Abstract
면역요법제, 예를 들어 면역관문 억제제와 병용하는 암 치료제가 개시된다.
Description
본 개시는 암 치료제에 관한 것으로서, 보다 상세하게는 유효성분으로 보툴리눔 독소를 포함하는, 면역요법제와 병용하는 암 치료제에 관한 것이다.
보툴리눔 독소(Botulinum neurotoxin, BoNT)는 혐기성 박테리아 클로스트리디움 보툴리눔(Clostridium botulinum)의 폴리펩티드 생성물로서, 신경세포에 특이적으로 작용하는 독성물질이다. 보툴리눔 독소는 본래 치사를 일으키는 독성 물질이지만, 연축성 사경(cervical dystonia: CD), 안검경련(blepharospasm), 다한증(hyperhidrosis), 사시(strabismus), 이완불능(achalasia), 신경성 방광(neurogenic bladder), 비뇨기 질환(urologic disease), 편두통(migraine) 등의 치료를 위해 사용되고 있다.
국제공개 WO2010/062955호는 신생물 근처의 비-신생성 부위(nonneoplastic area)에 치료 유효량의 보툴리눔 독소를 항암 약물이나 항암 요법과 병용 투여하되, 치료 유효량의 보툴리눔 독소가 신생물을 투과(penetrate)하지 않는, 신생물의 성장이나 전이를 저해하는 방법을 개시하고 있다. 또한, 국제공개 WO2006/094539호는 보툴리눔 독소를 포함하는 제약 조성물을 암 조직 또는 세포에 투여하는 단계를 포함하는, 화학요법 또는 방사선 요법과 같은 세포독성 요법에 의한 암 치료를 감작시키는(sensitizing) 방법 및 이를 위한 조성물을 개시하고 있다.
항암 면역요법(immunotherapy)은 인체의 면역체계를 활성화시켜 자가면역력을 높여 면역세포가 암세포를 공격하도록 하는 치료법이다. 면역요법제는 크게 면역관문 억제제(immune checkpoint inhibitor)와 면역세포 치료제(immune cell therapy), 치료용 항체(therapeutic antibody), 항암 백신(anticancer vaccine), 항암 바이러스(oncolytic virus)로 분류할 수 있다. 면역관문 억제제는 T 세포 억제에 관여하는 면역관문 단백질(immune checkpoint protein)의 활성을 차단하여 T 세포를 활성화시켜 암세포를 공격하는 약제로서, 대표적으로 CTLA-4, PD-1, PD-L1을 인식하는 항체를 사용한다. 면역세포 치료제는 환자 체내의 T 세포를 채취하여 강화 및 변형시켜 다시 체내에 주입하여 암세포에 대한 세포성 면역을 강화시키는 약제로 NK 세포치료제, T 세포치료제, CAR-T 세포치료제 등이 있다. 치료용 항체는 항체-약물 결합체가 암세포에 결합하면 약물이 유리되어 암세포를 공격하는 약제로서 항체-약물 접합체(antibody-drug conjugate, ADC)를 대표적인 예로 들 수 있다. 항암 백신은 암세포가 갖고 있는 종양특이적 항원 또는 체내 전반적인 면역반응을 향상시킬 수 있는 단백질 또는 펩티드 분자를 암환자에게 투여하여 면역체계를 활성화시킴으로써 체내 면역기능이 활발하게 만들어 암세포를 공격하도록 하는 것이다. 항암 바이러스는 암을 공격하는 바이러스로 직접 암세포를 사멸시키는 작용을 하지만 이에 더하여 면역세포의 활성화를 유도하여 항암 능력을 증강시킨다.
지금까지 보툴리눔 독소를 면역요법제와 병용하여 암을 치료하려는 시도는 알려져 있지 않았다.
본 개시의 제1 목적은 유효성분으로 보툴리눔 독소를 포함하는, 면역요법제와 병용하여 암을 치료하기 위한 조성물을 제공하는 것이다.

일 태양에서, 면역요법제는 각각의 사용 목적에 맞게 통상의 방법에 따라 액제, 현탁제, 산제, 과립제, 정제, 캡슐제, 환제, 엑스제, 에멀젼, 시럽제, 에어로졸 등의 경구 제형, 멸균 주사용액의 주사제 등의 비경구 제형과 같이 다양한 형태로 제형화하여 사용할 수 있다. 면역요법제는 경구 투여되거나, 정맥 내, 복강 내, 피하, 피내, 근육 내, 척추, 척추강 또는 직장 내 국소 투여 또는 주입 등을 포함한 다양한 경로를 통해 비경구 투여될 수 있다. 투여량은 환자의 체중, 연령, 성별, 건강상태, 식이, 투여 시간, 투여 방법, 투여 기간 또는 간격, 배설율, 체질 특이성, 제제의 성질, 질환의 중증도 등에 따라 그 범위가 다양할 수 있으며, 통상의 기술자에 의해 적절하게 선택될 수 있다. 예를 들어, 약 0.1 내지 10,000 ㎎/㎏의 범위일 수 있으나 이에 제한되지 않으며, 하루 일회 내지 수 회에 나누어 투여될 수 있다.
일 태양에서, 엑소좀-매개된 암을 치료하는 방법은 하기 단계를 포함하는 것일 수 있다:
1) 대상으로부터 수득된 생물학적 시료에서 분비된 엑소좀을 정량하는 단계; 및,
2) 엑소좀 분비가 증가된 대상에게 치료 유효량의 보툴리눔 독소를 면역요법제와 단일의 제제 또는 별개의 제제로 동시에 또는 순차적으로 병용하여 투여하는 단계.
분비된 엑소좀은 당업계에 알려져 있는 통상적인 방법에 따라, 대상의 생물학적 시료, 예를 들어 혈액으로부터 분리하여 정량할 수 있으며, 시료의 종류, 분리 및 정량 방법에 특별한 제한이 있는 것은 아니다.
보툴리눔 독소 또는 이를 포함하는 조성물이 투여되는 대상은 인간이나 동물, 예를 들어 인간, 돼지, 개, 고양이, 소, 말, 쥐 등을 제한 없이 포함할 수 있다.
본 개시에 따르면, 보툴리눔 독소 또는 이를 포함하는 조성물은 면역요법제, 예를 들어 면역관문 억제제와 병용하여 암 치료제로 유용할 수 있다.

이하 본 개시를 실시예를 들어 보다 상세히 설명하나, 이는 본 개시를 설명하기 위한 것일 뿐 그 범위를 어떤 식으로든지 제한하고자 하는 것은 아니다.
실시예 1: 마우스 악성 흑색종 이식 모델에 대한 보툴리눔 독소와 항-PD-1 항체의 항암 효능 시험
수컷 6주령 C57BL/6(구입처: 오리엔트바이오) 마우스를 구입하여 1주간 순화 및 검역 후 7주령 마우스를 실험에 사용하였다. 사료는 멸균된 실험동물용 고형사료(R40-10, SAFE, France)를 자유 급이하였으며, 음수는 상수도수를 고온고압멸균(autoclave)하여 자유 급이하였다. 순화, 검역 및 실험기간 동안 마우스는 온도 23±3 ℃, 상대습도 55±15%, 조명시간 12시간(오전8시~오후8시), 환기횟수 15회/시간 및 조도 150~300 Lux로 설정된 특정-병원체-부재 조건 하에서 사육되었다. 본 실험은 ㈜메디톡스 동물실험윤리위원회의 검토 및 승인(A-2020-004) 후 수행되었다.
B16-F10 마우스 악성 흑색종 세포주(KCLB No: 80008)를 한국세포주 은행(KCLB: Korean Cell Line Bank)으로부터 수득하였다. B16-F10 세포를 10% FBS(CAT No: 10082-147, Gibco) 및 1% 페니실린-스트렙토마이신(CAT No: 15140-122, Gibco) 함유 DMEM 배지(CAT No: 11965-092, Gibco)에서 37 ℃, 5% CO2 조건으로 인큐베이터에서 배양하였다. 마취된 C57BL/6 마우스 우측 옆구리에 주사기를 이용하여 5×105 세포를 0.1 ㎖ 이식하였다.
비히클로서 보툴리눔 독소 조제에 사용된 멸균 생리식염수(Lot No: M8T7AF3, 대한약품공업)를 사용하였으며, 보툴리눔 독소로서 100 units 코어톡스 (Lot No: NSA19007A, 메디톡스)를 사용하였다. 항-PD-1 항체(Clone: RMP1-14, Cat No: BE0146, Lot No: 760220J1)는 Bio X cell에서 구매하여 사용하였다.
각 시험물질은 아래 표 1에 따라 조제하여 투여하였다. 보툴리눔 독소 100 units 바이알에 멸균 생리식염수 6.667 ㎖을 첨가하여 15 units/㎖ 농도로 조제하여 1 ㎖/㎏, 15 units/㎏ 용량으로 종양 내로 투여하였다. 항-PD-1 항체는 투여일에 멸균 PBS(Cat No: 10010, Gibco)를 이용하여 최종 1 mg/㎖ 농도로 조제하여 5 ㎖/㎏, 5 ㎎/㎏ 용량으로 복강 내로 투여하였다.
마우스 악성 흑색종 이식 후 8일째 체중 및 종양 크기를 측정하여 평균 약 33 ㎣ 크기에서 마우스를 시험군에 따라 각 그룹 당 10마리씩 시험군간 차이가 없게 무작위 배정하였다. 종양 이식 후 8일째 체중을 바탕으로 비히클 및 보툴리눔 독소를 표 1에 따라 종양 내로 투여하였다. 항-PD-1 항체의 경우 표 1에 따라 종양 이식 8, 11, 14, 17일 째 복강 내로 투여하였다. 종양 이식 후 주 2회 종양 크기를 측정하였다. 종양 크기는 버니어 캘리퍼로 측정하였고 측정치를 사용해 다음의 식을 이용하여 종양 부피를 계산하였다.

종양 이식 후 19일째, 종양의 크기가 2000 ㎣에 도달하기 전에 모든 동물을 안락사하였다. 종료 후 종양 조직을 적출하고 무게를 측정하였으며, 종양 조직 내 T 세포의 수를 유세포분석기(FACSVerse, BD)를 이용하여 측정하였다. 종양 조직 내 T 세포 분석은 종양 조직을 gentle MACS C tube(Cat No: 130-093-237, Miltenyi Biotec)를 이용해 단세포로 분리 후 LIVE/DEAD Fixable Aqua Dead Cell Stain(Cat No: L34957, Invitrogen), Fc block(Cat No: 553142, BD), CD45-PECy7(Cat No: 25-0451-82, eBioscience), TCRbeta-FITC(Cat No: 109206, Biolegend), CD4-pacific blue(Cat No: 100428, Biolegend), CD8a-PerCPCy55(Cat No: 100734, Biolegend) 항체로 염색하여 유세포분석기로 분석하였다. 종양 내 CD4+ 및 CD8+ T 세포의 수는 종양 무게를 바탕으로 계산되었으며, 비율은 생존세포 기준으로 CD45+TCRbeta+CD4+ 및 CD45+TCRbeta+CD8a+ T 세포의 비율을 나타내었다.
혈액 내 엑소좀을 Exoquick exosome precipitation solution(Cat No: EXOQ20A-1, Lot No: 200107-001, System Biosciences)을 이용하여 분리하였고 ExoELISA-ULTRA Complet Kit(Cat No: EXEL-ULTRA-CD63-1, Lot No: 200226-002, System Biosciences)을 이용해 측정하였다.
Graphpad Prism(version 7.05, GraphPad Software Inc, CA, USA)은 그래프 제시에 사용되었으며 SPSS software(version 25.0, SPSS Inc, IL, USA)는 통계 분석에 사용되었다. 모수 데이터는 양측(two-tailed) t-검정을 수행하였고 비 모수 데이터는 Mann-Whitney 검정 통계분석을 유의 수준 p = 0.05에서 수행되었다.
종양 성장 억제율 측정 결과를 표 2 및 도 1에 나타내었다.

표 2 및 도 1에 나타낸 바와 같이, 마우스 악성 흑색종 이식 후 18일째, 보툴리눔 독소 단독 종양 내 투여에 의해 평균 1433 ㎣의 종양 크기와 15%의 종양 성장 억제율을 보였으며, 항-PD-1 항체는 평균 1415 ㎣, 16% 종양 성장 억제율을 보였으나 비히클 투여군과 비교하여 통계적 유의미성은 없었다. 이에 비해, 보툴리눔 독소와 항-PD-1 항체를 병용 투여한 경우 평균 종양 크기가 평균 489 ㎣, 71% 종양 성장 억제율을 보여 각각을 단독 투여한 경우 보다 유의적으로 종양 성장이 억제된 것을 확인하였다. 또한, 종양 이식 후 19일째 종양을 적출하여 무게를 측정한 경우에도 보툴리눔 독소와 항-PD-1 항체를 병용 투여한 경우 비히클 투여군과 보툴리눔 독소, 항-PD-1 항체 단독 투여군 보다 유의적으로 종양의 무게가 감소되었다.
종양 조직을 적출하여 종양 내 침윤된 CD4+ 및 CD8+ T 세포를 분석한 결과, 보툴리눔 독소를 단독 투여한 경우 종양 내 CD8+ T 세포의 수가 비히클 처리군 대비 증가하는 경향을 보인다(도 2a, 도 2b). CD4+ 및 CD8+ T 세포의 비율은 보툴리눔 독소 및 항-PD-1 항체를 단독 투여한 경우 비히클 처리군과 유의미한 차이가 없으나, 보툴리눔 독소와 항-PD-1 항체를 함께 처리한 군에서 비히클군에 비하여 유의적으로 많은 비율의 CD4+ 및 CD8+ T 세포가 종양 내 침윤된 것을 확인할 수 있다(도 2c, 도 2d). 이는 항-PD-1 항체 단독 처리군보다 유의적으로 증가된 것이다. 보툴리눔 독소와 항-PD-1 항체를 함께 처리한 군에서 두 물질간 상승작용에 의해 종양 내 T 세포가 유의적으로 증가되어 종양 성장이 유의적으로 감소되었음을 설명할 수 있다.
혈액 내 엑소좀의 수를 측정한 결과를 도 3에 나타내었다. 도 3에 나타낸 바와 같이, 혈액 내 엑소좀의 수는 보툴리눔 독소 투여군에서 비히클 처리군 대비 감소하는 경향을 보이며, 항-PD-1 항체 및 보툴리눔 독소+항-PD-1 항체 투여군에서 유의적으로 감소되었다. 또한, 보툴리눔 독소와 항-PD-1 항체 병용 투여군은 보툴리눔 독소 및 항-PD-1 단독 투여군에 비해 혈액 내 엑소좀의 수가 유의적으로 감소되었다.
실시예 2: 마우스 악성 흑색종 이식 모델에 대한 용량별 보툴리눔 독소와 항-PD-1 항체의 항암 효능 시험

마우스 악성 흑색종 이식 후 8일째 종양 크기를 측정하여 마우스를 시험군에 따라 각 그룹 당 12마리씩 시험군간 차이가 없게 무작위 배정하였다. 종양 이식 후 8일째 비히클 및 보툴리눔 독소를 시험군 구성에 따라 종양 내로 투여하였다. 항-PD-1 항체의 경우 표 3에 따라 주 2회 총 4회 복강 내로 투여하였다. 종양 이식 후 주 2회 종양 크기를 측정하였다. 종양 크기는 버니어 캘리퍼로 측정하였고 측정치를 사용해 실시예 1에 나타낸 식을 이용하여 종양 부피를 계산하였다.

Graphpad Prism(version 7.05, GraphPad Software Inc, CA, USA)은 그래프 제시에 사용되었으며 SPSS software(version 25.0, SPSS Inc, IL, USA) 및 Excel (2013, MS, USA)는 통계 분석에 사용되었다. 종양 크기는 양측(two-tailed) t-검정을 수행하였고, 생존율은 만텔-콕스 로그 등급분석을 유의수준 p = 0.05에서 수행되었다.

p<0.001). 도 4에 나타낸 바와 같이, 마우스 악성 흑색종 모델에서 보툴리눔 독소를 1, 3, 10 U/㎏ 용량으로 단회 종양 내 투여한 경우 14일째 10 U/㎏ 투여군에서 유의적인 종양 성장 억제가 관찰되었다. 종양 이식 후 18일째에는 보툴리눔 독소를 단독 투여한 시험군과 비히클 + 5 ㎎/㎏ 항-PD1 항체를 투여한 시험군에서 비히클 투여군 대비 유의적인 종양 성장 억제 효과는 관찰되지 않았다. 그러나, 3 또는 10 U/㎏ 보툴리눔 독소와 5 ㎎/㎏ 항-PD1 항체를 병용 투여한 시험군에서는 비히클 투여군 대비 유의적인 종양 성장 억제 효과를 관찰할 수 있었다.
생존률 측정 결과를 도 5에 나타내었다. 도 5에 나타낸 바와 같이, 3 또는 10 U/㎏ 보툴리눔 독소와 5 ㎎/㎏ 항-PD1 항체를 병용 투여한 시험군에서 유의적으로 마우스 생존율이 증가되었다. 또한 비히클 + 5 ㎎/㎏ 항-PD1 투여군에서도 유의적인 생존율 증가가 관찰되었다. 상기 결과로부터 마우스 흑색종 모델에서 보툴리눔 독소 3 U/㎏ 이상의 용량에서 항-PD1 항체와 병용 투여할 경우 종양 성장 억제 효과가 있음을 확인하였다.
실시예 3: 마우스 악성 흑색종 이식 모델에서 복합체화된 보툴리눔 독소와 비-복합체화된 보툴리눔 독소의 항-PD-1 항체 병용 투여 항암 효능 시험
수컷 6주령 C57BL/6(구입처: 오리엔트바이오) 마우스를 구입하여 1주간 순화 및 검역 후 7주령 마우스를 실험에 사용하였다. 사료는 멸균된 실험동물용 고형사료(R40-10, SAFE, France)를 자유 급이하였으며, 음수는 상수도수를 고온고압멸균(autoclave)하여 자유 급이하였다. 순화, 검역 및 실험기간 동안 마우스는 온도 23±3 ℃, 상대습도 55±15%, 조명시간 12시간(오전8시~오후8시), 환기횟수 15회/시간 및 조도 150~300 Lux로 설정된 특정-병원체-부재 조건 하에서 사육되었다. 본 실험은 ㈜메디톡스 동물실험윤리위원회의 검토 및 승인(A-2020-004) 후 수행되었다.
B16-F10 마우스 악성 흑색종 세포주(KCLB No: 80008)를 한국세포주 은행(KCLB: Korean Cell Line Bank)으로부터 수득하였다. B16-F10 세포를 10% FBS(CAT No: 10082-147, Gibco) 및 1% 페니실린-스트렙토마이신(CAT No: 15140-122, Gibco) 함유 DMEM 배지(CAT No: 11965-092, Gibco)에서 37 ℃, 5% CO2 조건으로 인큐베이터에서 배양하였다. 마취된 C57BL/6 마우스 우측 옆구리에 주사기를 이용하여 5×105 세포를 0.1 ㎖ 이식하였다.
비히클로서 비-복합체화된 보툴리눔 독소 제품에서 보툴리눔 독소만 제외되고 나머지 부형제 성분은 동일한 플라시보가 사용되었다. 비-복합체화된 보툴리눔 독소로는 100 units 코어톡스(Lot No: NSA19007A, 메디톡스)가 사용되었고, 복합체화된 보툴리눔 독소로는 100 units 메디톡신(Lot No: TFAA20024, 메디톡스)이 사용되었다. 항-PD-1 항체(Clone: RMP1-14, Cat No: BE0146, Lot No: 780120J2)는 Bio X cell에서 구매하여 사용하였다. 각 시험물질은 아래 표 4에 따라 조제하여 투여되었다. 보툴리눔 독소 100 units 바이알에 멸균 생리식염수를 첨가하여 15 units/㎖ 농도로 조제하여 1 ㎖/㎏, 15 units/㎏ 용량으로 종양 내로 투여하였다. 항-PD-1 항체는 투여일에 멸균 PBS(Cat No: 10010, Gibco)를 이용하여 최종 1 ㎎/㎖ 농도로 조제하여 5 ㎖/㎏, 5 ㎎/㎏ 용량으로 복강 내로 투여하였다.
마우스 악성 흑색종 이식 후 8일째 종양 크기를 측정하여 마우스를 시험군에 따라 각 그룹 당 12마리씩 시험군간 차이가 없게 무작위 배정하였다. 종양 이식 후 8일째 비히클 및 보툴리눔 독소를 시험군 구성에 따라 종양 내로 투여하였다. 항-PD-1 항체의 경우 표 4에 따라 주 2회 총 4회 복강 내로 투여하였다. 종양 이식 후 주 2회 종양 크기를 측정하였다. 종양 크기는 버니어 캘리퍼로 측정하였고 측정치를 사용해 실시예 1에 나타낸 식을 이용하여 종양 부피를 계산하였다.

p<0.01) 도 6에 나타난 바와 같이, 마우스 악성 흑색종 이식 후 19일째, 15 U/㎏의 비-복합체화된 보툴리눔 독소와 복합체화된 보툴리눔 독소를 5 ㎎/㎏ 항-PD1 항체와 병용 투여하였을 경우 유의적인 종양 성장 억제를 관찰하였다. 비-복합체화된 보툴리눔 독소와 복합체화된 보툴리눔 독소를 단독투여한 시험군 사이의 유의적인 차이는 관찰되지 않았다. 또한 비-복합체화된 보툴리눔 독소 + 항-PD1 항체 투여군과 복합체화된 보툴리눔 독소 + 항-PD1 항체와 투여군 간의 유의적인 차이도 관찰되지 않았다. 상기 결과로부터 비-복합체화된 보툴리눔 독소와 복합체화된 보툴리눔 독소 모두 항-PD1 항체와 병용 투여하였을 경우 보툴리눔 독소의 종류에 관계 없이 종양 성장 억제 효과가 있음을 확인하였다.
실시예 4: 마우스 폐종양 이식 모델에 대한 보툴리눔 독소와 항-CTLA4 항체의 항암 효능 시험

LLC1 마우스 폐암 세포주(ATCC No: CRL-1642)를 ATCC로부터 수득하였다. LLC1 세포를 10% FBS(CAT No: 10082-147, Gibco) 및 1% 페니실린-스트렙토마이신(CAT No: 15140-122, Gibco) 함유 DMEM 배지(CAT No: LM001-05, Welgene)에서 37 ℃, 5% CO2 조건으로 인큐베이터에서 배양하였다. 마취된 C57BL/6 마우스 우측 옆구리에 주사기를 이용하여 5×105 세포를 0.1 ㎖ 이식하였다.

p<0.001). 도 7에 나타난 바와 같이, 마우스 폐종양 이식 후 21일째, 15 U/㎏ 보툴리눔 독소 또는 비히클 + 5 ㎎/㎏ 항-CTLA4 항체 투여군에서는 비히클 시험군 대비 유의적인 종양 성장 억제 효과가 관찰되지 않았으나, 15 U/㎏ 보툴리눔 독소 + 5 ㎎/㎏ 항-CTLA4 병용 투여군에서 비히클 시험군 대비 유의적인 종양 성장 억제 효과가 관찰하였다. 또한, 15 U/㎏ 보툴리눔 독소 + 5 ㎎/㎏ 항-CTLA4 투여군은 15 U/㎏ 보툴리눔 독소를 단독 투여한 시험군과 비히클 + 5 ㎎/㎏ 항-CTLA4 항체를 단독투여한 군 대비 유의적으로 종양 부피가 감소되었다. 상기 결과로부터 마우스 LLC1 폐종양 모델에서 보툴리눔 독소와 항-CTLA4 항체의 병용 투여에 의한 종양 성장 억제 효과가 있음을 확인하였다.
실시예 5: 마우스 악성 흑색종 양쪽 이식 모델에 대한 보툴리눔 독소와 항-PD-1 항체의 항암 효능 시험

실험은 동일한 시험군 및 방법으로 2회 반복하여 수행되었다. 우측 옆구리에 악성 흑색종 이식 후 8일째 종양 크기를 측정하여 마우스를 시험군에 따라 각 그룹 당 12마리씩 시험군간 차이가 없게 무작위 배정하였다. 우측 옆구리에 종양 이식 후 8일째 비히클 및 보툴리눔 독소를 시험군 구성에 따라 우측 옆구리의 종양 내로 투여하였다. 항-PD-1 항체의 경우 시험군 구성에 따라 주 2회 총 4회 복강 내로 투여하였다. 종양 이식 후 주 2회 우측 및 좌측에 이식된 종양 크기를 측정하였다. 종양 크기는 버니어 캘리퍼로 측정하였고 측정치를 사용해 실시예 1에 나타낸 식을 이용하여 종양 부피를 계산하였다.

우측 종양 이식 후 21일째, 모든 동물을 안락사하여 실험을 종료하였다. 종료 후 모든 개체의 좌측 옆구리에 이식된 종양 조직 유무를 관찰하였고 좌측 종양을 적출하여 종양 조직 내 면역세포를 유세포분석기(FACSVerse, BD)를 이용하여 분석하였다. 종양 조직 내 면역세포 분석은 종양 조직을 gentle MACS C tube(Cat No: 130-093-237, Miltenyi Biotec)를 이용해 단세포로 분리 후 LIVE/DEAD Fixable Aqua Dead Cell Stain(Cat No: L34957, Invitrogen), Fc block(Cat No: 553142, BD), CD45-PE-Cy7(Cat No: 25-0451-82, eBioscience), TCRbeta-APC-eFluor780(Cat No: 47-5961-82, Invitrogen), CD4-pacific blue(Cat No: 100428, Biolegend), CD8a-PerCP-Cy55(Cat No: 100734, Biolegend), CD11b-A647(Cat No: 101218, Biolegend) 항체로 염색하여 유세포 분석기로 분석하였다. 종양 내 CD4+ 및 CD8a+ T 세포와 CD11b+ 세포의 비율은 생존세포 기준으로 CD45+TCRbeta+CD4+, CD45+TCRbeta+CD8a+, CD45+CD11b+ 세포의 비율을 나타낸다.
종양 이식 후 각 마우스의 우측 및 좌측 종양 크기의 합이 2000 ㎣에 도달한 경우 해당 개체는 사망한 것으로 기록하였다.

p<0.001). 우측 및 좌측에 이식된 악성 흑색종 모델에서 보툴리눔 독소를 15 U/kg 용량으로 단회 우측 종양 내 투여하고 5 mg/kg 항-PD-1 항체를 복강으로 병용 투여할 경우 우측 종양의 크기가 비히클 대비 유의적으로 감소된 것이 관찰되었으며, 보툴리눔 독소 및 항-PD-1 항체를 각각 단독으로 투여한 시험군 대비하여 유의적으로 감소된 것이 관찰되었다(도 8a). 좌측 종양 크기의 경우 보툴리눔 독소와 항-PD-1 항체를 병용 투여한 시험군에서 비히클 투여군 대비 유의적으로 감소되었다(도 8b).

표 7에 나타낸 바와 같이, 좌측 종양의 형성률은 항-PD-1 항체 단독 투여군 대비 보툴리눔 독소와 항-PD-1 항체 병용 투여군에서 유의적으로 감소되었다.

p<0.01) 보툴리눔 독소와 항-PD-1 항체를 병용 투여한 시험군에서 생존율이 유의적으로 개선된 것을 확인하였다.
좌측 비투여 종양에 침윤된 생존 세포 기준 CD11b+ 세포와 CD4+ 및 CD8a+ T 세포의 비율을 양측(two-tailed) t-검정을 사용하여 계산하였다(*
p<0.05). 좌측에 이식된 종양 조직을 적출하여 종양 내 침윤된 CD4+ 및 CD8a+ T 세포와 CD11b+ 세포를 분석한 결과, 보툴리눔 독소와 항-PD1 항체 병용 투여군에서 CD11b+ 세포 비율은 비히클 투여군 대비 유의적인 감소가 관찰되었고(도 10a), CD4+ T 세포의 경우 항-PD-1 항체 단독 투여군 대비 유의적으로 증가되었으며(도 10b), CD8a+ T 세포의 비율은 보툴리눔 독소와 항-PD-1 항체 병용 투여군에서 비히클 및 보툴리눔 독소 단독 투여군 대비 유의적으로 증가되었다(도 10c).
상기 결과로부터 마우스 양쪽 흑색종 이식 모델에서 보툴리눔 독소와 항-PD-1 항체가 병용 투여될 경우 생존율의 유의적인 개선과 보툴리눔 독소가 투여된 종양의 성장뿐만 아니라 투여되지 않은 종양의 형성 및 성장 억제에 효과가 있음을 확인하였다.
실시예 6: 마우스 대장암 이식 모델에서 보툴리눔 독소와 면역항암제(항-PD-1 및 항-CTLA4)의 병용 투여 항암 효능 시험

MC38 세포주를 10% FBS(CAT No: 10082-147, Gibco) 및 1% 페니실린-스트렙토마이신(CAT No: 15140-122, Gibco), 10 mM HEPES(CAT No: 15630-080, Gibco), 0.1 mM MEM NEAA(CAT No: 11140-050, Gibco), 1 mM 소듐 파이루베이트(CAT No: 11360-070, Gibco)를 함유하는 DMEM 배지(CAT No: 11965-092, Gibco)에서 37 ℃, 5% CO2 조건으로 인큐베이터에서 배양하였다. 마취된 C57BL/6 마우스 우측 옆구리에 주사기를 이용하여 2.5×105 세포를 0.1 mL 이식하였다.
비히클로서 보툴리눔 독소 제품에서 보툴리눔 독소만 제외되고 나머지 부형제 성분은 동일한 플라시보가 사용되었고 보툴리눔 독소는 100 units 코어톡스(Lot No: NSA20015, 메디톡스)가 사용되었다. 항-PD-1 항체(Clone: RMP1-14, Cat No: BE0146, Lot No: 798921J1C)와 항-CTLA4 항체(Clone: 9H10, Cat No: BE0131, Lot No: 755620A2)는 Bio X cell 에서 구매하여 사용하였다. 각 시험물질은 아래 표 6에 따라 조제하여 투여되었다. 보툴리눔 독소 100 units 바이알에 멸균 생리식염수를 첨가하여 15 units/mL 농도로 조제하여 1 mL/kg, 15 units/kg 용량으로 종양 내로 투여하였다. 항-PD-1 및 항-CTLA4 항체는 PBS (Cat No: LB004-02, Welgene)를 이용하여 5 mg/kg 용량으로 복강 내로 투여하였다. 프로프라놀롤 (Cat No: P0884, Sigma)은 PBS를 이용해 2 mg/mL으로 제조하여 10 mg/kg 용량으로 복강 내로 투여하였다.
마우스 대장암 세포주 이식 후 7일째 종양 크기를 측정하여 마우스를 시험군에 따라 각 그룹 당 10마리씩 시험군간 차이가 없게 무작위 배정하였다. 종양 이식 후 7일째 비히클 및 보툴리눔 독소를 시험군 구성에 따라 종양 내로 투여하였다. 항-PD-1 및 항-CTLA4 항체의 경우 표 8에 따라 주 2회 총 4회 복강 내로 투여하였으며, 프로프라놀롤은 종양 이식 후 7일에서 16일 사이 총 8회 복강 내로 투여하고, 종양 이식 후 주 2회 종양 크기를 측정하였다. 종양 크기는 버니어 캘리퍼로 측정하였고 측정치를 사용해 실시예 1에 나타낸 식을 이용하여 종양 부피를 계산하였다.

종양 이식 후 각 마우스의 종양의 크기가 2000 ㎣ 에 도달할 때까지 측정을 진행하였으며 안락사 기준 (2000 ㎣)을 초과한 개체는 즉시 안락사하고 사망한 것으로 기록하였다.
Graphpad Prism(version 705, GraphPad Software Inc, CA, USA)은 그래프 제시에 사용되었으며 SPSS software (version 25.0, SPSS Inc, IL, USA) 및 Excel (2013, MS, USA)는 통계 분석에 사용되었다. 종양 크기는 양측 (two-tailed) t-검정을 수행하였고 생존율은 만텔-콕스 로그 등급분석을 유의수준 p = 0.05에서 수행되었다.
도 11 및 도 13에 경시적인 종양 성장을 종양 부피(㎣)로 나타내었다(도 11: **

p<0.001 vs 비히클). 마우스 대장암 이식 후 20일째, 15 U/kg 보툴리눔 독소와 5 mg/kg 항-PD-1 항체 병용 투여군에서 비히클과 보툴리눔 독소, 항-PD-1 단독 투여군 대비 유의적인 종양 성장 억제를 관찰하였다(도 11). 또한 대장암 이식 후 20일째, 5 mg/kg 항-CTLA4 단독 투여군과 보툴리눔 독소와 항-CTLA4 병용 투여군에서 비히클 투여군 대비 유의적인 종양 성장 억제가 관찰되었다(도 13).

p<0.05 vs 항-CTLA4) 보툴리눔 독소와 항-PD1 항체를 병용 투여한 시험군에서 비히클 및 항-PD-1 항체 단독 투여군 대비 유의적으로 마우스 생존율이 증가되었으며, 10 mg/kg 프로프라놀롤과 항-PD-1 항체를 병용 투여한 시험군에서도 비히클 및 항-PD-1 항체 단독 투여군 대비 유의적으로 생존율이 증가되었다. 항-CTLA4 항체 단독 투여군에서도 유의적인 생존율 증가가 관찰되었으며, 보툴리눔 독소와 항-CTLA4 항체 병용 투여군에서 비히클 및 항-CTLA4 항체 단독 투여군 대비 유의적으로 증가된 생존율이 관찰되었다(도 14).
상기 결과로부터, 마우스 대장암 모델에서 보툴리눔 독소와 항-PD-1 또는 항-CTLA4 면역항암제와의 병용 투여로 인한 종양 성장 억제 상승효과(synergistic effect) 및 생존율 개선 효과를 확인하였다. 그리고, 베타 차단제(propranolol)의 면역항암제와 병용 투여에 의한 상승효과를 확인하였고, 보툴리눔 독소와 면역항암제를 병용 투여할 경우 베타 차단제와 면역항암제 병용 투여군 대비 동등 이상의 효과를 확인하였다.
Claims (23)
- 유효성분으로 보툴리눔 독소(botulinum neurotoxin)를 포함하는, 면역요법제(immunotherapeutic agent)와 병용하여 암을 치료하기 위한 조성물.
- 제1항에 있어서, 보툴리눔 독소가 혈청형 A, B, C, D, E, F, G, H, 또는 이들의 조합인 것인 조성물.
- 제1항 또는 제2항에 있어서, 면역요법제는 면역관문 억제제(immune checkpoint inhibitor)인 것인 조성물.
- 제3항에 있어서, 면역관문 억제제는 PD-1 길항제, PD-L1 길항제, CTLA-4 길항제, TIM3 길항제, LAG3 길항제, TIGIT 길항제, VISTA 길항제, BTLA 길항제 또는 이들의 조합인 것인 조성물.
- 제4항에 있어서, 면역관문 억제제는 항-PD-1 항체, 항-PD-L1 항체, 항-CTLA4 항체, 항-TIM3 항체, 항-LAG3 항체, 항-TIGIT 항체, 항-VISTA 항체, 항-BTLA 항체 또는 이의 항원-결합 단편으로 이루어진 군으로부터 선택되는 하나 이상의 것인 조성물.
- 제5항에 있어서, 면역관문 억제제는 항-PD-1 항체, 항-PD-L1 항체, 항-CTLA4 항체, 또는 이의 항원-결합 단편인 것인 조성물.
- 제1항 내지 제6항 중 어느 한 항에 있어서, 암이 엑소좀-매개된(exosome-mediated) 것인 조성물.
- 제1항 내지 제7항 중 어느 한 항에 있어서, 암이 고형암(solid tumor)인 것인 조성물.
- 제8항에 있어서, 고형암이 흑색종, 대장암, 유방암, 폐암, 췌장암, 전립선암, 방광암 및 위암으로 이루어진 군으로부터 선택되는 하나 이상의 것인 조성물.
- 제1항 내지 제9항 중 어느 한 항에 있어서, 암은 전이성 암(metastatic cancer)인 것인 조성물.
- 제1항 내지 제10항 중 어느 한 항에 있어서, 엑소좀 분비가 증가된 대상에서 사용하기 위한 조성물.
- 제1항 내지 제11항 중 어느 한 항에 있어서, 약제학적으로 허용되는 부형제 또는 첨가제를 추가로 포함하는 조성물.
- 제12항에 있어서, 약제학적으로 허용되는 부형제 또는 첨가제는 안정화제, 이온 화합물, 계면활성제, 완충제, 동결건조 보호제, 또는 이들의 조합인 것인 조성물.
- 제13항에 있어서, 약제학적으로 허용되는 부형제 또는 첨가제는 아미노산, 염, 완충액, 비이온성 계면활성제, 당, 당 알코올, 또는 이들의 조합인 것인 조성물.
- 제12항 내지 제14항 중 어느 한 항에 있어서, 알부민 또는 동물 유래 성분이나 폴리사카라이드는 포함하지 않는 조성물.
- 제12항 내지 제15항 중 어느 한 항에 있어서, 동결건조 분말, 액상, 또는 프리필드 시린지 제제 형태의 조성물.
- 제12항 내지 제16항 중 어느 한 항에 있어서, 국소 투여하기 위한 조성물.
- 제17항에 있어서, 종양 내 또는 종양 주변으로 투여하기 위한 조성물.
- 제1항 내지 내지 제18항 중 어느 한 항에 있어서, 보툴리눔 독소를 0.01 내지 100 units/㎏으로 포함하는 조성물.
- 제1항 내지 제19항 중 어느 한 항에 있어서, 보툴리눔 독소는 면역요법제와 동시에 또는 순차적으로 투여되는 조성물.
- 제1항 내지 제20항 중 어느 한 항에 있어서, 보툴리눔 독소는 면역요법제와 단일의 제제 또는 별개의 제제로 투여되는 조성물.
- 제1항 내지 제21항 중 어느 한 항에 있어서, 면역요법제는 비경구 투여되는 것인 조성물.
- 제22항에 있어서, 면역요법제는 정맥 내, 복강 내, 피하, 피내, 근육 내, 척추, 척추강 또는 직장 내 국소 투여 또는 주입되는 것인 조성물.
