Selbsttätig arbeitendes Blutdruckmessgerät Die Erfindung betrifft ein selbsttätig arbeitendes Blutdruckmessgerät mit einer aufblasbaren Abschnür manschette, mit einem Mikrophon zur Abnahme der Korotkoffimpulse sowie mit einer Einrichtung, die einen Manschettendruckzyklus in der Weise steuert, dass bei ansteigendem Manschettendruck in Abhängigkeit vom Auftreten der Korotkoffimpulse der diastolische Blut druckwert zur Anzeige gebracht, anschliessend der Manschettendruck bis über den geschätzten höchsten systolischen Blutdruck weiter ansteigt und dann langsam abgesenkt wird, wobei während der Absenkung, und zwar wiederum abhängig von dem Auftreten der Korot- koffimpulse, der systolische Blutdruck angezeigt wird,
worauf die Manschette selbsttätig entlüftet wird.
Bei selbsttätig arbeitenden Blutdruckmessgeräten dieser Art kann es vorkommen, dass bei ansteigendem Manschettendruck kein diastolischer Blutdruckwert ge messen wird, beispielsweise weil bei allgemeiner Kreis laufschwäche eines frisch operierten Patienten die vom Mikrophon abgegebenen Spannungsimpulse zu klein sind, um das Gerät auszulösen. Bei dem bekannten Ge rät wird die Abschnürmanschette in diesem Fall so weit aufgepumpt, bis ein oberhalb des geschätzten höchsten systolischen Blutdruckwertes ansprechender Maximal druckschalter anspricht, die Manschette entlüftet und alle Teile des Blutdruckmessgerätes wieder in den Ruhezustand bringt, von dem aus ein neuer Messzyklus erneut von Hand ausgelöst werden muss.
Dadurch wird einerseits im Falle des Ausbleibens einer diastolischen Messung der Manschettendruck unnötig weiter gestei gert (was den Patienten beunruhigt und wegen der Quet schung des abgeschnürten Gewebes die Ergebnisse nachfolgender Messungen verfälscht) und andererseits für die Einleitung eines neuen Messzyklus Zeit versäumt und eine ständige Überwachung des Sphygmomanorne- ters notwendig.
Es kann aber auch vorkommen, dass aus dem schon genannten Grund bei fallendem Manschettendruck kein systolischer Blutdruckwert gemessen wird. In einem sol chen Fall wird die bis über den geschätzten höchsten systolischen Blutdruck abgeblasene Manschette nur langsam entlüftet - weil während dieses Vorganges das Eintreffen des ersten Korotkofftones erwartet wird -, so dass der Arm des Patienten unnötig lang bei relativ hohem Manschettendruck eingeschnürt bleibt, was wie schon erwähnt - den Patienten belästigt, in der Folge zu Fehlmessungen führen kann und ausserdem die selbsttätige Einleitung eines neuen Messzyklus verzö gert.
Aus den genannten Gründen ist das bekannte Gerät nicht in erforderlichem Masse wartungsfrei und gegen Fehlmessungen nicht gesichert. Es ist Aufgabe der Er findung, diese Nachteile zu vermeiden. Dies ist erfin- dungsgemäss dadurch erreicht, dass zum Abbruch des Druckzyklus bei infolge einer Störung ausbleibender Druckmessung innerhalb eines Druckzyklus ein an das pneumatische System der Abschnürmanschette ange schlossener, auf einen im Bereich zwischen dem höch sten und niedrigsten zu erwartenden systolischen Blut druckwert liegenden Druckwert ansprechender pneuma tischer Schalter zur Öffnung des Druckablassventiles vorhanden ist in Verbindung mit einem von der Steuer einrichtung betätigten Kontaktsatz,
welcher bei stö- rungsfreiem Ablauf der Messvorgänge im Sinne einer Verhinderung der Öffnung des Druckablassventiles durch den pneumatischen Schalter wirksam wird.
Zweckmässig ist der Ansprechdruck des pneumati- schen Schalters auf einen Druckwert zwischen dem höchsten zu erwartenden diastolischen und dem höch sten zu erwartenden systolischen Blutdruckwert vor zugsweise auf<B>150</B> Torr eingestellt; dadurch wird ver mieden, dass die Manschette auch dann noch unnütz weiter aufgeblasen wird, wenn eine Messung des diasto- lischen Blutdruckwertes (wegen schon zu hohen Man schettendruckes) nicht mehr möglich ist.
Vorteilhaft ist der pneumatische Schalter als Minimaldruckschalter mit einem Ansprechwert, der unterhalb des niedrigsten zu erwartenden systolischen Blutdruckwertes liegt, vor zugsweise 60 Torr, ausgebildet und ihm ein Kontaktsatz zugeordnet, der die Wirkung des Schalters so lange ver- hindert, bis eine diastolische Messung erfolgt ist; dadurch wird vermieden, dass nach Ausfall einer systolischen Messung noch unnütz lange Druck in der Manschette aufrechterhalten wird.
Beim Ausführungsbeispiel sind Druckschalter so wohl für den einen wie auch für den anderen Fall (Aus fall der diastolischen bzw. systolischen Messung) vorge sehen, damit unnötige Belästigung des Patienten und Fehlerquellen für die Messung (Fehlmessungen können dadurch entstehen, dass das von der Manschette. unnötig lange und, stark zusammengedrückte Gewebe von seiner natürlichen Elastizität einbüsst) bei dem Gerät, das den diastolischen und den systolischen Blutdruckwert in einem Druckzyklus hintereinander misst, ausgeschieden sind.
Natürlich ist die Verwendung eines Minimaldruck schalters allein auch bei einem Gerät sinnvoll, das ledig lich den systolischen Blutdruckwert bei fallendem Man schettendruck misst.
Die Erfindung wird nachstehend anhand eines in der Zeichnung veranschaulichten Ausführungsbeispiels er läutert.
In den Arm 1 eines nicht mitgezeichneten Patienten sind die Abschnürmanschette 2 und der Pulstonabneh mer 3 angelegt. Der Pulstonabnehmer 3 verwandelt die an der eingeschnürten Arterie auftretenden Korotkoff- töne in Spannungsimpulse, die einem Verstärker zuge führt werden, der aus den Verstärkerstufen 4 und 6 mit den zugehörigen Mitteln 5 und 7 zur Einstellung des Verstärkungsfaktors dieser beiden Verstärkerstufen so wie dem auf einen bestimmten Ansprechwert eingestell ten Normimpulsgenerator 8 besteht.
Die in den Stufen 4 und 6 verstärkten Impulse werden - falls ihre Ampli tude grösser ist als der .eingestellte Ansprechwert vom Normimpulsgenerator in Einheitsimpulse einer be stimmten Amplitudenhöhe und einer bestimmten Zeit dauer (Rechteckimpulse) umgewandelt. Zu diesem Zweck enthält der Normimpulsgenerator einen mono stabilen Multivibrator. Die vom Normimpulsgenerator 8 abgegebenen elektrischen Impulse werden einem opti schen und einem akustischen Anzeigemittel 9 und 10 sowie der Steuereinheit 11 zugeführt.
Die Steuereinheit steht u. a. - wie mit den gestri chelten Linien angedeutet ist, deren Sinn sich aus der später folgenden Funktionsbeschreibung des Gerätes von selbst ergibt - in Verbindung mit dem Handschal ter 12, mit den Magnetventilen 13 und 14, den Schalt kontakten 15 und 16, einem weiteren Magnetventil 17 sowie einem Kontakt 18, über den der Motor 19 für den Kompressor 20 eingeschaltet werden kann. Der Kom pressor 20 erzeugt Druckluft, die über das Puffergefäss 21 und das Rückhalteventil 22 (welches durch die Kugel 47 und die Feder 48 symbolisiert ist) der gemeinsamen Druckleitung 23 -des pneumatischen Systems zugeführt wird und zum Aufblasen der Abschnürmanschette 2 dient.
An dieses pneumatische System sind pneumatische Schalter 24 und. 25 angeschlossen, die bei einenm be stimmten Druck in der Leitung 23 elektrische Kontakt sätze - symbolisiert durch 26 und 27 dargestellt - be tätigen, ferner steht die Leitung 23 - unter Einfügung der bereits erwähnten Magnetventile 13 und 14 - mit einem Manometer 28 für die Anzeige des diastolischen und einem Manometer 29 für die Anzeige des systoli- schen Blutdruckwertes in Verbindung.
Diesen Manome tern sind zur Fernanzeige Ferngeber zugeordnet, und zwar steht das Manometer 29 mit dem Widerstandsfern geber 30 in Verbindung, während das Manometer 28 mit dem induktiven Ferngeber 31 verbunden ist, der im Prinzip aus der Spule 32 besteht, deren Induktivität durch einen von der Membran 33 innerhalb der Spule verschiebbaren Eisenkern 34 in Abhängigkeit von dem am Manometer 28 anliegenden Druck verändert wird. An die Leitung 23 ist noch sein viertes Magnetventil 35 angeschlossen, welches ebenfalls mit der Steuereinheit 11 in Verbindung steht.
Die zu einer Baueinheit 5 zusammengefassten Mittel zur Einstellung des Verstärkungsgrades des Verstärkers bestehen aus einem Einstellpotentiometer 36 mit dem Schleifer 37, aus einem festen Spannungsteiler mit den Widerständen 38 und 39 sowie aus dem Umschalter 40, der die Verstärkerstufe 6 wahlweise an den Schaltpunkt 41 oder den Punkt 42 dieses Spannungsteilers legt.
Die mit dem Bezugszeichen 7 bezeichneten weiteren Mittel zur Änderung des Verstärkungsgrades des Verstärkers bestehen aus dem festen Spannungsteiler mit den Widerständen 43 und 44, dessen Teilungsverhältnis da durch geändert werden kann, dass mittels des Kontaktes 46, welcher von der Steuereinheit 11 betätigt wird, der Widerstand 45 dem Spannungsteilerwiderstand 44 parallel geschaltet werden kann.
Im folgenden soll nun erläutert werden, wie die Empfindlichkeit des Gerätes vor den eigentlichen Mes sungen an den jeweils zu untersuchenden Patienten an- gepasst wird. Nach Anlegen der Abschnürmanschette 2 und des Pulstonabnehmers 3 wird durch Druck auf den Handkontakt 12 das Gerät eingeschaltet und von der Einheit 11 der Kontakt 46 geöffnet. Der Schalter 40 bleibt in der gezeichneten Stellung. Die Steuereinheit 11 legt auch den Schalter 18 um, so dass der Motor 19 an läuft und der von ihm angetriebene Kompressor 20 die Abschnürmanschette 2 aufpumpt.
Im Laufe des Druck anstieges in der Manschette treten bei einem bestimm- ten Druck in der Arterie des Armes 1 die sog. Korot- kofftöne auf, welche im Pulstonabnehmer 3 in elektri sche Spannungspulsation umgeformt werden, die nach Verstärkung in den Stufen 4 und 6 den Normimpulsge- nerator 8 anstossen, der seinerseits bewirkt, dass die Anzeigemittel 9, 10 optische und elektrische Signale ab geben. Bei weiter steigendem Manschettendruck bleiben die Signale schliesslich wieder aus (d. h. die Arterie ist von der Manschette vollkommen abgeschnürt).
Nach dem letzten Signal schaltet das Gerät durch Öffnen des Schalters 18 den Motor 19 ab (die Abschaltung kann natürlich auch von Hand vorgenommen werden), so dass die Manschette 2 auch nicht mehr weiter aufgebla sen wird. Vielmehr wird nun über einen nicht dargestell ten Schalter der Steuereinheit 11 das Magnetventil (Druckablassventil) 35 geöffnet, so dass der im pneuma tischen System herrschende Luftdruck langsam abgebaut wird. Bei fallendem Manschettendruck treten die Signale an den Geräten 9, 10 wieder auf, und beim ersten auf tretenden Signal wird der Luftdruck im pneumatischen System abgelesen und das Ventil 35 nach einem weite ren Druckabfall von etwa 15 Torr geschlossen.
Wie eingangs schon erwähnt wurde, haben die bei etwa diesem Manschettendruck auftretenden Korotkoff- töne relativ grosse Amplituden und sind deshalb auch keinen merklichen respiratorischen Schwankungen un terworfen, so dass sie besonders gut als patientencha- rakteristisches Bezugsnormal geeignet sind.
Deshalb wird nun das Blutdruckmessgerät bei diesem Druck ge eicht, und zwar dadurch, dass bei geschlossenem Schal ter 46 und bei am Schaltpunkt 42 liegendem Kontakt 40 (geringe Verstärkung!) an den Verstärkereinstellmitteln 5 durch Verstellen des Schleifers 37 des Potentiometers 36 die Verstärkung so einreguliert wird, dass die Anzei gemittel 9, 10 gerade noch Signale abgeben. Auf diese Weise ist das Blutdruckmessgerät individuell an den jeweils untersuchten Patienten angepasst.
Da nun aber die Korotkofftöne beim diastolischen und beim systoli- schen Blutdruck wesentlich kleinere Amplituden aufwei sen als bei einem Manschettendruck, bei dem das Gerät geeicht wurde, würde das Gerät bei den interessierenden Drücken überhaupt nicht ansprechen.
Deshalb wird zu Beginn eines Messzyklus (wobei der diastolische Blut druck - wie bei dem bekannten Gerät - bei steigendem und der systolische Blutdruck bei fallendem Manschet tendruck gemessen wird), der ebenfalls durch Betätigen des Schalters 12 eingeleitet wird, auch der Schalter 40 (von Hand) wieder in die gezeichnete Stellung gebracht (während des Eichvorganges lag dagegen der Eingang der Verstärkerstufe 6 am Schaltpunkt 42; Kontakt 46 ist weiterhin geschlossen). Durch die Betätigung des Schal ters 40 beim Übergang beim Eichen zum eigentlichen Messvorgang liegt nun die gesamte am Potentiometer 37 abgegriffene Spannung am Eingang der Verstärkerstufe 6, so dass der Verstärkungsfaktor der gesamten Verstär keranordnung (beispielsweise um den Faktor 2) erhöht ist.
Dadurch ist der Tatsache Rechnung getragen, dass die Amplitude der Korotkofftöne beim diastolischen Blutdruck etwa um den Faktor 2 kleiner ist als die Amplituden der Korotkofftöne beim Eichdruck, der etwa 15 Torr unter dem systolischen Druck liegt.
Gesteuert von der Einheit 11 wird nach Betätigen des Schalters 12 der Manschettendruck erhöht, bis der erste den Normimpulsgenerator 8 anstossende Korot- koffton (beim diastolischen Druck) die Steuereinheit veranlasst, das Magnetventil 13 zu schliessen, so dass am Instrument 28 der diastolische Blutdruckwert ange zeigt wird. Die Manschette wird weiter aufgeblasen bis zu einem voreinstellbaren Wert, der mit Sicherheit über dem systolischen Blutdruckwert liegt. Wenn dieser Wert erreicht ist, schaltet die Einheit 11 den Motor 19 durch Öffnen des Kontaktes 18 ab, öffnet das Auslassventil 35 und öffnet ausserdem den Kontakt 46, wodurch eine weitere Erhöhung des Verstärkungsfaktors der Verstär kungsanordnung - beispielsweise wieder um einen Fak tor 2 - erreicht wird.
Auf diese Weise berücksichtigt das Blutdruckmessgerät automatisch, dass die Amplitu den der Korotkofftöne beim systolischen Blutdruck ebenfalls um einen Faktor von ungefähr 2 kleiner sind als die Amplituden der Korotkofftöne beim diastoli- schen Druck. Selbstverständlich stellen die angegebenen Faktoren nicht den Anspruch auf absolute Gültigkeit. Vielmehr kann es vorkommen, dass z. B. bei der über wachung von mit bestimmten Drogen narkotisierten Patienten wesentlich von den angegebenen Werten ab weichende Werte eingestellt werden müssen. Messreihen haben jedoch ergeben, dass im Normalfall die angegebe nen Werte zu sehr brauchbaren Ergebnissen führen.
Wenn der Druck in der Manschette wegen des geöff neten Ventils 35 so weit abgesunken ist, dass die Korot- kofftöne wieder auftreten (systolischer Blutdruck), schliesst die Steuereinheit 11 das Magnetventil 14, so dass am Instrument 29 der systolische Blutdruck ange zeigt bleibt, während durch Öffnen des Schnellablass- ventils 17 nach der systolischen Blutdruckmessung die Manschette wieder entlüftet wird. Vor Beginn eines neuen Messzyklus werden dann alle Schaltelemente wieder in ihren Ausgangszustand gebracht.
Bei der Messung eines Patienten kann es vorkom- men, dass wegen eines Kreislaufversagens oder einer ungünstigen Verschiebung des Pulstonabnehmers 3 während des Aufblasens der Manschette 2 keine diasto- lische Messung erfolgt. In einem solchen Fall würde die Manschette bis zu einem Höchstdruck (bei dem ein Überdruckventil anspricht) aufgeblasen werden, was einersetts den Patienten unnötig belästigen und anderer seits die von der Manschette eingeschnürten Organe derart quetschen würde, dass bei folgenden Messungen beträchtliche Messfehler auftreten könnten.
Aus diesem Grund ist der pneumatische Schalter 24 vorhanden, der bei einem bestimmten Druck, der mindestens dem höch sten vorkommenden diastolischen Druck entspricht aber unterhalb des höchsten systolischen Druckwertes liegt, beispielsweise bei 150 Torr, durch Öffnen des Kontakt satzes 26 das Schnellablassventil 17 öffnet, so dass sich das pneumatische System schnell entleeren kann. Gleichzeitig wird durch den Kontaktsatz 26 eine Rück stellung aller an dem Ablauf eines Messzyklus beteilig ten Schaltglieder, z. B. Relais, der Steuereinrichtung 11 auf den Ausgangszustand bewirkt, so dass das Gerät be reit ist, einen neuen Messzyklus zu durchlaufen.
Damit aber nach einer stattgefundenen normalen diastolischen Messung durch das Öffnen des Kontaktsatzes 26 kein unerwünschter Schnellablass des Manschettendruckes durch Öffnen des Magnetventiles 17 erfolgt, wird durch die Steuereinheit 11 nach Vornahme der diastolischen Messung automatisch der Kontakt 15 geschlossen und dadurch das Öffnen des Kontaktsatzes 26 beim einge stellten Druck (beispielsweise 150 Torr) unwirksam ge macht.
Aus ähnlichen Gründen - wie oben geschildert kann es auch vorkommen, dass bei fallendem Man schettendruck keine systolische Messung erfolgt, so dass der relativ hohe Manschettendruck über das geöffnete Ablassventil 35 nur sehr langsam abgebaut wird, wodurch die gleichen nachteiligen Wirkungen - wie oben angeführt ist - eintreten können. Deshalb ist ein zweiter pneumatischer Schalter 25 vorhanden, der auf einen Kontaktsatz 27 im Sinne einer Öffnung einwirkt, wenn bei einem Manschettendruck, der unterhalb des niedersten zu erwartenden systolischen (etwa 50 Torr) Blutdruckes liegt, keine systolische Messung erfolgt ist.
Mit dem Öffnen des Kontaktsatzes 27 wird wiederum das Schnellablassventil 17 geöffnet und eine Rückstel lung der an einem Messzyklus beteiligten Schaltglieder auf den Ausgangszustand bewirkt. Damit das Schnellab- lassventil 17 nicht bereits bei steigendem Manschetten druck zu Beginn eines Messzyklus, also beispielsweise unterhalb des d'iastolischen Druckwertes geöffnet ist (wodurch ein Aufblasen der Manschette verhindert würde), ist der Kontaktsatz 27 normalerweise durch den Kontakt 16 überbrückt, der erst nach einer erfolgten diastolischen Messung durch die Steuereinrichtung 11 geöffnet wird.
Die pneumatischen Schalter 24, 26 bzw. 25, 27 sind handelsüblicher Art, so dass von einer näheren Darstel lung abgesehen wird. Die Kontaktsätze 26 und 27 kön nen auch durch einen gemeinsamen Stellmotor betätigt werden.
Das Rückhalteventil 22 arbeitet im Rahmen der ge samten Pneumatik wie folgt: Beim Schnellablass des Manschettendruckes durch das Magnetventil 17 am Ende eines Messzyklus entleeren sich sowohl die Man schette 2 als auch das Ausgleichsgefäss 22 zunächst ge meinsam bis zu einem Druck von etwa 40-50 mm./Hg. Für diesen Wert ist die Feder 48 bemessen, welche die Ventilkugel 47 gegen ihren Ventilsiz drückt. Demzu folge wird beim weiteren Absinken des in der Man schette 2 und der Pneumatikleitung 23 herrschenden Druckes das im Ausgleichsgefäss 21 enthaltene Luftvo lumen durch das Rückhalteventil zurückgehalten.
Da durch wird erreicht, dass am Beginn des nächsten Mess- zyklus bis zu dem Wert von 40-50 mm/Hg nur die Manschette mit dem Leitungssystem 23 aufgepumpt zu werden braucht, so dass die Zeit für die Füllung des ver gleichsweise grossen Puffergefässes 21 eingespart wird.

With automatic blood pressure measuring devices of this type it can happen that when the cuff pressure increases, no diastolic blood pressure value is measured, for example because the voltage pulses emitted by the microphone are too small to trigger the device in the case of general circulatory weakness in a recently operated patient. In the case of the known device, the constriction cuff is inflated until a maximum pressure switch above the estimated highest systolic blood pressure value responds, the cuff is vented and all parts of the blood pressure monitor are in the idle state, from which a new measuring cycle starts again Hand must be raised.
As a result, if a diastolic measurement is not carried out, the cuff pressure is unnecessarily increased (which worries the patient and falsifies the results of subsequent measurements due to the squeezing of the constricted tissue) and, on the other hand, there is no time for initiating a new measurement cycle and constant monitoring of the Sphygmomanornters necessary.
However, it can also happen that, for the reason already mentioned, no systolic blood pressure value is measured when the cuff pressure falls. In such a case, the cuff that has been deflated above the estimated maximum systolic blood pressure is only deflated slowly - because the first Korotkoff sound is expected during this process - so that the patient's arm remains constricted for an unnecessarily long time at a relatively high cuff pressure, which is how already mentioned - annoys the patient, can lead to incorrect measurements as a result and also delays the automatic initiation of a new measuring cycle.
For the reasons mentioned, the known device is not maintenance-free to the required extent and is not secured against incorrect measurements. It is the task of the invention to avoid these disadvantages. This is achieved according to the invention in that, to abort the pressure cycle in the event of a failure to measure the pressure within a pressure cycle, a pressure value connected to the pneumatic system of the constriction cuff is more responsive to a pressure value lying in the range between the highest and lowest expected systolic blood pressure value Pneumatic switch for opening the pressure relief valve is available in connection with a contact set actuated by the control device,

The response pressure of the pneumatic switch is expediently set to a pressure value between the highest expected diastolic and the highest expected systolic blood pressure value, preferably to <B> 150 </B> Torr; This prevents the cuff from being inflated further unnecessarily if it is no longer possible to measure the diastolic blood pressure value (because the cuff pressure is already too high).
The pneumatic switch is advantageously designed as a minimum pressure switch with a response value that is below the lowest expected systolic blood pressure value, preferably 60 Torr, and is assigned a contact set that prevents the switch from working until a diastolic measurement has taken place ; this avoids maintaining pressure in the cuff for an unnecessarily long time after a systolic measurement fails.
In the exemplary embodiment, pressure switches are provided for one case as well as for the other (failure of the diastolic or systolic measurement), so that unnecessary annoyance for the patient and sources of error for the measurement (incorrect measurements can arise because the cuff. unnecessarily long and strongly compressed tissue loses its natural elasticity) are eliminated from the device that measures the diastolic and systolic blood pressure values in one pressure cycle one after the other.
Of course, the use of a minimum pressure switch alone also makes sense with a device that only measures the systolic blood pressure value when the cuff pressure falls.
The invention is explained below with reference to an embodiment illustrated in the drawing.

The pulses amplified in stages 4 and 6 are - if their amplitude is greater than the set response value - converted by the standard pulse generator into unit pulses of a certain amplitude and a certain period of time (square-wave pulses). For this purpose, the standard pulse generator contains a monostable multivibrator. The electrical pulses emitted by the standard pulse generator 8 are fed to an optical and acoustic display means 9 and 10 and the control unit 11.
The control unit is u. a. - As indicated by the dashed lines, the sense of which is evident from the functional description of the device which follows later - in connection with the manual switch 12, with the solenoid valves 13 and 14, the switching contacts 15 and 16, another solenoid valve 17 and a contact 18 via which the motor 19 for the compressor 20 can be switched on. The compressor 20 generates compressed air which is supplied to the common pressure line 23 of the pneumatic system via the buffer vessel 21 and the retaining valve 22 (which is symbolized by the ball 47 and the spring 48) and is used to inflate the constriction cuff 2.

The further means denoted by the reference numeral 7 for changing the gain of the amplifier consist of the fixed voltage divider with the resistors 43 and 44, the division ratio of which can be changed by means of the contact 46, which is actuated by the control unit 11, the resistor 45 the voltage divider resistor 44 can be connected in parallel.
The following explains how the sensitivity of the device is adjusted to the patient to be examined before the actual measurements. After the constriction cuff 2 and the pulse sound pick-up 3 have been applied, the device is switched on by pressing the hand contact 12 and the contact 46 is opened by the unit 11. The switch 40 remains in the position shown. The control unit 11 also flips the switch 18 so that the motor 19 starts up and the compressor 20 driven by it inflates the constriction cuff 2.
In the course of the pressure increase in the cuff, at a certain pressure in the artery of the arm 1, the so-called Korotkoff tones occur, which are converted into electrical voltage pulsations in the pulse pickup 3, which are amplified in stages 4 and 6 Trigger the standard pulse generator 8, which in turn causes the display means 9, 10 to emit optical and electrical signals. If the cuff pressure continues to rise, the signals ultimately fail again (i.e. the artery is completely constricted from the cuff).
After the last signal, the device switches off the motor 19 by opening the switch 18 (it can of course also be switched off manually), so that the cuff 2 is no longer inflated. Rather, the solenoid valve (pressure relief valve) 35 is now opened via a switch, not shown, of the control unit 11, so that the air pressure prevailing in the pneumatic system is slowly reduced. When the cuff pressure falls, the signals on the devices 9, 10 reappear, and when the first signal occurs, the air pressure in the pneumatic system is read and the valve 35 is closed after a further pressure drop of about 15 Torr.
As already mentioned at the beginning, the Korotkoff tones that occur at about this cuff pressure have relatively large amplitudes and are therefore not subject to any noticeable respiratory fluctuations, so that they are particularly suitable as patient-characteristic reference standards.
Therefore, the sphygmomanometer is now calibrated at this pressure, in that with the switch 46 closed and with the contact 40 (low gain!) At the switch point 42 on the amplifier setting means 5 by adjusting the slider 37 of the potentiometer 36, the gain is adjusted is that the display means 9, 10 are just emitting signals. But since the Korotkoff tones with diastolic and systolic blood pressure have significantly smaller amplitudes than with a cuff pressure at which the device was calibrated, the device would not respond at all at the pressures of interest.
Therefore, at the beginning of a measuring cycle (whereby the diastolic blood pressure - as in the known device - is measured when the cuff pressure rises and the systolic blood pressure when the cuff pressure falls), which is also initiated by actuating the switch 12, the switch 40 (by hand ) brought back into the position shown (during the calibration process, however, the input of amplifier stage 6 was at switching point 42; contact 46 is still closed). By actuating the switch 40 during the transition during calibration to the actual measuring process, the entire voltage tapped at the potentiometer 37 is now at the input of the amplifier stage 6, so that the gain factor of the entire amplifier arrangement is increased (for example by a factor of 2).
This takes account of the fact that the amplitude of the Korotkoff tones in diastolic blood pressure is about a factor of 2 smaller than the amplitudes of the Korotkoff tones in the calibration pressure, which is about 15 Torr below the systolic pressure.
Controlled by the unit 11, after the switch 12 has been actuated, the cuff pressure is increased until the first Korotkoff tone (at diastolic pressure) to trigger the standard pulse generator 8 causes the control unit to close the solenoid valve 13 so that the diastolic blood pressure value is displayed on the instrument 28 becomes. The cuff is inflated further up to a presettable value which is definitely above the systolic blood pressure value. When this value is reached, the unit 11 switches off the motor 19 by opening the contact 18, opens the outlet valve 35 and also opens the contact 46, whereby a further increase in the amplification factor of the amplification arrangement - for example again by a factor 2 - is achieved becomes.
In this way, the blood pressure monitor automatically takes into account that the amplitudes of the Korotkoff tones for systolic blood pressure are also smaller by a factor of approximately 2 than the amplitudes of the Korotkoffs tones for diastolic pressure. Of course, the specified factors do not claim to be absolutely valid. Rather, it can happen that z. B. in the monitoring of patients anesthetized with certain drugs must be set significantly deviating from the specified values. However, series of measurements have shown that the specified values normally lead to very useful results.
When the pressure in the cuff has dropped so far because of the opened valve 35 that the Korotkoff tones occur again (systolic blood pressure), the control unit 11 closes the solenoid valve 14 so that the systolic blood pressure remains displayed on the instrument 29 while by opening the quick release valve 17 after the systolic blood pressure measurement, the cuff is deflated again. Before the start of a new measuring cycle, all switching elements are then returned to their initial state.
When measuring a patient, it can happen that no diastolic measurement is made due to a circulatory failure or an unfavorable displacement of the pulse pickup 3 while the cuff 2 is being inflated. In such a case, the cuff would be inflated to a maximum pressure (at which a pressure relief valve responds), which on the one hand unnecessarily annoy the patient and on the other hand would squeeze the organs constricted by the cuff in such a way that considerable measurement errors could occur in subsequent measurements.
For this reason, the pneumatic switch 24 is available, which at a certain pressure, which corresponds to at least the highest occurring diastolic pressure but is below the highest systolic pressure value, for example at 150 Torr, by opening the contact set 26 opens the quick release valve 17, so that the pneumatic system can drain quickly. At the same time through the contact set 26 a return position of all participating th switching elements in the course of a measuring cycle, z. B. Relay causes the control device 11 to the initial state so that the device is ready to go through a new measurement cycle.
But so that after a normal diastolic measurement has taken place by opening the contact set 26 there is no undesired rapid release of the cuff pressure by opening the solenoid valve 17, the control unit 11 automatically closes the contact 15 after the diastolic measurement has been carried out, thereby opening the contact set 26 when the put pressure (e.g. 150 Torr) ineffective ge.
For similar reasons - as described above, it can also happen that when the cuff pressure falls, there is no systolic measurement, so that the relatively high cuff pressure is only reduced very slowly via the open drain valve 35, whereby the same disadvantageous effects - as stated above - can enter. There is therefore a second pneumatic switch 25 which acts on a contact set 27 in the sense of an opening if no systolic measurement has been made at a cuff pressure below the lowest expected systolic (about 50 Torr) blood pressure.
When the contact set 27 is opened, the quick release valve 17 is in turn opened and the switching elements involved in a measuring cycle are reset to the initial state. So that the quick release valve 17 is not already opened when the cuff pressure rises at the beginning of a measuring cycle, for example below the d'iastolic pressure value (which would prevent the cuff from inflating), the contact set 27 is normally bridged by the contact 16, which is opened by the control device 11 after a diastolic measurement has taken place.
The pneumatic switches 24, 26 and 25, 27 are of a commercial type, so that a closer presen- tation is not required. The contact sets 26 and 27 can also be operated by a common servomotor.
The restraint valve 22 works as part of the overall pneumatic system: When the cuff pressure is rapidly released by the solenoid valve 17 at the end of a measuring cycle, both the cuff 2 and the expansion tank 22 are initially emptied together to a pressure of around 40-50 mm ./Hg. The spring 48, which presses the valve ball 47 against its valve seat, is dimensioned for this value.
