Definitions
- the invention is related to an aspiration catheter system that is used to remove the thrombus or particles that obstruct a vessel out of the body and to enable reopening of the vessel.
- TR201200555 discloses a catheter system that can be used for concurrent fluid infusion and aspiration in humans, animals and biological materials at a wide range of flow rates without any blockage problems. This system has devices that control the infused liquid's pressure changes accordingly to the aspirated liquid's pressure changes, in order to allow accurate and safe fluid exchange flow control and create changes of pressures at the catheter's tip that do not allow the catheter's blockage.
- an aspiration catheter in which vacuum may be accumulated within the catheter and released suddenly within a distal region of the catheter to induce ingestion of intravascular matter into the catheter through an open distal port while avoiding large losses of suction common with catheters having long aspiration lumens.
- a catheter developed for the aspiration of foreign matter in a blood vessel comprises a flexible outer tube and a flexible inner tube that is inserted into the flexible outer tube having a front end part capable of protruding beyond the flexible outer tube.
- the aim of the invention is to provide a catheter system having a configuration that comprises a motor that can function for both catheters, single direction valves connected to the motor, aspirator trap, and waste bag and that enables to remove the thrombus inside the vessel without taking the catheter out of the vessel during clogging, by using a single motor.
- Another aim of the invention is to provide an aspiration catheter system that comprises an outer catheter and an inner catheter that is moveable inside the outer catheter.

- Figure 10 Is the detailed view of the head portion of the inner catheter in the system subject to the invention.
- Skin The invention is an aspiration catheter system comprising,
- a balloon inflation apparatus (7) that is located on the outer catheter (2) and that is connected to the balloon (4),
- An apparatus connection section (8) which is located at the end part of the outer catheter (2) that remains outside the vessel,
- a single direction check valve (13) located at the inlet of the waste bag (10) and on the outlet path of the aspirator trap (9) that is connected to the motor (11),
- the inner catheter (3) is moveable back and forth inside the outer catheter (2).
- the outer catheter (2) comprises an inner check valve (5) and an outer check valve (6) at both ends, that prevents blood and fluid passage through.
- the inner check valve (5) and an outer check valve (6) only allow the passage of a guide wire (15) and the inner catheter (3). (Similar to cork stoppers and IV stoppers)
- the outer catheter (2) is placed according to the position of the thrombus. At this point, the balloon (4) is uninflated. ( Figure 5) Following this, the balloon (4) that is located at the end section of the outer catheter (2) is inflated by means of the inflation apparatus (7) located on the outer catheter (2) in order to prevent blood flow. ( Figure 6) By this means, it is allowed for the thrombus to enter the distal end and it is enabled for the thrombus to be easily aspired.
- a pressurized controlled inflation process is carried out. Following the inflation process, the outer catheter (2) should not be moved.
- the inner catheter (3) is pushed towards the thrombus inside the outer catheter (2).
- the motor (11) is operated and aspiration is started through the inner catheter (3).
- the thrombus is sucked through the inner catheter (3).
- Figure 7a and Figure 9 The thrombus that is sucked through the inner catheter (3) is transferred into the waste bag (10).
- the single direction check valve (13) located on the outlet path of the aspirator trap (9) that is connected to the motor (11)
- the thrombi that have been removed are prevented from entering into the aspirator trap (9).
- the end part of the inner catheter (3) is suitably soft and flexible and at the same time, it has peripheral endurance that can endure aspiration power.
- the negative pressure indicator (12) is tracked during aspiration. As long as the pressure continues to remain below a certain pressure value (for example up to -200mmHg) the aspiration process is continued. However, if it is noted that the pressure exceeds a certain value, this may indicate that the inner catheter (3) is filled with thrombi and that the lumen is obstructed or that the end part of the inner catheter (3) is bearing onto the vessel lumen and it is trying to vacuum the vessel endothelium.
- the motor (11) that is carrying out the aspiration process shall be stopped.
- the inner catheter (3) is retracted back and the end thereof is enabled to enter back into the outer catheter (2).
- Figure 3 and Figure 7b Following this, the motor (11) is reversely operated and inner cleaning is carried out. In other words, the process of removing the thrombi from the inner catheter (3) that is obstructing the inner catheter (3) is started.
- Level line (0) the ends of the inner catheter (3) and outer catheter (2) are at the same level.
- Level line (-) value (-1,-2, -5): The end of the inner catheter (3) is further in, when compared with the end of the outer catheter (2).
- an angiography can be taken and it can be ensured that thrombi have been removed and that the vessel remains fully open.
- images can be taken after a contrast agent is delivered through the lumen of the inner catheter (3) while the level line is at +1 position after the inner catheter (3) lumen has been cleaned.
- the motor (11) can be operated automatically. By this means the thrombi cleaning process can be accelerated.
- the system subject to the invention can be used to intervene in every vessel including small brain vessels, by producing series having suitable diameters and lengths for every kind of vessel.
- 1.41 ratio shall be sufficient according to the diameter ratio to volume calculation of the outer catheter (2) and the inner catheter (3).
- the diameter of the inner catheter (3) is 2mm and the diameter of the outer catheter (2) to be 2.82mm, as the thrombi particles that need to be carried may have a solid structure, it seems suitable for the diameter ratio to be 2.
- the inner catheter (3) diameter is necessary for the inner catheter (3) diameter to be 2mm, and the outer catheter (2) diameter to be 4mm in order to ensure that the apparatus operates smoothly without being obstructed.
- the possible damage that may be given to vessels has not only been minimized by means of the system subject to the invention but it also ensures that the entire thrombi are removed from the vessel. Additionally in previous systems, due to the end disrupting apparatus, the catheter diameter could not reduce much, however, the problem of entering smaller veins has also been overcome by means of the system subject to the invention.
