Abstract
本発明は、内耳性難聴の抑制又は予防のための組成物であって、エルゴチオネイン又はその塩を含む、組成物等、を提供する。
Description
本発明は、エルゴチオネインの新規用途、特に、内耳性難聴の抑制又は予防のための使用等に関する。
アミノ酸の一種であるエルゴチオネインは高い抗酸化作用を有することが知られているが、人間の体内で合成できない物質であるため、エルゴチオネインを配合した化粧品、食品、医薬、飼料等への開発が期待されている。しかしながら、エルゴチオネインが人間の体内で果たす機能は解明されていない。
エルゴチオネインの抗酸化作用はグルタチオン等の他の抗酸化物質よりも優れていることが報告されているが、あらゆる抗酸化物質が体内の酸化に起因する疾患の治療乃至予防に役立つわけではなく、その効能、効果又は医薬用途を見出すためには個別の検討が必要となる。
難聴の多くは耳を構成する外耳、中耳、内耳のうちのいずれかに障害があることが多く、その部位により伝音難聴(外耳、中耳)と感音難聴(内耳)の2つに大きく分類される。これらの難聴は更に原因により細分類されるが、感音難聴の一つとして加齢性難聴が知られている。
加齢性難聴は、文字通り加齢に伴い進行する難聴であり、その根本的な治療方法は存在しない。
特許第5421366号公報では、耳への鼓室内投与によって耳の疾患の処置の際に使用するための無菌の医薬組成物であって、前記医薬組成物は、ポリオキシプロピレンとポリオキシエチレンのコポリマーを含む耳に許容可能な熱可逆性の水性ゲルと、耳への徐放が少なくとも3日間起こるような耳の構造体変性剤とを含み、前記耳の構造体変性剤が骨再構築のモジュレーターであることを特徴とする医薬組成物、が開示されている(請求項1等)。
同公報は上記組成物の対象疾患として種々の疾患を挙げており、その中に難聴加齢性難聴も含まれているが、実際に上記医薬組成物が加齢性難聴等の内耳性難聴の予防等に有効であることを確認した試験は行われていない。
難聴の原因は様々であり、ある難聴の予防に有効である物質が必ずしもそれ以外の難聴に有効であるかは不明である。
驚くべきことに、本発明者らは、エルゴチオネインが加齢性難聴等の内耳性難聴の予防等に有効であることを見出し、本発明を完成させるに至った。
即ち、本願は以下の発明を包含する。

エルゴチオネインはヒトの体内での安全性が確認されているため、本発明により、サプリメント等にエルゴチオネインを配合して内耳性難聴を予防することが期待される。
以下、本発明の実施の形態(以下、「本実施形態」という。)について説明するが、本発明の範囲は以下の実施形態に限定して解釈されない。
(組成物)
第一の態様において、内耳性難聴の抑制又は予防のための組成物であって、エルゴチオネイン又はその塩を含む、組成物、が提供される。

内耳性難聴の原因として先天性又は後天性のものがある。老人性難聴としても知られる加齢性難聴は後天性の内耳性難聴であり、感音性難聴に分類される。内耳性難聴の場合、加齢性難聴であることが好ましい。エルゴチオネインは、可聴領域の低周波領域(例えばマウスの場合1~40kHz、ヒトの場合2~20kHz)、好ましくは可聴領域の低周波領域側から0%~90%の周波数帯域(例えばマウスの場合1~36.1kHz、ヒトの場合2~18.2kHz)、より好ましくは0%~70%の周波数帯域(例えばマウスの場合1~28.3kHz、ヒトの場合2~14.6kHz)での難聴抑制効果を有する。なお、本明細書で使用する場合、「可聴領域の低周波領域側から0%~90%の周波数帯域」は、[下限周波数]+([上限周波数]-[下限周波数])に該当の百分率を割合で掛けた値であり、例えば、下限周波数が1kHzで、上限周波数が40kHzの場合、50%の周波数は、20.5となる(1+(40-1)×0.5)。
その他の後天性の内耳性難聴の原因として、遺伝性疾患(例、コネキシン26遺伝子変異、ワールデンブルグ症候群、Usher症候群、ペンドレッド症候群)、騒音曝露、聴器毒性のある薬剤(例、アスピリン、アミノグリコシド系薬剤、バンコマイシン、シスプラチン、フロセミド、エタクリン酸、キニーネ)、感染症(例、髄膜炎、化膿性内耳炎)、自己免疫疾患(例、関節リウマチ、全身性エリテマトーデス)、メニエール症候群、気圧外傷(外リンパ瘻を伴う)、頭部外傷(頭蓋底の骨折または蝸牛の振盪を伴う)、Auditory neuropathy(聴覚神経障害)に起因するものが知られている。これらの後天性内耳性難聴、特に薬剤により誘導される難聴と加齢性難聴は明確に区別される。
先天性の内耳性難聴として、遺伝性、特発性(原因不明の)奇形、先天性感染症(例、風疹、サイトメガロウイルス感染症、トキソプラズマ症、梅毒)、Rh式血液型不適合、無酸素症、聴器毒性のある薬剤の母体摂取(例、結核や重度の感染症に対するもの)、薬剤性奇形(例、サリドマイドによるもの)に起因するものがある。
有効成分であるエルゴチオネインは含硫アミノ酸の一種である。エルゴチオネインを有効量含む組成物を摂取することで内耳性難聴の抑制、予防又は治療が達成される。エルゴチオネインはL体であることが好ましい。
エルゴチオネインは塩の形態であってもよい。塩は薬学的に許容可能なものである限りその種類は特に限定されず、塩酸、リン酸、硫酸、硝酸、ホウ酸等との塩(無機酸塩)や、ギ酸、酢酸、乳酸、フマル酸、マレイン酸、酒石酸、クエン酸等との塩(有機酸塩)をその例として挙げることができる。本明細書で使用する「エルゴチオネイン」は、特に断らない限り、その薬学的に許容可能な塩も含み得る。
エルゴチオネインは市販のものを使用してもよいし、合成してもよい。エルゴチオネインの製造方法として、例えば化学合成法、エルゴチオネインを産生する菌類(ササクレヒトヨタケ、ヒラタケ、タモギタケ、エノキタケ、エリンギ、シイタケ等のきのこ類を含む)を培養後、エルゴチオネインを抽出精製する方法、発酵法、エルゴチオネインを含有する動物血液等から抽出する方法等が挙げられる。
エルゴチオネインを産生する菌の例として酵母が挙げられる。本明細書で使用する場合、「酵母」とは、培養条件下でエルゴチオネインを産生し得るものであれば特に限定されない。本明細書における酵母は出芽酵母が好ましく、不完全酵母類であることがより好ましい。
不完全酵母の例としてロドトルラ(Rhodotorula)属、クリプトコッカス(Cryptococcus)属などの酵母が挙げられ、これらの中でもロドトルラ属が好ましい。ロドトルラ属の酵母にはロドトルラ・ムシラギノーサ(Rhodotorula mucilaginosa)、ロドトルラ・グルチニス(Rhodotorula glutinis)等が含まれる。
エルゴチオネインは、酵母等の菌に由来する場合、菌体に含まれた状態で組成物に配合されてもよいし、菌から抽出した後に、抽出物として組成物に配合されてもよい。エルゴチオネインは菌体中又は抽出物中に任意の濃度で含まれる。例えば、抽出物はエルゴチオネインを固形分中濃度として0.1~20%含んでもよい。エルゴチオネインは所望とする純度に応じて適宜精製され得る。精製方法は公知の方法を使用することができ、例えば順相カラム、逆相カラム、イオン交換カラム等を用いるカラムクロマトグラフィー、結晶化、膜処理、活性炭処理等を適宜組み合わせて行うことができる。
組成物は、菌体の状態でもよいし、あるいは、他の成分と組み合わせて医薬、食品又はサプリメントの形態で存在してもよい。食品の例として特定保健用食品、機能性表示食品又は栄養機能食品が挙げられる。組成物は、有効成分としてのエルゴチオネインの効果を損なわない範囲で、内耳性難聴の抑制又は予防のための追加の成分や、通常用いられる担体、例えば賦形剤、崩壊剤、滑沢剤、緩衝剤、結合剤、乳化剤、懸濁剤、安定剤、保存剤、防腐剤、生理食塩水等を含んでもよい。安定剤の例としてシクロデキストリンが挙げられる。シクロデキストリンは、α-シクロデキストリン、β-シクロデキストリン、γ-シクロデキストリン又はそれらの組み合わせを使用することができるが、内耳性難聴の抑制又は予防の効果を増大させる観点からはγ-シクロデキストリンが好ましい。エルゴチオネインが菌に由来する場合、組成物は菌体又はその抽出物を含んでもよいし、精製後のエルゴチオネインを含むこともできる。菌の抽出物は粉末状であってもよい。
組成物が投与される対象はヒトに限定されず、非ヒト動物、例えばマウスであってもよい。組成物は、内耳性難聴を将来発症することが予想される対象、既に発症している対象、例えば内耳性難聴に罹患していると診断された対象に投与される。内耳性難聴の診断方法は特に限定されないが、加齢性難聴の診断方法として聴性脳幹反応(ABR)が挙げられる。
組成物におけるエルゴチオネインの量は組成物が投与されるヒトの症状、年齢、性別、投与頻度等により適宜変更されるが、一日当り0.01mg/kg体重以上、100mg/kg体重未満の範囲で適宜調整され得る。ヒトの場合、一日当たり0.1mg~1000mg程度となるようにエルゴチオネインを投与することが好ましく、0.2~800mg程度となるようにエルゴチオネインを投与することがより好ましく、0.5~500mg程度となるようにエルゴチオネインを投与することがとくに好ましい。
投与期間や投与回数は対象の症状や投与量によって変動するが、少なくとも1週間以上、好ましくは10週間以上、例えば13週以上、一日一回投与することが好ましい。32kHz程度の高周波数帯域での難聴には、長期間の治療、例えば13週間以上の治療を行うことで難聴抑制効果が増大し得る。
組成物の剤型としては錠剤、散剤、細粒剤、顆粒剤、カプセル剤、シロップ剤、注射剤、外用剤、座剤等が挙げられる。
賦形剤としては乳糖、デンプン、ソルビトール、D-マンニトール等を用いることができる。崩壊剤としてはデンプン、カルボキシメチルセルロース、炭酸カルシウム等を用いることができる。緩衝剤としてはリン酸塩、クエン酸塩、酢酸塩等を用いることができる。乳化剤としてはアラビアゴム、アルギン酸ナトリウム、トラガント等を用いることができる。結合剤としては、プルラン、アラビアゴム、ゼラチン、デンプン等を用いることができる。滑沢剤としてはステアリン酸マグネシウム、メチルセルロース、ケイ酸マグネシウムを用いることができる。懸濁剤としてはモノステアリン酸グリセリン、モノステアリン酸アルミニウム、メチルセルロース、カルボキシメチルセルロース、ヒドロキシメチルセルロース、ラウリル硫酸ナトリウム等を用いることができる。安定剤としてはプロピレングリコール、ジエチリン亜硫酸塩、アスコルビン酸等を用いることができる。保存剤としてはフェノール、塩化ベンザルコニウム、ベンジルアルコール、クロロブタノール、メチルパラベン等を用いることができる。防腐剤としては塩化ベンザルコニウム、パラオキシ安息香酸、クロロブタノール等を用いることができる。
組成物の投与形態は内耳性難聴の抑制又は予防が可能な限り特に限定されず、例えば、経口投与、又は静脈内、腹腔内、皮下若しくは経皮等の非経口投与などを挙げることができる。
(方法)
第二の態様において、対象における内耳性難聴の抑制又は予防のための方法であって、エルゴチオネイン又はその塩を対象に投与することを含む、方法、が提供される。

エルゴチオネインが投与される対象は内耳性難聴の抑制又は予防の必要性がある者である。対象はヒトに限定されず、非ヒト動物、例えばマウスであってもよい。
エルゴチオネインは、内耳性難聴の抑制又は予防に効果が知られている他の成分と組み合わせて投与することができ、それぞれの投与は同時でもよいし、異なるタイミングでもよい。
エルゴチオネインの剤型、投与量、投与経路等は上述したとおりである。
以下に実施例を挙げて本発明を更に具体的に説明するが、本発明はこれらに限定されるものではない。
実施例1:
1.材料及び方法
1.1.エルゴチオネイン及び媒体
1.1.1.エルゴチオネインを含む酵母抽出粉末の製造
ロドトルラ・ムシラギノーサ(Rhodotorula mucilaginosa)酵母を液体培地中で培養した培養液から、遠心処理(12,000×g、90分)により菌体を回収した。得られた菌体を水で洗浄後、遠心処理(12,000×g、90分)を2回繰り返した。菌体に水を加え、95℃で10分間処理し、菌体内のエルゴチオネインを抽出した。その後、遠心処理(12,000×g、90分)により抽出液を得、UF膜(分画分子量:6kDa、25℃)処理により、粗精製液を得た。粗精製液をエバポレーターで濃縮後、80℃で30分間の加熱殺菌処理を行った。殺菌処理後の粗精製液を噴霧乾燥することにより、酵母抽出物の粉末を得た。

本実施例では、Asahipak NH2P-50カラムを用いたHPLCでエルゴチオネイン含有量を測定した。溶出液A(0.1 容量%トリエチルアミン、50 mMリン酸ナトリウムバッファー: pH 7.3)及び溶出液B(100 mM NaCl)の濃度勾配液を用いてエルゴチオネインを溶出した。波長254nmの吸光度で測定し、約5.7分にエルゴチオネインの溶出が認められた。酵母抽出物10μg/ml濃度水溶液を0.2μmフィルターで膜ろ過し、ろ過物についてHPLC測定を行い、エルゴチオネイン75mg/gを含有することを確認した。

2.試験系

加齢性難聴モデルで一般的に用いられる動物種で、その系統維持が確立されている動物種である以下のマウス(SPF)系統を使用した。

入手後、14日間の予備飼育期間を設けた。この間に体重測定を3回、ABR検査を1回、一般状態の観察を1日1回行い、体重推移、ABR検査及び一般状態に異常が認められない動物を群分けに使用した。

予備飼育期間中のABR検査において24及び32kHzの音圧閾値が高い動物3例(4例以上見られた場合は除外しない)を除外し、コンピュータプログラム(IBUKI、株式会社日本バイオリサーチセンター)を用いて、体重を層別に分けた後、無作為抽出法により、各群の体重の分散がほぼ等しくなるように群分けした。
予備飼育期間中のABR検査において24及び32kHzの音圧閾値が高い動物3例(4例以上見られた場合は除外しない)を除外し、コンピュータプログラム(IBUKI、株式会社日本バイオリサーチセンター)を用いて、体重を層別に分けた後、無作為抽出法により、各群の体重の分散がほぼ等しくなるように群分けした。

投与方法:ディスポーザブルマウス用経口ゾンデ(有限会社フチガミ器械)を取り付けたポリプロピレン製ディスポーザブル注射筒(テルモ株式会社)を用いて経口投与した。投与時はマグネティックスターラーを用いて撹拌し、注射筒に吸引後、経口ゾンデに付着した投与検体を拭き取った。
投与時刻:8時30分から11時00分に投与した。
投与液量:投与日に最も近い体重を基準として10mL/kgで算出した。

5.3.ABR(聴性脳幹反応)検査
ABR検査は、投与前(11週齢)及び最終投与日の翌日(25週齢)に行った。動物に、27G 注射針(テルモ株式会社)を取り付けたポリプロピレン製ディスポーザブル注射筒(テルモ株式会社)を用いて、混合麻酔薬[ケタミン塩酸塩(75mg/kg)とキシラジン塩酸塩(10mg/kg)]を皮下投与(投与液量:2mL/kg)した。麻酔後、関電極を検耳(右耳)の外耳付近の皮下、不関電極を頭頂部皮下、アース電極を頸背部皮下にそれぞれ装着した。電極からABRの電位を生体電位アンプ(Model:ER-1、Cygnus Technology Inc.)に誘導し、データ収録・解析システム(PowerLab、Sampling soft:LabChart ver.8、ADInstruments)に記録した(記録条件:Sampling time:10ms、Sampling rate:40kHz、Bandpass filter:1-3000Hz、加算回数 500回)。

音刺激は、Coupler type speaker(Model:ES1spc、バイオリサーチセンター株式会社)を右外耳道に挿入し、TDT音響システム(ZBus for system3、Tucker-Davis Technologies Inc.)より与えた(音圧範囲:10-90dB、音の種類:24及び32kHzのTone burst)。最初に90dBの音刺激を与えてABRを記録した。その後、音圧を適宜変更し、ABRの波形が消失する最大音圧とABRの波形が検出される最小音圧を5dB刻みで確認した。ABRの波形が検出された最小音圧を音圧閾値(ABR thresholds;dB SPL)とした。なお、90dBの音刺激でABRの波形が検出されない場合は、音圧閾値を95dB SPLとみなした。測定後にABRの波形データを電子媒体(CD-R)に保存した。
7.結果
以下の表2(24kHzのABR閾値の結果)及び表3(32kHzのABR閾値の結果)に示すとおり、エルゴチオネインを含む抽出粉末を1kg体重あたり1日300mg(エルゴチオネインの含有量は7.5%)、13週間にわたり投与されたマウスは平均ABR閾値の変化がコントロール群より低下した。
以下の表2(24kHzのABR閾値の結果)及び表3(32kHzのABR閾値の結果)に示すとおり、エルゴチオネインを含む抽出粉末を1kg体重あたり1日300mg(エルゴチオネインの含有量は7.5%)、13週間にわたり投与されたマウスは平均ABR閾値の変化がコントロール群より低下した。
実施例2:
噴霧乾燥時に固形分中含量が20重量部となるようにγ-シクロデキストリンを添加した以外は、実施例1と同様の方法で酵母抽出粉末を調製した。得られた酵母抽出粉末について実施例1と同様の方法でHPLC測定を行い、該粉末がエルゴチオネイン58mg/gを含有することを確認した。それ以降の手順も実施例1と同じであり、エルゴチオネインを含む酵母抽出粉末は、実施例1の30mg/mLと異なり、38.8mg/mLの濃度液となるように調製したものの、動物に投与されるエルゴチオネインの量は実施例1と同量である。

動物モデルを用いた試験系は実施例1と同じく10週齢の雄を使用したが、その数を28匹に増やした。また、予備飼育期間は15日間とした。試験群は以下に示す2群とした。各群の動物数は12匹とした。

投与後の動物について、実施例1と同様に観察及びABR(聴性脳幹反応)検査を行った。投与前(11週齢)、投与8週(20週齢)及び最終投与日の翌日(25週齢)に行った点のみが実施例1と相違する。群ごとの各時点における音圧閾値及び体重について平均値及び標準誤差を算出し、得られた結果を統計学的に解析した。有意差検定は、最初にコントロール群の11週齢(Pre)と25週齢(post)の間で24及び32kHzでの音圧閾値についてWilcoxonの符号付順位和検定を行い、加齢性に難聴が生じていることを確認した。次に、コントロール群と被検物質群の間で各時点でWilcoxonの順位和検定を行った。有意水準は5%とし、5%未満(p<0.05)と1%未満(p<0.01)とに分けて表示した。有意差検定には、市販の統計プログラム(SASシステム、SAS Institute Japan株式会社)を用いて行った。
その結果、以下の表5(24kHzのABR閾値の結果)及び表6(32kHzのABR閾値の結果)に示すとおり、エルゴチオネインを含む抽出粉末を1kg体重あたり1日388mg(エルゴチオネインの含有量は5.8%)、13週間にわたり投与されたマウスは平均ABR閾値の変化がコントロール群より低下した。

表5及び6に示すとおり、いずれの周波数でもエルゴチオネインの難聴抑制効果が確認されたが24kHzでは特に顕著であった。難聴の症状は通常高音域から発症するため(Exp Anim. 2015;64(3):241-251)、高周波数(32kHz)ではばらつきも含め抑制効果が認められ難いが、高周波数の場合、投与期間を延ばすことで難聴抑制効果が増大する。
上記の結果は、エルゴチオネインにより難聴の進行が遅れ、その結果、測定した時期においてエルゴチオネイン投与群では高周波数帯でのABRの波形が検出されず、低周波数帯でのABRの波形が検出されたと解される。換言すると、エルゴチオネインが投与されない場合には可聴周波数が短時間で減少するのに対し、エルゴチオネインが投与されると、減少速度が遅くなると考えられる。
Claims (21)
- 内耳性難聴の抑制又は予防のための組成物であって、エルゴチオネインを含む、組成物。
- エルゴチオネインが一日当り0.01mg/kg体重以上、100mg/kg体重未満の範囲の量で投与される、請求項1に記載の組成物。
- 一日一回又は複数回投与される、請求項2に記載の組成物。
- 投与期間が少なくとも一週間以上である、請求項2又は3に記載の組成物。
- 投与が経口投与を含む、請求項2~4のいずれか一項に記載の組成物。
- 内耳性難聴が加齢性難聴である、請求項1~5のいずれか一項に記載の組成物。
- 内耳性難聴が薬剤誘導性難聴ではない、請求項1~6のいずれか一項に記載の組成物。
- エルゴチオネインを投与される対象が、聴性脳幹反応(ABR)により加齢性難聴と診断されている、請求項1~7のいずれか一項に記載の組成物。
- エルゴチオネインが酵母由来である、請求項1~8のいずれか一項に記載の組成物。
- 酵母が、ロドトルラ(Rhodotorula)属の不完全酵母である、請求項9に記載の組成物。
- 酵母抽出物を更に含む、請求項8又は9に記載の組成物。
