活被阻断,这表明在感染的细胞中HCV复制可以阻断IFN-α信号转导。还令人感兴趣的是,当与前炎症性分子IFN-γ孵育时,这些细胞中的STAT1活化不受影响,这表明HCV产生的STAT1活化阻断特异于I型IFN信号转导级联,并且不影响II型IFN信号转导途径。
存在着激活Jak-STAT信号转导途径的其他细胞因子,特别是IL-6家族的成员,其包括IL-6、IL-11、白血病抑制因子(LIF)、制瘤素(OSM)、心肌营养蛋白-1(CT-1)、纤毛神经营养因子(CNTG)和心肌营养蛋白样细胞因子(CLC)[7]。这些细胞因子与细胞质膜的受体复合物结合,所述受体复合物包含共同的gpl30传导信号受体链[7]。信号的传导需要Jak酪氨酸激酶家族成员的活化,引起转录因子STAT1和STAT3的活化。这些细胞因子通过共同的gpl30受体亚基潜在地活化STAT3,并且以教少的程度活化STAT1。然而,虽然显示了IL-6诱导某些抗病毒效果[8],这种细胞因子的抗病毒活性比干扰素-α的低得多。
本发明涉及I L-6家族白介素,优选的心肌营养蛋白-1(CT-1)或制瘤素M(OSM)的用途:
(1)来增强干扰素-α(IFN-α)的抗病毒活性;
(2)来克服在不响应IFN-α治疗的、患有慢性病毒感染的患者中观察到的对干扰素的抗性(本身或与其他抗病毒成分相关);
(3)来实现IL-6家族白介素,优选的CT-1或OSM加上IFN-α的组合治疗,作为用于任何类型的病毒感染、优选的C型肝炎病毒(HCV)感染的改进的抗病毒治疗,其中优选的组合CT-1-IFN-α或OSM-IFN-α被证明在抑制HCV复制中特别有效。
本发明实现了以下目标:
(1)显示了与单独的细胞因子(IFN或IL-6家族细胞因子)所诱导的相比,IFN-α与IL-6家族白介素,优选的与CT-1和OSM的组合产生了更强力的抗病毒效果;以及
(2)显示了与IL-6家族细胞因子,特别是CT-1或制瘤素M相关联的IFN-α能够克服IFN-α信号转导级联的阻断(并且因而,克服了当病毒、优选的HCV在受感染细胞中复制时产生的IFN-α效果的弱化)。
发明的描述
首先,本发明涉及至少一种IL-6家族细胞因子-gpl30家族-或编码它的DNA序列在制备药物组合物中的用途,所述药物组合物用于在病毒疾病的治疗中与至少一种IFN-α或编码它的DNA序列组合施用,所述细胞因子选自心肌营养蛋白-1、IL-11、白血病抑制因子、制瘤素M、纤毛神经营养因子、心肌营养蛋白样细胞因子、以及其组合;以及,再更优选的,所述细胞因子是心肌营养蛋白-1或制瘤素M。
如本发明中使用的,“IL-6家族”细胞因子,例如,CT-1,涉及:
-所述细胞因子的完整的天然形式;
-所述细胞因子的任何活性片段,其是维持了本发明中要求的完整细胞因子的生理学效果的所述细胞因子的任何部分多肽序列;以及
-所述细胞因子的任何多肽衍生物,其是与所述天然细胞因子具有大于80%的同源性、并且维持了本发明中要求的完整细胞因子的生理学效果的任何多肽序列。
IL-6家族细胞因子(无论是完整的、活性片段还是多肽衍生物)可以来自天然形式以及任何形式的重组细胞因子,从编码完整细胞因子、活性片段或多肽衍生物的任何多核苷酸形式开始。
此外,在IL-6家族的蛋白质中,被认为完整的或作为活性片段,或在重组细胞因子之内,一个或几个氨基酸可以通过任何提及的方法被删除、替换或添加到蛋白质,只要维持了它在本发明上预期的活性。
另一方面,根据本发明,本发明的IFN-α是任何类型的IFN-α。在特定的实施方式中,所述IFN-α选自IFN-α-2a、IFN-α-2b、IFN-α-5、共有序列干扰素、纯化的IFN-α、聚乙二醇化的IFN-α以及其组合。在另一个特定的实施方式中,IFN-α选自聚乙二醇化的IFN-α-b、聚乙二醇化的IFN-α-2a、聚乙二醇化的IFN-α-5以及其组合。
IFN-α和IL-6家族细胞因子的组合使用被设计用于优选的病毒疾病的治疗。作为可以通过干扰素和IL-6家族细胞因子的组合使用来治疗的病毒疾病的实例,在其余的之中可以提及以下的:由脑心肌炎病毒、肝炎B和C、HIV引起的疾病、皮肤病毒感染(小鸡痘、疱疹、measles)、呼吸病毒感染、中枢神经系统的病毒感染、肝病毒感染、唾液腺的病毒感染、传染性单核细胞增多症和生殖器疣。
优选的,所述病毒疾病是C型肝炎。
此外,根据本发明,IL-6家族细胞因子-或细胞因子-和IFN-α可以单独地施用,存在于不同的药物组合物中;或它们可以共同地施用,存在于同一药物组合物中。
因而,本发明的其他目标是一种药物组合物,其包含药学上可接受量的至少一种IL-6家族细胞因子-gpl30家族-,或编码它的DNA序列,以及药学上可接受量的至少一种IFN-α,或编码它的DNA序列。
在包含至少一种IFN-α或编码它的DNA序列,以及至少一种IL-6家族细胞因子或编码它的DNA序列的所述药物组合物中,IL-6家族细胞因子优选的选自IL-6、IL-11、白血病抑制因子、制瘤素M、心肌营养蛋白-1、纤毛神经营养因子、心肌营养蛋白样细胞因子以及其组合;再更优选的,所述IL-6家族细胞因子是心肌营养蛋白-1或制瘤素M。
在本发明的药物组合物中,所述IFN-α是任何类型的IFN-α。在优选的实施方式中,所述IFN-α选自IFN-α-2a、IFN-α-2b、IFN-α-5、共有序列干扰素、纯化的IFN-α、聚乙二醇化的IFN-α以及其组合。在另一个其他优选的实施方式中,所述IFN-α选自聚乙二醇化的IFN-α-b、聚乙二醇化的IFN-α-2a、聚乙二醇化的IFN-α-5以及其组合。
在特定的实施方式中,编码IL-6家族细胞因子(无论它是完整的、活性片段还是多肽衍生物)或IFN-α的DNA序列被掺入到表达载体中,例如,质粒或病毒载体,其优选的与调节所述细胞因子或IFN-α表达的控制序列可操作地连接。使用所述DNA序列构建所述表达载体可以通过在操作手册中包含的常规重组技术来进行,所述操作手册例如“Molecular Clong;A Laboratory Manual”,by J.Sambrook,D.W.RusselEds.2001,3rd ed.Cold Spring Harbor,New York。这些药物组合物实施方式是利用基因转移的治疗(基因治疗)感兴趣的。
本发明的药物组合物可以进一步包含至少一种赋形剂,其与IL-6家族细胞因子或编码它的DNA序列是药学上相容的,以及与IFN-α或编码它的DNA序列是药学上相容的。
此外,在所述药物组合物中,IL-6家族细胞因子或编码它的DNA序列以及IFN-α或编码它的DNA序列可以在各自的载体试剂中携带。
本发明的药物组合物的各种实例包括但不限于任何固体组合物(例如,片剂、胶囊、颗粒剂,等等)或液体组合物(例如,溶液、悬浮液、乳剂,等等)用于通过任何适合的施用途径,例如,口服、鼻部、胃肠外、体表、透皮、直肠的途径等等。

在特定的实施方式中,所述药物组合物被设计用于胃肠外施用,优选的,皮下的、静脉内的、肌肉内的或腹膜内的施用。
在本发明的特定实施方式中,所述IFN-α是聚乙二醇化的形式。聚乙二醇化形式的组合物制备的一些实例可以在US5,762,923、US5,766,582中找到。还可能商业上地购买这些聚乙二醇化形式的某一些,例如,Schering Corporation(Kenilworth,N.J.,U.S.A.)的PEG-Intron(聚乙二醇化的IFN-α-2b)和Hoffmann La Roche(Nutley,N.J.U.S.A.)的PEGASYS(IFN-α-2a)。
对于治疗中的应用,IL-6家族细胞因子和IFN-α优选的都应处于药学上可接受的形式或基本上纯的形式,即,它们应当具有药学上可接受的纯度水平,排出药学上可接受的赋形剂并且不包括被认为在正常剂量水平有毒的材料。IL-6家族细胞因子和IFN-α的纯度水平优选的高于50%,更优选的,高于70%,更优选的,高于90%。在优选的实施方式中,它们高于90%。
一般地,要施用的IL-6家族细胞因子和IFN-α的药学有效数量应当取决于,在其他因素之中,要治疗的个体、该个体患上的疾病的严重度、选择的施用形式,等等。为此,本发明中提及的剂量应当根据对本领域技术人员的指导单独地考虑,后者应当在所述变量的基础上调整剂量。然而,IL-6家族细胞因子和IFN-α可以每天施用一次或多次,例如,每天1、2、3或4次。
作为示范性的实例,并且这不限制保护的范围,在组合心肌营养蛋白-1和IFN-α-2a(或2b)的特定实施方式中,心肌营养蛋白-1的一般总每日数量应当在1μg/kg和10mg/kg体重之间;IFN-α-2a的一般总每日数量在1.5和10MIU每日之间,或每周40到300微克聚乙二醇化的IFN-α之间。通常,在治疗的第一周期间剂量水平将会更高,在随后的阶段剂量降低。类似地,施用方案可以是每天、每周三次或每周地。另一方面,心肌营养蛋白-1和IFN-α可以根据不同的施用方案来施用(例如,不同的施用途径或不同的频率)。
本发明的其他目标是用于病毒疾病的治疗的药盒,其至少包括:
-第一成分,其包含至少一种IL-6家族细胞因子-gpl30家族-(上文定义的完整的、活性片段或多肽衍生物)或编码所述细胞因子的DNA序列;和
-第二成分,其包含至少一种IFN-α或编码所述IFN-α的DNA序列。
根据本发明的药盒优选的包含选自IL-6、IL-11、白血病抑制因子、制瘤素M、心肌营养蛋白-1、纤毛神经营养因子、心肌营养蛋白样细胞因子和其组合的IL-6家族细胞因子,以及更优选的,IL-6家族细胞因子是心肌营养蛋白-1或制瘤素M。
根据本发明的药盒包含任何类型的IFN-α,优选的选自IFN-α-2a、IFN-α-2b、IFN-α-5、共有序列干扰素、纯化的IFN-α、聚乙二醇化的IFN-α以及其组合的一种。在另一个其他优选的实施方式中,IFN-α选自聚乙二醇化的IFN-α-2b、聚乙二醇化的IFN-α-2a、聚乙二醇化的IFN-α-5以及其组合。
在特定的实施方式中,在所述药盒中编码IL-6家族细胞因子(无论是完整的、活性片段还是多肽衍生物)或IFN-α的DNA序列被掺入到表达载体中。
在本发明的药盒中,所述第一成分和第二成分可以另外包含至少一种药学上可接受的赋形剂,其与IL-6家族细胞因子-或编码它的DNA序列-以及与IFN-α-或编码它的DNA序列是相容的。
根据本发明,以上定义的药盒可以包含在独立的药物组合物中的第一和第二成分;或所述第一和第二成分可以在同一药物组合物中在药盒中存在。
这种药盒还可以包含第三成分,其包含一种或多种赋形剂,所述赋形剂与IL-6家族细胞因子-或编码它的DNA序列-以及与IFN-α-或编码它的DNA序列是药学上相容的。

本发明的其他目的是治疗病毒疾病的方法,其包括以组合的方式施用治疗有效量的至少一种IL-6家族-gpl30家族-细胞因子(无论是完整的、活性片段还是多肽衍生物,它们在先前已经被定义)或编码它的DNA序列,以及治疗有效量的至少一种IFN-α或编码它的DNA序列。
在特定的实施方式中,该方法的编码IL-6家族细胞因子或IFN-α的DNA序列被掺入到表达载体中。
在上文定义的方法中,病毒疾病可以由脑心肌炎病毒、肝炎B和C、HIV、皮肤病毒感染(小鸡痘、带状疱疹、麻疹)、呼吸病毒感染、中枢神经系统的病毒感染、肝病毒感染、唾液腺的病毒感染、传染性单核细胞增多症和生殖器疣产生。
根据本发明的方法的优选的实施方式,所述病毒疾病是C型肝炎。
根据本发明的方法,IL-6家族细胞因子优选的选自IL-6、IL-11、白血病抑制因子、制瘤素M、心肌营养蛋白-1、纤毛神经营养因子、心肌营养蛋白样细胞因子和其组合;再更优选的,IL-6家族细胞因子是心肌营养蛋白-1或制瘤素M。
在根据本发明定义的方法中,所述IFN-α是任何类型的IFN-α。在优选的实施方式中,它选自IFN-α-2a、IFN-α-2b、IFN-α-5、共有序列干扰素、纯化的IFN-α、聚乙二醇化的IFN-α以及其组合;在另一个其他优选的实施方式中,IFN-α选自聚乙二醇化的IFN-α-2b、聚乙二醇化的IFN-α-2a、聚乙二醇化的IFN-α-5以及其组合。
此外,根据本发明的方法,后者可以包括白介素家族细胞因子、优选的心肌营养蛋白-1与IFN-α的组合的以及同时的施用。
在这种方法中,IL-6家族细胞因子(无论是完整的、活性片段还是多肽衍生物)和IFN-α可以存在于向患者施用的同一药物组合物中;患者IL-6家族细胞因子和IFN-α可以在独立的药物组合物中施用。
本发明显示了,当包含完整HCV复制子的肝细胞用IFN-α和IL-6家族细胞因子、特别是心肌营养蛋白-1或制瘤素M刺激时:
(1)当细胞与单独的IFN-α或与单独的IL-6家族细胞因子(例如,CT-1或OSM)孵育时发生的HCV对STAT3磷酸化的抑制效果被克服。
(2)存在着同与单独的细胞因子孵育细胞时相比干扰素敏感性基因(ISG),例如2’-5’-寡聚腺苷酸合成酶(2’-5’OAS)的更高的诱导,以及更高水平的STAT1和STAT3。
(3)与单独使用细胞因子时相比,病毒的复制被更有效地抑制。
(4)IFN-α和CT-1之间,或IFN-α和OSM之间的相互作用是强烈、sinergism的。
附图的简要说明
附图1显示了STAT1、STAT2和STAT3的磷酸化的分析。Huh7细胞(Huh7)和含有完整基因组HCV复制子(Core-3’)的Huh7细胞用50IU/ml的IFN-α-2(IFN-α)或用20ng/ml的心肌营养蛋白-1(CT-1)、或用IFN-α-2和CT-1的组合处理15、60和120分钟,通过Wetstern印迹的方式分析细胞提取物中存在的磷酸化的STAT1、STAT2和STAT3的数量。
附图2显示了使用单独(-CT)的或与20ng/ml CT-1组合的(+CT)5或50IU/ml的IFN-α-2(IFN5,IFN50);单独(-OSM)的或与20ng/mlOSM组合的(+OSM)5或50IU/ml的IFN-α-2处理3天、含有完整HCV复制子的Huh7细胞中存在的STAT1、STAT3和2’-5’OAS mRNA的定量实时RT-PCR分析。对照:未处理的细胞。
附图3显示了用5或50IU/ml的IFN-α-2或IFN-α-5,以及20ng/ml的CT-1或OSM或IL-6处理3天、含有完整HCV复制子的Huh7细胞中存在的HCV RNA的定量实时RT-PCR分析。
附图4显示了通过用细胞因子处理3天、含有完整HCV复制子的Huh7细胞中存在的HCV RNA的实时的RT-PCR,组合IFN-α-2或IFN-α-5(5U/ml)加上CT-1或OSM或IL-6(20ng/ml)的抗病毒比较效果。
附图5显示了对抗脑心肌炎病毒感染保护的Huh7细胞的百分比。Huh7细胞用5ng/ml或50ng/ml的CT-1以及不同数量的IFN-α-2预处理24小时,并用105PFU的脑心肌炎病毒(EMCV)感染,在24小时后,使用结晶紫染料染色细胞来测量存活细胞的数量。
发明实施方式
实验1
IFN-α-2和CT-1的组合对维持了HCV复制子的细胞中的信号转导级联的影响的研究(附图1)。
在未转染的Huh7细胞中(肝细胞瘤细胞系),添加50IU/ml的IFN-α-2(IFN-α-2)诱导STAT1、STAT2和STAT3的磷酸化,在1小时和2小时STAT1和STAT2的最大活化,在1小时STAT3最大活化(参见附图1A)。然而,在用全长HCV复制子转染的Huh7细胞中,与IFN-α-2孵育后观察到STAT1、STAT2和STAT3磷酸化的显著抑制。因而,存在着激活的STAT3和STAT1的完全缺乏以及STAT2活化的显著抑制(参见附图1A)。
另一方面,在未转染的Huh7细胞中,发现添加20ng/ml CT-1引起STAT3和STAT1两者的活化(在STAT3的情况下更强烈),在15分钟和1小时有最大值,在2小时充分降低(附图1B)。如预期的,CT-1不诱导任何STAT2活化。在用完整HCV复制子转染的Huh7细胞中,CT-1对STAT3的活化被相当大地降低,而STAT1的磷酸化仅轻微受影响(附图1B)。
当用50IU/ml的IFN-α-2和20ng/ml的CT-1的混合物孵育未转染的Huh7细胞时,与用每种单独的细胞因子孵育细胞时相比,检测到更强烈和持久的STAT3与STAT1的磷酸化。STAT2的活化类似于使用单独的IFN-α-2所发现的。明显的是,当用IFN-α-2(50IU/ml)加CT-1(20ng/ml)混合物孵育完整HCV复制子转染的Huh7细胞时,没有任何损害地发生STAT3磷酸化,与使用单独的IFN-α-2孵育未转染的细胞时相比,STAT3的活化不仅更强烈,而且更持久(附图1C)。这个事实是值得注意的,因为,如已经提及的(如附图1A和1B所示),在Huh7细胞中HCV复制严重地降低通过单独的IFN-α-1和CT-1的STAT3活化,因而令人惊讶的是通过共同使用两种细胞因子孵育细胞这种阻断消失了;这为病毒治疗中有前景的测量打开了新的期望。还必需注意的是,当组合IFN-α-2和CT-1来治疗具有HCV复制的细胞时,不仅产生了强力的、持续的STAT3活化,STAT1活化类似于用单独的CT-1孵育未转染细胞时所发现的,并且比用单独的IFN-α-2孵育未转染细胞时的更强烈(比较附图1A和1B)。重要的是注意到,在具有持续的HCV复制的细胞中IFN-α-2不能激活STAT1,而CT-1加IFN-α-2的组合能够在具有HCV复制的细胞中非常有效地激活这种重要的抗病毒因子。这清楚地显示了,组合IFN-α-2和CT-1引起了抗病毒活性的改进、容许STAT2磷酸化,虽然在水平上明显低于使用没有HCV复制的细胞时的(参见附图1C)。
总之,当以独立的方式用IFN-α-2处理具有持续HCV复制的细胞时,没有STAT1和STAT3活化,仅观察到低水平的STAT2。细胞抗病毒状态的两种重要诱导物活化的STAT1和STAT3的缺乏可能阻止STAT1-STAT2异二聚体、STAT1-STAT3异二聚体、以及STAT1和STAT3同二聚体的形成,因而破坏细胞的抗病毒防御。与IFN-α-2组合使用CT-1容许形成高水平的活化的STAT1和STAT3,因而恢复了细胞的病毒抵抗机制。
在附图1所示的实验中,人们可以看到HCV感染不仅引起缺血性STAT1活化,还引起STAT3和STAT2蛋白水平的降低。附图1中呈现的研究显示了使用IFN-α-2或CT-1、或两者的组合孵育的短期效果。以下描述的实验显示了,孵育72小时,人们可以观察到使用CT-1与IFN-α-2、或OSM与IFN-α-2的组合处理产生STAT3的提高的表达,因而对抗了感染的细胞中HCV的影响(参见附图2)。
实验方法1
携带全长HCV复制子的Huh7细胞系的建立。如已经描述的[9],建立了表达全长HCV复制子的Huh7细胞。总言之,pI389/Core3’/5.1使用ScaI(New England Biolabs,USA)线性化,用作模板用于使用T7RAN聚合酶(Promega,USA)进行RNA合成。使用20μg合成的RNA来电泳107个Huh7细胞,24小时后,添加500μg/ml G418(Gibco,USA)。一周两次,用G418替换补充的培养基,转染后4周,收集对G418抗性的混合的克隆,用于随后的分析。
Western印迹分析:表达全长HCV复制子或不表达的Huh7细胞以200,000个/孔播种到6孔平板中具有10%FCS(Gibco)的D-MEN(Gibco)中。添加50IU/ml的IFN-α-2(Intron A,Schering-Plough)或20ng/ml的CT-1(R&D Systems,UK)、或IFN-α-2(50IU/ml)加CT-1(20ng/ml)的组合持续不同的时间:15分钟、1小时和2小时。随后在裂解缓冲液(60mM Tris-HCl pH6.8,2%SDS,2.5%甘油,0.7M2-巯基乙醇和0.02%溴酚蓝)中裂解Huh7细胞。样品在还原条件下在7.5%的SDS-聚丙烯酰胺凝结(Bio-Rad Laboratories,CA)中分辨。在电泳之后,将它们转移到硝酸纤维素膜(Bio-Rad Laboratories)上,用Ponceau红溶液(Sigma-Aldrich,Germany)染色,来验证有等量的蛋白加载。在具有5%脱水牛奶的TBS-T(50mM Tris-HCl (pH 7.6),200mM NaCl和0.1%Tween-20)中孵育所述膜。通过在TBS-T中用特异性初级抗体孵育1小时来检测蛋白。随后在TBS-T中洗涤膜,添加结合了过氧化物酶的二级抗体1小时。膜在TBS-T中经历广泛的洗涤,根据厂家的指导使用“Western Lighting Chemiluminescence ReagentPlus”化学荧光检测系统(Perkin Elmer,USA)检视特异性蛋白条带。随后,膜进行自动放射成像,通过Molecular Analyst/PC程序(Bio-RadLaboratories)的方式进行的光密度分析方式来对条带定量。
抗体:抗-磷酸-STAT1tyr701和抗-磷酸-STAT3tyr705抗体和与HRP结合的抗兔IgG抗体购自Cell Signaling Technology(USA)。抗-STAT3、抗-磷酸-STAT1ser727、抗STAT2和抗-磷酸-STAT2tyr689抗体从UpstateBiotechnology(USA)获得。抗STAT1抗体来自Santa Cruz Biotechnology(Santa Cruz,CA)。抗肌动蛋白抗体来自Sigma-Aldrich(Germany)。
为了确定IL-6家族细胞因子与IFN-α的组合是否引起细胞中更强的抗病毒状态,我们进行了其他实验,设计以:a)评估IFN-α加IL-6家族细胞因子的组合治疗是否比任何细胞因子本身更强烈地提高干扰素敏感基因的表达;b)确定IFN-α加IL-6家族细胞因子的组合治疗是否比每种单独的细胞因子更强力地抑制HCV复制;c)评估IFN-α-2加CT-1的组合治疗是否在使细胞防御HCV非相关病毒的细胞病变效果中更加有效,以及;d)确定IFN-α和IL-6家族细胞因子之间相互作用的种类。
实验2.IFN-α-2加CT-1、或IFN-α-2加OSM的组合治疗对于干扰素敏感性基因(1SG)的诱导效果的评估(附图2)
在与IFN-α-2(5或50IU/ml)或CT-1(20ng/ml)、或OSM(20ng/ml)、或组合的IFN-α-2+OSM孵育72小时后在携带HCV复制子的Huh7细胞上研究ISG 2’-5’OAS、STAT1和STAT3的表达(参见附图2A-2F)。我们观察到,虽然CT-1或OSM本身不能提高或微弱地提高这些ISG的表达,添加CT-1或OSM到低剂量或高剂量的IFN-α-2中引起ISG表达的显著提高,其表明CT-1或OSM可以显著地增强IFN-α-2在支持病毒复制的细胞中提高性调节抗病毒基因的能力。虽然在此分析的三种ISG具有显著的抗病毒效果,通过组合CT-1或OSM与IFN-α-2在STAT3表达方面的提高是特别相关的,因为这种因子不仅具有抗病毒性质,而且展现了强力的细胞保护和抗验证性质。
实验方法2
ISG的mRNA表达的实时RT-PCR分析。表达全长HCV复制子的Huh7细胞以100,000个/孔播种到6孔平板中具有10%FCS(Gibco)的D-MEN(Gibco)中。添加50或5IU/ml的IFN-α-2本身、或与20ng/ml的CT-1、或与20ng/ml的OSM组合。细胞培养维持3天。用所述细胞因子每天替换补充的培养基。根据“Ultraspec RNA Isolation System”方案(Biotech,USA)获得总RNA,其是基于Chomczynski和Sacchi[10]描述的方法。用DNAase(Gicbo-BRL,UK)处理两微克的总RNA,之后在存在RNaseOUT(Gibco-BRL)的情况下用M-MLV ReverseTranscriptase(Gibo BRL)逆转录。STAT、2-5OAS和β-肌动蛋白的表达通过使用Icycler和IQ SYBR Green Supermic(Bio-Rad Laboratories,CA)的实时RT-PCR的方式测量。2μl等分量的cDNA库用于每个PCR,其在最终20μl的体积中含有针对每个基因的特异性正向和反向引物(表1)。为了确定获得的PCR产物的特异性,分析它们的解离温度。在相同样品中β-肌动蛋白的量的基础上将结果标准化。每个转录产物的数量通过式2ct(肌动蛋白)-ct(基因)来表示,ct是在背景荧光上荧光信号显著提高的点。

实验3.IFN-α-2加IL-6家族细胞因子的组合对用全长HCV复制子转染的Huh7细胞中HCV复制的影响的研究(附图3)
由于在组合IFN-α-2加CT-1或OSM时观察到的更强ISG诱导,期望的是分析这种组合在具有全长HCV的细胞中病毒加载的降低方面是否优于IFN-α-2本身,或优于IL-6家族细胞因子本身。因而,携带HCV复制子的Huh7细胞与IFN-α(IFN-α-2或IFN-α-5,5或50IU/ml)加上或减去IL-6家族细胞因子(IL-6、CT-1、OSM;20ng/ml);或与IL-6家族细胞因子本身孵育。在培养72小时后测量HCV RNA的数量(附图3a、3B、3C)。在附图3中,人们可以看到IL-6家族细胞因子:IL-6、CT-1和OSM本身具有中度的抗病毒效果。然而,CT-1和OSM连同IFN-α-2以及IFN-α-5在这些细胞因子低剂量(5IU/ml)或高剂量(50IU/ml)使用时强烈地增强抗病毒效果(附图3A和3B)。IL-6连同IFN-α-2或IFN-α-5以较弱的方式增强抗病毒效果(附图3C)。因而IFN-α-2或IFN-α-5加上CT-1或OSM的组合治疗分别提高了IFN-α的抗病毒效果大约5和10的因数。进一步的,IFN-α-2或IFN-α-5加上IL-6的组合治疗提高了大约2的因数。
在附图4中,比较性地呈现了IL-6、CT-1和OSM(20ng/ml)对IFN-α-2(5U/mL)(附图4A)或IFN-α-5(附图4B)的抗病毒作用的增强效果。清楚的观察到组合IFN-α/CT-1和IFN-α/OSM相对于组合IFN-α/IL-6的更高的增强抗病毒效果。
实验方法3
HCV RNA的定量实时PCR分析。表达全长HCV复制子的Huh7细胞以100,000个/孔播种到6孔平板中具有10%FCS(Gibco)的D-MEN(Gibco)中。添加50或5IU/ml的IFN-α-2或IFN-α-5本身、或与20ng/ml的CT-1或OSM或IL-6组合。细胞培养维持3天。用所述细胞因子每天替换补充的培养基。

实验4.IFN-α-2加CT-1在针对脑心肌炎病毒(EMCV)的细胞病变效应的防御中作用的研究(图5)。
为了确定在不同于HCV的其他病毒感染中是否也产生组合治疗的协同效果,我们分析了在Huh7细胞中使用这种处理对抗EMCV的细胞病变效应获得的保护。在存在或缺乏低剂量(5ng/ml)或高剂量(50ng/ml)的CT-1的情况下用从250到1IU/ml的降低剂量的IFN-α-2孵育Huh7细胞,24小时后用EMCV感染。我们观察到,CT-1本身(在低剂量或高剂量)对EMCV感染的细胞具有很少的细胞保护效果,因为在非常低剂量的IFN-α-2下添加这种细胞因子没有改善细胞存活。然而,在低剂量和高剂量下CT-1都显著提高了IFN-α-2诱导的针对EMCV细胞病变效应的保护。在使用250到28IU/ml之间的IFN-α-2剂量时观察到这种协同作用,在83和28IU/ml的IFN-α-2剂量下协同作用最为显著。这些数据显示,IFN-α-2和CT-1的抗病毒协同作用不限于HCV,而适合于广泛的病毒疾病。
实验方法4
IFN-α-2和CT-1在Huh7细胞中对抗EMCV的细胞保护分析。通过测量这些细胞因子保护Huh7细胞对抗脑心肌炎病毒的细胞病变效应的能力来测定IFN-α-2和CT-1的细胞保护活性。在96孔微滴定板上进行分析。首先,将2×104个细胞/孔播种到150μl的培养基中,所述培养基含有IFN-α-2本身的系列稀释(250到1IU/ml)或IFN-α-2的这些系列稀释加50或5ng/ml CT-1,将它们孵育24小时。105PFU的EMC病毒添加到每个孔中,在24小时如下测量细胞病变效应:在清除了培养基之后,用PBS洗涤反应孔两次,用晶体紫染色溶液(在甲醇-水1∶4v/v中0.5%)染色。读取540nm处的光密度。结果表示为对抗EMCV细胞病变效应保护了的细胞的百分比。
实验5.IFN-α和各种白介素6家族细胞因子之间对全长HCV转染的Huh7细胞中HCV复制的影响的研究。
由于IFN-α和IL-6家族细胞因子的组合所显示的增强的抗病毒效果,期望的是确定在IFN-α-2或IFN-α-5与IL-6家族细胞因子:IL-6、OSM和CT-1的组合中建立的相互作用的类型。
组合固定浓度的IFN-α-2和IFN-α-5(5或50UI/ml)、有或没有IL-6、OSM和CT-1(20ng/ml)在全长HCV复制子转染的Huh7细胞中进行几项抗病毒测试。根据T.C.Chou(11)描述的方法通过多变量分析对IFN-α和IL-6家族细胞因子之间建立的相互作用类型进行数学分析。通过称为相互作用指数“I”的因子来测量两种物质之间的相互作用类型,其中I=d1/D1+d2/D2;d1、d2是组合的抑制物速率,D1、D2是将与该组合产生相同效果的每个抑制物的速率。从而,“I”等于1意味着两种物质在它们之间不反应(叠加效果);“I”低于1意味着该组合是协同性的;大于1的“I”值意味着该组合是拮抗性的。表2显示了对于“I”值的标准协同速率。

IFN-α和先前描述的IL-6家族细胞因子的不同组合获得的相互作用指数在所有情况下低于1,因而IFN-α(2或5)和这些细胞因子IL-6、OSM和CT-1的组合是协同性的(表3)。此外,获得的数据显示,在组合α/OSM和IFN-α/CT-1(I<0.3强协同作用)下协同效果总是显著高于组合IFN-α/IL-6(I<0.7协同作用)的情况。

实验方法5
HCV RNA的定量实时PCR分析。表达全长HCV复制子的Huh7细胞以20,000个/孔播种到24孔平板中具有10%FCS的D-MEN中。进行不同的实验;添加50或5IU/ml的IFN-α-2或IFN-α-5、有或没有20ng/ml的CT-1、OSM或IL-6(R&D Systems,UK)。细胞培养维持3天。用所述细胞因子每天替换补充的培养基。
使用Kit Nucleic Acid Purification Lysis Solution(AppliedBioSystems,Foster City,CA)和半自动化系统ABI PRISM 6100 NucleicAcid PrepStation(Applied BioSystems)获得用全长HCV复制子转染的Huh7细胞的总RNA。用DNAase(Gicbo-BRL,UK)处理两微克的总RNA,之后在存在RNaseOUT(Gibco-BRL)的情况下用M-MLV ReverseTranscriptase(Gibo BRL)逆转录。HCV RNA和β-肌动蛋白mRNA的表达通过使用Icycler和IQ SYBR Green Supermic(Bio-RadLaboratories)的定量实时RT-PCR测量。2μl等分量的cDNA库用于每个PCR,其在最终20μl的体积中含有针对HCV或β-肌动蛋白基因的5非翻译区的特异性正向和反向引物(表1)。为了确定获得的PCR产物的特异性,分析它们的解离温度。在相同样品中β-肌动蛋白的量的基础上将结果标准化。HCV RNA的数量通过式2ct(肌动蛋白)-ct(HCV)来表示,ct是在背景荧光上荧光信号显著提高的点。

Claims (23)
1.至少一种IL-6家族细胞因子在制备药物组合物中的用途,所述药物组合物设计用于在病毒疾病的治疗中与至少一种IFN-α组合施用,所述细胞因子选自制瘤素M、心肌营养蛋白-1、以及其组合。
2.根据权利要求1的用途,特征在于所述IFN-α选自IFN-α-2a、IFN-α-2b、IFN-α-5、共有序列干扰素、纯化的IFN-α、聚乙二醇化的IFN-α以及其组合。
3.根据权利要求1的用途,特征在于所述IFN-α选自聚乙二醇化的IFN-α-2b、聚乙二醇化的IFN-α-2a、聚乙二醇化的IFN-α-5以及其组合。
4.根据权利要求1的用途,特征在于所述IL-6家族细胞因子选自完整天然蛋白以及维持了所述完整细胞因子的生理学效果的所述细胞因子的部分多肽序列。
5.根据权利要求1的用途,特征在于所述病毒疾病是C型肝炎。
6.根据权利要求1的用途,特征在于所述IL-6家族细胞因子和IFN-α在不同的药物组合物中分别施用。
7.根据权利要求1的用途,特征在于所述IL-6家族细胞因子和IFN-α在同一药物组合物中共同施用。
8.一种药物组合物,特征在于它包含药学上可接受量的至少一种IL-6家族细胞因子,以及药学上可接受量的至少一种IFN-α,其中所述细胞因子选自制瘤素M、心肌营养蛋白-1、以及其组合。
9.根据权利要求8的药物组合物,特征在于所述IL-6家族细胞因子选自完整天然蛋白、以及维持了所述完整细胞因子的生理学效果的所述细胞因子的部分多肽序列。
10.根据权利要求8的药物组合物,特征在于所述IFN-α选自IFN-α-2a、IFN-α-2b、IFN-α-5、共有序列干扰素、纯化的IFN-α、聚乙二醇化的IFN-α以及其组合。
11.根据权利要求8的药物组合物,特征在于所述IFN-α选自聚乙二醇化的IFN-α-2b、聚乙二醇化的IFN-α-2a、聚乙二醇化的IFN-α-5以及其组合。
12.根据权利要求8的药物组合物,特征在于它另外含有至少一种赋形剂,其与所述IL-6家族细胞因子以及与所述IFN-α是药学上相容的。
13.根据权利要求8的药物组合物,特征在于所述IL-6家族细胞因子和所述IFN-α在各自的载体试剂中携带。
14.一种用于病毒疾病的治疗的药盒,特征在于它至少包括:
-第一成分,其包含至少一种IL-6家族细胞因子;所述细胞因子选自制瘤素M、心肌营养蛋白-1、以及其组合,
-第二成分,其包含至少一种IFN-α。
15.根据权利要求14的药盒,特征在于所述IL-6家族细胞因子选自完整天然蛋白以及维持了所述完整细胞因子的生理学效果的所述细胞因子的部分多肽序列。
16.根据权利要求14的药盒,特征在于所述IFN-α选自IFN-α-2a、IFN-α-2b、IFN-α-5、共有序列干扰素、纯化的IFN-α、聚乙二醇化的IFN-α以及其组合。
17.根据权利要求14的药盒,特征在于所述IFN-α选自聚乙二醇化的IFN-α-2b、聚乙二醇化的IFN-α-2a、聚乙二醇化的IFN-α-5以及其组合。
18.根据权利要求14的药盒,特征在于它另外包含第三成分,其包含一种或多种赋形剂,其与所述IL-6家族细胞因子以及与所述IFN-α是药学上相容的。
19.根据权利要求14的药盒,特征在于它另外包含第三成分,其包含一种或多种载体试剂,所述载体试剂与所述IL-6家族细胞因子以及与所述IFN-α是药学上相容的。
20.根据权利要求14的药盒,特征在于所述第一成分另外包含至少一种赋形剂,所述赋形剂是药学上可接受的并与所述IL-6家族细胞因子以及与所述IFN-α是药学上相容的。
21.根据权利要求14的药盒,特征在于所述第二成分另外包含一种或多种赋形剂,所述赋形剂是药学上可接受的并与所述IL-6家族细胞因子以及与所述IFN-α是药学上相容的。
22.根据权利要求14的药盒,特征在于所述第一和第二成分存在于独立的药物组合物中。
23.根据权利要求14的药盒,特征在于所述第一和第二成分存在于同一药物组合物中。
