Dispositif intra-utérin à efficacité accrue.
La présente invention a pour objet un dispositif intra-utérin du type mentionné dans la partie introductive de la revendication 1.
De tels dispositifs, dont la forme du support assure le maintien dans la matrice, sont bien connus et affectent généralement une forme en T, en V ou en 7, tandis que le filament qu'ils portent est généralement un filament de cuivre. Sur de tels dispositifs, à ce jour les filaments sont enroulés en spires jointives, serrées sur le support. La retenue du filament s'effectue de manière générale par serrage de l'enroulement sur le support.
Dans ces dispositifs connus, l'efficacité de l'élément libérant une substance active est réduite du fait que seule sa surface extérieure est susceptible de venir au contact des liquides de la matrice, et donc de libérer la substance active.
Le brevet DE-A-3209290 prévoit, à un stade de fabrication intermédiaire du dispositif intra-utérin, la retenue d'éléments en une matière libérant une substance active dans la cavité de la matrice de manière lâche sur une armature du support, cette retenue s'effectuant à l'aide d'une déformation du support. Ces éléments sont cependant ensuite enclavés dans une couche finale du support de sorte que, de manière conventionnelle, seule leur surface extérieure demeure susceptible de libérer une substance active. Ce document n'apporte donc pas de solution permettant un accroissement de l'efficacité de l'élément libérant une substance active.

Il a par ailleurs aussi été proposé d'amener sur des supports du type mentionné ci-avant d'autres éléments que des filaments, en vue d'accroître la surface d'échange de l'élément libérant une matière active dans la matrice. Tel est le cas notamment du dispositif décrit au brevet EP-A- 0147274, qui révèle un empilage, sur un support du type mentionné ci-dessus, de perles en une matière libérant une substance active dans la cavité de la matrice. La surface extérieure des perles est profilée pour comporter des rainures et nervures en vue d'augmenter la surface d'échange dans la matrice, alors que les perles sont retenues sur le support par une déformation de ce dernier constituant butée pour les perles.
L'efficacité du dispositif est dans ce cas augmentée en conférant aux éléments libérant une substance active dans la matrice un profil de la surface extérieure très particulier.
La présente invention a pour but d'augmenter, dans des dispositifs intra-utérins conventionnels du type mentionné dans l'introduction, l'efficacité de l'élément libérant une substance active dans la cavité de la matrice, tout en permettant l'usage d'un tel élément sous forme d'un filament.
Ce but est atteint conformément aux dispositions de la partie caractérisante de la revendication 1.
Des modes de réalisation avantageux de l'invention sont donnés dans les revendications subsidiaires.
L'invention sera mieux comprise en se reportant à la description ci-après, ainsi qu'au dessin annexé qui représente, uniquement à titre d'exemple et de manière totalement schématique, divers modes de réalisation de l'invention et dans lequel : - La fig. 1 est une vue d'un dispositif intra-utérin en T comportant un filament suivant l'invention - les fig 2a, 2b sont des vues partielles agrandies, respectivement en élévation en coupe en plan, du mode de réalisation de la fig 1 ; - de même les fig 3a, 3b et 4a, 4b sont des vues partielles

En se reportant au mode de réalisation illustré aux fig 1, 2a et 2b, un dispositif intra-utérin 1 suivant l'invention comporte un support 2, en Té en l'occurrence, sur la branche verticale 3 duquel est enroulé un filament 4 en une matière préformable libérant une substance active dans la matrice. Le filament est préformé sous forme d'un enroulement hélicoïdal 5 de diamètre intérieur supérieur au diamètre extérieur de la branche 3, de manière à ménager un jeu 6 entre le filament 4 et la branche 3, et présentant un pas permettant de ménager un jeu 7 entre deux spires 8,8' successives.
Suivant un autre mode de réalisation de l'invention, représenté aux figures 3a et 3b, un enroulement 5'est constitué d'une succession sensiblement hélicoïdale de spires présentant, par rapport à la direction axiale de l'hélice, un décalage latéral entre deux spires 8", 8"' successives, les spires successives se recouvrant au moins partiellement pour ménager un espace interne 9 de dimensions correspondant au moins au diamètre externe du support 3'. En fait dans ce mode de réalisation les spires 8", 8"', de forme sensiblement circulaire, présentent un décentrage entre deux spires successives pour former une double hélice.

Suivant une exécution préférée de ce mode de réalisation, le grand axe 11 des spires ovales 10 se situe dans un plan perpendiculaire au plan du support 2 constituant armature du dispositif intra-utérin. Ceci permet de ne pas accroître sensiblement l'encombrement du dispositif lorsque la branche supérieure du Té est repliée en position d'introduction du dispositif intra-utérin dans la matrice.
Les divers modes de réalisation illustrés et décrits ci-avant à titre d'exemple permettent ainsi d'exploiter les propriétés d'émission de la substance active dans la matrice sur l'ensemble de la surface du filament. En effet, c'est de manière générale l'ensemble de la surface du filament qui est accessible aux fluides de la matrice, et qui ainsi est susceptible de libérer la substance active, du fait que les fluides de la matrice sont susceptibles de s'insérer entre les spires successives, ainsi qu'entre l'enroulement et son support.
Ceci est particulièrement évident dans les divers modes de réalisation illustrés au dessin.
Il est par ailleurs évident aussi que dans des modes de réalisation tels que ceux illustrés aux figures 3a, 3b et 4a, 4b, le décalage latéral réalisé entre deux spires successives donne déjà un accès satisfaisant à la majeure partie de la surface du filament tant entre spires que dans l'espace entre l'enroulement et son support. Dans ce cas, une augmentation satisfaisante de l'efficacité du filament est encore obtenue lorsque le pas entre deux spires est extrêmement réduit, jusqu'à même correspondre à l'épaisseur du filament, et ainsi obtenir une structure d'enroulement jointive aux endroits où les spires successives viennent au contact l'une de l'autre.
Bien d'autres variantes d'enroulement du filament sur son support apparaîtront immédiatement à l'homme de métier,

qui permettent que les fluides de la matrice aient accès à la majeure partie de la surface du filament. Ces diverses variantes entrent bien entendu dans le domaine de la présente invention.
Par ailleurs, il va de soi que le montage du filament avec jeu sur le support nécessite de prévoir des moyens de retenue de l'enroulement sur le support. De tels moyens sont connus dans la technique. A titre d'exemple, divers moyens de retenue sont représentés aux figures 1, 5 et 6.
Plus particulièrement, à la fig. l l'enroulement 5 est retenu sur le support 2 par une protubérance 12 formée à l'extrémité de la branche 3. Suivant d'autres modes de réalisation possibles, à la fig. 5 l'enroulement 5 est retenu sur la branche 3 par une partie d'enroulement 13 serrée sur le support 3, tandis qu'à la fig 6 la partie serrée 13'de l'enroulement 5 coopère avec une rainure 14 formée dans le support 3.

Intrauterine device with increased efficiency.
The present invention relates to an intrauterine device of the type mentioned in the introductory part of claim 1.
Such devices, the shape of the support of which ensures retention in the matrix, are well known and generally have a T, V or 7 shape, while the filament which they carry is generally a copper filament. On such devices, to date the filaments are wound in contiguous turns, clamped on the support. The retention of the filament is generally carried out by tightening the winding on the support.
In these known devices, the effectiveness of the element releasing an active substance is reduced because only its outer surface is capable of coming into contact with the liquids of the matrix, and therefore of releasing the active substance.
Patent DE-A-3209290 provides, at an intermediate manufacturing stage of the intrauterine device, the retention of elements made of a material releasing an active substance in the cavity of the matrix loosely on a frame of the support, this retention effected by means of a deformation of the support. These elements are however then enclosed in a final layer of the support so that, conventionally, only their outer surface remains capable of releasing an active substance. This document therefore does not provide a solution allowing an increase in the effectiveness of the element releasing an active substance.

It has also been proposed to bring on supports of the type mentioned above elements other than filaments, in order to increase the exchange surface of the element releasing an active material in the matrix. This is particularly the case of the device described in patent EP-A-0147274, which reveals a stack, on a support of the type mentioned above, of beads of a material releasing an active substance in the cavity of the matrix. The outer surface of the beads is profiled to have grooves and ribs in order to increase the exchange surface in the matrix, while the beads are retained on the support by a deformation of the latter constituting an abutment for the beads.
The efficiency of the device is in this case increased by giving the elements releasing an active substance in the matrix a very particular profile of the external surface.
The present invention aims to increase, in conventional intrauterine devices of the type mentioned in the introduction, the effectiveness of the element releasing an active substance in the cavity of the matrix, while allowing the use of 'such an element in the form of a filament.
This object is achieved in accordance with the provisions of the characterizing part of claim 1.
Advantageous embodiments of the invention are given in the dependent claims.

Referring to the embodiment illustrated in FIGS. 1, 2a and 2b, an intrauterine device 1 according to the invention comprises a support 2, in Tee in this case, on the vertical branch 3 from which a filament 4 is wound. a preformable material releasing an active substance in the matrix. The filament is preformed in the form of a helical winding 5 with an internal diameter greater than the external diameter of the branch 3, so as to provide a clearance 6 between the filament 4 and the branch 3, and having a pitch making it possible to provide a clearance 7 between two successive 8.8 'turns.

The various embodiments illustrated and described above by way of example thus make it possible to exploit the emission properties of the active substance in the matrix over the entire surface of the filament. It is also obvious also that in embodiments such as those illustrated in FIGS. 3a, 3b and 4a, 4b, the lateral offset produced between two successive turns already gives satisfactory access to the major part of the surface of the filament, both between turns only in the space between the winding and its support. In this case, a satisfactory increase in the efficiency of the filament is still obtained when the pitch between two turns is extremely reduced, even to correspond to the thickness of the filament, and thus obtain a contiguous winding structure in the places where successive turns come into contact with each other.

which allow the fluids of the matrix to have access to most of the surface of the filament. These various variants are of course within the scope of the present invention.
Furthermore, it goes without saying that mounting the filament with play on the support requires providing means for retaining the winding on the support. Such means are known in the art. By way of example, various retaining means are shown in FIGS. 1, 5 and 6.
