APPARAAT VOOR HET HEELKUNDIG BEHANDELEN VAN PROSTAAT-
WEEFSELAANDOENINGEN
De uitvinding betreft een apparaat voor het nauwkeurig heelkundig behandelen van de prostaat door microgolfstraling.
Een belangrijk doel der uitvinding is een gecontroleerd en doeltreffend radiatiepatroon in de prostaat tot stand te brengen, in overeenstemming met de grootte en de vorm van de te behandelen prostaat.
Uit de Europese oktrooiaanvrage nr. 89403199. 6 is een apparaat gekend dat in hoofdzaak bestaat uit een catheter die in de urethra wordt geschoven, een antenne coaxiaal voorzien in genoemde catheter voor het uitstralen van electromagnetische golven voor de behandeling van de prostaat, een opblaasbare ballon aangebracht rondom de catheter om deze juist in te stellen en op zijn plaats te houden gedurende de behandeling, temperatuursensoren vast aangebracht in de langsrichting zijdelings van de catheter voor het meten van de temperatuur van de te behandelen prostaat en koelkanalen om het behandelde weefsel af te koelen en op de gewenste temperatuur te houden, waarbij nog gebruik wordt gemaakt van een sonde die in het rectum van de patiënt wordt aangebracht voor het meten van de temperatuur van de rectale wand.
Bij dit apparaat vormen de catheter, de antenne en de temperatuursensoren echter een onafscheidelijk geheel.
Een belangrijk nadeel hiervan is dat het center van de antenne die de microgolven uitstraalt steeds een vaste welbepaalde positie inneemt ten opzichte van de te behandelen prostaat, ongeacht de vorm en de grootte ervan.

Het gevolg hiervan is dat het prostaatweefsel niet steeds in alle richtingen gelijkmatig wordt bestraald en bovendien beschadiging van het omringende weefsel mogelijk is.
Het is dus noodzakelijk over een aantal catheters met anders ingestelde antennes te beschikken en dit naargelang het type van de te behandelen prostaat, om genoemd nadeel enigszins te ondervangen. Een nadeel hiervan is dat de gebruikskosten betrekkelijk hoog uitvallen en dat bovendien dit soort apparaat moeilijk is te steriliseren.
Bovendien is het met dit soort apparaat nooit mogelijk de antenne juist in te stellen ten opzichte van het center van de prostaat.
Om hieraan te verhelpen is volgens het voornaamste kenmerk der uitvinding een apparaat verwezenlijkt waarbij de antenne en de temperatuursensoren uitneembaar en vervangbaar in de catheter zijn ondergebracht, waarbij middelen zijn voorzien op de catheter voor het juist instellen en vastzetten van het center van de antenne ten opzichte van de prostaat en voor het in de juiste stand inbrengen van de catheter in de urethra, en waarbij de catheter in zijn langsrichting is voorzien van een of meer reflectiestrippen om het stralingsveld van de antenne in bepaalde richtingen te beperken naargelang de grootte en de vorm van de prostaat.
Hierdoor kan de catheter zelf na gebruik worden weggeworpen, terwijl de uitneembare antenne en de temperatuursensoren opnieuw kunnen worden gebruikt en het center van de antenne nauwkeurig centraal kan worden ingesteld ten opzichte van de prostaat, ongeacht de grootte en de vorm van de te behandelen prostaat. De gebruikskosten worden hierdoor beperkt. Door de aangebrachte reflectiestrippen op de catheter kan het stralingsveld van de antenne derwijze worden beinvloed dat de vorm van dit stralingsveld in overeenstemming kan worden gebracht met de vorm en de grootte van de te behandelen prostaat, zo-

dat beschadiging van het omringend weefsel praktisch uitgesloten wordt.
Andere voordelen zullen blijken uit de hiernavolgende beschrijving van een verkozen doch geenszins beperkte uitvoeringsvorm van het apparaat overeenkomstig de uitvinding. In deze figuren bemerkt men de catheter 1 van soepel materiaal en die bovenaan is voorzien van een afgeronde kop 2 met stop 3 om het in de urethra 4 inbrengen van de catheter 1 te vergemakkelijken. Over het voorste gedeelte van de catheter 1 is een rubberen kap 5 aangebracht waarvan het voorste en achterste gedeelte

op de catheter is vastgekleefd en het middelste vrije gedeelte tot een ballon 6 kan worden opgeblazen in de blaas 39 van de patiënt, om de catheter 1 vast in te stellen in de urethra 4. In de catheter 1 is een coaxiaal kanaal 7 voorzien waarin met een kleine speling een uitneembare antenne 8 voor het uitstralen van microgolven kan worden versteld, zodat het stralingscenter 9 van de antenne een gewenste stand kan innemen ten opzichte van de ballon 6 en van de prostaat, zoals hierna zal worden verduidelijkt.
Het achterste einde van de catheter 1 is afgedekt door een versterkingselement 10. De antenne is van een schaalverdeling 11 voorzien om de instelling van de antenne 8 te bepalen en af te lezen. Voor het opblazen van de ballon 6 is in de catheter 1 een in de langsrichting ervan lopend kanaal 12 voorzien dat zijdelings door een opening 13 met de ballon in verbinding staat. Achteraan is dit kanaal verlengd door een uit de catheter stekende soepele leiding 14 met klep 15 waarin een spuit 16 wordt gestoken, om in de ballon een vloeistof of lucht te spuiten en deze op te blazen en om de vloeistof of lucht terug te zuigen en de ballon leeg te zuigen. In de langsrichting van de catheter 1 zijn twee diametraal gelegen groeven 17 voorzien.
In elke groef is een soepele slang 18 gekleefd die lichtjes uit de buitenomtrek van de catheter springt en waarin een of meer uitneembare temperatuursensoren 19 worden geschoven voor het meten van de temperatuur van het weefsel dat wordt behandeld door het apparaat. Doordat de soepele slangen 18 een weinig uit de catheter steken, wordt een goed contact van de temperatuursensoren met het weefsel bekomen. De soepele slangen 18 kunnen eveneens een dwarsdoorsnede hebben zoals voorgesteld in fig. 6 en waarbij een kanaal 20 is voorzien voor het inbrengen van de temperatuursensor en een kanaal 21 dat dienst doet als luchtisolatiekanaal tussen de tem-

peratuursensor 19 die in het kanaal 20 wordt geschoven en de koelkanalen 22 van de catheter 1.
Een voordeel hiervan is dat een beter kontakt wordt bekomen met de urethrale wand waardoor een betere temperatuurmeting mogelijk is. Om het weefsel af te koelen is de catheter 1 in langsrichting voorzien van koelkanalen 22 met een ingang 23 en een uitgang 24 en waardoorheen de koelvloeistof stroomt. Deze kanalen zijn derwijze uitgevoerd dat zij het grootste gedeelte van de omtrek van de catheter kunnen afkoelen (fig. 2). Om het patroon van het stralingsveld van de antenne 8 aan te passen aan de vorm en grootte van de prostaat 25 is op twee diametraal gelegen plaatsen en in de langsrichting van de catheter 1 een reflectiestrook 26 gekleefd waarvan de breedte kan verschillen naargelang het te behandelen geval (fig. 2 en 9).
Om de catheter in de juiste positie in de urethra te schuiven ten opzichte van de prostaat, is op de catheter een oriëntatieteken aangebracht (niet voorgesteld). De catheter wordt vastgezet in een houder 27 door middel van twee klauwen 28 en een spanelement 29, waardoor gelijktijdig de soepele catheter op de antenne wordt vastgeklemd wanneer deze in de gewenste stand is opgesteld, hetgeen op de schaalverdeling 11 van de antenne 8 kan worden afgelezen. De antenne 8, de temperatuursensoren 19 en de in- en uitgang 23-24 van de koelkanalen 22, worden respectievelijk op de houder 27 aangesloten door een koppeling 30,31 en 32. De houder is anderzijds aangesloten op een temperatuurmeter 33, een koeleenheid 34 en een generator 35 voor het voortbrengen van electromagnetische golven. Het geheel wordt bestuurd door een centrale 36.
Verder wordt voor het behandelen van een prostaat 25, gebruik gemaakt van een gekende sonde 37 die in het rectum 38 van de patiënt wordt geschoven, om de positie van de in de urethra geschoven catheter 1 te controleren

en het koelen en meten van de temperatuur van de wand van het rectum te controleren. Een opblaasbare ballon kan zijn voorzien op de sonde 37 om een beter kontakt met de rectale wand te verzekeren en het effect van de koeling te verbeteren. Belangrijk is dat de koeling in de sonde 37 alleen dan ingeschakeld wordtwanneer een vooraf ingestelde limietwaarde van de temperatuur van de rectale wand is bereikt. Door de uit de catheter stekende slangen 18 met temperatuursensoren, wordt een perfecte aansluiting tussen de catheter en het weefsel bekomen.
Nadat de ballon 6 van de catheter 1 in de blaas 39 van de patiënt is opgeblazen door middel van een spuit 16, zit de catheter vast en wordt de stand van het stralingscenter 9 van de antenne 8 door verschuiving van de antenne in de catheter derwijze ingesteld, dat genoemd stralingscenter 9 zieh in het centrum van de prostaat 25 bevindt. Hierdoor en door de samenwerking van de op de catheter gekleefde reflectiestrippen 26 en de in het rectum van de patiënt geschoven sonde 37, wordt een perfecte gecontroleerde bestraling en koeling aangepast aan de vorm en grootte van de prostaat bekomen, zonder het omringende weefsel te beschadigen.
Doordat de antenne en de temperatuursensoren uit de catheter verwijderbaar zijn, kan voor elke behandeling een nieuwe hygiënische catheter worden gebruikt, zonder dat de kosten te hoog oplopen, waarbij echter de antenne en de temperatuursensoren meermaals kunnen worden aangewend.

APPARATUS FOR THE DENTAL TREATMENT OF PROSTATE
TISSUE DISORDERS
The invention relates to an apparatus for the precise surgical treatment of the prostate by microwave radiation.
An important object of the invention is to establish a controlled and effective radiation pattern in the prostate according to the size and shape of the prostate to be treated.
European patent application No. 89403199. 6 discloses an apparatus consisting essentially of a catheter slid into the urethra, an antenna coaxially provided in said catheter for radiating electromagnetic waves for the treatment of the prostate, an inflatable balloon applied around the catheter to properly adjust and hold in place during treatment, temperature sensors fixed longitudinally to the catheter to measure the temperature of the prostate to be treated and cooling channels to cool the treated tissue and maintain at the desired temperature using a probe inserted into the patient's rectum to measure the temperature of the rectal wall.
With this device, however, the catheter, antenna and temperature sensors form an inseparable whole.
An important drawback of this is that the center of the antenna emitting the microwaves always occupies a fixed, well-defined position relative to the prostate to be treated, regardless of its shape and size.

The consequence of this is that the prostate tissue is not always irradiated uniformly in all directions and, moreover, damage to the surrounding tissue is possible.
It is therefore necessary to have a number of catheters with differently adjusted antennas, depending on the type of prostate to be treated, in order to somewhat overcome this drawback. A drawback of this is that the operating costs are relatively high and, moreover, this type of device is difficult to sterilize.
Moreover, with this type of device it is never possible to adjust the antenna correctly in relation to the center of the prostate.
In order to remedy this, according to the main feature of the invention, a device has been realized in which the antenna and the temperature sensors are accommodated in the catheter in a removable and replaceable manner, wherein means are provided on the catheter for correctly adjusting and securing the center of the antenna relative to of the prostate and for properly inserting the catheter into the urethra, and the catheter being longitudinally provided with one or more reflective strips to limit the radiation field of the antenna in certain directions according to the size and shape of the catheter the prostate.
This allows the catheter to be disposed of after use, while the removable antenna and temperature sensors can be reused and the center of the antenna can be precisely adjusted centrally relative to the prostate, regardless of the size and shape of the prostate to be treated. The operating costs are thereby limited. Due to the applied reflection strips on the catheter, the radiation field of the antenna can be influenced in such a way that the shape of this radiation field can be matched to the shape and size of the prostate to be treated, such as

that damage to the surrounding tissue is practically excluded.
Other advantages will become apparent from the following description of a preferred but by no means limited embodiment of the device according to the invention. In these figures, the catheter 1 is made of flexible material and which is provided at the top with a rounded head 2 with plug 3 to facilitate the insertion of the catheter 1 into the urethra 4. A rubber cap 5 is provided over the front part of the catheter 1, of which the front and rear parts

is affixed to the catheter and the center free section can be inflated into a balloon 6 in the bladder 39 of the patient, in order to fix the catheter 1 in the urethra 4. In the catheter 1 a coaxial channel 7 is provided, in which a small clearance a removable antenna 8 for radiating microwaves can be adjusted so that the radiation center 9 of the antenna can assume a desired position with respect to the balloon 6 and the prostate, as will be explained below.
The rear end of the catheter 1 is covered by a reinforcing element 10. The antenna is provided with a scale 11 to determine and read the setting of the antenna 8. For inflating the balloon 6, a catheter 1 running in its longitudinal direction is provided in the catheter 1 and communicates laterally through an opening 13 with the balloon. At the back, this channel is extended by a flexible conduit 14 with valve 15 protruding from the catheter, into which a syringe 16 is inserted, in order to inject and inflate a liquid or air in the balloon and to draw back the liquid or air and the to empty balloon. Two diametrically located grooves 17 are provided in the longitudinal direction of the catheter 1.
In each groove is adhered a flexible tube 18 that pops slightly out of the outer circumference of the catheter and into which one or more removable temperature sensors 19 are slid to measure the temperature of the tissue being treated by the device. Because the flexible tubes 18 protrude slightly from the catheter, good contact of the temperature sensors with the tissue is obtained. The flexible hoses 18 can also have a cross-section as shown in Fig. 6 and wherein a channel 20 is provided for the insertion of the temperature sensor and a channel 21 that serves as an air insulation channel between the temperatures.

temperature sensor 19 which is slid into the channel 20 and the cooling channels 22 of the catheter 1.
An advantage of this is that a better contact is obtained with the urethral wall, so that a better temperature measurement is possible. In order to cool the tissue, the catheter 1 is provided in longitudinal direction with cooling channels 22 with an inlet 23 and an outlet 24 through which the cooling liquid flows. These channels are designed to cool most of the circumference of the catheter (Fig. 2). In order to adapt the pattern of the radiation field of the antenna 8 to the shape and size of the prostate 25, a reflective strip 26, the width of which may differ according to the case to be treated, is affixed in two diametrically located places and in the longitudinal direction of the catheter 1. (Figures 2 and 9).
In order to slide the catheter in the correct position in the urethra with respect to the prostate, an orientation mark is applied to the catheter (not shown). The catheter is secured in a holder 27 by means of two claws 28 and a clamping element 29, whereby the flexible catheter is simultaneously clamped to the antenna when it is arranged in the desired position, which can be read on the scale 11 of the antenna 8 . The antenna 8, the temperature sensors 19 and the input and output 23-24 of the cooling channels 22 are respectively connected to the holder 27 by a coupling 30, 31 and 32. The holder is on the other hand connected to a temperature meter 33, a cooling unit 34 and a generator 35 for generating electromagnetic waves. The whole is controlled by a central 36.
Furthermore, to treat a prostate 25, a known probe 37 inserted into the patient's rectum 38 is used to check the position of the catheter 1 inserted into the urethra

and control the cooling and measurement of the temperature of the wall of the rectum. An inflatable balloon may be provided on the probe 37 to ensure better contact with the rectal wall and to improve the effect of the cooling. It is important that the cooling in the probe 37 is only switched on when a preset limit value of the rectal wall temperature has been reached. The tubes 18 with temperature sensors protruding from the catheter ensure a perfect connection between the catheter and the tissue.
After the balloon 6 of the catheter 1 is inflated in the bladder 39 of the patient by means of a syringe 16, the catheter is fixed and the position of the radiation center 9 of the antenna 8 is adjusted by shifting the antenna in the catheter that said radiation center 9 is located in the center of the prostate 25. Through this, and through the cooperation of the reflective strips 26 affixed to the catheter and the probe 37 inserted into the patient's rectum, perfect controlled irradiation and cooling adapted to the shape and size of the prostate is obtained without damaging the surrounding tissue.
Because the antenna and the temperature sensors are removable from the catheter, a new hygienic catheter can be used for every treatment, without the costs being too high, but the antenna and the temperature sensors can be used several times.
