Abstract
본 발명은 항암제인 시스플라틴의 부작용으로 나타나는 이독성의 예방 또는 치료용 약학적 조성물에 관한 것으로, 구체적으로 넬피나비르가 시스플라틴으로인해 이독성이 유발된 생쥐에서 청성뇌간반응의 역치를 감소시키는 것을 확인하였으며, 코르티기관의 외유모세포 및 내유모세포의 배열이상 및 세포사멸을 억제하고, 청각유모세포주의 세포사멸 및 염증매개인자 TNF-α, IL-1β 및 IL-6 발현이 억제하며, NF-κB의 활성이 억제하는 것을 확인함으로써, 본 발명의 넬피나비르는 시스플라틴 부작용으로 나타나는 이독성의 예방 또는 치료용 조성물로써 유용하게 사용될 수 있다.
Description
본 발명은 넬피나비르(nelfinavir)를 유효성분으로 함유하는, 항암제 시스플라틴의 부작용인 이독성의 예방 또는 치료용 약학적 조성물에 관한 것이다.
이독성(Ototoxicity)은 독소에 의해 귀, 특히 달팽이관이나 청신경, 정전기관 등이 손상되는 것을 말하며 감각신경성청력손실이나 평형이상, 또는 두 가지 모두를 야기할 수 있다. 이독성은 일반적으로 약물에 의해 발생하는데, 내이 신경에 독성을 갖는 약물로는 아미노글리코사이드계 젠타마이신과 같은 항생제, 푸로세미드와 같은 루우프이뇨제, 시스플라틴과 같은 항암제 등이 있으며, 아스피린, 멜록시캄과 같은 비스테로이드성 항염제도 이독성이 있다고 밝혀졌다. 또한 퀴닌이나 수은, 납 등의 중금속에 의해서도 발생한다.
항암제인 시스플라틴(cisplatin)은 두경부 악성종양 및 비뇨생식기계의 종양치료에 우수한 효과를 나타내어 단독 또는 병용요법으로 널리 이용되고 있다. 그러나 시스플라틴은 이독성, 신독성, 신경독성 및 골수 억제(myelosuppression)에 의한 감염 위험성 증가 등의 심한 부작용을 유발하는 것으로 알려져있다. 그 중 이독성으로는 고음력의 청력저하, 때때로 난청, 이명이 있다. 이런 부작용은 시스플라틴의 투여량의 증가에 따라 청각 장애의 발현빈도가 증가하며, 특히 1일투여량 80mg/m2이상, 총 300mg을 초과하는 경우 청각장애 현상이 현저하게 나타나는 것으로 알려져 있다. 또한, McKeage 에 따르면, 시스플라틴 투여 환자의 75-100%에 이르는 환자에게서 청각역치의 증가가 보고되었다(Hear Res, 1995, 13, 228-244). 만니톨의 투여와 다량의 수액섭취에 의해 시스플라틴에 의한 신독성을 감소시킬 수 있는 것으로 알려져 있으나, 이독성이 나타나는 원인과 그 예방법에 대해서는 몇몇 가설이 제시되고 있을 뿐 정확히 알려져 있지 않다.
시스플라틴으로 이독성을 유발한 동물 모델에서 와우내 전위(endocochlear potential)의 감소, 복합활동전위(compound action potential) 또는 와우마이크로폰작용(cochlear microphonic)등의 현상이 보고되었고. 와우관의 형태적 손상에 관해서 적어도 세개의 기관:코르티기관(organ of Corti), 나선신경절세포(spiral ganglion cell) 손상, 와우관 외측벽(lateral wall)의 손상이 보고되었다. 상기 동물 모델에서 시스플라틴 투여 후 코르티기관의 유모세포와 나선신경절세포의 사멸이 관찰되었는데 이것이 청각손상의 주요 원인으로 생각된다.
시스플라틴에 의한 청각손상의 분자적 기전으로 현재까지 외유모세포의 칼슘 통로 차단, 활성산소(reactive oxygen speicies; ROS), 염증 등이 거론되고 있다(이정구 외, Korea J Otolaryngol, 2002, 45, 741-746; So HS et al., J Associ Res Otolaryngol. 2007, 8, 338-355; Rybak et al., Hearing Res. 2007, 226., 157-167).
활성산소는 시스플라틴에 의한 청각세포사멸의 주요 원인으로 생각되어 지는데, 활성산소 조절인자로 잘 알려진 SOD, GSH, GSSG 및 lipid peroxidation등의 발현변화 및 이로 인한 세포사멸이 보고되었다(Rybak et al., Hearing Res. 2007, 226., 157-167). 또한 N-acetyl cystein(NAC), sodium thiosulphate, amifostine 및 lipoic acid등의 항산화제가 시스플라틴 유도 이독성에 보호효과를 가지는 것이 알려져 있어(Tohoku J Exp Med. 2009, 219, 177-186) 활성산소 제어의 중요성이 대두되었다.
염증은 활성산소와 함께 청각세포 사멸의 주요 원인으로 생각된다. So H. 등은 청각세포(HEI-OC1)에 시스플라틴을 처리했을 때, TNF-α, IL-1β 및 IL-6등의 염증성 사이토카인의 합성 및 분비가 증가되는 것을 확인하였고, 시스플라틴을 투여하여 이독성을 유발한 동물(rat)에서 와우관의 나선인대(spiral ligament), 나선연(spiral limbus) 및 코르티기관에서 TNF-α가 증가하는 것을 확인함으로써, 시스플라틴에 의한 청각기관의 손상이 염증과 밀접한 관계가 있음을 보고하였다(J Associ Res Otolaryngl., 2007, 8, 338-355). 또한, Kaur T.등은 NOX3에 의해 발생한 활성산소가 STAT1의 활성화를 통해 그 하위 신호전달자들인 염증관련인자 iNOS, COX-2 및 TNF-α 발현을 증가시키며, siRNA에 의한 STAT1 knock down에 의해 상기 iNOS, COX-2 및 TNF-α이 억제되고 p53, Bax 발현 및 caspase절단을 감소시켜 시스플라틴으로 유도된 와우세포주 UB/OC-1의 세포사멸에 대해 보호효과를 보이는 것을 보고하였다. 또한, TNF-α 길항제인 etanercept가 시스플라틴으로 인한 유모세포 사멸을 억제하는 것을 확인하였다. (Kaur T. et al., Cell death dis, 2011, 2, e180) 이같은 보고들은 염증반응의 제어가 시스플라틴 유도 이독성 치료에 매우 중요한 인자임을 제시한다.
넬피나비르(Nelfinavir)는 사람면역결핍바이러스(human immunodeficiency virus; HIV) 치료용 항바이러스 제제로, HIV가 분비하는 프로테아제와 단단히 결합하여 프로테아제의 작용을 막아, 바이랄 파티클(viral particle)의 성숙을 억제하는 항바이러스 제제이다. 넬피나비르는 경쟁적 억제제로서 프로테아제와 결합하는 특이적 잔기를 제공함으로써 교차저항성을 줄여 다른 프로테아제 억제제와 달리 HIV-1과 HIV-2에 모두 적용 가능하다.
그런데 최근 항바이러스제제인 넬피나비르가 폐암, 간암, 전립성암, 유방암 등 여러 고형암에대한 저해 효과가 보고되고, 항암제로서 임상시험이 실시되고 있는 등, 적응증이 다른 질병으로 확대되고 있다. U.S. National Cancer Institute의 Gills J. 등은 프로테아제 억제제타입의 HIV 치료제 리토나비르(ritonavir), 사퀴나비어(saquinavir) 및 넬피나비르(Nelfinavir)가 암세포 성장을 억제하는 것을 확인하였고(Gills J.J. et al., Clin Cancer Res, 2007, 13, 5183-5194) 이 중 넬피나비르는 세 약물중 암세포 성장 억제효과가 가장 뛰어난 것을 확인하였다. 이러한 넬피나비르의 항암효과는 MMP-9, MMP-2, NF-kB, STAT3, CDK-2, VEGF, EGFR등을 억제함으로써 나타난다고 보고되었고(Koltai T. F1000Research, 2015, 4, 1-19),
그러나 현재까지 넬프나비르의 이독성과 관련한 보고로는 넬피나비르가 청각세포 HEI-OC1의 사멸을 유도하는 것을 확인하여 이독성의 잠재적 유발자일수 있음이 개시되었을 뿐(Thein P. et al., Hear Res, 2014, 310, 27-35), 본 발명의 시스플라틴 부작용에 의한 이독성의 예방 및 치료용 약학적 용도로써의 넬피나비르의 역할에 대해서는 현재까지 알려진 바가 없다.
이에, 본 발명의 발명자들은 항바이러스제 넬피나비르를 항암제인 시스플라틴의 부작용으로 유발되는 이독성 치료를 위한 새로운 물질로 제시하고자 하며, 넬피나비르가 시스플라틴으로 인해 이독성이 유발된 생쥐의 청성뇌간반응의 역치를 감소시키고, 코르티기관의 유모세포손상 및 세포사멸을 억제하고, 청각유모세포주의 세포사멸과, 염증매개인자 TNF-α, IL-1β 및 IL-6 발현을 억제하며, NF-κB의 활성을 억제하는 것을 확인하여 넬피나비르의 시스플라틴 유도 이독성에 대한 치료효과를 밝힘으로써 본 발명을 완성하였다.
본 발명의 목적은 넬피나비르(nelfinavir)를 유효성분으로 함유하는 항암제 시스플라틴의 부작용인 이독성의 예방 또는 치료용 약학적 조성물을 제공하는 것이다.
상기 목적을 달성하기 위하여, 본 발명은 넬피나비르(Nelfinavir)를 유효성분으로 함유하는 시스플라틴 부작용으로 유발된 이독성의 예방 또는 치료용 약학적 조성물을 제공한다.
또한 본 발명은 넬피나비르(Nelfinavir)를 유효성분으로 함유하는 시스플라틴 부작용으로 유발된 이독성의 예방 또는 개선용 건강기능식품을 제공한다.
본 발명은 넬피나비르가 시스플라틴으로인해 이독성이 유발된 생쥐에서 청성뇌간반응의 역치를 감소시키는 것을 확인하였으며, 코르티기관의 외유모세포 및 내유모세포의 배열이상 및 세포사멸을 억제하고, 청각유모세포주에서 세포사멸 및 염증매개인자 TNF-α, IL-1β 및 IL-6 발현을 억제하며, NF-κB 활성을 억제하는 것을 확인함으로써, 본 발명의 넬피나비르는 시스플라틴 부작용으로 나타나는 이독성의 예방 또는 치료용 조성물로써 유용하게 사용될수 있다.

이하, 본 발명을 상세히 설명한다.
본 발명은 넬피나비르(Nelfinavir)를 유효성분으로 함유하는 시스플라틴에 의한 이독성 예방 또는 치료용 약학적 조성물을 제공한다.
상기 넬피나비르는 청각유모세포 사멸을 억제시키는 것을 특징으로한다.
상기 넬피나비르는 청각유모세포에서 TNF-α 또는 IL-6 유전자 또는 단백질 발현을 감소시키는 것을 특징으로한다.
상기 넬피나비르는 청각유모세포에서 NF-κB 단백질의 활성을 억제 시키는 것을 특징으로한다.
본 발명의 구체적인 실시예에서, 넬피나비르는 시스플라틴으로 이독성을 유발한 생쥐의 청성뇌간반응 역치를 감소시켰고(도 1참조), 코르티기관의 체외배양에서 유모세포 손상을 억제하였으며(도 2참조), 청각유모세포주 HEI-OC1의 세포생존율을 증가시키고, 시스플라틴에 의해 증가된 세포사멸유도인자인 cleaved PARP-1, active caspase 3 및 phospho-H2AX 단백질 발현을 감소시키는 것을 확인하였다(도 3 참조). 또한 넬피나비르가 청각유모세포주에서 시스플라틴에 의해 증가한 TNF-α 및 IL-6 mRNA 발현을 억제하고, NF-κB의 활성을 억제하는 것을 확인하였다(도 4 및 도 5참조).
본 발명의 이독성 예방 및 치료용 약학적 조성물은 상기 넬피나비르에 추가로 동일 또는 유사한 기능을 나타내는 유효성분을 1종 이상 함유할 수 있다. 투여를 위해서는 추가로 약제학적으로 허용 가능한 담체를 1종 이상 포함하여 제조할 수 있다. 약제학적으로 허용 가능한 담체는 식염수, 멸균수, 링거액, 완충 식염수, 덱스트로스 용액, 말토 덱스트린 용액, 글리세롤, 에탄올 및 이들 성분 중 1 성분 이상을 혼합하여 이용할 수 있으며, 필요에 따라 항산화제, 완충액, 정균제 등 다른 통상의 첨가제를 첨가할 수 있다. 또한, 희석제, 분산제, 계면활성제, 결합제 및 윤활제를 부가적으로 첨가하여 수용액, 현탁액, 유탁액 등과 같은 주이용 제형, 산제, 정제, 캡슐제, 환, 과립 또는 주사액제로 제제화 할 수 있다. 더 나아가 당 분야의 적정한 방법으로 또는 Remington's Pharmaceutical Science(Mack Publishing Company, Easton PA, 18th, 1990)에 개시되어 있는 방법을 이용하여 각 질환에 따라 또는 성분에 따라 바람직하게 제제화할 수 있다.
본 발명의 이독성 예방 및 치료용 약학적 조성물은, 통상적인 방법에 의해 정제, 캅셀제, 산제, 과립제, 현탁제, 유제, 시럽제, 기타 액제로 제형화될 수 있다.
구체적으로 본 발명의 약학적 조성물은 경구 투여용 제형, 예를 들면 정체, 트로치제(troches), 로젠지(lozenge), 수용성 또는 우성현탁액, 조제분말 또는 과립, 에멀젼, 하드 또는 소프트 캡슐, 시럽 또는 엘릭시르제(elixirs)로 제제화된다. 정제 및 캡슐 등의 제형으로 제제하기 위해 락토오스, 사카로오스, 솔비톨, 만니톨, 전분, 아밀로펙틴, 셀룰로오스 또는 젤라틴과 같은 결합제, 디칼슘 포스페이트와 같은 부형제, 옥수수 전분 또는 고구마 전분과 같은 붕해제, 스테아르산 마르네슘, 스테아르산 칼슘, 스테아릴푸마르산 나트륨 또는 폴리에틸렌글리콜 왁스와 같은 윤활유가 함유된다. 캡슐제형의 경우는 상기에서 언급한 물질 이외에도 지방유와 같은 액체 담체를 함유한다.
또한, 본 발명의 약학적 조성물은 경구 또는 비경구 투여할 수 있으며, 비경구 투여시 피하주사, 정맥주사, 근육내 주사 또는 흉부내 주사 주입방식을 선택하는 것이 바람직하다. 비경구 투여용 제형으로 제제화하기 위해서는 본 발명의 넬피나비어와 함께 물에서 혼합하여 현탁액으로 제조하고 이를 앰플 또는 바이알의 단위 투여형으로 제제한다.
본 발명에 따른 유효성분의 투여량은 체내에서 활성성분의 흡수도, 불활성화율 및 배설속도, 환자의 연령, 성별 및 상태, 치료할 질병의 중증 정도에 따라 적절히 선택되나, 경구 투여제의 경우 일반적으로 성인에게 1일에 체중 1 kg당 본 발명의 넬피나비어를 0.0001 ~ 500 mg의 양으로 1회 내지 수회 나누어 투여할 수 있으며, 0.001 ~ 100 mg의 양으로 투여하는 것이 바람직하다.
본 발명의 조성물은 단독으로, 또는 수술, 방사선 치료, 호르몬 치료, 화학 치료 및 생물학적 반응 조절제를 사용하는 방법들과 병용하여 사용할 수 있다.
아울러 본 발명은, 넬피나비르를 유효성분으로 함유하는 시스플라틴에 의한 이독성 예방 또는 치료용 건강기능식품을 제공한다.
본 명세서의 "건강기능식품"이란 일상 식사에서 결핍되기 쉬운 영양소나 인체에 유용한 기능을 가진 원료나 성분 (기능성 원료)을 사용하여 제조한 것으로, 인체의 정상적인 기능을 유지하거나 생리기능 활성화를 통하여 건강을 유지하고 개선하는 식품으로 식품의약품안전처장이 정한 것을 의미하나, 이에 한정되지 않으며 통상적인 의미의 건강식품을 배제하는 의미로 사용된 것이 아니다.
본 발명의 건강기능식품은 넬피나비르를 그대로 첨가하거나 다른 식품 또는 식품 성분과 함께 사용될 수 있고, 통상적인 방법에 따라 적절하게 사용될 수 있다.
또한, 본 발명의 건강기능식품은 식품학적으로 허용 가능한 식품 보조 첨가제를 더 포함한다. 본 발명에 이용될 수 있는 식품학적으로 허용가능한 식품 보조 첨가제는 포도당, 과당, 말토스, 슈크로스, 덱스트린, 시클로덱스트린과 같은 당 및 자일리톨, 소르비톨, 에리트리톨 등의 당알코올과 같은 천연 탄수화물, 타우마틴, 스테비아 추출물 등의 천연 향미제, 사카린, 아스파르탐 등의 합성 향미제, 착색제, 펙트산 또는 그의 염, 알긴산 또는 그의 염, 유기산, 보호성 콜로이드 증점제, pH 조절제, 안정화제, 방부제, 글리세린, 알코올, 탄산화제 등을 포함하나, 이에 한정되는 것은 아니다.
상기 외에 본 발명의 건강기능식품은 여러 가지 영양제, 비타민, 광물(전해질), 합성 풍미제 및 천연 풍미제 등의 풍미제, 착색제 및 중진제(치즈, 초콜릿 등) 등을 함유할 수 있다.
이하 본 발명을 실시예 및 실험예에 의해서 상세히 설명한다.
단 하기 실시예 및 실험예는 본 발명을 예시하기 위한 것일 뿐, 본 발명이 하기 실시예 및 실험예에 의해서 한정되는 것은 아니다.

생쥐 모델 준비
시스플라틴 부작용에 의한 이독성 생쥐 모델 구축을 위해서, 8주령의 C57BL/6 생쥐(오리엔트)를 사용하였다. 실험에 사용한 모든 생쥐들은 항온(22~26℃) 및 항습(55~60%) 조건이 유지되는 무균 동물실에서 사육하였으며 모든 실험은 원광대학교 임상시험 관리위원회의 실험실 동물 관리 및 윤리 규정에 따라 임상시험 관리위원회의 승인을 받은 후 시행하였다. 실험군은 대조군(control; PBS 주사), 이독성 유발군(ciplatin; 시스플라틴(20 mg/kg/day, 일동제약) 1회 투여), 넬피나비르-이독성 치료군(cisplatin+NFV; 시스플라틴을 투여하기 6시간 넬피나비르(40 mg/kg, TOCRIS)를 투여하고, 시스플라틴(20 mg/kg/day)을 투여한 후, 넬피나비르(40 mg/kg)를 3일 동안 1일 1회 투여) 및 넬피나비르 단독처리군(넬피나비르; 4일간 넬피나비르(40 mg/kg)만 투여)으로 나누어 실험을 진행하으며, 모든 약물은 복강주사로 투여되었다.

효과
소리가 외이, 중이, 내이의 코르티기관까지 전달이 되면, 코르티기관에서는 이 음파에 의한 자극을 전기적인 신호로 바꾸어 와우신경세포에 신경자극을 일으키게 된다. 이러한 여러 신경세포에서의 전기신경자극은 뇌척수액과 뇌조직이 전도체 역할을 하기때문에 두피의 여러 부분에서 포착되며, 전달속도가 각각 다르기 때문에 자극의 시작 부위도 구별 가능하다. 청성뇌간반응검사는 상기와 같은 원리를 이용하여 음자극 후 약 0~10 msec 이내에 와우에서 중추신경에 이르기까지 발생하는 전위를 측정하는 검사이다.
넬피나비르의 청력회복 효과를 확인하고자, 시스플라틴으로 이독성을 유발한 생쥐에 넬피나비르를 투여한 뒤 청성뇌간반응검사를 수행하였다. 청성뇌간 반응 검사를 위하여 모든 실험동물을 xylazine(5 mg/kg)과 ketamine(30 mg/kg)의 혼합액 Rompun(1.1 mL)과 ketamine(4.0 mL)과 Normal Saline(4.9 mL)의 혼합액으로 생쥐 한마리 기준인 100μL/20g {{럼푼주사액(rompunInj)-바이엘코리아주식회사, 케타민50주사(Ketamine50Inj)-유한양행, 대한멸균생리식염수(Normal daline, NS)-대한약품공업(주)}} 으로 근육마취를 시행하고 방음실에서 TDT사의 TDT system (Tucker-Davis Technologies, FL, USA)을 사용하여 청성뇌간반응의 역치변화를 측정하였다. 삽입형 ear tip (3.5 mm, Nicolet Biomedical, Inc.)을 외이도에 위치시키고, 활동전극을 두정부에, 기준전극은 측정할 귀의 후외부에, 접지전극은 반대측 귀의 후외부에 위치시킨 후 4, 8, 16, 32 kHz에서 90 dB HL의 강도부터 10 dB씩 낮추면서 역치를 측정하였다. 그 결과, 이독성 유발군(cisplatin; 시스플라틴만 투여)에서는 모든 영역에서 청성뇌간반응의 역치가 평균 40 dB SPL 이상 상승한 반면, 넬피나비르 치료군(cisplatin+NFV)은 모든 영역에서 이독성 유발군에 비하여 청성뇌간반응 역치가 회복되었다. 대조군(control)과 넬피나비르 단독처리군(nelfinavior)은 모든 영역에서 뚜렷한 청성뇌간반응 역치의 변화가 관찰되지 않았다(도 1). 이같은 결과는 넬피나비르가 시스플라틴에 의한 청력손상을 회복시키는 효과가 있음을 의미한다.

코르티기관은 기저막(basilar membane)위에는 개막(tectorial membrane), 유모세포로 구성되어 있고, 기저막 위에는 유모세포들이 위치해 있는데, 안쪽에 위치한 세포들을 내유모세포(inner hair cell)이라 하고, 바깥쪽에 위치한 세포를 외유모세포(outer hair cell)라고 한다. 내유모세포는 약 3500개의 내유모세포가 한줄로 배열되어 있고, 외유모세포는 약 1만 2000개의 외유모세포가 세줄로 배열되어 있어 조직 이상을 형태적으로 구별 가능하다.
상기 <실험예 1>에서 확인한 넬피나비르의 시스플라틴에 의한 청력손상 회복효과가가, 청각유모세포의 사멸의 직접적인 억제에 따른 결과인지 확인하기 위해서, 코르티기관 체외배양조직(explant)에 시스플라틴 및/또는 넬피나비르 처리하여 유모세포의 형태이상을 관찰하였다.
코르티기관의체외배양은 생후 3일의 생쥐의 내이에서 코클리아(cochlea)를 분리하여 차가운 1xPBS 에서 코클리아 내의 외유모세포, 내유모세포를 분리한다음 10% 우태아혈청이 첨가된 DMEM세포 배양액을 첨가한 후 5% 이산화탄소의 배양조건에서 12시간동안 배양한다. 12시간 후 시스플라틴(15μM)과 넬피나비르(6μM)를 분주 후 30시간 배양한다. 30시간 후 1xPBS로 3번 세척하고, 상온에서 30분간 팔로이딘 염색(1:4000)을 한다. 1xPBS 3번 세척 후 고정액을 분주 후 유리덮게로 덮는다. 시스플라틴은 대학병원에 사용되어지고있는 1바이알 50mg/100mL이 함량되어진 일동제약주식회사의 주사액을 사용하였고, 넬피나비르는 TOCRIS 에서 구입하여 사용하였다.
그 결과, 코르티기관의 체외배양조직에 시스플라틴을 처리한 이독성유도군(cisplatin)에서 외유모세포 및 내유모세포의 배열이 흐트러지며 수가 감소하는 것이 관찰되었고, 넬피나비르 치료군(Cisplatin + Nelfinavir)에서는 시스플라틴에 의한 유모세포손상이 회복되어 대조군과 유사한 형태의 배열과 수를 보이는 것을 확인하였다(도 2). 상기 결과는, 넬피나비르가 시스플라틴에 의한 청각유모세포의 손상을 회복시키는 것을 나타낸다.

넬피나비르의 청각유모세포 사멸 억제효과를 확인하고자, 청각유모세포주인 HEI-OC1 세포에 시스플라틴과 넬피나비르를 처리한 후 세포생존율을 측정하였다.
구체적으로 24well 판에 HEI-OC1 세포를 각 well에 40% 정도 분주 한 후 5% 이산화탄소의 배양조건에서 배양한다. 다음 날 50% 정도의 세포가 자라면 시스플라틴(15 μM)과 넬피나비르(6μM)를 처리 한 후 30시간뒤 세포의 생존 응력을 정하기 위해 MTT(0.5mg)를 4시간 동안 5% 이산화탄소의 배양조건에서 배양한다. 4시간 후 배양액을 빨아들인후 상온에 말린 다음 디메틸 설폭 시드(DMSO)에 용해시키고, 각 배양 well의 흡광도(OD)를 측정 한다. 그 결과, 시스플라틴(15 uM)의 단독처리는 HEI-OC1 세포의 생존율을 40%까지 감소시키는 반면, 시스플라틴과 넬피나비르(6 uM)를 함께 처리하였을 때는 생존율을 80%까지 증가시켰다(도 3A). 또한 넬피나비르 단독처리는 세포 생존에 영향을 미치지 않았다. 상기 결과는 넬피나비르가 시스플라틴 처리에 의한 청각유모세포사멸을 억제하는 효과가 있음을 나타낸다.
또한, 넬피나비르의 청각유모세포사멸 관련 인자 억제 효과를 확인하고자, 청각유모세포주인 HEI-OC1세포를 이용하여 세포사멸 관련 인자의 발현 변화를 western blot분석을 이용하여 관찰하였다. 60cm 접시에 60% HEI-OC1 세포를 분주후 5% 이산화탄소의 배양조건에서 배양한다. 세포가 70% 정도 자라면 시스플라틴(15μM)과 넬피나비르(6μM) 처리 후 24시간 동안 5% 이산화탄소의 배양조건에서 배양한다. 24시간 후 차가운 1xPBS로 세척 후 세포를 수확하고 1xRIPA를 이용해 세포를 터트린 후 원심분리기를 이용해 단백질을 추출한다. 단백질을 정량 후 SDS-PAGE 를 이용해 전기영동을 하고 젤에 있는 단백질을 막으로 옮긴 후 항체들이 필요 없는 부위에 붙는 것을 막기 위해 5% 우유를 사용해 3시간 상온 처리를 한 후 3% 우유로 항원과 항체반응을 이용해 현상을 한 후 단백질 분석을 진행한다. 그 결과, 시스플라틴에 의해 세포사멸을 유도하는 인자인 cleaved PARP-1, active caspase 3 및 phosphor-H2AX 단백질 양이 증가하는 반면, 시스플라틴과 넬피나비르를 함께 처리하였을 때는 상기 cleaved PARP-1, active caspase 3 및 phosphor-H2AX 발현이 감소하여, 넬피나비르가 시스플라틴에 의한 청각유모세포사멸을 억제하는 것을 단백질 수준에서 확인하였다(도 3B).

발현 억제 효과
시스플라틴이 이독성을 유발하는 주요 원인으로 TNF-α, IL-1β, IL-6, NO, ROS, STAT, ERK 및 NF-κB등 염증매개인자들이 알려져 있으므로, 넬피나비르의 이독성 치료효과를 확인하기 위하여, 염증매개인자들인 TNF-α 및 IL-6의 mRNA발현을 RT-PCR으로 확인하였다. 6 well에 60% HEI-OC1 세포를 분주 후 5% 이산화탄소의 배양조건에서 배양한다. 세포가 70%정도 자라면 시스플라틴(15μM)과 넬피나비르(6μM) 처리후 30시간 동안 5% 이산화탄소의 배양조건에서 배양한다. 30시간 후 TRIZOL을 각 well당 1mL씩 분주 후 잘 섞고 1.5mL 관에 옮긴 후 얼음에서 실험을 진행한다. 200 μl의 Chloroform을 각 1.5mL 관에 분주 한 후 잘 섞은 후 4도의 원심분리기를 이용해 상층액을 1.5mL 관에 옮긴 후 다시500 μl의 2-propanol을 사용해 잘 섞은 후 원심분리기를 이용해 RNA를 침전시키고 70%의 에탄올로 세척 후 DW에 녹여 RNA 정략을 해 Rochet사의 Transcriptor First strand cDNA Synthesis Kit를 이용해 RNA와 olig(dT)를 섞은 후 PCR기계를 이용해 65도에서 10분 동안 반응후 PCR기계를 이용해 55도에서 30분 85도에서 5분 후 cDNA가 완성되면 프라이머와 싸이버그린과 함께 RT-PCR (Hold 95도 for 600초, 3-step Amplification 45 cycle of 95도 to 54도 to 72도, Melting 65도 to 95도 at 0.1도c/s)을 실행한다. 마크로젠에서 주문한 마우스 프라이머를 사용하였고, 프라이머 서열을 표 1에 나타내었다.

그 결과, 시스플라틴으로 이독성을 유발한 군에서 염증성 사이토카인인 TNF-α 및 IL-6 mRNA발현이 증가하는 것을 확인하였으며, 넬피나비르에 의해 TNF-α 및 IL-6 mRNA 발현이 억제되는 것을 확인하였다(도 4A, B).

활성 억제 효과
NF-κB는 염증매개인자들의 발현을 조절하는 단백질로써, NF-κB는 IκB와 결합에 의해 활성이 억제되어 있다가, IκB의 인산화에 의해 결합이 해제되면 핵 내로 이동하여 염증매개인자들의 발현을 유도한다. 따라서, 넬피나비르의 NF-κB 활성 조절 효과를 확인하기 위해 HEI-OC1 세포의 핵분획과 세포질 분획을 얻어 western blot으로 분석함으로써, NF-κB(Sant Cruz)의 핵으로 이동 변화를 확인하였다. CREB(Cell signaling) 및 β-actin(Sant Cruz)은 각각 핵 분획과 세포질 분획의 마커로 사용하였다. 그 결과, 도 4C에 나타낸 바와 같이, 시스플라틴에 의해 NF-κB가 핵으로 이동하는 것이 관찰되었으며, 넬피나비르가 NF-κB의 핵으로 이동을 억제하는 것을 확인하였다. 또한, 넬피네비르의 NF-κB의 핵내 이동억제 효과를 면역형광염색을 이용하여 재차 확인하였다. 그 결과, 시스플라틴 처리군에서 NF-κB가 핵을 염색하는 DAPI와 같은 위치에서 발현되는 것을 관찰하여, NF-κB가 핵내로 이동하는 것을 것을 확인하였고, 이것이 넬피나비르에 의해 억제되는 것을 관찰하였다(도 5). 따라서, 상기 결과는 넬피나비르가 염증인자의 활성을 효과적으로 억제함으로써 시스플라틴에 의한 청각유모세포의 사멸과 청력손상을 억제하는 효과가 있음을 나타낸다.
