Abstract

Description
本発明は、人工股関節の寛骨臼コンポーネントに関する。
従来、人工股関節は、人工骨頭置換術(BHA)や人工股関節全置換術(THA)によって人体に取り付けられる。BHAは、大腿骨頸部骨折や大腿骨頭壊死症等の疾患に対して、大腿骨頭側のみ置換する手術であり、THAは、変形性股関節症や大腿骨壊死症等の疾患に対して、大腿骨頭側と臼蓋側の両方を置換する手術である。BHA用の人工股関節は、大腿骨ステム、ステムヘッド及びステムヘッドに取り付けられ臼蓋に配置されるバイポーラカップを備えている。一方、THA用の人工股関節は、大腿骨ステム、ステムヘッド及びステムヘッドに取り付けられ、臼蓋にスクリューで取り付けられる、臼蓋側に相当する寛骨臼ライナー及び寛骨臼シェルを備えている。
ここで、特許文献1に示されるように、寛骨臼ライナーが寛骨臼シェルに取り付けられた状態において、寛骨臼ライナーの底面は寛骨臼シェルの底面より突出するようになっているので、手術時に、寛骨臼ライナーが寛骨臼シェルに正常に取り付けられた状態と、寛骨臼ライナーが傾いて寛骨臼シェルに取り付けられた状態や寛骨臼ライナーが寛骨臼シェルに取り付け途中である状態とを区別することが困難であった。
そこで、本発明では、寛骨臼ライナーが寛骨臼シェルに取り付けられた状態の確認を容易に行うことができる寛骨臼コンポーネントを提供することを目的とする。なお、ここでは、寛骨臼ライナーと寛骨臼シェルとを合わせて寛骨臼コンポーネントと呼ぶ。

前記構成によれば、寛骨臼ライナーが寛骨臼シェルに取り付けられた状態において、寛骨臼ライナーの底面が、寛骨臼シェルの底面より内方に位置しているので、寛骨臼ライナーが寛骨臼シェルに正常に取り付けられたことを目視や触覚での確認により、容易に確認することができる。

前記構成(1)によれば、マーカーによって、寛骨臼シェルの曲面におけるスクリューホールが形成されている部分を目視又は触覚(マーカーが溝等の場合)で確認できるので、寛骨臼シェルから寛骨臼ライナーを取り外す際、スクリューホールが形成されていない部分に、寛骨臼ライナー取外器を容易に挿入することができる。
前記構成(2)によれば、非対応部分にマーカーが形成されていることにより、マーカーが形成されている位置に寛骨臼ライナー取外器を挿入することとなり、寛骨臼ライナー取外器を挿入する位置を間違えにくい。
前記構成(3)によれば、寛骨臼シェルの球面部の内周面に形成された凹部と、寛骨臼ライナーの球面部の外周面に形成された突起部が係合することにより、寛骨臼シェルに寛骨臼ライナーを取り付けることができる。
前記構成(4)によれば、突起部の上下で寛骨臼ライナーの球面部の曲率を変更することによって、寛骨臼ライナーが寛骨臼シェルに挿入しやすく、外れにくい、寛骨臼ライナーの具体的な構造を提供できる。
前記構成(5)によれば、突起部を外周面の回旋防止タブの近傍に設けることによって、寛骨臼ライナーが寛骨臼シェルに挿入しやすく、外れにくい構造とすることができる。
要するに、本発明によると、寛骨臼ライナーが寛骨臼シェルに取り付けられた状態の確認を容易に行うことができる寛骨臼コンポーネントを提供することができる。
図1は、本発明の実施形態に係る人工股関節10の分解概略図である。図1に示されるように、人工股関節10は、大腿骨ステム1、ステムヘッド2、寛骨臼コンポーネント3を備えている。
大腿骨ステム1は、本体部11及びネック部12を備えている。本体部11は、大腿骨髄腔内に挿入されるようになっており、ネック部12は、ステムヘッド2が取り付けられるようになっている。
ステムヘッド2は、略球欠形状を有しており、底面から内方に凹む略球欠形状の収容空間が形成されている。大腿骨ステム1のネック部12がステムヘッド2の収容空間に係合されるようにして、ステムヘッド2は大腿骨ステム1に取り付けられている。
寛骨臼コンポーネント3は、臼蓋にスクリューで取り付けられる寛骨臼シェル4と、寛骨臼シェル4に取り付けられ、ステムヘッド2が摺動する寛骨臼ライナー5と、を備えている。寛骨臼シェル4の母材には、チタン合金、純チタン、コバルトクロム合金、ステンレス等が使用され、臼蓋と接触する面には、コーティング材として、純チタン、チタン合金、ハイドロキシアパタイト、リン酸水素カルシウム等が使用されている。また、寛骨臼ライナー5の母材には、高度架橋超高分子量ポリエチレン、超高分子量ポリエチレン、セラミック等が使用され、添加剤として、ビタミンE等が使用されている。
図2は、寛骨臼コンポーネント3の分解概略図である。図2に示されるように、寛骨臼シェル4は、略半球形状を有しており、底面41から内方に凹む略半球形状の収容空間42が形成されている。収容空間42は、底面41に対して開口している。寛骨臼シェル4は、底面41を構成する底部43と、球面を構成する球面部44と、を備えている。球面部44には、臼蓋に寛骨臼シェル4をスクリューで取り付けるためのスクリューホール45が球面部44を貫通するように複数形成されている。寛骨臼シェル4の内周面46には、底面41に略平行に、内方に凹む円周状の凹部47が形成されている。凹部47の高さは0.2~1.2mm程度であり、中心軸X方向長さ(幅)は0.5~3.0mm程度である。好ましくは、凹部47の高さは0.2~0.6mm程度であり、幅は0.5~1.5mm程度である。ここで中心軸X方向とは、寛骨臼ライナー5の底面51の中心51cと頂部56の中心56cとを結ぶ方向すなわち、上下方向を示している。
寛骨臼ライナー5は、略半球形状を有しており、底面51から内方に凹む略半球形状の収容空間52が形成されている。収容空間52は、底面51に対して開口している。寛骨臼ライナー5は、底面51を構成する底部53と、球面を構成する球面部54と、を備えている。底部53は、球面部54より外周方向に突出するフランジ形状を有しており、寛骨臼ライナー5が寛骨臼シェル4に対して回旋しないように、回旋防止タブ53aが底部53に形成されている。なお、球面部54の頂部56は、平坦となっている。
図3は、図2の寛骨臼ライナー5の一部拡大図である。図2及び図3に示されるように、球面部54の外周面には、寛骨臼シェル4の凹部47に係合する、外方に突出する突起部55が形成されている。突起部55は、底面51に平行に、円周状に形成されている。突起部55は、球面部54の外周面に対して外方に突出しており、その高さは、0.1~1.0mm程度である。突起部55の中心軸X方向長さ(幅)は、凹部47の幅より小さくなっており、0.4~2.8mm程度である。好ましくは、突起部55の高さは0.1~0.5mm程度であり、幅は0.4~1.4mm程度である。
突起部55は、寛骨臼ライナー5の球面部54の外周面において、回旋防止タブ53aの近傍に設けられている。具体的には、突起部55の頂点55aと底面51の中心軸X方向における距離は4.0~10.0mm程度であり、好ましくは5.0~8.0mmである。
球面部54の外周面において、突起部55より球面部54の頂部側54aの曲率と、突起部55より底部側54bの曲率とが異なっており、頂部側54aの曲率は、底部側54bの曲率より大きくなっている。
図4は、寛骨臼ライナー5が寛骨臼シェル4に取り付けられている状態を示す断面図である。図4に示されるように、寛骨臼ライナー5は、寛骨臼シェル4の収容空間42に収容され、寛骨臼ライナー5の底面51は、寛骨臼シェル4の底面41より内方に位置するようになっている。また、寛骨臼ライナー5の突起部55は、寛骨臼シェル4の凹部47に係合している。
図5は、寛骨臼ライナー5が寛骨臼シェル4に取り付けられている状態を、寛骨臼ライナー5及び寛骨臼シェル4の底面側から見たときの概略図である。図5に示されるように、寛骨臼シェル4の底面41には、寛骨臼シェル4の球面部44におけるスクリューホール45が形成されている部分を示すマーカー48が形成されている。ここで、本実施形態では、マーカー48は、球面部44におけるスクリューホール45が形成されていない部分に対応する底面41の非対応部分に形成されており、その結果、マーカー48は、寛骨臼シェル4の球面部44におけるスクリューホール45が形成されている部分を間接的に示すようになっている。このようなマーカー48は底面41上に複数形成してもよい。
より具体的には、マーカー48は、マーカー48と球面部44の頂点43Tとを結ぶ測地線43Lがスクリューホール45を通過しないように、寛骨臼シェル4の底面41に形成されており、この場合、底面41の非対応部分にマーカー48が形成されているとする。
マーカー48は、目視で確認できるような凹凸のない印でもよく、また、目視や触覚で確認できるような凹凸のある溝やブラスト加工でもよい。マーカー48の大きさは、マーカーの形状が四角形の場合、1辺が0.5~3.0mm程度、好ましくは、1.0~1.5mm程度であり、マーカーの形状が円形の場合、直径0.5~3.0mm程度、好ましくは、1.0~1.5mm程度である。マーカー48は、レーザーマーキングや陽極酸化処理等により形成されることができる。なお、マーカー48の形状は、四角形、三角形、円、星形状等、どのような形状でもよい。
前記構成の寛骨臼コンポーネント3によれば、次のような効果を発揮できる。
(1)寛骨臼ライナー5が寛骨臼シェル4に取り付けられている状態において、寛骨臼ライナー5の底面51は、寛骨臼シェル4の底面41より内方に位置しているので、寛骨臼ライナー5が寛骨臼シェル4に取り付けられた状態の確認を容易に行うことができる。
(2)寛骨臼シェル4の底面41には、球面部44におけるスクリューホール45が形成されている部分を示すマーカー48が形成されており、マーカー48によって、寛骨臼シェル4の球面部44におけるスクリューホール45が形成されている部分を目視又は触覚(マーカーが溝等の場合)で確認できるので、寛骨臼シェル4から寛骨臼ライナー5を取り外す際、スクリューホール45が形成されていない部分に、寛骨臼ライナー取外器を容易に挿入することができる。
(3)マーカー48は、球面部44におけるスクリューホール45が形成されていない部分に対応する底面41の非対応部分に形成されていることにより、マーカー48が形成されている位置に寛骨臼ライナー取外器を挿入することとなり、寛骨臼ライナー取外器を挿入する位置を間違えにくい。
(4)寛骨臼ライナー5の球面部54の外周面において、突起部55の上下で寛骨臼ライナー5の球面部54の曲率を変更することによって、寛骨臼ライナー5が寛骨臼シェル4に挿入しやすく、外れにくい構造とすることができる。
(5)より具体的には、球面部54の外周面において、突起部55より頂部側の曲率は、突起部55より底部側の曲率より大きくなっているので、寛骨臼ライナー5が寛骨臼シェル4に挿入しやすく、外れにくい、寛骨臼ライナー5の構造を提供できる。
(6)突起部55は、球面部54の外周面において、回旋防止タブの近傍に設けられているので、寛骨臼ライナー5が寛骨臼シェル4に挿入しやすく、外れにくい構造とすることができる。
上記実施形態では、マーカー48は、球面部44におけるスクリューホール45が形成されていない部分に対応する底面の非対応部分に形成されているが、逆に、球面部44におけるスクリューホール45が形成されている部分に対応する底面の対応部分に形成されてもよい。この場合、マーカー48が形成されていない位置に寛骨臼ライナー取外器を挿入することになる。
上記実施形態では、寛骨臼ライナー5の突起部55は、底面51に平行に、円周状に形成されているが、寛骨臼ライナー5の底面51に平行な円周上を断続的に形成されてもよい。なお、突起部55と係合する寛骨臼シェル4の凹部47は、底面41に平行に円周状に連続的に形成されることが好ましい。
上記実施形態では、寛骨臼ライナー5の回旋防止タブ53aは、寛骨臼ライナー5の底面51に形成されているが、図6に示されるように、最底面51aからオフセットした位置に設けられてもよい。その場合、本発明の寛骨臼ライナー5の底面51は、回旋防止タブ53aの下面に相当する。なお、寛骨臼ライナー5に回旋防止タブが形成されない場合、本発明の寛骨臼ライナー5の底面は、寛骨臼ライナー5の最底面に一致する。
特許請求の範囲に記載された本発明の精神及び範囲から逸脱することなく、各種変形及び変更を行うことも可能である。
本発明では、寛骨臼ライナーが寛骨臼シェルに取り付けられた状態の確認を容易に行うことができる寛骨臼コンポーネントを提供できるので、産業上の利用価値が大である。

- 人工股関節の寛骨臼コンポーネントであって、
臼蓋に取り付けられる寛骨臼シェルと、
前記寛骨臼シェルに取り付けられる寛骨臼ライナーと、を備えており、
前記寛骨臼ライナーが前記寛骨臼シェルに取り付けられている状態において、前記寛骨臼ライナーの底面は、前記寛骨臼シェルの底面より内方に位置している、寛骨臼コンポーネント。
- 前記寛骨臼シェルの球面部には、臼蓋にスクリューで取り付けられるためのスクリューホールが形成されており、
前記寛骨臼シェルの底面には、前記球面部における前記スクリューホールが形成されている部分を示すマーカーが形成されている、請求項1記載の寛骨臼コンポーネント。
- 前記マーカーは、前記球面部における前記スクリューホールが形成されていない部分に対応する前記底面の非対応部分に形成されている、請求項2記載の寛骨臼コンポーネント。
- 前記寛骨臼ライナーの球面部の外周面には、前記寛骨臼シェルに形成された凹部に係合する突起部が形成されており、
前記外周面において、前記突起部より前記外周面の頂部側の曲率と、前記突起部より前記外周面の底部側の曲率とが異なっている、請求項1記載の寛骨臼コンポーネント。
- 前記寛骨臼ライナーの球面部の外周面には、前記寛骨臼シェルに形成された凹部に係合する突起部が形成されており、
前記外周面において、前記突起部より前記外周面の頂部側の曲率と、前記突起部より前記外周面の底部側の曲率とが異なっている、請求項2記載の寛骨臼コンポーネント。
- 前記寛骨臼ライナーの球面部の外周面には、前記寛骨臼シェルに形成された凹部に係合する突起部が形成されており、

- 前記外周面において、前記突起部より前記頂部側の曲率は、前記突起部より前記底部側の曲率より大きくなっている、請求項4記載の寛骨臼コンポーネント。
- 前記外周面において、前記突起部より前記頂部側の曲率は、前記突起部より前記底部側の曲率より大きくなっている、請求項5記載の寛骨臼コンポーネント。
- 前記外周面において、前記突起部より前記頂部側の曲率は、前記突起部より前記底部側の曲率より大きくなっている、請求項6記載の寛骨臼コンポーネント。
- 前記突起部は、前記外周面において、前記寛骨臼ライナーの回旋防止タブの近傍に設けられている、請求項4記載の寛骨臼コンポーネント。
- 前記突起部は、前記外周面において、前記寛骨臼ライナーの回旋防止タブの近傍に設けられている、請求項5記載の寛骨臼コンポーネント。
- 前記突起部は、前記外周面において、前記寛骨臼ライナーの回旋防止タブの近傍に設けられている、請求項6記載の寛骨臼コンポーネント。
