Description

"Rotator cuff reinsertion system"
The present invention relates to a system for reinserting the rotator cuff.

The rotator cuff is a set of four muscles starting from the scapula and fitting at the level of the upper end of the humerus (trochiter and trochin).

These four muscles are the subscapularis, the supraspinatus, the subspinous and the small round. Their function is to ensure the stability of the humeral head during the various movements of the glenohumeral joint.

Traumatic or mechanical damage (friction friction) can lead to the rupture of one or more of these muscles, at the level of the tendons which insert them on the upper end of the humerus. These pathologies are characterized by pain and a significant functional deficit in the shoulder joint complex.

Repair, with the principle of reinserting the ruptured tendon (s), is possible when the affected structures have retained good mechanical integrity.

The principle of the treatment aims to reposition the injured tissue at its initial insertion. Once the tissue is repositioned, it will be held in place stably to allow it to heal and thus provide physiological repair.

In order to allow healing in good conditions, the reinsertion bone area is sharpened before the reinsertion of the injured tissue.

The devices used so far to ensure the reinsertion of the rotator cuff all have significant drawbacks.

It is known, for example, to use an osteosynthesis screw associated with a washer. This implies the need to prepare a type of drilling using a drill bit as well as the obligation to use a material to bring the retracted tendon into good position.

The passage of the screw through the tendon requires having previously pierced the latter. In addition, the screw head with its washer creates an excess thickness at the upper part of the tendon, causing friction conflicts with the adjacent structures.

It is also known to use a suture thread.

This entails the obligation to prepare transosseous tunnels. It is also difficult to pass the wires through these tunnels and to recover them. I1 also produces a "wire to cut the butter" effect under the effect of a tension in the bone of poor quality, causing a phenomenon of abrasion of the wire at the entrance and exit of the tunnels. To this must be added the difficulty of tensioning the wire and the possibility of shearing of the tendon by the wire. The mechanical resistance of the wire is also low.

It is also known to use an intraosseous implant, that is to say a more wire anchor system. This involves preparing the bone with a wick. Furthermore, the intraosseous implant cannot be removed. To these inherent drawbacks are added the drawbacks linked to the use of wire.

The object of the present invention is to provide a system for reinserting the rotator cuff which does not have the drawbacks of the systems used until now.

The invention meets this object and provides a system for reinserting the rotator cuff consisting of an implant consisting of a head with a conical hexagon, a neck, a threaded section and a distal part. brooch-shaped with diamond point (three sides).

la Figure 1 représente une vue schématique générale du système de réinsertion de la coiffe des rotateurs objet de l'invention;
la Figure 2 représente une vue en coupe transversale selon II de la Figure 1;
The invention will now be described in more detail with reference to the accompanying drawings in which
Figure 1 shows a general schematic view of the rotator cuff reinsertion system object of the invention;
Figure 2 shows a cross-sectional view along II of Figure 1;
Figure 3 shows a cross-sectional view along line III in Figure 1.

The implant 10 consists of a head 1 with a conical hexagon 2, connected to a neck 3 extending by a threaded section 4 and ending in a distal part 5 in the form of a pin with diamond point 6 (three faces) .

- tête 1 : diamètre 9 mm + 1 mm; épaisseur 0,80 mm 1,4 mm;
- six pans conique 2 : longueur d'entrée 1,5 mm 3,0 mm;
- col 3 : diamètre 3,5 mm - 4,5 mm; longueur 3,0 mm 6,4 mm;
- section filetée 4 : diamètre 3,5 mm - 4,5 mm; longueur 6,5 mm - 12,6 mm;
- partie distale 5 avec pointe diamant 6 : diamètre 1,8 mm - 2,2 mm; longueur 3 mm - 6,0 mm.
The dimensions of the implant according to the invention are as follows:
- head 1: diameter 9 mm + 1 mm; thickness 0.80 mm 1.4 mm;
- conical hexagon 2: inlet length 1.5 mm 3.0 mm;
- neck 3: diameter 3.5 mm - 4.5 mm; length 3.0 mm 6.4 mm;
- threaded section 4: diameter 3.5 mm - 4.5 mm; length 6.5 mm - 12.6 mm;
- distal part 5 with diamond tip 6: diameter 1.8 mm - 2.2 mm; length 3 mm - 6.0 mm.

According to an exemplary embodiment of the invention, the head 1 has a diameter of 9 mm and a thickness of 1 mm; the conical hexagon 2 has an entry length of 2.5 mm; the neck 3 has a diameter of 4.5 mm and a length of 4.5 mm; the threaded section 4 has a diameter of 4.5 mm and a length of 10.5 mm; the distal part 5 with diamond tip 6 has a diameter of 2 mm and a length of 5 mm.

Le matériau constitutif de l'implant selon l'invention est choisi parmi l'alliage au titane TA 6 V ELI et l'acier inoxydable 316 L. The material of the implant according to the invention is chosen from the titanium alloy TA 6 V ELI and 316 L stainless steel.

- l'implant 10 est positionné sur un tournevis, le six pans conique 2 et l'insertion à frottement dur assurent une parfaite stabilité au système;
- la zone osseuse de réinsertion ayant été avivée, on transfixe le tendon à réinsérer à l'aide de la partie distale en forme de broche 5 de l'implant;
- le tendon étant accroché à l'implant, on exerce une traction sur l'ensemble avec le tournevis pour l'amener au niveau de la zone de réinsertion. On stabilise le système en piquant l'os à l'aide de la pointe diamant 6 de la partie distale en forme de broche 5;
- on pénètre dans l'os en exerçant une pression sur l'implant avec le tournevis. Lorsque le filetage distal de la section filetée 4 arrive au contact de l'os, on commence à visser;
- on arrête le vissage quand le tendon est solidement maintenu entre l'os et la tête 1;
The technique for placing the implant according to the invention is as follows:
- The implant 10 is positioned on a screwdriver, the conical hexagon 2 and the hard friction insert ensure perfect stability to the system;
- the reinsertion bone area having been sharpened, the tendon to be reinserted is transfixed using the distal pin-shaped part 5 of the implant;
- the tendon being attached to the implant, a pull is exerted on the assembly with the screwdriver to bring it to the level of the reinsertion zone. The system is stabilized by pricking the bone using the diamond point 6 of the distal pin-shaped part 5;
- one enters the bone by exerting pressure on the implant with the screwdriver. When the distal thread of the threaded section 4 comes into contact with the bone, one begins to screw;
- We stop screwing when the tendon is firmly held between the bone and the head 1;
- care will be taken not to allow head 1 to enter the tendon, which could damage it.

Although the invention has been described with reference to a specific embodiment, it should be understood that variants can be envisaged by those skilled in the art while remaining within the scope of the present invention.
