Abstract

医療デバイス
 本明細書に開示される技術は、医療デバイスに関する。
 血管等における狭窄部や閉塞部(以下、「病変部」という。)の治療に用いられる医療デバイスとして、アテレクトミーデバイスが知られている。アテレクトミーデバイスは、病変部を掘削することにより、病変部を形成する血栓やプラーク等の除去を行う。
 従来、先端カッタ(掘削部)を備え、ガイドワイヤ(ガイド部材)が挿通されるガイドワイヤオリフィスが形成されたアテローム切除アセンブリ(医療デバイス)が開示されている(例えば、特許文献1参照)。当該アテローム切除アセンブリでは、まず、ガイドワイヤオリフィスにガイドワイヤを挿通し、予めガイドワイヤを血管内の血栓等の位置まで進行させる。次に、先端カッタを血管内の血栓等の位置まで進行させ、先端カッタを回転させることにより、血管壁からの血栓等の除去を行う。当該アテローム切除アセンブリでは、ガイドワイヤオリフィスにガイドワイヤを挿通した場合、先端カッタの先端が、ガイドワイヤの外縁の全周を覆う構成となっている。

 病変部を掘削するために掘削部を回転させる際、上記先行技術のように掘削部の先端がガイド部材の外縁の全周を覆っている構成では、ガイド部材の外表面周辺に存在する病変部に、絶えず掘削部の先端面が接触することとなる。そのため、掘削可能なように所定の角度に形成された掘削部の縁(エッジ)がガイド部材の外表面周辺に存在する病変部に接触しにくく、ガイド部材の外表面周辺に存在する病変部が削れずに残存するという課題がある。
 本明細書では、上述した課題を解決することが可能な技術を開示する。
 本明細書に開示される技術は、例えば以下の形態として実現することが可能である。
 本明細書に開示される医療デバイスは、チューブと、掘削部と、を備える。前記チューブは、ガイド部材を挿通するためのルーメンを有する。前記掘削部は、前記チューブの先端よりも先端側に突出し、前記チューブの中心軸に直交する第1の方向の幅の最大値が、前記中心軸および前記第1の方向に直交する第2の方向の幅の最大値よりも大きい。前記掘削部の先端は、前記チューブに対して前記第2の方向の一方側に位置するように構成されている。

A.実施形態:
A-1.医療デバイス10の構成:
 図1は、医療デバイス10の側面の構成を概略的に示す説明図である。図1には、医療デバイス10の側面の構成とともに、医療デバイス10による治療に用いられるガイドワイヤ20の側面の構成も示されている。図1およびこれ以降に示す各図には、方向を特定するための互いに直交するXYZ軸が示されている。図1において、Z軸正方向側が、体内に挿入される先端側(遠位側)であり、Z軸負方向側が、医師等の手技者によって操作される基端側(近位側)である。図1では、医療デバイス10の一部分の図示が省略されている。図1では、医療デバイス10の中心軸AX1がZ軸方向に平行な直線状となった状態を示しているが、医療デバイス10は、湾曲させることができる程度の可撓性を有している。
 本明細書では、医療デバイス10およびその各構成部材について、先端側の端を「先端」といい、先端およびその近傍を「先端部」といい、基端側の端を「基端」といい、基端およびその近傍を「基端部」という。また、医療デバイス10およびその各構成部材の縦断面とは、中心軸AX1を含む断面(例えばYZ断面)を意味し、医療デバイス10およびその各構成部材の横断面とは、中心軸AX1に直交する断面(XY断面)を意味する。これらの点は、以降の図においても同様である。
 医療デバイス10は、血管内における病変部(狭窄部や閉塞部)を治療するために、血管に挿入される長尺状のデバイスである。医療デバイス10は、病変部を掘削することにより、病変部を形成する血栓やプラーク等の除去を行う。医療デバイス10の全長は、例えば1500mm以上、2000mm以下程度である。
 ガイドワイヤ20は、医療デバイス10による血管内における病変部の治療に用いられる。ガイドワイヤ20は、医療デバイス10の中心軸AX1に沿って延びる長尺状の部材であり、例えば金属線材により構成されている。ガイドワイヤ20の各位置における横断面の形状は任意の形状を取り得るが、例えば円形である。ガイドワイヤ20の外径は、例えば0.2mm以上、0.8mm以下程度である。ガイドワイヤ20の全長は、例えば2000mm以上、3000mm以下程度である。ガイドワイヤ20は、ガイド部材の一例である。
 ガイドワイヤ20を形成する材料としては、例えば、ステンレス鋼(SUS302、SUS304、SUS316等)、Ni-Ti合金、ピアノ線等が用いられる。ガイドワイヤ20は、全体が同じ材料により構成されていてもよいし、部分毎に互いに異なる材料により形成されていてもよい。
 図2は、医療デバイス10の縦断面の構成を概略的に示す説明図である。図3は、図2のIII-IIIの位置における医療デバイス10の横断面の構成を概略的に示す説明図である。図4は、図2のIV-IVの位置における医療デバイス10の横断面の構成を概略的に示す説明図である。
 医療デバイス10は、チューブ100と、掘削部200と、固定部300と、内側コイル体400と、外側コイル体500と、内側カテーテル600と、外側カテーテル700と、コネクタ800と、モータードライブ900と、を備える。
 チューブ100は、医療デバイス10の中心軸AX1に平行に延びる中空管状の部材である。図4に示すように、チューブ100は、ガイドワイヤ20を挿通するためのルーメン102を有する。
 掘削部200は、チューブ100の先端よりも先端側に突出する部材である。図4に示すように、掘削部200の基端部は、チューブ100と接続されている。掘削部200とチューブ100とは、例えばロウ材(アルミニウム合金ロウ、銀ロウ、金ロウ等)により接合されている。掘削部200の詳細な構成については、後述する。
 固定部300は、医療デバイス10の中心軸AX1に平行に延びる中空管状の部材である。図2および図4に示すように、固定部300の内面は、掘削部200の基端部と、チューブ100とのそれぞれと接続しており、掘削部200の基端部と、チューブ100とを束ねるようにして固定している。固定部300は、チューブ100および掘削部200のそれぞれと例えばロウ材(アルミニウム合金ロウ、銀ロウ、金ロウ等)により接合されている。
 内側コイル体400は、医療デバイス10の中心軸AX1に平行に延びる中空管状の部材である。内側コイル体400は、1本以上の線材が巻き回されることにより構成される。本実施形態においては、内側コイル体400は、複数の線材が巻き回された多条コイルにより構成されており、内側コイル体400を構成する各線材は、単一の素線により構成されている。また、本実施形態の内側コイル体400は、1つの層からなる単層コイルである。図3に示すように、内側コイル体400の内部には、ガイドワイヤ20を挿通するためのルーメン402が形成されている。また、図2に示すように、内側コイル体400の先端部の外面は、固定部300の内面と接続されている。内側コイル体400と、固定部300とは、例えばロウ材(アルミニウム合金ロウ、銀ロウ、金ロウ等)により接合されている。内側コイル体400の外径は、例えば0.3mm以上、1.0mm以下程度である。
 外側コイル体500は、例えば1本の線材が巻き回されることにより構成されたコイル状の部材である。図2および図3に示すように、外側コイル体500は、内側コイル体400の外面に沿うように構成されており、外側コイル体500の内面は、内側コイル体400の外面と接続されている。外側コイル体500と、内側コイル体400とは、例えばロウ材(アルミニウム合金ロウ、銀ロウ、金ロウ等)により接合されている。
 内側カテーテル600は、医療デバイス10の中心軸AX1に平行に延びる中空管状の部材である。内側カテーテル600の内部には、内側カテーテル600の先端から基端まで伸びるルーメンが形成されている。図1および図2に示すように、内側カテーテル600に形成されたルーメンは、中心軸AX1に直交する方向において、固定部300や外側コイル体500の幅よりも大きく、固定部300や外側コイル体500を収容することが可能である。内側カテーテル600の内径は、例えば1.8mm程度である。
 外側カテーテル700は、医療デバイス10の中心軸AX1に平行に延びる中空管状の部材である。外側カテーテル700の内部には、外側カテーテル700の先端から基端まで伸びるルーメンが形成されている。図1に示すように、外側カテーテル700に形成されたルーメンは、中心軸AX1に直交する方向において、内側カテーテル600の幅よりも大きく、内側カテーテル600を収容することが可能である。外側カテーテル700の内径は、例えば2.2mm程度である。
 コネクタ800は、医療デバイス10の中心軸AX1に平行に延びる中空管状の部材である。コネクタ800は、3つの開口を有している。コネクタ800における3つの開口のうち、Z軸方向に面する2つの開口は、コネクタ800の先端側に配置された内側コイル体400と、コネクタ800の基端側に配置されたモータードライブ900と、を接続するために設けられている。また、コネクタ800における3つの開口のうち、Z軸方向に交差する方向に面する1つの開口は、例えば吸引機が接続され、掘削部200により掘削された削りくずを吸引するための吸引口として機能する。
 モータードライブ900は、内側コイル体400の回転を制御するための装置である。モータードライブ900は、内部にモーターを備えており、モーターの回転が内側コイル体400に伝達される。
A-2.掘削部200の詳細構成:
 図5および図6は、実施形態における掘削部200の詳細な構成を示す説明図である。図5では、X軸方向視における医療デバイス10の先端部の構成が拡大して示されており、図6では、Y軸方向視における医療デバイス10の先端部の構成が拡大して示されている。
 掘削部200は、Y軸方向の幅の最大値が、X軸方向の幅の最大値よりも大きい。換言すれば、掘削部200は、扁平な形状を有している。また、掘削部200は、ループ形状を有している。具体的には、掘削部200は、例えば金属線材により構成されている。掘削部200を構成する金属線材の一端と他端とは、いずれも掘削部200における基端側に配置され、チューブ100とともに固定部300に束ねられるようにして固定されている。掘削部200を構成する金属線材の一端と他端との間の部分は、X軸方向視において環を形成している。掘削部200のループ形状の内部には、X軸方向に延びる貫通孔210が形成される。掘削部200は、チューブ100の外周の近傍においてZ軸方向に延び、掘削部200の先端は、チューブ100の先端よりも先端側に設けられている。Y軸方向は、第1の方向の一例であり、X軸方向は、第2の方向の一例である。
 掘削部200を構成する金属線材の断面の形状は、各位置によって異なる。具体的には、掘削部200のY軸方向の幅の最大値を構成する位置(図5におけるA1-A1の位置)、および、掘削部200のY軸方向の幅の最大値を構成する位置よりも基端側の位置における金属線材の断面は、円形である。また、掘削部200のY軸方向の幅の最大値を構成する位置よりも先端側の位置(例えば、図5におけるA2-A2の位置)における金属線材の断面は、ループ形状の外側に面する部分が直線状であり、ループ形状の内側に面する部分が円弧状である。つまり、掘削部200のY軸方向の幅の最大値をなす位置よりも先端側の位置における金属線材の断面は、半円形である。掘削部200は、超弾性材料により構成され、例えばNi-Ti合金である。
 図6に示すように、掘削部200の先端は、チューブ100に対してX軸方向の一方側(X軸正方向側)に位置するように構成されている。また、ガイドワイヤ20の挿通時において、掘削部200の先端は、ガイドワイヤ20に対してX軸方向の一方側(X軸正方向側)に位置するように構成されている。換言すれば、ガイドワイヤ20の挿通時において、掘削部200の先端は、ガイドワイヤ20の外縁の全周を覆わない。また、掘削部200のX軸方向の中心線CLは、医療デバイス10の中心軸AX1に対してX軸方向の一方側(X軸正方向側)に位置し、チューブ100の中心軸AX2は、医療デバイス10の中心軸AX1に対してX軸方向の他方側(X軸負方向側)に位置している。すなわち、X軸方向において、掘削部200の中心線CLとチューブ100の中心軸AX2とのそれぞれは、医療デバイス10の中心軸AX1に対してずれている。
 図7および図8は、医療デバイス10の先端部の構成を概略的に示す説明図である。図7に示すように、掘削部200が内側カテーテル600に覆われない場合、掘削部200のY軸方向の幅は、内側カテーテル600や外側カテーテル700のY軸方向の幅よりも大きい。これにより、内側カテーテル600や外側カテーテル700の外径よりも大きい直径の孔を病変部に形成することができる。一方、図8に示すように、掘削部200が内側カテーテル600に覆われる場合、掘削部200のY軸方向の幅は、内側カテーテル600のY軸方向の幅よりも小さくなる。このように、掘削部200は、収縮状態(例えば図8の状態)と、収縮状態よりもY軸方向に拡張した拡張状態(例えば図7の状態)と、になり得るように構成されている。先に述べたように、掘削部200は、超弾性材料により構成されているため、掘削部200は、収縮状態において形状が変形しても、内側カテーテル600の外部に移動したときに容易に元の形状に戻ることができる。拡張状態において、掘削部200のY軸方向の幅は、2mmから6mm程度である。
A-3.医療デバイス10の使用方法:
 以下、医療デバイス10の使用方法の一例について詳細に説明する。図9は、医療デバイス10の使用方法の一例を示す説明図である。はじめに、医師等の手技者は、ルーメン402およびルーメン102にガイドワイヤ20を挿通させる。次に、手技者は、血管30内にガイドワイヤ20を挿入し、ガイドワイヤ20が病変部32(例えば、慢性完全閉塞)を通過するまで、ガイドワイヤ20を進行させる。次に、手技者は、血管30内に医療デバイス10を挿入し、医療デバイス10の先端が病変部32の位置に達するまで医療デバイス10を進行させる。このとき、医療デバイス10の先端が病変部32の位置に達するまでは、掘削部200を収縮状態のまま進行させ、医療デバイス10の先端が病変部32の位置に達したところで、掘削部200を拡張状態に移行させる。
 次に、手技者は、モータードライブ900により内側コイル体400を回転させながら、掘削部200を病変部32に接触させ、さらに遠位側に向かって医療デバイス10を進行させる。このとき、内側コイル体400は、固定部300を介して掘削部200と接続されているため、内側コイル体400を回転させることにより、図9に示すように、内側コイル体400の回転が掘削部200に伝達される。これにより、掘削部200が病変部32内に侵入しつつ、掘削部200が病変部32内において回転することによって病変部32を掘削する。なお、このときガイドワイヤ20が病変部32を通過することでガイドワイヤ20の径方向の位置が病変部32によって一時的に固定されているため、掘削部200は、ガイドワイヤ20のまわりを回転することとなる。また、これと同時に、手技者は、病変部32を掘削することによって生じる削りくずを、コネクタ800の開口から吸引機によって吸引する。これにより、削りくずが内側コイル体400と内側カテーテル600との間の空間等を通過し、コネクタ800の開口から削りくずが回収される。その後、手技者が医療デバイス10による病変部32の掘削を続けることにより、医療デバイス10は、最終的に病変部32を通過する。
A-4.本実施形態の効果:
 以上説明したように、医療デバイス10は、チューブ100と、掘削部200と、を備える。チューブ100は、ガイドワイヤ20を挿通するためのルーメン102を有する。掘削部200は、チューブ100の先端よりも先端側に突出し、チューブ100の中心軸AX2に直交するY軸方向の幅の最大値が、中心軸AX2およびY軸方向に直交するX軸方向の幅の最大値よりも大きい。掘削部200の先端は、チューブ100に対してX軸方向の一方側に位置するように構成されている。
 本実施形態の医療デバイス10によれば、掘削部200の先端が、チューブ100に対してチューブ100の中心軸AX2に直交するX軸方向の一方側に位置するように構成されている。すなわち、ガイドワイヤ20の挿通時においては、掘削部200の先端が、ガイドワイヤ20に対してX軸方向の一方側に位置するように構成されている。これにより、病変部32を掘削するために掘削部200を回転させる際、掘削部200の縁が病変部32に接触しやすくなり、ガイドワイヤ20の外表面周辺に存在する病変部32が削れずに残存することを抑制することにより、医療デバイス10の病変部32に対する掘削性能を向上することができる。
 また、本実施形態の医療デバイス10では、掘削部200は、収縮状態と、収縮状態よりもY軸方向に拡張した拡張状態と、になり得るように構成されている。本実施形態の医療デバイス10によれば、掘削部200が、収縮状態と拡張状態になり得るため、収縮状態で血管30内を進行すること、および、拡張状態で病変部32を掘削することが可能となる。そのため、例えば掘削部200が収縮状態と拡張状態とになり得ない構成と比較して、血管30内における医療デバイス10の進行のしやすさを担保しつつ、医療デバイス10の掘削径を大きくすることができ、医療デバイス10の病変部32に対する掘削性能をより効果的に向上することができる。
 また、本実施形態の医療デバイス10では、掘削部200は、超弾性材料により構成され、かつ、ループ形状を有する。本実施形態の医療デバイス10によれば、掘削部200が、超弾性材料により構成されるため、形状変化に対する耐久性を向上することができる。なおかつ、掘削部200が、ループ形状を有するため、医療デバイス10を用いた手技の安全性を向上することができる。
 なお、本実施形態の医療デバイス10では、掘削部200のY軸方向の幅の最大値を構成する位置よりも先端側の位置における掘削部200を構成する金属線材の断面は、ループ形状の外側に面する面が直線状である。そのため、掘削部200における掘削部200が回転する方向の縁にエッジが形成されることとなり、医療デバイス10の病変部32に対する掘削性能をより効果的に向上することができる。さらに、掘削部200のY軸方向の幅の最大値を構成する位置における掘削部200を構成する金属線材の断面は、円形である。そのため、医療デバイス10が病変部32の壁面を掘削するリスクを低減することができる。
B.変形例:
 本明細書で開示される技術は、上述の実施形態に限られるものではなく、その要旨を逸脱しない範囲において種々の形態に変形することができ、例えば次のような変形も可能である。
 図10は、変形例における掘削部200aの詳細な構成を示す説明図である。図10では、X軸方向視における医療デバイス10aの先端部の構成が拡大して示されている。以下では、変形例の医療デバイス10aの構成のうち、上述した実施形態の医療デバイス10と同一の構成については、同一の符号を付すことによってその説明を適宜省略する。
 変形例の医療デバイス10aを構成する掘削部200aは、実施形態の掘削部200と構成が異なる。具体的には、変形例の掘削部200aは、ループ形状ではなく、パドル状である。すなわち、変形例の掘削部200aは、実施形態の掘削部200における貫通孔210が埋められた構成を有する。このように、医療デバイスの掘削部は、必ずしもループ形状を有していなくてもよく、少なくとも扁平な形状であればよい。なお、変形例の掘削部200aは、図10に示すようにX軸方向に面する表面が平坦なパドル状であるが、掘削部は、例えばX軸方向に面する表面の中央付近が窪んだスプーン状であってもよい。
 上記実施形態では、内側コイル体400は、複数の線材が巻き回された多条コイルにより構成されているが、内側コイル体は、多条コイルではなく、1本の線材が巻き回された単条コイルであってもよい。また、上記実施形態では、内側コイル体400を構成する各線材は単一の素線であるが、内側コイル体を構成する各線材は、複数の素線が撚り合わされた撚線であってもよい。また、上記実施形態では、内側コイル体400は、1つの層からなる単層コイルであるが、内側コイル体は、複数の層からなる多層コイルであってもよい。
 上記実施形態では、掘削部200が、収縮状態と拡張状態とになり得るように構成されているが、掘削部は、必ずしも形状が変化しなくてもよい。同様に、掘削部は、必ずしも超弾性材料により構成される必要はない。
 上記実施形態における掘削部200を構成する金属線材の断面の形状は、あくまで一例であり、種々変形が可能である。例えば、掘削部を構成する金属線材の断面の形状は、一端から他端までの全体にわたって、矩形や円形であってもよい。また、ループ形状の外側に面する部分と、内側に面する部分とで形状が異なっていてもよく、例えば、一端から他端までの全体にわたって、ループ形状の外側に面する面が円弧状で、ループ形状の内側に面する面が矩形等であってもよい。
 上記実施形態では、ガイド部材としてガイドワイヤ20が用いられているが、ガイドワイヤ以外のガイド部材が用いられてもよい。
 上記実施形態では、医療デバイス10は、血管内における病変部の治療に用いられているが、本明細書に開示される技術は、血管以外の生体管腔(胆管、胆嚢、膵臓、食道、十二指腸、小腸、大腸等の消化器官、血管、尿管、気管等)内においても同様に適用可能である。

Claims (3)

  1.  医療デバイス(10)であって、
     ガイド部材(20)を挿通するためのルーメン(102)を有するチューブ(100)と、
     前記チューブ(100)の先端よりも先端側に突出する掘削部(200)であって、前記チューブ(100)の中心軸(AX2)に直交する第1の方向の幅の最大値が、前記中心軸(AX2)および前記第1の方向に直交する第2の方向の幅の最大値よりも大きい掘削部(200)と、
    を備え、
     前記掘削部(200)の先端は、前記チューブ(100)に対して前記第2の方向の一方側に位置するように構成されている、医療デバイス(10)。
  2.  請求項1に記載の医療デバイス(10)であって、
     前記掘削部(200)は、収縮状態と、前記収縮状態よりも前記第1の方向に拡張した拡張状態と、になり得るように構成されている、医療デバイス(10)。
  3.  請求項2に記載の医療デバイス(10)であって、
     前記掘削部(200)は、超弾性材料により構成され、かつ、ループ形状を有する、医療デバイス(10)。
