Appareil pour le traitement de fractures. L'objet de l'invention est un appareil pour le traitement de fractures des os longs des membres, de l'avant-bras, de la jambe, d'un doigt par exemple ou d'autres os du corps, du calcaneum par exemple.
II existe des appareils servant au traite ment des fractures de la jambe et compor tant une broche métallique que l'on fait passer â travers le calcaneum, un étrier prenant appui sur la broche, un poids agissant sur l'étrier pour maintenir les deux parties de l'os fracturé à la position voulue pendant la durée de la formation du cal. Ils présentent divers inconvénients: le patient est condamné à l'immobilité, les articulations voisines ris quent de s'ankyloser par suite de leur immo bilisation, rien n'empêche mécaniquement les deux parties de l'os fracturé de se déplacer angulairement l'une par rapport à l'autre, ce qui gêne la formation du cal.
Ces divers inconvénients peuvent être évités dans l'appareil selon l'invention gràce au fait qu'il comporte deux broches métalli- ques destinées à être introduites au travers des deux parties de l'os fracturé ou d'os ri gidement reliés à ces deux parties, deux jougs rigidement reliés aux broches et des organes reliant les deux jougs de chaque côté de l'os fracturé et établis de manière qu'on puisse écarter ces derniers, le tout disposé de façon à permettre une réduction exacte de la frac ture et le maintien subséquent de la réduction pendant la formation du cal en immobilisant uniquement ces parties fracturées de l'os et éventuellement les os reliés de façon rigide à ces deux parties.
Le dessin annexé représente une forme d'exécution de l'appareil selon l'invention, donnée à titre d'exemple, et servant à pro duire l'extension dans le cas d'une fracture du tibia.
La fig. 1 en est une vue de face; La fig. 2 en est une élévation latérale, partie en coupe; La fig. 3 est une coupe suivant la ligné III-III de la fig. 1. Les deux broches a, b, faites en corde à piano en acier par exemple, sont rectilignes dans leur ensemble, traversent les parties c, d du tibia fracturé e vers le haut et le bais de celui-ci, et sont disposées entre les deux extrémités des jougs métalliques f, g qui ont en plan la forme générale d'un croissant.
Chacun des jougs f, y se compose de deux parties symétriques h, <I>i</I> venues séparé- nient de fabrication. Elles sont réunies par une cheville cylindrique j solidaire de la partie h et introduite dans un logement k de même diamètre de la partie i, où elle peut coulisser, ainsi que par une cheville l;
cette dernière se compose d'une partie cylin drique lisse in placée dans un logement n de la partie i où elle peut coulisser, d'une partie filetée o vissée dans un trou taraudé p de la partie h et d'un carré cg permettant de faire tourner la cheville 1. .Ainsi les deux parties<I>h., i</I> du joug sont guidées de façon à ne pouvoir tourner l'une par rapport à l'autre, mais peuvent être écartées l'une de l'autre pour tendre la broche a ou b en ame nant la partie filetée o de la cheville 1 en contact avec la partie i et en la vissant.
La broche a ou b passe dans des canaux r, de diamètre plus grand que le sien, des par ties h, <I>i</I> du joug<I>f</I>ou<I>g</I> et y est maintenue solidement par des vis s qui l'appliquent sur des sièges concaves t en la déformant; des carrés u permettent de man#uvrer ces vis.
Les deux jougs<I>f,</I> g sont reliés l'un à l'autre de part et d'autre du tibia e par deux paires d'organes constitués, l'un par une tige filetée v, l'autre par une douille allongée iv taraudée intérieurement et recevant le filetage de la tige<I>v.</I> En faisant tourner la douille iv par rapport à la tige v, grâce à un outil quelconque, introduit dans un trou x, on produit l'écartement ou le rapprochement des jougs. Les tiges<I>v,</I> les douilles<I>iv</I> se terminent par des sphères J placées dans des logements sphériques z ménagés dans les parties li, <I>i</I> des jougs f, g et s'ouvrant à l'extérieur de manière qu'on puisse y introduire les sphères.
Celles-ci peuvent être immobilisées dans leurs logements par des vis 2 munies de carrés de manoeuvre 3.
Les jougs<I>f,</I> g, les tiges<I>v,</I> les douilles iv sont faites par exemple en métal dit "anti- corodal"; dans ce cas, des sièges; en métal dur, sont prévus dans les canaux r pour les broches a, b à cause de la résistance insuffi sante de ce métal par rapport à l'acier des broches a, b.
On se sert comme suit de la forme d'exé cution décrite: L'opérateur fait passer les broches a, b, encore rectilignes, à travers les parties<I>c, d</I> du tibia fracturé; il les introduit dans les canaux r des parties des deux jougs f, g, qui sont encore séparées les unes des autres et fait pénétrer en même temps les chevilles j, t des parties 7i dans les logements<I>k, n</I> des parties i, il immobilise ces broches dans ces parties en agissant sur les vis s qui les déforment légèrement, enfin il les tend en faisant tourner les chevilles 1 à l'aide des carrés q. Dans ces conditions les deux jougs f;
g sont rigidement solidaires des broches a, b et des deux parties<I>c, d</I> du tibia e.
L'opérateur diminue la longueur totale des tiges v et des douilles iv de manière à les faire passer entre les deux jougs f, g, il dispose leurs sphères y en regard des loge ments z, il augmente leur longueur totale en agissant sur les douilles iv jusqu'à ce que leurs sphères y reposent dans les logements z et exercent un effort tendant à écarter les deux jougs f, g pour amener les parties c, d du tibia e à la position axiale relative voulue; lorsque cet effort atteint la valeur désirée, il immobilise les sphères y dans les logements z et par suite tout l'appareil à la position choisie.
S'il y a lieu d'augmenter ledit effort par la suite, il suffit de faire tourner les douilles <I>w</I> par rapport aux tiges<I>v.</I>
Le fait que ces douilles iv et ces tiges v sont reliées aux deux jougs f, g par des articulations à rotule permet de donner à ces derniers des positions très diverses: des posi tions où ils sont parallèles l'un à l'autre, où ils sont perpendiculaires au plan de la fig. 1 et où ils sont perpendiculaires à la jambe ou obliques par rapport à celle-ci, des positions où ils sont encore parallèles l'un à l'autre, mais où ils se trouvent dans un plan oblique par rapport au plan de la fig. 1; des positions où ils sont parallèles l'un à l'autre, mais sont décalés d'un certain angle, des positions où ils ne sont plus parallèles et sont en outre décalés d'un cer tain angle.
L'opérateur a de la sorte la possi bilité de réduire exactement la fracture et d'immobiliser les deux parties<I>c, d</I> du tibia fracturé à la position la plus favorable.
D'autre part, on se rend compte par l'inspection du dessin que cette forme d'exé cution prend peu de place, est portative et permet de réduire la fracture du tibia e, puis de maintenir cette réduction pendant la durée de la formation du cal, en immobilisant uni quement les parties<I>c, d</I> du tibia. Dans ces conditions il n'est plus nécessaire de con damner le patient tout entier à l'immobili sation; il est notamment possible de conser ver l'usage des articulations du pied et du genou, ce qui évite leur ankylose.
Le patient pourrait s'habiller, mettre son pantalon, mar cher dès la mise en place de la forme d'exé cution décrite, avant la formation du cal, sans que les parties<I>e, d</I> du tibia e risquent de se déplacer l'une par rapport à l'autre; si par hasard elles arrivaient à se déplacer pour un moment, la correction se ferait aussitôt.
L'appareil peut servir au traitement des fractures d'autres os longs des membres que le tibia, à celui de fractures de l'avant-bras, d'un doigt, ou d'autres os du corps, du cal- caneum par exemple, à condition qu'on lui donne les dispositions et les dimensions voulues. Les broches<I>a,</I> b suivant les cas peuvent traverser non pas des parties de l'os fracturé, mais bien des os rigidement reliés à ces parties.
Les douilles iv et les tiges filetées<I>v</I> peu vent être remplacées par des douilles médianes à taraudage droit et gauche ainsi que par deux paires de tiges filetées correspondantes: en faisant tourner les douilles on oblige les tiges soit à y rentrer, soit à en sortir.

These various drawbacks can be avoided in the apparatus according to the invention by virtue of the fact that it comprises two metal pins intended to be introduced through the two parts of the fractured bone or of bone firmly connected to these two. parts, two yokes rigidly connected to the pins and members connecting the two yokes on each side of the fractured bone and established so that they can be separated, the whole arranged in such a way as to allow an exact reduction of the fracture and the subsequent maintenance of the reduction during callus formation by immobilizing only those fractured parts of the bone and possibly the bones rigidly connected to these two parts.
The accompanying drawing shows an embodiment of the apparatus according to the invention, given by way of example, and serving to produce extension in the event of a fracture of the tibia.

The yokes <I> f, </I> g, the rods <I> v, </I> the sockets iv are made for example of so-called "anti-corodal" metal; in this case, seats; made of hard metal, are provided in the channels r for pins a, b because of the insufficient resistance of this metal compared to the steel of pins a, b.
The embodiment described is used as follows: The operator passes the pins a, b, still straight, through the parts <I> c, d </I> of the fractured tibia; he introduces them into the channels r of the parts of the two yokes f, g, which are still separated from each other and at the same time makes the pegs j, t of the parts 7i penetrate into the housings <I> k, n </ I > parts i, he immobilizes these pins in these parts by acting on the screws s which deform them slightly, finally he tightens them by rotating the plugs 1 using the squares q. Under these conditions the two yokes f;
g are rigidly attached to the pins a, b and to the two parts <I> c, d </I> of the tibia e.
The operator decreases the total length of the rods v and the bushings iv so as to pass them between the two yokes f, g, he places their spheres y opposite the housings z, he increases their total length by acting on the bushings iv until their spheres rest there in the housings z and exert a force tending to separate the two yokes f, g to bring the parts c, d of the tibia e to the desired relative axial position; when this force reaches the desired value, it immobilizes the spheres y in the housings z and consequently the whole apparatus in the chosen position.
If it is necessary to increase said force later, it suffices to rotate the bushings <I> w </I> with respect to the rods <I> v. </I>
The fact that these sockets iv and these rods v are connected to the two yokes f, g by ball joints allows the latter to be given very diverse positions: positions where they are parallel to each other, where they are perpendicular to the plane of FIG. 1 and where they are perpendicular to the leg or oblique with respect to the latter, positions where they are still parallel to one another, but where they are in an oblique plane with respect to the plane of FIG. 1; positions where they are parallel to each other but are shifted by a certain angle, positions where they are no longer parallel and are further shifted by a certain angle.
In this way, the operator has the possibility of reducing the fracture exactly and immobilizing the two parts <I> c, d </I> of the fractured tibia in the most favorable position.
On the other hand, one realizes by inspection of the drawing that this form of execution takes up little space, is portable and makes it possible to reduce the fracture of the tibia e, then to maintain this reduction for the duration of the training. callus, immobilizing only the <I> c, d </I> parts of the tibia. Under these conditions it is no longer necessary to condemn the whole patient to immobilization; it is in particular possible to conserve the use of the joints of the foot and of the knee, which prevents their ankylosis.
The patient could dress, put on his pants, walk as soon as the described form of execution is put on, before the callus is formed, without the <I> e, d </I> parts of the tibia. are likely to move relative to each other; if by chance they were able to move for a moment, the correction would be made immediately.
The device can be used for the treatment of fractures of other long bones of the limbs than the tibia, for fractures of the forearm, of a finger, or of other bones of the body, for example calcaneum. , on condition that it is given the desired layouts and dimensions. The pins <I> a, </I> b depending on the case may cross not parts of the fractured bone, but bones rigidly connected to these parts.
The iv bushings and the <I> v </I> threaded rods can be replaced by middle bushings with right and left tapping as well as by two pairs of corresponding threaded rods: by turning the bushings, the rods are forced to either to go in or out.
