Abstract

本発明は、急性膵炎の発症を早期に予測することができ、早期に適切な治療介入を可能にする方法、より詳細には、急性膵炎の治療に必要なデータを提供する方法であって、 (1)急性膵炎が疑われる被験体の検体中のトリプシン濃度を測定すること、 (2)該測定値に基づく急性膵炎の診断に必要なデータを提供し、トリプシン濃度が一定値以上の場合に急性膵炎の治療を推奨すること、 を含む、方法に関する。

Description

急性膵炎における膵炎マーカーの迅速測定法
 本特許出願は、日本国特許出願第2023-107374号について優先権を主張するものであり、ここに参照することによって、その全体が本明細書中へ組み込まれるものとする。
 本発明は、急性膵炎の迅速測定方法に関する。
 急性膵炎は、膵臓の急性炎症であり、ときに隣接する他の臓器にまで影響を及ぼし得るものである。急性膵炎の最も多い症状は、上腹部痛であるが、背部まで痛みが広がることもあり、嘔吐、発熱などの症状を伴うこともある。まれに重症化することがあり、重症化すると、呼吸、循環、腎不全などの多臓器不全や、感染症を併発することがある。また、急性膵炎の2大原因は、アルコールと胆石であるが、内視鏡的逆行性胆管膵管造影(ERCP)などを受けた患者にも発生することが知られている。
 一般に、次の項目:
1.上腹部に急性腹痛発作と圧痛がある
2.血中または尿中に膵酵素の上昇がある
3.超音波、CTまたはMRIで膵に急性膵炎に伴う異常所見がある
の2項目以上を満たし、他の膵疾患および急性腹症を除外したものが急性膵炎と診断されており、さらに、膵酵素としては、アミラーゼ、リパーゼを測定することが望ましいとしている(非特許文献1)。

 本発明の目的は、急性膵炎の発症を早期に予測することができ、早期に適切な治療介入を可能にする方法を提供することである。
 本発明者らは、上記課題を解決するため鋭意検討した結果、急性膵炎の早期予測因子(膵炎マーカー)として、トリプシン濃度を用いることができることを見出し、本発明を完成した。
 すなわち、本発明は、以下の態様を含む。
[1] 急性膵炎の治療に必要なデータを提供する方法であって、
(1)急性膵炎が疑われる被験体の検体中のトリプシン濃度を測定すること、
(2)該測定値に基づく急性膵炎の診断に必要なデータを提供し、トリプシン濃度が一定値以上の場合に急性膵炎の治療を推奨すること、
を含む、方法。
[2] 前記トリプシン濃度が570ng/mL以上の場合に、急性膵炎の治療を推奨する、項1に記載の方法。
[3] 該検体のトリプシン濃度の測定と同時に該検体のアミラーゼ濃度を測定した場合、該アミラーゼ濃度が125U/L以下である、項2に記載の方法。
[4] 該検体のトリプシン濃度の測定と同時に該検体のリパーゼ濃度を測定した場合、該リパーゼ濃度が55U/L以上である、項2または3に記載の方法。
[5] 前記急性膵炎の治療が、輸液投与、薬剤投与、または外科的除去から選択される、項1~4のいずれかに記載の方法。
[6] 前記工程(2)の後、さらに、
(3)該急性膵炎の重症度を判定すること、
(4)該工程(3)の結果、重症急性膵炎であると判定された場合、3時間以内に治療を開始することを推奨すること、
を含む、項1~5のいずれかに記載の方法。
[7] 前記被験体が、内視鏡検査、内視鏡手術または切開手術が行われた被験体である、項1~6のいずれかに記載の方法。
[8] 前記トリプシン濃度の測定を、免疫学的測定用試薬または免疫学的測定系を用いて行う、項1~7のいずれかに記載の方法。
[9] 前記免疫学的測定用試薬または免疫学的測定系が、ラテックス凝集法、ELISA、RIAまたは化学発光免疫測定法を用いるための試薬または測定系である、項8に記載の方法。
 本発明によれば、急性膵炎の発症またはその起点を早期に予測することができるので、早期に適切な治療介入を可能にする。
図1は、内視鏡的胆管膵管造影(endoscopic retrograde cholangiopancreatography;ERCP)検査を受けた翌日に急性膵炎発症と診断された患者群(患者群)と、発症なしと診断された患者群(膵炎無し群)の、トリプシン、リパーゼおよびアミラーゼ測定値を、基準値上限(トリプシン570ng/mL、リパーゼ55U/L、アミラーゼ125U/L)との比率で比較した結果を示す。 無群を陰性、膵炎患者群を陽性として検査直後の各マーカー(トリプシン、リパーゼ、アミラーゼ)のROC曲線を作成し、AUCを算出した結果を示す。 無群を陰性、膵炎患者群を陽性として検査後2時間後の各マーカー(トリプシン、リパーゼ、アミラーゼ)のROC曲線を作成し、AUCを算出した結果を示す。
 本明細書における「急性膵炎」は、次の項目:
 (1) 上腹部の急性腹痛発作と圧痛を認める、
 (2) 血中または尿中膵酵素の上昇を認める、
 (3) 画像診断(超音波、CT、MRI)で膵に急性膵炎に伴う異常所見を認める、
のうち、2項目以上を満たすものである。
 また、当該急性膵炎の原因としては、特に限定されないが、例えば、アルコール、胆石、内視鏡的逆行性胆管膵管造影検査などが挙げられる。
 本明細書における「治療する」または「治療」とは、被験体(患者)における疾患状態を処置することを意味し、これには、疾患状態を抑制(例えば進行の抑制、遅延)、軽減、軽快、改善、治癒することなどが含まれる。
 また、本明細書における「急性膵炎を治療する」または「急性膵炎の治療」には、輸液投与、薬剤投与、外科的除去などが挙げられる。
 上記「輸液」には、生理食塩水、リンゲル液などが挙げられる。
 上記「外科的除去」には外科的ネクセクトミーなどが挙げられる。
 本明細書における「診断する」または「診断」とは、被験体(患者)における疾患の現在または未来の状態を判定(判断)することを意味し、これには、疾患に罹患するか否かの予測または判定、疾患状態の重症化が進行するか否かの予測または判定、疾患に罹患しているか否かの判定、疾患状態の度合または進行度の判定、疾患に対する治療効果の判定、治療後に疾患を再発する危険性が存在するか否かの判定などが含まれる。
 本明細書において「重症度」としては、軽症、中等症、重症等が挙げられ、「重症化」としては、軽症から中等症または重症への重症化、中等症から重症への重症化が挙げられる。
 本明細書において「発症」とは、疾患の症状が現れることをいう。
 本明細書において「被験体」とは、検査対象となる生物、特に動物(例えば、ヒト、マウス、ラット、ハムスター、モルモット、サル、ウシ、ブタ、ウマ、ウサギ、ヒツジ、ヤギ、ネコまたはイヌ等の哺乳動物)を意味し、特にヒト(この場合、「被験者」とも称する)が挙げられる。
 また、本明細書における「被験体」(ヒトの場合の「被験者」)には、疾患に罹患している被験体(患者)、罹患が疑われる被験体(被験者)、罹患するリスクがある被験者(被験者)、疾患に罹患していない被験体(健常者)などが含まれ、特に、内視鏡検査(例えば、ERCP検査)、内視鏡手術または切開手術が行われた被験体が挙げられる。
 本明細書における「検体」としては、上記トリプシンが含まれ得る検体であれば特に限定されないが、例えば、ヒト被験体から採取された血液から調製した血液検体が挙げられる。本明細書における「血液検体」とは、血液成分の少なくとも一部を含む検体を意味し、全血、血清、および血漿のいずれであってもよく、これらを希釈したものであってもよい。血液検体は、好ましくは血清または血漿である。血液検体の調製は、既知の方法で行うことができる。
 本明細書における「トリプシン」は、そのトリプシンまたはその前駆体(トリプシノーゲン)として生体内に単独で、もしくは対応する結合分子と結合した状態で存在し得る。当該対応する結合分子としては、特に限定されないが、例えば、αアンチトリプシンまたはαマクログロブリンが挙げられる。
 以下、特に言及しない限り、「トリプシンまたはその前駆体」および「対応する結合分子と結合した複合体」を、まとめて「トリプシン」と称する。
 本明細書における「トリプシンまたはその対応する結合分子と結合した複合体に対する抗体またはその抗原結合フラグメント」は、上記トリプシンまたはその対応する結合分子と結合した複合体に特異的結合能を有するものであれば特に限定されない。
 ここで、「トリプシン」に特異的結合能を有する抗体またはその抗原結合フラグメントとしては、トリプシンの一部をエピトープとして認識して結合可能である抗体またはそのフラグメントなどが挙げられる。
 また、トリプシンと結合分子との「複合体」に特異的結合能を有する抗体またはその抗原結合フラグメントとしては、複合体の状態で表面に表れているトリプシンの一部をエピトープとして認識して結合可能である抗体またはその抗原結合フラグメント、結合分子の一部をエピトープの一部または全部として認識して結合可能である抗体またはその抗原結合フラグメントなどが挙げられる。
 上記「抗体」は、モノクローナル抗体でも、ポリクローナル抗体でもよい。また、当該抗体は、市販品でもよく、トリプシンまたはその一部分を抗原として用いて細胞融合技術、遺伝子組換え技術、ファージディスプレイ等の既知の方法にしたがって生産したものでもよい。
 また、上記「抗体の抗原結合フラグメント」は、上記抗体のフラグメントであって、トリプシンに結合能を有するフラグメントを意味し、例えば、上記抗体をパパイン等で部分分解して得られるFabフラグメント、ペプシン等で部分分解して得られるF(ab’)2フラグメント、F(ab’)2フラグメントを還元処理して得られるFab’フラグメント等が挙げられる。
 好ましくは、モノクローナル抗体またはその抗原結合フラグメントが挙げられる。
 以下、特に言及しない限り、「抗体またはその抗原結合フラグメント」を、まとめて「抗体」と称する。
 本発明は、一態様として、急性膵炎の治療に必要なデータを提供する方法であって、
(1)急性膵炎が疑われる被験体の検体中のトリプシン濃度を測定すること、
(2)該測定値に基づく急性膵炎の診断に必要なデータを提供し、トリプシン濃度が一定値以上の場合に急性膵炎の治療を推奨すること、
を含む、方法を提供する。
 上記工程(1)は、急性膵炎が疑われる被験体の検体中のトリプシン濃度を測定する工程である。当該工程(1)におけるトリプシン濃度の測定は、如何なる方法を用いてもよいが、免疫学的測定用試薬または免疫学的測定系を用いて行うことが好ましい。
 上記免疫学的測定用試薬または免疫学的測定系としては、特に限定されないが、例えば、トリプシンまたはその対応する結合分子と結合した複合体に対する抗体またはその抗原結合フラグメントを用いるものが挙げられ、具体的には、酵素免疫測定法(ELISA)、蛍光酵素免疫測定法、化学発光酵素免疫測定法、化学発光免疫測定法、電気化学発光測免疫測定法、蛍光抗体法、ラジオイムノアッセイ(RIA)、ウェスタンブロット法、イムノブロット法、ラテックス凝集法、イムノクロマトグラフィー法、ネフェロメトリー法などを用いるための試薬または測定系が挙げられる。好ましくは、ラテックス凝集法、ELISA、RIAまたは化学発光免疫測定法を用いるための試薬または測定系が挙げられる。
 これらの試薬または測定系は、種々の構成、形状、状態などは特に限定されず、既知の方法にしたがって調製したものでもよく、市販されているものでもよい。
 上記試薬または測定系において、上記抗体またはその抗原結合フラグメントは、1種でもよく、2種以上でもよい。2種以上の抗体またはその抗原結合フラグメントを用いる場合、それらは、例えば、異なるエピトープに結合する2種以上の抗体またはその抗原結合フラグメントであってもよく、同じエピトープに結合する2種以上の抗体またはその抗原結合フラグメントであってもよい。また、トリプシンが遊離体としても複合体としても検体中に存在し得る場合、2種以上の抗体のうち、少なくとも1種は、複合体に対する抗体またはその抗原結合フラグメントであってもよい。
 例えば、トリプシンの異なる部分をエピトープとして認識して結合可能である2種以上の抗体またはその抗原結合フラグメントの組合せ;トリプシンの一部をエピトープとして認識して結合可能である少なくとも1種の抗体と、その抗体のエピトープとは異なるエピトープを認識して結合可能である少なくとも1種の抗体の抗原結合フラグメントとの組合せ;トリプシンの一部をエピトープとして認識して結合可能である少なくとも1種の抗体またはその抗原結合フラグメントと、複合体の一部をエピトープの一部または全部として認識して結合可能である少なくとも1種の抗体またはその抗原結合フラグメントの組合せ;トリプシンの一部をエピトープとして認識して結合可能である少なくとも1種の抗体またはその抗原結合フラグメントと、結合分子の一部をエピトープの一部または全部として認識して結合可能である抗体またはその抗原結合フラグメントなどが挙げられる。
 上記試薬または測定系において、上記抗体は、支持体に固定化されていてもよい。支持体は、当該試薬を用いる方法に応じて、適宜選択することができる。支持体としては、例えば、ウェルプレート(例、96ウェルマイクロプレートなど)、メンブレン(例、ニトロセルロース膜、ポリフッ化ビニリデン膜など)、スライドガラス、磁気ビーズ、ラテックス粒子等が挙げられる。抗体の支持体への固定化は、支持体の材質に応じて選択した既知の方法により行うことができる。
 また、上記抗体が支持体に固定されている場合、上記トリプシンの異なるエピトープに結合する第二の抗体を含んでいてもよい。この場合、第一の抗体と第二の抗体の両方が抗体でもよく、一方が抗体であり、他方が抗原結合フラグメントでもよく、両方が抗原結合フラグメントでもよい。
 また、上記抗体は、標識化されたものであってもよい。標識化のための標識物質は、特に限定されず、既知のものを用いればよい。標識物質としては、例えば、ペルオキシダーゼ、アルカリホスファターゼなどの酵素標識;フルオレセインイソチオシアネート(FITC)などの蛍光標識;ヨウ素125などの放射性同位体標識;ルテニウム錯体などの電気化学発光標識;ビオチン;金属ナノ粒子等が挙げられる。当該抗体の標識化は、標識物質の種類に応じて選択した既知の方法により行うことができる。
 上記試薬または測定系は、キットの形態であってもよい。当該キットには、上記抗体に加えて、他の要素を含んでいてもよい。他の要素としては、例えば、標識物質の検出試薬、トリプシンの標準検体、血液検体調製用試薬、希釈液、バッファー類、支持体、使用説明書等が挙げられる。
 上記試薬または測定系は、界面活性剤を含むことができる。界面活性剤を含む試薬または測定系においては、検体中にトリプシンと結合分子との結合体の状態で存在していても、界面活性剤の作用により、トリプシンと結合分子を分離し、トリプシンを遊離させることができるので、検体中に遊離体として存在していたトリプシンばかりでなく、結合体の状態で存在していたトリプシンも検出または測定することができる。したがって、当該界面活性剤を含む試薬または測定系においては、生体内で遊離体と結合体の両方の状態で存在し、生体内の環境によって存在比が異なり得るトリプシンを、精度よく検出することができ、その量(濃度)も精度よく測定することができる。
 このような試薬または測定系の一例として、以下のAおよびB:
 (A)界面活性剤、
 (B)該トリプシンに対する抗体
を含む、
試薬または測定系などが挙げられる。
 本明細書における「界面活性剤」としては、例えば、非イオン性界面活性剤、両性界面活性剤などが挙げられる。
 非イオン性界面活性剤の具体例としては、ポリオキシエチレン(20)ソルビタンモノラウレート(商品名Tween 20)、ポリオキシエチレンソルビタンモノオレアート(商品名Tween 80)、ポリエチレングリコールモノ-4-オクチルフェニルエーテル(商品名Triton X-100)、オクチルフェノキシポリ(エチレンオキシ)エタノール(商品名Nonidet(R) P40)などが挙げられる。
 両性界面活性剤の具体例としては、3-[(3-コラミドプロピル)ジメチルアンモニオ]-1-プロパンスルホネート(CHAPS)などが挙げられる。
 好ましくは非イオン性界面活性剤が挙げられ、特にポリオキシエチレン(20)ソルビタンモノラウレートが挙げられる。
 上記試薬または測定系における界面活性剤の含有量は、特に限定されないが、例えば、試薬または測定系全体の0.02重量%以上であり、好ましくは0.03~3重量%、特に0.05~1重量%である。
 上記試薬または測定系は、緩衝液を含むことができる。緩衝液としては、抗体がトリプシンに結合するのを妨げる性質を有しないものであればよく、例えばpH5.0~10.0、好ましくはpH5.5~8.5の中性付近に緩衝作用を有する、例えばMES緩衝液、HEPES緩衝液、リン酸緩衝液、トリス緩衝液、グッド緩衝液、グリシン緩衝液、ホウ酸緩衝液などが挙げられる。また、緩衝液中の緩衝剤の濃度としては、通常10~500mM、好ましくは10~300mMの範囲から適宜選択される。
 上記試薬または測定系は、抗体がトリプシンに結合するのを妨害しない量で、当該分野で既知の添加剤を含むことができる。例えば、緩衝剤、防腐剤(アジ化ナトリウムなど)タンパク質(アルブミン)、水溶性高分子(糖類、ポリエチレングリコール、デキストランなど)、塩類(塩化ナトリウム、アミノ酸など)などを含む。
 上記試薬または測定系において、上記抗体、界面活性剤、緩衝液、添加剤などの各成分は、2個以上の別個の試薬に分かれて含まれていてもよい。その一例として、例えば、以下の試薬A1および試薬B1:
 (A1)界面活性剤を含む試薬A1、
 (B1)該トリプシンに対する抗体を含む、試薬B1
を含み、
検体の測定時に、検体と試薬A1と試薬B1が混合される、
試薬または測定系などが挙げられる。
 上記試薬または測定系において、検体の測定時に検体と試薬A1と試薬B1が混合されるが、その際の混合条件は、測定対象のトリプシンと抗体の結合反応(抗原抗体反応)が妨害されない条件であれば、特に限定されない。
 上記検体または測定系の測定時における検体と試薬A1と試薬B1の混合物中の界面活性剤の含有量は、特に限定されないが、例えば、0.02重量%以上であり、好ましくは0.03~3重量%、特に0.05~1重量%である。
 上記試薬A1は、検体の測定時に、検体と試薬B1と混合して得られる混合物中の界面活性剤の含有量が、上記所定の範囲内になればよく、界面活性剤そのものであっても、界面活性剤を含む組成物であってもよい。
 上記試薬A1は、界面活性剤に加えて、上記緩衝液、添加剤などを含んでいてもよい。また、上記試薬B1は、抗体に加えて、上記緩衝液、添加剤などを含んでいてもよい。これらの試薬A1およびB1に含まれる成分は、各々の試薬の安定性、取扱性などを考慮して適宜決定すればよい。
 上記試薬または測定系のうち、ラテックス凝集法を用いるための試薬または測定系としては、(i)トリプシンの一部をエピトープとして認識して結合可能である少なくとも1種の抗体またはその抗原結合フラグメントを結合させたラテックス粒子を含む抗体感作ラテックス液と、(ii)結合分子(例えば、αアンチトリプシン)の一部をエピトープの一部または全部として認識して結合可能である抗体またはその抗原結合フラグメントを結合させたラテックス粒子を含む抗体感作ラテックス液を含む、試薬など、またはこのような試薬を用いた測定系などが挙げられる。
 上記試薬がラテックス凝集法を用いる試薬(以下、「ラテックス試薬」と称する)である場合、当該分野で既知のラテックスを使用することができる。例えば、ポリスチレンラテックスなどのスチレン系ラテックス、アクリル酸系ラテックス、種々の変性ラテックス(例えば、上記ポリスチレン中にカルボキシル基を導入したカルボン酸変性ラテックス)、着色ラテックス、蛍光ラテックス、などが挙げられる。
 上記ラテックスは、既知の方法で製造することができるが、市販されているものでもよい。また、上記ラテックスは、2種以上を併用してもよい。
 上記ラテックス試薬におけるラテックス粒子の平均粒子径は、特に限定されないが、例えば、測定対象となる抗原物質と抗体との抗原抗体反応により生じた凝集体が肉眼または光学的に検出できる大きさを示すものが挙げられる。具体的には、例えば、100~500nmである。
 また、ラテックス粒子の平均粒子径は、2種以上であってもよい。
 本明細書における「平均粒子径」は、動的光散乱法(Dynamic light scattering)により測定した数値を意味する。
 上記ラテックス試薬は、トリプシンに対する抗体が結合したラテックス粒子を含む。ラテックスに抗体を結合する方法は、特に限定されず、既知の方法が挙げられる。例えば、抗体とラテックスを、pH5.0~10.0の緩衝液中で混合し、20~30℃の条件で2~3時間反応させた後、遠心分離、ブロッキング処理、加熱(エイジング)処理などの既知の後処理を行うことにより、ラテックスと抗体を結合させることができる。
 この際の緩衝液としては、上述した試薬に含まれる緩衝液でよい。
 上記免疫学的測定用試薬または免疫学的測定系を用いるトリプシン濃度の測定は、次の工程:
(i)上記試薬または測定系において、上記検体中に含まれるトリプシンと、該試薬または測定系に含まれる抗体とを結合させること、
(ii)上記工程(i)によって得られた該トリプシンと該抗体との結合体の量または濃度を測定すること
を含む方法により行うことができる。
 上記工程(i)および(ii)は、特に限定されず、使用する試薬または測定系に関する既知のプロトコール、市販品の場合の添付文書などにしたがって適宜行うことができる。
 例えば、上記試薬または測定系としてラテックス凝集法を用いる場合、トリプシンとラテックス粒子に結合した抗体との抗原抗体反応により生じる凝集の度合いを例えば吸光度を用いて測定し、予め求めてあった標準品の検量線から、試料中のトリプシンの濃度を求めることができる。なお、吸光度の測定波長は、通常340~1000nm、好ましくは500~900nmが挙げられる。また、凝集の度合いは、吸光度に限定されるものではなく、既知の方法を用いることができ、例えばネフェロメトリー、カウンティングイムノアッセイなどの方法により測定することもできる。
 上記本発明の方法の一態様における工程(2)は、上記工程(1)で得られたトリプシン濃度の測定値に基づく急性膵炎の診断に必要なデータを提供し、トリプシン濃度が一定値以上の場合に急性膵炎の治療を推奨することを含む。
 トリプシンの血中濃度は、急性膵炎の早期に顕著に上昇するので、急性膵炎の早期予測因子(膵炎マーカー)として用いることができる。すなわち、トリプシンの濃度は、急性膵炎の発症の起点、急性膵炎が発症するか否か、急性膵炎の重症化が進行するか否かを判定(判断)するための指標として用いることができる。
 例えば、複数の患者から得られたデータを統計処理してカットオフ値を決定し、このカットオフ値に基づいて急性膵炎が発症するか否か、または急性膵炎の重症化が進行するか否かを予測することができる。
 例えば、所定のカットオフ値以上である場合、急性膵炎が発症すると予測することができ、当該カットオフ値未満である場合、急性膵炎は発症しないと予測することができる。
 同様に、所定のカットオフ値以上である場合、急性膵炎の重症化が進行すると予測することができ、当該カットオフ値未満である場合、急性膵炎の重症化は進行しないと予測することができる。
 したがって、トリプシンの濃度は、急性膵炎が発症するか否か、または急性膵炎の重症化が進行するか否かを予測するためのデータとして用いることができる。
 また、トリプシンの濃度に基づいて、急性膵炎の重症度を評価することができる。例えば、複数の患者から得られたデータを統計処理してカットオフ値(病態識別値)を決定し、このカットオフ値に基づいて急性膵炎の重症度を評価することができる。
 例えば、第一のカットオフ値未満を軽症、第一のカットオフ値以上から第二のカットオフ値未満を中等症、第二のカットオフ値以上を重症と評価することができる。
 したがって、トリプシンの濃度は、急性膵炎の重症度の評価、その評価の補助、その評価を行うためのデータとして用いることができる。
 さらに、トリプシンの血中濃度に基づいて、急性膵炎の発症リスクまたは重症化リスクの有無または高低を判定することができる。例えば、複数の患者から得られたデータを統計処理してカットオフ値を決定し、このカットオフ値に基づいて急性膵炎の発症リスクまたは重症化リスクの有無または高低を判定することができる。
 例えば、所定のカットオフ値以上である場合、急性膵炎の発症リスクが存在するまたは高いと判定することができ、逆に当該カットオフ値未満である場合、そのようなリスクが存在しないまたは低いと判定することができる。
 同様に、所定のカットオフ値以上である場合、急性膵炎の重症化リスクが存在するまたは高いと判定することができ、逆に当該カットオフ値未満である場合、そのようなリスクが存在しないまたは低いと判定することができる。
 したがって、上記試薬は、急性膵炎の発症リスクまたは重症化リスクの評価、その評価の補助、その評価を行うためのデータの収集等に用いることができる。
 したがって、急性膵炎の診断に必要なデータには、複数の患者から得られたデータを統計処理して得られた、急性膵炎が発症するか否か、重症化が進行するか否か、重症度、発症リスクまたは重症化リスクの有無または高低などを決定するためのカットオフ値などが含まれる。
 上記工程(2)において、上記のような急性膵炎の診断に必要なデータに基づいて判断し、トリプシン濃度が一定値以上の場合に急性膵炎の治療を推奨する。
 上記急性膵炎の治療を推奨するトリプシン濃度としては、例えば、570ng/mL以上が挙げられる。
 上記急性膵炎の治療としては、特に限定されないが、例えば、輸液投与、薬剤投与、または外科的除去から選択される治療が挙げられる。また、治療の選択は、上記トリプシン濃度、トリプシン濃度以外の診断指標のデータ、急性膵炎の重症度、症状、被験体(患者)の年齢、体重、性別、病歴などに応じて適宜選択することができる。
 トリプシン濃度以外の急性膵炎の診断に必要なデータまたは診断指標としては、例えば、(1) 上腹部の急性腹痛発作と圧痛の有無、(2) 血中または尿中膵酵素(アミラーゼ、リパーゼ等)の上昇の有無、(3) 画像診断(超音波、CT、MRI)で膵に急性膵炎に伴う異常所見の有無、(4) 考えられる原因(アルコール、胆石、内視鏡検査(例えば、ERCP検査)、内視鏡手術、切開手術など)などが挙げられる。なかでも、特に、トリプシン以外の膵酵素(アミラーゼ、リパーゼ等)の濃度が挙げられる。アミラーゼ濃度およびリパーゼ濃度の測定は、特に限定されず、既知の試薬または測定系を用いたものでよい。
 また、当該データまたは診断指標には、複数の患者から得られたデータを統計処理して得られた、急性膵炎が発症するか否か、重症化が進行するか否か、重症度などを決定するための各種のカットオフ値などが含まれる。
 このようなデータまたは指標と、トリプシン濃度を合わせて考慮することで、急性膵炎の重症度の診断・判定をより的確にすることができ、より適切な治療を選択することが可能になる。
 例えば、該検体のトリプシン濃度の測定と同時に該検体のアミラーゼ濃度を測定した場合、トリプシン濃度が570ng/mL以上であり、かつ、アミラーゼ濃度が125U/L以下である場合、急性膵炎であると診断することができ、早期に治療を開始できる。
 また、例えば、該検体のトリプシン濃度の測定と同時に該検体のリパーゼ濃度を測定した場合、トリプシン濃度が570ng/mL以上であり、かつ、リパーゼ濃度が55U/L以上である場合、急性膵炎であると診断することができ、早期に治療を開始できる。
 さらに、例えば、該検体のトリプシン濃度の測定と同時に該検体のアミラーゼ濃度およびリパーゼ濃度を測定した場合、トリプシン濃度が570ng/mL以上であり、アミラーゼ濃度が125U/L以下であり、かつ、リパーゼ濃度が55U/L以上である場合、急性膵炎であると診断することができ、早期に治療を開始できる。
 上記本発明の方法の一態様は、上記工程(2)の後、
(3)該急性膵炎の重症度を判定すること、
(4)該工程(3)の判定の結果、重症急性膵炎であると判定された場合、3時間以内に治療を開始することを推奨すること、
を含んでいてもよい。
 上記工程(3)における急性膵炎の重症度の判定は、上記のように急性膵炎の診断に必要なデータに基づいて行うことができる。例えば、複数の患者から得られたトリプシンの濃度のデータを統計処理してカットオフ値(病態識別値)を決定し、このカットオフ値に基づいて急性膵炎の重症度を評価することができる。具体的には、例えば、第一のカットオフ値未満を軽症、第一のカットオフ値以上から第二のカットオフ値未満を中等症、第二のカットオフ値以上を重症と評価することができる。
 上記工程(4)は、上記工程(3)の判定の結果、重症急性膵炎であると判定された場合、3時間以内に治療を行うことを推奨する工程である。急性膵炎は、早期に重症化することがあり、重症と判定された場合、できるだけ早く治療を開始することが好ましく、遅くても判定から3時間以内、好ましくは2時間以内、特に好ましくは1時間以内に治療を開始することが推奨される。
 以下、参考例、実施例を参照して本発明をさらに具体的に説明するが、本発明はこれらに限定されるものではない。
[参考例1]
<モノクローナル抗体の作製>
 以下の手順にしたがって、抗体A、抗体Bを得た。
(免疫原)
 ヒト精製トリプシン(SCRIPPS社製、Cat. No.T0614、精製度≧95%)1mgを1mM TLCK (Tosyl-L-lysyl-chloromethane hydrochloride、ナカライ、Code 34219-94)を含む生理食塩水溶液5mLに溶解し、37℃にて30分間加温後、生理食塩水溶液にて透析し使用した。
(免疫方法)
 上記免疫原をアジュバント(Complete Freund’s adjuvant:CFA、GIBCO社製)に懸濁し、25μgトリプシン/匹となるようBALB/cマウス(5週齢、雌)に2週間間隔で免疫した。途中で部分採血を行い、抗血清レベルでの力価上昇度を免疫原であるトリプシンとの反応性を指標に確認した。複数回の免疫にて抗体力価が十分上昇していることを確認した上で、上記免疫原の生理食塩水溶液を腹腔内もしくは静脈内投与した。脾臓より抗体産生細胞を採取し、ミエローマ(P3X63Ag8.653、ECACC)と細胞融合した。細胞融合はPEG法で行い、融合細胞を培養プレートに播種した。その後、37℃の炭酸ガスインキュベーター内で培養した。
(スクリーニング)
 融合細胞を分注した各ウェルの培養上清を採取し、抗体力価をトリプシンとの反応性を指標に確認した。そして抗体産生クローンの細胞を継代培養し、限界希釈法によりクローニングを行った。得られた単一コロニー由来の細胞を抗ヒトトリプシンモノクローナル抗体産生ハイブリドーマとした。
(モノクローナル抗体調製)
 次に得られたモノクローナル抗体産生ハイブリドーマを大量培養した。予め腹腔内にプリスタン(Sigma-Aldrich社製)を投与し、BALB/cマウスに対し、ハイブリドーマを投与した。10~25日間飼育し、腹水の貯留を待った後、腹水を採取し、得られた腹水抗体を硫安塩析、アフィニティー精製を行うことによりマウス抗ヒトトリプシンモノクローナル抗体を取得した。
(モノクローナル抗体の組合せ予備検討)
 上記で得られた複数のモノクローナル抗体について、各抗体をラテックスに感作し、ラテックスの凝集のシグナル強度を指標に、シグナルが大きくエピトープが異なると考えられる2種類の抗体A、抗体Bの組合せを得た。
<抗体感作>
 以下の材料および方法にしたがって、抗体A感作ラテックスα液、抗体B感作ラテックスβ液を調製した。
(材料)
・ラテックス液:ラテックス(JSR製、商品名:IMMUNTEX)、平均粒子径351 nm、固形分:10%
・抗体A溶液:4.9mg/mL 抗体A、40mM NaCl、20mM HEPES(pH7.4)
・抗体B溶液:4.5mg/mL 抗体B、40mM NaCl、20mM HEPES(pH7.4)
・MES緩衝液:1%BSA、20mM MES(pH6.0)、0.05%NaN
・HEPES緩衝液:2%BSA、20mM HEPES(pH7.0)、150mM NaCl 0.05%NaN
(方法)
 ラテックス液(10%w/vol)500μLと抗体A溶液(4.9mg/mL)500μLを混和し、20℃で90分間振とう撹拌した。また、別途、同様に調製した混合液に、抗体B溶液(4.5mg/mL)550μLを追加し、20℃で90分間振とう撹拌した。
 得られた混合液を、遠心分離機(ローター:R15A(HI抗体ACH))を用いて、10000rpmの条件で、10℃で30分間遠心分離した。上清をデカンテーションし、得られた沈査にMES緩衝液25mLを添加し、ソニケーションにて分散した。その後、37℃で60分間振とう撹拌した。
 得られた分散液を、同遠心分離機を用いて、10000rpmの条件で、10℃で30分間遠心分離した。上清をデカンテーションし、得られた沈査にHEPES緩衝液50mLを添加し、ソニケーションにて分散させ、各抗体感作ラテックスα、β液を得た。
<測定用試薬>
 下記の組成を有する試薬Aおよび試薬Bを調製した。試薬Aは、各構成成分を所定の量または濃度になるように混合することにより調製した。試薬Bは、各抗体感作ラテックスα、β液を等量の割合で混合し、さらに他の構成成分を所定の量または濃度になるように混合して調製した。
(1)試薬A

[実施例1]
 内視鏡的胆管膵管造影(endoscopic retrograde cholangiopancreatography;ERCP)検査を受ける患者(54例:男性26例、女性28例)から、検査前、検査直後、検査後2時間後、検査後24時間後に採取した血清を、上記参考例1で構築した血清トリプシン測定試薬を用い、血清中のトリプシン濃度を測定した。また、同様に同検体群の血中リパーゼ濃度、および血中アミラーゼ濃度を測定した。
 測定可能だった検体群(41例)の内、ERCP後膵炎診断ガイドラインに従い検査を受けた翌日に急性膵炎発症と診断された患者群(膵炎患者群8例:男性4例、女性4例)と、発症なしと診断された患者群(無群33例:男性16例、女性17例)の各測定値を比較した。
 トリプシン測定値は、無群では大きな変化はないものの、膵炎患者群では、検査前と比べ、検査直後および検査2時間後には8倍以上、検査24時間後には、16倍以上に上昇した。リパーゼおよびアミラーゼも同様に上昇が見られた(表1)。

 トリプシン測定値を、基準値上限(570ng/mL)との比率で比較したところ、膵炎患者群では検査前と比べ、検査直後に約11.7倍、検査2時間後には約11.9倍、検査24時間後には約23倍に上昇した。リパーゼおよびアミラーゼも同様に基準値上限(リパーゼ55U/L、アミラーゼ125U/L)との比率で比較したところ、検査直後のリパーゼとアミラーゼはそれぞれ約8.7倍と約1.6倍で、検査後2時間後のリパーゼとアミラーゼはそれぞれ約13.6倍と約2.5倍で、検査後24時間後のリパーゼとアミラーゼはそれぞれ約29.4倍と約6.6倍であった(図1)。
 無群を陰性、膵炎患者群を陽性として検査直後と検査後2時間後の各マーカーのROC曲線を作成しAUCを算出したところ、検査直後でトリプシン0.84、リパーゼ0.82、アミラーゼ0.65となり(図2)、検査後2時間後でトリプシン0.93、リパーゼ0.89、アミラーゼ0.79という結果になった(図3)。それぞれのカットオフは、検査直後でトリプシン1487.1ng/mL、リパーゼ115.5U/L、アミラーゼ136.6U/Lで、検査後2時間後でトリプシン1701.3ng/mL、リパーゼ165.6U/L、アミラーゼ159.6U/Lであった。
 トリプシンは膵炎発症患者の血中で最も早く高値になり、早期の膵炎の診断補助として高い精度を示した。

Claims (9)

  1.  急性膵炎の治療に必要なデータを提供する方法であって、
    (1)急性膵炎が疑われる被験体の検体中のトリプシン濃度を測定すること、
    (2)該測定値に基づく急性膵炎の診断に必要なデータを提供し、トリプシン濃度が一定値以上の場合に急性膵炎の治療を推奨すること、
    を含む、方法。
  2.  前記トリプシン濃度が570ng/mL以上の場合に、急性膵炎の治療を推奨する、請求項1に記載の方法。
  3.  該検体のトリプシン濃度の測定と同時に該検体のアミラーゼ濃度を測定した場合、該アミラーゼ濃度が125U/L以下である、請求項2に記載の方法。
  4.  該検体のトリプシン濃度の測定と同時に該検体のリパーゼ濃度を測定した場合、該リパーゼ濃度が55U/L以上である、請求項2または3に記載の方法。
  5.  前記急性膵炎の治療が、輸液投与、薬剤投与、または外科的除去から選択される、請求項1~4のいずれかに記載の方法。
  6.  前記工程(2)の後、さらに、
    (3)該急性膵炎の重症度を判定すること、
    (4)該工程(3)の判定の結果、重症急性膵炎であると判定された場合、3時間以内に治療を開始することを推奨すること、
    を含む、請求項1~5のいずれかに記載の方法。
  7.  前記被験体が、内視鏡検査、内視鏡手術または切開手術が行われた被験体である、請求項1~6のいずれかに記載の方法。
  8.  前記トリプシン濃度の測定を、免疫学的測定用試薬または免疫学的測定系を用いて行う、請求項1~7のいずれかに記載の方法。
  9.  前記免疫学的測定用試薬または免疫学的測定系が、ラテックス凝集法、ELISA、RIAまたは化学発光免疫測定法を用いるための試薬または測定系である、請求項8に記載の方法。
