Definitions

  • This disclosure relates to a drug used to improve cancer treatment response in cancer immunotherapy using a PD-1 inhibitor or a PD-L1 inhibitor.
  • Cancer immunotherapy using PD-1 (Programmed cell death-1) inhibitors or PD-L1 (Programmed cell death 1-Ligand 1) inhibitors has been reported to be effective against various types of cancer, including malignant melanoma, lung cancer, and gastric cancer, and has become a new trend in cancer treatment.
  • PD-1/PD-L1 inhibitors cancer immunotherapy using a single agent of PD-1/PD-L1 inhibitors has the disadvantage that only about 30-40% of patients respond to treatment, and there is a risk of immune-related adverse events that are different from conventional cancer treatments. Therefore, various studies are being conducted to improve the treatment response and reduce immune-related adverse events in cancer immunotherapy using PD-1/PD-L1 inhibitors.
  • Non-Patent Document 1 reports that aged cancer-bearing mice have a lower response to treatment with PD-1/PD-L1 inhibitor therapy than young mice, and measures to improve this are being sought.
  • Non-Patent Document 1 reports that when PD-1 inhibitor therapy was administered to aged cancer-bearing mice, organ damage occurred that was not observed in young cancer-bearing mice.
  • Non-Patent Document 2 reports that in cancer immunotherapy using PD-1/PD-L1 inhibitors, blocking IL-6 signals can improve therapeutic response.
  • the purpose of this disclosure is to provide a drug that enhances the therapeutic effect of cancer immunotherapy using PD-1/PD-L1 inhibitors.
  • Non-Patent Document 5 In light of this background technology, the present inventor conducted studies to develop a new cancer immunotherapy that takes into account environmental factors specific to elderly people, and found that administration of PD-1/PD-L1 inhibitors to elderly individuals causes an increase in serum IL-6 concentrations and an excessive inflammatory response.
  • the present inventor believes that one of the factors that reduces the therapeutic response to cancer immunotherapy using PD-1/PD-L1 inhibitors is the excessive inflammatory response associated with aging, and hypothesized that a drug that can control the excessive inflammatory response associated with aging would lead to improved therapeutic response to cancer immunotherapy using PD-1/PD-L1 inhibitors.
  • the inventors therefore conducted further intensive research and discovered a fatty acid amide hydrolase (FAAH) inhibitor as a drug that exhibits senescent macrophage-specific IL-6 production inhibitory activity. Furthermore, the inventors found that the combination of a FAAH inhibitor with cancer immunotherapy using a PD-1/PD-L1 inhibitor exhibits excellent therapeutic effects even in individuals (especially elderly individuals) that do not respond to cancer immunotherapy using a PD-1/PD-L1 inhibitor alone. They also found that combination therapy of a PD-1/PD-L1 inhibitor and a FAAH inhibitor exhibits excellent therapeutic effects even in cancers that are resistant to treatment. The present disclosure was completed through further research based on these findings.

Description

Lancet Oncol., 2021, Vol.22 No. 9, pp.1265-1274, doi: 10.1016/S1470-2045(21)00333-8. Hirotake Tsukamoto et al., Nature Communications, 2015, Vol.6, doi: 10.1038/ncomms7702.

若いマクロファージと老化マクロファージを2×104cells/wellの濃度で96ウェルプレートに播種し、FAAH阻害薬として、FAAHi1(URB597, Selleck chemicals)、FAAHi2(PF-04457845, Selleck chemicals)、又はFAAHi3(JNJ-42165279, MedChemExpress)を各ウェルに0~20μMとなるように添加して12時間処理した後、R848 (100 ng/ml)にて刺激し、更に18時間培養した。
 マウスマクロファージ細胞株Raw264.7を1×105cells/wellの濃度で24ウェルプレートに播種し、Faah遺伝子に対するsiRNA(Faah1及びFaah2, Thermo Fisher Scientific)又はコントロールsiRNA(Thermo Fisher Scientific)をTransIT TKO transfection reagent (Mirus Bio)を用いて遺伝子導入した。
