Abstract
본 발명은 장질환 또는 영양실조의 예방, 개선 또는 치료용 조성물에 관한 것으로, 본 발명에 따르면, 상기 조성물을 처리하여 장 누수 현상을 완화시키고, 체중, 소장 융모 길이, 소장움 깊이 및 소장 융모의 고블릿 세포와 증식 세포의 개수를 개선시킬 수 있으므로, 본 발명의 조성물을 장질환 또는 영양실조의 예방, 개선 또는 치료에 유용하게 활용할 수 있다.

본 발명은 장질환의 예방, 개선 또는 치료용 조성물에 관한 것이다.
영양실조는 섭취하거나 흡수하는 에너지와 소비하는 에너지 사이에 불균형이 있을 때 발생한다. 영양실조는 전 세계적으로 중요한 건강 문제로, 5세 미만 어린이 사망의 5분의 1을 차지한다. 유아기의 영양실조는 장내 미생물 구성의 상당한 변화와 함께 어린 시절의 성장 발육부진 및 인지 발달 감소를 초래한다.
이러한 영양실조는 장의 만성 염증성 질환에 의해 야기될 수 있다. 열대성 장병증 또는 환경성 장 기능 장애라고도 하는 환경성 장병증(environmental enteropathy)은 개발 도상국 등 자원이 부족한 환경에 사는 어린이들에게 일반적으로 나타나는 만성 장 염증 장애이다. 환경성 장병증의 원인은 다양하다, 일반적으로 위생이 열악한 지역에서 오염된 음식과 물에 노출됨으로써 발병할 수 있으며, 이는 미생물에의 노출 또는 미생물군의 변화가 환경성 장병증 발병의 주요 원인이 될 수 있음을 시사한다. 아동기 영양실조의 주요 원인으로 널리 인식되고 있는 환경성 장병증은 소장의 기능에 영향을 미치며, 만성 장 염증, 융모 둔화 및 장 투과성 증가를 유발하고, 궁극적으로 영양소 흡수를 감소시켜 성장 및 발달에 악영향을 미친다.
또한, 외부 세균이나 바이러스가 내부로 유입될 수 없도록 하기 위해 장관 점막세포가 세포 사이의 간격을 일정하고 치밀하게 유지하고 있으나, 이에 손상이 가해지면 세포 사이의 간격이 벌어지면서 장관의 투과성이 증가하게 된다. 즉, 장내 세균 균형이 무너져 장 점막에 염증이 생기게 되면 이러한 결합 조직이 약해져 외부의 유해 물질이 체내로 유입되거나, 반대로 체내 물질이 새는 현상이 벌어지는데, 이를 '장누수 증후군'이라고 한다. 장누수 증후군이 심해질 경우 크론병으로 발전하기도 하며, 이는 영양분이 흡수되는 부위인 소장을 침범하는 경우가 많아 영양 결핍으로 이어질 수 있다.
이렇듯, 장에서의 염증으로 인해 장내 미생물 변화나 불균형이 생기면, 이는 곧 영양 결핍을 초래할 수 있으며, 결국 영양실조, 빈혈, 성장 둔화, 발달 지연, 뇌 발달 장애 등의 다른 질환으로 이어질 수 있다. 따라서, 이의 치료를 위해 단순히 영양성분만 제공하여 증상을 개선하기 어렵고, 장 질환의 치료를 통해 근본적으로 장내 조직과 장내 환경을 개선함으로써 장 질환의 치료하거나 증상을 개선하는 것이 필요하다.

따라서 본 발명의 일 측면에서, 바실러스 서브틸리스, 바실러스 코아귤런스, 및 클로스트리듐 부티리큠으로 이루어진 군에서 선택되는 적어도 1종을 유효성분으로 포함하는, 장질환 또는 영양실조의 치료 또는 예방용 약학 조성물을 제공한다.
본 발명의 또 다른 일 측면예에서, 바실러스 서브틸리스, 바실러스 코아귤런스, 및 클로스트리듐 부티리큠으로 이루어진 군에서 선택되는 적어도 1종을 유효성분으로 포함하는, 장질환 또는 영양실조의 예방 또는 개선용 식품 조성물을 제공한다.

이하, 본 발명을 상세하게 설명한다.
본 발명자들은 유아 분변, 김치, 치즈로부터 분리된 여러 가지 균주들을 대상으로 여러 가지 측면에서 각 균주 개별 효능을 스크리닝 하였으며 또한 여러 균주들을 다양한 조합으로 이용하여 여러 가지 측면에서 효능 평가를 수행하였다. 그 결과 본 발명자들은 바실러스 서브틸리스 (Bacillus subtilis), 바실러스 코아귤런스 (Bacillus coagulans), 클로스트리듐 부티리큠 (Clostridium butyricum)의 3종 균주 중 적어도 하나 또는 이들의 조합을 처리할 시에 장 누수 완화나 체중 등의 개선에 있어 예기치 못한 상승효과를 확인하였으며, 뿐만 아니라 장질환 또는 영양실조에 대해 약리적 측면에서 실질적인 치료 효능이 현저함을 확인하였다.

본 발명의 식품 조성물은 식품 제조시에 통상적으로 첨가되는 성분을 포함할 수 있으며, 예를 들어, 단백질, 탄수화물, 지방, 영양소, 조미제 및 향미제를 포함한다. 상술한 탄수화물의 예는 모노사카라이드, 예를 들어, 포도당, 과당 등; 디사카라이드, 예를 들어 말토스, 슈크로스, 올리고당 등; 및 폴리사카라이드, 예를 들어 덱스트린, 사이클로덱스트린 등과 같은 통상적인 당 및 자일리톨, 소르비톨, 에리트리톨 등의 당알콜이다. 향미제로서 천연 향미제 (타우마틴, 스테비아추출물 (예를 들어 레바우디오시드A, 글리시르히진 등)) 및 합성향미제(사카린, 아스파르탐 등)를 사용할 수 있다. 또한, 비타민 A, 비타민 B1, 비타민 B2, 비타민 B3, 비타민 B6, 비타민 B12, 엽산 (folic acid), 비타민 C, 비타민 D3, 비타민 E 등의 비타민을 포함할 수 있다. 예컨대, 본 발명의 식품 조성물이 드링크제와 음료류로 제조되는 경우에는 구연산, 액상과당, 설탕, 포도당, 초산, 사과산, 과즙, 및 각종 식물 추출액 등을 추가로 포함시킬 수 있다.
본 발명의 조성물에서, 각 균주의 유효량의 콜로니 형성 단위(CFU)는 당해 기술 분야의 당업자가 목적하는 바(질병 예방, 건강 또는 치료적 처치) 또는 필요에 의해 결정될 수 있으며, 또한 최종 제형에 따라 결정될 수 있다.
예를 들어, 본 발명의 상기 3종 균주 각각의 생균 또는 사균체를 식품 또는 약품의 제조 시에 전체 조성물 1g(또는 ㎖) 당 균을 1 x 106 내지 1 x 1012개(균수 또는 CFU) 첨가할 수 있고, 바람직하게는 1 x 107 내지 1 x 1010개의 양으로 첨가할 수 있으나, 이에 제한되지 않는다.
또한, 상기한 생균 또는 이의 사균체가 포함된 조성물의 1일 섭취량은 총 균체수 기준으로 1 x 106 내지 1 x 1012 CFU/kg, 바람직하게는 일례로 1 x 107 내지 1 x 1010 CFU/kg일 수 있고, 섭취는 하루에 한 번 섭취할 수도 있고, 수회 나누어 섭취할 수도 있으나, 이에 제한되는 것은 아니다.
본 발명의 조성물은 바실러스 서브틸리스 (Bacillus subtilis), 바실러스 코아귤런스 (Bacillus coagulans), 클로스트리듐 부티리큠 (Clostridium butyricum) 3종 중 적어도 하나 이상의 균주를 총 조성물 100 중량부를 기준으로 1 내지 10 중량부로 포함하는 것일 수 있다. 예컨대, 총 조성물 100 중량부를 기준으로 상기 균주의 총 함량은 1 내지 5 중량부, 1 내지 3 중량부, 또는 2 내지 3 중량부일 수 있다.
본 발명의 조성물은 상기 3종 균주 외 추가의 유산균을 더 포함할 수 있다. 일 예로, 상기 추가의 유산균은 락토바실러스 존소니 (Lactobacillus johnsonii), 락토바실러스 플란타룸 (Lactobacillus plantarum), 비피도박테리움 아니말리스 락티스 (Bifidobacterium animalis subsp. lactis), 락토바실러스 람노서스 (Lactobacillus rhamnosus), 및 비피도박테리움 브레브 (Bifidobacterium breve)로 이루어진 군에서 선택되는 적어도 1종 이상일 수 있다. 상기 추가의 유산균은 총 조성물 100 중량부를 기준으로 0.1 내지 5.0 중량부로 포함하는 것일 수 있다. 예컨대, 상기 추가의 유산균은 총 조성물 100 중량부를 기준으로 0.1 내지 3.0 중량부, 0.1 내지 1.0 중량부, 또는 0.1 내지 0.5 중량부로 포함될 수 있다.
본 발명의 이점 및 특징, 그리고 그것들을 달성하는 방법은 상세하게 후술되어 있는 실시예들을 참조하면 명확해질 것이다. 그러나 본 발명은 이하에서 개시되는 실시예들에 한정되는 것이 아니라 서로 다른 다양한 형태로 구현될 것이며, 단지 본 실시예들은 본 발명의 개시가 완전하도록 하고, 본 발명이 속하는 기술 분야에서 통상의 지식을 가진 자에게 발명의 범주를 완전하게 알려주기 위해 제공되는 것이며, 본 발명은 청구항의 범주에 의해 정의될 뿐이다.
[실시예]
실험방법
1) 3종 복합 균주(Forbaby) 제제의 제조
실험에 사용된 본 발명의 3종 복합 균주 제제는 바실러스 서브틸리스 IDCC 1101, 바실러스 코아귤런스 IDCC 1201, 및 클로스트리듐 부티리큠 IDCC 1301의 세 가지 생균 균주를 혼합하여 제조되었다. 또한, 상기 3종의 복합 균주에 추가 유산균으로 락토바실러스 존소니 IDCC 9203, 락토바실러스 플란타룸 IDCC 3501, 비피도박테리움 아니말리스 락티스 IDCC 4301, 락토바실러스 람노서스 IDCC 3201, 및 비피도박테리움 브레브 IDCC 4401 균주를 혼합하였다. 상기 제제의 구체적인 성분 및 이의 함량은 표 1에 나타내었다.

2) 대조군(placebo) 제제의 제조
상기 3종 복합 균주 제제의 효과를 확인하기 위해, 3종 복합 균주, 추가 유산균 및 아미노산을 포함하지 않는 제제를 제조하였다. 상기 제제의 구체적인 성분 및 이의 함량은 표 2에 나타내었다.

3) 마우스의 환경성 장병증(environmental enteropathy, EE) 모델
마우스 EE 모델은 Brown, E., Wlodarska, M., Willing, B. et al. Diet and specific microbial exposure trigger features of environmental enteropathy in a novel murine model. Nat Commun 6, 7806 (2015) (https://doi.org/10.1038/ncomms8806)의 내용을 참고하여 제작되었다.
이유 분리가 끝난 3주 된 어린 마우스(C57BL/6 black mouse)를 한 그룹 당 4~5 마리의 마우스로 하여 정상 대조군(normal), 미처리군(EE), 대조군(EE+placebo) 및 실험군(EE+Forbaby)을 준비하였다. 정상 대조군을 제외한, 각 그룹에 4주 동안 영양성분이 부족한 사료를 급여하면서 영양부족을 유발하고, 사료 급여가 시작된 3주차에 감염균을 3번 경구로 투여하여 감염을 유발하여 환경성 장병증 모델을 준비하였다. 관찰 4주차에 마우스를 희생하여 효과를 확인하였다.
구체적으로, 정상 대조군에는 정상 사료(단백질 28%, 지방 22%)를 4주 동안 급여하고, 미처리군, 대조군, 및 실험군에는 각각 영양부족 사료(단백질 9%, 지방 13%)를 4주 동안 급여하였다. 그 중 대조군에는 2)의 대조군 제제(플라시보 제제)를 함께 급여하고, 실험군에는 1)의 3종 복합 균주 제제(Forbaby 제제)를 함께 급여하였다. 구체적으로 24시간 마다 음수 50ml에 3종 복합 균주 제제 또는 대조군 제제를 각각 3000mg씩 섞어 케이지 마다(마우스 5마리 기준) 제공하였다.
한편, 감염균은 사람에게서 분리된 총 7가지의 균을 사용하였고, 구체적으로 Bacteroides vulgatus 3/1/40A, Bacteroides fragilis 3/1/12, Bacteroides ovatus 3/8/47, Bacteroides dorei 5/1/36 (D4), Parabacteroides distasonis 2/1/33B, Escherichia coli 3/2/53, Escherichia coli 4/1/47을 사용하였다. 각각의 균은 모두 FAA(fastidious anaerobe)배지에서 혐기 조건으로 배양되었다. 접종 하루 뒤 각각의 균을 PBS에 109 cells /ml로 만들어서 동량을 섞고, 각각의 마우스에 100μl씩 gavage를 통하여 위에 주입하였다.
이러한 감염은 영양부족 사료 급여 후 16일, 18일, 20일째에 이루어졌고, 감염 후 8일 후인 28일째에 마우스를 희생하여 샘플링 하였다.
4) 마우스 장 샘플링 및 소장 조직 관찰을 위한 파라핀 블록 제작
마우스의 환경성 장병증 모델에서, 관찰 4주째에 마우스를 해부하여 장 샘플을 획득하였다. 마우스를 마취한 뒤 장을 적출하여 스위스 롤 기술(Swiss-rolling technique)을 이용하여 말아서 4% 파라포름알데히드 용액에 24시간 이상 보관한다. 이후, 파라핀 블록을 제작하여 소장의 융모 길이, 소장움의 깊이, 소장 융모에 존재하는 고블릿 세포(goblet cell)의 개수, 그리고 소장 융모의 증식 세포(Proliferating Cell) 개수를 확인하였다.
상기에서 고정된 조직을 물에 세수하여 고정 시약을 제거하였다. 조직의 수분을 제거하기 위해서 조직을 자동조직처리기에 넣어 탈수와 투명화 과정이 진행되도록 처리하였다 (표 3).

수분과는 혼합되지 않는 파라핀을 조직으로 침투시키기 위해서 조직을 알코올을 이용하여 탈수하였다. 탈수는 상기 표 3과 같이, 저농도의 알코올 용액에서 고농도의 알코올 용액으로 점진적으로 처리한 후 순수한 100% 알코올과 자일렌으로 처리하는 과정에서 이루어졌다. 유기용매인 자일렌에 용해시킨 파라핀을 탈수한 조직으로 침투시킨 다음 고온(60℃)에서 액체상태인 순수한 파라핀으로 처리하여 완전히 침투시켰다.몰드에 들어있는 조직은 파라핀을 식혀 고체상태로 만든 후 적당한 크기로 잘라서 다듬었다. 이후, 박절기에 적당한 두께(보통 5μm)로 절단하였다. 알코올 10% 용액이 담긴 55℃의 수조(water bath)에 절단된 파라핀 절편을 띄워 놓은 후 파라핀이 살짝 녹으면 슬라이드로 떠서 수분을 말렸다. 슬라이드를 상온에서 하루 건조시킨 다음 조직의 파라핀을 제거하였다.

준비된 슬라이드는 헤마톡실린 & 에오신 염색을 하였다. 슬라이드를 상기 표 4의 과정과 반대로 진행하여 탈수한 후 마운팅 용액(mounting solution)을 사용하여 커버 슬라이드를 덮고, 현미경을 사용하여 관찰하였다.
실험예 1: 3종 복합 균주 제제의 체중 개선 효과 확인
실험방법 3)의 기재에 따라 한 그룹 당 4~5 마리의 마우스로 구성된 정상 대조군 및 실험군을 준비하여, 각각에 3종 복합 균주 제제 및 대조군 제제를 처리하였다. 먹이는 매일 음수에 섞어 공급하였고 2일 마다 체중의 지표를 추적하였다. 3주령의 마우스의 처음 몸무게를 기준으로 하여 1달 뒤의 최종 무게를 비율로 나타내었다.
그 결과, 미처리군(EE)에 비하여 3종 복합 균주 제제를 처리한 군에서 유의하게 체중이 증가됨을 확인하였으며, 특히 대조군(placebo) 제제를 처리한 경우에 비해 3종 복합 균주 제제를 처리한 군에서 체중이 유의하게 증가됨을 확인하였다 (도 1, 도 2).
실험예 2: 3종 복합 균주 제제의 장누수 개선 효과 확인
경구로 투여된 FITC particle이 장 누수 정도에 따라서 얼마나 혈액으로 새어 나오는지를 관찰하여 동물 모델의 장 누수 현상의 정도를 확인하는 실험을 진행하였다.
실험방법의 3)에 기재된 마우스 모델과 동일한 방법으로 한 그룹 당 4~5 마리의 마우스로 구성된 정상 대조군(normal), 미처리군(EE), 대조군(EE+placebo) 및 실험군(EE+Forbaby)을 준비하고, 각각 사료와 3종 복합 균주 제제 및 대조군 제제를 처리하였다. 관찰 4주째 마우스를 해부하여 장 누수(FITC) 지표를 확인하였다.
장 누수를 측정하기 위해 형광을 띄는 물질인 FITC-dextran (Fluorescein isothiocyanate-dextran, sigma #FD4)를 이용하여 각각의 마우스에 경구로 동량의 FITC-dextran을 투여하였다. 투여 4시간 후 혈액을 채취하여 혈장 시료를 획득한 뒤 샘플 내 FITC 형광을 측정하였다.
그 결과, 미처리군에서 장 누수 현상이 촉진되어 FITC의 양이 증가한 것이 관찰되었다. 이에 반해, 영양실조가 있는 실험군에 3종 복합 균주 제제를 처리한 경우, 미처리군 및 대조군에 비해 FITC의 양이 감소하여 장 누수 현상이 개선되는 것으로 관찰되었다 (도 3).
실험예 3: 3종 복합 균주 제제의 소장 기능 개선 효과 확인
3종 복합 균주 제제의 소장 기능 개선 효과를 확인하기 위해, 소장의 융모 길이, 소장움 깊이, 소장 융모에 존재하는 고블릿 세포의 개수, 및 소장 융모의 증식 세포 개수를 확인하였다.
실험방법의 3)에 기재된 마우스 모델과 동일한 방법으로 한 그룹 당 4~5 마리의 마우스로 구성된 정상 대조군(normal), 미처리군(EE), 대조군(EE+placebo) 및 실험군(EE+Forbaby)을 준비하고, 각각 사료와 3종 복합 균주 제제 및 대조군 제제를 처리하였다. 관찰 4주째 마우스를 해부하고 장 샘플을 획득하였으며, 실험방법 5)에서 제작한 파라핀 블록을 이용하여 각 지표를 확인하였다.
그 결과, 미처리군(EE)에 비하여 유산균 제제를 처리한 군에서 유의하게 소장 융모 길이가 증가함을 확인하였고, 특히 대조군(placebo) 제제를 처리한 경우에 비해 3종 복합 균주 제제를 처리한 군에서 소장 융모 길이가 유의하게 증가됨을 확인하였다 (도 4).
또한, 미처리군에 비해 유산균 제제를 처리한 군에서 유의하게 소장움의 깊이가 증가함을 확인하였으며, 특히 대조군 제제를 처리한 경우에 비해 3종 복합 균주 제제를 처리한 군에서 소장움 깊이가 유의하게 증가됨을 확인하였다 (도 5).
미처리군에 비해 유산균 제제를 처리한 군에서 유의하게 소장 융모에 존재하는 고블릿 세포의 개수 및 증식 세포의 개수가 증가함을 확인하였으며, 특히 대조군 제제를 처리한 경우에 비해 3종 복합 균주 제제를 처리한 군에서 세포의 개수가 유의하게 증가됨을 확인하였다 (도 6, 도 7).

- 바실러스 서브틸리스, 바실러스 코아귤런스, 및 클로스트리듐 부티리큠으로 이루어진 군에서 선택되는 적어도 1종을 유효성분으로 포함하는,장질환 또는 영양실조의 예방 또는 개선용 식품 조성물.
- 제1항에 있어서,바실러스 서브틸리스, 바실러스 코아귤런스, 및 클로스트리듐 부티리큠을 유효성분으로 포함하는,장질환 또는 영양실조의 예방 또는 개선용 식품 조성물.
- 제1항에 있어서,락토바실러스 존소니, 락토바실러스 플란타룸, 비피도박테리움 아니말리스 락티스, 락토바실러스 람노서스, 및 비피도박테리움 브레브로 이루어진 군에서 선택되는 적어도 1종 이상을 더 포함하는,장질환 또는 영양실조의 예방 또는 개선용 식품 조성물.
- 제1항 내지 제3항 중 어느 한 항에 있어서,아미노산을 더 포함하는,장질환 또는 영양실조의 예방 또는 개선용 식품 조성물.
