Abstract
臓器や腸間膜、腹膜等の表面に細胞や組織を移植するための被覆固定剤を提供する。移植物を被覆固定するための製剤であって、アルギン酸塩を含有する前記製剤。アルギン酸塩を含有する製剤と2価以上の金属塩とを組み合わせた、移植物を被覆固定するための製剤キット。ヒト又は非ヒト動物の移植部位に移植物を移植し、アルギン酸塩で移植物を被覆することを含む、移植物の移植方法。
Description
本発明は、被覆固定剤に関する。
これまで臓器不全症に対する根治的な治療法は臓器移植があった。肝不全に対する根治的治療法として肝移植が挙げられる。肝移植は摘出したレシピエントの肝臓と同じ場所に移植し、門脈、肝動脈、肝静脈、胆管を吻合する。肝移植のドナーの絶対的不足を補うべく開発された肝細胞移植や間葉系幹細胞移植は、門脈や末梢静脈から細胞を投与する(非特許文献1及び2)。

本発明者らは、全く新しい移植法として肝臓をはじめとした実質臓器の被膜を剥離し、直接組織を貼付する手法を考案した。従来、腹腔内で被覆可能な製剤としては、癒着性イレウスを予防するための酸化セルロースやヒアルロン酸が挙げられる。また肝切離面からの胆汁漏出や再出血を予防する目的でフィブリン糊が用いられているがこれらの製剤は臓器表面に組織移植するための被覆固定剤としては用いられていない。
これまで臓器表面への移植組織の被覆固定剤は全く開発されていない。肝臓をはじめとする臓器表面に組織を貼付する際に達成すべき項目は、
1. 移植組織への侵襲性が低い
2. 移植組織の長期生着が可能であること
3. 移植組織の増殖促進や成熟化に寄与すること
等があげられるが、従来技術ではそれらの課題を解決できていない。

本発明は、臓器や腸間膜、腹膜等の表面に細胞や組織を移植するための被覆固定剤を提供することを目的とする。
本発明者らは、アルギン酸ナトリウムにCaイオンを添加して、ゲル化したものを被覆材として用いることにより、細胞や組織を臓器表面に移植し、その後生着させることに成功し、本発明を完成させるに至った。また、アルギン酸ナトリウムに増殖因子を添加することにより、移植組織の増殖が促進された。
本発明の要旨は、以下の通りである。
(1)移植物を被覆固定するための製剤であって、アルギン酸塩を含有する前記製剤。
(2)アルギン酸塩が、アルギン酸ナトリウム、アルギン酸カリウム、アルギン酸カルシウム及びアルギン酸アンモニウムからなる群より選択される少なくとも1種である(1)記載の製剤。
(3)製剤が、アルギン酸ナトリウム、アルギン酸カリウム及びアルギン酸アンモニウムからなる群より選択される少なくとも1種を含有する水溶液であり、使用時に、2価以上の金属イオンを添加することによりアルギン酸塩をゲル化させる(1)記載の製剤。
(4)2価以上の金属イオンがカルシウムイオンである(3)記載の製剤。
(5)さらに、増殖因子を含む(1)~(4)のいずれかに記載の製剤。
(6)増殖因子が、EGF、TGF beta、bFGF、IGF1、EGF、PDGF、NGF、HGF、VEGF及びS1Pからなる群より選択される少なくとも1種である(5)記載の製剤。
(7)移植物が、細胞、細胞集合体、組織又はそれらの組み合わせである(1)~(6)のいずれかに記載の製剤。
(8)移植物を臓器及び/又は組織の表面に被覆固定するための(1)~(7)のいずれかに記載の製剤。
(9)ヒト又は非ヒト動物の移植部位に移植物を移植し、移植物にアルギン酸塩を含有する水溶液を滴下し、さらに、2価以上の金属イオンを滴下して、アルギン酸塩をゲル化して、移植物を被覆することを特徴とする、(1)~(8)のいずれかに記載の製剤。
(10)移植部位が、ヒト又は非ヒト動物の組織及び/又は臓器を覆っている被膜を剥離した部位である(9)記載の製剤。

以下、本発明を詳細に説明する。
本発明は、移植物を被覆固定するための製剤であって、アルギン酸塩を含有する前記製剤を提供する。
アルギン酸塩としては、アルギン酸ナトリウム、アルギン酸カリウム、アルギン酸アンモニウムなどのアルギン酸の1価金属塩、アルギン酸カルシウムなどのアルギン酸の2価金属塩を例示することができ、このうち、アルギン酸の1価金属塩が好ましく、アルギン酸ナトリウムがより好ましい。また、アルギン酸塩は、1種のみならず、2種以上の組み合わせであってもよい。
アルギン酸は、褐藻類の藻体に含まれる高分子多糖類であり、D-マンヌロン酸(M)とL-グルロン酸(G)という2種類のウロン酸が直鎖状に重合したポリマーである。D-マンヌロン酸とL-グルロン酸の構成比(M/G比)は由来となる海藻の種類によって異なり、また同じ生物種であっても、生育場所や季節によっても異なる。アルギン酸塩は、アルギン酸のカルボキシル基が金属イオンやアンモニウムイオンと結合した形態の中性塩である。アルギン酸は水に不溶性であるが、アルギン酸ナトリウム、アルギン酸カリウム、アルギン酸アンモニウムなどのアルギン酸の1価金属塩は冷水、温水に良く溶けて、粘稠な水溶液となる。アルギン酸ナトリウム、アルギン酸カリウム、アルギン酸アンモニウムなどのアルギン酸の1価金属塩の水溶液に2価以上の金属イオン(例えば、カルシウムイオン)を加えると、瞬時にイオン架橋が起こり、ゲル化する。2価以上の金属イオンとの架橋反応には、主に、アルギン酸のL-グルロン酸(G)が関与し、Gの比率が高いアルギン酸では、2価以上の金属イオンとの反応が速く、形成されるゲルは硬く、もろい性質を持つ。
製剤が、アルギン酸ナトリウム、アルギン酸カリウム及びアルギン酸アンモニウムからなる群より選択される少なくとも1種を含有する水溶液である場合、使用時に、2価以上の金属イオンを添加することによりアルギン酸塩をゲル化させるとよい。ゲル化することにより被膜が形成され、長期の組織固定力を有するようになる。2価以上の金属イオンとしては、マグネシウムイオン、カルシウムイオン、ストロンチウムイオン、バリウムイオンなどを例示することができるが、カルシウムイオンが好ましい。
2価以上の金属イオンの水溶液としては、塩化カルシウム、塩化マグネシウム、硫酸カルシウム、塩化バリウム、塩化ストロンチウムなどの金属塩の水溶液を例示することができる。
アルギン酸の1価金属塩(例えば、アルギン酸ナトリウム、アルギン酸カリウム、アルギン酸アンモニウムなど)と2価以上の金属イオン(例えば、カルシウムイオン)との比は、0.1~4.0%アルギン酸ナトリウム溶液に、1mM~500mM塩化カルシウム、より望ましくは0.3~3.0%アルギン酸ナトリウム溶液に、5mM~300mM塩化カルシウムが好ましい。
本発明の製剤は2価以上の金属塩(例えば、塩化カルシウム、塩化マグネシウム、硫酸カルシウム、塩化バリウム、塩化ストロンチウムなど)と組み合わせて、キットとしてもよい。
キットには、本発明の製剤や2価以上の金属塩の溶液を調製するための溶媒(例えば、精製水、蒸留水、イオン交換水、MilliQ水、生理食塩水、リン酸緩衝生理食塩水など)、本発明の製剤や2価以上の金属塩の溶液を移植部位に滴下するための器具(シリンジと注射針、シリコン等の型(アルギン酸や塩化カルシウムが周囲に飛散しないように移植操作時に一時的に移植組織を囲み、被覆固定後に外す)など)、取扱説明書などを含めてもよい。
アルギン酸の1価金属塩は、低粘度(20~100mPa・s)、中粘度(100~200mPa・s)、高粘度(400~600mPa・s)のいずれの粘度のものであってもよいが、低粘度のものが好ましい。後述の実施例が示すように、低粘度のアルギン酸ナトリウムに増殖因子を添加することにより増殖因子の徐放作用を認めた。増殖因子を添加した低粘度アルギン酸ナトリウムを用いて生着組織を被覆固定したところ増殖を促進した。
アルギン酸の1価金属塩のM/G比は、1.0~1.6程度であるとよい。
アルギン酸水溶液の粘度は、例えば、回転粘度測定器(コーンプレートタイプ)などを用いて、公知の方法で測定することができる。公知の方法は、例えば、第16改正日本薬局方 一般試験法粘度測定法(円すい-平板形回転粘度計)である。第16改正日本薬局方 一般試験粘度測定法に従い、アルギン酸の1価金属塩の粘度は、アルギン酸の1価金属塩をMilliQ水に溶解して1w/w%濃度の溶液とし、コーンプレート型粘度計を用いて、測定温度は20℃、コーンプレート型粘度計の回転数は1rpm、読み取り時間は2分間とし、開始1分から2分までの平均値を算出するという条件で粘度測定を行ったときの見掛け粘度で示すことができる。
本発明の製剤は、さらに、増殖因子を含んでもよい。増殖因子としては、EGF、TGF beta、bFGF、IGF1、EGF、PDGF、NGF、HGF、VEGF、S1P又はそれらの組み合わせであるとよい。
生体内で用いる医療材料は、従来のアルギン酸ナトリウム(アルギン酸 Na)よりも低エンドトキシンであることが望ましい。市販されている医療用医薬品(経口剤)や創傷被覆材の基材とされたアルギン酸 Na は低エンドトキシン処理がなされていないアルギン酸塩を原料としている(通常エンドトキシン含量が数万~十数万EU(エンドトキシンユニット)/g といわれている)。このエンドトキシンを多量に含有する天然由来の素材を、生体内でより安全に用いることができる原料として改良することが好ましい。低エンドトキシン処理は、洗浄、フィルターによるろ過、限外ろ過、カラムを用いた精製、樹脂や活性炭への吸着、有機溶剤処理、界面活性剤処理などの公知の方法又はそれに準じる方法によって行うことができる。エンドトキシンレベルは、リムルス試薬による方法、トキシノメーターによる方法などの公知の方法により測定することができる。
本発明の製剤に含有されるアルギン酸塩の低エンドトキシン処理方法は特に限定されないが、エンドトキシン含有量が、リムルス試薬によるエンドトキシン測定を行った場合に、500EU/g以下であるとよく、好ましくは、50EU/g以下、より好ましくは、30EU/g以下である。低エンドトキシン処理されたアルギン酸ナトリウムとしては、Sea Matrix(登録商標)(持田製薬株式会社)などが市販されており、入手可能である。
本発明の製剤は、液剤、粉末、顆粒などいかなる剤型であってもよい。液剤の場合には、溶媒は、医薬的に許容される溶媒であればよく、例えば、精製水、蒸留水、イオン交換水、MilliQ水、生理食塩水、リン酸緩衝生理食塩水などを例示することができる。これらの溶媒は低エンドトキシン処理がなされていることが好ましい。製剤が液剤である場合、製剤中のアルギン酸塩の濃度は、0.001~10w/w%であるとよく、好ましくは、0.005~8w/w%、より好ましくは、0.01~5.0w/w%である。本発明の製剤が、粉末、顆粒である場合は、賦形剤、結合剤、崩壊剤などを添加してもよい。製剤が、粉末、顆粒である場合、製剤中のアルギン酸塩の濃度は、0.1~100w/w%であるとよく、好ましくは、1~80w/w%、より好ましくは、10~50w/w%である。製剤が、粉末、顆粒である場合は、使用時に水などの溶媒を加えて、溶液として使用するとよい。
本発明の製剤には、溶解補助剤、可溶化剤、乳化剤、分散剤、抗酸化剤、保存剤、遮光剤などの医薬的に許容される他の添加剤を添加してもよい。
本発明の製剤は、移植物を臓器及び/又は組織の表面に被覆固定するために用いることができる。
臓器とは、腹腔内の器官をいうが、肺、心臓、血管も含む。臓器としては、肝臓、胆管、腸管、膵臓、腎臓、心臓、肺、血管、気管等を例示することができる。
組織とは、1種類又は2種類以上の細胞が一定のパターンで集合した構造体をいい、結合組織(皮膚、腱、軟骨、骨など)、筋組織、神経組織(脳、脊髄といった中枢神経、末梢神経など)を例示することができる。
移植物としては、細胞、細胞集合体、組織、それらの組み合わせなどを例示することができる。
移植する細胞は、生体に移植することにより機能を発揮することが期待できるものであればよく、臓器又は組織を構成する機能細胞、又は機能細胞へと分化する未分化細胞などを挙げることができる。未分化な臓器又は組織細胞としては、例えば、脳、脊髄、副腎髄質、表皮、毛髪・爪・皮膚腺、感覚器、末梢神経、水晶体などの外胚葉性器官を構成する機能細胞又は該器官に分化可能な細胞、腎臓、尿管、心臓、血液、生殖腺、副腎皮質、筋肉、骨格、真皮、結合組織、中皮などの中胚葉性器官を構成する機能細胞又は該器官に分化可能な細胞、肝臓、膵臓、腸管、肺、甲状腺、副甲状腺、尿路などの内胚葉性器官を構成する機能細胞又は該器官に分化可能な細胞などを挙げることができる。当業者間で使用されている用語のうち、hepatoblast、hepatic progenitor cells、pancreatoblast、hepatic precursor cells、pancreatoblast、 pancreatic progenitors、pancreatic progenitor cells、pancreatic precursor cells、endocrine precursors、intestinal progenitor cells、intestinal precursor cells、intermediate mesoderm、metanephric mesenchymal precursor cells、multipotent nephron progenitor、renal progenitor cell、cardiac mesoderm、cardiovascular progenitor cells、cardiac progenitor cells、(JR. Spence, et al. Nature.;470(7332):105-9.(2011)、Self, et al. EMBO J.; 25(21): 5214-5228.(2006)、J. Zhang, et al. Circulation Research.; 104: e30-e41(2009)、G. Lee, et al. Nature Biotechnology 25, 1468 - 1475 (2007))などは、未分化な臓器又は組織細胞に含まれる。未分化な臓器又は組織細胞は、人工多能性幹細胞(iPS細胞)、胚性幹細胞(ES細胞)などの多能性幹細胞から公知の方法に従って作製することができる。細胞は、臓器又は組織に由来するものの他、癌に由来するものであってもよく、癌細胞を非ヒト動物に移植することにより、癌モデル動物を作製することができる。
移植する細胞集合体は、器官芽(オルガノイド)、スフェロイド、セルアグリゲート、セルスフェアなど、いかなる細胞の集合体であってもよく、1種類の細胞を含むものであっても、2種類以上の細胞を含むものであってもよい。「器官芽」とは、成熟することで器官に分化することができる構造体であって、その一例として、WO2013/047639では、臓器又は組織細胞(前駆体細胞)、血管細胞(好ましくは、血管内皮細胞)、及び未分化間葉系細胞若しくはそれから分化した細胞の三種類の細胞から器官芽を作製する方法が開示されており、本発明の製剤を用いることにより、この方法で作製した器官芽を好適に生体に移植し、生着させることができた(後述の実施例)。
移植する組織は、個体より分離した組織(例えば、臓器のような器官を構成する組織の全部又は一部)であってもよいし、臓器や組織を構成する機能細胞、又は機能細胞へと分化する未分化細胞や多能性細胞より誘導された組織であってもよい。本発明の製剤を用いることにより、生体由来の組織を好適に生体に移植し、生着させることができた(後述の実施例)。
本発明の製剤は、フィブリン糊、酸化セルロース、ヒアルロン酸ナトリウムと比較して、移植組織の増殖促進効果が認められた。
本発明の製剤は、ヒト又は非ヒト動物の移植部位に移植物を移植し、移植物にアルギン酸塩を含有する水溶液を滴下し、さらに、2価以上の金属イオンを滴下して、アルギン酸塩をゲル化して、移植物を被覆するという態様で用いることができる。移植部位は、ヒト又は非ヒト動物の組織及び/又は臓器を覆っている被膜を剥離した部位であるとよい。
本発明は、ヒト又は非ヒト動物の移植部位に移植物を移植し、アルギン酸塩で移植物を被覆することを含む、移植物の移植方法も提供する。例えば、ヒト又は非ヒト動物の移植部位に移植物を移植し、移植物にアルギン酸ナトリウム、アルギン酸カリウム及びアルギン酸アンモニウムからなる群より選択される少なくとも1種を含有する水溶液を滴下し、さらに、2価以上の金属イオンを滴下して、アルギン酸塩をゲル化して、移植物を被覆することができる。移植部位は、ヒト又は非ヒト動物の組織及び/又は臓器を覆っている被膜を剥離した部位であるとよい。
本発明は、ヒト又は非ヒト動物の移植部位に移植物を移植し、アルギン酸塩で移植物を被覆することを含む、移植物の移植方法も提供する。例えば、ヒト又は非ヒト動物の移植部位に移植物を移植し、移植物にアルギン酸ナトリウム、アルギン酸カリウム及びアルギン酸アンモニウムからなる群より選択される少なくとも1種を含有する水溶液を滴下し、さらに、2価以上の金属イオンを滴下して、アルギン酸塩をゲル化して、移植物を被覆することができる。移植部位は、ヒト又は非ヒト動物の組織及び/又は臓器を覆っている被膜を剥離した部位であるとよい。
非ヒト動物としては、マウス、ラット、ウサギ、ブタ、イヌ、サル、ウシ、ウマ、ヒツジ、ニワトリなどを例示することができる。
組織及び/又は臓器を覆っている被膜としては、腸間膜、腹膜、脳軟膜、筋膜、心外膜、臓側胸膜、腸管の漿膜、肝被膜、腎被膜などを例示することができる。
被膜の剥離は、注射針、電気メス、双極式電気メス、外科用メス、超音波手術器(CUSA)などを用いて行うことができる。
アルギン酸塩の使用量は、移植部位1 cm2当たり、0.1~100mgであるとよく、1~10mgが好ましく、2~5mgがより好ましい。
アルギン酸塩及び2価以上の金属イオンの滴下は、注射器、スポイト、噴霧器などを用いて行うことができる。
本発明の製剤及び移植方法は、肝硬変治療に向けた肝表面への組織移植、非アルコール性脂肪性肝炎、アルコール性肝炎などの慢性肝炎の治療法、肺、腎臓、膵臓、その他の臓器又は組織を対象とする再生医療などに利用することができる。
以下、実施例により本発明を更に詳細に説明する。
〔実施例1〕
実験方法
・移植用被覆剤
移植用被覆剤として、アルギン酸ナトリウム(持田製薬SeaMatrix を1%希釈して使用、AL20/粘度20~100mPa・s, AL100/粘度100~200mPa・s, AL500/粘度 400~600mPa・s)、マトリゲル (Corning)、フィブリノーゲン(Sigma Aldrich F8630), トロンビン(Sigma Aldrich T7513), フィブリンはフィブリノーゲンとトロンビンを100:1で使用直前に混合させ用いた。酸化セルロース(ジョンソン エンド ジョンソン)、ヒアルロン酸ナトリウム(科研製薬)を用いた。アルギン酸ナトリウムは組織上に添加後、塩化カルシウム(ナカライテスクを蒸留水にて10%に希釈して使用)溶液を滴下してゲル化した。
〔実施例1〕
実験方法
・移植用被覆剤
移植用被覆剤として、アルギン酸ナトリウム(持田製薬SeaMatrix を1%希釈して使用、AL20/粘度20~100mPa・s, AL100/粘度100~200mPa・s, AL500/粘度 400~600mPa・s)、マトリゲル (Corning)、フィブリノーゲン(Sigma Aldrich F8630), トロンビン(Sigma Aldrich T7513), フィブリンはフィブリノーゲンとトロンビンを100:1で使用直前に混合させ用いた。酸化セルロース(ジョンソン エンド ジョンソン)、ヒアルロン酸ナトリウム(科研製薬)を用いた。アルギン酸ナトリウムは組織上に添加後、塩化カルシウム(ナカライテスクを蒸留水にて10%に希釈して使用)溶液を滴下してゲル化した。
・増殖因子
bFGF(Sigma B5887 0.1pg/ml), EGF (Sigma E9644 1ng/ml), NGF (Sigma N1408 0.2ng/ml), IGF-1(Sigma I3769 15ng/ml), TGFbeta (Sigma H8541 2.5μg/ml), PDGF (Sigma P3201 12pg/ml)。

・肝硬変モデルラットの作製
DPP4- F344ラット (日本チャールズリバー、神奈川、日本)3週齢を2週間順化させ、週3回連日でN-Nitrosodimethylamin (WAKO) (DMN)を10 mg/kg (Body Weight)の濃度で3週間腹腔内に注射した。IL2rg KO F344ラットは京都大学より提供されたものを使用した。

・胎仔肝組織の調製方法
DPPIV+ F344ラット (日本SLC、静岡、日本)の胎齢14日の胎仔より肝組織を採取して使用した。

・移植方法
ラットの門脈血流を血管クランプ鉗子で遮断したのち、中葉表面を18G注射針 (TERUMO)で鋭利に剥離した。剥離後、綿棒で圧迫止血し、胎齢14日目の胎仔肝組織を移植し、上から被覆剤を用いて被覆し、血管クランプ鉗子を外したのちに閉腹した。移植14日後に肝臓を摘出して生着組織を比較検討した。

凍結組織切片をAcetone (WAKO)とChloroform (WAKO)を等量混合した溶液を用いて組織固定した。その後1xPBS 1mLに対して1mgの割合でFast Blue BB Salt hemi (zinc chloride)salt (SIGMA-ALDRICH)を溶解した液と、Dimethyl Sulfoxide (WAKO)1mLに対して8mgの割合でGly-Pro 4-methoxy-β-naphthylamide hydrochloride (SIGMA-ALDRICH)を溶解した液を20:1で混合し作成した染色液で常温で20分、酵素組織化学染色を行なった。染色後、硫酸銅 (II)五水和物 (WAKO)をMilli Q水に溶解して作製した2%硫酸銅水溶液に5分浸け、色素を定着させた。その後、10%ホルマリンに10分浸し組織固定を行い、Milli Q水に置換した後、Carrazi’s Haematoxylin (武藤化学)で10分間、染色を行った。水道水で洗浄後、流水で30分間色だしを行った。最後にアパチ封入剤 (WAKO)を滴下し、スライドグラス (MATSUNAMI)を被せて封入した。

・免疫染色法
凍結組織切片をAcetone (WAKO)とMethanol (WAKO)を等量混合した溶液で組織固定を行なった。風乾後、染色対象を撥水ペン (DAKO)で囲み、撥水処理後、0.05% Tween20-PBS (PBST)で10分、3回透過処理を行った。次に、Blocking One (ナカライテスク)を用い、室温で1時間ブロッキングを行なった。その後、Blocking Oneで適切な濃度に希釈した一次抗体溶液をのせて、4℃でOver Night反応させた。反応後、PBSTで10分、3回洗浄した後、Blocking Oneで適切に希釈した二次抗体溶液をのせて、室温で1時間反応させた。その後、PBSで10分間洗浄を行い、DAPI (4’,6-diamidino-2-phenylindole dihydrochloride、invitrogen)をアパチ封入剤 (WAKO)に1:1000で混合したものを滴下し、スライドグラス (MATSUNAMI)を被せて封入した。
凍結組織切片をAcetone (WAKO)とMethanol (WAKO)を等量混合した溶液で組織固定を行なった。風乾後、染色対象を撥水ペン (DAKO)で囲み、撥水処理後、0.05% Tween20-PBS (PBST)で10分、3回透過処理を行った。次に、Blocking One (ナカライテスク)を用い、室温で1時間ブロッキングを行なった。その後、Blocking Oneで適切な濃度に希釈した一次抗体溶液をのせて、4℃でOver Night反応させた。反応後、PBSTで10分、3回洗浄した後、Blocking Oneで適切に希釈した二次抗体溶液をのせて、室温で1時間反応させた。その後、PBSで10分間洗浄を行い、DAPI (4’,6-diamidino-2-phenylindole dihydrochloride、invitrogen)をアパチ封入剤 (WAKO)に1:1000で混合したものを滴下し、スライドグラス (MATSUNAMI)を被せて封入した。

・プロトロンビン時間測定
実験に用いた動物から採取した血液をコアグチェック(登録商標)XS (ロシュ)を用いて測定を行った。

・血液血算検査
実験に用いた動物から採取した血液をEDTAで抗凝固処理したのちに、全自動血球計数器 MEK-6550 セルタックα (日本光電)を用いて測定を行った。
血液生化学検査
実験に用いた動物から採取した血液を4000rpm、20分間遠心し、血清を回収した。回収した血清を富士ドライケムスライド (富士フィルム)を用いてAST (アスパラギン酸アミノトランスフェラーゼ)、ALT (アラニンアミノトランスフェラーゼ)、NH3 (アンモニア)、ALB (アルブミン)、T-Bil (総ビリルビン)について解析を行った。測定には、DRI-CHEM 7000V (富士フィルム)を使用した。

血液生化学検査

・ヒアルロン酸測定方法
ラット血清を採取し、ヒアルロン酸ELISA kit(duoset, インビトロジェン)を用いて測定した。

・ヒトiPSC肝芽の作製方法
(1)肝内胚葉細胞(HE)の調製
HEについてはヒトiPS細胞由来肝内胚葉細胞(Cell Reports 21, 2661-2670, 2017)、PXB細胞(フェニックスバイオ社)などを用いた。培地はGM BulletKit(Lonza社製)とHCM BulletKit(Lonza社製)よりhEGF(組換えヒト上皮細胞成長因子)を除いたものとを1:1で混ぜたものに、Dexamethasone、Oncostatin Mを添加したものを用いた。
(2)間葉系細胞(MC)の調製
MCについては、ヒト骨髄より分離した細胞 (Lonza, cat. No. PT-2501)他、ヒト臍帯間質(ワルトン鞘)より分離した細胞、ヒトiPS細胞由来間葉系細胞(Cell Reports 21, 2661-2670, 2017)などのいずれかを用いた。本実験で主として用いた、ヒト骨髄より分離した間葉系幹細胞(Mesenchymal Stem Cell: hMSC) は、hMSC培養に調製された専用の培地(MSCGM2TM (登録商標))(Promocell C-28009)を用いて培養した。
(3)血管細胞(EC)の調製
ECについては、ヒトiPS細胞由来血管内皮細胞(Cell Reports 21, 2661-2670, 2017)、正常臍帯静脈内皮細胞(Normal Umbilical Vein Endothelial Cells: HUVEC)などのいずれかを用いた。HUVECは、説明と同意を取得した妊産婦の分娩時に提供を頂いた臍帯より分離した細胞、ないし購入した細胞(HUVECs (Lonza, cat. No. 191027)他)をEGM(登録商標)Bulletkit(登録商標)(Lonza CC-4133)を用いて、5回以内の継代回数で培養したものを用いた。
(4)3種細胞集合体
マトリゲルコーティング(Corning(登録商標)Matrigel(登録商標)の原液、ないしマトリゲルと培地を1:1の割合で混合した溶液を1ウェル毎に300μlずつ入れ、37℃、5% CO2のインキュベーター内に10分以上静置し固めた)を行った、24ウェルプレートの1ウェルに細胞数として5x105 cellsのiPS細胞由来肝内胚葉細胞、ないしヒト成体肝細胞と、3.5x105 cellsのヒトiPS細胞由来血管細胞またはヒト臍帯静脈由来血管内皮細胞、1x104 cellsのヒトiPS細胞由来間葉系細胞またはヒト間葉系細胞と混合後、37℃のインキュベーターで2日間培養した。マイクロパターンプレートを用いて3種細胞集合体を作製することも可能である。
(1)肝内胚葉細胞(HE)の調製
HEについてはヒトiPS細胞由来肝内胚葉細胞(Cell Reports 21, 2661-2670, 2017)、PXB細胞(フェニックスバイオ社)などを用いた。培地はGM BulletKit(Lonza社製)とHCM BulletKit(Lonza社製)よりhEGF(組換えヒト上皮細胞成長因子)を除いたものとを1:1で混ぜたものに、Dexamethasone、Oncostatin Mを添加したものを用いた。

マトリゲルコーティング(Corning(登録商標)Matrigel(登録商標)の原液、ないしマトリゲルと培地を1:1の割合で混合した溶液を1ウェル毎に300μlずつ入れ、37℃、5% CO2のインキュベーター内に10分以上静置し固めた)を行った、24ウェルプレートの1ウェルに細胞数として5x105 cellsのiPS細胞由来肝内胚葉細胞、ないしヒト成体肝細胞と、3.5x105 cellsのヒトiPS細胞由来血管細胞またはヒト臍帯静脈由来血管内皮細胞、1x104 cellsのヒトiPS細胞由来間葉系細胞またはヒト間葉系細胞と混合後、37℃のインキュベーターで2日間培養した。マイクロパターンプレートを用いて3種細胞集合体を作製することも可能である。
結果
・ラット肝臓表面への胎仔肝組織移植における最適な被覆剤の検討
N-Nitrosodimethylamin (WAKO) (DMN)投与2週間後の肝硬変状態のラット肝表面に胎仔肝組織を移植し、アルギン酸ナトリウム、フィブリン、酸化セルロース、ヒアルロン酸ナトリウムでそれぞれ被覆固定を行った(図1a)。DPPIV染色により移植組織の生着を確認したところ、全ての被覆剤において7割以上の生着率を確認した (図1b)。それぞれの被覆剤による生着組織の最大割面積を比較したところ、4種の被覆剤の中ではアルギン酸ナトリウムが有意に高いことを確認した (図1c)。
・ラット肝臓表面への胎仔肝組織移植における最適な被覆剤の検討
N-Nitrosodimethylamin (WAKO) (DMN)投与2週間後の肝硬変状態のラット肝表面に胎仔肝組織を移植し、アルギン酸ナトリウム、フィブリン、酸化セルロース、ヒアルロン酸ナトリウムでそれぞれ被覆固定を行った(図1a)。DPPIV染色により移植組織の生着を確認したところ、全ての被覆剤において7割以上の生着率を確認した (図1b)。それぞれの被覆剤による生着組織の最大割面積を比較したところ、4種の被覆剤の中ではアルギン酸ナトリウムが有意に高いことを確認した (図1c)。
また、生着組織における脈管構造の形成状態と、肝組織の成熟の程度を調べるために、免疫組織化学染色を行い、生着組織のマーカーであるCD26、血管内皮細胞マーカーであるCD31と胆管内皮細胞マーカーであるCK19、肝細胞マーカーであるHNF4αと類洞内皮細胞マーカーであるSE-1の発現を検討した (図1d)。その結果、生着組織内のCD31陽性部位の割合はアルギン酸ナトリウムが他の3種の被覆剤より有意に高いことを確認した (図1e)。また、生着組織におけるHNF4α陽性割合は、アルギン酸ナトリウムとヒアルロン酸ナトリウムが有意に他の臨床応用可能な被覆剤よりも高かった (図1f)。
・ラット肝臓表面への胎仔組織移植における治療効果の検討
ラット非代償性肝硬変モデルにおける肝臓表面へのラット胎仔肝移植による治療効果を検討した。その結果、アルギン酸ナトリウム被覆による肝表面へのラット胎仔肝組織移植において、移植組織の生着及び脈管構造の構築と肝組織の成熟を確認した (図2a)。また、sham手術群(肝被膜の剥離操作のみ施行)と比較検討し、肝表面へのラット胎仔肝組織移植において生存率が改善することが確認された (図2b)。血液生化学検査を行ったところ、移植一週間後に肝機能マーカーであるAST, ALTの改善が見られた。またChild-Pugh分類関連マーカーであるT-Bil、NH3の改善、プロトロンビン時間の短縮、血中ALB、移植1週間後の血中ヒアルロン酸が有意に改善した(図2c)。

アルギン酸ナトリウムの徐放作用を検討する為、高粘度、中粘度、低粘度のアルギン酸ナトリウムにヒトEGFを添加し、塩化カルシウムでゲル化した。周囲のPBSへのヒトEGFの徐放作用を検討すると低粘度アルギン酸ナトリウムが最もEGFを周囲に放出した(図3a)。肝硬変の肝表面に胎仔肝組織を移植し低粘度アルギン酸ナトリウムで被覆した。このときマトリゲルに含まれる増殖因子(EGF, TGF beta, bFGF, IGF1, EGF, PDGF, NGF)を添加すると生着組織が増大した(図3b)。低粘度アルギン酸ナトリウムに増殖因子を加えた被覆固定剤により肝硬変の肝表面にヒトiPS細胞由来3種細胞集合体の生着も認められた(図3c)。
考察
本研究で用いた組織移植法は、従来の細胞治療として行われている経門脈的移植に用いる細胞よりも、大きな組織を肝臓へ同所的に生着させることが可能な手法である。そのため、経門脈的移植における課題である移植細胞数の解決に寄与することが考えられる。マトリゲルに含まれている増殖因子 (bFGF, EGF, IGF-1, PDGF, NGF, TGF beta)を添加したアルギン酸ナトリウムを用いる事により移植組織の増殖性を更に高めることが可能である。
本研究で用いた組織移植法は、従来の細胞治療として行われている経門脈的移植に用いる細胞よりも、大きな組織を肝臓へ同所的に生着させることが可能な手法である。そのため、経門脈的移植における課題である移植細胞数の解決に寄与することが考えられる。マトリゲルに含まれている増殖因子 (bFGF, EGF, IGF-1, PDGF, NGF, TGF beta)を添加したアルギン酸ナトリウムを用いる事により移植組織の増殖性を更に高めることが可能である。
本発明による新たな被覆固定剤は、
1. 移植組織への侵襲性が低い
2. 移植組織の長期生着が可能である
3. 移植組織の増殖促進や成熟化に寄与する
という点で従来技術より大幅な改善が見られている。さらに低粘度化したアルギン酸ナトリウムに任意の増殖因子を添加することにより、さらなる移植組織の増殖促進に寄与することが出来る。

3. 移植組織の増殖促進や成熟化に寄与する
という点で従来技術より大幅な改善が見られている。さらに低粘度化したアルギン酸ナトリウムに任意の増殖因子を添加することにより、さらなる移植組織の増殖促進に寄与することが出来る。
本明細書で引用した全ての刊行物、特許および特許出願をそのまま参考として本明細書にとり入れるものとする。
本発明は、細胞、細胞集合体や組織の生体への移植に利用できる。
Claims (14)
- 移植物を被覆固定するための製剤であって、アルギン酸塩を含有する前記製剤。
- アルギン酸塩が、アルギン酸ナトリウム、アルギン酸カリウム、アルギン酸カルシウム及びアルギン酸アンモニウムからなる群より選択される少なくとも1種である請求項1記載の製剤。
- 製剤が、アルギン酸ナトリウム、アルギン酸カリウム及びアルギン酸アンモニウムからなる群より選択される少なくとも1種を含有する水溶液であり、使用時に、2価以上の金属イオンを添加することによりアルギン酸塩をゲル化させる請求項1記載の製剤。
- 2価以上の金属イオンがカルシウムイオンである請求項3記載の製剤。
- さらに、増殖因子を含む請求項1~4のいずれかに記載の製剤。
- 増殖因子が、EGF、TGF beta、bFGF、IGF1、EGF、PDGF、NGF、HGF、VEGF及びS1Pからなる群より選択される少なくとも1種である請求項5記載の製剤。
- 移植物が、細胞、細胞集合体、組織又はそれらの組み合わせである請求項1~6のいずれかに記載の製剤。

- 移植部位が、ヒト又は非ヒト動物の組織及び/又は臓器を覆っている被膜を剥離した部位である請求項12又は13記載の移植物の移植方法。
