Lit d'hôpital
La présente invention a pour objet un lit d'hôpital pour le traitement de patients à des pressions autres que la pression atmosphérique. Ce lit est caractérisé en ce qu'il comprend une partie supérieure et une partie inférieure, et des moyens pour joindre la partie supérieure de façon étanche à la partie inférieure, les parties lorsqu'elles sont jointes ensemble de façon étanche formant une chambre fermée dans laquelle le patient peut être traité, la partie supérieure présentant une partie transparente à travers laquelle le patient enfermé de façon étanche dans le lit peut voir au dehors et une entrée traversant l'une des parties étant disposée pour l'introduction de gaz dans ladite chambre.

Le lit d'hôpital présente une partie inférieure 1 en forme de cuvette allongée peu profonde dans laquelle peut être logé un matelas pour recevoir le patient à traiter. La partie inférieure 1 est montée à pivot, autour d'un axe horizontal transversal 2 situé au ou près du milieu du lit, sur un berceau 3, présentant en plan substantiellement la forme d'un H, muni de roulettes 4 afin de permettre de déplacer facilement le lit en entier. Le montage de la partie inférieure 1 sur le berceau 3 est effectué au moyen de tourillons 5 portés par des paliers 6 aux sommets de supports 7 disposés l'un de chaque côté du berceau 3. L'inclinaison de la partie inférieure 1 par rapport au berceau 3 peut être réglée par un vérin hydraulique 8 reliant la partie centrale du berceau 3 au côté inférieur de la partie inférieure 1.
Le vérin 8 est incliné sur l'horizontale et présente un bras 9 relié à pivot à une saillie 10 du côté inférieur de la partie inférieure 1 et un cylindre 11 relié à pivot à une saillie 12 d'une traverse centrale 13 du berceau 3. Le fluide pour le vérin 8 est fourni au cylindre 11 à travers un tuyau flexible 14 par une pompe 15 montée à l'extérieur du berceau 3. La pompe 15 présente une poignée de manoeuvre 16 et un cylindre à fluide 17. Le vérin 8 et la résistance de frottement exercée par les paliers 6 sur les tourillons 5 suffisent pour maintenir la partie inférieure 1 dans toute position inclinée désirée, dès que la pompe n'est plus actionnée, sans qu'un dispositif de blocage soit nécessaire.
Le lit comprend également une partie supérieure 18 qui est en forme de cuvette allongée peu profonde inversée, complémentaire de la partie inférieure, qui est pourvue à une extrémité d'un dôme transparent 19. La partie supérieure 18 et le dôme 19 sont conformés de manière à permettre au patient de prendre une position couchée lorsque les parties sont fermées l'une sur l'autre, la tête du patient se trouvant dans la partie inférieure du dôme de façon que le patient puisse voir au-dehors. La partie supérieure 18 est articulée à la partie inférieure 1 à l'ex trémité éloignée du dôme 19 autour d'un axe 20 parallèle à l'axe 2.
Le dispositif d'articulation comprend deux bras 21 s'étendant longitudinalement fixés à la partie supérieure 18 I'un de chaque côté du lit par des consoles 22 et s'étendant à partir du dôme 19 pour surplomber le pied du lit par des joues 23 s'étendant vers le bas. La partie inférieure 1 présente un support formé de deux bras 24 dont les extrémités sont adjacentes à et disposées à l'intérieur des joues 23 et sont reliées par une plaque incurvée 23, centrée sur l'axe 20, s'étendant en travers du pied du lit. Une charnière ou fusée 26, dont l'axe est l'axe 20, s'étend, à travers les joues 23 et les bras 24, et une tige 27, parallèle à l'axe 20 et ayant un axe qui se trouve dans un plan tangent à une partie intermédiaire de la plaque 25 lorsque les parties 1 et 18 sont fermées ensemble, relie les bras 21 audessus de l'extrémité de la partie supérieure 18 au pied du lit.
Des ressorts à lame 28 ont chacun une extrémité fixée au bord inférieur de la plaque 25 et l'autre extrémité accrochée autour de la tige 27.
Ces ressorts 28 sont disposés en une série en travers du lit - on peut par exemple en disposer douze - et un couvercle 29 articulé autour de la tige 27 les cache normalement. Un bras 30 pivoté à une extrémité sur l'un des bras 21 a son autre extrémité formée avec une fente dans laquelle s'engage un goujon 31 porté par le bras 24 adjacent afin de limiter le mouvement de basculement vers le haut de la partie supérieure 18.
Une courroie sans fin 32 est disposée autour du bord de la partie inférieure 1 et présente une partie 33 s'étendant le long de chacun des deux côtés de la partie inférieure 1 et une partie d'extrémité courbe 34 s'étendant autour de chacune des extrémités de la partie inférieure 1, chaque paire de parties adjacentes 33, 34 de la courroie étant reliée par une boucle 35. Une extrémité d'une des parties latérales 33 de la courroie porte un bossage faisant saillie à l'extérieur 36 formé avec un alésage horizontal fileté intérieurement. Une tige 37, filetée à une extrémité est portée de manière à pouvoir tourner librement mais sans mouvement axial par une console 38 fixée à la partie inférieure 1 et a son extrémité filetée engagée dans l'alésage du bossage 36.
La tige 37 peut être entraînée en rotation par une manivelle 39 agissant par l'intermédiaire d'un engrenage d'angle (non représenté) logé dans un carter 40 fixé à la partie inférieure 1. La rotation de la manivelle 39 provoque, par l'effet de vissage de la tige 37 dans le bossage 36, le déplacement de la courroie 32 autour du bord de la partie inférieure 1.
Le bossage 36, la tige 37 et la console 38 sont cachés par un prolongement du carter 40, ce prolongement présentant sur sa surface supérieure deux repères avec lesquels coopère un repère sur une oreille 41 portée par la courroie 32 afin d'indiquer le verrouillage et le < x déverrouillage , comme expliqué ci-dessous. Chaque partie 33, 34 de la courroie 32 a une section transversale en forme de U avec les branches du U dirigées horizontalement et s 'étendant vers l'intérieur par rapport au lit. La branche horizontale supérieure de chaque partie a son bord intérieur crénelé. Le bord de la partie inférieure 1 du lit porte une nervure 42 dirigée vers l'extérieur, sur laquelle sont formées des oreilles 43 espacées à intervalles réguliers autour du lit.
Les créneaux de la courroie 32 recouvrent les oreilles 43, en engageant les côtés des oreilles 43 faisant face à la partie supérieure 18, lorsque la courroie 32 est en position de déverrouillage par rapport à la partie inférieure 1. La branche horizontale inférieure de chaque partie de courroie 33, 34 est engagée sous la nervure 42, c'est-à-dire engage le côté de celle-ci faisant face à l'opposé de la partie supérieure 18. Le bord de la partie supérieure du lit est également pourvu d'oreilles 44 qui sont complémentaires des oreilles 43 de la partie inférieure et qui se déplacent pour s'engager entre ces dernières lorsque les deux parties 1 et 18 sont fermées ensemble avec la courroie 32 dans ladite position de déverrouillage.
La partie inférieure 1 du lit présente autour de son bord immédiatement à l'intérieur des oreilles 43 une rainure peu profonde 45 dans laquelle est disposé un organe d'étanchéité formé par un anneau O 46 qui fait saillie de la rainure 45. Lorsque les deux parties sont fermées ensemble, la courroie 32 est libre de se déplacer autour des bords lors de la rotation de la manivelle 39 de manière que les créneaux recouvrent ou engagent au moins en partie les côtés des oreilles 44 faisant face à l'opposé de la partie inférieure 1 dans la position verrouillée j > de la courroie 32, le déplacement progressif de la courroie 32 de ladite position déverrouillée forçant les oreilles 44 de la partie supérieure 18 progressivement plus fort contre la nervure 42 de la partie inférieure 1,
en serrant ainsi ensemble les deux parties 1 et i 8 pour former une chambre étanche.
Un dispositif avertisseur (fig. 7) est prévu pour éviter que le lit soit fermé lorsque la courroie 32 se trouve par inadvertance dans la position de verrouillage. Ce dispositif comprend une lumière clignoteuse 47 portée par le carter 40 et montée en série avec un micro-interrupteur 48, monté sur l'un des bras 21 (fig. 1) et agencé pour être actionné par contact avec la courroie 32 lorsqu'on ferme le lit, un autre microinterrupteur 49 fixé à la partie inférieure 1 et agencé pour être actionné par un bloc 50 porté par la courroie 32, et une batterie 51. L'interrupteur 48 est fermé lorsque la partie supérieure 1 est soulevée et l'interrupteur 49 est fermé lorsque la courroie 32 est en position de verrouillage de sorte que pour cette condition du lit la lampe avertisseuse 47 se met à clignoter.
Des tuyaux d'admission et d'échappement 52, 53 pour du gaz, par exemple de l'oxygène, dans lequel le patient doit être traité, mènent à et de la partie inférieure 1. Le tuyau d'admission bifurque en 52A, 52B et mène à l'intérieur du lit derrière un dossier 54 pour le patient. Le tuyau d'échappement 53 se termine par une queue de poisson 55 débouchant à l'intérieur de lit, à proximité du pied de celui-ci, mais sort du lit près du tuyau d'admission 52. Les tuyaux 52, 53 sont flexibles à l'extérieur du lit et mènent à un coffret mobile (non représenté) qui contient des commandes et des instruments indicateurs au moyen desquels un surveillant peut régler ou contrôler la pression, l'humidité, la température et la pureté du gaz contenu dans la chambre étanche formée par les parties inférieure et supérieure serrées ensemble.
Le lit peut faire partie d'un circuit fermé, les gaz d'échappement venant du tuyau 53 étant reconditionnés et renvoyés par le tuyau 52. Des fenêtres 56 sont prévues dans la partie supérieure 18 afin de permettre à un surveillant d'observer des parties du corps du patient subissant un traitement, et les deux parties du lit sont de construction rigide précontrainte afin de pouvoir résister aux différences de pression entre l'intérieur de la chambre étanche et l'atmosphère. La partie supérieure 18 du lit est de construction à double paroi afin d'assurer sa légèreté tout en conservant sa robustesse, et une poignée 57 est fixée sur un côté de la partie supérieure 18 au-dessous du dôme 19 afin de faciliter l'ouverture et la fermeture du lit.
La pompe 15, la manivelle 39 et la poignée 57 sont toutes disposées pour plus de commodité à l'extrémité du lit présentant le dôme, et du même côté du lit.
Lors de l'utilisation du lit, le patient est couché sur un matelas disposé dans la partie inférieure 1 de manière que sa tête se trouve à un endroit venant au-dessous du dôme transparent. On comprendra que l'emploi du dossier 54 est facultatif. Les ressorts 28 maintiennent la partie supérieure 18 dans sa position soulevée extrême définie par le bras 30 pendant que le patient est placé dans le lit, mais le couple en excès exercé par les ressorts 28 à l'encontre du couple dû au poids de la partie supérieure 18 est suffisamment faible pour permettre que cette dernière soit facilement rabattue au moyen de la poignée 57. Lorsque le lit est presque fermé, le poids de la partie supérieure 18 surmonte le couple exercé par les ressorts 28 et le lit se fermera sans autre aide.
La partie supérieure 18 est alors scellée à la partie inférieure en agissant sur la manivelle 39 qui déplace la courroie 32 à la position de verrouillage, et du gaz est introduit dans la chambre fermée par le tuyau d'admission 52 et s'échappe par le tuyau 53. L'inclinaison du lit peut être modifiée selon les besoins en faisant fonctionner le vérin 8 au moyen de la pompe 15. Le traitement du patient peut avoir lieu à une pression sous-atmosphérique ou à une pression supérieure à l'atmosphérique. La pression supérieure à la pression atmosphérique peut aller jusqu'à deux ou trois atmosphères.
Lorsque l'on cesse le traitement, la pression dans la chambre est ramenée à la pression atmosphérique et le lit est ouvert par des opérations correspondantes inverses de celles nécessaires pour fermer le lit.
Un équipement d'intercommunication (non représenté) peut être commodément prévu afin de permettre au surveillant et au patient de communiquer lorsque le patient est scellé à l'intérieur de la chambre. Dans des agencement alternatifs (non représentés) la partie supérieure du lit pourrait être articulée à la partie inférieure autour d'un axe s'étendant le long du côté du lit ou bien la partie supérieure peut être agencée de manière qu'elle puisse être soulevée et enlevée entièrement de la partie inférieure.
Hospital bed

The hospital bed has a lower part 1 in the form of a shallow elongated bowl in which a mattress can be accommodated for receiving the patient to be treated. The lower part 1 is pivotally mounted, around a transverse horizontal axis 2 located at or near the middle of the bed, on a cradle 3, having in plan substantially the shape of an H, provided with wheels 4 in order to allow easily move the entire bed. The mounting of the lower part 1 on the cradle 3 is carried out by means of journals 5 carried by bearings 6 at the tops of supports 7 arranged one on each side of the cradle 3. The inclination of the lower part 1 with respect to the cradle 3 can be adjusted by a hydraulic cylinder 8 connecting the central part of the cradle 3 to the lower side of the lower part 1.
The cylinder 8 is inclined to the horizontal and has an arm 9 pivotally connected to a projection 10 on the lower side of the lower part 1 and a cylinder 11 pivotally connected to a projection 12 of a central cross member 13 of the cradle 3. The fluid for the cylinder 8 is supplied to the cylinder 11 through a flexible pipe 14 by a pump 15 mounted outside the cradle 3. The pump 15 has an operating handle 16 and a fluid cylinder 17. The cylinder 8 and the frictional resistance exerted by the bearings 6 on the journals 5 are sufficient to maintain the lower part 1 in any desired inclined position, as soon as the pump is no longer actuated, without a locking device being necessary.
The bed also includes an upper portion 18 which is in the shape of an inverted shallow elongated bowl, complementary to the lower portion, which is provided at one end with a transparent dome 19. The upper portion 18 and dome 19 are conformably shaped. allowing the patient to assume a supine position when the parts are closed on top of each other, with the patient's head located in the lower part of the dome so that the patient can see out. The upper part 18 is articulated to the lower part 1 at the end remote from the dome 19 about an axis 20 parallel to the axis 2.
The articulation device comprises two arms 21 extending longitudinally fixed to the upper part 18, one on each side of the bed by consoles 22 and extending from the dome 19 to overhang the foot of the bed by cheeks 23. extending down. The lower part 1 has a support formed of two arms 24, the ends of which are adjacent to and arranged inside the cheeks 23 and are connected by a curved plate 23, centered on the axis 20, extending across the foot bed. A hinge or rocket 26, the axis of which is the axis 20, extends, through the cheeks 23 and the arms 24, and a rod 27, parallel to the axis 20 and having an axis which lies in a plane tangent to an intermediate part of the plate 25 when the parts 1 and 18 are closed together, connects the arms 21 above the end of the upper part 18 at the foot of the bed.
Leaf springs 28 each have one end attached to the lower edge of plate 25 and the other end hooked around rod 27.
These springs 28 are arranged in a series across the bed - for example twelve can be arranged - and a cover 29 articulated around the rod 27 normally hides them. An arm 30 pivoted at one end on one of the arms 21 has its other end formed with a slot into which engages a stud 31 carried by the adjacent arm 24 in order to limit the upward tilting movement of the upper part. 18.
An endless belt 32 is disposed around the edge of the lower part 1 and has a part 33 extending along each of the two sides of the lower part 1 and a curved end part 34 extending around each of the sides. ends of the lower part 1, each pair of adjacent parts 33, 34 of the belt being connected by a loop 35. One end of one of the side parts 33 of the belt has an outwardly projecting boss 36 formed with a internally threaded horizontal bore. A rod 37, threaded at one end, is carried so as to be able to rotate freely but without axial movement by a console 38 fixed to the lower part 1 and has its threaded end engaged in the bore of the boss 36.
The rod 37 can be driven in rotation by a crank 39 acting via an angle gear (not shown) housed in a housing 40 fixed to the lower part 1. The rotation of the crank 39 causes, by the The effect of screwing the rod 37 into the boss 36, the movement of the belt 32 around the edge of the lower part 1.
The boss 36, the rod 37 and the console 38 are hidden by an extension of the housing 40, this extension having on its upper surface two marks with which a mark cooperates on a lug 41 carried by the belt 32 in order to indicate the locking and the <x unlocking, as explained below. Each portion 33, 34 of the belt 32 has a U-shaped cross section with the legs of the U directed horizontally and extending inwardly of the bed. The upper horizontal branch of each part has its crenellated inner edge. The edge of the lower part 1 of the bed carries a rib 42 directed outwards, on which are formed ears 43 spaced at regular intervals around the bed.
The slots of the strap 32 cover the ears 43, engaging the sides of the ears 43 facing the upper part 18, when the belt 32 is in the unlocked position relative to the lower part 1. The lower horizontal branch of each part 33, 34 is engaged under the rib 42, that is to say engages the side thereof facing away from the upper part 18. The edge of the upper part of the bed is also provided with 'ears 44 which are complementary to the ears 43 of the lower part and which move to engage between the latter when the two parts 1 and 18 are closed together with the strap 32 in said unlocked position.
The lower part 1 of the bed has around its edge immediately inside the ears 43 a shallow groove 45 in which is disposed a sealing member formed by an O-ring 46 which protrudes from the groove 45. When both parts are closed together, the strap 32 is free to move around the edges upon rotation of the crank 39 so that the slots overlap or at least partially engage the sides of the ears 44 facing away from the part. lower 1 in the locked position j> of the belt 32, the progressive displacement of the belt 32 from said unlocked position forcing the ears 44 of the upper part 18 progressively stronger against the rib 42 of the lower part 1,

A warning device (Fig. 7) is provided to prevent the bed from being closed when the belt 32 is inadvertently in the locked position. This device comprises a flashing light 47 carried by the housing 40 and mounted in series with a microswitch 48, mounted on one of the arms 21 (Fig. 1) and arranged to be actuated by contact with the belt 32 when closes the bed, another microswitch 49 fixed to the lower part 1 and arranged to be actuated by a block 50 carried by the belt 32, and a battery 51. The switch 48 is closed when the upper part 1 is lifted and the switch 49 is closed when the belt 32 is in the locked position so that for this condition of the bed the warning lamp 47 starts flashing.
Intake and exhaust pipes 52, 53 for gas, for example oxygen, in which the patient is to be treated, lead to and from the lower part 1. The intake pipe branches off at 52A, 52B and leads inside the bed behind a backrest 54 for the patient. The exhaust pipe 53 ends with a fishtail 55 opening inside the bed, near the foot of the latter, but exits the bed near the intake pipe 52. The pipes 52, 53 are flexible. outside the bed and lead to a mobile cabinet (not shown) which contains controls and indicating instruments by means of which a supervisor can adjust or control the pressure, humidity, temperature and purity of the gas in the bed. sealed chamber formed by the lower and upper parts clamped together.
The bed may be part of a closed circuit, with the exhaust gases coming from the pipe 53 being reconditioned and returned through the pipe 52. Windows 56 are provided in the upper part 18 to allow an overseer to observe parts. of the body of the patient undergoing treatment, and both parts of the bed are of rigid pre-stressed construction so as to be able to withstand the pressure differences between the interior of the sealed chamber and the atmosphere. The upper part 18 of the bed is of double-walled construction to ensure its lightness while maintaining its sturdiness, and a handle 57 is fixed to one side of the upper part 18 below the dome 19 to facilitate opening. and closing the bed.
Pump 15, crank 39, and handle 57 are all located for convenience at the end of the dome bed, and on the same side of the bed.
When using the bed, the patient is lying on a mattress arranged in the lower part 1 so that his head is at a place coming below the transparent dome. It will be understood that the use of the file 54 is optional. The springs 28 keep the upper part 18 in its extreme raised position defined by the arm 30 while the patient is placed in the bed, but the excess torque exerted by the springs 28 against the torque due to the weight of the part. top 18 is small enough to allow the latter to be easily folded down by means of the handle 57. When the bed is almost closed, the weight of the top 18 overcomes the torque exerted by the springs 28 and the bed will close without further assistance. .
The upper part 18 is then sealed to the lower part by acting on the crank 39 which moves the belt 32 to the locking position, and gas is introduced into the closed chamber by the inlet pipe 52 and escapes through the hose 53. The inclination of the bed can be changed as needed by operating the cylinder 8 by means of the pump 15. The treatment of the patient can take place at sub-atmospheric pressure or at a pressure above the atmosphere. The pressure above atmospheric pressure can be up to two or three atmospheres.
When the treatment is ceased, the pressure in the chamber is brought back to atmospheric pressure and the bed is opened by corresponding operations reverse to those necessary to close the bed.
Intercom equipment (not shown) can be conveniently provided to allow the supervisor and the patient to communicate when the patient is sealed within the chamber. In alternative arrangements (not shown) the upper part of the bed could be hinged to the lower part about an axis extending along the side of the bed or the upper part could be arranged so that it could be lifted. and removed entirely from the lower part.
