Abstract
虚血性疾患に対する効果的な治療効果を有する細胞製剤を提供する。CD18陽性細胞を有する細胞製剤である。この細胞製剤は、造血幹細胞を含む試料からバフィーコート層を取得し、バフィーコート層から治療効果を阻害する阻害細胞を除去することで単核球細胞を取得し、単核球細胞からCD34陽性であるCD34+単核球細胞を取得し、CD34陽性単核球細胞からCD18を高発現しているCD34+CD18high単核球細胞を取得することで製造される。
Description
本発明は、組織新生を促進することにより治療が期待できる疾患を治療するための細胞製剤等に関する。
虚血性疾患により障害された組織の機能回復には、組織新生が重要であり、近年この機能回復には細胞移植が有効であることが判明している。非特許文献1には、心筋梗塞に対する、比重遠心法で得られた骨髄単核球細胞の投与が、心臓の微小血管網を新生し心機能の向上をもたらすことが記載されている。非特許文献2には、四肢虚血に対する、特異的細胞表面抗原を用いて精製された末梢血由来のCD34陽性細胞の投与が、四肢の微小血管網を新生し虚血症状の改善をもたらすことが記載されている。また非特許文献3には、四肢虚血に対する、比重遠心法で得られた骨髄単核球細胞の投与が、四肢の微小血管網を新生し虚血症状の改善をもたらすことが記載されている。非特許文献4には脳梗塞に対する、特異的細胞表面抗原を用いて精製された臍帯血由来のCD34陽性細胞の投与が、脳の微小血管網を新生し神経機能の向上をもたらすことが記載されている。また非特許文献5には、脳梗塞に対する、比重遠心法で得られた骨髄単核球細胞の投与が、脳の微小血管網を新生し神経機能の向上をもたらすことが記載されている。
一方、自家造血幹細胞を用いた虚血性疾患の治療にはいくつかの問題点も存在する。例えば、急性期の虚血性脳梗塞患者では、血圧、心拍、呼吸状態や各種生理機能を含めた全身状態が不安定である。また、患者の意識レベルが低下していることも多く、通常は骨髄細胞採取に困難を伴う。そのため、麻酔科医や血液内科医の応援が困難な状況や自家骨髄液採取に伴う病状悪化や有害事象発生等のリスクが高い症例では、前記治療は困難である。このように、幹細胞輸注による虚血性疾患治療の普及には、採取法と調製・保存法が確立されている他家造血幹細胞の利用が理想的と考えられる。
もっとも他家造血幹細胞の虚血性疾患治療への応用に際し、効果的な治療効果を示す細胞とそうではない細胞とが混在していることが知られている。非特許文献6には、他家造血幹細胞移植により微小血管網が障害され心筋梗塞後の症状悪化をもたらすことが記載されている。非特許文献7には、他家造血幹細胞移植により微小血管網が障害され脳梗塞後の症状悪化をもたらすことが記載されている。以上より、細胞移植、特に他家細胞移植による治療効果の発現のためには、効果的な治療効果を示す細胞とそうでない細胞とを選別し、有益な細胞の単離あるいは有害な細胞の除去による細胞製剤が必要となる。
また、脳神経分野においても新生の重要性が示唆されている。これまでヒトやげっ歯類の成体の脳では神経細胞は増えることはないと考えられていたが、近年、海馬では絶えず新しいニューロンが生まれることが解った(非特許文献8,9)。
新たに産み出されたニューロンは、神経ネットワークに組み込まれて機能する。海馬におけるニューロン新生(神経新生)は学習や豊かな環境下でその頻度が上昇する。
しかしながら逆に加齢によって低下することが報告されている(非特許文献10,11)。University of Illinois at Chicago, The University of British Columbia, Rush University Medical Centerの研究グループは、海馬におけるニューロン新生が成人でも持続するが老齢者では激減することを報告し、ニューロン新生と認知状態が相関することを示唆した(非特許文献12)。
うつ病や統合失調症等の精神疾患でも、脳内における神経新生が低下していることが報告され、これらの疾患の病態への関与が示唆されている。海馬における神経新生は、うつ病発症の危険因子とされるストレスによって抑制され、逆に抗うつ薬を慢性投与することによって促進される(非特許文献13,14)。
アルツハイマー病では、海馬における神経新生が低下していることが報告され、病態に関与していることが示唆されている。アルツハイマー病に関連が深いアミロイド前駆蛋白遺伝子(APP)の遺伝子改変マウスを用いた検討により、アミロイドβ(Aβ)が海馬の神経新生低下に関与していることが示唆された(非特許文献15)。
従って、海馬における神経新生を促進させることができれば、加齢に伴う神経機能低下に対しても、精神疾患に対しても、そして、認知症に対しても、新しい治療法の可能性が示唆されることになる。

本発明はかかる問題点に鑑みてなされたものであって、組織新生を促進することにより治療が期待できる疾患に対する効果的な治療効果を有する細胞製剤を提供することを目的とする。
本発明にかかる細胞製剤は、CD18陽性細胞を有することを特徴とする。
本発明によれば、虚血性疾患に対して効果的な治療効果を有する細胞製剤が得られる。また、本発明によれば、海馬での神経新生を効果的に促進させることができる。
以下、添付の図面を参照して本発明の実施形態について具体的に説明するが、当該実施形態は本発明の原理の理解を容易にするためのものであり、本発明の範囲は、下記の実施形態に限られるものではなく、当業者が以下の実施形態の構成を適宜置換した他の実施形態も、本発明の範囲に含まれる。
本発明にかかる細胞製剤は、CD18陽性細胞を有することを特徴とする。CD18陽性細胞は幹細胞であることが好ましい。
(1)幹細胞
幹細胞は、様々な細胞への分化能、及び自己複製能を持つ細胞であり、例えば、成体幹細胞及び多能性幹細胞等が挙げられる
成体幹細胞は、成体の各組織中に存在し、最終分化が未完了で、ある程度の多分化能を有する幹細胞であり、例えば、造血幹細胞、間葉系幹細胞、神経幹細胞、腸管上皮幹細胞、毛包幹細胞、色素幹細胞等が挙げられる。
幹細胞は、様々な細胞への分化能、及び自己複製能を持つ細胞であり、例えば、成体幹細胞及び多能性幹細胞等が挙げられる
成体幹細胞は、成体の各組織中に存在し、最終分化が未完了で、ある程度の多分化能を有する幹細胞であり、例えば、造血幹細胞、間葉系幹細胞、神経幹細胞、腸管上皮幹細胞、毛包幹細胞、色素幹細胞等が挙げられる。
多能性幹細胞は、生体を構成する全ての種類の細胞に分化することができる多分化能を有し、適切な条件下のインビトロでの培養において多能性を維持したまま無限に増殖を続けることができる細胞であり、例えば、胚性幹細胞(ES細胞)、胚性生殖幹細胞(EG細胞)、生殖系幹細胞(GS細胞)、人工多能性幹細胞(iPS細胞)等が挙げられる。
本発明にかかる細胞製剤において、幹細胞は、好ましくは、造血幹細胞である。
造血幹細胞は、血球の全ての血液細胞分化系列に分化し得る多分化能を有し、かつその多分化能を維持したまま自己複製することが可能な細胞である。
造血幹細胞は、通常当該細胞に特異的な表面マーカー(造血幹細胞特異的マーカー)の発現を指標として、前記組織から単離される。かかるマーカーの発現に関し、ヒト由来の造血細胞は、通常、CD34陽性で特徴づけられる。より具体的には造血幹細胞は、CD34陽性(+)CD38陰性(-)CD90陽性(+)CD45RA陰性(-)細胞であり、好ましくは、CD34+CD38-CD90+CD45RA-CD49f+細胞である。
造血幹細胞が由来とする組織としては、当該細胞が存在する限り特に制限はないが、造血組織であることが好ましく、例えば、骨髄、臍帯血、末梢血、肝臓等が挙げられる。
CD34陽性細胞は、CD34陽性について富化された細胞を意味し、骨髄、末梢血、胎盤、臍帯血等の試料から、当該分野で周知の方法に従い、例えばCD34陽性細胞を濃縮することにより得ることができる。CD34陽性細胞の濃縮は、例えばCD34固定化ビーズやセルソーターを使用して行うことができる。CD34陽性細胞の好ましい一例はCD34陽性単核球細胞である。
本発明の造血幹細胞は、医療の利便性からは他家細胞であるが、自家細胞を排除するものではない。ある実施形態では、造血幹細胞は、本発明の医薬組成物の他家臍帯血試料から調製され、別な実施形態では、造血幹細胞は、バンク化された細胞集団から調製される。
(2-1)CD18
CD18はβ2インテグリン(integrin)ともいわれる。インテグリンはさまざまな細胞に発現をしている細胞接着に重要な役割を果たす分子である。インテグリンはα鎖とβ鎖からなる2量体の細胞膜貫通型蛋白質であり、18個のα鎖と8個のβ鎖から24種類のインテグリンが形成される。
CD18はβ2インテグリン(integrin)ともいわれる。インテグリンはさまざまな細胞に発現をしている細胞接着に重要な役割を果たす分子である。インテグリンはα鎖とβ鎖からなる2量体の細胞膜貫通型蛋白質であり、18個のα鎖と8個のβ鎖から24種類のインテグリンが形成される。
CD18は白血球インテグリンともよばれ、白血球に特異的に発現する。しかしながら本発明者はCD18が白血球以外の造血幹細胞等でも発現することに着目し、造血幹細胞の細胞接着に重要な役割を果たすことを見いだした。CD18は通常はリガンド結合ができないが、活性化を受けると可逆的な立体的な構造変化を起こしてリガンドに結合できるようになる。
CD18陽性細胞は、例えばフローサイトメトリーによる解析でCD18陽性反応を示す細胞割合が60%以上、好ましくは70%以上、より好ましくは80%以上、更に好ましくは90%以上である細胞集団である。
また例えばCD18陽性細胞は、フローサイトメトリーアッセイにおいて、アイソタイプコントロール抗体を反応させた場合のMIF(平均蛍光強度)の2倍以上、好ましくは3倍以上、より好ましくは4倍以上のCD18抗原のMIFが維持できている細胞集団とすることが可能である。
(2-2)グルコーストランスポーター(Glut)
グルコーストランスポーター(Glut)は、グルコースが生体膜を通過するために必要とされる細胞膜上の糖輸送担体と呼ばれる膜タンパク質である。Glutには、現在13種類(GLUT-1, -2, -3, -4, -5, -6, -7, -8, -9, -10, -11, -12, HMIT)が報告されているが、その発現部位や役割が比較的よく解明されているのはクラスIに属するGlut 1からGlut4の4種類の分子であり、その分子量はいずれも約50kDaで、細胞膜を12回貫通する共通した構造を有している。I型グルコーストランスポーター(Glut1)は脳の血管内から脳へブドウ糖を運ぶ重要な役割をしており、本発明者はGlut1が造血幹細胞の効果発現に深く関係することを見いだした。

コネキシンは、ギャップ結合を構成する結合膜タンパク質ファミリーの総称である。コネキシンは、その分子量の違いによって、12を越えるサブタイプが発見されている。分子量が26kDaのものをコネキシン26、43kDaのものをコネキシン43と称されている。コネキシン43は、コネキシンファミリーの中で、中枢神経や心筋細胞に多く存在しており、その量が減少すると、不整脈などの悪影響を及ぼすと推定されている。本発明者はコネキシン43が造血幹細胞の効果発現に深く関係することを見いだした。

(2-4)Jagged1

即ち、造血幹細胞の血管再生促進メカニズムは、細胞投与直後のシンプルな細胞-細胞相互作用(造血幹細胞と障害を受けた血管内皮細胞との相互作用)であり、移植幹細胞の生着・増殖・分化は不要である。CD18が発現していれば投与された造血幹細胞が血管内皮細胞に効果的に接着する。コネキシン(Connexin)が発現していれば投与された造血幹細胞は血管内皮細胞にギャップ結合により効果的に連結する。グルコーストランスポーター(Glut)が発現していればグルコースを豊富に有する造血幹細胞から血管内皮細胞にギャップ結合を介して効果的にエネルギーが供与される。Jagged1が発現していれば投与された造血幹細胞と血管内皮細胞との細胞間情報伝達が効果的に可能となる。

ゆえに優れた治療効果が期待できる細胞製剤は、CD18陽性細胞を有することを特徴とする細胞製剤である。CD18陽性細胞はグルコーストランスポーターGlut(特に好ましくはGlut1である。)を発現していることが好ましく、CD18陽性細胞はコネキシン(特に好ましくはConnexin43である。)が発現していることが好ましく、CD18陽性細胞はJagged1が発現していることが好ましい。
また後述するように、VEGF取込み促進能の指標としてのグルコーストランスポーター(Glut)の発現強度は、治療効果の高い単核球の選別マーカーとして有用である。従って、優れた治療効果が期待できる細胞製剤は、Glut陽性細胞(好ましくはGlut1陽性細胞である。)を有することを特徴とする細胞製剤である。
また後述するように、VEGF取込み促進能の指標としてのコネキシン(の発現強度は、治療効果の高い単核球の選別マーカーとして有用である。従って、優れた治療効果が期待できる細胞製剤は、コネキシン(Connexin)陽性細胞(好ましくはConnexin43陽性細胞である。)を有することを特徴とする細胞製剤である。
また後述するように、幹細胞におけるJagged1の発現が再生促進作用に重要であることが確認されてる。従って、優れた治療効果が期待できる細胞製剤は、Jagged1陽性細胞を有することを特徴とする細胞製剤である。
(3)細胞製剤
本発明にかかる細胞製剤は組織新生により治療が期待できる疾患の治療及び/又は予防の為のものである。本明細書において「治療」には、症状を治癒すること、症状を改善すること及び症状の進行を抑えることが含まれる。一方「予防」には疾患の発症を抑えること及び遅延させることが含まれ、疾患になる前の予防だけでなく、治療後の疾患の再発に対する予防も含まれる。
本発明にかかる細胞製剤は組織新生により治療が期待できる疾患の治療及び/又は予防の為のものである。本明細書において「治療」には、症状を治癒すること、症状を改善すること及び症状の進行を抑えることが含まれる。一方「予防」には疾患の発症を抑えること及び遅延させることが含まれ、疾患になる前の予防だけでなく、治療後の疾患の再発に対する予防も含まれる。
本発明にかかる細胞製剤は、虚血性疾患も対象としており、対象となる疾患は、例えば脳梗塞、心筋梗塞、四肢虚血、腎梗塞、肺梗塞、脾梗塞、腸管梗塞等である。本発明にかかる細胞製剤は、CD18を高発現する幹細胞を投与することにより、虚血周囲部での微小血管を新生させ、これにより虚血性疾患を解消するものであり、対象となる疾患は実施例記載のものに限定されるものではない。
本発明にかかる細胞製剤を脳梗塞に使用する場合、超急性期、急性期、亜急性期、及び、慢性期の何れにおいても使用可能であるが、急性心筋梗塞や脳梗塞等、急性発症する虚血性疾患に対しては特に急性期及び亜急性期において好適に使用可能である。ここで、超急性期は発症してから12時間以内の期間であり、ステント術や血栓溶解療法、血栓除去等により組織細胞死を阻止できる可能性が高い時期である。急性期は発症してから12時間~7日以内の期間であり、亜急性期は発症してから1週~4週以内の期間である。慢性期は発症してから1か月以上の期間である。一方、慢性循環障害による四肢虚血等、慢性的に経過する虚血性疾患に対しては、時期に関係なく好適に使用可能である。
本発明にかかる細胞製剤は、海馬での神経新生が治療に貢献する疾患も対象としている。即ち、本発明にかかる細胞製剤は、CD18陽性細胞を有することを特徴とし海馬での神経新生を可能とする神経新生促進剤である。
近年では特定の脳領域である海馬においては成熟期においても神経幹・前駆細胞が存在し、それらが増殖、分化することにより神経細胞が新生されることが明らかにされている。新生された神経細胞は、神経ネットワークを形成し、記憶形成に関わる等重要な役割を果たしている。
しかし加齢によって海馬における神経新生が阻害され、高齢者の神経機能の低下の一因となることが示唆されている。
また、ストレスはうつ病等の精神疾患発症の危険因子と考えられているが、動物に拘束ストレスのような物理的・精神的ストレスを負荷した場合だけでなく、resident -intruder stress といった精神的ストレスのみを負荷した場合でも海馬神経幹・前駆細胞が減少することから、海馬神経新生の抑制がうつ病等の精神疾患の病因に関与している可能性が示唆されている。
神経変性疾患に伴う認知症は、海馬における神経新生が低下していることが報告され、これらの疾患の病態への関与が示唆されている。
従って、海馬での神経新生を促進させることにより、加齢による神経機能の低下の予防及び/又は治療が可能となり、また、加齢による神経機能の障害に伴う疾患の予防及び/又は治療が可能となる。更には、神経変性疾患に伴う認知症の予防及び/又は治療が可能となり、また、精神疾患の予防及び/又は治療が可能となる。
海馬は大脳側頭葉の内側部で側脳室下角底部に位置し、エピソード記憶等の顕在性記憶の形成に不可欠な皮質部位である。歯状回は海馬の付属物であるが本明細書では海馬の一部とする。歯状回は三層からなり、中心をなす層として顆粒細胞層(granule cell layer)があり、すぐ上に位置するのが細胞密度の極めて低い分子層(molecularlayer)、そして下に位置する層は細胞がまばらに見える多形細胞層(polymorphic cell layer)である。
CD18陽性細胞はコネキシン43陽性であることが好ましい。成体のあらゆる組織、臓器はその組織としての恒常性を維持するために隣接する細胞間にさまざまな形態の結合を有する。コネキシン43は細胞と細胞とのギャップ結合に重要な分子であり、例えば骨芽細胞、骨細胞、血管内細胞、神経細胞等において発現している。本発明者はコネキシン43陽性細胞が海馬における神経新生(特に歯状回における神経新生)に重要な役割を果たすことを見いだした。コネキシン43陽性細胞とは、コネキシン43を高発現している細胞集団である。コネキシン43を高発現しているとは、特に限定されるものではないが例えば、フローサイトメトリーによる解析でコネキシン43陽性反応を示す細胞割合が60%以上、好ましくは70%以上、より好ましくは80%以上、更に好ましくは90%以上である細胞である。
本発明にかかる細胞製剤の投与経路は、静脈内投与、動脈内投与、門脈内投与、局所組織投与が好適である。投与される細胞数は、特に限定されるものではなく、対象患者の年齢、体重、疾患や症状、投与方法に応じて適宜設定される。例えば、四肢虚血の場合であれば、本発明の医薬組成物は、造血幹細胞として、104個~106個/Kg程度、好ましくは104個/Kg以上、より好ましくは5×104個/Kg以上、さらに好ましくは5×105個/Kg以上、必要に応じて2×106個が投与されるように調製される。脳梗塞の場合であれば、本発明の医薬組成物は、造血幹細胞として、104個~106個/Kg程度、好ましくは104個/Kg以上、より好ましくは5×104個/Kg程度が投与されるように調製される。総細胞数(単核球細胞)は、上記造血幹細胞の数値よりも約100倍多くなる。
本発明においては、更に血管新生促進因子を含有することも可能である。血管新生促進因子は、特に限定されるものではないが、例えば、VEGF、アンジオポエチン(angiopoietin)、PDGF、TGF-β、FGF、PlGF、マトリックスメタロプロテアーゼ、プラスミノーゲンアクチベータ等を使用することができるが、好ましくはangiopoietinである。angiopoietinは、脈管形成(vasculogenesis)あるいは血管新生(angiogenesis)を促進する糖タンパク質の成長因子であり、angiopoietinには、アンジオポエチン1(Ang1)、アンジオポエチン2(Ang2)、アンジオポエチン3(Ang3)、アンジオポエチン4(Ang4)の4種類があるだけでなく、類似のタンパク質として6種類のアンジオポエチン関連タンパク質(ANGPTL:angiopoietin-related protein、または、angiopoietin-like protein)がある。
本発明にかかる細胞製剤を注射用製剤とする場合は、当技術分野で通常使用されている添加剤を適宜用いることができる。添加剤としては、例えば、等張化剤、安定化剤、緩衝剤、保存剤、キレート剤、抗酸化剤、又は溶解補助剤等が挙げられる。等張化剤としては、例えば、ブドウ糖、ソルビトール、マンニトール等の糖類、塩化ナトリウム、グリセリン、プロピレングリコール、ポリエチレングリコール等が挙げられる。安定化剤としては、例えば亜硫酸ナトリウム等が挙げられる。緩衝剤としては、例えば、ホウ酸緩衝剤、リン酸緩衝剤、クエン酸緩衝剤、酒石酸緩衝剤、酢酸緩衝剤等が挙げられる。保存剤としては、例えば、パラオキシ安息香酸エステル、ベンジルアルコール、クロロクレゾール、フェネチルアルコール、塩化ベンゼトニウム等が挙げられる。キレート剤としては、例えば、エデト酸ナトリウム、クエン酸ナトリウム等が挙げられる。抗酸化剤としては、例えば、亜硫酸ナトリウム、亜硫酸水素ナトリウム、アスコルビン酸ナトリウム、チオ硫酸ナトリウム等が挙げられる。溶解補助剤としては、例えば、デキストラン、ポリビニルピロリドン、安息香酸ナトリウム、エチレンジアミン、サリチル酸アミド、ニコチン酸アミド、ポリオキシエチレン硬化ヒマシ油誘導体等が挙げられる。
(4)細胞製剤の製造方法
CD18高発現細胞を有する細胞製剤の場合、特に限定されるものではないが下記方法によって製造される。

例えば、造血幹細胞を含む試料からバフィーコート層を取得する工程と、バフィーコート層から治療効果を阻害する阻害細胞を除去することで単核球細胞を取得する工程と、単核球細胞からCD18を高発現しているCD18high単核球細胞を取得する工程と、を有する。
また例えば、造血幹細胞を含む試料からバフィーコート層を取得する工程と、バフィーコート層から治療効果を阻害する阻害細胞を除去することで単核球細胞を取得する工程と、単核球細胞からCD34陽性であるCD34+単核球細胞を取得する工程と、CD34陽性単核球細胞からCD18を高発現しているCD34+CD18high単核球細胞を取得する工程と、を有する。
また例えば、複数の患者から採取した複数の血液試料からCD18高発現細胞を多く含む試料を選択する工程と、その血液試料から治療効果を阻害する阻害細胞を除去する工程と、を有する。
また例えば、採取した血液試料にTNFα等のサイトカインを作用させることによりCD18高発現細胞を増加させる工程と、そのCD18高発現細胞を増加させ血液試料から治療効果を阻害する阻害細胞を除去する工程と、を有する。
一般に臍帯血や血液、骨髄液は凝固しやすいためヘパリンを抗凝固剤として用いることが多いが、本発明ではヘパリンを使用せず、生理食塩水による希釈および二価イオンのキレート剤で、凝固を抑制した。
造血幹細胞を含む試料として、臍帯血、骨髄液、又は末梢血試料を使用でき、好ましくは臍帯血試料である。
臍帯血試料につき密度勾配遠心分離法によりバフィーコートに集積する単核球細胞を得る。密度勾配遠心分離法には、例えば、市販のフィコール(登録商標)、パーコール(登録商標)等の密度勾配遠心用媒体を用いることが好適である。
バフィーコート中には細胞製剤の治療効果の有効性を阻害する阻害細胞が包含される場合がある。そのためバフィーコート中の阻害細胞を除去することが望ましい。バフィーコート中の阻害細胞は好中球や死細胞である。
バフィーコート中の阻害細胞を除去するためには、バフィーコート層にバフィーコート層と等量のPBSを添加し、遠心分離をすることで阻害細胞を分離する。阻害細胞を分離するための遠心力は、例えば、上限は、1000g、900g、800g、700g、600gであり、下限は500g、400gであり、例えば400~1000g、400~900g、400~800g、400~700g、400~600g、の範囲である。好ましくは400~800g、より好ましくは500~800gである。
遠心分離したバフィーコート層の沈殿は、上層に好中球や死細胞が濃縮された分画、下層に単核球が濃縮された分画となる。遠心分離後に上清を除去する時に、容器を徐々に水平にしていくことで、沈殿上層部分の好中球や死細胞が選択的に除去される。さらに、単核級分画中に含まれる好中球や死細胞を除去するために、同条件での遠心分離及び40μmのフィルターを通すのが望ましい。
次にバフィーコート中の単核球細胞から造血幹細胞を得る。造血幹細胞は、周知の方法にしたがって、CD34陽性について細胞を濃縮することにより得る。CD34陽性細胞の濃縮は、CD34固定化ビーズやセルソーターを使用して行うことができる。
次にバフィーコート中のCD34陽性単核球細胞からCD18陽性細胞を濃縮することにより得る。CD18陽性細胞の濃縮は、CD18固定化ビーズやセルソーターを使用してCD18が高発現している細胞を得ることができる。
このようにしてCD34+CD18high単核球細胞を得る。なおCD34+CD18highの順はこの実施形態に限定されず、バフィーコート中の単核球細胞からまずCD18が高発現している細胞を得た後に、次にそこから更にCD34陽性細胞を濃縮することも可能である。
(1)臍帯血単核球治療における凝集塊の影響
骨髄単核球を用いた脳梗塞治療において、凝集塊が発生したロットでは著しく脳梗塞治療効果が低下することを見出している。そこで、臍帯血単核球においても同様のことが起こるのかを確かめる実験を行った。

凝集塊が発生しなかった臍帯血と大きな凝集塊が発生した臍帯血からそれぞれ単核球をフィコールパーク比重遠心法で精製し、PBSで洗浄後、HUVECとの共培養試験を実施した(図1)。なお図1においてCBMNCはヒト臍帯血由来単核細胞を示す。
共培養試験は下記のように実施した。即ち、臍帯血を当量のフィコールパーク上に静かに上層し、400 gで30分間遠心分離した。この時の遠心分離機のアクセル、ブレーキの設定は最小値で実施した。遠心分離後の臍帯血検体からバフィーコート層を分取し、当量の10%の抗凝固剤-クエン酸-デキストロース溶液(ACD液)を含むPBSを加え、再び400 gで5分間遠心分離した。この時の遠心分離機のアクセル、ブレーキの設定は最小値である必要はない。遠心分離したバフィーコート層の沈殿は、上層に好中球や死細胞が濃縮された分画、下層に単核球が濃縮された分画となるため、上清を除去する時に、容器を徐々に水平にしていくことで、沈殿上層部分の好中球や死細胞が選択的に除去した。さらに、単核級分画中に含まれる好中球や死細胞を除去するために400 gで5分間での遠心分離し、40μmのフィルターを通した。次に、HUVECをメーカーのプロトコルに従いHuMedia-EB2培地に血清及び増殖添加剤共に培養した後、トリプシンで剥離し回収した。回収したHUVEC(1 X 105個)にAllophycocyanin(APC)ラベルした血管内皮細胞増殖因子(VEGF、終濃度3μg/mL)及び上記の臍帯血単核球(1 X 106個)を加え、24ウェルプレートで37℃、5%CO2で、3時間インキュベートした。細胞混合物をリン酸緩衝液食塩水(PBS)で2回洗浄し、BD Bioscience 社から購入したPhycoerythrin(PE)標識抗ヒトCD31抗体(BD Bioscience)、FITC(Fluorescein isothiocyanate)標識抗ヒトCD45抗体、及び7AAD(7-Aminoactinomycin D)で染色し、血管内皮細胞中(CD31陽性、CD45陰性及び7AAD陰性)に取り込まれたAPCラベル化したVEGFのレベルをFACS Canto IIセルアナライザー(BD Bioscience)で評価した。血管新生が発生した血管内皮細胞は、細胞外からVEGFを取込むため、この数値が高かった臍帯血ほど血管新生を強く促す、別の言い方をすれば、脳梗塞治療効果の高い臍帯血と考えた。
図1に示されるように、凝集塊が発生しなかったCBNMCの場合、CBNMCが存在することにより血管内皮細胞は多くVEGFを取り込んだ。しかしながら凝集塊が発生したCBNMCの場合、CBMNCが存在しても血管内皮細胞はVEGFをほとんど取り込むことはなかった。これにより臍帯血単核球も骨髄単核球と同様に、凝集塊が存在した単核球では、VEGF取込みを強く促進するものは一つもないと判明した。
(2)ヘパリン処理による治療効果の低下
共培養試験において、10%の抗凝固剤-クエン酸-デキストロース溶液(ACD液)を含むPBSを添加することを誤って、ヘパリンを終濃度2 unit/mLになるように調製したPBSを添加した。

ヘパリン処理の場合、通常のACD液を添加したPBSの場合と比較して、その臍帯血単核球を用いた共培養試験では、凝集塊が発生した場合と同等のVEGF取込みの低下が観察された(図2)。
(3)接着因子の重要性の発見
図2の結果から、造血幹細胞とHUVECとの細胞・細胞結合が重要と考えられる。そこで、造血幹細胞において、細胞・細胞結合に関与する各種接着因子の発現変化を、ヘパリン処理前後で比較した(図3)。

即ち、臍帯血を当量のフィコールパーク上に上層し、400 gで30分間遠心分離した。遠心分離後の臍帯血検体からバフィーコート層を分取し、当量の10%の抗凝固剤-クエン酸-デキストロース溶液(ACD液)を含むPBSを加え、再び400 gで5分間遠心分離した。バフィーコート層の沈殿の分取は、上清除去は容器を徐々に水平にしていくことで実施し、同沈殿をPBSに再懸濁し、400 gで5分間の遠心分離後に40μmのフィルターを通した。フィルター通過後の単核球懸濁液を等分し、400 gで5分間遠心分離した。上清除去後、一方をPBSで、他方をヘパリンを終濃度2 unit/mLになるように調製したPBSでそれぞれ懸濁し、室温で5分間インキュベートした。次に、細胞懸濁液をPBSで洗浄し、PE(phycoerythrin)標識抗ヒトCD34抗体(BD Bioscience)、7AAD(7-Aminoactinomycin D) (BD Bioscience)、FITC(Fluorescein isothiocyanate)標識抗ヒトCD45抗体(BD Bioscience)、APC(allophycocyanin)標識した各種接着因子抗体〔インテグリンα4抗体(Beckman Coulter)、インテグリンα5抗体(Beckman Coulter)、CD18抗体(Beckman Coulter)、もしくはCXCR4抗体(MBL)で染色し、造血幹細胞(CD34陽性及び7AAD陰性)での各種接着因子の発現レベルをFACS Canto IIセルアナライザー(BD Bioscience)で評価した。
図3に示されるように、造血幹細胞のホーミング、細胞接着に機能する代表的な接着因子であるインテグリンα4はヘパリン処理によって発現に有意な変化が認められなかった。
ヘパリン処理によって有意に発現を変化させた接着因子は、インテグリンα5、C-X-Cケモカイン受容体タイプ4(CXCR4)、CD18で、これら分子にもヘパリン処理によってその発現を大きく減ずるものと発現をほとんど低下させないものが存在した。とりわけ、ヘパリン処理によって大きな発現変化を示したのがCD18であった。
(4)治療効果の高い単核球の選別マーカーの発見
このようにVEGF取込みを強く促進する臍帯血単核球とそうでないものとが観察された。これら臍帯血造血幹細胞でのCD18の発現とVEGF取込み促進能との関係を調べたところ、両者には強い正相関が認められた(図4)。

さらに、残りの懸濁液を用いてHUVECとの共培養試験(前述の(1)参照)実施した。共培養実験は、HUVEC(1 X 105個)にAllophycocyanin(APC)ラベルした血管内皮細胞増殖因子(VEGF、終濃度3μg/mL)及び臍帯血単核球(CD34陽性細胞が1 X 104個含まれている細胞数)を加える条件で実施した。
図4左は共培養した臍帯血ごとのHUVECでのVEGF取込み能の違いを示している。図4右は造血幹細胞(CD34陽性及び7AAD陰性)におけるCD18の発現と共培養実験におけるHUVECのVEGF取込み能との相関を示したものである。偶然のヘパリン処理という誤操作によって見出されたVEGF取込み促進能の指標としてのCD18の発現強度は、治療効果の高い単核球の選別マーカーとして有用であることが見出された。
また、前述の実験において、バフィーコート層の沈殿をPBSに再懸濁し、400 gで5分間の遠心分離後に40μmのフィルターを通し、再度PBSに懸濁し、次に、同懸濁液の一部を分取し、PE標識抗ヒトCD34抗体、7AAD、FITC標識抗ヒトCD45抗体、APC標識グルコーストランスポーター(Glut1)抗体で染色した。さらに、残りの懸濁液を用いてHUVECとの共培養試験(前述の(1)参照)実施した。共培養実験は、HUVEC(1 X 105個)にAllophycocyanin(APC)ラベルした血管内皮細胞増殖因子(VEGF、終濃度3μg/mL)及び臍帯血単核球(CD34陽性細胞が1 X 104個含まれている細胞数)を加える条件で実施した。図5右は造血幹細胞(CD34陽性、CD45陽性及び7AAD陰性)におけるグルコーストランスポーター(Glut1)の発現と共培養実験におけるHUVECのVEGF取込み能との相関を示したものである。VEGF取込み促進能の指標としてのグルコーストランスポーター(Glut1)の発現強度は、治療効果の高い単核球の選別マーカーとして有用であることが見出された。
また、前述の実験において、バフィーコート層の沈殿をPBSに再懸濁し、400 gで5分間の遠心分離後に40μmのフィルターを通し、再度PBSに懸濁し、次に、同懸濁液の一部を分取し、PE標識抗ヒトCD34抗体、7AAD、FITC標識抗ヒトCD45抗体、APC標識コネキシン43(Connexin43)抗体で染色した。さらに、残りの懸濁液を用いてHUVECとの共培養試験(前述の(1)参照)実施した。共培養実験は、HUVEC(1 X 105個)にAllophycocyanin(APC)ラベルした血管内皮細胞増殖因子(VEGF、終濃度3μg/mL)及び臍帯血単核球(CD34陽性細胞が1 X 104個含まれている細胞数)を加える条件で実施した。図5左は造血幹細胞(CD34陽性、CD45陽性及び7AAD陰性)におけるコネキシン43(Connexin43)の発現と共培養実験におけるHUVECのVEGF取込み能との相関を示したものである。VEGF取込み促進能の指標としてのコネキシン43(Connexin43)の発現強度は、治療効果の高い単核球の選別マーカーとして有用であることが見出された。
(5)臍帯血単核球における凍結前と凍結融解後とでのCD18の発現
実際、臍帯血単核球の凍結前と凍結融解後のCD18の発現を調べたところ、ヘパリン処理の場合と同様に、大きく発現低下するものと、ほとんど変化しないものがあった。前者はCD18を高発現しており、後者は発現が低いものであった(図6)。

即ち、臍帯血から上述のフィコールパーク比重遠心法で単核球分画を分取し、その単核球分画を等分した。
一方の単核球分画をPE標識抗ヒトCD34抗体、7AAD、FITC標識抗ヒトCD45抗体、APC標識CD18抗体で染色し、造血幹細胞(CD34陽性、CD45陽性及び7AAD陰性)でのCD18抗原の発現レベルをFACS Canto IIセルアナライザー(BD Bioscience)で評価した。
次に、残りの単核球分画に凍害保護液(Cell Banker)を加え、program freezerで凍結させ、液体窒素中で保存した。1週間後に凍結保存した単核球を解凍し、上記と同様の方法で造血幹細胞でのCD18抗原の発現レベルを評価した。
図6に示されるように、CD18を高発現している臍帯血単核球は凍結前と凍結融解後でCD18の発現が大きく低下したが、CD18を低発現している臍帯血単核球は凍結前と凍結融解後でCD18の発現は低いままであった。
次にCD18陽性細胞の有用性をさらに検証するために、下記の検討を行った。
(6)CD18陽性細胞の採取
本発明における治療効果の高い細胞回収方法は下記工程を有するものであった。即ち、ヘパリンを包含しない希釈液にてヒト骨髄液を採取する採取工程と、採取したヒト骨髄液にヘパリンを包含しない比重遠心液を加えて比重遠心を行う分離工程と、を有する細胞回収方法である。本実施例において具体的には以下であった。2.5mlシリンジに、2mlの生理食塩水及び23G注射針をセットした。マウス大腿骨の両端を手術用ハサミで切断し、骨髄腔の一端に注射針を差し込み、生理食塩水を素早く注入することにより、反対側の端より流出した骨髄液を得た。一般に骨髄液は凝固しやすいためヘパリン等の抗凝固剤を用いることが多いが、本発明ではヘパリンを使用せず、生理食塩水による希釈だけで、骨髄液の凝固を抑制した。
本発明における治療効果の高い細胞回収方法は下記工程を有するものであった。即ち、ヘパリンを包含しない希釈液にてヒト骨髄液を採取する採取工程と、採取したヒト骨髄液にヘパリンを包含しない比重遠心液を加えて比重遠心を行う分離工程と、を有する細胞回収方法である。本実施例において具体的には以下であった。2.5mlシリンジに、2mlの生理食塩水及び23G注射針をセットした。マウス大腿骨の両端を手術用ハサミで切断し、骨髄腔の一端に注射針を差し込み、生理食塩水を素早く注入することにより、反対側の端より流出した骨髄液を得た。一般に骨髄液は凝固しやすいためヘパリン等の抗凝固剤を用いることが多いが、本発明ではヘパリンを使用せず、生理食塩水による希釈だけで、骨髄液の凝固を抑制した。
生理食塩水で希釈し凝固を抑制した骨髄液を、比重遠心液(Ficoll-premium GE製)に重層し、600gで20分間遠心後、バフィーコート層にある細胞集団を回収した。
(7)CD18陽性細胞の検定
細胞集団回収後、FACS(BD FACSCalibur)で、抗CD18抗体を用いて表面マーカー解析を行った。

死細胞は7AAD(7-Amino-Actinomycin D; BD Bioscience製、製品番号559925、希釈倍率50倍)試薬を用いて、解析から除外した。ネガティブコントロールにはそれぞれのIso typeコントロールを用いた。
細胞集団の特徴として、フローサイトメトリーによる解析で90%以上の細胞がCD18抗原を発現していることが確認できた(図7)。フローサイトメトリーによる解析とは、抗体を直接的又は間接的に蛍光標識し、対象となる細胞集団をフローサイトメータで測定して、陽性反応を示す細胞割合を算出する解析である。フローサイトメトリーによる反応が陽性というのは、標識に用いた蛍光色素が発光する特有の波長の蛍光の強度の中で、一次抗体を用いずに計測したバックグラウンドの強度より高い強度の蛍光域に含まれる場合をいう。
同じように、細胞集団回収後、FACS(BD FACSCalibur)で、抗Jagged1抗体を用いて表面マーカー解析を行った。CD34陽性細胞は、Jagged1を発現していることが判明している(図8)。そこで、Jagged1分子の、再生促進効果に関して、検討を行うため、HUVECの培養液にJagged1活性を有するJagged1ペプチド(H-Cys-Asp-Asp-Tyr-Tyr-Tyr-Gly-Phe-Gly-Cys-Asn-Lys-Phe-Cys-Arg-Pro-Arg-OH)を添加し、そのVEGF取込促進作用を検討した。
その結果、無刺激では1.04%のHUVECがVEGFを取り込んでいるのに対して、Jagged1による刺激で、5.30%のHUVECがVEGFを取り込むことが判明した(図9)。これらの結果より、幹細胞におけるJagged1の発現が、再生促進作用に重要であることが、確認された。
(8)CD18陽性細胞の加齢マウスに対する投与
上記細胞1x105個を、100 μlのPBSに懸濁し、尾静脈より合計5回、80週令以上のCB-17マウス(高齢マウス)に隔日投与した。コントロールは同量のPBSを静注した。

最後の静脈内投与から24時間後に、脳を取り出し、2%のパラフォルムアルデヒドで固定後、ビブラトーム(ライカ製)で、厚さ20 μmの切片を作成し、抗Nestin抗体 (Merck Millipore製,製品番号MAB5326、希釈倍率1:200), 抗Doublecortin抗体 (DCX; Merck Millipore製, 製品番号AB2253、希釈倍率1:350)及び核染色用DAPI (Thermo Fisher製, 製品番号D1306、希釈倍率1:1,000)で、染色を行った(CD18細胞投与マウス:マウス7匹、総切片22枚、PBS投与マウス:マウス6匹、総切片20枚)。
共焦点イメージはキーエンス製蛍光顕微鏡(BZ-X810)で撮像し、海馬歯状回の顆粒細胞層における新生ニューロンマーカー(Nestin)陽性細胞、及び顆粒細胞層における新生ニューロン(DCX)陽性細胞を評価した。CD18陽性細胞投与の効果を定量的に評価するために、各新生ニューロンマーカー陽性細胞数をカウントした結果、CD18陽性細胞投与により、切片一枚あたりのNestin陽性細胞(図10A)及びDCX陽性細胞(図10B)の両者が、統計学的にも有意に増加することが判明した。
加齢による神経機能の障害に伴う疾患患者では、海馬における神経新生が障害されているが本発明によれば海馬における神経新生が促進されるのでかかる疾患の予防及び/又は治療が可能となる。加齢により神経機能が低下している場合でも、本発明によれば海馬における神経新生が促進されるので老化を改善できるしまた老化を遅延させることができる。
脳梗塞治療や加齢による神経障害の治療に利用できる。

- CD18陽性細胞を有することを特徴とする細胞製剤。
- 前記CD18陽性細胞はグルコーストランスポーター(Glut)が発現していることを特徴とする請求項1に記載の細胞製剤。
- 前記グルコーストランスポーターはI型グルコーストランスポーター(Glut1)であることを特徴とする請求項2に記載の細胞製剤。
- 前記CD18陽性細胞はコネキシン(Connexin)が発現していることを特徴とする請求項1に記載の細胞製剤。
- 前記コネキシン(Connexin)はコネキシン43(Connexin43)であることを特徴とする請求項4に記載の細胞製剤。
- 前記CD18陽性細胞はJagged1が発現していることを特徴とする請求項1に記載の細胞製剤。
- 前記CD18陽性細胞は、グルコーストランスポーター(Glut1)及びコネキシン43(Connexin43)が発現していることを特徴とする請求項1に記載の細胞製剤。
- 前記CD18陽性細胞は、グルコーストランスポーター(Glut1)、コネキシン43(Connexin43)及びJagged1が発現していることを特徴とする請求項1に記載の細胞製剤。
- 前記CD18陽性細胞は幹細胞であることを特徴とする請求項1記載の細胞製剤。
- 前記幹細胞は成体幹細胞であることを特徴とする請求項9に記載の細胞製剤。

- 前記細胞製剤は、脳機能改善剤、脳代謝賦活剤、脳循環改善剤、神経伝達機能改善剤、脳保護剤、又は、記憶障害改善剤であることを特徴とする請求項1乃至13の何れか1項に記載の細胞製剤。
- 前記CD18陽性細胞は、フローサイトメトリーによる解析でCD18陽性反応を示す細胞割合が90%以上である細胞集団であることを特徴とする請求項1乃至14の何れか1項に記載の細胞製剤。
- CD18陽性細胞を有することを特徴とする、海馬での神経新生促進剤。
- 前記海馬は海馬歯状回であることを特徴とする請求項16に記載の神経新生促進剤。
