Abstract
皮下組織(92)に挿入可能な針先(12b)を備える導入針(12)と、細胞が付着することで組織再生を促す能力を有する長尺な埋込体(16)と、導入針(12)の外方を覆う管状の埋込体カバー(14)と、を備え、埋込体カバー(14)と導入針(12)との間に軸方向に延びる空間(13)が設けられており、空間(13)に埋込体(16)が配置される埋込体留置器具(10)が提供される。
Description
本発明は、皮膚から穿刺して体内に埋込体を留置する埋込体留置器具に関する。
機能が低下した組織を治療するために、当該部位に組織再生を促す埋込体を留置する治療法が種々提案されている。例えば、内部に無数の孔が形成された多孔質のコラーゲン繊維を損傷部位に埋入する方法が提案されている(特表2012-500203号公報)。このようなコラーゲン繊維を埋め込むと周囲に細胞が集まり、損傷組織の再生が促される効果がある。
一般的に、埋込体は、皮膚組織を切開して、損傷組織を露出させ、埋入するコラーゲン繊維を結紮クリップや組織用接着剤で損傷組織に固定する方法で埋め込まれている。しかし、このような手法では、侵襲が大きく患者への負担が大きいため、より負担の少ない手法が求められている。
そこで、本発明の一観点は、組織再生を促す能力を有する埋込体を損傷組織に留置するのに好適な埋込体留置器具を提供することを目的とする。
本発明の一観点は、皮下組織に挿入可能な針先を備える導入針と、細胞が付着することで組織再生を促す能力を有する長尺な埋込体と、前記導入針の外方を覆う管状の埋込体カバーと、を備え、前記埋込体カバーと前記導入針との間に軸方向に延びる空間が設けられており、前記空間に前記埋込体が配置される、埋込体留置器具にある。
他の態様としては、皮下組織に挿入可能な針先を備える導入針と、細胞が付着することで組織再生を促す能力を有し、皮下組織への係止を可能とする係止部を備える先端部を有する長尺な埋込体と、前記導入針の外方を覆う管状の埋込体カバーとを含み、前記導入針は前記埋込体カバーに対して軸方向に相対移動可能である埋込体留置器具である。
上記観点に係る埋込体留置器具によれば、埋込体を埋込体カバーで保護しつつ留置部位に移送することができる。
以下、本発明の好適な実施形態を挙げ、添付の図面を参照して詳細に説明する。なお、本明細書において、導入針ハブ17から導入針12の針先12bに向かう方向を先端側又は先端方向と呼び、その逆方向を基端側又は基端方向と呼ぶものとする。
(第1実施形態)
本実施形態に係る埋込体留置器具10は、損傷した組織のある留置部位に埋込体16を埋め込む際に使用される。図1Aに示すように、埋込体留置器具10は、鋭利な針先12bを有する導入針12と、導入針12の外周を覆うように配置された埋込体カバー14と、導入針12と埋込体カバー14との間の空間13に収容された埋込体16とを備える。
本実施形態に係る埋込体留置器具10は、損傷した組織のある留置部位に埋込体16を埋め込む際に使用される。図1Aに示すように、埋込体留置器具10は、鋭利な針先12bを有する導入針12と、導入針12の外周を覆うように配置された埋込体カバー14と、導入針12と埋込体カバー14との間の空間13に収容された埋込体16とを備える。
導入針12は、中実針として構成されている。導入針12の先端部には軸方向に対して傾斜した傾斜面よりなる穿刺部12cが形成されており、その穿刺部12cの先端に鋭利な針先12bが形成されている。導入針12の外周部には溝状の凹部12aが形成されている。凹部12aは、埋込体カバー14との間に埋込体16を収容する空間13を構成する。凹部12aは、図1Bに示すように、埋込体16を収容するべく、埋込体16の直径よりも大きな幅及び深さに形成されている。図1Aに示すように、凹部12aは、導入針12の先端の穿刺部12cから基端部12dまで軸方向に延びて形成されている。
導入針12の直径は、例えば1.6mm程度とすることができ、その全長は例えば100mm程度とすることができる。また、凹部12aの幅(周方向の長さ)及び深さ(径方向の長さ)は例えば0.5~0.6mm程度とすることができる。
このような導入針12の構成材料としては、例えば、ステンレス鋼、アルミニウム合金、チタン又はチタン合金等の金属材料が挙げられる。また、導入針12の先端部には、X線透視装置による視認性を高めるために、重元素(原子番号の大きな元素)を含んだマーカー部材が設けられていてもよい。また、導入針12の外周部が抗菌剤で被覆されていてもよい。さらに、導入針12の先端にはエコーでの視認性向上を目的とした凹凸状の加工が設けられていてもよい。
導入針12の基端部12dには導入針12を操作するための導入針ハブ17が固定されている。導入針ハブ17は、基端側が閉塞された筒状の部材で構成され、先端側には、後述する埋込体カバーハブ18の軸孔18aに挿入される挿入部17bが形成されている。導入針ハブ17の中心部には、導入針12の外径と略同じ内径の針固定孔17aが形成されている。針固定孔17aは、挿入部17bを貫通して基端側に向けて延びて形成されている。
導入針ハブ17の針固定孔17aには、導入針12が挿入されて固定されている。導入針12の導入針ハブ17の固定は、例えば、嵌合、カシメ、融着、接着剤による接着等の方法で行うことができる。
導入針ハブ17の構成材料としては、樹脂材料を用いることができる。特に限定されないが、導入針ハブ17は、例えば、ポリエチレン、ポリプロピレン、エチレン-酢酸ビニル共重合体等のポリオレフィン、ポリウレタン、ポリアミド、ポリエステル、ポリカーボネート、ポリブタジエン、ポリ塩化ビニル、ポリアセタール等の各種樹脂材料を用いることができる。
埋込体カバー14は、内部に軸方向に延びる貫通孔14aが形成された略円筒状の部材であり、軸方向に細長く延びて形成されている。貫通孔14aは、導入針12をスムーズに移動可能とするべく、導入針12の外径よりも大きな内径に形成されている。また、貫通孔14aの先端部14bには、導入針12のガタツキを抑制するべく、導入針12の外径と略同じ内径にまで縮径された縮径部14dが形成されている。埋込体カバー14は円筒状部材とも呼ぶことができる。
埋込体カバー14の先端部14bは、皮下組織92への穿刺を容易にするべく、先端側に向かうにしたがって外径が細くなるテーパー状の傾斜面で構成されている。なお、先端部14bはテーパー状に限定されるものではなく、軸方向に垂直な端面で構成されてもよい。また、先端部14bは、所定の曲率半径の曲面で構成されてもよい。
埋込体カバー14は、導入針12の周囲を覆うことで、導入針12とともに収容される埋込体16を保護するように構成されている。埋込体カバー14の全長は、導入針12の全長よりも短く形成されている。埋込体カバー14は、外径が例えば2.1mm程度に形成され、貫通孔14aの内径は例えば1.7mm程度に形成されてもよい。埋込体カバー14の全長(先端部14bから基端部14cまでの長さ)は、例えば100mm程度とすることができる。埋込体カバー14は、初期状態においてその先端部14bから基端部14cまで導入針12及び埋込体16を覆っている。
埋込体カバー14は、適度な可撓性及び弾力性を有して形成されていることが好ましく、埋込体カバー14の構成材料としては、樹脂材料や金属材料を用いることができる。具体例としては、例えば、ステンレス鋼、アルミニウム合金、チタン又はチタン合金等の金属材料、PTFE、ETFE、PFA等のフッ素系樹脂、ポリエチレン、ポリプロピレン等のオレフィン系樹脂、ポリウレタン、ポリエステル、ポリアミド、ポリエーテルナイロン樹脂等が挙げられる。なお、埋込体カバー14の全部又は一部が、重元素を含んで造影機能を有してもよい。また、埋込体カバー14は、抗菌剤で被覆されていてもよい。また、近赤外蛍光色素が混錬されていてもよい。さらに、埋込体カバー14の先端内面又は外表面にはエコーでの視認性向上を目的とした凹凸状の加工が設けられていてもよい。
埋込体カバー14の基端部14cには、埋込体カバー14を操作するための埋込体カバーハブ18が設けられている。埋込体カバー14の基端部14cは、例えば、カシメ、融着、接着剤による接着等の方法により、埋込体カバーハブ18に固定されている。埋込体カバーハブ18は、中心部に軸孔18aが形成された略円筒状の部材として構成されている。軸孔18aは、埋込体カバー14の貫通孔14aと連通しており、その内部に導入針12が収容されている。軸孔18aの内径は、導入針ハブ17の挿入部17bの外径と略同じ大きさに形成されている。埋込体カバーハブ18は、導入針ハブ17と同様の材料で構成することができる。
軸孔18aの基端側から導入針ハブ17の挿入部17bが挿入されることで、埋込体カバーハブ18及び導入針ハブ17が連結した組立状態となる。組立状態では、図示のように導入針12の針先12bが埋込体カバー14の先端から所定の長さだけ先端側に突出するように構成されている。また、導入針ハブ17は、挿入部17bを軸孔18aから引き抜くことで、埋込体カバーハブ18から取り外すことができる。埋込体カバーハブ18を固定することにより、軸孔18aを通じて導入針12を埋込体カバー14から完全に引き抜くことができる。
埋込体16は、導入針12の凹部12a(空間13)内に収容されている。埋込体16は、軸方向に長く延びた繊維状の部材であり、生体内において損傷した組織に埋め込んだ際に、組織を再生させる細胞が集まりやすい部材よりなる。このような部材としては、断面に無数の微細孔が形成された多孔質状のコラーゲン繊維が挙げられる。埋込体16は、例えば径の平均値が0.4mm程度で、長さが100mm以上の糸状のものを用いることができる。埋込体16の外径は、導入針12の凹部12aの幅及び深さよりも小さくなっており、埋込体16に張力が掛からない状態で凹部12a内に収容されている。
埋込体16は、先端側が埋込体カバー14の先端から突出するとともに、屈曲部16aで基端側に折り返され、その先端側の折返部16bが埋込体カバー14の先端部14bの外周面(傾斜面)に沿って配置されている。すなわち、埋込体16の先端側の一部は、埋込体カバー14の外側に露出している。この折返部16bは、皮下組織92(図2参照)に係合することで、埋込体16を皮下組織92の所望の位置に係止するように構成されている。
なお、埋込体16は、X線透視装置による視認性を高めるために、重元素(原子番号の大きな元素)を含んだマーカー部材を備えていてもよいし、繊維の内部に重元素を含んだ造影剤を混入させてもよい。また、埋込体16は、繊維の内部又は外表面に抗菌剤を含有してもよい。さらに、埋込体16をがん治療の一環として行われるリンパ節郭清の治療に用いる場合には、埋込体16の内部に抗がん剤を含有させておいてもよい。
本実施形態に係る埋込体留置器具10は、以上のように構成され、以下その使用方法と作用を説明する。
特に用途を限定するものではないが、本実施形態の埋込体留置器具10は、図2に示すように、損傷したリンパ管があるリンパ節付近に埋込体16を留置する手技に用いることができる。リンパ管の損傷は、腫瘍の治療の際のリンパ節郭清や放射線治療等により生じる。リンパ管が損傷を受けて機能不全となると、リンパ液が正常に流れず浮腫を生ずることがある。このような機能不全を改善するために、コラーゲン繊維よりなる埋込体16を留置することで、細胞増殖性及び組織化を誘導して、リンパ管の再生を促すことができる。図示のように、リンパ管の再生をより確実に行うために、複数本の埋込体16が平行に埋め込まれてもよい。以下、埋込体留置器具10の具体的な使用方法について説明する。
まず、図3に示すように、埋込体留置器具10を、留置部位の近くの皮膚90に穿刺する。穿刺は、埋込体カバー14の先端から導入針12を突出させた組立状態で行う。導入針12の鋭利な針先12bにより、皮膚90に穿刺を容易に行うことができる。その後、穿刺した部位から留置部位に向けて導入針12及び埋込体カバー14を皮下組織92内で前進させる。導入針12及び埋込体カバー14を前進させる操作は、導入針ハブ17を手で持って操作することで行う。導入針12及び埋込体カバー14は一体となって前進するため、実質的にそれらの位置は変化しない。
導入針12及び埋込体カバー14の皮下組織92内での前進は、埋込体16が凹部12a内に収容され、埋込体カバー14で覆われた状態で行われる。そのため、埋込体カバー14で覆われた埋込体16には引っ張り力等の負荷が作用することがなく、埋込体16の微細組織が壊れて細胞の定着性が低下するのを防ぐことができる。
その後、図4に示すように、埋込体カバーハブ18から導入針ハブ17の挿入部17bを外し、導入針ハブ17を引くことで、埋込体カバー14から導入針12を引き抜く。埋込体16は、折返部16bが埋込体カバー14の先端部14bに引っ掛かっているため、埋込体カバー14の貫通孔14aの内部に残る。
その後、埋込体カバーハブ18を引くことにより、皮膚90から埋込体カバー14を引き抜く。その際に、図5に示すように、埋込体16の先端の折返部16bは、皮下組織92に押圧されることで、留置部位に係合した状態に保たれる。これにより、埋込体16を残して、埋込体カバー14を引き抜くことができ、埋込体16に引っ張り力を作用させることなく、最小限の負荷で埋込体16を留置させることができる。その後、皮膚90から突出した部分の埋込体16をはさみ等で切断する。以上の操作により、埋込体16の留置が完了する。
本実施形態の埋込体留置器具10及び埋込体16は、以下の効果を奏する。
上記の埋込体留置器具10は、皮下組織92に挿入可能な針先12bを備える導入針12と、細胞が付着することで組織再生を促す能力を有する長尺な埋込体16と、導入針12の外方を覆う管状の埋込体カバー14と、を備え、埋込体カバー14と導入針12との間に軸方向に延びる空間13が設けられており、空間13に埋込体16が配置される。これにより、埋込体16に負荷を与えることなく、埋込体16を皮下組織92内に留置できる。
上記の埋込体留置器具10において、埋込体カバー14と導入針12との間に設けられる空間13は、導入針12の一部を切り欠いて溝状に形成された凹部12aよりなる。これにより、導入針12に溝を形成するだけで、埋込体16を収容可能な凹部12a(空間13)を形成することができる。この場合、導入針12を中実針とすると、溝状の凹部12aの加工が容易となり好適である。
上記の埋込体留置器具10において、埋込体16の先端部には、皮下組織92への係止を可能とする係止部(折返部16b)が設けられていてもよい。このような係止部(折返部16b)を設けることにより、埋込体留置器具10を皮下組織92から引き抜く際に、埋込体16がともに引き抜かれるのを防ぐことができる。その結果、埋込体16を留置部位に確実に留置することができる。
上記の埋込体留置器具10において、係止部(折返部16b)は、埋込体16の先端側の一部が埋込体カバー14の先端部14bよりも先端側に突出するとともに、埋込体カバー14の外面に沿って基端側に折り返して形成してもよい。埋込体16に簡単な加工を施すだけで係止部を実現できて好適である。
(第1実施形態の変形例1)
図6に示すように、本変形例の埋込体留置器具10Aでは、埋込体16を収容する空間13を、埋込体カバー14Aに設けている。すなわち、本実施形態の埋込体留置器具10Aにおいて、埋込体カバー14Aの貫通孔14aの内周面に凹部14eが設けられている一方で、導入針12Aは溝状の凹部12aが設けられていない中実針として構成されている。その他の構成は、図1A及び図1Bに示す埋込体留置器具10と同様であり、同様の構成には同一符号を付してその詳細な説明は省略する。
図6に示すように、本変形例の埋込体留置器具10Aでは、埋込体16を収容する空間13を、埋込体カバー14Aに設けている。すなわち、本実施形態の埋込体留置器具10Aにおいて、埋込体カバー14Aの貫通孔14aの内周面に凹部14eが設けられている一方で、導入針12Aは溝状の凹部12aが設けられていない中実針として構成されている。その他の構成は、図1A及び図1Bに示す埋込体留置器具10と同様であり、同様の構成には同一符号を付してその詳細な説明は省略する。
溝状の凹部14eは、埋込体カバー14Aの先端部14bから基端部14c(図1A参照)にまで軸方向に延びて形成されている。凹部14eは、埋込体16を収容可能とするべく、幅(周方向の長さ)及び深さ(径方向の長さ)が埋込体16の直径よりも大きく形成されている。凹部14eの幅及び深さは例えば0.5~0.6mm程度とすることができる。この場合には、凹部14eに直径0.4mm程度の埋込体16を収容することができる。
このように、本変形例に係る埋込体留置器具10Aでは、導入針12Aと埋込体カバー14Aとの空間13が、埋込体カバー14Aに形成された溝状の凹部14eとして形成されている。これにより、本変形例の埋込体留置器具10Aによっても、図1A~図5を参照しつつ説明した埋込体留置器具10と同様の効果が得られる。また、本変形例では、樹脂材料で埋込体カバー14Aを押出成形等で形成する際に、溝状の凹部14eを同時に作り込むことも可能である。そのため、生産性にも優れる。
なお、本変形例はこれに限られるものではなく、埋込体16を収容する空間13を、埋込体カバー14Aの凹部14eと、導入針12Aの凹部12aとの両方で構成してもよい。
(第1実施形態の変形例2)
図7に示すように、本変形例に係る埋込体留置器具10Bは、埋込体16Bの先端に折返部16b(図1A参照)に代えて、他の部分よりも径が大きく形成された係止部22が取り付けられている点で、図1Aの埋込体留置器具10と異なる。なお、埋込体留置器具10Bにおいて、図1Aの埋込体留置器具10と同様の構成については同一符号を付してその詳細な説明は省略する。
図7に示すように、本変形例に係る埋込体留置器具10Bは、埋込体16Bの先端に折返部16b(図1A参照)に代えて、他の部分よりも径が大きく形成された係止部22が取り付けられている点で、図1Aの埋込体留置器具10と異なる。なお、埋込体留置器具10Bにおいて、図1Aの埋込体留置器具10と同様の構成については同一符号を付してその詳細な説明は省略する。
図示のように、埋込体16Bの先端部には、生体内分解材料より形成されてなる係止部22が取り付けられている。係止部22は、断面の径が導入針12の凹部12aよりも大きく形成されており、導入針12の先端側において凹部12aの外側に突出するようにして配置されている。係止部22の中央部には、縮径して形成された巻付部22aが形成されており、その巻付部22aに埋込体16Bの先端部24が巻き付けられて固定されている。さらに、係止部22にはエコーでの視認性向上を目的とした凹凸状の加工が設けられていてもよい。導入針12の穿刺部12cは、穿刺の際に係止部22が妨げとならないように、係止部22の一部を収容する凹面12eを備えている。
本変形例の埋込体留置器具10Bによれば、埋込体16Bが、径の大きな係止部22を備えている。その係止部22は、導入針12の先端側に突出して配置されているため、導入針12及び埋込体カバー14を皮下組織92に穿刺したあと、導入針12を引き抜くと、埋込体16Bが皮下組織92に係止した状態となる。その結果、導入針12及び埋込体カバー14を引き抜く際に埋込体16Bを皮下組織92に係止した状態に保つことができ、埋込体16Bを留置部位に確実に留置できる。
(第2実施形態)
図8Aに示すように、本実施形態の埋込体留置器具20は、埋込体26、導入針ハブ27及び埋込体カバーハブ28において、図1Aの埋込体留置器具10と異なる。なお、図1Aの埋込体留置器具10と同様の構成については、同一符号を付してその詳細な説明は省略する。

図8Aに示すように、埋込体26は、その先端に他の部分よりも大きな径に形成された係止部23を備える。係止部23は、図8Bに示すように、その幅が導入針12の溝状の凹部12aの幅と略同じ幅を有しており、組立状態において、大部分が凹部12a内に収容されている。係止部23の側部は凹部12aとの間に摩擦抵抗が生じるように嵌合しており、導入針12及び埋込体カバー14を皮下組織92内で前後させても、係止部23が凹部12aから脱落しないように構成されている。
図8Bに示すように、係止部23の高さは、凹部12aの高さよりも高く形成されており、導入針12の外周部よりも外方に突出している。そのため、係止部23の上端部は、埋込体カバー14の縮径部14dよりも外方に突出しており、縮径部14dを通過できないように構成されている。そのため、組立状態において、図8Aに示すように、係止部23は埋込体カバー14よりも先端側に配置されている。係止部23の先端部23aは、導入針12の穿刺部12cの傾斜面から突出しないようにするべく、斜めに切り欠かれた傾斜面で構成されるとともに、穿刺部12cよりも基端側に後退した位置に配置されている。これにより、係止部23は、導入針12の皮下組織92内への穿刺を妨げないように構成されている。係止部23の材料としては、ポリ乳酸(PLA)やポリグリコール酸(PGA)等の生体内分解材料が挙げられる。さらに、係止部23にはエコーでの視認性向上を目的とした凹凸状の加工が設けられていてもよい。
係止部23の長手方向の中央部には、縮径してなる巻付部23bが形成されている。この巻付部23bに、埋込体26の先端部26aが巻き付けられて固定されている。埋込体26は、図1Aを参照しつつ説明した埋込体16と同様の、組織の再生を促す能力を有する材料よりなる糸状の部材であり、例えば、多孔質コラーゲン繊維で構成することができる。
導入針12の基端部12dには導入針12を操作するための導入針ハブ27が固定されている。導入針ハブ27は、基端側が閉塞された筒状の部材で構成され、先端側には筒状の挿入部27bが形成されている。導入針ハブ27の中心部には、導入針12の外径と略同じ内径の針固定孔27aが軸方向に延びて形成されている。
導入針ハブ27の針固定孔27aには、導入針12が挿入されて固定されている。導入針12の導入針ハブ27の固定は、例えば、嵌合、カシメ、融着、接着剤による接着等の方法で行うことができる。
埋込体カバーハブ28は、埋込体カバー14の基端部14cに、例えば、カシメ、融着、接着剤による接着等の方法により固定されている。埋込体カバーハブ28は、中心部に挿通孔28aが形成された略円筒状の部材として構成されている。挿通孔28aは、埋込体カバー14の貫通孔14aと連通しており、その内部を導入針12が貫通している。挿通孔28aの内径は、導入針ハブ27の挿入部27bの外径と略同じ大きさに形成されている。埋込体カバーハブ28は、導入針ハブ27と同様の材料で構成することができる。
挿通孔28aには、導入針ハブ27を前進又は後退させるための、案内凹部28bが形成されている。案内凹部28bは、挿通孔28aを径方向に拡大させた断面形状に形成されており、その内径は導入針ハブ27の挿入部27bの案内突起27cの外径と略同じ大きさに形成されている。案内凹部28bは軸方向に所定の長さ範囲に形成されており、案内凹部28bの形成された範囲で案内突起27cが摺動しつつ変位可能に構成されている。すなわち、案内凹部28b及び案内突起27cにより、埋込体カバー14を導入針12に対して前進又は後退させる操作機構29が構成される。図示の例では、案内突起27cが案内凹部28bの基端部に引っ掛かって停止して導入針ハブ27が埋込体カバーハブ28から完全に引き抜けないように構成されている。ただし、本実施形態はこれに限定されるものではなく、導入針ハブ27が埋込体カバーハブ28から完全に引き抜けるように構成されていても構わない。
埋込体留置器具20は、埋込体カバーハブ28の挿通孔28aに導入針ハブ27の挿入部27bを完全に挿入した組立状態で製品提供される。後述するように、埋込体26を皮下組織92内に留置して引き抜くまでは、埋込体留置器具20は組立状態で使用される。図示のように組立状態において、導入針12の針先12bが埋込体カバー14の先端から所定の長さだけ先端側に突出するように構成されている。また、導入針12の突出部分の凹部12aには、埋込体26の係止部23が収容されている。
本実施形態に係る埋込体留置器具20は、以上のように構成され、以下その使用方法と作用を説明する。
埋込体留置器具20は、図8Aに示す組立状態において、皮下組織92内の留置部位に導入針12及び埋込体カバー14を穿刺して使用される。埋込体留置器具20において、埋込体26の係止部23の大部分が、導入針12の凹部12a内に収容されているため、導入針12の穿刺が係止部23に邪魔されることなくスムーズに穿刺を行うことができる。係止部23は、導入針12の凹部12aに嵌合しているため、導入針12及び埋込体カバー14を基端側に引いた場合であっても、導入針12の凹部12aから外れない。そのため、埋込体留置器具20によれば、留置部位に進める際に血管、神経、靭帯等を損傷させないように後退を含む繊細な操作を行うことができる。
その後、図9Aに示すように、埋込体留置器具20の先端から、係止部23を離脱させる操作を行う。この場合には、導入針ハブ27を埋込体カバーハブ28に対して基端側に引く。これにより、操作機構29の案内凹部28b内を案内突起27cが摺動して導入針ハブ27が基端側に引き出される。そして、導入針ハブ27とともに、導入針12の先端部が基端側に後退する。係止部23は、導入針12よりも径方向外方に突出しているため、埋込体カバー14の内部に後退することができず、図9A及び図9Bに示すように、導入針12の凹部12aから押し出され、係止部23が導入針12の外方に大きく突出した状態となる。その結果、係止部23が皮下組織92に係止され、埋込体26が皮下組織92内に留置可能になる。
その後、導入針ハブ27及び埋込体カバーハブ28を基端側に引いて、導入針12及び埋込体カバー14を皮下組織92から引き抜くことで、埋込体26が皮下組織92内に留置される。
本実施形態の埋込体留置器具20は、以下の効果を奏する。
上記の埋込体留置器具20において、導入針12は、導入針12を切り欠いて軸方向に延びる溝状に形成された凹部12a(空間13)を備え、係止部23は凹部12a(空間13)に嵌合するように収納されている。この構成により、埋込体留置器具20を引いた場合であっても、係止部23が皮下組織92に係止されて意図しない場所で埋込体26が留置されてしまうのを防ぐことができる。
上記の埋込体留置器具20は、さらに、埋込体カバー14及び導入針12の基端部12dに設けられ、埋込体カバー14を導入針12に対して前進又は後退させる操作機構29を備える。これにより、所望の留置部位において、係止部23を導入針12の凹部12aから外すことができ、係止部23を皮下組織92に係止させることができる。
(第2実施形態の変形例)
図10Aに示すように、本変形例に係る埋込体留置器具20Aは、埋込体26Aの先端に係止部23(図8A参照)に代えて、埋込体26Aを絡ませて球状に形成した係止部26cを備えて構成されている。なお、埋込体留置器具20Aにおいて、図9Aの埋込体留置器具20と同様の構成については同一符号を付してその詳細な説明は省略する。
図10Aに示すように、本変形例に係る埋込体留置器具20Aは、埋込体26Aの先端に係止部23(図8A参照)に代えて、埋込体26Aを絡ませて球状に形成した係止部26cを備えて構成されている。なお、埋込体留置器具20Aにおいて、図9Aの埋込体留置器具20と同様の構成については同一符号を付してその詳細な説明は省略する。
埋込体26Aの先端側には、埋込体26Aを絡ませて球状に形成した係止部26cが設けられている。図10Bに示すように、係止部26cは、導入針12の凹部12aの深さよりも大きく形成されており、凹部12aに嵌合して収容されている。係止部26cからは、埋込体26Aの先端部26dが突出してもよい。この場合には、図10Aに示すように、先端部26dは、基端側に折り返して、埋込体カバー14の先端部14bの外表面に沿わせて配置される。
本変形例の埋込体留置器具20Aは、図10Aの組立状態のまま皮下組織92の留置部位に穿刺される。その後、図11に示すように、導入針ハブ27を基端側に引き出すことで、係止部26cを導入針12の凹部12aから離脱させることができる。これにより、係止部26cが皮下組織92に係止されて埋込体26Aを皮下組織92内に留置可能となる。その後、導入針12及び埋込体カバー14を皮下組織92から引き抜くことにより、留置部位に埋込体26Aを留置することができる。
以上のように、本変形例の埋込体留置器具20Aも、図8Aの埋込体留置器具20と同様の効果を奏する。
(第3実施形態)
図12Aに示すように、本実施形態に係る埋込体留置器具30は、導入針ハブ37に埋込体16の基端部を脱着自在に保持する埋込体保持機構38を備えている点で、図1Aの埋込体留置器具10と異なる。なお、埋込体留置器具30において、図1Aの埋込体留置器具10と同様の構成については、同一符号を付してその詳細な説明は省略する。
図12Aに示すように、本実施形態に係る埋込体留置器具30は、導入針ハブ37に埋込体16の基端部を脱着自在に保持する埋込体保持機構38を備えている点で、図1Aの埋込体留置器具10と異なる。なお、埋込体留置器具30において、図1Aの埋込体留置器具10と同様の構成については、同一符号を付してその詳細な説明は省略する。
本実施形態の導入針ハブ37は、先端側に埋込体カバーハブ18の軸孔18aに挿入される挿入部37bが形成されており、基端側に埋込体保持機構38の一部を構成するキャップ装着部37cが設けられている。導入針ハブ37の中心部には、導入針12の外径と略同じ内径の針固定孔37aが軸方向に貫通して形成されている。すなわち、針固定孔37aは、挿入部37bの先端からキャップ装着部37cの基端までを貫通して形成されている。
針固定孔37aには、導入針12が挿入されて固定されている。また、キャップ装着部37cの基端部には、図12Bに示すように針固定孔37aの内方に半円形状に突出して針固定孔37aの略半分の範囲を塞ぐ突起部37eが形成されている。キャップ装着部37cの外周部には、ねじ機構37dが形成されている。図12Aに示すように、キャップ装着部37cには、キャップ部材39が外側から装着されている。
キャップ部材39は、有底円筒状に形成されており、内側部に、キャップ装着部37cのねじ機構37dに螺合可能なねじ機構39aが形成されている。キャップ部材39は、ねじ機構39aを通じてキャップ装着部37cに螺合されている。キャップ部材39の基端部には、半円形の開口部39bが形成されている。キャップ部材39を回転させることにより、開口部39bが突起部37eで塞がれる閉位置と、開口部39bが針固定孔37aと連通する開位置とに変位可能に構成されている。
埋込体16の基端部は、針固定孔37a及び開口部39bを通って埋込体留置器具30の基端側に延び出ている。図12Bに示すように、キャップ部材39の閉位置では、埋込体16の基端部がキャップ部材39と突起部37eとの間に挟まれることで、埋込体16が埋込体留置器具30に固定されるように構成されている。また、キャップ部材39の開位置(図13参照)では、埋込体16が埋込体留置器具30から離脱可能となる。埋込体留置器具30の組立状態において、キャップ部材39は閉位置に固定されている。
本実施形態の埋込体留置器具30は、図12Aに示す組立状態において、皮下組織92内の留置部位に穿刺される。その後、図13に示すように、キャップ部材39を回転させて、開口部39bと針固定孔37aが連通する開位置に回動させる。これにより、埋込体16が埋込体留置器具30から離脱可能となる。
その後、図示のように、導入針12を皮下組織92から引き抜く。さらに、埋込体カバー14を皮下組織92から引き抜く。そして、皮膚90から突出した部分の埋込体16を切断処理することで、埋込体16の皮下組織92への留置が完了する。
以上のように本実施形態の埋込体留置器具30は、導入針12の基端側に、埋込体16の基端部を脱着自在に保持する埋込体保持機構38を備えている。これにより、所望の位置で埋込体16を留置することができる。また、埋込体留置器具30を引く操作を行っても埋込体16が脱落しないので、埋込体留置器具30を自由に操作することが可能となり、血管、神経、靭帯等を損傷させないように後退を含む繊細な操作を行うことができる。
(第4実施形態)
図14Aに示すように、本実施形態の埋込体留置器具40は、導入針12の基端部の導入針ハブ47にスライド可能な操作レバー42を有する埋込体保持機構48を備えている点で、図12Aの埋込体留置器具30と異なる。なお、埋込体留置器具40において、図12Aの埋込体留置器具30と同様の構成については、同一符号を付してその詳細な説明は省略する。
図14Aに示すように、本実施形態の埋込体留置器具40は、導入針12の基端部の導入針ハブ47にスライド可能な操作レバー42を有する埋込体保持機構48を備えている点で、図12Aの埋込体留置器具30と異なる。なお、埋込体留置器具40において、図12Aの埋込体留置器具30と同様の構成については、同一符号を付してその詳細な説明は省略する。
導入針ハブ47は、先端側に埋込体カバーハブ18の軸孔18aに挿入される挿入部47bが形成されている。導入針ハブ47の中心部には、針固定孔47aが挿入部47bの先端側から導入針ハブ47の基端側まで軸方向に貫通して形成されている。針固定孔47aには、導入針12の基端部が挿入されて固定されている。また、針固定孔47aの基端部付近には、導入針ハブ47の一端側(図の下側)から他端側(図の上側)に向けて突出した突出部44が形成されている。突出部44は、針固定孔47aの中心付近まで延び出て形成されており、軸方向に所定長さに形成されている。
また、導入針ハブ47の他端側には断面がクランク状に屈曲した案内孔46が形成されている。案内孔46の一端46a(下端)は針固定孔47aに開口し、他端46b(上端)は導入針ハブ47の他端側で開口している。一端46aと他端46bとは、軸方向に延びた連絡部46cを通じて連通している。案内孔46の一端46aは、突出部44に対向する部分から、突出部44よりも基端側の部分にかけて形成されており、突出部44よりも軸方向に長く形成されている。また、案内孔46の他端46bは、一端46aと軸方向に同じ長さに形成されている。
案内孔46には、クランク状に屈曲した操作レバー42が挿入されている。操作レバー42は、案内孔46の連絡部46cに挿入されて軸方向に摺動する中間部42cと、中間部42cの基端側から導入針ハブ47の中心側に向けて延び出た押圧部42aと、中間部42cの先端側から導入針ハブ47の外方に向けて屈曲した操作部42bとを備えている。操作部42bは、案内孔46の他端46bを通じて導入針ハブ47の外方に突出しており、使用者が操作力を入力可能に構成されている。押圧部42aは、突出部44の端面に対応する当接面42a1を備えており、その当接面42a1と突出部44との間で埋込体16の基端部を押圧して固定するように構成されている。
図14Bに示すように、操作レバー42の幅は案内孔46と略同じ幅に形成されている。案内孔46と操作レバー42との間で摩擦力が発生するようにして、意図しないタイミングで操作レバー42が移動しないようにすることが好ましい。図14Aに示すように、操作レバー42の押圧部42a及び操作部42bの導入針12の軸方向の長さは、案内孔46の一端46a及び他端46bの導入針12の軸方向の長さよりも短く形成されている。これにより、操作レバー42は、案内孔46に沿って軸方向にスライド可能に構成されている。操作レバー42が先端側に位置している場合には、押圧部42aが突出部44と対向して埋込体16の基端部を固定する。また、操作レバー42を基端側に移動させると、押圧部42aが突出部44から外れて、埋込体16の固定が解除される。
導入針ハブ47の挿入部47bは、埋込体カバーハブ18の軸孔18aから引き抜くことができるように構成されている。なお、埋込体留置器具40は、図14Aに示すように、導入針ハブ47の挿入部47bが埋込体カバーハブ18の軸孔18aに完全に押し込まれ、且つ、操作レバー42が先端側に位置した組立状態において製品提供される。
本実施形態の埋込体留置器具40は、図14Aに示す組立状態において、皮下組織92内の留置部位に穿刺される。その後、図15に示すように、操作レバー42を基端側に移動させて、操作レバー42の押圧部42aを突出部44から外す。これにより、埋込体16の固定が解除されて、埋込体16が埋込体留置器具40から離脱可能となる。
その後、図示のように、導入針ハブ47の挿入部47bを埋込体カバーハブ18の軸孔18aから引き抜く。その後、導入針ハブ47を引き抜いて導入針12を埋込体カバー14から引き抜く。次いで、埋込体カバー14を皮下組織92から引き抜く。これにより、埋込体16が皮下組織92内に留置される。最後に、皮膚90から突出した部分の埋込体16を切断処理することで、埋込体16の皮下組織92への留置が完了する。
以上のように、本実施形態の埋込体留置器具40は、図12Aの埋込体留置器具30と同様の効果を奏する。
上記において、本発明について好適な実施形態を挙げて説明したが、本発明は前記実施形態に限定されるものではなく、本発明の趣旨を逸脱しない範囲において、種々の改変が可能なことは言うまでもない。例えば、導入針12の針先12bは鋭利ではなく鈍でもよい。
Claims (9)
- 皮下組織に挿入可能な針先を備える導入針と、
細胞が付着することで組織再生を促す能力を有する長尺な埋込体と、
前記導入針の外方を覆う管状の埋込体カバーと、を備え、
前記埋込体カバーと前記導入針との間に軸方向に延びる空間が設けられており、前記空間に前記埋込体が配置される、埋込体留置器具。 - 請求項1記載の埋込体留置器具であって、前記埋込体カバーと前記導入針との間に設けられる前記空間は、前記導入針及び前記埋込体カバーのいずれか又は両方の一部を切り欠いて溝状に形成される、埋込体留置器具。
- 請求項1又は2記載の埋込体留置器具であって、前記埋込体の先端部には、皮下組織への係止を可能とする係止部が設けられている、埋込体留置器具。
- 請求項3記載の埋込体留置器具であって、前記係止部は、前記埋込体の先端側の一部が前記埋込体カバーの先端部よりも先端側に突出するとともに、前記埋込体カバーの外面に沿って基端側に折り返されてなる、埋込体留置器具。
- 請求項3記載の埋込体留置器具であって、前記係止部は、前記埋込体の他の部分よりも大きな径に形成されてなる、埋込体留置器具。
- 請求項5記載の埋込体留置器具であって、前記空間は、前記導入針を切り欠いて軸方向に延びる溝状の凹部を備え、前記係止部は前記凹部に嵌合するように収納される、埋込体留置器具。
- 請求項1~6のいずれか1項に記載の埋込体留置器具であって、前記導入針は前記埋込体カバーに対して軸方向に相対移動可能である、埋込体留置器具。
- 請求項1~7のいずれか1項に記載の埋込体留置器具であって、さらに、前記導入針の基端部に設けられ、前記埋込体の基端部を脱着自在に保持する埋込体保持機構を備える、埋込体留置器具。
- 請求項1記載の埋込体留置器具であって、前記埋込体カバーの先端部は、先端側に向かうにしたがって外径が細くなるテーパー状の傾斜面で構成される、埋込体留置器具。
