DESCRIPTION
Les fiches transcutanées sont connues depuis de nombreuses années et sont utilisées dans la chirurgie osseuse, notamment dans l'ostéosynthèse externe. Ces fiches traversent la chair et les os et sont en général utilisées avec des appareils de fixation squelettique externe développés par le Dr Hoffmann. Ces appareils de fixation squelettique externe comprennent des rotules, des barres de liaison, des barres coulissantes, des articulations, des étaux d'ancrage destinés à retenir et à positionner les fiches transcutanées. Les fixateurs externes d'Hoffmann consistent en un cadre de montage placé autour d'un membre ou d'une partie du corps humain, ce cadre étant agencé pour maintenir depuis l'extérieur des os ou fragments d'os à l'aide de fiches ou de groupes de fiches transcutanées.
Il existe actuellement sur le marché un grand nombre de types de fiches transcutanées. Ces fiches comprennent généralement une partie filetée avec à leur extrémité un taraud agencé pour effectuer une opération de taraudage lorsque la partie filetée de la fiche est vissée dans un fragment ou élément d'os. Avant la mise en place d'une telle fiche, l'os ou le fragment d'os a été préalablement percé au diamètre voulu, lequel est sensiblement égal ou supérieur au diamètre intérieur du filetage. L'extrémité de la fiche est alors introduite dans le trou percé et la partie filetée de la fiche est vissée à l'intérieur du trou. Lors de cette opération, le ou les tarauds prévus à l'extrémité de la fiche effectuent le taraudage du trou préalablement percé.
Pour visser la fiche dans le trou, on utilise habituellement une clé ou un mandrin venant s'introduire dans une partie à section polygonale, par exemple une section carrée située à l'une des extrémités de la fiche. Dans les fiches de l'art antérieur, l'extrémité de la partie filetée peut se présenter comme l'extrémité d'une mèche de perçage ou présenter une simple pointe.
Il existe également un type de fiche, appelée fiche transfixiante, dans laquelle la partie filetée est précédée d'une tige présentant à son extrémité libre une pointe de perçage. Les fiches transfixiantes peuvent être mises en place sans prévoir préalablement un perçage dans l'os. Dans la fiche transfixiante, le perçage sera effectué par l'extrémité de la pointe et le taraudage par des tarauds prévus au début du filetage.
Les fiches transcutanées de l'art antérieur ne permettent pas d'obtenir un perçage ou un taraudage de très bonne qualité. Comme dans toute opération de perçage ou de taraudage, la température augmente à l'endroit du perçage ou du taraudage, ce qui crée des nécroses et des détériorations dans les tissus osseux. D'autre part, les fiches de l'art antérieur ne permettent pas d'obtenir des surfaces de perçage de bonne qualité. Ces surfaces présentent des parties arrachées qui ne permettront pas par la suite d'obtenir une liaison solide entre le fragment d'os et la fiche lorsque cette dernière est mise en place. Finalement, les fiches de l'art antérieur, de par leur mauvais fonctionnement quant à l'opération de perçage ou de taraudage, peuvent provoquer des contraintes dans l'os lorsqu'elles sont en place.
Le but de la présente invention est de proposer une fiche transcutanée assurant un perçage ou un taraudage de bonne qualité, tout en garantissant une augmentation de température minimum lors de l'insertion.
La fiche transcutanée de fixation d'un fragment ou élément osseux selon l'invention, comprenant une partie filetée avec à l'extrémité avant de la partie filetée des goujures régulièrement disposées sur la circonférence de la fiche et agencées pour effectuer une opération de taraudage lorsque l'extrémité de la partie filetée pénètre dans le fragment de l'élément osseux, est caractérisée en ce que les goujures se présentent sous la forme d'une encoche définissant sur le filetage une arête de coupe du taraud, un dégagement de dépouille étant réalisé à la suite de l'arête de coupe du taraud et allant en se rapprochant de l'axe de la fiche jusqu'à l'encoche suivante, de manière à former un angle de dépouille.
La présence du dégagement de dépouille permet un taraudage de bonne qualité et une pénétration plus facile de la fiche dans l'os. De plus, étant donné que les copeaux ne restent pas à l'intérieur de l'os durant l'opération et ne forment ainsi pas obstacle pour l'arête de coupe suivante, l'augmentation dé température est beaucoup moins élevée qu'avec les fiches de l'art antérieur.
La partie avant du filetage de la fiche peut se présenter sous la forme d'un cône d'entrée, les goujures définissant les arêtes de coupe s'étendant sur le cône d'entrée du filetage et sur le début de la partie centrale cylindrique dudit filetage.
Différentes variantes peuvent être prévues pour réaliser les goujures formant l'arête de coupe. Les goujures peuvent se présenter sous la forme d'une encoche formée de deux surfaces sensiblement perpendiculaires et sont parallèles à l'axe de la fiche. En variante, les goujures peuvent se présenter sous la forme d'une encoche de section arrondie et disposée en hélice sur le filetage. Les goujures sont, de préférence, réalisées par meulage, après que le filetage avec son cône d'entrée a été réalisé.
La fiche peut présenter une pointe arrondie placée au début du cône d'entrée du filetage.
Dans une forme particulière, l'invention comprend également une fiche transfixiante, dans laquelle le cône d'entrée du filetage est précédé d'une tige de plus faible diamètre que la partie centrale du filetage, la tige se terminant par une pointe plate de perçage en forme d'ogive. La pointe plate en forme d'ogive peut présenter à sa partie supérieure deux saignées de dégagement placées en regard l'une de l'autre.
La pointe plate en forme d'ogive peut présenter une section en forme de parallélogramme allongé définissant deux surfaces de coupe opposées, chacune suivie d'une surface formant un angle de dépouille, lesdites surfaces de coupe et de dépouille formant ensemble une arête de coupe, les surfaces de coupe présentant un évidement allongé sous forme de cuillère placé adjacent à l'arête de coupe, de manière à modifier l'angle de coupe pour l'amener à O ou vers une valeur positive.
Le dessin représente, à titre d'exemples, deux modes d'exécution de la fiche transcutanée objet de l'invention.

La fiche transcutanée 1 représentée dans les fig. 1 et 3 comprend une partie filetée 2 qui est la partie avant de la fiche et une partie arrière que nous appellerons partie lisse 3. La fiche 1 est une fiche cylindrique de faible diamètre par rapport à sa longueur et la partie arrière ou partie lisse 3 présente une rainure 4 et une partie d'extrémité à quatre pans 5. La partie d'extrémité 5 permettra l'introduction d'une clé ou d'un mandrin qui servira à visser la fiche 1 dans un fragment ou élément osseux. La partie avant 2 de la fiche 1 (voir en particulier la fig. 3) comprend un filetage 6 dont la partie avant se présente sous la forme d'un cône d'entrée 7 terminé par une pointe arrondie 8.
Trois goujures 9 (voir également la coupe de la fig. 6) sont placées à 120 rune par rapport à l'autre sur le pourtour de l'extrémité avant de la fiche, de manière à permettre l'opération de taraudage. Ces goujures 9 se présentent sous la forme de deux surfaces perpendiculaires 10 et 11, la surface 10 définissant à son extrémité libre la tête de coupe 12 du taraud. Chacune des arêtes de coupe 12 est suivie dans le sens de rotation de la fiche par un dégagement de dépouille 13 allant en se rapprochant de l'axe de la fiche jusqu'à son intersection avec la surface 11 de la goujure suivante 9.
Ce dégagement de dépouille 13 permettra aux copeaux d'être évacués lors de l'opération de taraudage. Ainsi, les copeaux ne seront pas repris et pressés contre la surface du trou qui est en train d'être taraudé par l'arête de coupe 9 suivante. Des contraintes sur l'os seront ainsi évitées et l'échauffement de l'os diminué. On évitera aussi par ce fait des parties arrachées sur les bords du trou qui est en train d'être taraudé et on obtiendra une surface de perçage de bonne qualité. Cela a pour but de diminuer les nécroses éventuelles, tout en permettant une meilleure tenue de la fiche à l'intérieur du trou taraudé lorsque celle-ci sera en place.
Les goujures 9 sont de préférence réalisées par meulage. L'opération de meulage est réalisée à l'aide d'une simple meule circulaire déplacée tangentiellement sur l'extrémité du filetage 6. La réalisation des goujures 9 par meulage donne une bonne qualité de surface à l'arête de coupe et permet d'éviter une opération d'ébavurage ultérieure. La réalisation du dégagement de dépouille 13 se fait de la même manière en approchant une meule de l'extrémité de la fiche en faisant tourner celle-ci tout en rapprochant la meule de l'axe de la fiche.
Bien que la réalisation des goujures 9 et du dégagement de dépouille ait donné de très bons résultats par meulage, il est évident pour l'homme du métier qu'elle peut également être faite par fraisage ou par tout autre moyen adéquat.
La fiche 15 de la fig. 2, qui est une fiche transfixiante, comprend comme la fiche de la fig. 1 une partie filetée 2, une partie arrière 3 à l'extrémité de laquelle est prévue une rainure annulaire 4 et une partie d'extrémité 5 à quatre pans destinée à recevoir une clé ou un mandrin de vissage.
La partie filetée 2 comprend un filetage 6 avec un cône d'entrée 7. A l'avant du cône d'entrée 7 se trouve une tige 16 terminée par une pointe 17. Les goujures de taraudage 9 sont réalisées de la même manière que celles décrites en regard des fig. 1 et 3 au moyen d'une meule circulaire qui sera déplacée tangentiellement à l'axe de la fiche 15 en partant de la tige 16 pour se déplacer sur le cône d'entrée 7 et déboucher sur le filetage 6. De même, le dégagement de dépouille 13 est réalisé de la même manière que le dégagement correspondant de la fiche de la fig. 1 sur le cône d'entrée. La coupe selon la ligne VI-VI de la tige de la fig. 4 présente par conséquent exactement la même configuration que la coupe de la fig. 3.
La fig. 5 représente une variante de la partie avant du filetage de la fiche transfixiante de la fig. 2. Dans cette variante, le filetage 6 est réalisé de la même manière que dans le mode d'exécution de la fig. 2 avec son cône d'entrée 7. Les goujures de taraudage sont réalisées sous la forme d'une rainure 20 (voir également la fig. 7), un des côtés de la rainure circulaire définissant les arêtes de coupe 21. Les arêtes de coupe 21 sont suivies, de même que dans les deux modes d'exécution précédents des fig. 1 et 2, d'un dégagement de dépouille 13 réalisé de la même manière que les dégagements 13 des fig. 3 et 4. Il est évident pour l'homme du métier que la variante des fig. 5 et 7 fonctionne de la même manière que les modes d'exécution des fig. 1 et 2 et permet de réaliser les mêmes résultats et d'obtenir les mêmes qualités de taraudage.
La pointe 17 placée à l'extrémité de la tige 16 de la fiche transfixiante représentée dans la fig. 2 se présente sous la forme d'une ogive 22,1'ogive ayant à sa partie supérieure deux saignées de dégagement 23 (fig. 8 et 9). La pointe en forme d'ogive 22 présente des caractères constructifs particulièrement intéressants qui sont visibles sur la fig. 9. La coupe selon la ligne IX-IX à travers l'ogive 22 de la fig. 8 présente une configuration en forme de parallélogramme allongé comprenant deux surfaces de coupe 24 définissant une arête de coupe 25 suivie d'une surface de dépouille 26.
Si l'on fait tourner la pointe dans le sens de rotation représenté par la flèche F, on remarque que les surfaces 25 définissent un angle de dépouille suffisamment important pour permettre le dégagement des copeaux obtenus lors du perçage d'un os au moyen de l'ogive 22. Par contre, l'homme du métier remarquera, dans la fig. 9, que les surfaces planes 24 définissent des surfaces de coupe avec un angle de coupe a négatif. Pour modifier cet angle de coupe a et l'amener à O ou vers une valeur positive, il a été prévu deux évidements allongés 27 placés adjacents à l'arête de coupe 25. Ainsi, l'angle de coupe a peut être corrigé et choisi en fonction des désirs de l'utilisateur en modifiant la profondeur des évidements 27.
La pointe des fig. 8 et 9 avec sa forme en ogive, ses surfaces de dépouille 26 et ses évidements 27 ramenant l'angle de coupe vers 0 ou vers une valeur positive, permet d'effectuer des perçages de très bonne qualité par rapport aux fiches de perçage de l'art antérieur. La pointe en forme d'ogive est d'autre part particulièrement adéquate, car elle se fiche dans l'os et permet d'effectuer un perçage selon un axe sans aucune déviation latérale.
La fiche qui vient d'être décrite en regard des fig. 1 et 3 du dessin présente une longueur totale qui peut varier entre 75 et 200 mm pour des diamètres allant de 3 à 6 mm. La fiche représentée selon le mode d'exécution des fig. 2, 4 et 5, avec une partie filetée centrale, aura une longueur totale allant de 175 à 350 mm pour des diamètres allant également de 3 à 6 mm. La partie avant de la fiche représentée dans la fig. 2, consistant en la tige 16, aura un diamètre légèrement plus petit que le diamètre du filetage 6. Pour une fiche de 3, respectivement 4, 5 et 6 mm, la partie 16 aura respectivement 2, 3, 4 et 5 mm.
DESCRIPTION
Transcutaneous cards have been known for many years and are used in bone surgery, in particular in external osteosynthesis. These plugs pass through the flesh and bones and are generally used with external skeletal fixation devices developed by Dr. Hoffmann. These external skeletal fixation devices include ball joints, connecting bars, sliding bars, joints, anchoring vices intended to retain and position the transcutaneous sheets. Hoffmann external fixators consist of a mounting frame placed around a limb or part of the human body, this frame being arranged to hold bones or bone fragments from the outside using plugs or groups of transcutaneous sheets.
There are currently a large number of types of transcutaneous cards on the market. These plugs generally comprise a threaded part with at their end a tap arranged to perform a tapping operation when the threaded part of the plug is screwed into a fragment or element of bone. Before the installation of such a plug, the bone or the bone fragment has been previously drilled to the desired diameter, which is substantially equal to or greater than the internal diameter of the thread. The end of the plug is then introduced into the drilled hole and the threaded part of the plug is screwed inside the hole. During this operation, the tap or taps provided at the end of the plug perform the tapping of the previously drilled hole.
To screw the plug into the hole, a key or a mandrel is usually used which is introduced into a part with a polygonal section, for example a square section located at one end of the plug. In the sheets of the prior art, the end of the threaded part may appear as the end of a drill bit or have a single point.
There is also a type of plug, called a transfixing plug, in which the threaded part is preceded by a rod having at its free end a piercing point. The transfixing cards can be put in place without first providing for a hole in the bone. In the transfixing sheet, the drilling will be carried out by the end of the point and the tapping by taps provided at the start of the threading.
The transcutaneous sheets of the prior art do not make it possible to obtain very good quality drilling or tapping. As in any drilling or tapping operation, the temperature increases at the location of the drilling or tapping, which creates necrosis and deterioration in bone tissue. On the other hand, the sheets of the prior art do not make it possible to obtain good quality drilling surfaces. These surfaces have parts torn off which will not subsequently make it possible to obtain a solid bond between the bone fragment and the plug when the latter is put in place. Finally, the plugs of the prior art, due to their poor functioning as regards the drilling or tapping operation, can cause stresses in the bone when they are in place.
The purpose of the present invention is to provide a transcutaneous plug ensuring good quality drilling or tapping, while guaranteeing a minimum temperature increase during insertion.
The transcutaneous plug for fixing a bone fragment or element according to the invention, comprising a threaded part with flutes at the front end of the threaded part regularly arranged on the circumference of the plug and arranged to perform a tapping operation when the end of the threaded part penetrates into the fragment of the bone element, is characterized in that the flutes are in the form of a notch defining on the thread a cutting edge of the tap, a clearance of relief being produced following the cutting edge of the tap and moving closer to the axis of the plug to the next notch, so as to form a draft angle.
The presence of the clearance clearance allows good quality tapping and easier penetration of the plug into the bone. In addition, since the chips do not remain inside the bone during the operation and thus do not form an obstacle for the next cutting edge, the increase in temperature is much lower than with the sheets of the prior art.
The front part of the thread of the plug may be in the form of an inlet cone, the flutes defining the cutting edges extending over the inlet cone of the thread and on the beginning of the cylindrical central part of said thread.
Different variants can be provided for making the flutes forming the cutting edge. The flutes can be in the form of a notch formed from two substantially perpendicular surfaces and are parallel to the axis of the plug. Alternatively, the flutes can be in the form of a notch of rounded section and arranged helically on the thread. The flutes are preferably produced by grinding after the threading with its inlet cone has been carried out.
The plug may have a rounded point placed at the beginning of the thread entry cone.
In a particular form, the invention also comprises a transfixing plug, in which the thread entry cone is preceded by a rod of smaller diameter than the central part of the thread, the rod ending in a flat piercing point. in the shape of a warhead. The flat point in the shape of an ogive may have at its upper part two relief grooves placed opposite one another.
The flat point in the shape of a warhead can have a section in the form of an elongated parallelogram defining two opposite cutting surfaces, each followed by a surface forming a draft angle, said cutting and draft surfaces together forming a cutting edge, the cutting surfaces having an elongated recess in the form of a spoon placed adjacent to the cutting edge, so as to modify the cutting angle to bring it to 0 or towards a positive value.

The transcutaneous sheet 1 represented in FIGS. 1 and 3 includes a threaded part 2 which is the front part of the plug and a rear part which we will call smooth part 3. The plug 1 is a cylindrical plug of small diameter compared to its length and the rear part or smooth part 3 has a groove 4 and a four-sided end part 5. The end part 5 will allow the introduction of a key or a mandrel which will be used to screw the plug 1 into a fragment or bone element. The front part 2 of the plug 1 (see in particular fig. 3) comprises a thread 6, the front part of which is in the form of an inlet cone 7 terminated by a rounded point 8.
Three flutes 9 (see also the section in fig. 6) are placed 120 rune from one another around the periphery of the front end of the plug, so as to allow the tapping operation. These flutes 9 are in the form of two perpendicular surfaces 10 and 11, the surface 10 defining at its free end the cutting head 12 of the tap. Each of the cutting edges 12 is followed in the direction of rotation of the plug by undercut clearance 13 approaching the axis of the plug until it intersects with the surface 11 of the next flute 9.
This clearance clearance 13 will allow the shavings to be removed during the tapping operation. Thus, the chips will not be picked up and pressed against the surface of the hole which is being tapped by the next cutting edge 9. Stresses on the bone will thus be avoided and the heating of the bone reduced. This will also prevent parts torn off the edges of the hole which is being tapped and we will obtain a good quality drilling surface. This is to reduce possible necrosis, while allowing better grip of the plug inside the tapped hole when it is in place.
The flutes 9 are preferably produced by grinding. The grinding operation is carried out using a simple circular grinding wheel moved tangentially over the end of the thread 6. The production of the flutes 9 by grinding gives a good surface quality to the cutting edge and makes it possible to avoid a subsequent deburring operation. The clearance clearance 13 is produced in the same way by approaching a grinding wheel from the end of the plug by rotating the latter while bringing the grinding wheel closer to the axis of the plug.
Although the production of the flutes 9 and the clearance clearance has given very good results by grinding, it is obvious to a person skilled in the art that it can also be made by milling or by any other suitable means.
Sheet 15 of fig. 2, which is a transfixing card, includes, like the card in FIG. 1 a threaded part 2, a rear part 3 at the end of which is provided an annular groove 4 and an end part 5 with a hexagon intended to receive a wrench or a screwing chuck.
The threaded part 2 comprises a thread 6 with an inlet cone 7. At the front of the inlet cone 7 is a rod 16 terminated by a point 17. The tapping flutes 9 are produced in the same manner as those described next to fig. 1 and 3 by means of a circular grinding wheel which will be moved tangentially to the axis of the plug 15 starting from the rod 16 to move on the inlet cone 7 and lead to the thread 6. Similarly, the clearance of undercut 13 is produced in the same way as the corresponding clearance of the plug of FIG. 1 on the inlet cone. The section along line VI-VI of the rod of FIG. 4 therefore has exactly the same configuration as the section of FIG. 3.
Fig. 5 shows a variant of the front part of the thread of the transfixing plug of FIG. 2. In this variant, the thread 6 is produced in the same manner as in the embodiment of FIG. 2 with its inlet cone 7. The tapping flutes are produced in the form of a groove 20 (see also fig. 7), one side of the circular groove defining the cutting edges 21. The cutting edges 21 are followed, as in the two previous embodiments of FIGS. 1 and 2, of a clearance clearance 13 produced in the same manner as the clearances 13 of FIGS. 3 and 4. It is obvious to a person skilled in the art that the variant of FIGS. 5 and 7 works in the same way as the embodiments of FIGS. 1 and 2 and allows the same results to be achieved and the same tapping qualities to be obtained.
The point 17 placed at the end of the rod 16 of the transfixing plug shown in FIG. 2 is in the form of a warhead 22, 1 warhead having at its upper part two relief grooves 23 (fig. 8 and 9). The pointed tip 22 has particularly interesting constructive characteristics which are visible in FIG. 9. The section along line IX-IX through the warhead 22 of FIG. 8 has an elongated parallelogram configuration comprising two cutting surfaces 24 defining a cutting edge 25 followed by a draft surface 26.
If the point is rotated in the direction of rotation represented by the arrow F, it is noted that the surfaces 25 define a clearance angle sufficiently large to allow the release of the chips obtained during the drilling of a bone by means of the 'warhead 22. On the other hand, a person skilled in the art will note, in FIG. 9, that the planar surfaces 24 define cutting surfaces with a negative cutting angle. To modify this cutting angle a and bring it to O or towards a positive value, two elongated recesses 27 have been provided placed adjacent to the cutting edge 25. Thus, the cutting angle a can be corrected and chosen according to the wishes of the user by modifying the depth of the recesses 27.
The point of fig. 8 and 9 with its pointed shape, its draft surfaces 26 and its recesses 27 bringing the cutting angle towards 0 or towards a positive value, makes it possible to perform very good drilling compared to the drilling plugs of the prior art. The point in the shape of a warhead is on the other hand particularly adequate, because it is plugged into the bone and makes it possible to perform a drilling along an axis without any lateral deviation.
The sheet which has just been described with reference to FIGS. 1 and 3 of the drawing has a total length which can vary between 75 and 200 mm for diameters ranging from 3 to 6 mm. The sheet shown according to the embodiment of FIGS. 2, 4 and 5, with a central threaded part, will have a total length ranging from 175 to 350 mm for diameters also ranging from 3 to 6 mm. The front part of the plug shown in fig. 2, consisting of the rod 16, will have a diameter slightly smaller than the diameter of the thread 6. For a plug of 3, respectively 4, 5 and 6 mm, the part 16 will have respectively 2, 3, 4 and 5 mm.
