Description

输尿管软镜的导入装置及其使用方法和输尿管肾盂镜系统 技术领域
本发明涉及医疗器械技术领域,具体涉及一次性输尿管软镜的导入装置及其使用方法和输尿管肾盂镜系统。
背景技术
近年来,输尿管肾盂镜成为用于诊断和治疗肾脏疾病例如肾结石的常用辅助设备。,它改变了长期以来输尿管部位难以直观检查及输尿管疾病必须进行开放式手术治疗的状况。目前,市面上分别已有单独的软性输尿管镜和硬性输尿管镜。硬性输尿管镜操作方便,可以方便通过输尿管到达肾盂,但由于硬性输尿管镜前端不可弯曲,它进入肾盂后不能调节方向进入各组肾盏进行各种诊断和治疗;软性输尿管镜虽然前端可单向或多向大角度弯曲、转向,却难以经输尿管进入肾盂及肾盏,在x射线的引导下虽然可以进入肾盂及肾盏,但x射线对医生和病人损伤较大。
现有技术中,输尿管肾盂镜从人体尿道进入到体内,需要多次重复操作才能将软镜放置到位:第一步,将带镜头的输尿管硬镜从人体尿道插入找到预定位置,一般为病灶所在位置或者接近病灶所在位置,例如到达输尿管内,第二步,将斑马导丝通过输尿管硬镜的工作通道插入到肾盂位置(可以通过镜头观察到),第三步,将输尿管硬镜退出,但斑马导丝保留在肾盂位置,待输尿管硬镜完全退出,第四步,将带有细小圆头的导入鞘组件顺着斑马导丝推进到肾盂位置(这时没有镜头监视,属于盲插),第五步,将导入鞘组件内部的引导鞘拔出,同时将斑马导丝也拔出,将导入鞘保留在人体内,第六步,将输尿管软镜顺着导入鞘插入到肾盂部位。这种方法大大增加了医生的操作步骤和患者器官被损伤的程度,操作复杂,工作效率低,手术成本高,使用效果欠佳。
发明内容
为了克服现有技术的不足,本发明的目的在于提供一种输尿管软镜的导入装置本导入装置能够减少手术的操作步骤,提高工作效率。
本发明所采用的技术方案如下:
一种输尿管软镜导入装置,所述导入装置包括导入鞘组件,所述导入鞘组件包括具有弹性的导入鞘,所述导入鞘为与所述输尿管软镜相匹配的长条型管体,当所述输尿管软镜插接在所述导入鞘内时,所述输尿管软镜的外端部与所述导入鞘的外端部相平齐或者略凸出于所述导入鞘的外端部。
本导入装置中,输尿管软镜具有输尿管硬镜的功能,导入鞘搭载输尿管软镜经尿道依次进入输尿管至肾盂口,然后导入鞘停止被推进,输尿管软镜被继续推进直至到达肾盂内。
作为优选,所述导入鞘的外径为2-8mm。
作为优选,所述导入鞘组件还包括与所述导入鞘相连接的导鞘主体,所述输尿管软镜能够穿过所述导鞘主体并插入所述导入鞘中,所述输尿管软镜的外端部与所述导入鞘的外端部相平齐或者略凸出于所述导入鞘的外端部,所述导入鞘组件还包括用于固定所述输尿管软镜的固定支架,所述固定支架连接在所述导鞘主体的另一端,所述输尿管软镜的内端部可拆卸地设置于所述固定支架上。
作为优选,所述导鞘主体上靠近所述导入鞘的一端内设置有导鞘通道,所述导鞘主体的另一端设置有用于供所述软镜穿过的凹槽,所述凹槽与所述导鞘通道相连通,所述导鞘通道和所述导入鞘相连通。
作为优选,所述导鞘主体上还设有出水通道,所述出水通道与所述导鞘通道相连通。
作为优选,所述导鞘通道上靠近所述凹槽处设置有用于对所述软镜和所述导鞘通道进行密封的导鞘密封件,所述导鞘密封件与所述导鞘通道过盈配合。
作为优选,所述导鞘主体的端部上设置有固定筋,所述固定支架的对应的端部上设置有与所述固定筋相匹配的固定扣,所述固定支架通过所述固定筋与固定扣的扣合与所述导鞘主体可拆卸连接。
作为优选,所述导鞘主体的侧面上设置有导鞘手柄。
本发明的另一个目的在于,提供一种上述的导入装置的使用方法。
该使用方法包括以下步骤:
1)开通灌注液;
2)将搭载有所述输尿管软镜的导入鞘组件的导入鞘由人体的尿道口插入尿道中;
3)导入鞘搭载输尿管软镜经尿道依次进入输尿管和肾盂口,然后停止推进所述导入鞘,所述输尿管软镜被继续推进,直至达到肾盂内。
本发明的另一个目的在于,提供一种输尿管肾盂镜系统。
本输尿管肾盂镜系统包括输尿管软镜和上述的导入装置,所述输尿管软镜插接在所述导入鞘中,所述输尿管软镜的外端部与所述导入鞘的外端部相平齐或者略凸出于所述导入鞘的外端部。
本输尿管肾盂镜系统中,所述导入鞘用于搭载所述输尿管软镜经尿道进入输尿管至肾盂口,然后所述导入鞘停止被推进,所述输尿管软镜被继续推进直至到达肾盂内。
作为优选,所述输尿管软镜包括软镜主体和软镜手柄,所述软镜主体包括软管和通过弯曲件连接于所述软管的外端部的镜头组件,所述软管的内端部与所述软镜手柄固定连接,所述镜头组件位于所述输尿管软镜的外端部。
作为优选,所述导入鞘的内径尺寸均一,所述弯曲件和镜头组件的外径尺寸均大于所述软管的外径尺寸,所述软管的外径尺寸小于所述导入鞘的内径尺寸和所述导鞘通道的径向尺寸,所述软管和所述导入鞘之间、所述软管和所述导鞘通道的内壁之间均形成通水间隙,肾盂内的液体和手术碎块经由所述通水间隙和所述出水通道排出。优选的,所述弯曲件和镜头组件的外径尺寸为2.5-4.5mm,所述软管的外径尺寸为1.5-4.0mm。
作为优选,当所述导入鞘组件包括所述固定支架时,所述软镜手柄可拆卸地设置于所述固定支架上,所述软镜主体设置于所述导入鞘内,所述镜头组件的外端部和所述导入鞘的外端部相平齐或者略凸出于所述导入鞘的外端部。
作为优选,所述镜头组件包括镜头支架,所述输尿管软镜内设有工作通道,所述工作通道从所述软镜手柄处依次穿过所述软管和弯曲件,并伸入至所述镜头支架内,所述镜头支架上设置有用于与所述工作通道相连通的贯通孔,贯通孔用于输入灌注液;所述镜头组件还包括设置于所述镜头支架的内的摄像通道和照明通道,所述摄像通道内设置有摄像头装置,所述照明通道内设置有照明装置;所述弯曲件包括蛇骨组件和用于固定所述蛇骨组件的端部的固定架;所述蛇骨组件的一个端部通过所述固定架与所述软管连接,所述蛇骨组件的另一个端部与所述镜头支架连接。
相比现有技术,本发明的有益效果在于:
本发明提供一种输尿管肾盂镜系统,通过将输尿管软镜固定在硬性导入鞘组件上,使输尿管软镜具有输尿管硬镜的硬度和功能,使得输尿管软镜可快捷到达肾盂位置;同时,利用输尿管软镜前端的摄像头等镜头组件进行实时观察,使插入过程在内窥下进行,可进一步确 保操作的准确性和可靠性;硬性导入鞘组件搭载所述输尿管软镜进入到肾盂口处后,利用输尿管软镜前端可控制弯曲的特点,输尿管软镜进入结构复杂的肾盂内进行检查治疗,整个过程中,医生的操作步骤只需一步,无需更换手术器械,手术简单快捷,工作效率高,使用设备少,消毒步骤少,损耗少,手术成本大大降低,减少手术时间,从而减少了对患者身体的损伤以及发生交叉感染的概率。

具体实施方式
下面结合附图和具体实施方式对本发明作进一步详细说明。
参考图1和图2,本实施例所提供的一次性输尿管软性肾盂镜系统,其包括输尿管软镜10和作为导入组件的导入鞘组件20,输尿管软镜10搭载固定在导入鞘组件20上进入肾盂组织。
具体的,如图3所示,输尿管软镜10包括软镜主体100和与软镜主体100连接的软镜手柄110,软镜主体100包括依次连接的软管102、弯曲件120和镜头支架130。
如图4及图5所示,输尿管软镜10的结构如下:软镜主体100内设置有工作通道101;软镜主体100的一端(即软管102的一端)穿过软镜手柄110并与软镜手柄110固定连接;工作通道101由软镜手柄110的自由端插入软管102,从软管102内穿过后,再穿过弯曲件120后伸入到镜头支架130的内部。
如图6所示,镜头支架130内部设置有用于与工作通道101相连通的贯通孔131,设置贯通孔131以便于输入灌注液。镜头支架130上对应于贯通孔131的下方的位置上设置有摄像通道132,摄像通道132内设置有摄像头装置,用于实时监控手术过程。镜头支架130上对应于贯通孔131的一侧的位置上设置有照明通道133,照明通道133内设置有照明装置,该照明装置通过导光束用以照明,在与照明通道133相对的另一侧,设有两个用于安装弯曲件120的转向钢丝的盲孔134。镜头支架130、摄像头装置和照明装置构成镜头组件,镜头组件的外径尺寸即为镜头支架130的外径尺寸。
如图11和图12所示,输尿管导入鞘组件20主要包括依次连接的导入鞘200、导鞘主体210和用于固定输尿管软镜10的固定支架220。导入鞘200为长条管体,其结构与拉直的软镜主体100的结构相匹配,导入鞘200的材质为金属或者塑胶,刚性较强,同时具有一定的弹性,在外力作用下可发生弹性变形,导入鞘200的一端固定设置在导鞘主体210内。当输尿管软镜10与硬性输尿管导入鞘组件20结合使用时,软镜主体100经导鞘主体210插入导入鞘200中且软镜主体100的头端与导入鞘200的头端相平齐或者接近平齐,软镜手柄110可拆卸地设置在固定支架220上。其中接近平齐的含义是:软镜主体100的头端可以凸出于导入鞘200的头端0-3mm,它是肉眼可见的平齐,而为了避免尿道和输尿管内壁损伤,导入鞘200的头端不凸出于软镜主体100的头端。
具体的,如图13所示,导鞘主体210上靠近导入鞘200的一端设置有导鞘通道211,导鞘主体210的另一端设置有用于供软镜主体100穿过的凹槽212,该凹槽212与导鞘通道211相连通。导入鞘200的端部密封地插接于导鞘通道211内,导鞘通道211远离导入鞘200的一端的内径朝靠近凹槽212方向逐渐增大。导鞘通道211上靠近凹槽212的位置上设置有导鞘密封件213,该导鞘密封件213用于对软镜主体100和导鞘通道211进行密封,导鞘密封件213与导鞘通道211过盈配合,能够有效防止灌注液外泄。导鞘主体210对应于导鞘通道211上方的位置上设置有导鞘手柄215,方便医生抓握,同时也不用改变原有的使用习惯。导鞘主体210上靠近凹槽212的端部上设置有固定筋216,相应地,固定支架220的端部上设 置有与固定筋216相匹配的固定扣221,固定支架220通过固定筋216与固定扣221的扣合与导鞘主体210可拆卸连接。固定支架220上设置软镜卡扣222,软镜手柄110卡接于软镜卡扣222上,对输尿管软镜10起到很好的固定和定位作用。同时参考图1和图2,将软镜主体100经由凹槽212和导鞘通道211插入导入鞘200中,使软镜主体100的头端与导入鞘200的头端平齐,软镜手柄110卡接在固定支架220的软镜卡扣222上,从而使输尿管软镜10固定在硬性的导入鞘组件20上。
所述软管102优选是采用金属硬管切割成弹性体制成的,即通过在金属硬管的外表面沿其轴向螺旋开槽后形成软性金属管,使得软管102在确保一定的结构强度的同时,又具有很好的柔软度,从而使得软镜主体100能够实现连续弯曲。软镜主体100还包括软管密封件103,该软管密封件103设置于软管102的外表面上,对软管102起到密封和保护作用。软管102与软管密封件103之间可以采用胶粘的方式进行密封固定,也可以采用热缩管作为软管密封件103,通过热缩的方式使软管密封件103固定在软管102的外表面上,对其进行密封固定,有效防止软管102被腐蚀。
所述弯曲件120具有至少一个摆动自由度,使其可通过转向钢丝来调整其弯曲方向,以便于工作通道101的入口准确到达肾盂位置,这就要求弯曲件120能够实现随意弯曲。在本实施例中,弯曲件120包括蛇骨组件和用于固定蛇骨组件的端部的固定架121,具体连接关系如下:
如图8所示,蛇骨组件的一个端部通过固定架121与软管102连接,蛇骨组件的另一个端部与镜头支架130连接,由此实现镜头支架130和工作通道101的转向功能。
如图3、9和图10所示,本实施例中,工作通道101通过进水接头170固定设置在软镜手柄110内。进水接头170包括具有连接腔171的接头本体,接头本体内设置有用于连接工作通道101的连接通道172,该连接通道172与连接腔171连通。接头本体靠近连接通道172的一端上设置有支承板173,该支承板173上设置有凹槽,用于支承工作通道101。接头本体的外周面上设置有至少一个卡槽174,进水接头170通过该卡槽174卡接于软镜手柄110上。
一种优选的实施方式是,如图13所示,所述导鞘主体21还设有出水通道214,所述出水通道214与所述导鞘通道211连通,所述导入鞘200的内径尺寸均一,所述弯曲件120及所述镜头组件的外径尺寸均大于所述软管102的外径尺寸,使得弯曲件120和镜头组件的外径尺寸与导入鞘200的内径尺寸匹配,而所述软管102的外径尺寸小于所述导入鞘200的内 径尺寸和导鞘通道211的径向尺寸,软管102和所述导入鞘200之间、软管102和导鞘通道211的内壁之间均形成通水间隙,肾盂内的液体和手术碎块经由所述通水间隙和所述出水通道214排出。具体的,所述弯曲件120和镜头支架130的外径尺寸为2.5-4.5mm,所述软管102的外径尺寸为1.5-4.0mm,弯曲件120和镜头组件的长度在35-100mm范围内,而软管102的长度在550-650mm之间。此设计的目的在于:一方面,弯曲件120和镜头组件的外径尺寸与导入鞘200的内径尺寸匹配,使得输尿管软镜的头端与导入鞘的头端之间的径向间隙较小,不会形成明显的台阶,避免由人体尿道插入时增加病人的痛苦,而且,在由人体尿道插入的过程中,可以保证输尿管软镜的头端和导入鞘的头端的相对稳定;另一方面,当导入鞘组件停止被推进,输尿管软镜被继续推进,直至镜头组件和弯曲件120伸入肾盂内进行碎石手术时,由于软管102的径向尺寸较小,因此软管102和导入鞘200及导鞘通道211之间的间隙较大,有利于通过出水通道214排出注入肾盂内的液体及击碎的结石,与进水通道形成对流并可调节水流量,既对腔内目标起到了对流冲洗、排放碎石的作用,又可以避免腔内压力过大,导致液体逆流引起尿脓毒血症,甚至危及病人生命。如果弯曲件120外还套设有橡皮层,则橡皮层的外径尺寸大于所述软管102的外径尺寸。固定好输尿管软镜10和导入鞘组件20后,本导入装置的使用方法具体包括如下步骤:
1)开通灌注液,开启摄像头装置和照明装置,使与输尿管软镜10连接的显示屏实时显示镜头支架130前端的实时图像;
2)将导入鞘组件20的导入鞘200由人体的尿道口插入尿道中,软镜主体100随导入鞘200同时置入体内:医生只需手握导鞘手柄215将导入鞘200和软镜主体100插入患者体内,在插入过程中,通过显示屏观察整个插入过程的情况,及时调整插入方向和速度;
3)在通过膀胱、找到输尿管下端入口,并继续在输尿管内通过一定距离到达肾盂口后,导入鞘组件20停止向前推进并预固定,取下固定支架220上的软镜手柄110,将软镜主体100沿导入鞘200继续向前推进,进入肾盂,即可进行检查或手术。
由此可见,本发明通过将输尿管软镜10固定在导入鞘组件20上,使得输尿管软镜10搭载在导入鞘组件20上,与现有的肾盂镜相比,仅需一步操作便可准确到达肾盂位置,操作简单快捷,大大提高了工作效率,不需要斑马导丝等设备,使用设备少,有效降低了手术成本,节省手术时间,减少患者的损伤。同时,由于摄像头装置和照明装置始终设置在镜头支架130前端,使得整个插入过程和手术过程可视。
本发明采用的软硬结合的输尿管肾盂镜系统的原理以及结构,亦可应用于其他内窥镜,如膀胱镜、肠镜或宫腔镜等,适用范围较广。
上述实施方式仅为本发明的优选实施方式,不能以此来限定本发明保护的范围,本领域的技术人员在本发明的基础上所做的任何非实质性的变化及替换均属于本发明所要求保护的范围。

  1. 一种输尿管软镜的导入装置,其特征在于:所述导入装置包括导入鞘组件(20),所述导入鞘组件(20)包括具有弹性的导入鞘(200),所述导入鞘(200)为与所述输尿管软镜相匹配的长条型管体,当所述输尿管软镜插接在所述导入鞘(200)内时,所述输尿管软镜的外端部与所述导入鞘(200)的外端部相平齐或者略凸出于所述导入鞘(200)的外端部。
  2. 根据权利要求1所述的输尿管软镜的导入装置,其特征在于:所述导入鞘组件(20)还包括与所述导入鞘(200)相连接的导鞘主体(210),所述输尿管软镜能够穿过所述导鞘主体(210)并插入所述导入鞘(200)中,所述导入鞘组件(20)还包括用于固定所述输尿管软镜的固定支架(220),所述固定支架(220)连接在所述导鞘主体(210)的远离所述导入鞘(200)的一端,所述输尿管软镜的内端部可拆卸地固定于所述固定支架(220)上。
  3. 根据权利要求2所述的输尿管软镜的导入装置,其特征在于:所述导鞘主体(210)上靠近所述导入鞘(200)的一端设置有导鞘通道(211),所述导鞘主体(210)的另一端设置有用于供所述软镜穿过的凹槽(212),所述凹槽(212)与所述导鞘通道(211)相连通,所述导鞘通道(211)和所述导入鞘(200)相连通。
  4. 根据权利要求3所述的输尿管软镜的导入装置,其特征在于:所述导鞘主体(210)上还设有出水通道(214),所述出水通道(214)与所述导鞘通道(211)相连通。
  5. 一种输尿管肾盂镜系统,其特征在于:所述输尿管肾盂镜系统包括输尿管软镜(10)和权利要求1所述的导入装置,所述输尿管软镜(10)插接在所述导入鞘(200)中,所述输尿管软镜(10)的外端部与所述导入鞘(200)的外端部相平齐或者略凸出于所述导入鞘(200)的外端部。
  6. 根据权利要求5所述的输尿管肾盂镜系统,其特征在于:所述输尿管软镜(10)包括软镜主体(100)和软镜手柄(110),所述软镜主体(100)的内端部与所述软镜手柄(110)固定连接,所述软镜主体(100)包括软管(102)和通过弯曲件(120)连接于所述软管(102)的外端部的镜头组件。
  7. 根据权利要求6所述的输尿管肾盂镜系统,其特征在于:所述弯曲件(120)和镜头组件的外径尺寸均大于所述软管(102)的外径尺寸,所述软管(102)的外径尺寸小于所述导入鞘(200)的内径尺寸和所述导鞘通道(211)的径向尺寸,所述软管(102)和所述导入鞘(200)之间、所述软管(102)和所述导鞘通道(211)的内壁之间均形成通水间隙。
  8. 根据权利要求6所述的输尿管肾盂镜系统,其特征在于:当所述导入鞘组件(20)包 括所述固定支架(220)时,所述软镜手柄(110)可拆卸地设置于所述固定支架(220)上,所述软镜主体(100)设置于所述导入鞘(200)内,所述镜头组件的外端部和所述导入鞘(200)的外端部相平齐或者略凸出于所述导入鞘(200)的外端部。
  9. 根据权利要求8所述的输尿管肾盂镜系统,其特征在于:所述镜头组件包括镜头支架(130),所述输尿管软镜(10)内设有工作通道(101),所述工作通道(101)从所述软镜手柄(110)处依次穿过所述软管(102)和弯曲件,并至所述镜头支架(130)内,所述镜头支架(130)上设置有用于与所述工作通道(101)相连通的贯通孔(131),所述镜头组件还包括设置于所述镜头支架(130)内的摄像通道(132)和照明通道(133),所述摄像通道(132)内设置有摄像头装置,所述照明通道(133)内设置有照明装置;所述弯曲件(120)包括蛇骨组件和用于固定所述蛇骨组件的端部的固定架(121);所述蛇骨组件的一个端部通过所述固定架(121)与所述软管(102)连接,所述蛇骨组件的另一个端部与所述镜头支架(130)连接。
  10. 一种权利要求1所述的导入装置的使用方法,其特征在于,包括以下步骤:
    1)开通灌注液;
    2)将搭载有所述输尿管软镜(10)的导入鞘组件(20)的导入鞘(200)由人体的尿道口插入尿道中;
    3)导入鞘(200)搭载输尿管软镜(10)经尿道依次进入输尿管和肾盂口,然后停止推进所述导入鞘(200),所述输尿管软镜(10)被继续推进,直至伸入肾盂内。
