Attelle La présente invention se rapporte à une attelle destinée aux patients ou handicapés contraints à un alitement prolongé ou contraint à une longue immobilisation d'une partie du corps, p. e. un membre.
On connaît ainsi les nombreux inconvénients d'un alitement ou d'une immobilisation prolongée consécutifs aux contraintes d'appui des zones à risques au niveau du sacrum et des membres inférieurs, en particulier le talon ou le tendon d'achille. Pour le talon, il apparait en effet notamment des escarres talonniers et d'autres symptômes comme l'équinisme de la cheville.
Actuellement il existe principalement trois systèmes anti-escarres talonniers, dont deux permettent le positionnement des segments jambe-pied.
Un premier type de système répartit la charge pondérale sur la plus grande surface possible en épousant plus ou moins bien les morphologies individuelles. Il s'agit des systèmes basés sur des matelas à eau, des matelas anti-escarres et de recouvrement par tissu type'peau de mouton'. Aucun de ces systèmes n'implique le positionnement des segments ou le transfert des charges d'appui sur des surfaces cutanées sans risques.

Un second type de dispositif présente un dégagement standard et limité des zones à risques et est basé sur l'utilisation d'une structures en mousse dans laquelle un évidement-type pour le ou les talons est prévu afin d'éviter toute charge d'appui sur ceux-ci. La structure en mousse prédécoupée n'épouse que très mal les variables morphologiques individuelles. Le positionnement des segments n'est cependant pas variable, l'angulation jambe-pied est laissée au hasard de l'enfoncement talonnier dans l'excavation. Les échanges naturels des surfaces cutanées sont contrecarrés par l'enfermement des zones à risque.
Enfin un troisième type de système consiste en une attelle garnie de mousse préformée dont la découpe standard n'épouse qu'imparfaitement les morphologies individuelles et dont les caractéristiques de résistance mécanique à la pression ne permettent qu'imparfaitement de répondre aux variables pondérales des segments coirporels concernés. La découpe standard dégage complètement l'appui d'un talon standard et permet des choix de positions limités à la rotation interne et externe de la hanche. Quant à la position angulaire jambe-pied, elle est impérativement fixée à 900 de par la position inamovible de l'attelle.

Aux inconvénients précités s'ajoute l'enfermement du talon entre la mousse et la structure plastique de l'attelle ainsi que la transposition d'appui de ala charge pondérale du segment jambe sur une zone circulaire autour du talon.
De plus l'appui de la jambe et du pied sur une surface de mousse entraîne une sudation peu favorable et un incomfort certain. Cet élément mousse n'est pas stérilisable et doit donc par mesure d'hygiène être complètement renouvelé quand les nécessités thérapeutiques l'exigent. xxxxxxxxx L'attelle selon l'invention permet d'éviter les inconvénients précités tel que toute position vicieuse du membre concerné, en particulier le pied et se présente sous la forme d'une structure tubulaire préformée, de préférence inoxydable et stérilisable, et qui est garnie d'un tissu à densité variable.
Dans le cadre de la présente description l'expression 'tubulaire'est utilisée pour qualifier la forme d'un tube qui peut être creux mais, pour l'invention, sera de préférence plein.

Selon une particularité de l'invention, le tissu est un tissu élastique se présentant sous la forme d'un ou de plusieurs manchons glissés sur la structure tubulaire préformée faisant fonction d'armature.
Avantageusement, la tension du tissu peut être modulée en faisant varier la forme de la structure tubulaire l'cartement de part et d'autre du plan médian de l'attelle pouvant avantageusement varier en fonction des propriétés de soutien désirées à différents endroits du tissu.
Le tissu présente une élasticité telle que une bande de 7,5 x 30 cm peut s'allonger transversalement jusqu'à environ 19 cm sous une force correspondant à un poids de 5 kg.
Différents types de tissu tubulaire susmentionnés sont commercialement disponibles sous une forme anallergique. On les appelle fréquemment tissu, coton ou'Jersey'élastique et/ou tubulaire. Certains de ceux-ci sont fréquemment utilisés en établissements hospitaliers.
Ce type de tissu est en particulier disponible sous forme de manchon enroulé et peut être découpé transversalement selon les longueurs souhaitées.

Aux extrémités de l'armature, la partie non sous tension ou la partie excédentaire du tissu peut être rabattue vers l'intérieur du manchon c'est à dire entre les parois internes du tissu tubulaire. xxxxxxxxxxxx L'invention propose en particulier une attelle qui a un effet préventif et curatif des escarres, en particulier les escarres talonniers.
L'invention propose également un dispositif complémentaire permettant la recherche de positions thérapeutiques pour le membre inférieur et l'articulation de la hanche.
Selon un mode de réalisation particulier de l'invention consistant en une attelle anti-équin, on propose un dispositif ne présentant aucune surface d'appui pour le talon ou le tendon d'Achille et comportant un cadre de soutien et d'appui plantaire. L'ensemble du système est utilisable comme arceau de protection de façon à éviter tout appui de la literie sur l'avant-pied.
La structure, de préférence tubulaire préformée, par exemple en aluminium mais avantageusement en acier inoxydable et stérilisable sous forme de barre de 8 mm de

diamètre, se présente sous la forme d'un cadre tridimensionnel, symétrique par rapport à un plan vertical longitudinal et comprenant une partie ou élément jambe et une partie ou élément pied essentiellement perpendiculaire l'une à l'autre.
Celles-ci sont sous-tendues par un tissu à élasticité comme défini ci-dessus définie solidaire des éléments tubulaires et pouvant être fixés par un système de type'Velcro' (marque déposée) qui laisse libre de tout appui l'ensemble du talon. De préférence cependant selon l'invention la partie en tissu se présentera sous la forme de deux manchons textiles élastiques pouvant être glissés d'une part sur l'armature de l'élément jambe et d'autre part sur l'armature de l'élément pied.
L'armature de l'attelle antiéquin selon un mode particulier de réalisation de l'invention se présente sous la forme d'un cadre tridimensionnel dont la projection en plan est approximativement rectangulaire. Les parties proximales et distales par rapport à l'utilisateur repose sur une surface d'appui, p. e. un lit. La partie proximale présente un profil transversal arrondi.
Le cadre, en l'absence du manchon entissu élastique, comprend deux branches ou longerons longitudinaux parallèles se terminant par une courbure ou un coude ramenant la structure tubulaire sur un segment longitudinal plus court en

contact avec la surface d'appui, avant de se relever, formant ainsi l'élément'pied', en des montants approximativement verticaux dont les extrémités sont réunies par une branche transversale complétant le cadre.
Avantageusement les branches verticales de l'armature ne sont pas exactement parallèles. Leurs dispositions sont choisies de manière à faire varier la tension du tissu élastique en fonction des segments corporels à soutenir localement. En absence de manchon, l'écartement de la base de l'élément pied peut être supérieure d'environ lcm par rapport à l'écartement du dessus.
La partie en tissu se compose de deux manchons introduits autour de l'élément jambe d'une part et autour de l'élément pied d'autre part. On veillera avantageusement à ce que le manchon de l'élément jambe dépasse la courbure ou le coude du cadre sans toutefois recouvrir la région du talon.
La forme finale des surfaces de soutien est influencée par la tension du tissu élastique qui exerce une force rapprochant les parties latérales du cadre antérieur, non complètement rigide, éloignées des deux éléments transversaux. Ainsi la forme trapézoidale de la surface de soutien pied s'inverse par rapport à la forme trapezoïdale

du cadre nu, lorsque le manchon est placé. La largeur à la partie supérieure est en effet alors plus grande qu'au niveau de la base.
La mise sous tension du tissu élastique provoque donc une déformation de l'armature métallique qui peut être contrôlée.
Il en résulte que les surfaces de tissu non seulement de l'élément pied mais également de l'élément jambe sont avantageusement trapézoïdales.
L'attelle recouverte du tissu élastique se présente donc sous la forme de deux trapèzes disposés approximativement perpendiculaires l'un à l'autre par leurs côtés plus petit.
De ce fait c'est à la hauteur du 1/3 supérieur de l'élément pied recouvert du tissu que l'appui du pied est maximum et que la tension du tisu l'est également. Il y a concordance entre la zone d'appui concernée et la fonction de cette même zone lors de l'appui normal du pied sur le sol.

Les longerons du cadre de l'élément jambe sont parallèles en l'absence de manchon mais ne sont plus parallèles une fois celui-ci installé. Il y a un raccourcissement de la largeur de la partie antérieur alors que la partie postérieure reste plus rigide.
La tension du tissu dans la partie supérieure du membre soutenu (mollet) ést alors plus élevée et la partie du membre s'y reposant subira la plus grande partie du poids.
Au niveau antérieur (tendon d'Achille) il y aura une diminution de tension.
Quant à la tension à la hauteur de la partie arrondie de l'attelle, proche du genoux, la forme même de la structure crée une diminution importante de la tension qui évite tout excès de pression d'appui dans une zone sensible proche du creux poplité (partie arrière du genoux).
L'élément jambe longitudinal n'est pas parallèle au plan d'appui mais présente une déclivité vers la partie postérieure. La différence de hauteur peut être de 2 cm. Elle permet d'éviter un porte à faux de l'articulation du genoux et de mettre le pied dans une position fonctionnelle par rapport à la jambe, soit avec un angle d'environ 50).

Selon ce mode de réalisation particulièrement avantageux de l'invention, une structure métallique préformée mobile permet un positionnement angulaire du pied par rapport à la jambe. Cette dernière structure métallique est un élément modulaire adaptable à la structure de base, c'est à dire au cadre susmentionné.
Complémentairement à la structure tubulaire de base, un élément tubulaire peut en effet être adapté pour répondre aux exigences de traitement de l'équinisme du pied, ou d'un positionnement spécifique en flexion plantaire ou en flexion dorsale. Ce dispositif complémentaire peut consister en un cadre rectangulaire ou de préférence en un U inversé inscrit dans la structure tubulaire préformée et dont deux extrémités sont attachées en pivotement à la base de la partie pied de la structure tubulaire.
Le manchon sera alors appliqué le long du cadre mobile inscrit dans la structure métallique formant élément pied, et non plus le long de ladite structure.
La fixation se fait par l'intermédiaires de deux blocs ou modules latéraux remplissant trois fonctions : - moyen de pivotement de l'élément modulaire pied (flexion) - moyen de réglage de la déclivité

- moyen de rotation interne ou externe de l'attelle et donc du membre qu'elle supporte.
Les deux blocs modulaires jouent donc le rôle de moyen de fixation comprenant l'axe de pivotement de la partie'pied' mobile.
Les deux éléments (armature de base, armature mobile) sont donc maintenus de par et d'autre par deux modules, moyens de fixation et de positionnement, incorporés à l'élément modulaire à la base des branches verticales de l'armature de l'attelle. La fixation peut être assurée par tout moyen connu par exemple un système à clip, à broches ou par vissage.
Ce type d'adaptation ou de fixation sur le cadre peut se présenter sous la forme d'éléments, par exemple des disques mais de préférence des blocs préformés et préforés, qui s'encastrent à la base de la partie pied de la structure de base et joue le rôle de support des axes de la structure mobile. Sur ces mêmes blocs sont disposés le mécanisme de fixation angulaire de la structure mobile en fonction du positionnement souhaité du pied.

La structure pied est dans une position initiale, quasi perpendiculaire (5 0 de flexion plantaire) à la structure jambe et reproduit donc la réalité des configurations anatomiques normales. La plage de mobilité de l'élément pied peut varier dans une étendue angulaire de 40 0 répartie par exemple en 30 0 de flexion plantaire et 10 0 de flexion dorsale. Le pivotement peut être prévu continu ou non. En pratique, cinq positions sont suffisantes, p. e., de 10 en 100, 1 position de flexion dorsale, 1 position neutre et 3 positions de flexion plantaire.
Les moyens de pivotement de l'élément modulaire pied peuvent prendre différentes formes connues en soi. Le cadre intérieur peut ainsi être bloqué dans certaines positions définies par des ergots se logeant dans des renfoncements transversaux ou des trous. Un système connu de bouton poussoir peut également assurer le réglage du pivotement.
Tout autre système mécanique équivalent peut être utilisé, avec ou sans position de pivotement privilégiée.

Avantageusement, cette mise en position déclive de l'attelle peut être obtenue par rotation partielle et symétrique des deux blocs ou disques susmentionnés autour d'un axe décentré et solidaire de la structure tubulaire de l'attelle.
Le réglage de la déclivité est ainsi obtenu par un réglage des modules latéraux comprenant une pièce extérieure apte à pivoter autour d'un axe décentré et de dimension telle qu'au moins pour un degré de pivotement déterminé, elle est en contact avec le plan de support du cadre, p. e. le lit et détermine ainsi la hauteur, par rapport au lit, de la partie du cadre au droit du module.
Si la rotation n'est effectuée que sur l'un des deux polygones de soutien, on obtiendra une inclinaison latérale vers la droite ou vers la gauche.
Par exemple un polygone de soutien à quatre côtés permettra deux positions d'inclinaison latérale et deux positions de rotation, interne ou externe, soit quatre possibilités.

********** L'un des buts de la présente invention est donc de proposer une attelle à structure modulaire, de préférence tubulaire, qui peut donc entre autres présenter des dimensions répondant à la variabilité des morphologies sans pour autant, par son encombrement, gêner le positionnement du membre inférieur qui ne serait pas contraint dans l'attelle.
La résistance mécanique de la structure doit répondre aux variabilités pondérales des segments qu'elle aura à supporter et avoir une inclinaison de l'élément jambe par rapport au plan du lit en relation avec la différence de volume entre partie cheville et partie mollet. Cette inclinaison nécessaire doit compenser cette différence et éviter qu'en position couchée ou assise du malade, l'attelle n'induise un flexum de genou trop important. De plus cette

Selon une variante non préférée, la longueur de la partie jambe de l'attelle peut être modifiée, les tubes latéraux faisant parties de deux éléments solidaires par exemple par un système d'attaches télescopiques permettant de faire varier la longueur du cadre dans le sens longitudinal. En pratique cependant, le choix entre deux attelles de dimension différente est largement suffisant et un dispositif complémentaire pour faire varier la longueur de l'attelle ne s'avèrera pas économiquement justifié.
Les parties jambe et pied de la structure tubulaire selon l'invention sont garnies d'un tissu tubulaire élastique dont la tension est donc variable en fonction de la forme même de la structure tubulaire à laquelle il est fixé. Le tissu tubulaire présente ainsi les caractéristiques suivantes :

Selon les figures 1 et 2 l'attelle comporte un cadre tubulaire 1, large de 20 cm, dont une des extrémités 2 , antérieure, présente, vu latéralement, un profil en U relevé et l'autre 3 ; postérieure, est rabaissée et présente un profil transversal arrondi. Ce cadre prend lui-même appui, par exemple sur un lit, par l'intermédiaire de son extrémité arrondie 5 et par les bases des profils en U relevé constitués d'un court segment 9 d'appui distal. Les branches verticales 10 sont reliées à leurs extrémités supérieures par une branche transversale 11 complétant le cadre.
