Abstract

技术领域
本发明一般涉及医疗图像及视频的存储、存档、联网和检索,更具体地涉及在具有不同的可能带宽的网络中运行的图片存档和通信系统(PACS)中的改进。
背景技术
PACS是一种医疗图像的存储、检索和显示系统。PACS典型地由一个或更多的联网计算机连同例如RAID(冗余廉价硬盘阵列)、磁带存储器或光盘形式的大量半永久性数字存储器组成。PACS还典型地包括存储、检索和显示图像的软件,以及用于数字介质的物理管理(例如,机器人磁带装载设备)、显示和输入需要的硬件。

在很多情况中,可接受的是,选择适合的网络带宽用于PACS的实施,并且选择被优化与这种期望的网络工作的通信协议。但是,经验表明在不同的网络带宽下这种系统将会经常结束操作,并且有利的是有一个能够关于各种网络带宽有效运行的PACS。

另一种可能性是待流传输的图像数据的传统压缩。但是,在很多医疗应用中,一些压缩类型,尤其是无损压缩,或者不受欢迎或者被彻底禁止。所关心的是医疗诊断工作是非常重要并且精细的任务而不能负担可能由有意地从其原始数量的像素、位深度和其他特性降低图像质量而导致的额外不确定性。无损压缩技术,即使被允许时,也通常仅仅将图像文件大小降低一个数量级,这在很多情况下不足以在低带宽信道上提供实时流传输。医疗图像特别地不适合无损压缩,因为很多医疗成像模态例如CT或超声固有地产生具有巨大纯噪声成分的图像,其使得它们本身不适合于使用已知的无损方法进行大量压缩。
即使压缩是可行的,仍有困难需要解决。例如,基于小波的JPEG2图像压缩标准提供了渐进的压缩能力,它允许图像在分开的质量“层”中传输。最初传输的图像随着更多数据的到达而改进质量。在“无损”模式中,JPEG2处理允许数据传输直到接收的图像与原始图像的质量相同为止。在一些情况中,这种格式的使用提供了某种对变化的带宽固有的自适应性方式,因为一旦带宽下降至低于实时无损传输需要的大小,简单地以稍微更低的质量继续传输,在图像到达原始图像质量之前被丢弃。
再一个已知的利用JPEG2的改进是当全部带宽原始质量传输是不可能的时候,仅为特定感兴趣区域(ROI)传输高质量图像。
这些解决方案在原始图像以遵从JPEG2的DICOM格式可用的情况下是可用的,但是很多成像模态不使这种格式的图像可用。即使在这种图像可用的情况下,无损JPEG2编码在计算上过于苛求,典型地比上述JPEG标准下的可比无损编码慢5-6倍。相对地,解码和再现这种JPEG2图像同样要求更加密集的计算,并且可能导致在传统的观察工作站上不可接受的性能。
美国专利6,314,452公开了一种利用小波流传输部分解决使用JPEG2中的困难的部分解决方案。但是,这种及其他的已知技术仍旧施加了不必要的开销,例如当宽的网络带宽可用并且不需额外处理可完成图像传输时仍需进行编码/解码图像。
一些请求式的视频应用还通过可伸缩的视频流传输和服务质量来解决类似的问题。利用MPEG视频压缩标准的高级能力,能够在每个设备(即,观察设备)基础上控制速率/失真折衷。因此,能够在各种观察节点之中动态地分配比特流并且把更高解析度视频流传输至更高解析度观察设备以及向某些用户提供更高的可视质量。但是,这些解决方法典型地假设从单个预处理的由解析度和质量层组成的MPEG文件流传输视频,这是一种通常在很多医疗应用中不实际的假设。
在医疗远程放射学应用中,传统的解决方案在非高峰期内安排预先传输大的无损DICOM图像,由此放射线学者或其他的医护人员可接收、存储并在以后审阅图像。使用各种技术试图预测何时将会出现非高峰时段并且将其与待传输的图像相关的医护需求相匹配。
美国专利no.6,848公开了一种网页中富媒体(rich-media)内容的自适应递送系统。该文献公开了客户端-服务器系统,其中客户端计算带宽而服务器自适应地传输网页内容,当可使用更高的带宽时传输更丰富的内容。这种技术依赖于动态地装载例如JAVA小应用程序的小应用程序技术的可用性,其可能无法在所有的情况中可用。
同样,美国专利no.6,243,761公开了一种通过服务器根据有效带宽和/或等待时间特性而动态地调整网页的多媒体内容的方法。在这个实例中,例如通过减少大小、解析度、或图形图像的图像数量来调整发送至客户端计算机的内容。而这种内容的改变可能在很多网页浏览器应用中是可接受的,但是对于很多的医疗应用却并不合适。

发明内容
为了解决传统图像存档和通信系统的上述问题,根据本发明,当用户请求图像时,观察器子系统首先确定用于流传输图像的可用带宽或在一些实施例中的其他相关的参数,例如用于预/后处理图像的计算资源、数据类型和大小。在检测到关于图像数据大小的高带宽的情况下,选择使用以其原来(例如,DICOM)形式的图像的流传输模式。在较少带宽可用的情况下,选择若干模式中的一个用于传输,所述若干模式中的每个模式需要较少的带宽但是需要更大的处理开销。
在一个实施例中,一系列较低带宽模式包括无损压缩、ROI流传输像素数据、无损压缩ROI像素数据、基线小波无损压缩和渐进小波无损压缩。

本发明包括在PACS中传输医疗图像的系统和方法。在这里详细说明的实施例中,流传输模式被选择来适合于给定的网络带宽和稳定性。带宽为高并且稳定的场合,使用原始图像文件的简单文件传输。带宽为低或网络不稳定的场合,采用更慢且允许更多错误的传输模式。

在一个实施例中,水平“5”近似地为100MB/s的带宽,水平4近似的为50MB/s,水平3近似的为20MB/s和水平2近似的为5MB/s。在可替换的实施例中,使用不同数量的水平和不同的阈值。例如,可以使用标记为1的较低带宽,其中允许有损压缩并且警告用户流传输的图像是有损的。一些医疗应用可能允许使用这种有损压缩,但很多是不允许的。
在可替换的实施例中,一些存储在服务器120内的图像信息可能已经是既以有损形式又以无损形式,在这种情况下,替换的模式(例如整个有损形式的直接传输)可被使用。在一些应用中,为了其他目的,其他处理系统已经产生了不同版本的图像文件,例如有损压缩版本,并且它们可能存储在高速缓存121中用于当需要时被迅速访问。如果关心服务器端的处理开销,则这种高速缓存121的使用减少了对服务器120重复压缩处理的需要。
带宽阈值和传输模式的选择不限于上述示例。例如,如果工作站101仅有普通的处理能力,则与如果工作站101具有强壮的处理能力相比,需要工作站101重大解码的传输模式将不受欢迎。
同样地,网络120和图像自身的特点需要传输模式和阈值的特殊选择。如果图像相对网络带宽非常大,有益的是,比其他情况更快速地切换至基于ROI的模式。
模式的选择不限于先验选择。在一个实施例中,用户可适应地按照需要在可用模式之间切换。在另一个实施例中,当检测到网络性能的变化时和实际性能不同于最初期望的性能时,模式切换被自动完成。
上述实施例在客户端(即,在观察工作站101处)执行控制。在另一个实施例中,服务器120,或甚至外部设备(未示出)能够进行带宽确定和模式选择。
在此描述的实施例中,服务器120仅需要存储原始(例如DICOM)形式的图像,而不需要预处理或复制数据。在另一方面,如果这种作为其他处理结果的另外的图像版本已经在服务器上,那些版本也可被使用,并且可存储在高速缓存121中。

Claims (7)
1.一种成像系统(100),包括:
服务器(120),存储医疗图像信息并且以多个传输模式传送所述医疗图像信息;
网络(110),适于从所述服务器(120)传送所述医疗图像信息;和
观察工作站(101),可操作地与所述网络(110)连接,所述观察工作站(101)包括带宽测量系统(102),所述带宽测量系统(102)确定用于从所述服务器向所述观察工作站传送医疗图像信息的可用带宽的估计,其中所述观察工作站接收所述可用带宽的估计,基于所述可用带宽的估计和可靠性的范围、在所述观察工作站和所述服务器上期望的可用处理能力以及对要在所述工作站上被观察的医疗图像的医疗实践需求从所述多个传输模式选择一个传输模式,并且请求以所选传输模式从所述服务器(120)传送所述图像信息,并且其中所述服务器向所述观察工作站传送所述医疗图像信息。
2.根据权利要求1所述的成像系统,其中所述多个传输模式包括原始图像文件传输(203)、无损压缩图像文件数据传输(205)、感兴趣区域像素数据传输(207)、小波无损压缩图像文件数据传输(209)和感兴趣区域小波无损压缩数据传输(210)。
3.根据权利要求1所述的成像系统,其中所述观察工作站(101)配置为响应于相对于所述医疗图像信息的特点的所述观察工作站(101)的性能特点请求以所述多个传输模式中的所述一个进行传输。
4.一种在包括与观察工作站(101)相耦合的服务器(120)的图像存档和通信系统(PACS)内通过网络(110)传送医疗图像信息的方法,所述方法包括:
确定所述网络的性能度量(201);
确定所述服务器的期望的可用处理能力;
确定所述观察工作站的期望的可用处理能力;
确定对要在所述工作站上被观察的医疗图像的医疗实践需求;
用所述观察工作站从多个传输模式中选择一个传输模式,其中传输模式的选择基于所述性能度量、所述服务器的期望的可用处理能力、所述观察工作站的期望的可用处理能力和所述医疗实践需求;
用所述观察工作站请求以所选传输模式从所述服务器(120)传送所述图像信息;和
以所选传输模式通过所述网络从所述服务器向所述观察工作站传送所述图像信息。
5.根据权利要求4的方法,其中所述性能度量为带宽测量系统所估计的网络带宽。
6.根据权利要求5的方法,其中选择包括所述观察工作站将所述带宽与多个阈值进行比较,所述多个阈值中的每个阈值与所述多个传输模式中之一的压缩技术相关联。
7.根据权利要求4的方法,其中,所选传输模式是包括以下的多个模式中之一:原始图像文件传输(203)、无损压缩图像文件数据传输(205)、感兴趣区域像素数据传输(207)、小波无损压缩图像文件数据传输(209)、感兴趣区域小波无损压缩图像数据传输(210)以及压缩感兴趣区域图像传输。
